ABSTRACT

Despite careful selection for liver transplantation (LT) of patients with hepatocellular carcinoma (HCC), HCC may still recur after LT and is frequently associated with dismal outcome. Tumor factors, including serum alpha-fetoprotein (AFP), the presence of microvascular invasion, tumor grade/differentiation, and largest tumor size are amongst the most important predictors of recurrence after transplantation. The nature of recurrence can be highly variable, but often presents with extra-hepatic involvement. As such, management of patients with HCC can be challenging, and consensus guidelines are lacking. Curative options, with surgery or ablation, which may be applicable in patients with isolated intra-or extrahepatic metastases, offer the best chance for improved long-term outcome in patients with HCC recurrence after transplantation. Most patients with recurrence have unresectable disease, and may benefit from palliative treatments, including intra-arterial therapies and/or systemic therapy.

ABSTRACT

PURPOSE: Germline mutations in the homologous recombination genes BRCA1, BRCA2, and PALB2 confer an increased risk for pancreatic ductal adenocarcinoma (PDAC). Tumors associated with mutations in homologous recombination genes are sensitive to DNA-damaging agents. We retrospectively studied patients with resected PDAC and a pathogenic germline mutation in one of these three genes. The planned analyses included overall survival (OS) and changes therein when platinum chemotherapy was used in the perioperative setting. MATERIALS AND METHODS: Thirty-two individuals with pathogenic germline mutations in BRCA1, BRCA2, or PALB2 and resected PDAC (mutation positive) were matched in a 1:2 fashion to patients who were noncarriers or untested (mutation negative) by age, year of diagnosis, stage, and sex. Patients were identified via one of two available databases at University of Pennsylvania: the Basser Center for BRCA Registry or the electronic medical record. The primary outcome was OS. RESULTS: Patients in the mutation-positive group had a median OS (mOS) of 46.6 months; those in the mutation-negative group had an mOS of 23.2 months (hazard ratio [HR], 0.49; 95% CI, 0.27 to 0.88). With platinum exposure in the perioperative setting, mOS in the mutation-positive group had not yet been met versus a mOS of 23.1 months in the mutation-negative group (HR, 0.12; 95% CI, 0.01 to 1.00). When neither group was treated with platinum, there was no significant OS difference between groups (HR, 0.52; 95% CI 0.12 to 2.24). Patients in the mutation-positive group who received perioperative treatment with platinum had a trend toward improved mOS compared with those who did not (HR, 0.15; 95% CI, 0.02 to 1.23; P = .07). CONCLUSION: Platinum-based chemotherapy may confer a survival benefit in patients with resected PDAC and a pathogenic germline BRCA1, BRCA2, or PALB2 mutation. Knowledge of a germline mutation may be important to determine best choice of perioperative chemotherapy.

We performed a retrospective review of palliative care services among patients with ESLD unsuitable for liver transplantation (LT) at a tertiary care center. Material and methods. Deceased ESLD patients considered unsuitable for LT from 2007-2012 were identified. Patients were excluded if they received a transplant, had an incomplete workup, were lost to follow up or whose condition improved so LT was not needed. Of the 1,175 patients reviewed, 116 met inclusion criteria. Results. Forty patients (34.4%) received an inpatient palliative care (PC) consultation and forty-one patients (35.3%) were referred directly to hospice. Thirty-three patients (28.4%) transitioned to comfort measures without PC consultation (median survival < 1 day). The median interval between LT denial and PC consultation or hospice was 28 days. Median survival after PC consult or hospice referral was 15 days. In conclusion, in a single center retrospective review of ESLD patients, palliative care services, when utilized, were for care at the very end of life. Without consultation, aggressive interventions continued until hours before death. We propose that ESLD patients could benefit from PC consultation at time of LT evaluation or based on MELD scores.

ABSTRACT

This study evaluated: (1) the effect of different ceramics on light attenuation that could affect microhardness, measured as the Knoop Hardness Number (KHN), of a resin cement immediately and 24 hours after polymerization and (2) the effect of different activation modes (direct light-activation, light activation through ceramics and chemical activation) on the KHN of a resin cement. Resin cement Rely XARC (3M ESPE) specimens 5.0 mm in diameter and 1.0 mm thickwere made in a Teflon mold covered with a polyester film. The cement was directly light activated for 40 seconds with an XL 2500 curing unit (3M ESPE) with 650 mW/cm2, light activated through ceramic discs of Duceram Plus (DeguDent), Cergogold (DeguDent), IPS Empress (Ivoclar), IPS Empress 2 (Ivoclar), Procera (NobelBiocare), In Ceram Alumina (Vita) and Cercon (DeguDent), having a 1.2 mm thickness or chemically activatedwith-out light application. The resin cement specimens were flattened, and KHN wasobtained using an HMV 2 microhardnesstester (Shimadzu) with a load of 50 g applied for 15 seconds 100 microm from the irradiated surface immediately and after storage at 37 degrees C for 24 hours. Ten measurements were made for each specimen, with three specimens for each group at each time. The data were submitted to ANOVA and Tukey's test (p = 0.05). The KHN of the resin cement was not only affected by the mode of activation, but also by the post-activation testing time. The mean KHN of the resin cementfor chemical activation and through all ceramics showed statistically significant lower values compared to direct activation immediately and at 24 hours. The KHN for 24 hourspost-activation was always superior to the immediate post-activation test except with direct activation. The most opaque ceramics resulted in the lowest KHN values.
