ABSTRACT

INTRODUCTION: Management of the patient with cirrhosis of the liver that requires surgical treatment has been relatively unexplored. In Mexico, there is currently no formal stance or expert recommendations to guide clinical decision-making in this context. AIMS: The present position paper reviews the existing evidence on risks, prognoses, precautions, special care, and specific management or procedures for patients with cirrhosis that require surgical interventions or invasive procedures. Our aim is to provide recommendations by an expert panel, based on the best published evidence, and consequently ensure timely, quality, efficient, and low-risk care for this specific group of patients. RESULTS: Twenty-seven recommendations were developed that address preoperative considerations, intraoperative settings, and postoperative follow-up and care. CONCLUSIONS: The assessment and care of patients with cirrhosis that require major surgical or invasive procedures should be overseen by a multidisciplinary team that includes the anesthesiologist, hepatologist, gastroenterologist, and clinical nutritionist. With respect to decompensated patients, a nephrology specialist may be required, given that kidney function is also a parameter involved in the prognosis of these patients.

ABSTRACT

The first clinical guidelines on hepatic encephalopathy were published in 2009. Almost 14 years since that first publication, numerous advances in the field of diagnosis, treatment, and special condition care have been made. Therefore, as an initiative of the Asociación Mexicana de Gastroenterología A.C., we present a current view of those aspects. The manuscript described herein was formulated by 24 experts that participated in six working groups, analyzing, discussing, and summarizing the following topics: Definition of hepatic encephalopathy; recommended classifications; epidemiologic panorama, worldwide and in Mexico; diagnostic tools; conditions that merit a differential diagnosis; treatment; and primary and secondary prophylaxis. Likewise, these guidelines emphasize the management of certain special conditions, such as hepatic encephalopathy in acute liver failure and acute-on-chronic liver failure, as well as specific care in patients with hepatic encephalopathy, such as the use of medications and types of sedation, describing those that are permitted or recommended, and those that are not.

ABSTRACT

The growing use of biomaterials with different therapeutic purposes increases the need for their physiological understanding as well as to seek its integration with the human body. Chronic inflammatory local pathologies, generally associated with infectious or autoimmunity processes, have been a current therapeutic target due to the difficulty in their treatment. The recent development of biomaterials with immunomodulatory capacity would then become one of the possible strategies for their management in local pathologies, by intervening in situ, without generating alterations in the systemic immune response. The treatment of periodontal disease as an inflammatory entity has involved the use of different approaches and biomaterials. There is no conclusive, high evidence about the use of these biomaterials in the regeneration of periodontitis sequelae, so the profession keeps looking for other different strategies. The use of biomaterials with immunomodulatory properties could be one, with a promising future. This review of the literature summarizes the scientific evidence about biomaterials used in the treatment of periodontal disease.

ABSTRACT

The most relevant property that a quality indicator (QI) is expected to have is Pareto compliance, which means that every time an approximation set strictly dominates another in a Pareto sense, the indicator must reflect this. The hypervolume indicator and its variants are the only unary QIs known to be Pareto-compliant but there are many commonly used weakly Pareto-compliant indicators such as R2, IGD+, and ε+. Currently, an open research area is related to finding new Pareto-compliant indicators whose preferences are different from those of the hypervolume indicator. In this article, we propose a theoretical basis to combine existing weakly Pareto-compliant indicators with at least one being Pareto-compliant, such that the resulting combined indicator is Pareto-compliant as well. Most importantly, we show that the combination of Pareto-compliant QIs with weakly Pareto-compliant indicators leads to indicators that inherit properties of the weakly compliant indicators in terms of optimal point distributions. The consequences of these new combined indicators are threefold: (1) to increase the variety of available Pareto-compliant QIs by correcting weakly Pareto-compliant indicators, (2) to introduce a general framework for the combination of QIs, and (3) to generate new selection mechanisms for multiobjective evolutionary algorithms where it is possible to achieve/adjust desired distributions on the Pareto front.

ABSTRACT

RESUMEN Las evaluaciones clínica y radiológica son técnicas que se emplean para el diagnóstico de ruptura de ligamento cruzado anterior (RLCA) y para la evaluación posoperatoria de cirugías correctivas. Se planteó como objetivo describir los resultados posoperatorios de la técnica avance de tuberosidad tibial en caninos con RLCA utilizando hueso de origen bovino conservado en glicerina al 98%. Fueron intervenidos 5 caninos con la patología y sin cambios radiológicos marcados en las columnas óseas relacionadas. El heteroim-plante óseo se cortó a manera de cuña y, para determinar su tamaño, se tuvo en cuenta la medida prequirúrgica que necesitaba la rodilla para alcanzar el ángulo de avance de la cresta tibial. Seguidamente se fijó al receptor local con implantes metálicos (agujas de Kirschner y alambre ortopédico). En el posoperatorio, se evaluaron por 90 días signos clínicos como claudicación, dolor al movimiento flexión-extensión e inflamación, según la circunferencia de la rodilla, y se evaluó por 120 días el ángulo de avance tibial y la radiopacidad del heteroimplante con respecto al hueso circunvecino (tuberosidad tibial y tibia caudal). El estudio estableció que los parámetros clínicos y radiológicos tienen un efecto estadísticamente significativo; sin embargo, la evaluación del ángulo de avance tibial no tiene un efecto significativo entre el paciente y los días posquirúrgicos. Durante el periodo de evaluación el heteroimplante óseo bovino demostró ser efectivo para estabilizar una rodilla con RLCA en caninos.


ABSTRACT The clinical and radiological evaluations are techniques used for the diagnosis of the anterior cruciate ligament rupture (LCAR) and the post-operatory assessment of corrective surgeries. It was proposed as an objective to describe the post-operatory results of the advance technique of tibial tuberosity in canines with LCAR using bovine bone conserved in 98% glycerin. Five canines were intervened and with no other radiological changed in the related bone columns. The bone heteroimplant was cut in a wedge-like manner and for its size it was taken into account the pre-operatory measurement that the knee needed to reach the advancement angle of the tibial crest. Next, the bone implant was fixated to the local receptor with metallic implants (Kirschner needles and orthopedic wire). In the post-op, the canines were evaluated for 90 days for clinical signs such as claudication, pain when flexing and extending and inflammation according to the knee circumference, and for 120 days the advancement angle of the tibia and the radiopacitry of the heteroimplant in relation to the surrounding bone. The trial established that the clinical and radiological parameters have a statistically significant effect; however, the evaluation of the tibial advancement angle does not have a significant effect between the patient and the post-operatory days. During the evaluation period, the heteroimplant bovine bone proved to be effective to stabilize the knee in canines with LCAR.

ABSTRACT

We examine a one-dimensional nonlinear (Kerr) waveguide array which contains a single "void" waveguide where the nonlinearity is identically zero. We uncover a family of nonlinear localized modes centered at or near the void, and their stability properties. Unlike a usual impurity problem, here the void acts like a repulsive impurity causing the center of the simplest mode to lie to the side of the void's position. We also compute the stability of extended nonlinear modes showing significant differences from the usual homogeneous nonlinear array. The transmission of a nonlinear pulse across the void shows three main regimes-transmission, reflection, and trapping at the void's position-and we identify a critical momentum for the pulse below (above) where the pulse gets reflected (transmitted), or trapped indefinitely at the void's position. For relatively wide pulses, we observe a steep increase from a regime of no transmission to a regime of high transmission, as the amplitude of the soliton increases beyond a critical wave-vector value. Finally, we consider the transmission of an extended nonlinear wave across the void impurity numerically, finding a rather complex structure of the transmission as a function of wave vector, with the creation of more and more transmission gaps as nonlinearity increases. The overall transmittance decreases and disappears eventually, where the boundaries separating passing from nonpassing regions are complex and fractal-like.

ABSTRACT

The magnetocaloric effect and the universal character for the magnetic entropy change regarding the cubic crystal structures (SC, BCC, FCC) were investigated, in a qualitative way, using Monte Carlo simulations. A classical Heisenberg Hamiltonian with nearest neighbors, and next nearest neighbors interactions was implemented. In order to compute the critical temperature of the system depending on the coordination number, it was calculated the dependence of the magnetization and magnetic susceptibility as a function of temperature. Magnetic field dependence on the magnetization for isothermal processes was performed considering a magnetocrystalline anisotropy term. In this way, the magnetic entropy change (ΔSm) was computed. Results show that the rescaled ΔSm as well as the exponent (n) characterizing the field dependence of the magnetic entropy change curves, collapse onto a single curve for the studied crystal structures. By this reason, it can be assured that ΔSm exhibits a universal behavior regarding the strength and contribution of the magnetic exchange energy to the total magnetic energy.

ABSTRACT

We present an adaptive algorithm for the optimal phase space sampling in Monte Carlo simulations of 3D Heisenberg spin systems. Based on a golden rule of the Metropolis algorithm which states that an acceptance rate of [Formula: see text] is ideal to efficiently sample the phase space, the algorithm adaptively modifies a cone-based spin update method keeping the acceptance rate close to [Formula: see text]. We have assessed the efficiency of the adaptive algorithm through four different tests and contrasted its performance with that of other common spin update methods. In systems at low and high temperatures and anisotropies, the adaptive algorithm proved to be the most efficient for magnetization reversal and for the convergence to equilibrium of the thermal averages and the coercivity in hysteresis calculations. Thus, the adaptive algorithm can be used to significantly reduce the computational cost in Monte Carlo simulations of 3D Heisenberg spin systems.

ABSTRACT

A combined approach merging stable isotopes and fatty acids was applied to study anthropogenic pollution in the Río Negro estuary. Fatty acid markers of vegetal detritus indicated considerable allochthonous inputs at freshwater sites. Correlative evidence of diatom fatty acids, δ13C, chlorophyll and particulate organic matter suggested the importance of diatoms for the autochthonous organic matter production at the river mouth. Low δ15N values (~0‰) and high fatty acid 18:1(n-7) concentrations in the suspended particulate matter, in combination with the peaks of coliforms and ammonium, indicated a strong impact of untreated sewage discharge. The 15N depletion was related to oxygen-limited ammonification processes and incorporation of 15N depleted ammonium to microorganisms. This work demonstrates that the combined use of lipid and isotopic markers can greatly increase our understanding of biogeochemical factors and pollutants influencing estuaries, and our findings highlight the urgent need for water management actions to reduce eutrophication.

In this work, Monte Carlo simulations based on metropolis algorithm were performed to study the critical and compensation temperatures of a core-shell nanowire with spins [Formula: see text] and [Formula: see text], respectively, considering an Ising antiferromagnetic system. The influence of nearest neighbors exchange interactions and crystal field anisotropy on the critical and compensation behaviors of the system has been analyzed. The effects of the nanowire height in the critical and compensation temperatures were evaluated. The results show that, for a system with given values of exchange interaction constants and crystal field anisotropy, a compensation point only appears if two requirements are satisfied. First, the weight of the core magnetization in the total magnetization must be greater than the weight of the shell magnetization at zero temperature. And second, the exchange constant of shell ions must be greater than a certain value. This value is, at the same time, greater than the exchange constant of core ions. The critical and compensation temperatures are very sensitive to variations in the exchange constant of the shell ions and core ions, respectively, while the crystal field anisotropy affects both temperatures.

BACKGROUND/AIM: First-line bevacizumab-based therapies have been shown to improve clinical outcomes in patients with non-squamous non-small-cell lung cancer (NSCLC). We aimed to descriptively analyse patients with non-squamous NSCLC who received a long-term period of maintenance bevacizumab. PATIENTS AND METHODS: This retrospective study included 104 patients who had already reached a progression-free survival (PFS) of at least 9 months. RESULTS: Median overall survival and PFS were 30.7 and 15.1 months, respectively. The overall response rate was 83 %. Weight loss ≤5 %, ECOG PS = 0, or low number of metastatic sites seem to be predictive factors of good evolution. The incidence of bevacizumab-related adverse events appeared to be similar as the previous studies. CONCLUSION: Our findings show that there is a long-term survivor group whom the administration of bevacizumab resulted in a relevant prolongation of response without new safety signals. Due to the population heterogeneity, it was not possible to identify the standardised predictive factors.

Se examinaron los pulmones de 582 bovinos machos tipo exportación, de razas cebuínas y diferentes procedencias, faenados en el frigorífico FrigosinÚ del municipio de Montería (Córdoba, Colombia), con la finalidad de identificar las lesiones encontradas en este órgano. El muestreo se hizo en forma sistemática durante 10 días, evaluando animales escogidos al azar. Durante el tiempo de estudio se decomisaron los pulmones de 190 animales (32,6%) con algún tipo de alteración pulmonar mientras el 67,4% no presentó lesiones macroscópicas evidentes. Estos resultados se analizaron mediante estadística descriptiva, encontrándose que el enfisema intersticial fue la causa principal de decomiso en 64,7% de los animales, seguido de hemorragias (15,8%), neumonía lobar (10,5%), congestión pulmonar (8,9%) y broncoaspiración de contenido ruminal (9,0%); sin embargo, el diagnóstico histopatológico no fue contemplado en este estudio y, por lo tanto, el diagnóstico presuntivo se estableció según las lesiones macroscópicas. Se concluyó que la mayoría de las alteraciones encontradas en los pulmones decomisados obedece a fallas en el proceso de insensibilización y sacrificio de los animales, aunque no se pueden descartar problemas infecciosos.


The aim of this study was identify lesions found in lungs in 582 male bovine type export of Zebu breed, from different origins and slaughtered in the refrigerator Frigosinu in the municipality of Montería (Córdoba, Colombia). Sampling was conducted for 10 days systematically and selection of animals for evaluation was made randomly. Results were analyzed using descriptive statistics. During the study, lungs of 190 animals were found with some form of lung lesion, which corresponded to 32.6%. 67.4% of the sampled animals showed no obvious macroscopic alterations. Interstitial emphysema was the main cause of the confiscation, were observed in 64.7% of the animals, followed by hemorrhages (15.8%), lobar pneumonia (10.5%), pulmonary congestion (8.9%) and broncoaspiration of ruminal content (9.0%). However, the histopathologic diagnosis was not done in this study; therefore, presumptive diagnosis according to macroscopic lesions was established. Macroscopic analysis of the lungs evaluated, suggests that most of the alterations found in the lungs was due to flaws in the process of stunning and slaughter of animals, but can not be discard infectious problems in some alterations.

Supplementation with protected polyunsaturated fatty acids (PPUFA) has positive effects on cow reproduction. Therefore, the aim of this study was to evaluate the effects of adding a source of PPUFA to energy supplements for embryo recipient heifers on productive performance and plasma concentrations of progesterone, cholesterol and insulin. For this purpose, 44 Angus x Hereford embryo recipient heifers (average body weight = 385 kg) raised on pasture were studied in a completely randomized design. The effects of PPUFA added to isocaloric energy supplements for 60 days on production parameters and serum concentrations of cholesterol, progesterone and insulin were evaluated. The treatments consisted of individual supplementation with: 1) control (no supplement); 2) corn (corn, 70%; soybean meal, 30%); 3) PPUFA supplement (Megalac-E®, 30%; soybean meal, 20%; commercial ration, 50%). The treatments did not affect (P>0.05) dry matter intake, pregnancy rates, or serum insulin concentration. However, PPUFA supplement increased (P<0.05) serum cholesterol and progesterone concentrations when compared to the other treatments. Despite the lack of a difference (P>0.05) in dry matter intake between treatments, PPUFA supplement increased (P<0.05) average daily gain compared to the control and corn treatments. The inclusion of PPUFA in energy supplements offered to heifers used in an embryo transfer program increased average daily gain and serum concentrations of cholesterol and progesterone, but did not affect pregnancy rates...(AU)


A suplementação com ácidos graxos poliinsaturados protegidos (AGPIP) tem efeitos positivos sobre a reprodução de fêmeas bovinas. O objetivo deste estudo foi analisar os efeitos da adição de uma fonte de AGPIP em suplementos energéticos para novilhas receptoras de embriões e avaliar seu efeito sobre o desempenho produtivo e concentrações plasmáticas de progesterona, colesterol e insulina. Foram utilizadas quarenta e quatro novilhas Angus x Hereford receptoras de embriões (peso médio = 385 kg) em pastagem, em um delineamento experimental completamente casualizado. Foi avaliado o efeito de 60 dias de suplementação com ácidos graxos poliinsaturados protegidos da degradação ruminal dentro de suplementos energéticos isocalóricos sobre parâmetros produtivos e concentrações plasmáticas de colesterol, progesterona e insulina. Os tratamentos foram: 1) Controle (sem suplemento), 2) Milho (milho, 70%; farelo de soja, 30%), 3) AGPIP (Megalac-E®, 30%; farelo de soja, 20%; ração comercial, 50%). ABSTRACT

A neosporose tem grande importância em bovinos, e sua patogênese neste hospedeiro, em termos gerais, está esclarecida, porém, em equídeos é pobremente conhecida. Na Colômbia ainda não foram reportados anticorpos Anti-Neospora spp. em equídeos, sendo assim objetivou-se no presente estudo avaliar a ocorrência do parasito em jumentos (Equus asinus) de fazendas que apresentavam risco da doença nestes animais. Foram utilizados 56 animais no estado de Sucre (Colômbia), escolhidos aleatoriamente dentro das fazendas selecionadas. Utilizou-se um peptídeo recombinante originado de Neospora caninum (NcGRA1) para o diagnóstico por Dot-ELISA, e o soro foi diluído em 1:200. The neosporosis has great importance in cattle, and its pathogenesis in this host has been generally clarified, however, in horses, neosporosis is poorly known, and in Colombia anti-Neospora spp antibodies have not been reported. Therefore, the main objective in the present study was to evaluate the occurrence of this parasite in donkeys (Equus asinus) from farms that presented a risk of disease in these animals, as well as no health plan for them. Were used 56 animals randomly chosen inside selected farms in the state of Sucre (Colombia). A recombinant peptide originated from Neospora caninum (NcGRA1) was used for the diagnosis with Dot-ELISA and serum was diluted 1:200. ABSTRACT

La pythiosis cutánea equina es una enfermedad crónica, granulomatosa, pruriginosa y rápidamente progresiva, que ocurre en regiones con temperaturas que varían de clima templado a caliente (tropical y subtropical) y es causada por el oomiceto Pythium insidiosum. En la Costa Atlántica colombiana, la pythiosis se ve favorecida por las condiciones medioambientales y las inundaciones. El objetivo del presente estudio fue determinar la frecuencia de presentación de pythiosis cutánea en caballos de producción en explotaciones ganaderas del departamento de Córdoba, Colombia. A tal fin, se realizó un estudio descriptivo prospectivo entre los meses de noviembre (2012) y enero (2013); fueron evaluados clínica, epidemiológica e histopatológicamente 47 equinos con lesiones cutáneas granulomatosas compatibles con pythiosis, de un total de 175 equinos pertenecientes a siete explotaciones ganaderas ubicadas en zonas inundables de tres municipios del departamento de Córdoba. La frecuencia de la enfermedad fue de 26,9%. Las manifestaciones clínico-epidemiológicas, las características macroscópicas de las lesiones cutáneas y los hallazgos histopatológicos, indicaron una ocurrencia moderada de pythiosis cutánea en equinos de producción en explotaciones ganaderas de las zonas inundables de dicha zona colombiana.


The equine cutaneous pythiosis is a chronic, granulomatous, often pruriginous, rapidly progressive condition, caused by the oomycete Pythium insidiosum, and with occurrence in regions with temperatures ranging from temperate to hot (tropics and subtropics), as in the Colombian Atlantic Coast, where pythiosis is favored by environmental conditions and floods. The objective of this study was to determine the frequency of occurrence of cutaneous pythiosis in dairy horses of farms of the department of Cordoba, Colombia. We performed a prospective study in animals of convenience, between November of 2012 and January of 2013. We evaluated clinical, epidemiological and histopathologically 47 horses with granulomatous skin lesions compatible with pythiosis, from 175 equine belonging to 7 farms located in flood zones of 3 municipalities of the department of Cordoba, Colombia. The frequency of occurrence of equine cutaneous pythiosis in farms of Cordoba department, was 26.9%. It is concluded that clinical and epidemiological manifestations, macroscopic characteristics of the skin lesions and pathological findings, show a median pythiosis presence of equine dermal in 7 farms located in flood zones of the department of Cordoba, Colombia.

ABSTRACT

This article describes the process of cultural adaptation at the start of the implementation of the Parent Management Training intervention-Oregon model (PMTO) in Mexico City. The implementation process was guided by the model, and the cultural adaptation of PMTO was theoretically guided by the cultural adaptation process (CAP) model. During the process of the adaptation, we uncovered the potential for the CAP to be embedded in the implementation process, taking into account broader training and economic challenges and opportunities. We discuss how cultural adaptation and implementation processes are inextricably linked and iterative and how maintaining a collaborative relationship with the treatment developer has guided our work and has helped expand our research efforts, and how building human capital to implement PMTO in Mexico supported the implementation efforts of PMTO in other places in the United States.

Objetivo: explorar los beneficios de la terapia de la risa en un grupo de mujeres adultas pertenecientes a un centro gerontológico del Municipio de Envigado (Colombia). Metodología: estudio con enfoque cualitativo mediante entrevista semiestructurada a 10 adultas entre 59 y 97 años de edad, antes y después de la aplicación de 5 sesiones, una semanal, de terapia de la risa por parte del grupo Mediclaun ôpayasos hospitalariosõ. Resultados: muchos de los cambios percibidos en las adultas coinciden con la literatura en lo que respecta a una actitud más positiva, incremento de la confianza en el otro y la aceptación; expresan el pasado con menos dolor y un mayor agrado frente al acompañamiento familiar. Conclusiones: La terapia de la risa permite al adulto mayor encontrar fortalezas que puedan ayudarle a mejorar su presente, a su vez, lograr cambios en el individuo que se le revierten positivamente y a su relación con el entorno...


Objective: to explore the benefits of laughter therapy in a group of elderly women belonging to a gerontology center (Envigado, Colombia). Methodology: a qualitative study using a semi-structured interview with 10 adult women aged 59 to 97. The participants were interviewed before and after applying 5 weekly sessions of laughter therapy. Said therapy was administered by the Mediclaun "hospital clowns" group. Results: many of the changes observed in adult women are consistent with the results described in the literature in relation to a more positive attitude, increased confidence in people and acceptance; the participants expressed the past with less pain and had a greater liking towards family support. Conclusions: laughter therapy allows seniors to find the strength to improve their present situation, thus causing positive changes in their own self and in their relationship with their surroundings...
