ABSTRACT

OBJECTIVE: To investigate the effect of bronchodilator on the respiratory mechanics and pulmonary function of children and adolescents with cystic fibrosis. METHODS: Cross-sectional study on clinically stable children and adolescents with cystic fibrosis aged from six to 15 years. Participants underwent impulse oscillometry and spirometry evaluations before and 15 minutes after bronchodilator inhalation. The Kolmogorov-Smirnov test was applied to verify the sample distribution, and the Student's t-test and Wilcoxon test were used to compare the data before and after bronchodilator inhalation. RESULTS: The study included 54 individuals with a mean age of 9.7±2.8 years. The analysis showed a statistically significant improvement in impulse oscillometry and spirometry parameters after bronchodilator inhalation. However, according to the American Thoracic Society (ATS) and European Respiratory Society (ERS) recommendations (2020 and 2021), this improvement was not sufficient to classify it as a bronchodilator response. CONCLUSIONS: The use of bronchodilator medication improved respiratory mechanics and pulmonary function parameters of children and adolescents with cystic fibrosis; however, most patients did not show bronchodilator response according to ATS/ERS recommendations.

RESUMO Objetivo: Investigar o efeito do broncodilatador na mecânica respiratória e função pulmonar de crianças e adolescentes com fibrose cística. Métodos: Estudo transversal em crianças e adolescentes com fibrose cística clinicamente estáveis, com idade entre seis e 15 anos. Os participantes realizaram avaliações por meio do sistema de oscilometria de impulso e espirometria antes e 15 minutos depois da inalação de broncodilatador. Foi aplicado o teste Kolmogorov-Smirnov para verificar a distribuição da amostra. Para comparar os dados antes e depois do broncodilatador foram utilizados os testes t de Student e Wilcoxon. Resultados: Participaram do estudo 54 indivíduos com média de idade de 9,7±2,8 anos. Houve melhora estatisticamente significativa nos parâmetros do oscilometria de impulso e espirometria após a inalação com broncodilatador. No entanto, de acordo com recomendações da American Thoracic Society (ATS) e European Respiratory Society (ERS) (2020 and 2021), essa melhora não foi suficiente para classificar como reposta broncodilatadora. Conclusões: O uso de medicamento broncodilatador melhorou a mecânica respiratória e função pulmonar de crianças e adolescentes com fibrose cística, no entanto a maioria da amostra não apresentou resposta ao broncodilatador de acordo com as recomendações da ATS/ERS.

ABSTRACT

Introdução: intervenções musicais, como canto coral, têm caráter artístico e são capazes de promover benefícios psicológicos e fisiológicos. Não há estudos na literatura sobre a repercussão do canto coral em parâmetros do sistema respiratório de escolares cantores, em comparação aos não cantores. Objetivo: comparar parâmetros do sistema respiratório entre crianças e adolescentes cantores de coral e não cantores. Método: estudo observacional transversal quantitativo incluiu escolares de 7 a 14 anos, constituindo o grupo intervenção alunos de canto coral (GCC) pareados com escolares não cantores (GNC) como controles. Realizou-se avaliação antropométrica, seguida de espirometria e manovacuometria, segundo recomendações da American Thoracic Society. Para comparação entre os grupos considerou-se os valores espirométricos absolutos e valores preditos, assim como para força de musculatura respiratória. Aplicou-se teste de Shapiro-Wilk e conduziu-se os testes U de Mann-Witney e Teste-T independente, com nível de significância de 5%. Resultados: participaram 40 crianças (95% meninas), 20 em cada grupo, com idade média de 11,25±1,80 anos no GNC e 11,20±1,64 anos no GCC. Volume expiratório forçado no primeiro segundo em porcentagem do predito (VEF1%) se apresentou maior no GNC (98,58±12,62%), em comparação ao GCC (87,10±8,84%) (p=0,001), por sua vez, GCC apresentou maior valor absoluto de pico de fluxo expiratório (PFE) (GNC:4,21±0,99l/s x GCC: 4,95±1,29l/s; p=0,048). Conclusão: os escolares cantores de canto coral não apresentaram melhores parâmetros de função pulmonar e FMR, em comparação aos não cantores (AU)


Introduction: musical interventions, such as choral singing, are artistic in character and are capable of promoting psychological and physiological benefits. There are no studies in the literature on the repercussion of choir on respiratory system parameters of practicing students compared to non-singers. Objective: compare respiratory system parameters between choir and non-choir children and adolescents. Method: cross-sectional observational study included students from 7 to 14 years old, with the intervention group constituted students of choir singing (GCC) paired with non-singing students (GNC). Anthropometric assessment was performed, followed by spirometry and manovacuometry, according to ATS recommendations. For comparison between groups, absolute spirometric values and predicted values were considered, as well as respiratory muscle strength. Shapiro-Wilk test was applied and the Mann-Witney U test and independent T-test were conducted, with a significance level of 5%. Results: 40 children (95% girls) participated, 20 in each group, with a mean age of 11.25 ± 1.80 years in the GNC and 11.20 ± 1.64 years in the GCC. Forced expiratory volume in the first second as a percentage of predicted (FEV1%) was higher in GNC (98.58 ± 12.62%) compared to GCC (87.10 ± 8.84%) (p = 0.001), GCC presented the highest absolute value of peak expiratory flow (PEF) (GNC: 4.21 ± 0.99l/s x GCC: 4.95 ± 1.29l/s; p=0.048). Conclusion: schoolchildren singing in the choir did not present better parameters of pulmonary function and FMR, compared to non-singers (AU)

Objetivo: avaliar o comportamento de parâmetros do sistema respiratório durante internação para antibioticoterapia intravenosa (AIV) como tratamento da exacerbação pulmonar aguda (EPA) em escolares com fibrose cística (FC). Métodos: estudo do tipo analítico observacional, before-after, realizado no Hospital Infantil Joana de Gusmão, Florianópolis ­ Santa Catarina. Foram incluídas crianças com diagnóstico de FC, entre seis e 15 anos, em internação para tratamento da exacerbação pulmonar aguda, no início (T1), durante (T2) e ao final (T3) da internação foi conduzida avaliação de escores específicos de EPA, dados antropométricos e realizada avaliação dos parâmetros do sistema respiratório pelo sistema de oscilometria de impulso (IOS) e espirometria. Foram obtidos, em prontuário, dados de colonização bacteriana, genótipo, gravidade da doença (Escore de Schwachman-Doershuk-ESD) e espirometria mais recente em estabilidade clínica. Aplicou-se o teste Shapiro-Wilk para análise da distribuição dos dados e os testes ANOVA de medidas repetidas, teste de Friedman, teste T pareado e Wilcoxon, com nível de significância de 5%. Resultados: participaram 16 crianças/adolescentes (68.8% meninas, 12.88±1.67anos). Houve aumento dos parâmetros da espirometria e dados antropométricos (p<0.005) no T3, bem como redução dos escores de EPA e do X5 (p<0.005) no T3. Conclusão: os dados apresentados nesse trabalho mostram melhora dos escores de EPA, dados antropométricos, parâmetros da espirometria e do parâmetro de recolhimento elástico do IOS (X5).RESUMODescritores: Fibrose cística, Exacerbação dos sintomas, Testes de função pulmonar, Mecânica respiratória (AU)

The Shapiro-Wilk test was applied to analyze data distribution, and the repeated measure ANOVA, Friedman test, Tpaired test, and Wilcoxon test were performed to compare data, with a significance level set at 5%. Results: sixteen children/adolescents participated in the study (68.8% girls, 12.88±1.67 years old). The spirometric parameters, X5 parameter, and anthropometric data increased (p<0.005) and the APE scores decreased (p<0.005) at T3. Conclusion: APE scores, anthropometric data, spirometric parameters, and IOS elastic recoil parameter (X5) improved at the end of hospitalization (AU)

INTRODUCTION: The aim of this study was to investigate the immediate effect of coughing episodes and diaphragmatic breathing exercise (DBE) on respiratory mechanics of children/adolescents with cystic fibrosis (CF). METHODS: It is a cross-sectional analytical study that occurred in a reference center for children with CF. Forty-five children/adolescents with CF (60% male; mean age 10.22 ± 2.84 years old; mean forced expiratory volume in 1 second 73.74 ± 21.38% predicted) were divided into 3 groups according to the R5 parameter response to the DBE: G1 (increased R5), G2 (no change R5), and G3 (decreased R5). The children/adolescents performed 5 successive coughs and 10 DBE. The main outcome measures were the impulse oscillometry system (IOS) parameters evaluated before, during, and after the interventions. RESULTS: In the total sample, the IOS parameters (Z5, R5, and R20) were worse after coughing, and they did not change after the DBE. In the G1, the parameters were progressively worsening during the interventions. In the G2, they worsened after coughing and after the DBE. In the G3, they worsened after coughing; however, after the DBE, the IOS parameters have improved and returned close to the baseline. CONCLUSIONS: The children/adolescents with CF airway resistance got worse after coughing episodes, and 10 DBE repetitions did not affect the respiratory mechanics in most of the sample. Meanwhile, in the group with older children, the DBE worsened the respiratory mechanics, yet in the younger group it improved.

INTRODUCTION: It is recommended the association of inhalation therapies and physiotherapy on the management of cystic fibrosis (CF); however, it is still necessary to understand the effect on respiratory mechanics of these therapies combined. This study aimed to evaluate the immediate effect of inhalation with Dornase-Alfa (DNase) and hypertonic saline solution (HSS), as well as the impact of these inhalation therapies associated with an oral high-frequency oscillation (OHFO) physiotherapy device, on the respiratory mechanics of children and adolescents with CF. METHOD: Children/adolescents with CF were allocated into two groups (DNaseG and HSSG), where they performed inhalation therapy before using the OHFO device for physiotherapy. In each group, the Impulse Oscillometry System was conducted before and after inhalation therapy, and after OHFO. ANOVA was carried out to analyse the respiratory mechanics at different moments of DNaseG and HSSG. The Mann-Whitney test compared the immediate effect of each inhalation therapy and after OHFO. RESULTS: 30 children (6-14 years old) were studied. In DNaseG, the mean value of most oscillometric parameters decreased in the evaluated moments; in HSSG, only reactance showed an immediate increase. CONCLUSION: Children/adolescents with CF showed an immediate decrease in airway resistance and reactance after the use of DNase and associated with OHFO, indicating improvement. The inhalation with HSS has an immediate effect on peripheral airways.

INTRODUCTION: Asthma is characterized by a narrowing and inflammation of the bronchi, with symptoms of dyspnea, fatigue and exercise limitation. Physical therapy includes inspiratory muscle training and breathing exercises, given that an increase in inspiratory muscle strength and resistance can improve the symptoms of the diseaseOBJECTIVE: To describe the effects of inspiratory muscle training (IMT) and breathing exercises in children with asthmaMETHODS: This is a systematic review of the literature using the Cochrane, PubMed Scopus e Web of Science databases. The following descriptors were used: asthma, inspiratory muscle training, breathing exercises and child in Portuguese, English and Spanish. Two independent evaluators screened studies that used breathing exercises and IMT in children with asthmaRESULTS: Of a total of 312 titles, eight studies were included, of which six are randomized clinical trials and two are observational studies All the studies included breathing exercises, with the objective of adjusting breathing patterns and pulmonary ventilation, reducing pulmonary hyperinflation, bronchospasm and sensation of dyspnea. However, as these exercises were not performed solely, the effects of this intervention could not be verified. Two studies performed IMT and showed an increase in maximal respiratory pressureCONCLUSION: Breathing exercises are widely used in clinical practice as part of the management of asthma in children; however it is not possible to measure the effects in this population. IMT seems to improve inspiratory and expiratory muscle strength, but its indication in the pediatric population is not a standard procedure


INTRODUÇÃO: Asma é caracterizada por estreitamento e inflamação dos brônquios, com sintomas de dispneia, fadiga e limitação aos exercícios. O tratamento fisioterapêutico engloba treinamento muscular inspiratório e exercícios respiratórios, pois o aumento da força e resistência da musculatura inspiratória podem melhorar os sintomas da doençaOBJETIVO: Descrever os efeitos do treinamento muscular inspiratório (TMI) e dos exercícios respiratórios na criança com asmaMÉTODO: Revisão sistemática da literatura conduzida nas bases de dados Cochrane, PubMed, Scopus e Web of Science. Utilizou-se os descritores: asma, treinamento muscular inspiratório, exercícios respiratórios e criança e seus correlatos em inglês e espanhol. Dois avaliadores independentes elencaram estudos que realizaram intervenção com exercícios respiratórios e TMI na criança com asmaRESULTADOS: De um total de 312 títulos, foram incluídos oito estudos, dos quais seis são ensaios clínicos randomizados e dois observacionais. Todos os trabalhos incluíram exercícios respiratórios, com o objetivo de adequar o padrão respiratório e a ventilação pulmonar, reduzir a hiperinsuflação pulmonar, o broncoespasmo e a sensação de dispneia, porém como não foram realizados de forma isolada, comprometeu a verificação dos efeitos dessa intervenção. Dois estudos realizaram TMI e evidenciaram aumento nas pressões respiratórias máximasCONCLUSÃO: Exercícios respiratórios são muito utilizados na prática clínica como parte do manejo da criança com asma, entretanto ainda não é possível mensurar seus efeitos nessa população. O TMI parece melhorar a força muscular inspiratória e expiratória, mas sua indicação na população pediátrica ainda não é rotineira

OBJECTIVE: To verify whether the overlapping of ventilatory stimuli, resulting from playing with blowing toys, changes the respiratory mechanics of healthy schoolchildren. METHODS: Cross-sectional study with healthy schoolchildren aged seven to 14 years old from Florianópolis, Santa Catarina, Southern Brazil. Spirometric data were obtained, a health questionnaire and the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire were also applied. The procedure consisted of playing with the following blow toys in a random order: soap bubbles, party whistles and balloon. Before and after the intervention, the assessment of respiratory mechanics was carried out by impulse oscillometry - IOS (Erich Jaeger, Germany®). The ANOVA for repeated measures test was applied. RESULTS: 71 students of both genders with mean age of 9.7±2.1 years participated in the study. Results showed a progressive decrease of impedance (Z5), total airway resistance (R5) and resonance frequency (Fres) when the moment before the use of the first toy was compared with the moment after the third toy (Z5/p=0.048; R5/p=0.049; Fres/p=0.004). Fres also differed between the moment before the first and the second toy (p=0.048). After the use of each of the three blowing toys, the oscillometric parameters did not differ. CONCLUSIONS: The difference in oscillometric parameters of R5 before the use of each toy indicates that the overlap of ventilatory stimuli produced by them provided a reduction in the R5.

Métodos: Estudo transversal com escolares saudáveis de sete a 14 anos de idade, provenientes de Florianópolis, Santa Catarina, Brasil. Foram obtidos dados espirométricos e realizada aplicação de um recordatório de saúde e do questionário International Study of Asthma and Allergies in Childhood (ISAAC). A coleta de dados consistiu na aplicação dos brinquedos bola de sabão, língua de sogra e balão de forma aleatória. Antes e após a intervenção foi realizada a avaliação da mecânica respiratória por meio da oscilometria de impulso - IOS (Erich Jaeger, Germany®). Aplicou-se o teste de ANOVA para medidas repetidas. Resultados: Participaram do estudo 71 escolares de ambos os sexos, com média de idade de 9,7±2,1 anos. Houve redução progressiva na impedância respiratória a 5 hertz (Z5), na resistência total das vias aéreas (resistência a 5 hertz - R5) e na frequência de ressonância (Fres) ao comparar o momento antes do uso do primeiro e do terceiro brinquedo (Z5/p=0,048; R5/p=0,049; Fres/p=0,004). Fres também diferiu no momento antes do primeiro e do segundo brinquedo (p=0,048). Após o uso de cada um dos três brinquedos, os parâmetros oscilométricos não diferiram. Conclusões: Observando a diferença nos parâmetros oscilométricos da R5 antes do uso de cada um dos brinquedos, notou-se que a sobreposição de estímulos ventilatórios produzidos por eles proporcionou uma redução na R5.

ABSTRACT

ABSTRACT Purpose: to analyze whether deleterious oral habits can influence the number of attempts of forced spirometry maneuvers performed by healthy children. Methods: this observational and cross-sectional analytical study included 149 healthy children aged 6-12 years attending public and private schools in Florianópolis, SC, Brazil. A validated protocol was applied for the analysis of deleterious oral habits. The children were grouped according to the number of spirometry maneuvers needed to achieve successful spirometry results, as follows: G1) children who needed 3 maneuvers; G2) 4 maneuvers; G3) 5-8 maneuvers. Data were analyzed with the Kolmogorov-Smirnov test and the Kruskal-Wallis test was applied to compare quantitative variables between the groups. The Chi-square test was used to assess the association between the groups and qualitative variables. Results: there was no association between the number of attempts and the qualitative variables evaluated by the protocol. There was also no difference between the groups regarding quantitative variables for breastfeeding time, breastfeeding occurrence, use of pacifiers, and thumb sucking. Conclusion: the presence of DOH did not influence the number of forced spirometry maneuvers, performed by the healthy children in this study.


RESUMO Objetivo: analisar se hábitos orais deletérios influenciam no número de tentativas de manobras espirométricas executadas por crianças saudáveis. Métodos: pesquisa analítica observacional e transversal, incluiu 149 crianças saudáveis entre 6 e 12 anos, de escolas públicas e particulares de Florianópolis- SC/Brasil. Foi aplicado um protocolo validado para a análise de hábitos orais deletérios. As crianças foram agrupadas de acordo com o número de manobras espirométricas necessárias para realizar o exame: G1) crianças que realizaram as manobras forçadas em 3 tentativas; G2) em 4 tentativas e G3) 5 a 8 tentativas. Para análise de dados verificou-se a distribuição dos dados por meio do teste de Kolmogorov-Smirnov e aplicou-se Kruskal-Wallis para comparar o comportamento das variáveis quantitativas entre os grupos. O teste qui-quadrado foi utilizado para avaliação da associação entre os grupos e as variáveis qualitativas. Resultados: não houve associação entre a quantidade de tentativas e as variáveis qualitativas avaliadas pelo protocolo. Também não foi encontrada diferença entre os grupos com relação às variáveis quantitativas tempo de amamentação, ocorrência de aleitamento materno, uso de chupeta e sucção de dedo. Conclusão: a presença de HOD não influenciou o número de tentativas de manobras espirométricas forçadas, executadas por crianças saudáveis.

INTRODUCTION: Reference values and equations show strong variability and regional differences, despite a well-established role of the assessment of respiratory muscle strength (RMS) in children, with the objective to follow up on diseases that affect the function of respiratory muscles and enable adequate growth and development. OBJECTIVE: To describe reference values and prediction equations of maximal respiratory pressures for Brazilian children METHODS: Literature review was conducted using databases LILACS, MEDLINE and Science Direct, and descriptors established by DeCS of the Virtual Health Library: reference values, child, respiratory muscle strength, predictive equations and their respective synonyms in English RESULTS: Six clinical trials were carried out, which determined reference values for children in various Brazilian regions. There was a relationship among RMS values and other factors, such as age, height and body mass. However, there was a significant difference among the data in the Brazilian states and divergence among the predicted values CONCLUSION: There is a consensus on the relationship among RMS, anthropometric factors and regional influences. The articles studied reported diverging predicted values and reference equations


INTRODUÇÃO: Apesar da reconhecida importância da análise das pressões respiratórias máximas (PRM) em pediatria, tanto no seguimento de enfermidades que afetam a função dos músculos respiratórios, quanto da adequação do crescimento e desenvolvimento infantil, existe ampla variabilidade e regionalização quantos aos valores e equações de referência OBJETIVO: Descrever os valores de referência e equações preditivas das pressões respiratórias máximas para crianças brasileiras MÉTODO: Revisão de literatura realizada nas bases de dados LILACS, MEDLINE e Science Direct, utilizando os descritores estabelecidos pelo DeCS da Biblioteca Virtual em Saúde: valores de referência, criança, força muscular respiratória, equações preditivas, e seus sinônimos em inglês RESULTADOS: Encontrou-se seis ensaios clínicos, com valores de referência para a população pediátrica, realizados em diferentes regiões brasileiras. Observou-se relação entre os valores das PRM e fatores como idade, estatura e massa corporal. Porém, houve relevante diferença entre os dados nos estados brasileiros e divergência entre os valores preditos CONCLUSÕES: Há consenso ao relacionar as pressões respiratórias máximas e fatores antropométricos, bem como uma influência regional. Os valores preditos e as equações de referência foram divergentes entre os trabalhos encontrados PALAVRAS-CHAVE: valores de referência, criança, força muscular respiratória, equações preditivas
