ABSTRACT

Anthropometric studies of the scapula have been rare in Spanish populations, nevertheless they are of current interest in forensic anthropology for estimation of sex. Although the estimation of sex is usually carried out on the pelvis and skull, other measurements related to the scapula can be helpful when the skeletal remains are incomplete. Glenohumeral osteoarthritis development is influenced, among others, by the morphology of the scapula, which is one of the less studied aspects. We carried out a descriptive study of anthropometric parameters in a series of 157 scapulae (82 individuals) on bone remains dated to the 20th century from a population of Granada (Southern Spain). Seventy seven (49%) were right-side and 80 (51%) left-side; 72 (45.9%) were from males and 85 (54.1%) from females, and the mean age at death was 70.76±11.7 years. The objective was to develop a discrimination function for sex estimation based on anthropometric parameters of the scapula other than those considered to date, and to analyze the prevalence of glenohumeral osteoarthritis in relation to selected anthropometric parameters. A logistic regression model based on parameters of the upper-external segment of the scapula was done. The obtained formula: 1/1+e^ (- (-57.911 + 0.350*B + 0283*C + 0.249*b + 0.166*a +-0.100*ß) classifies male sex with 98.3% accuracy and female sex with 92.1%. Glenohumeral osteoarthritis was detected in 16.6% of individuals and was related to age (p<0.05), scapular length (p<0.05), glenoid width (p<0.05), glenopolar angle (p<0.05), and α angle (p<0.05) in bivariate analyses but showed no significant associations in multivariate analyses. This approach can be useful for anthropological-forensic identification when scapula remains are incomplete. Glenohumeral osteoarthritis is significantly associated with a smaller α angle.

PURPOSE: The present study aimed to identify, through a critical review of the literature, the success factors associated with the splinting of fixed prostheses on adjacent implants of the posterior sectors in partially edentulous patients compared with those not splinted. STUDY SELECTION: A MEDLINE strategy was implemented based on a research question to systematically search and extract information from databases (PubMed and Scopus) using MeSH terms/keywords identified for each domain. Systematic reviews, clinical and in vitro studies were selected and classified according to eligibility criteria based on the research question and level of evidence using the PRISMA flowchart. RESULTS: A total of 32 studies were selected for data extraction and analysis according to study design (three systematic reviews, 14 clinical studies, and 15 in vitro studies). Overall, the studies found no significant difference in the association between the survival rate and prosthesis type. In clinical studies, there have been no differences in marginal bone loss between splinted and non-splinted prostheses, and the influence of peri-implant status and restorative materials has been poorly evaluated. The distribution of stress and loads determined in the in vitro studies showed results that could favor splinted prostheses; however, are generally associated with implant design. CONCLUSIONS: The need for splinted or non-splinted adjacent implant-supported prostheses remains controversial. The reviewed evidence indicates that factors such as implant size and its relationship with coronal height could be important in decision-making.

ABSTRACT

Sexual experience improves copulatory performance in male rats. Copulatory performance has been associated with dendritic spines density in the medial prefrontal cortex (mPFC) and nucleus accumbens (NAcc), structures involved in the processing of sexual stimuli and the manifestation of sexual behavior. Dendritic spines modulate excitatory synaptic contacts, and their morphology is associated with the ability to learn from experience. This study was designed to determine the effect of sexual experience on the density of different types or shapes of dendritic spines in the mPFC and NAcc of male rats. A total of 16 male rats were used, half of them were sexually experienced while the other half were sexually inexperienced. After three sessions of sexual interaction to ejaculation, the sexually-experienced males presented shorter mount, intromission, and ejaculation latencies. Those rats presented a higher total dendritic density in the mPFC, and a higher numerical density of thin, mushroom, stubby, and wide spines. Sexual experience also increased the numerical density of mushroom spines in the NAcc. In both the mPFC and NAcc of the sexually experienced rats, there was a lower proportional density of thin spines and a higher proportional density of mushroom spines. Results show that the improvement in copulatory efficiency resulting from prior sexual experience in male rats is associated with changes in the proportional density of thin and mushroom dendritic spines in the mPFC and NAcc. This could represent the consolidation of afferent synaptic information in these brain regions, derived from the stimulus-sexual reward association.

ABSTRACT

The heuristic approach to decision-making holds that the selection process becomes more efficient when part of the information available is ignored. One element involved in selecting information is emotional valence. If emotional congruency is related to simplified decision-making strategies, then the interaction of this factor with task complexity should exist. The present study explored how factors of this nature influence decision-making efficiency. We hypothesized that emotional congruency would have a positive effect on task execution and that the magnitude of that effect would increase with greater task complexity because in that condition the amount of information to be processed is greater, meaning that a heuristic approach to the problem would be more efficient. We design a decision in browser decision-making task in which participants had to select emotional images to gain points. Depending on the correlation between emotional valence and in-task image value, we defined three emotional congruency conditions: direct, null, and inverse. Our results show that distinct types of emotional congruency have differential effects on behavior. While direct congruency-enhanced overall decision-making performance, inverse congruency interacted with task complexity to modify the pace at which task feedback affected behavior.

ABSTRACT

COVID-19 infection has been associated, particularly in severely ill patients requiring hospitalization, with a hypercoagulable state. The case presented herein was a 66-year-old man with SARS-CoV-2 infection who did not have any respiratory symptoms. He presented with the following clinical manifestations: portal vein and hepatic artery thrombosis, liver infarction, and a superimposed abscess of the liver. In this case, early detection and the administration of anticoagulants and antibiotics led to a significant improvement within weeks of the diagnosis. We encourage physicians to be aware of COVID-19-associated hypercoagulable state and its potential complications, regardless of the acuity of the presentation or the absence of respiratory symptoms.

ABSTRACT

Stress induces diverse effects on sexual behavior, ranging from enhanced execution to the complete abolishment of sexual interaction. However, it is not clear whether some characteristics intrinsic to the individual that experiences stress could also explain this differential effect. This study seeks to relate sexual execution to susceptibility to stress (as post-stress sexual motivation). To this end, we designed a three-session experimental paradigm. In the first session, male rats were allowed to copulate with a female. In the second, the male rats received electric foot shocks as they attempted to approach the female. The third and final session was used to determine the effects of stress on sexual behavior by separating the rats into two groups: a motivation-impaired group (rats that did not cross to achieve copulation), and an unimpaired group (rats that did cross). Mount latency was affected immediately by stress in both groups, though only the non-crossing group presented a reduced number of copulatory events. The rats that did not cross showed slower-paced sexual execution even before stress was applied compared to the rats that crossed. These results show that rats that are more susceptible to stress present higher ejaculation latency even before the application of stress.

INTRODUCTION: Respiratory conditions are the most common reason for admission to the neonatal unit for both term and premature newborns. It is known that nutritional imbalances during pregnancy affect the maturation and functional capacity of organs. OBJECTIVE: to describe the pulmonary histomorpholo- gy of newborn rats due to maternal sugar intake by light microscopy. MATERIAL AND METHOD: Twenty 4-week-old female Wistar rats divided into control and experimental groups with sucrose before and during pregnancy were used. At week 15, the females mated with males overnight. We recorded va lues from the body and lung weight of the newborns. The lungs were stained using Hematoxylin and Eosin, Masson's trichrome, Periodic acid-Schiff, and Verhoeff. RESULTS: Newborns from the experi mental group presented significantly lower body and lung weight (6.980 ± 0.493* g, 0.164 ± 0.022* g; *p < 0,05) compared with controls (7.854 ± 0.497 g, 0.189 ± 0.005 g). The lungs of the experimental group showed structural alterations in the lung parenchyma, as well as changes in glycogen deposits, collagen fibers, and elastin compared with the control group. CONCLUSION: Alterations in newborn lung growth and development are associated with maternal sucrose intake. It is important to re member that interventions on the maternal diet have beneficial effects for both the mother and the newborn.

ABSTRACT

The evaluation of external and internal stimuli permits the ongoing actualization of choice-related information and, thus, the association between stimuli and outcomes. This process is essential to decision-making as it allows constant adaptation to environmental changes in order to maximize gains and minimize losses. Reversal learning paradigms are used to study this process, which has been associated with prefrontal cortex activity (frontopolar, dorsolateral) in conjunction with posterior areas (parietal, temporal), due to their participation in integrating and processing the stimuli-reward relation. The aim of this study was to assess the cortical functionality associated with reversal learning during the decision-making process. The EEG activity of 22 young men was recorded while performing a decision-making task in a reversal learning condition compared to an initial learning condition. EEG data were analyzed during evaluation of the stimuli, before motor execution (formation of preferences), and during task feedback (outcome evaluation). The formation of preferences stage was characterized by a higher correlation of the alpha2 band between the parietal cortices. In the feedback stage of the reversal learning condition, a higher absolute power of the theta band in the left dorsolateral (F3), and a lower correlation of the alpha1 band between the right frontopolar and dorsolateral (Fp2-F4), as well as between the right frontopolar and temporal (Fp2-T4), were observed. The data obtained show that the EEG activity of the areas recorded changed in the evaluation of the stimuli information in the reversal learning condition during a decision-making task.

Decision-making is a process that allows adapting behavior in response to feedback to achieve a goal. Previous studies have suggested that the cerebral cortex shows different activation patterns in response to feedback. However, the effects of reward and punishment on learning contexts and decision-making are not clear. Thus, this experiment compared the effects of reward and punishment on behavior and the electroencephalographic activity of cortical areas related to decision-making in a no-risk context. Twenty healthy males were asked to perform a decision-making task under two conditions in which the goal was to finish in the shortest time possible. In the reward condition, the more points the participant accumulated the sooner the task ended, while in the punishment condition, the more points accumulated the longer the task lasted. Lower reaction times were found in the reward condition, characterized by a higher absolute power of the slow bands in almost all the cortices recorded. Changes in the interhemispheric correlation were also obtained in the comparison of the two feedback conditions. Results suggest that changes in the type of feedback affect cortical functionality and behavioral execution during decision-making, with the reward being related to a quick emotional response strategy and punishment associated with slower and, likely, more reasoned responses.

ABSTRACT

Decision-making is a process that allows individuals to choose an option or alternative in order to maximize a subjective gain or achieve a set goal by evaluating and establishing a preference based on contextual and internal information. Ernst and Paulus proposed a three-stage temporal division of this process: 1) the assessment and formation of preferences among possible options; 2) the selection and execution of an action; and 3) the experience or evaluation of an outcome. Each stage involves the participation of several brain regions, including the prefrontal, parietal, and temporal cortices. There are reports of distinct functionalities of these cortices for each stage of decision-making, but those studies focus on individual stages and do not provide any direct comparisons among them. Therefore, using a task that allows the clear temporal separation of the three stages of decision-making, we characterized the electroencephalographic activity (EEG) of those cortices in 30 healthy right-handed men during preference changes that occurred while performing a decision-making task. As the trials progressed, the preference for the stimulus shifted towards maximizing gains on the task. Forty trials sufficed to maintain these behavioral changes. Specific EEG patterns for each stage of decision-making were obtained, and it was possible to associate them with the cognitive processes involved in each one. These EEG data support the temporal division of the decision-making process proposed by Ernest and Paulus and show that the task designed could be a useful tool for determining behavioral and cerebral changes associated with stimuli preference during decision-making.

ABSTRACT

OBJECTIVE: Strongyloidiasis is a parasitic infection classified by the World Health Organization as a neglected tropical disease. Although predominantly asymptomatic, it can become a life-threatening disease in an immunocompromised host. Epidemiologic studies in the Western Hemisphere are scarce, but even more scarce are descriptions of the natural course of this disease. Our objectives were to identify the different manifestations and outcomes of Strongyloides stercoralis infection in the Hispanic veteran population in Puerto Rico. We also discuss the importance of pursuing a timely diagnosis in high-risk patients migrating from or traveling to endemic areas, regardless of their symptomatic status. METHODS: This was a single-center, retrospective record review study involving patients diagnosed with S. stercoralis via serology, stool samples, or organ biopsies, from 2008 through 2014. RESULTS: A total of 270 patients tested positive; 210 (77.8%) were asymptomatic. The mean age at diagnosis was 75.4 years old. Symptomatic patients had pulmonary (n = 25), gastrointestinal (n = 21), and dermatologic (n = 5) symptoms; 9 had multiple symptoms. Five had hyperinfection, mostly after treatment with systemic steroids or preexisting immunosuppression. The most common laboratory abnormality was eosinophilia. Reasons for testing were eosinophilia, asthma, diarrhea, screening for parasites, and unexplained skin rash. CONCLUSION: Our study highlights the importance of being aware of this potentially fatal infection, especially when treating patients traveling from endemic countries. It also highlights the importance of timely screening, diagnosis, and treatment of S. stercoralis infection in order to prevent potentially fatal outcomes, especially when considering immunosuppressive drugs.

ABSTRACT

OBJECTIVE: Inflammatory bowel disease (IBD) comprises a group of related conditions characterized by idiopathic inflammation of the gastrointestinal (GI) tract. Several studies have described the epidemiology of IBD in Puerto Rico (PR) but none have included the US military and Veteran population. The goal of this study was to describe the prevalence of IBD in this population as well as to describe patients' characteristics. METHODS: We conducted a retrospective analysis of 163 patients using data gathered from electronic medical records of Veterans who received care at the VA Caribbean Healthcare System (VACHS) and the diagnosis of IBD between October 1, 2010 and September 30, 2015. The prevalence out of 100,000 in our study was calculated for each Fiscal Year. RESULTS: Overall, the prevalence of IBD markedly increased among VA patients. Among 163 patients who met inclusion criteria, 7 (4.3%) patients had a diagnosis of undetermined IBD, 45 (27.6%) had Crohn's disease (CD) and 111 (68.0%) had ulcerative colitis (UC). This information was used to compare the epidemiologic data of IBD in the population receiving care at the VACHS with the epidemiologic data of IBD in the general population in PR. CONCLUSION: This is the first study to describe the prevalence of IBD and to describe additional characteristics of patients with IBD receiving care at the VACHS.

ABSTRACT

Sexual motivation requires the processing of sexual stimuli. The prefrontal cortex (PFC) and nucleus accumbens (NAcc) receive dopaminergic innervation from the ventral tegmental area (VTA). Both structures participate in processing stimuli, and their adequate functioning is modulated by dopamine and other neurotransmitters. This study was designed to determine the effect of inactivation of the VTA on sexual motivation, relative power (RP) and electroencephalographic (EEG) correlation of the PFC and NAcc in male rats. A total of 20 rats implanted with electrodes in the left medial prefrontal cortex (mPFC) and NAcc, and with bilateral cannulae in the VTA, were divided into two groups of 10 rats each, one injected with tetrodotoxin (TTX), the other with a vehicle solution (VEH). EEGs from the mPFC and NAcc were recorded during the awake-quiet state in the presence of either a receptive or non-receptive female. The TTX group showed a lower preference for the receptive female accompanied by a lower RP of the 8-13 and 14-30 Hz bands in the mPFC. Also, in the presence of the receptive female, the TTX group had a lower RP of the 8-13 Hz band in the NAcc, but a higher prefronto-accumbens correlation in the same band. These results provide evidence that VTA activity is necessary for the adequate functioning of the mPFC and NAcc and, therefore, also for the adequate processing of sexually-relevant stimuli that allows the induction and maintenance of sexual motivation in male rats.

ABSTRACT

OBJETIVO: Conocer los factores que influyen en los fracasos instrumentales lumbosacros después de fusiones espinopélvicas largas. MÉTODO: Estudio retrospectivo de pacientes diagnosticados con escoliosis, tratados quirúrgicamente por vía posterior, realizándo se fusión espinopélvica de T2 o T3 a ilíaco, utilizando tornillos pediculares e ilíacos. Se analizaron los fracasos instrumentales y su asociación con diferentes parámetros clínicos y radiológicos. RESULTADOS: Se presentan 44 pacientes con edad promedio de 24 años, con diferentes etiologías. El valor promedio preoperatorio de la curva mayor era de 74,2º, y en la revisión final la corrección promedio fue 67%. El desequilibrio anteroposterior y la inclinación pélvica, la cifosis torácica, la lordosis lumbar y el desequilibrio lateral mejoraron significativamente en la revisión final. Hubo 41% de fracasos de instrumentación, todos a nivel lumbosacro. Se encontró asociación significativa con más fracasos instrumentales en mayores de 17 años y en los que tenían deambulación autónoma. En 24 pacientes, se utilizó un solo tornillo ilíaco bilateralmente y en 20 pacientes, dos o más. Los dos grupos tuvieron una incidencia similar de fracasos. En el grupo de dos o más tornillos solo existieron roturas de barras sin desanclajes, ni lisis. El uso de implantes intersomáticos de L3 a S1 o cerclajes sublaminares con una tercera barra disminuyó la incidencia de fracasos. CONCLUSIONES: En esta serie se presentaron un 41% de fracasos instrumentales, todos localizados a nivel lumbosacro. Los pacientes con capacidad de deambulación autónoma y mayores de 17 años presentaron significativamente más fallas instrumentales. En los que se realizó fusión intersomática L3-S1 o una tercera barra con cerclajes sublaminares, disminuyó la incidencia.


OBJETIVO: Compreender os fatores que influenciam as falhas instrumentais lombossacrais depois de fusões espinopélvicas grandes. MÉTODOS: Estudo retrospectivo de pacientes com diagnóstico de escoliose, tratados cirurgicamente com fusão espinopélvica por via posterior, realizada de T2 ou T3 até o ilíaco, com parafusos pediculares e ilíacos. As falhas instrumentais foram analisadas, além de sua associação com diferentes parâmetros clínicos e radiológicos. RESULTADOS: Apresentamos 44 pacientes com média de idade de 24 anos, com diferentes etiologias. O valor médio da curva pré-operatória maior foi 74,2 graus, e na revisão final foi de 67%. O desequilíbrio anteroposterior e a inclinação pélvica, a cifose torácica, a lordose lombar e o desequilíbrio lateral melhoraram significantemente na revisão final. Houve falhas de instrumentação de 41%, todas na região lombossacral. Verificou-se associação significante com mais falhas instrumentais em pacientes com mais de 17 anos e nos que tinham deambulação independente. Em 24 pacientes, utilizou-se um único parafuso ilíaco bilateral e em 20 pacientes, foram usados dois ou mais. Os dois grupos tiveram incidência de falhas semelhante. No grupo de dois ou mais parafusos ocorreram apenas quebra de hastes, sem perda da ancoragem ou lise. Os implantes intersomáticos de L3 a S1 ou amarrias sublaminares com uma terceira haste diminuíram a incidência de fracassos. CONCLUSÕES: Nesta série ocorreram 41% de falhas instrumentais, todas localizadas na região lombossacral. Os pacientes com capacidade de marcha independente e com mais de 17 anos tiveram significantemente mais falhas instrumentais. Nos pacientes submetidos à fusão intersomática de L3-S1 ou com uma terceira haste com amarrias sublaminares, a incidência foi reduzida.


OBJECTIVE: To determine the factors that influence lumbosacral instrumentation failures following spino-pelvic fusions. METHODS: A retrospective study of patients diagnosed with scoliosis who underwent spinopelvic fusion via posterior, from T2 or T3 proximally to iliac crest, using pedicle and iliac screws. Instrumentation failures were analyzed, and the association of this complication with different parameters was studied. RESULTS: We present 44 patients, with an average age of 24 years, with different etiologies. The mean value of the largest preoperative major curve was 74.2°, and in the final review, it was 67%. The anterior-posterior imbalance, pelvic tilt, thoracic kyphosis, lumbar lordosis and lateral imbalance were significantly improved in the final review. There were instrumentation failures in 41% cases, all at the lumbosacral level. A significant association was found between increased instrumentation failures in patients over 17 years and in patients with independent walking ability. In 24 patients, a bilateral single iliac screw was used and in 20 patients, two or more screws were used. Both groups had a similar incidence of failures. In the group with two or more screws, only rod breakages occurred, without detachment or screw lysis. There was lower instrumentation failure incidence in the patients who underwent L3-S1 interbody fusion or a third rod attached with sublaminar compression hooks. CONCLUSIONS: This series had 41% instrumentation failures, all located at lumbosacral level. There were significant more instrumentation failures in patients with independent walking ability and those aged over 17 years. There was lower instrumentation failure incidence in the patients who underwent L3-S1 interbody fusion or a third rod attached with sublaminar compression hooks.

ABSTRACT

BACKGROUND: Arsenic exposure increases the risk of non-cancerous and cancerous diseases. In the Antofagasta region in Chile, an established relationship exists between arsenic exposure and the risk of cancer of the bladder, lung and skin. Platinum-based drugs are first-line treatments, and many works recognise selenium as a cancer-fighting nutrient. We characterised the short-term urinary excretion amounts of arsenic, selenium and platinum in 24-h urine samples from patients with lung cancer and those with cancer other than lung treated with cisplatin or/and carboplatin. As - Se - Pt inter-element relationships were also investigated. RESULTS: The amounts of platinum excreted in urine were not significantly different between patients with lung cancer and those with other cancers treated with cisplatin, despite the significant variation in platinum amounts supplied from platinum-based drugs. In general, the analytical amounts of excreted selenium were greater than those for arsenic, which could imply that platinum favours the excretion of selenium. For other types of cancers treated with drugs without platinum, excretion of selenium was also greater than that of arsenic, suggesting an antagonist selenium-anti-cancer drug relationship. CONCLUSIONS: Regards the baseline status of patients, the analytical amounts of excreted Se is greater than those for As, particularly, for cisplatin chemotherapy. This finding could imply that for over the As displacement Pt favours the excretion of Se. The analytical amounts of excreted Se were greater than those for As, either with and without Pt-containing drugs, suggesting an antagonist Se-anti-cancer drug relationship. However, it seemed that differences existed between As - Se - Pt inter-element associations in patients treated for lung cancer in comparison with those treated for cancer other than lung. Therefore, knowledge obtained in this work, can contribute to understanding the arsenic cancer mechanism and the As - Se - Pt inter-element association for lung cancer and other types of cancer, which in some cases respond at a linear mathematical model.

ABSTRACT

We measured the concentration of 12 metals in coastal waters of seven sites of San Jorge Bay in Antofagasta (northern Chile), in order to relate the presence of metals with the different uses of San Jorge Bay coastal border, and to evaluate the quality of the bay's bodies of water according to the proposed current Chilean Quality Guide for trace elements in seawater (CONAMA 2003). The results suggest that the coastal water of San Jorge Bay has very good quality according to the proposed regulation mentioned above. However, the distribution of metals such as Cu and Pb along the bay's coast line evidences a notorious effect of the industrial activity, which would involve different behavior patterns for some trace elements in some bodies of water, suggesting that the levels indicated in the environmental guideline of the Chilean legislation do not represent pollution-free environments.

ABSTRACT

Cu, Pb, and Hg concentrations were determined in surface sediment samples collected at three sites in San Jorge Bay, northern Chile. This study aims to evaluate differences in their spatial distribution and temporal variability. The highest metal concentrations were found at the site "Puerto", where minerals (Cu and Pb) have been loaded for more than 60 years. On the other hand, Hg does not pose a contamination problem in this bay. Cu and Pb concentrations showed significant variations from 1 year to another. These variations seem to be a consequence of the combination of several factors, including changes in the loading and/or storage of minerals in San Jorge Bay, the dredging of bottom sediments (especially at Puerto), and seasonal changes in physical-chemical properties of the water column that modify the exchange of metals at the sediment-water interface. Differences in the contamination factor and geoaccumulation index suggest that pre-industrial concentrations measured in marine sediments of this geographical zone, were better than geological values (average shale, continental crust average) for evaluating the degree of contamination in this coastal system. Based on these last two indexes, San Jorge Bay has a serious problem of Cu and Pb pollution at the three sampling locations. However, only Cu exceeds the national maximum values used to evaluate ecological risk and the health of marine environments. It is suggested that Chilean environmental legislation for marine sediment quality--presently under technical discussion--is not an efficient tool for protecting the marine ecosystem.
