ABSTRACT

Angioimmunoblastic T-cell lymphoma (AITL) is a rare type of non-Hodgkin lymphoma (NHL). We present a case of a 60-year-old female who attended the emergency department (ED) with fatigue, recurrent fever, weight loss, and adenopathy for six months. Laboratory findings showed anemia, lymphocytosis, eosinophilia, thrombocytosis, cholestasis, hypoproteinemia, and hypoalbuminemia. Abdominopelvic computed tomography (CT) revealed multiple adenopathies. A lymph node biopsy yielded inconclusive results in the outpatient clinic. Later, during admission, the patient underwent a positron emission tomography-computed tomography (PET-CT), revealing a cervical adenopathy cluster that was excised en bloc. Histology confirmed the diagnosis of AITL. The medical team initiated chemotherapy but opted for exclusive symptomatic treatment due to disease progression. The patient died six months after diagnosis. The fluctuating and nonspecific presentation of AITL can hinder and delay definitive diagnosis, therefore impacting treatment and prognosis.

ABSTRACT

Hospital managers should target occupational risks and harm prevention since this can contribute to the quality of life at work and patient safety. This article aims to elucidate the activity of prevention of occupational risks and injuries in the hospital setting based on analysis of historical and empirical contradictions of the activity system. An exploratory qualitative study grounded in the Cultural-Historical Activity Theory was conducted at a university hospital in the state of São Paulo. Data were collected between September 2021 and January 2022 via individual semi-structured interviews of 9 professionals from the Occupational Health and Safety services and of five hospital managers, involving 20 hours of field observation and document analysis. Despite the expansion of the object of prevention activity, the other elements of the activity system did not adapt to the new demands, causing incompatibilities and contradictions that compromised the attainment of the expected outcomes. The main response actions observed were centered on complying with regulatory items, such as team composition, medical examinations and others, that contribute little toward promoting occupational health and safety.


A prevenção de riscos e agravos à saúde dos trabalhadores nos hospitais deve ser foco dos gestores, pois contribui para a qualidade de vida no trabalho e a segurança do paciente. O objetivo deste artigo é compreender a atividade de prevenção de riscos e agravos à saúde dos trabalhadores no contexto hospitalar, a partir das contradições históricas e empíricas do sistema de atividade. Estudo qualitativo exploratório, ancorado na Teoria da Atividade Histórico-Cultural, desenvolvido em um hospital universitário do estado de São Paulo. Os dados foram coletados entre setembro de 2021 e janeiro de 2022 por meio de entrevistas semiestruturadas com nove profissionais do Serviço Especializado em Engenharia de Segurança e Medicina do Trabalho e cinco gestores do hospital; 20 horas de observação de campo; e análise documental. Apesar da expansão do objeto da atividade de prevenção, os demais elementos do sistema de atividade não se adaptaram às novas exigências, evoluindo com incompatibilidades e contradições que comprometeram o alcance dos resultados esperados. As principais ações de resposta observadas ficaram centradas em adequações a exigências de itens de normas, como composição de equipe, exames médicos e outras que pouco atuam na promoção e proteção da saúde.

ABSTRACT

OBJECTIVE: This work aims to review the existing use of robotics in plastic surgery. METHODS: A meticulous selection process identified 22 articles relevant to this scoping review. RESULTS: The literature on the use of robotics in plastic surgery is sparse. Nonetheless, this review highlights emerging benefits in microsurgery, breast reconstruction, and transoral surgery. CONCLUSION: This scoping review identifies critical articles reporting the emerging use of robotics in plastic surgery. While the scientific medical community has yet to extensively document its use, the available evidence suggests a promising future for robotics in this field.

ABSTRACT

OBJECTIVE: This systematic review and meta-analysis aimed to analyze the published randomized controlled trials (RCTs) that investigated the effects of exercise interventions on functioning and health-related quality of life following hospital discharge for recovery from critical illness. DESIGN: Systematic review and meta-analysis of RCTs. DATA SOURCES: We searched PubMed/MEDLINE, Cochrane Central Register of Controlled Trials, PEDro data base, and SciELO (from the earliest date available to January 2023) for RCTs that evaluated the effects of physical rehabilitation interventions following hospital discharge for recovery from critical illness. REVIEW METHODS: Study quality was evaluated using the PEDro Scale. Mean differences (MDs), standard MDs (SMD), and 95% confidence intervals (CIs) were calculated. RESULTS: Fourteen studies met the study criteria, including 1259 patients. Exercise interventions improved aerobic capacity SMD 0.2 (95% CI: 0.03-0.3, I2 = 0% N = 880, nine studies, high-quality evidence), and physical component score of health-related quality of life MD 3.3 (95% CI: 1.0-5.6, I2 = 57%, six studies N = 669, moderate-quality evidence). In addition, a significant reduction in depression was observed MD -1.4 (95% CI: -2.7 to -0.1, I2 = 0% N = 148, three studies, moderate-quality evidence). No serious adverse events were reported. CONCLUSION: Exercise intervention was associated with improvement of aerobic capacity, depression, and physical component score of health-related quality of life after hospital discharge for survivors of critical illness.

ABSTRACT

Objetivo: o presente estudo se destina a compreender a percepção das mulheres que fazem sexo com mulheres, referente ao cuidado que recebem nos serviços de saúde, no âmbito de sua saúde sexual e reprodutiva, a fim de verificar se o atendimento ofertado atende a integralidade de seu direito à saúde e, também, evidenciar o papel dos profissionais de saúde na garantia desses direitos. Metodologia: foi realizada revisão de literatura nacional para elaboração do instrumento de coleta de dados e pesquisa qualiquantitativa através de formulário e entrevista individual via internet. Resultados: Foram obtidas 287 respostas, as falas revelaram as seguintes categorias: A- Violência física sofrida em serviços de saúde, B- Negligência e falta de acolhimento em consultas ginecológicas e em serviços de saúde, C- Presunção da heterossexualidade nos serviços de saúde, D- Invisibilidade e violência moral e psicológica. Conclusão: evidenciou-se que mulheres que fazem sexo com mulheres ainda sofrem uma série de discriminações e violências nos serviços de saúde, ao buscarem cuidados integrais, principalmente no que tange aos seus direitos sexuais e reprodutivos.


Objective: this study is intended to understand the perception of women who have sex with women, regarding the care they receive in health services, within the scope of their sexual and reproductive health, in order to verify whether the care offered to them meets the integrality of their right to health and, also, highlight the role of health professionals in guaranteeing these rights. Methodology: A national literature review was carried out for the elaboration of the data collection instrument and qualitative and quantitative research through an individual form and individual interview via the internet. Results: 287 responses were obtained, the statements revealed the following categories: A- Physical violence suffered in health services, B- Negligence and lack of acceptance in gynecological consultations and health services, C- Presumption of heterosexuality in health services, D- Invisibility and, moral and psychological violence. Conclusion: it was evident that women who have sex with women still suffer a series of discrimination and violence in health services, when seeking comprehensive care, especially with regard to their sexual and reproductive rights.


Objetivo: este estudio tiene como finalidad conocer la percepción de las mujeres que tienen sexo con mujeres, respecto a la atención que reciben en los servicios de salud, en el ámbito de su salud sexual y reproductiva, con el fin de verificar si la atención que se les brinda cumple con la integralidad de su derecho a la salud y, además, destacar el papel de los profesionales de la salud en la garantía de estos derechos. Metodología: se realizó una revisión de literatura nacional para la elaboración del instrumento de recolección de datos e investigación cualitativa y cuantitativa a través de formulario individual y entrevista individual vía internet. Resultados: se obtuvieron 287 respuestas, los enunciados revelaron las siguientes categorías: A- Violencia física sufrida en los servicios de salud, B- Negligencia y falta de aceptación en las consultas ginecológicas y servicios de salud, C- Presunción de heterosexualidad en los servicios de salud, D- Invisibilidad y violencia moral y psicológica. ABSTRACT

Background: Soil animal communities include more than 40 higher-order taxa, representing over 23% of all described species. These animals have a wide range of feeding sources and contribute to several important soil functions and ecosystem services. Although many studies have assessed macroinvertebrate communities in Brazil, few of them have been published in journals and even fewer have made the data openly available for consultation and further use. As part of ongoing efforts to synthesise the global soil macrofauna communities and to increase the amount of openly-accessible data in GBIF and other repositories related to soil biodiversity, the present paper provides links to 29 soil macroinvertebrate datasets covering 42 soil fauna taxa, collected in various land-use systems in Brazil. A total of 83,085 georeferenced occurrences of these taxa are presented, based on quantitative estimates performed using a standardised sampling method commonly adopted worldwide to collect soil macrofauna populations, i.e. the TSBF (Tropical Soil Biology and Fertility Programme) protocol. This consists of digging soil monoliths of 25 x 25 cm area, with handsorting of the macroinvertebrates visible to the naked eye from the surface litter and from within the soil, typically in the upper 0-20 cm layer (but sometimes shallower, i.e. top 0-10 cm or deeper to 0-40 cm, depending on the site). The land-use systems included anthropogenic sites managed with agricultural systems (e.g. pastures, annual and perennial crops, agroforestry), as well as planted forests and native vegetation located mostly in the southern Brazilian State of Paraná (96 sites), with a few additional sites in the neighbouring states of São Paulo (21 sites) and Santa Catarina (five sites). Important metadata on soil properties, particularly soil chemical parameters (mainly pH, C, P, Ca, K, Mg, Al contents, exchangeable acidity, Cation Exchange Capacity, Base Saturation and, infrequently, total N), particle size distribution (mainly % sand, silt and clay) and, infrequently, soil moisture and bulk density, as well as on human management practices (land use and vegetation cover) are provided. These data will be particularly useful for those interested in estimating land-use change impacts on soil biodiversity and its implications for below-ground foodwebs, ecosystem functioning and ecosystem service delivery. New information: Quantitative estimates are provided for 42 soil animal taxa, for two biodiversity hotspots: the Brazilian Atlantic Forest and Cerrado biomes. Data are provided at the individual monolith level, representing sampling events ranging from February 2001 up to September 2016 in 122 sampling sites and over 1800 samples, for a total of 83,085 ocurrences.

Functional foods are gaining significant attention from people all over the world. When added to foods, probiotic bacteria can turn them healthier and confer beneficial health effects, such as improving the immune system and preventing cancer, diabetes, and cardiovascular disease. However, adding probiotics to foods is a challenging task. The processing steps often involve high temperatures, and intrinsic food factors, such as pH, water activity, dissolved oxygen, post-acidification, packaging, and cold storage temperatures, can stress the probiotic strain and impact its viability. Moreover, it is crucial to consider these factors during food product development to ensure the effectiveness of the probiotic strain. Among others, techniques such as microencapsulation and lyophilization, have been highlighted as industrial food functionalization strategies. In this review, we present and discuss alternatives that may be used to functionalize foods by incorporating probiotics and/or delivering bioactive compounds produced by probiotics. We also emphasize the main challenges in different food products and the technological characteristics influencing them. The knowledge available here may contribute to overcoming the practical obstacles to food functionalization with probiotics.

L'anorexie est une pathologie du vide de sens, qui presente une défaillance au niveau des processus de la pensée, avec des implications directes au niveau de la construction identitaire. Le Rorschach a été utilise comme instrument d'évaluation du fonctionnement mental et comme méthode d'analyse des transformations psychiques. Martha est une adolescente de dix-sept ans, avec deux hospitalisations, avec un diagnostic d'anorexie. L'analyse du Rorschach à mettre en évidence deux mouvements psychiques: la présence d'une forte tension pulsionnelle, mobilisée face à l'atteinte ressentie dans la relation Moi-Autre; la dévitalisation du Moi, par un fort mouvement d'annulation du Moi. La compréhension de ces mouvements a permis une intervention thérapeutique dans le but de restaurer son Être, de consolider son identité et de permettre un ensemble de transformations psychiques qui permettent la subjectivation et la croissance mentale.


Resumos A anorexia é uma patologia do vazio do sentido, que apresenta uma falha ao nível dos processos do pensamento, com implicações diretas ao nível da construção da identidade. O Rorschach foi usado como um instrumento para aceder ao funcionamento mental e como um método de análise das transformações psíquicas. Martha é uma adolescente de dezessete anos, com duas internações, com um diagnóstico de anorexia. A análise de Rorschach permitiu destacar dois movimentos psíquicos: a presença de uma forte tensão pulsional, mobilizada perante o ataque sentido na relação Eu-Outro; a desvitalização do Eu, através de um movimento de uma forte anulação do próprio eu. A compreensão desses movimentos permitiu uma intervenção terapêutica com o objetivo de restaurar o seu Ser, consolidar sua identidade e permitir um conjunto de transformações psíquicas que possibilitam a subjetivação e o crescimento mental.


Anorexia is a pathology of the emptiness of meaning, which presents a failure at the level of thought processes, with direct implications in self-identity. Rorschach test was used as an instrument to assess mental functioning and as a method to analyze the psychic transformations. Martha is a 17-year-old adolescent, with two previous hospitalizations, diagnosed with anorexia. Through Rorschach test, it was possible to highlight two movements: the presence of a strong instinctual drive, mobilized in the face of the perceived attack in the Self-Other relationship; the devitalization of the Self, through a strong annulment of the self. The understanding of such movements enabled a therapeutic intervention aimed at restoring her Being, consolidating her identity and enabling a set of psychic transformations that allow for subjectivation and mental growth.


La anorexia es una patología del vacío de sentido, que presenta una falla al nivel de los procesos del pensamiento, con implicaciones directas en la construcción de la identidad. El Rorschach se utilizó como instrumento para evaluar el funcionamiento mental y como método de análisis de las transformaciones psíquicas. Martha es una adolescente de 17 anos, con dos hospitalizaciones y diagnóstico de anorexia. El análisis del Rorschach reveló dos movimientos psíquicos: la presencia de una fuerte tensión instintiva, movilizada frente al ataque sentido en la relación Yo-Otro; la desvitalización del Yo, mediante un fuerte movimiento de anulación del Yo. La comprensión de estos movimientos permitió aplicar una intervención terapéutica con el objetivo de restaurar el Ser de la paciente, consolidar su identidad y permitir un conjunto de transformaciones psíquicas que permitan la subjetivación y el crecimiento psíquico.

Objective:to identify the knowledge produced about the actions/interventions of education in health related to Spotted Fever. Method:integrative literature review, carried out in the following databases: US National Library of Medicine/Medical Literature Analysis and Retrieval System Online, Web of Science, Science Direct, Cumulative Index to Nursing and Allied Health Literature, Scientific Electronic Library Online, Latin American and Caribbean Literature on Health Sciences and the Nursing Database. The inclusion criteria was original scientific articles and/or theoretical scientific articles, in English, Spanish and Portuguese and without time restriction. The search was conducted in January 2022. Results:five studies from Brazil, United States of Americaand Mexico were included. Changes in epidemiological indicators were observed in the two intervention studies. Conclusion: this study showed that health education for Spotted Fever is still a little explored field, highlighting the importance of educational actions for the prevention of the disease.

UU./Sistema de análisis y recuperación de literatura médica en línea, Web of Science, Science Direct, Cumulative Index to Nursing and Allied Health Literature, Biblioteca Científica Electrónica en Línea, Literatura Latinoamericana y del Caribe en Ciencias de la Salud y Base de Datos de Enfermería. Los criterios de inclusión fueron artículos científicos originales Y teóricos, en inglés, español y portugués. La búsquedase realizó en enero de 2022. Resultados: incluyeron cinco estudios, de Brasil, United States of America y México. Se observaron cambios en los indicadores epidemiológicos en dos estudios de intervención. Conclusión: la educación para la salud para la Fiebre Manchada es todavía un campo poco explorado, destacando la importancia de las acciones educativas para la prevención de la enfermedad.

ABSTRACT

The consumption of non-steroidal anti-inflammatory drugs (NSAIDs) have increased significantly in the last years (2020-2022), especially for patients in COVID-19 treatment. NSAIDs such as diclofenac, ibuprofen, and paracetamol are often available without restrictions, being employed without medical supervision for basic symptoms of inflammatory processes. Furthermore, these compounds are increasingly present in nature constituting complex mixtures discarded at domestic and hospital sewage/wastewater. Therefore, this review emphasizes the biodegradation of diclofenac, ibuprofen, and paracetamol by pure cultures or consortia of fungi and bacteria at in vitro, in situ, and ex situ processes. Considering the influence of different factors (inoculum dose, pH, temperature, co-factors, reaction time, and microbial isolation medium) relevant for the identification of highly efficient alternatives for pharmaceuticals decontamination, since biologically active micropollutants became a worldwide issue that should be carefully addressed. In addition, we present a quantitative bibliometric survey, which reinforces that the consumption of these drugs and consequently their impact on the environment goes beyond the epidemiological control of COVID-19.

ABSTRACT

Aortoenteric fistulas (AEFs) are a rare and deadly cause of gastrointestinal bleeding that can be easily overlooked, leading to massive bleeding. Secondary AEFs are more common than primary AEFs. An example of a secondary cause of anemia is postoperative hemorrhage due to a surgically placed aortic graft or after endovascular aneurysm repair. This report aims to increase the awareness of AEF as a differential diagnosis when anemia is detected. The clinical report presents a case of anemia in a 79-year-old man due to a secondary AEF, which occurred in a patient who had undergone abdominal aortic aneurysm surgery 10 years before. Surgical repair is considered the gold standard for AEF treatment; however, in this case, the patient was managed with medical therapy and discharged after two months.

O presente estudo trata-se de uma análise na literatura sobre o aspecto bioético da autonomia em relação a imunização contra Hepatite B e HPV na adolescência. Foi realizada uma revisão bibliográfica baseada no método prisma onde foram incluídos artigos em português e inglês os quais foram pesquisados nas plataformas Scientific Electronic Library Online (SciELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e National Library of Medicine (PUBMED), utilizando os seguintes descritores Hepatitis B Vaccines, bioethics, Humans. Os resultados demostraram a eficiência dos profissionais de saúde no contato com o indivíduo e o convencimento deste através de um diálogo aberto e criação de vínculos. Evidenciou-se ainda o estudo evidenciou o direito a autonomia progressiva do paciente, no qual este ganha o direito a algumas escolhas como as imunizações de doenças sexualmente transmissíveis, como é o caso da Hepatite B e HPV, no entanto é necessário que o adolescente seja instruído sobre a imunização. Assim sendo, reconhece-se o direito a autonomia progressiva do adolescente, referente a escolha das imunizações contra a Hepatite B e HPV, entretanto, deve-se considerar que estes indivíduos sejam instruídos sobre os benefícios individuais e/ou coletivos da vacinação, contemplados por atividades de educação em saúde por profissionais qualificados, objetivando ampliar a cobertura global de imunização com doenças que caracterizam problemas de saúde pública.


El presente estudio es un análisis de la literatura sobre el aspecto bioético de la autonomía en relación con la inmunización contra la Hepatitis B y el VPH en la adolescencia. Se realizó una revisión bibliográfica basada en el método del prisma, que incluyó artículos en portugués e inglés que fueron buscados en la Biblioteca Científica Electrónica en Línea (SciELO), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS) y la Biblioteca Nacional de Medicina (PUBMED), utilizando los siguientes descriptores Vacunas Hepatitis B, bioética, Humanos. Los resultados demostraron la eficiencia de los profesionales de la salud en contactar al individuo y convencerlo a través de un diálogo abierto y creación de vínculos. También se evidenció que el estudio evidenció el derecho del paciente a la autonomía progresiva, en la que adquiere el derecho a algunas opciones como las inmunizaciones de enfermedades de transmisión sexual, como la Hepatitis B y el VPH, sin embargo, es necesario que el adolescente sea educado sobre la inmunización. Por lo tanto, se reconoce el derecho del adolescente a la autonomía progresiva, en cuanto a la elección de las vacunas contra la Hepatitis B y el VPH, sin embargo, se debe considerar que estos individuos sean educados sobre los beneficios individuales y/o colectivos de la vacunación, contemplados por las actividades de educación en salud por profesionales calificados, con el objetivo de ampliar la cobertura mundial de inmunización con enfermedades que caracterizan los problemas de salud pública.


The present study is an analysis of the literature on the bioethical aspect of autonomy in relation to immunization against Hepatitis B and HPV in adolescence. A bibliographic review was carried out based on the prism method, which included articles in Portuguese and English which were searched on the Scientific Electronic Library Online (SCIELO), Latin American and Caribbean Literature on Health Sciences (LILACS) and the National Library of Medicine (PUBMED), using the following descriptors Hepatitis B Vaccines, bioethics, Humans. The results demonstrated the efficiency of health professionals in contacting the individual and convincing him/her through an open dialogue and creation of bonds. It was also evidenced the study evidenced the patient's right to progressive autonomy, in which he gains the right to some choices such as immunizations of sexually transmitted diseases, such as Hepatitis B and HPV, however it is necessary for the adolescent to be educated about immunization. Therefore, the adolescent's right to progressive autonomy is recognized, regarding the choice of immunizations against Hepatitis B and HPV, however, it must be considered that these individuals are instructed on the individual and/or collective benefits of vaccination, contemplated by health education activities by qualified professionals, aiming to expand the global coverage of immunization with diseases that characterize public health problems.

O estudo investiga os aspectos epidemiológicos da hepatite B em crianças e adolescentes, discutindo os resultados encontrados à luz dos princípios da bioética. Trata-se de um estudo transversal, retrospectivo, com base em uma série temporal de casos, cujos dados de notificação foram extraídos do Sistema de Informação de Agravos de Notificação (SINAN) e dados de vacinação do Sistema de Informação do Programa Nacional de Imunizações - SIPNI, no período de 2011 a 2020. Foram notificados 889 casos de hepatite B na população de estudo. A maioria dos participantes encontravam-se na faixa etária de 15 a 19 anos 738 (83%), do sexo feminino 549 (62%), cor da pele parda 637 (72%) residentes na zona urbana 490 (55%) e com ensino fundamental incompleto 279 (31%). A provável fonte de infecção foi por via sexual 450 (51%). A forma clínica mais comum foi a Hepatite Crônica 496 (56%). A maioria dos contaminados com Hepatite B foram indivíduos não vacinados 298 (34%). A luz da bioética os resultados demostram que a prática da vacinação pode contemplar os princípios bioéticos, já que o grau de autonomia depende de um tipo de intervenção planejada e quanto maior a relação entre benefício e ônus, tendo em conta os princípios de beneficência e não maleficência, menos decisiva é essa autonomia.


El estudio investiga los aspectos epidemiológicos de la hepatitis B en niños y adolescentes, discutiendo los resultados encontrados a la luz de los principios bioéticos. Se trata de un estudio transversal, retrospectivo, basado en una serie temporal de casos, cuyos datos de notificación fueron extraídos del Sistema de Información de Enfermedades de Declaración Obligatoria (SINAN) y datos de vacunación del Sistema de Información del Programa Nacional de Inmunizaciones SIPNI, desde 2011 hasta 2020. Se informaron 889 casos de hepatitis B en la población de estudio. La mayoría de los participantes tenían entre 15 y 19 años 738 (83 %), sexo femenino 549 (62 %), color de piel morena 637 (72 %), residentes en zona urbana 490 (55 ) y con educación primaria incompleta 279 (31 %). La fuente probable de infección fue a través de relaciones sexuales 450 (51%). La forma clínica más frecuente fue la Hepatitis Crónica 496 (56%). La mayoría de los infectados con Hepatitis B eran individuos no vacunados 298 (34%). A la luz de la bioética, los resultados muestran que la práctica de la vacunación puede contemplar principios bioéticos, ya que el grado de autonomía depende de un tipo de intervención planificada y mayor la relación entre beneficio y carga, teniendo en cuenta los principios de beneficencia y no maleficencia, menos decisiva es esta autonomía.


The study investigates the epidemiological aspects of hepatitis B in children and adolescents, discussing the results found in the light of bioethical principles. This is a cross-sectional, retrospective study, based on a time series of cases, whose notification data were extracted from the Notifiable Diseases Information System (SINAN) and vaccination data from the National Immunization Program Information System SIPNI, from 2011 to 2020. 889 cases of hepatitis B were reported in the study population. Most participants were aged between 15 and 19 years old 738 (83%), female 549 (62%), brown skin color 637 (72%) living in the urban area 490 (55%) and with incomplete elementary education 279 (31%). The probable source of infection was through sexual intercourse 450 (51%). The most common clinical form was Chronic Hepatitis 496 (56%). Most of those infected with Hepatitis B were unvaccinated individuals 298 (34%). In the light of bioethics, the results show that the practice of vaccination can contemplate bioethical principles, since the degree of autonomy depends on a type of planned intervention and the greater the relationship between benefit and burden, taking into account the principles of beneficence and not maleficence, the less decisive is this autonomy.

ABSTRACT

OBJECTIVES: Roux-en-Y gastric bypass (RYGB) promotes sustained weight loss, and the resulting new gastrointestinal anatomy can contribute to nutritional depletions. Folate deficiency is one of the most frequently observed nutritional deficiencies after RYGB. The aim of this study was to assess whether RYGB affects the expression of genes related to the intestinal folate metabolism pathway as an additional molecular mechanism contributing to its postoperative deficiency. METHODS: Biopsies from the duodenum, jejunum, and ileum of 20 obese women were collected before and 3 mo after RYGB. The expression of genes involved in intestinal folate metabolism was assessed by microarray and reverse transcriptase polymerase chain reaction (RT-qPCR). Folate intake (7-d food record) and plasma levels (electrochemiluminescence) also were measured. RESULTS: Compared with the preoperative phase, transcriptomic alterations were observed in all intestinal segments studied after RYBG, mainly marked by decreased expression of genes encoding folate transporters/receptors and increased expression of genes involved in folate biosynthesis (P < 0.05). Reduced folate intake and plasma folate levels were also observed simultaneously (P < 0.05). Plasma folate concentrations correlated inversely with intestinal FOLR2 and SHMT2 genes (P < 0.001). CONCLUSION: The present findings suggested that impaired expression of genes related to intestinal folate metabolism may contribute to the early systemic deficiency after RYGB and highlight a potential transcriptomic reprogramming of the intestine in response to RYGB to compensate for folate depletion induced by this surgical technique.

ABSTRACT

BACKGROUND: Understanding the variation between well and poorly adapted cattle breeds to local environments and pathogens is essential for breeding cattle with improved climate and disease-resistant phenotypes. Although considerable progress has been made towards identifying genetic differences between breeds, variation at the epigenetic and chromatin levels remains poorly characterized. Here, we generate, sequence and analyse over 150 libraries at base-pair resolution to explore the dynamics of DNA methylation and chromatin accessibility of the bovine immune system across three distinct cattle lineages. RESULTS: We find extensive epigenetic divergence between the taurine and indicine cattle breeds across immune cell types, which is linked to the levels of local DNA sequence divergence between the two cattle sub-species. The unique cell type profiles enable the deconvolution of complex cellular mixtures using digital cytometry approaches. Finally, we show distinct sub-categories of CpG islands based on their chromatin and methylation profiles that discriminate between classes of distal and gene proximal islands linked to discrete transcriptional states. CONCLUSIONS: Our study provides a comprehensive resource of DNA methylation, chromatin accessibility and RNA expression profiles of three diverse cattle populations. The findings have important implications, from understanding how genetic editing across breeds, and consequently regulatory backgrounds, may have distinct impacts to designing effective cattle epigenome-wide association studies in non-European breeds.

ABSTRACT

BACKGROUND: Stroke is a leading cause of death and disability worldwide. A major factor in brain damage following ischemia is excitotoxicity caused by elevated levels of the neurotransmitter glutamate. In the brain, glutamate homeostasis is a primary function of astrocytes. Amburana cearensis has long been used in folk medicine and seed extract obtained with dichloromethane (EDAC) have previously been shown to exhibit cytoprotective activity in vitro. The aim of the present study was to analyse the activity of EDAC in hippocampal brain slices. METHODS: We prepared a dichloromethane extract (EDAC) from A. cearensis seeds and characterized the chemical constituents by 1H and 13C-NMR. Hippocampal slices from P6-8 or P90 Wistar rats were used for cell viability assay or glutamate uptake test. Hippocampal slices from P10-12 transgenic mice SOX10-EGFP and GFAP-EGFP and immunofluorescence for GS, GLAST and GLT1 were used to study oligodendrocytes and astrocytes. RESULTS: Astrocytes play a critical role in glutamate homeostasis and we provide immunohistochemical evidence that in excitotoxicity EDAC increased expression of glutamate transporters and glutamine synthetase, which is essential for detoxifying glutamate. Next, we directly examined astrocytes using transgenic mice in which glial fibrillary acidic protein (GFAP) drives expression of enhanced green fluorescence protein (EGFP) and show that glutamate excitotoxicity caused a decrease in GFAP-EGFP and that EDAC protected against this loss. This was examined further in the oxygen-glucose deprivation (OGD) model of ischemia, where EDAC caused an increase in astrocytic process branching, resulting in an increase in GFAP-EGFP. Using SOX10-EGFP reporter mice, we show that the acute response of oligodendrocytes to OGD in hippocampal slices is a marked loss of their processes and EDAC protected oligodendrocytes against this damage. CONCLUSION: This study provides evidence that EDAC is cytoprotective against ischemia and glutamate excitotoxicity by modulating astrocyte responses and stimulating their glutamate homeostatic mechanisms.

ABSTRACT

Hepatocellular carcinoma (HCC) is the third leading cause of cancer-related deaths worldwide. Although many therapeutic options are available, several factors, including the presence of p53 mutations, impact tumor development and therapeutic resistance. TP53 is the second most frequently mutated gene in HCC, comprising more than 30% of cases. Mutations in p53 result in the formation of amyloid aggregates that promote tumor progression. The use of PRIMA-1, a small molecule capable of restoring p53, is a therapeutic strategy to pharmacologically target the amyloid state mutant p53. In this study, we characterize an HCC mutant p53 model for the study of p53 amyloid aggregation in HCC cell lines, from in silico analysis of p53 mutants to a 3D-cell culture model and demonstrate the unprecedented inhibition of Y220C mutant p53 aggregation by PRIMA-1. In addition, our data show beneficial effects of PRIMA-1 in several "gain of function" properties of mutant-p53 cancer cells, including migration, adhesion, proliferation, and drug resistance. We also demonstrate that the combination of PRIMA-1 and cisplatin is a promising approach for HCC therapy. Taken together, our data support the premise that targeting the amyloid-state of mutant p53 may be an attractive therapeutic approach for HCC, and highlight PRIMA-1 as a new candidate for combination therapy with cisplatin.
