ABSTRACT

INTRODUCTION: Endoscopy plays a fundamental role in inflammatory bowel disease (IBD), and becomes essential in diagnosis, treatment monitoring, and detection and management of complications. MATERIALS AND METHODS: The Pan American Crohn's and Colitis Organization (PANCCO) and the Inter-American Society of Endoscopy (SIED) appointed 22 Latin American experts in IBD to develop a consensus study using the modified Delphi method, based on the best available evidence. A working group of 22 members from 9 countries identified 15 topics and formulated 98 statements, who participated in 2 rounds of voting. It was defined as agreement of ≥80% of experts for each statement. RESULTS: After the voting of all the statements, 8 statements were obtained that did not reach 80% consensus among the participants, so the questions were reconsidered in the Coordinating Committee of the consensus with the participation of the expert reviewers of these questions and 7 final statements were voted again by all the experts in a second round and 1 was eliminated with consensus. After two rounds of voting, the experts reached consensus with literature review with the best available evidence, the most important issues were developed with scientific evidence supporting each of the statements around the topic of endoscopy in IBD. CONCLUSIONS: Consensus statements were developed and based on the best available evidence about endoscopy in inflammatory bowel disease.

ABSTRACT

Introduction: Ingesting foreign bodies is a common medical problem, especially in the emergency department. Some small studies describe experiences in this regard. Materials and methods: A descriptive retrospective study included patients with suspected ingestion of foreign bodies admitted to the gastroenterology and GI endoscopy service of the Clínica Universitaria Colombia between January 2007 and August 2020. Results: The age of occurrence of the event was 18 to 95 years, and the average age was 45 years. The foreign bodies ingested and found were variable. The most frequent was fish bones, representing 64.11% of the cases, followed by chicken bones and dietary impaction. Thirty-eight percent of patients required foreign body removal; the most frequently used tool was the foreign body forceps. The primary location was the esophagus in 12.53% of cases, followed by the cricopharynx in 11.18% and the hypopharynx in 10%. Conclusions: The Clínica Universitaria Colombia is a referral site for many gastroenterology emergencies due to its high technological level and extensive human resources. This paper probably describes the largest number of patients with this reason for consultation, which is why this retrospective descriptive study was designed. It shows the demographic characteristics, foreign body types, radiological and endoscopic findings, and associated complications, which help to provide a more accurate knowledge of this pathology.


Introducción: La ingesta de cuerpos extraños es un problema médico frecuente, especialmente en el servicio de urgencias. Existen algunos estudios pequeños que describen las experiencias al respecto. Materiales y métodos: Estudio descriptivo, retrospectivo, en el cual se incluyó a pacientes con sospecha de ingesta de cuerpos extraños, ingresados al servicio de gastroenterología y endoscopia digestiva de La Clínica Universitaria Colombia, entre enero de 2007 y agosto de 2020. Resultados: La edad de ocurrencia del evento se presentó en pacientes desde los 18 hasta los 95 años, y la edad promedio fue de 45 años. Los cuerpos extraños ingeridos y encontrados fueron variables; los más frecuentes fueron la ingesta de espinas de pescado, que representó el 64,11% de los casos, seguido por la ingesta de huesos de pollo y la impactación alimentaria. Un 38% de los pacientes requirieron la extracción de cuerpo extraño y la herramienta usada con mayor frecuencia fue la pinza de cuerpo extraño. La localización principal fue el esófago, en el 12,53% de los casos, seguido por la cricofaringe, en el 11,18%, y la hipofaringe, en el 10%. Conclusiones: La Clínica Universitaria Colombia es un sitio de referencia de una gran cantidad de urgencias en gastroenterología debido a su alto nivel tecnológico y al gran recurso humano que requieren. Este trabajo representa probablemente la cantidad más grande de pacientes con este motivo de consulta, razón por la que se diseñó este estudio descriptivo retrospectivo, que muestra las características demográficas, los tipos de cuerpo extraño, los hallazgos radiológicos y endoscópicos y las complicaciones asociadas, que son de utilidad para tener un conocimiento más real de esta patología.

ABSTRACT

INTRODUCTION: To practice colonoscopy in a Latin American country it is required to certify the acquisition of specific competence, but there is no consensus regarding the criteria that define it. Lack of training in digestive endoscopy skills is associated with an increased risk of diagnostic and therapeutic error; late diagnosis of cancer, increased risk of complications and incomplete procedures with negative consequences for patients. OBJECTIVE: To establish the performance criteria of the specific competence required by a specialist to perform colonoscopy, in order to impact with better results on the quality of health. MATERIALS AND METHODS: Qualitative study of consensus of experts. With semi-structured personal interviews, the information is documented to carry out the questionnaires that are applied in successive rounds until reaching consensus of more than 70% with the participation of more than 80% of the experts, using the Delphi method. RESULTS: Performance criteria are identified, which determine the specific competence required to perform colonoscopy with quality and safety. With significant findings due to the high percentage of agreement, they are presented grouped into 4 categories: general, before, during and after the procedure. Among the most important criteria that reached 100% agreement, are those related to cognitive, motor and integrative skills; quality, safety, screening, diagnostic and therapeutic techniques of this procedure. CONCLUSION: The criteria standardized by consensus, constitute a very valuable tool in the Latin American countries for the formation and evaluation of competences.

ABSTRACT

Objetivo: desarrollar una guía de práctica clínica con la evidencia más reciente para la tamización de cáncer de colon y recto en población asintomática de riesgo promedio, dirigida a pacientes, personal asistencial, administrativo y entes gubernamentales de cualquier servicio de atención en Colombia. Materiales y métodos: esta guía fue desarrollada por un equipo multidisciplinario con apoyo de la Asociación Colombiana de Gastroenterología, el Grupo Cochrane ITS y el Instituto de Investigaciones Clínicas de la Universidad Nacional de Colombia. Se desarrollaron preguntas clínicas relevantes y se realizó la búsqueda de guías nacionales e internacionales en bases de datos especializadas. Las guías existentes fueron evaluadas en términos de calidad y aplicabilidad; ninguna de ellas cumplió el criterio de adaptación, por lo que se decidió construir una guía de novo. El Grupo Cochrane realizó la búsqueda sistemática de la literatura. Las tablas de evidencia y recomendaciones fueron realizadas con base en la metodología GRADE. Las recomendaciones de la guía fueron socializadas en una reunión de expertos con entes gubernamentales y pacientes. Resultados: se desarrolló una guía de práctica clínica basada en la evidencia para la tamización del cáncer de colon y recto en Colombia. Conclusiones: la tamización de pacientes asintomáticos de riesgo promedio en Colombia en forma adecuada con estándares de calidad tiene el potencial de impactar la carga de cáncer de colon en el país.


Objective: To provide an evidence-based clinical practice guideline for the screening of colon and rectal cancer for patients, caregivers, administrative and government bodies at all levels of care in Colombia. Materials and Methods: This guide was developed by a multidisciplinary team with the support of the Colombian Association of Gastroenterology, Cochrane STI Group and Clinical Research Institute of the Universidad Nacional de Colombia. Relevant clinical questions were developed and the search for national and international guidelines in databases was performed. Existing guidelines were evaluated for quality and applicability. None of the guidelines met the criteria for adaptation, so the group decided to develop a de novo guideline. Systematic literature searches were conducted by the Cochrane Group. The tables of evidence and recommendations were made based on the GRADE methodology. The recommendations of the guide were socialized in a meeting of experts with government agencies and patients. Results: An evidence-based Clinical Practice Guidelines for the screening of colorectal cancer was developed for the Colombian context. Conclusions: The opportune detection of colon cancer would have an impact of the disease in Colombia.

ABSTRACT

La isquemia intestinal crónica es un cuadro poco frecuente que se encuentra asociado a una alta morbilidad y mortalidad. La causa más frecuente es la arterioesclerosis. Los pacientes sufren de dolor abdominal periumbilical o en epigastrio, que aparece de 10 a 30 minutos luego de la ingestión de alimentos. Presentamos un caso de isquemia intestinal crónica diagnosticado por colonoscopia con biopsias. Se practicó arteriografía que mostró estenosis de la arteria mesentérica superior y se trató con angioplastia percutánea e implante de stent. El paciente presentó completa mejoría clínica y endoscópica. Se comentan los hallazgos clínicos, endoscópicos, radiológicos y el procedimiento terapéutico. Creemos que el tratamiento con arteriografía, angioplastia y colocación de stent es una alternativa que puede mejorar los resultados clínicos disminuyendo la morbimortalidad en esta patología.


Chronic mesenteric arterial ischemia is an uncommon condition associated with a high morbidity and mortality. It is most commonly caused by atherosclerotic occlusive disease. Patients may suffer epigastric or periumbilical post prandial pain, ten to thirty minutes after eating. A case of chronic mesenteric ischemia is presented due to superior mesenteric artery stenosis. The diagnosis was performed with colonoscopy and biopsy. We present a case report of a patient with chronic mesenteric ischemia. Mesenteric arteriography was performed and documented estenosis of the mesenteric superior artery. Then, percutaneus arteriography with angioplasty and implant of stent was performed. The patient became completely asymptomatic and normal colon mucosa is observed in a control colonoscopy. The purpose of this report is to present the case, endoscopic, clinic and radiological features and to describe the percutaneous angioplasty and implant of stent.We believe that angioplasty treatment offers an improvement in this pathology with a low complication rate, and increasing patient confort degree.

ABSTRACT

Background: Colorectal cancer (CRC) is generally thought of as a disease of older persons; however a significant proportion of patients <40 years present with this disease. Many investigators have published single-institution series on CRC in the young, yet the data vary markedly. We performed a structured review of the current literature aiming to (1) characterize CRC in the young population and (2) determine how CRC in this population should be further addressed regarding detection and treatment. Data sources: A Medline literature search was completed. Articles were chosen to include those studies that examined patients <40 years old. A total of 55 articles were chosen from the search and review of the bibliographies. Conclusions: We found that CRC in the young population appears to be more aggressive, to present with later stage, and to have poorer pathologic findings. However, if detected early, young patients with Dukes’ stage A or B lesions have better overall 5-year survival rates. These findings emphasize the need for health care providers to have a heightened awareness when caring for this young population, particularly because excellent modalities exist to diagnose and treat colorectal cancer.
