ABSTRACT

BACKGROUND: Aseptic loosening and osteolysis are rarely associated with cemented posterior-cruciate-substituting total knee replacements. Consequently, there is a paucity of information on this topic. METHODS: After a mean follow-up interval of fifty-six months (range, thirty-seven to eighty-nine months), sixteen (2.9%) of 557 posterior-cruciate-substituting primary total knee replacements were revised by a single surgeon because of loosening and osteolysis. Clinical, radiographic, and retrieval analyses were conducted to determine the mechanism of loosening and to identify associated risk factors. RESULTS: All sixteen knees (fifteen patients) were rated as good or excellent at one year after the primary replacement, with mean clinical and functional Knee Society scores of 95 and 86 points, respectively. Nine of the fifteen patients who had a revision because of loosening and osteolysis had had a total knee arthroplasty on the contralateral side compared with only 18% of the patients who did not have a revision (p = 0.026). No evidence of transmission of substantial anteroposterior stresses from the posterior-cruciate-substituting mechanism was found. All twelve retrieved knee implants, however, had damage to the lateral and medial side walls of the polyethylene posterior-cruciate-substituting post. Damage to the inferior surface of the polyethylene inserts had a rotational pattern, with the axis of rotation in the medial compartment. Surface damage in a rotational pattern was also present on the superior and inferior surfaces of the titanium tibial base-plates. CONCLUSIONS: In the knees in our study, rotational forces were generated by impingement of the side walls of the intercondylar box on the polyethylene post. Such box-post impingement can occur throughout the range of motion. Rotational stresses are transmitted to the modular interfaces and to the metal-cement interfaces, resulting in loosening and osteolysis. A reduction in rotational constraint would be desirable. Patients with bilateral total knee replacement may be at increased risk for this type of loosening.

ABSTRACT

Oral non-steroidal anti-inflammatory drugs (NSAIDs) are effective pharmacotherapy for a wide variety of painful, inflammatory disorders. Development of an efficient means of topical administration of NSAIDs could increase local soft-tissue and joint concentrations while reducing systemic distribution of the drug, thereby reducing side-effects. With this in mind we studied the effects of a novel topical penetration enhancer for lipophilic compounds, a trans-phase delivery system (TPDS), a solution of benzyl alcohol, isopropanol and acetone, on the distribution of indomethacin in various tissues locally and remote from the site of application. We compared the TPDS with a 50:50 (v/v) mixture of propylene glycol and ethanol, a commonly used penetration enhancer, and with oral administration. We found that the TPDS was significantly superior to the other approaches at achieving high local-tissue concentrations in the vicinity of the site of application. In addition, comparison of these two carrier systems seems to clarify the different aqueous and hydrophobic pathways of drug penetration which emerge from various experimental findings and theoretical considerations. Our results suggest that this non-aqueous solvent system, and benzyl alcohol in particular, because of its unique physicochemical and solvating characteristics, might be able to deliver therapeutic levels of indomethacin to tissues close to the site of application in a safer and more effective manner than presently accepted forms of delivery.

ABSTRACT

Open plate osteosynthesis for high energy tibial plateau fractures with dissociation between the metaphysis and diaphysis has been plagued with frequent soft tissue complications. The Harbor-University of California at Los Angeles Medical Center's experience with small wire external fixation supplemented by limited internal fixation is examined. This alternative method of adequate stable fixation offers the advantage of minimal soft tissue compromise. Twenty-four patients with Schatzker Type VI tibial fractures were treated with small wire external fixation. Supplementary limited internal fixation was used with percutaneous screws in 10 patients and with open reduction in one patient. Sixteen patients had isolated fractures, and eight others suffered multiple injuries. Minimum followup was 12 months. All fractures healed. Complications included one septic knee, two infections at screw sites, and one 10 degrees knee flexion contracture. One knee had Grade 3 radiographic arthrosis, five had Grade 2, 10 had Grade 1, and eight showed no arthrosis. The outcomes (Knee Society clinical rating system) of this study compare favorably with outcomes described in reports published previously for this type of fracture, despite inclusion of eight multiply injured patients. This technique preserves the goals of early range of motion and stable fixation for these devastating injuries, while decreasing the observed major wound complications and nonunion rates. However, longer followup may reveal higher arthrosis rates, specifically in those fractures that were not anatomically reduced.
