ABSTRACT

BACKGROUND:Patients with femoral intertrochanteric fracture may have nutritional risks during proximal femoral nail anti-rotation surgery,and a clinical predictive model is established based on nutritional assessment tools. OBJECTIVE:To establish the nomogram of the nutritional risk prediction model for patients with intertrochanteric fracture after proximal femoral nail anti-rotation fixation and evaluate the accuracy of the model. METHODS:From December 2018 to July 2022,patients with femoral intertrochanteric fractures who underwent proximal femoral nail anti-rotation fixation in First Department of Orthopedics,Second Affiliated Hospital of Baotou Medical College were selected as the study subjects.The nutritional risk status of patients was assessed using nutritional risk screening 2002.Logistic regression was used to build the model.The receiver operating characteristic curve,Calibration plot calibration curve,and Hosmer-Lemeshow goodness-of-fit test were used to evaluate the discrimination,calibration and clinical applicability of the prediction model.The model was visualized using Nomogram diagrams. RESULTS AND CONCLUSION:(1)Seventy-six patients were included according to the inclusion criteria.The incidence of nutritional risk was 80%after being assessed using nutritional risk screening 2002.(2)The results of Logistic regression analysis demonstrated that age≥77 years old,body mass index<22.80 kg/m2,upper arm circumference<25.01 cm,hemoglobin<98.51 g/L,albumin<31.61 g/L,and prealbumin<138.56 g/L were all independent risk factors for nutritional risk(P<0.05).(3)The area under the receiver operating characteristic curve was 0.919(95%CI:0.843-0.994).(4)The results of the Hosmer-Lemeshow goodness-of-fit test displayed that nutritional risk prediction value was high.(5)It is concluded that the nutritional risk assessment model constructed in this study has a good degree of discrimination and calibration,and has a certain predictive ability,which can be used as a reference tool for nutritional risk assessment of patients after proximal femoral nail anti-rotation fixation.

ABSTRACT

Objective:To investigate the change trend of etiological burden of disease of liver cancer in the Chinese population from 1990 to 2019.Methods:The descriptive epidemiologic method was conducted. Based on the Global Burden of Disease data from the Institute for Health Metrics and Evaluation at the University of Washington, the data related to liver cancer burden caused by hepatitis B virus (HBV) infection, hepatitis C virus (HCV) infection, alcohol, nonalcoholic steatohepatitis (NASH) and other factors, including number of new cases, the crude incidence rate, age-specific incidence rate, number of deaths, crude mortality rate and age-specific mortality rate, in the Chinese population from 1990 to 2019 were collected. The age-standardized rate was calculated based on the world standardized population structure in 2019 from the Global Burden of Disease data. Observation indicators: (1) the incidence of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019; (2) the mortality of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019; (3) the change trend of age-specific incidence rate of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019; (4) the age-specific mortality rate of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019. Count data were expressed as absolute numbers, percentages and ratio. Based on the junction point regression model, the Joinpoint software (V.4.9.1.0) was used to calculate the annual percentage change, average annual percentage change (AAPC) and 95% confidence intervals ( CI) of age-specific incidence rate and age-specific mortality rate of liver cancer caused by different etiologies. Results:(1) The incidence of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019. From 1990 to 2019, the number of new cases of liver cancer in Chinese population decreased from 236 825 to 210 462, and the crude incidence rate decreased from 20.01/100,000 to 14.80/100,000. The new cases of liver cancer caused by HBV infection, HCV infection and other factors showed a downward trend, and the absolute change rates were ?14.76%, ?3.98% and ?26.67%, respectively. The new cases of liver cancer caused by alcohol and NASH showed a increase trend, and the absolute change rates were 9.31% and 13.91%, respectively. (2) The mortality of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019. From 1990 to 2019, the number of deaths of liver cancer in Chinese population decreased from 232 449 to 187 700, and the crude mortality rate decreased from 19.64/100,000 to 13.20/100,000. The number of deaths of liver cancer caused by HBV infection, HCV infection and other factors showed a down-ward trend, and the absolute change rates were ?23.34%, ?10.99% and ?33.75%, respectively. The number of deaths of liver cancer caused by alcohol showed a slow downward trend, and the absolute change rate was ?0.51%. The number of deaths of liver cancer caused by NASH showed a increase trend, and the absolute change rate was 6.03%. (3) The change trend of age-specific incidence rate of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019. From 1990 to 2019, the AAPC of age-specific incidence rate of liver cancer caused by HBV infection, HCV infection, alcohol, NASH and other factors was ?3.61%(95% CI as ?4.10% to ?3.11%), ?3.57%(95% CI as ?3.99% to ?3.14%), ?2.79%(95% CI as ?3.24% to ?2.33%), ?2.65%(95% CI as ?3.09% to ?2.21%) and ?3.62%(95% CI as ?4.05% to ?3.19%), respectively. (4) The age-specific mortality rate of liver cancer caused by different etiologies in the Chinese population from 1990 to 2019. From 1990 to 2019, the AAPC of age-specific mortality rate of liver cancer caused by HBV infection, HCV infection, alcohol, NASH and other factors was ?3.92%(95% CI as ?4.42% to ?3.41%), ?3.90%(95% CI as ?4.45% to ?3.35%), ?3.15%(95% CI as ?3.71% to ?2.58%), ?2.86%(95% CI as ?3.34% to ?2.38%) and ?4.09%(95% CI as ?4.64% to ?3.55%), respectively. Conclusions:From 1990 to 2019, the liver cancer burden of the Chinese population shows an overall downward trend, in which the liver cancer burden caused by HBV and HCV infection decreases the most, but HBV and HCV infection is still the main reason for the heavy burden of liver cancer. The age-specific incidence rate and age-specific mortality rate of liver cancer caused by alcohol and NASH show a downward trend, but the number of new cases of liver cancer caused by alcohol and NASH shows significant growth. The liver cancer burden caused by other factors shows a downward trend.

ABSTRACT

Objective:To investigate the clinical efficacy of percutaneous endoscopic interlaminar discectomy for lumbar disc prolapse.Methods:The clinical data of 60 patients with lumbar disc prolapse who underwent surgery at Luzhou People's Hospital between March 2019 and September 2021 were retrospectively analyzed. These patients were divided into an open lumbar discectomy (OLM) group ( n = 29, receiving treatment through a posterior approach) and a percutaneous endoscopic interlaminar discectomy (PEID) group ( n = 37). Perioperative conditions were compared between the two groups. The Visual Analogue Scale (VAS) and Oswestry Disability Index scores of the leg were recorded before surgery and 1 week, 1, 3, 6 and 12 months after surgery. At the final follow-up, clinical efficacy was assessed using the modified MacNab criteria. Results:The surgical time, blood loss, and length of hospital stay were (96.55 ± 15.18) minutes, 120.0 (100.0, 180.0) mL, and 10.0 (9.5, 12.0) days in the OLM group, while these values were (77.30 ± 11.03) minutes, 20.0 (15.0, 22.5) mL, and 6.0 (5.0, 7.0) days in the PEID group, respectively. Significant differences were observed in these indexes between the two groups ( t = 5.97, Z = -7.00, -6.68, all P < 0.001). At 1 week, 1 month, 3 months, 6 months, and 12 months after surgery, there was no significant difference in VAS score between the two groups (all P > 0.05). At 1 week, 1 month, and 3 months after surgery, the Oswestry Disability Index score in the PEID group was 12.0 (10.0, 24.0) points, 6.0 (9.0, 13.0) points, and 2.0 (4.0, 8.0) points, respectively, which were significantly lower than 24.0 (16.0, 31.0) points, 16.0 (10.0, 21.0) points, and 8.0 (8.0, 12.0) points in the OLM group, respectively ( Z = -3.64, -3.79, -3.26, all P < 0.05). According to the modified MacNab criteria for final follow-up assessment, the excellent and good rate was 86.21% (25/29) in the OLM group and 89.19% (33/37) in the PEID group; there was no significant difference in excellent and good rate between the two groups ( P > 0.05). Conclusion:The clinical efficacy of PEID in the treatment of lumbar disc prolapse is satisfactory.

ABSTRACT

Objective:To investigate the medium to long-term clinical outcomes of arthroscopic treatment for femoral acetabular impingement (FAI) without capsular closure.Methods:A retrospective analysis was performed on 62 patients (63 hips) with FAI who underwent hip arthroscopy in Huashan Hospital Affiliated to Fudan University from February 2015 to June 2018. There were 29 male patients (30 hips) and 33 female patients (33 hips) with an average age of 43.08±8.88 years. Thirty-two surgeries were performed on the left hip, thirty-one on the right, and one patient underwent bilateral surgery. All 62 patients underwent arthroscopic surgery without capsular closure. The preoperative and postoperative final follow-up assessments included the hip outcome score-activity of daily living scale (HOS-ADL), hip outcome score-sport specific subscale (HOS-SSS), Harris hip score (HHS), University of California at Los Angeles (UCLA) score, Non arthritic hip score (NAHS), visual analogue scale (VAS) for pain, and observation of complications.Results:All the patients completed the operation successfully, and the surgical incisions healed well. No complications such as wound infection, local incisional hematoma, local skin redness, lateral femoral cutaneous nerve injury, or perineal crush injury occurred in any one case. All 62 patients were followed up for 71 (60, 100) months. The VAS before surgery was 6 (0, 10) points and 1 (0, 6) point at the last follow-up, with statistical significance ( Z=-6.81, P<0.001). Preoperative scores were as follows: HOS-ADL: 78.03±5.30; HOS-SSS: 59.88±12.51; HHS: 78.98±7.50; UCLA: 2.00 (2.00, 5.00); NAHS: 83.75 (81.25, 97.50). Postoperative final follow-up scores were as follows: HOS-ADL: 94.77±4.79; HOS-SSS: 79.89±9.07; HHS: 90.75±5.59; UCLA: 4.00 (3.00, 6.00); NAHS: 92.50 (88.75, 98.75). At the final follow-up, HOS-ADL ( t=19.10, P<0.001), HOS-SSS ( t=12.29, P<0.001), HHS ( t=11.21, P<0.001), UCLA ( Z=-6.92, P<0.001), and NAHS ( Z=-6.52, P<0.001) showed significant improvement. Until the end of the follow-up period, no patients underwent revision surgery or hip arthroplasty on the treated hip. Conclusion:For patients with FAI without developmental dysplasia of the hip and generalized ligament laxity, arthroscopic treatment without capsular closure yields favorable medium to long-term clinical outcomes.

ABSTRACT

Objective:To compare the efficacy of arthroscopic all-inside ligament repair with suture augmentation and ligament reconstruction with tendons in the treatment of chronic ankle instability with poor remnant quality of the anterior talofibular ligament (ATFL) tissue.Methods:A retrospective cohort study was conducted to analyze the clinical data of 37 patients with chronic ankle instability treated at Huashan Hospital Affiliated to Fudan University from January 2018 to August 2020, including 34 males (34 ankles) and 3 females (3 ankles); aged 18-57 years [(32.2±7.2)years]. The time from injury to operation ranged from 3-360 months [48(12, 120)months]. All patients underwent arthroscopic all-inside ankle stabilization surgery, of which 19 underwent ligament repair with suture augmentation (augmented repair group) and 18 underwent traditional ligament reconstruction with allograft/autograft tendons (tendon reconstruction group). The degree of ATFL injury and intra-articular lesions (osteophytes, loose bodies and cartilage damage) were recorded during the operation. The Karlsson scale and Tegner scale were evaluated before operation and at the last follow-up. The number of patients who were able to complete partial weight-bearing/return to normal walking/return to sports postoperatively and the time required were compared between the two groups. Postoperative complications were observed.Results:All patients were followed up for 12-32 months [21(16, 28)months]. There were no significant differences in the degree of ATFL injury and intra-articular lesions (osteophytes, loose bodies and cartilage damage) seen during the operation between the two groups (all P>0.05). At the last follow-up, the Karlsson score in augmented repair group and tendon reconstruction group [95.0(90.0, 98.5)points and 95.0(87.8, 99.3)points] was significantly higher than the preoperative level [65.0(51.0, 75.0)points and 65.0(53.3, 78.0)points] (all P<0.01). At the last follow-up, the Tegner score in augmented repair group and tendon reconstruction group [5.0(3.5, 6.0)points and 5.0(3.3, 6.0)points] were significantly higher than the preoperative level [3.0(2.0, 4.0)points and 2.5(1.3, 4.0)points] (all P<0.01). There were no significant differences in Karlsson score and Tegner score between the two groups (all P>0.05). All patients completed partial weight-bearing after 3.0(2.0, 4.0)weeks in augmented repair group and 4.0(3.5, 6.0)weeks in tendon reconstruction group. All patients returned to normal walking after 8.0(6.0, 9.0)weeks in augmented repair group and 8.0(5.5, 12.0)weeks in tendon reconstruction group. A total of 13 patients (63%) in augmented repair group and 13 patients (72%) in tendon reconstruction group successfully returned to sports postoperatively and the time required was 6.0(3.5, 8.0) months and 6.0(4.5, 12.0)months, respectively. There were no significant differences in the above indicators between the two groups (all P>0.05), but augmented repair group had a trend of faster completion of partial weight-bearing than tendon reconstruction group. There was 1 patient [5%(1/19)] in augmented repair group and 1 patient [6%(1/18)] in tendon reconstruction group who reported feelings of instability during exercise postoperatively ( P>0.05). None of the patients in augmented repair group had limited ankle range-of-motion, not different from 1 patient [6%(1/18)] in tendon reconstruction group ( P>0.05). Conclusion:In the treatment of chronic ankle instability with poor remnant quality of the anterior talofibular ligament (ATFL) tissue, both arthroscopic all-inside ligament repair with suture augmentation and ligament reconstruction with tendons can improve the short-term postoperative ankle function and activity level of the patients, and the former one has advantages such as simple operative procedures and none use of grafts.

ABSTRACT

Bacillus spp. are probiotics and can secrete a variety of natural antimicrobiol active substances, of which lipopeptides are an important class. Up to now, about 90 lipopeptides have been identified, and most of them are cyclic lipopeptides. surfactin, iturin, fengycin, bacillomycin and polymyxins are widely studied, and the first three have huge potential for application due to their properties of surfactants and anti-fungal, anti-bacterial, anti-viral, anti-tumor and anti-inflammatory functions. In this paper, the research progress in the structure, function, synthesis regulation, separation, purification and production of surfactin, iturin and fengycin was reviewed. Synthetic biology is a vital means to increase the yield of lipopeptides, and in the future, lipopeptides can be used in crop cultivation, animal farming, food, medicine and petroleum industries as well as environmental protection. Future research should be strengthened on the discovery of new lipopeptides, synthesis of high-activity lipopeptides, economical production of lipopeptides on a large scale and their safety evaluation.

ABSTRACT

Objective:To investigate the serum hepcidin level and risk factors associated with peripheral arterial disease (PAD) and foot ulcer in type 2 diabetic patients.Methods:From January 2019 to June 2019, 70 patients with type 2 diabetes in Department of Endocrinology of Xiangya Third Hospital were selected, including 21 newly diagnosed patients with type 2 diabetes (DM group), 23 patients with lower extremity vascular disease (PAD group) and 26 patients with foot ulcer (DF group). Serum hepcidin was determined by enzyme linked immunosorbent assay (ELISA). The serum levels of hepcidin in different groups were compared, and the correlation between diabetic lower extremity vascular disease and foot ulcer was analyzed.Results:⑴ The hemoglobin, albumin, triglycerides and total cholesterol were significantly lower in DF group compared with PAD and DM groups ( P<0.05), while the DF group patients were with higher white blood cell (WBC) count and high sensitivity C reactive protein (hs-CRP) than patients in PAD and DM groups ( P<0.05). DF group also showed significantly higher WBC, hs-CRP and neutrophil ratio level (NEUT%) than DM group ( P<0.05). The inflammatory indicators of WBC, hs-CRP and NEUT% showed no significant difference between DM group and PAD group ( P>0.05). ⑵ The levels of hepcidin in DF and PAD groups were higher than those in DM group, while that in DF group were higher than those in PAD group ( P<0.05); Hepcidin was positively correlated with systolic blood pressure, WBC count, NEUT% and ferritin ( P<0.05), and negatively correlated with hemoglobin, glycosylated hemoglobin, albumin and 25-hydroxyvitamin D ( P<0.05). ⑶ Binary multivariate logistic regression analysis showed that elevated hepcidin level was an independent risk factor for diabetic foot ulcer [ OR=1.755, 95% CI: 1.063-2.897, P=0.028]. Conclusions:The fluctuation of serum hepcidin level in diabetic patients is related to the stimulation of inflammation, the degree of anemia and the nutritional status, which means it might be an early indicator of inflammation in diabetic patients with peripheral arterial disease. Moreover, the increase of hepcidin is an independent risk factor for diabetic foot ulcers in our study.

ABSTRACT

Objective:To develop a biomimetic nanoparticle probe of hematoporphyrin monomethyl ether (HMME) coated with breast cancer cell membrane, to observe its ability to target homologous breast cancer cells in vitro, and to investigate its effect of enhanced photoacoustic imaging and sonodynamic therapy (SDT) for breast cancer in vitro.Methods:The cell membrane of breast cancer 4T1 was extracted by chemical cleavage and repeated freezing and thawing. Then the HMME-coated polylactic acid-glycolic acid copolymer biomimetic nanoparticle was prepared by double emulsification and extrusion. The basic characteristics of nanoparticles were detected. The target ability of nanoparticles to homologous breast cancer cells and the enhancement of photoacoustic imaging were observed in vitro. Singlet oxygen sensor green (SOSG) was used to verify the reactive oxygen species (ROS) production of nanoparticles, and its SDT effect on breast cancer cells was evaluated by CCK8 cytotoxicity assay.Results:The size of the prepared CHP-NPs was uniform, the morphology was spherical "core-shell structure" , the particle size was (275.23±8.25)nm, and the surface potential was (-18.43±0.45)mV. It was observed that CHP-NPs could target homologous 4T1 cells under laser confocal microscopy. In vitro photoacoustic imaging experiments show that the photoacoustic signal of nanoparticles increases with the increase of its concentration. According to SOSG probe detection, CHP-NPs could produce ROS under ultrasonic irradiation.When CHP-NPs was incubated with 4T1 cells alone and no ultrasonic irradiation was used, the cell survival rate was not significantly affected. When the concentration was 0.6 mg/ml, the cell survival rate was still 95%. After ultrasonic irradiation, CCK8 experiment showed that the CHP-NPs had a significant SDT effect on breast cancer cells.Conclusions:The biomimetic nanomolecular probe of breast cancer cell membrane is successfully prepared. The probe has good ability to target homologous tumor, and can significantly enhance tumor photoacoustic imaging and SDT effect.

ABSTRACT

Objective To evaluate the effect of transitional care intervention on quality of life for patients with chronic obstructive pulmonary disease (COPD). Methods The Cochrane library, PubMed, Embase, Chinese biomedical literature database(CBM), Chinese scientific journals full-text database, Wanfang data were searched up to June 2015 to identify randomized controlled trials(RCTs) comparing continuity of patient care with conventional treatment.The data were analyzed using Revman 5.2 software. Results 26 RCTs involving 2 443 participants were included.The results of Meta-analysis showed continuity of care couldreduce the scores of The COPD Assessment Test [ SMD=-2.24, 95%CI=(-3.52--0.96), P<0.01],and St George′s Respiratory Questionnaire [SMD=-0.26, 95%CI(-0.50--0.02), P≤0.05], and could improve the scores of Chronic Respiratory Disease Questionnaires [SMD=0.46, 95%CI(0.08-0.84), P≤0.05]. Conclusions Transitional care can improve the quality of life of patients with COPD.

ABSTRACT

Objective To evaluate the efficacy of the carbon ion radiotherapy for prostate cancer by Meta-analysis.Methods We searched the Cochrane library,PubMed,EMBASE,China Journal Fulltext Database,Chinese Biomedical Database,and Wanfang Database from their inception to December 2015,in order to collect clinical trial data of carbon ion radiotherapy for prostate cancer.References included within these studies were also retrieved.Meta-analysis was performed using MetaAnalyst Beta 3.13 and STATA 12.0 software.Results Six studies (eight clinical trials) were included.The results of Meta-analysis show that,the overall survival rates of 3,4,5 and 8 years were 95.7%,90.9%,91.8% and 83.9%,respectively.The cause specific survival rate of 4 and 5 years were 97.1% and 97.6%.The bNED rate of 3,4,5 and 8 years were 88%,86.3% and 79.1%,respectively.The local control rates of 3,4 and 5 years were 98.1%,97.1% and 98.4%,respectively.The rate of total death,prostate cancer death and intercurrent death were 7%,2.4% and 7%,respectively.Different T-stage may affect the fiveyear of overall survival rate,bNED rate and cause specific survival rate.Conclusions The current evidence shows that carbon ion radiotherapy in gcncral is a fcasiblc trcatmcnt for prostate cancer,whether carbon ion is better than other radiotherapy,prospective,randomized,controlled clinical trial to get more evidence is required for carbon ion radiotherapy versus standard treatment for prostate cancer patients.

ABSTRACT

Objective To assess the efficacy and safety of the carbon ion radiotherapy for stage Ⅰ non-small cell lung cancer (NSCLC).Methods We searched multiple electronic bibliographic databases,including the Cochrane library,PubMed,EMBASE,China Journal Full-text Database,Chinese Biomedical Database,and the Wanfang Database to assemble the available studies of carbon ion radiotherapy for stage Ⅰ non-small cell lung cancer.MetaAnalyst Beta 3.13 and STATA 12.0 software were used to combine the extracted data.Results Ten studies (seven trials) were included.The overall survival rates of 1,3 and 5 years were 95.1%,68.5% and 45.9%,respectively.The local control rates of 1,3 and 5 years were 90.2%,76.7% and 81.2%,respectively.The cause specific survival rate of 1,3 and 5 years were 82.9%,58.5% and 57.3%,respectively.The rate of primary recurrence and total recurrence were 45.8% and 11%,respectively.The rates of total death and death from lung cancer were 52.4% and 28.3%,respectively.There were differences observed between T1 stage and T2 stage in regards to five year overall survival rate,total recurrence,lung cancer death,and grade 1 and grade 2 skin toxicities.Conclusions Available data demonstrate that carbon ion radiotherapy,in general,is a safe and feasible treatment modality for stage Ⅰ NSCLC.Although current results are promising,more evidence is required before carbon ion radiotherapy can become the international standard treatment for lung cancer patients.

Objective To investigate the health behavior and the exposure rate of risk factors of rural population with high risk of chronic obstructive pulmonary disease (COPD).Methods A total of 176 cold region rural COPD high-risk persons were recruited and investigated with Health-Promoting Lifestyle Profile Ⅱ (HPLP Ⅱ),Health Behaviour Self-efficacv Scale,general information questionnaire.Results The total score of HPLP was (92.70±11.22) points in the rural area of COPD,and the scores of the nutrition and interpersonal relationship were the highest,which were (20.06±2.75) points,(19.30±3.16) points,health responsibility and sports dimension were lower,which were respectively (13.74±2.60) points,(9.85±2.80) points.Health behavior self-efficacy scored (71.75±7.68) points,which showed significant correlation with HPLP Ⅱ score (P < 0.01).Conclusions COPD high-risk population of the cold area health promoting lifestyle and health behavior self-efficacy level are not high.Improving mral population at higher risk of COPD health behavior and self-efficacy level can form good healthy lifestyle and behavior.

ABSTRACT

Objective To understand the self-efficacy for health behaviors and its influencing factors among investigation of rural chronic obstructive pulmonary disease (COPD) high-risk groups.Methods November 2014 using the general questionnaire,Health Behavior Self-efficacy Scale for 176 over 40 yearsold of high-risk populations in rural COPD self-efficacy and health behavior factors research.Results In rural areas at high risk of health behavior self-efficacy scored (71.75±7.68) points,including nutrition and psychological comfortable highest scores which were (14.86±2.03) points and (19.75±3.03) points.Exercise and health responsibilities lower scored 18.90±2.87 and 18.42±2.52.Multiple regression analysis showed that marital status,education level and income were the main factors affecting rural health behavior at high risk of COPD self-efficacy (β=0.384,0.297,0.236,P <0.05).Conclusions COPD risk populations in rural areas at the middle level of health behavior self-efficacy,nurses should be given a lower level of education and income were more concerned to help them improve the health behavior self-efficacy.

BACKGROUND:Mesenchymal stem cells are found to have the immunoregulatory activities and a potential application prospect in the treatment of autoimmune diseases. OBJECTIVE:To explore the mechanism of transplanting mesenchymal stems cells on the treatment of multiple sclerosis. METHODS:The mouse mesenchymal stems cells were prepared, and injected into the al ogenic and syngenic normal mice, to detect the frequency of CD4+CD25+Foxp3+T cells in the spleen, thymus, and lymph nodes by flow cytometry, and to detect the Foxp3, transforming growth factor-β1, and interleukin-10 mRNA in the spleen, thymus, and lymph nodes by reverse transcription-PCR. RESULTS AND CONCLUSION:Transplantation of mesenchymal stem cells on normal mice led to a significant up-regulation of CD4+CD25+Foxp3+T cells, Foxp3, transforming growth factor-β1, and interleukin-10 mRNA in the spleen, thymus, and lymph nodes both in the al ogenic and syngenic transplant groups. Transplantation of mesenchymal stem cells may be an available method in the treatment of autoimmune diseases, and CD4+CD25+Foxp3+T cell, Foxp3, transforming growth factor-β1, and interleukin-10 may be involved in this process.

ABSTRACT

BACKGROUND: T cells are believed to play an important role in anti-infection, anti-tumor and immune function. However, the mechanism underlying the differentiation and development remains poorly understood. OBJECTIVE: To investigate the distribution of T cells in nude mice that are jointly transplanted human thymus and cord blood and the reconstruction of the immune function. METHODS: Thirty Balb/c nu/nu nude mice were randomly divided into two groups: an experimental group and a control group. In the experimental group, human thymus tissue was transplanted into the renal capsule of nude mice. Two weeks later, freshly isolated human cord blood CD34+ cells suspension was back perfused into the nude mice via the vein. In the control group, CD34+ cells transplantation was performed directly without thymus transplantation. After 60 days of breeding, the immune function of nude mice was detected in two groups. RESULTS AND CONCLUSION: Human thymus tissue in the renal capsule of nude mice survived and expressed CD3 and HLA-DR molecule. In the experimental group, CD3+ cells which distributed in the form of dots were observed in the mouse spleen. The proportion of CD3+, CD4+, CD8+, and CD4+CD25+ cells were significantly higher in the experimental group than in the control group. The nude mice from the experimental group rejected human gastric cancer BGC823 cells, while those from the control group did not. These findings demonstrated that combined human thymus and CD34+ cell transplantation allow nude mice to acquire T cell-mediated cellular immune function and possess the ability of anti-tumor.

ABSTRACT

Objective To investigate ATM deletion [del (ATM)] in chronic lymphocytic leukemia (CLL) and study its correlation with the clinical stage. Methods Spectrum Orange~(TM) labeled sequence specific DNA probe for ATM locus on 11q22.3 and fluorescence in Situ hybridization (FISH) was used to examine del (ATM) in 28 newly diagnose patients with CLL. FISH analysis were performed on bone marrow smears. Clinical staging was done according to Binet Method.Fisher exact propability was used to study the relations between del (ATM) and clinical feature. Results 4 out of the 28 cases were found with deletion of ATM. The incidence of del (ATM) in BinetA, BinetB and BinetC was 1/9 (11.1 %), 1/8 (12.5 %), 2/11 (18.2 %), respectively. Fisher exact propability showed that deletion of ATM was not associated with its clinical feature. Conclusion Application of FISH on archival bone marrow smears is a simple, liable method, and can be readly used to retrospective study of clonal blood system diseases. Deletion of ATM was common cytogenetic changes in CLL patients.And the significance of del (ATM) in the prognosis of CLL in China needs to be further investigated.

BACKGROUND: Further studies are needed to understand the cytobiological character, functional regulation, gene changes and expression difference of CD34~+ and CD133~+ stem cells induced by basic fibroblast growth factor (bFGF) using gene chip. OBJECTIVE: To compare the differences of gene expression and the response to bFGF of human umbilical cord CD34~+ and CD133~+ cells, and to explore gene expression changes of bFGF-induced umbilical cord CD34~+ and CD133~+ hematopotic stem cells/hemapoietic progenitor cells in vitro. METHODS: Human umbilical cord blood CD34~+ and CD133~+ cells were isolated and purified by MiniMACS immunomagnetic beads selection. The CD34~+ and CD133~+cells were cultured for 10 to 15 days in DMEM/F12 medium, supplemented with bFGF and B27. Total RNA from these cells was extracted and the genetic level of these cells was performed using Oligo GEArray(r) chip and GEArray software. Selected rate of CD34~+ and CD133~+ hematopoietic stem cells was detected using flow cytometry. CD34~+ and CD133~+ cell morphological changes were measured before and after bFGF induction. The concentration and purity of RNA were determined by agarose gel electrophoresis degeneration. Gene-chip test results were analyzed.

ABSTRACT

Objective To study the over-expression and clinical implications of the oncogene MDM2 in acute leukemia (AL). Methods The expression of MDM2 gene in 100 patients with newly diagnosed and relapse or refractory AL and 20 healthy as control was measured by relative quantitative reverse transcriptase polymerase chain reaction (RT-PCR),then the results was measured by χ2-test,t-test and one-way ANOVA to compare expession positive rate and intensity of MDM2. Results Among 100 patients,fifty-eight had the high expression of MDM2 gene (58 %). The expression level of MDM2 gene in patients was higher than that of health controls(P <0.05). The expression positive rate of MDM2 is higher in poor outcome group (67.9 %,19/28)than that in general outcome group (33.9%,19/56) (P<0.05). Conclusion Our results suggest that the expression of MDM2 gene plays an important role in the pathogenesis and poor outcome of AL.

Objective To study the expression of B7-H1 in gastric mucosa of patients with gastric carcinoma,and to identify its relationship with neoplasm metastasis and prognosis.Methods Expression of B7-H1 in gastric carcinoma cell line SGC-7901 and in freshly-resected gastric mucosa including gastric carcinoma,adjacent tumor tissue and distal normal gastric mucosa were examined by flow cytometric analysis,immunochemical staining,immunofluorescence staining and Western blot.The correlated data was analyzed statistically.And so the correlation among expression level of B7-H1 and the patients,clinicopathological parameters was established.Results B7-H1 expression was detected in SGC-7901 cell line.B7-H1 was found in cell membrane and little cytoplasm.The positive rate of B7-H1 expression in gastric carcinoma was(13/21)62%,and it was(7/21)33% in adjacent tumor tissue,Whereas B7-H1 was absent in distal normal gastric mucosa.Statistical analysis demonstrated a positive correlations of B7-H1 expression in gastric carcinoma with the depth of carcinoma infiltration,lymph node metastasis and pTNM stage(P
