ABSTRACT

Melioidosis is an endemic disease in Southeast Asia and Oceania caused by the gram-negative bacillus Burkholderia pseudomallei. We studied 15 adult patients from Colombia with microbiologically diagnosed pulmonary melioidosis. We reviewed 15 chest X-rays and 10 chest computed tomography (CT) studies. Of the 15 patients, 87% met the criteria for acute infection and 13% met the criteria for chronic infection. The most common findings on chest X-rays were consolidation (86%), nodules (26%), and cavitation (20%). On CT studies, consolidation and nodules were observed in 90% of cases; the areas of consolidation were predominantly located in the basal and central zones in 60%. Areas of cavitation were observed in 50%, pleural effusion in 60%, and mediastinal lymph nodes in 30%. In patients with acute pulmonary melioidosis (n=8), the findings observed were nodules (100%), mixed pattern with nodules and consolidation (87%), pleural effusion (88%), and mediastinal lymph nodes (25%). The two patients with chronic pulmonary melioidosis both had cavitation. Acute lung infection with B. Pseudomallei has radiologic manifestations similar to those of pneumonia due to other causes. In areas where the disease is endemic, it is essential to include acute melioidosis in the differential diagnosis of pulmonary nodules and chronic melioidosis in the differential diagnosis of cavitated chronic lung lesions.

La melioidosis es una enfermedad endémica en el suroeste asiático y Oceanía y está causada por la infección por el bacilo gramnegativo Burkholderia pseudomallei. Se estudian 15 pacientes adultos de Colombia con diagnóstico microbiológico de melioidosis pulmonar. Se revisaron 15 radiografías de tórax y 10 tomografías computarizadas (TC) de tórax. De los 15 pacientes, el 87% tenía criterios de infección aguda y el 13%, de infección crónica. Los hallazgos más frecuentes en la radiografía de tórax fueron: consolidación (86%), nódulos (26%) y cavitación (20%). En la tomografía computarizada se encontraron áreas de consolidación y nódulos en el 90% de los casos. En el 60% de los pacientes, las áreas de consolidación fueron de predominio basal y central. En el 50% de los casos se evidenciaron áreas de cavitación. Se evidenció derrame pleural en el 60% y adenopatías mediastínicas en el 30% de los casos. En los pacientes con presentación aguda (n=8), los hallazgos visualizados incluyeron: nódulos (100%), patrón mixto con nódulos y consolidación (87%), derrame pleural (88%) y adenopatías mediastínicas (25%). Los pacientes con melioidosis crónica (n=2) presentaron cavitación. La infección pulmonar aguda por B. Pseudomallei cursa con manifestaciones radiológicas similares a neumonías de otra etiología. En zonas endémicas debe considerarse la posibilidad de melioidosis aguda en el diagnóstico diferencial de nódulos pulmonares, y de melioidosis crónica en el diagnóstico diferencial de lesiones pulmonares crónicas cavitadas.(AU)

ABSTRACT

Heterobilharzia americana is a trematode parasite (family: Schistosomatidae) of mammalian wildlife that occasionally infects domestic dogs and horses in the southeastern United States. This report presents the first case of H. americana infection in a Grant's zebra (Equus burchelli boehmi). The parasite was diagnosed post mortem as an incidental finding by histopathologic identification of H. americana eggs in the liver parenchyma after the zebra succumbed in an accidental traumatic death. Molecular analysis of a portion of the parasite small subunit ribosomal RNA gene demonstrated 100% identity with DNA sequences from parasites previously characterized in domestic horses. Equine infections with H. americana are considered uncommon, but are probably underdiagnosed.

ABSTRACT

OBJECTIVE: This study sought to examine oral health beliefs and attitudes, and utilisation of oral health care services among individuals with diabetes and health professionals who serve them in Ghana. BASIC RESEARCH DESIGN: A qualitative study using grounded theory was conducted. CLINICAL SETTING: University of Ghana Dental School at Korle Bu, University of Ghana School of Public Health, National Diabetes Research and Management Centre at Korle Bu, and New York University College of Dentistry. PARTICIPANTS: A convenience sample of 59 patients comprised 7 focus groups conducted in either Twi or English. Seven key informant interviews with healthcare professionals and one spiritual leader were completed. RESULTS: Data from the focus groups and interviews reveal: 1, half of the participants with diabetes have oral manifestations (e.g., bleeding gums) and participants are generally unaware of interrelationship between diabetes and oral health; 2, dental treatment utilisation is minimal and associated almost exclusively with reparative and emergency care; and 3, medical health providers do not acknowledge the interrelationship between oral health and diabetes nor do they incorporate oral health issues into diabetes screening/treatment. CONCLUSION: Oral health knowledge and practices are limited among patients with diabetes in Accra, Ghana. Collaborative efforts for in-service education and training for oral health and medical professionals may be beneficial in serving the oral and general health care needs as well as improving the oral health-related quality of life of Ghanaians with diabetes.

ABSTRACT

Heterobilharzia americana is a trematode parasite (family Schistosomatidae) that infects a wide range of wild mammalian hosts. Canine cases have been reported in the Gulf coast and south Atlantic states, Kansas, and Oklahoma. A total of 238 canine H. americana cases in Texas were retrospectively collected for a period of approximately 22 years from case records at the Texas Veterinary Medical Diagnostic Laboratory and the Veterinary Medical Teaching Hospital pathology service, diagnostic parasitology service, and Gastrointestinal Laboratory at Texas A&M University College of Veterinary Medicine. Of these cases, 26 patients had 1-2 repeat positive tests for a total of 268 positive tests (26 biopsies, 39 necropsies, 160 fecal examinations, and 43 PCR). Multiple dogs were infected in 12 households. Cases were distributed primarily in the eastern region of Texas in 42 of 254 counties. Cases were seen as far west as Kerr county and in counties bordering Oklahoma, Louisiana, Mexico, and the Gulf of Mexico. The median dog age was 5.6 years (2.7 months to 17.2 years) and the median weight was 20.5 kg (1-61.6 kg). All American Kennel Club (AKC) breed groups were represented (n=186): crossbred (20%), herding (17.8%), sporting (16.1%), toy (10.8%), hounds (10.8%), working (10.1%), terrier (8.5%), non-sporting (4.9%), and miscellaneous (1%). No seasonal pattern of diagnosis was apparent. Clinical signs reported (n=90) were diarrhea (67%), weight loss (38%), anorexia/hyporexia (27%), vomiting (22%), hematochezia (20%), lethargy (17%), polyuria/polydipsia (6%), and collapse (3%). In 39 necropsy cases, trematode eggs were identified by histopathology in the small intestine (84%), liver (84%), large intestine (39%), pancreas (35%), lung (9%), lymph node (8%), spleen (4%), and stomach (3%). Adult parasites were identified histologically in four cases. Granulomatous inflammation associated with the eggs was the most commonly reported histopathologic change. Other changes reported were fibrosis, pigment in macrophages, and organ mineralization. Glomerulonephritis was identified in four cases. Of 20 necropsy cases where death was attributable to H. americana infection, only one case was diagnosed ante mortem. Eleven of these dogs were examined by a veterinarian but H. americana was included as a differential diagnosis in only two cases. Reported differential diagnoses included ethylene glycol toxicity, cholecalciferol toxicity, lymphoma, and pancreatitis. These data indicate that this parasite is more widely distributed and more common than is generally recognized. Increased awareness may aid in more diagnoses and timely therapy.
