ABSTRACT

The objective were to evaluate the effects of supplementation of standardized dry extract of Rosmarinus officinalis (RO) and the application of aesthetic radiofrequency on the oxidative stress markers catalase (CAT), superoxide dismutase (SOD), non-protein thiols (NP-SH), and thiobarbituric acid reactive species (TBARS) and the biochemical markers triglycerides, total cholesterol, high density lipoprotein (HDL) cholesterol, glutamic-oxaloacetic transaminase (TGO/AST), pyruvic-glutamic transaminase (TGP/ALT), gamma glutamyl transpeptidase (gamma-GT), and creatinine. This study included 32 women received the aesthetic therapy to reduce localized fat. They were divided into the control group (n = 8) receiving placebo capsules and the intervention group (n = 24) subdivided into Group A, B, and C, each with eight members receiving supplementation with 100, 500, and 1000 mg/day of standardized dry extract of RO, respectively. The Universal Trial Number (UTN) - U1111-1274-6255. Supplementation with RO (500 mg/day) demonstrated a reduction in oxidative stress (quantified with through a significant increase in NP-SH and a reduction in SOD and CAT enzymes). The radiofrequency aesthetic treatment did not promote an increase in oxidative stress; however, it caused significant changes in total cholesterol, HDL cholesterol, and creatinine. RO is a plant with antioxidant effects and its oral consumption is safe in selected women subjects in hepatic and renal markers.

ABSTRACT

Introduction: COVID-19 leads to severe clinical complications that culminate in respiratory failure and acute respiratory distress syndrome (ARDS). Despite reports of some comorbidities that contribute to the development of COVID-19-mediated ARDS, there is great variation in mortality rates among critical COVID-19 patients in the world. To date, no studies have described the incidence and comorbidities associated with ARDS due to COVID-19 in the northeastern region of Mexico. Aim of the study: To describe patients diagnosed with ARDS due to COVID-19 in the northeastern region of Mexico, as well as its variations in comparison with other regions of the country. Material and Methods: We conducted a prospective and observational study in a tertiary care center in Northeastern Mexico. All patients diagnosed with SARS-CoV-2 infection and requiring management in the intensive care unit were included. We developed a database that was updated daily with the patients' demographic information, pre-existing diseases, disease severity, and clinical variables. Results: The incidence of ARDS secondary to COVID-19 in HRAEV was high in comparison with other reports. Diabetes mellitus was the risk factor most associated with COVID-19-mediated ARDS. Conclusion: Based on our findings and those previously reported in the literature, there are differences that we discuss between northeastern and central Mexico, and analyze other areas evaluated around the world.

ABSTRACT

Biosurfactants, sustainable alternatives to petrochemical surfactants, are gaining attention for their potential in medical applications. This study focuses on producing, purifying, and characterizing a glycolipid biosurfactant from Candida sp. UFSJ7A, particularly for its application in biofilm prevention on siliconized latex catheter surfaces. The glycolipid was extracted and characterized, revealing a critical micellar concentration (CMC) of 0.98 mg/mL, indicating its efficiency at low concentrations. Its composition, confirmed through Fourier transform infrared spectroscopy (FT-IR) and thin layer chromatography (TLC), identified it as an anionic biosurfactant with a significant ionic charge of -14.8 mV. This anionic nature contributes to its biofilm prevention capabilities. The glycolipid showed a high emulsification index (E24) for toluene, gasoline, and soy oil and maintained stability under various pH and temperature conditions. Notably, its anti-adhesion activity against biofilms formed by Escherichia coli, Enterococcus faecalis, and Candida albicans was substantial. When siliconized latex catheter surfaces were preconditioned with 2 mg/mL of the glycolipid, biofilm formation was reduced by up to 97% for E. coli and C. albicans and 57% for E. faecalis. These results are particularly significant when compared to the efficacy of conventional surfactants like SDS, especially for E. coli and C. albicans. This study highlights glycolipids' potential as a biotechnological tool in reducing biofilm-associated infections on medical devices, demonstrating their promising applicability in healthcare settings.

ABSTRACT

Titanium implants are subject to bacterial adhesion and peri-implantitis induction, and biosurfactants bring a new alternative to the fight against infections. This work aimed to produce and characterize the biosurfactant from Bacillus subtilis ATCC 19,659, its anti-adhesion and antimicrobial activity, and cell viability. Anti-adhesion studies were carried out against Streptococcus sanguinis, Staphylococcus aureus, Fusobacterium nucleatum, Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Proteus mirabilis as the minimum inhibitory concentration and the minimum bactericidal concentration. Cell viability was measured against osteoblast and fibroblast cells. The biosurfactant was classified as lipopeptide, with critical micelle concentration at 40 µg mL- 1, and made the titanium surface less hydrophobic. The anti-adhesion effect was observed for Staphylococcus aureus and Streptococcus sanguinis with 54% growth inhibition and presented a minimum inhibitory concentration of 15.7 µg mL- 1 for Streptococcus sanguinis and Aggregatibacter actinomycetemcomitans. The lipopeptide had no cytotoxic effect and demonstrated high potential application against bacterial biofilms.

ABSTRACT

Antimicrobial peptides have been developed based on plant-derived molecular scaffolds for the treatment of infectious diseases. Chenopodin is an abundant seed storage protein in quinoa, an Andean plant with high nutritional and therapeutic properties. Here, we used computer- and physicochemical-based strategies and designed four peptides derived from the primary structure of Chenopodin. Two peptides reproduce natural fragments of 14 amino acids from Chenopodin, named Chen1 and Chen2, and two engineered peptides of the same length were designed based on the Chen1 sequence. The two amino acids of Chen1 containing amide side chains were replaced by arginine (ChenR) or tryptophan (ChenW) to generate engineered cationic and hydrophobic peptides. The evaluation of these 14-mer peptides on Staphylococcus aureus and Escherichia coli showed that Chen1 does not have antibacterial activity up to 512 µM against these strains, while other peptides exhibited antibacterial effects at lower concentrations. The chemical substitutions of glutamine and asparagine by amino acids with cationic or aromatic side chains significantly favoured their antibacterial effects. These peptides did not show significant hemolytic activity. The fluorescence microscopy analysis highlighted the membranolytic nature of Chenopodin-derived peptides. Using molecular dynamic simulations, we found that a pore is formed when multiple peptides are assembled in the membrane. Whereas, some of them form secondary structures when interacting with the membrane, allowing water translocations during the simulations. Finally, Chen2 and ChenR significantly reduced SARS-CoV-2 infection. These findings demonstrate that Chenopodin is a highly useful template for the design, engineering, and manufacturing of non-toxic, antibacterial, and antiviral peptides.

RESUMO: Objetivo: analisar a dor musculoesquelética em profissionais de saúde na atenção primária e sua vivência durante a pandemia da COVID-19. Método: estudo de métodos mistos, realizado no sul do Brasil, entre junho de 2021 e fevereiro de 2022. Amostra de 50 participantes, utilizando os questionários relacionados à dor e aos sintomas osteomusculares e entrevistas com roteiros pré-elaborados. Análise de dados com estatísticas descritivas, inferenciais e Análise Temática de Conteúdo. Resultados: a maior parte dos participantes foram da área da Enfermagem, com 40 horas semanais. Regiões mais acometidas: ombros, parte superior das costas e pescoço, variando de dor moderada (42%) e intensa (10%). Diferenças estatísticas significativas relacionadas à dor e medo de contrair a COVID-19, entre aqueles com problemas prévios de saúde e enfermeiros. Conclusão: tendo em vista que a dor pode estar associada às condições de trabalho, torna-se essencial a identificação dos fatores de risco, evitando maiores agravos para a saúde do trabalhador.


ABSTRACT Objective: To analyze musculoskeletal pain in primary care health professionals and their experience during the COVID-19 pandemic. Method: A mixed-methods study carried out in southern Brazil between June 2021 and February 2022. A sample of 50 participants, using questionnaires related to pain and musculoskeletal symptoms and interviews with pre-prepared scripts. Data analysis using descriptive and inferential statistics and Thematic Content Analysis. Results: Most of the nursing sector participants worked 40 hours a week. The most affected areas are the shoulders, upper back, and neck, ranging from moderate (42%) to severe (10%) pain. Statistically significant differences related to pain and fear of contracting COVID-19 between those with previous health problems and nurses. Conclusion: Given that pain can be associated with working conditions, it is essential to identify risk factors to avoid further damage to workers' health.

Una muestra de 50 participantes, mediante cuestionarios relacionados con el dolor y los síntomas musculoesqueléticos y entrevistas con guiones preparados de antemano. Análisis de datos mediante estadísticas descriptivas e inferenciales y análisis temático de contenido. Resultados: La mayoría de las participantes pertenecían al sector de la enfermería y trabajaban 40 horas semanales. Regiones más afectadas: hombros, parte superior de la espalda y cuello, con dolores que van de moderados (42%) a intensos (10%). Diferencias estadísticamente significativas relacionadas con el dolor y el miedo a contraer COVID-19 entre las personas con problemas de salud previos y las enfermeras. Conclusión: Dado que el dolor puede estar asociado a las condiciones de trabajo, es esencial identificar los factores de riesgo y evitar mayores daños a la salud de los trabajadores.

Resumo O objetivo deste artigo é verificar como o ensino de ética/bioética está inserido nas grades curriculares dos programas de pós-graduação stricto sensu na área medicina III, conforme classificação da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, nas regiões Norte e Nordeste. Trata-se de estudo documental, descritivo e exploratório, de natureza qualiquantitativa. Utilizou-se a Plataforma Sucupira para buscar as seguintes informações sobre os programas: estado, instituição, nome do programa, categoria (acadêmico/profissional), nota, obrigatoriedade e nome da disciplina, carga horária e site do programa. Foram encontrados nove programas na área medicina III, sobretudo mestrados profissionais, e a maioria deles oferece disciplinas ligadas a ética como obrigatórias. Verificou-se que disciplinas ligadas a ética/ bioética estão presentes em todos os programas, embora com diferentes cargas horárias. São necessários estudos futuros que investiguem as abordagens metodológicas para ensino de ética nos cursos da área.


Abstract The objective of this article is to examine how ethics/bioethics teaching is included in the curricula of stricto sensu graduate programs in the field of medicine III, according to the classification of the Coordination for the Improvement of Higher Education Personnel, in the North and Northeast regions of Brazil. This is a documentary, descriptive and exploratory study, of a qualitative and quantitative nature. The Sucupira Platform was used to search for the following information about the programs: state, institution, name of the program, category (academic/professional), score, compulsoriness and name of the course, workload and website of the program. We found nine programs in the field of medicine III, mainly professional master's degree programs, most of which offer ethics-related courses as compulsory courses. We found that ethics/ bioethics-related courses are present in all programs, although with different workloads. Further studies are necessary in order to research the methodological approaches to ethics teaching in programs in the field.


Resumen El objetivo de este artículo es comprobar la manera en que se incluye la enseñanza de la ética/bioética en los planes de estudio de los programas de posgrado stricto sensu en el campo de la medicina III, según la clasificación de la Coordinación de Perfeccionamiento de Personal de Nivel Superior, en las regiones Norte y Nordeste. Se trata de un estudio documental, descriptivo y exploratorio, de carácter cualicuantitativo. Se utilizó la Plataforma Sucupira para buscar la siguiente información sobre los programas: estado, institución, nombre del programa, categoría (académico/profesional), nota, obligatoriedad y nombre de la asignatura, horas lectivas y página web del programa. Se encontraron nueve programas en el campo de medicina III, sobre todo de másteres profesionales, y la mayoría ofrece asignaturas relacionadas con la ética con carácter obligatorio. Se constató que las asignaturas relacionadas con la ética/bioética están presentes en todos los programas, aunque con diferentes horas lectivas. Se requieren estudios futuros para investigar los enfoques metodológicos para la enseñanza de la ética en cursos de este ámbito.

Resumen Objetivos: Describir los patrones tomográficos de afectación pulmonar en pacientes con SARS-CoV2 y SDRA. Materiales y Métodos: Estudio retrospectivo, transversal y analítico. Pacientes atendidos en Hospital Regional de Alta Especialidad de Ciudad Victoria "Bicentenario 2010" (HRAEV), con diagnóstico confirmado de COVID-19 por RT-PCR y que contaran con tomografía computarizada de tórax realizada en la institución en el periodo comprendido de 1 marzo de 2020 a 31 julio de 2020. Se realizó un análisis descriptivo y analítico. Resultados: Se analizaron estudios de tomografía de 44 pacientes hospitalizados en HRAEV que cursaron con SDRA y requirieron IOT, de los cuales el 72.7% fueron del sexo masculino, la edad promedio fue de 64 años, de los cuales 47.7% cursaba con Diabetes Mellitus, 63.6 % con Hipertensión Arterial Sistémica 43.1% y con algún grado de sobrepeso u obesidad. Conclusiones: El hallazgo tomográfico característico de los pacientes con SDRA fue mixto de tipo vidrio despulido y consolidación con una afectación severa y generalizada.


Abstract Objective: Describe the tomographic patterns of lung involvement in patients with SARS-CoV2 and ARDS. Materials and methods: Retrospective, cross-sectional and analytical study. Patients treated at the Regional High Specialty Hospital of Ciudad Victoria "Bicentenario 2010" (HRAEV), with a confirmed diagnosis of COVID-19 by RT-PCR and who had a chest computed tomography performed at the institution in the period from March 1, 2020 to July 31, 2020. A descriptive and analytical analysis was carried out. Results: Tomography studies of 44 patients hospitalized in HRAEV who had ARDS and required IOT were analyzed, of which 72.7% were male, the average age was 64 years, of which 47.7% had Diabetes Mellitus, 63.6% with Systemic Arterial Hypertension and 43.1% with some degree of overweight or obesity. Conclusions: The characteristic tomographic finding of patients with ARDS was a mixed type of ground glass and consolidation with severe and generalized involvement. A high mortality rate was observed in these patients with OTI up to 70%.

ABSTRACT

The aim of this study was to evaluate the biochemical, hormonal, and hematological blood responses of native sheep subjected to five temperatures (20°, 24°, 28°, 32°, and 36 °C), using 24 sheep of the following breeds: Santa Inês (SI), Morada Nova (MN), Soinga (SO) and no defined racial pattern (NDRP), kept in collective pens inside the climatic chamber. The experimental design was completely randomized, in a 5 × 4 factorial scheme (5 temperatures and 4 breeds) with 6 animals of each breed at each temperature. There was interaction for urea (P < .0001), glucose (P < .0001), cholesterol (P < .0001), and magnesium (P < .0001) as a function of breed and air temperature. Urea (P = .001), glucose (P < .0001), triglycerides (P = .033), cholesterol (P = .004), and magnesium (P < .0001) showed differences according to breed. It was observed that T4 (P < .0001) and cortisol (P < .0001) showed an interaction between breed and temperature, while T4 (P = .001) and cortisol (P = .001) an effect of breed. The highest magnesium concentration was observed in MN, and the lowest was in NDRP, SO, and SI, which are statistically similar, and SI and NDRP are statistically similar. The hormones showed a significant effect (P < .0001) for temperature, where cortisol increased, and thyroid hormones decreased with increasing temperature. Temperatures of 32° and 36 °C contribute to changes in blood, hormone, and hematological biochemical responses of native sheep.

For more than three decades, major efforts in sampling and analyzing tree diversity in South America have focused almost exclusively on trees with stems of at least 10 and 2.5 cm diameter, showing highest species diversity in the wetter western and northern Amazon forests. By contrast, little attention has been paid to patterns and drivers of diversity in the largest canopy and emergent trees, which is surprising given these have dominant ecological functions. Here, we use a machine learning approach to quantify the importance of environmental factors and apply it to generate spatial predictions of the species diversity of all trees (dbh ≥ 10 cm) and for very large trees (dbh ≥ 70 cm) using data from 243 forest plots (108,450 trees and 2832 species) distributed across different forest types and biogeographic regions of the Brazilian Amazon. The diversity of large trees and of all trees was significantly associated with three environmental factors, but in contrasting ways across regions and forest types. Environmental variables associated with disturbances, for example, the lightning flash rate and wind speed, as well as the fraction of photosynthetically active radiation, tend to govern the diversity of large trees. Upland rainforests in the Guiana Shield and Roraima regions had a high diversity of large trees. By contrast, variables associated with resources tend to govern tree diversity in general. Places such as the province of Imeri and the northern portion of the province of Madeira stand out for their high diversity of species in general. Climatic and topographic stability and functional adaptation mechanisms promote ideal conditions for species diversity. Finally, we mapped general patterns of tree species diversity in the Brazilian Amazon, which differ substantially depending on size class.

ABSTRACT

BACKGROUND: Anal canal squamous cell carcinoma (SCC) is a relatively uncommon neoplasia, and it is mostly a local-regional cancer, of low metastatic potential (only 15%), resulting in cure in most cases treated with definitive chemoradiation. On the other hand, its incidence has been steadily increasing over the last decades, which makes it an important public health problem. In an effort to provide surgeons and oncologists who treat patients with anal cancer with the most updated information based on the best scientific evidence, the Brazilian Society of Surgical Oncology (SBCO) has produced the present guideline for the management of anal canal SCC, focused on the main topics related to daily clinical practice. OBJECTIVES: The SBCO developed the present guidelines to provide recommendations on the main topics related to the management of anal canal squamous cell carcinoma (SCC) based on current scientific evidence. METHODS: Between October 2022 and January 2023, 14 experts met to develop the guidelines for the management of anal canal cancer. A total of 30 relevant topics were distributed among the participants. The methodological quality of a final list with 121 sources was evaluated, all the evidence was examined and revised, and the management guidelines were formulated by the 14-expert committee. To reach a final consensus, all the topics were reviewed in a meeting that was attended by all the experts. RESULTS: The proposed guidelines contained 30 topics considered to be highly relevant in the management of anal canal cancer, covering subjects related to screening recommendations, preventive measures, tests required for diagnosing and staging, treatment strategies, response assessment after chemoradiotherapy, surgical technique-related aspects, and follow-up recommendations. In addition, screening and response assessment algorithms, and a checklist were proposed to summarize the important information and offer an updated tool to assist surgeons and oncologists who treat anal canal cancer and in providing the best care to their patients. CONCLUSION: These guidelines summarize recommendations based on the most current scientific evidence on relevant aspects of anal canal cancer management and are a practical guide to help surgeons and oncologists who treat anal canal cancer make the best therapeutic decisions.

ABSTRACT

BACKGROUND: Biosurfactants are natural bioactive compounds produced from fungi, bacteria and plants. These molecules have several properties that enable them to be involved in various industrial applications. The surface-active properties of biosurfactants allow their use in various sectors, such as agriculture, health, cosmetics, bioremediation/ petroleum, mining, and bioprocess. OBJECTIVE: The objective of this study was to analyze the patents deposited in intellectual property databases for applications of biosurfactants in the areas of agriculture, health, cosmetics, bioremediation/petroleum, mining, and bioprocesses, with the research period consisting of the last six years. METHODS: This study targeted the use of biosurfactants in various industrial sectors. The patent search was carried out using the Google Patents platform, a platform that is commonly used for this purpose and freely accessible. RESULTS: During the search for patents related to applications of biosurfactants in industry filed in the last 6 years, results were found to be distributed in the following areas: agriculture (70), cosmetics (2311), bioremediation/petroleum (179), health (1794), mining (5), and bioprocess (133). Six patents were selected from each area, except for health and mining, for which the search resulted in only 5 and 4 patents, respectively, to be discussed and provide information on the biotechnological applications of biosurfactants in the industry. CONCLUSION: The present study showed a wide area of application of biosurfactants in industry. The interest in the inclusion of biosurfactants in the industry is directly related to the need for more sustainable solutions to solve real market problems. The cosmetics sector presented the most patents that employ the use of biosurfactants.

This research determined the adaptability and stability of soybean yield for organic systems in different environments. Grain yield data (GY, kg ha-1) from experiments with six soybean genotypes evaluated in six environments in Brazil and one environment in Paraguay were used. The experimental design used was randomized blocks, organized in an incomplete factorial scheme, with six environments in Brazil (Toledo- PR, Palotina- PR, Mangueirinha- PR, Três Passos- RS, Passo Fundo- RS, Major Vieira- SC) and one in Paraguay (Bela Vista do Norte- PY) with six soybean genotypes (BRS 284, BRS 391, BRS 511, BRS 523, BRS 525, BRS 535) arranged in four replications per environment. BRS 511 genotype was characterized by high average grain yield and stability by the AMMI method. BRS 284 genotype was identified as the ideal genotype in the GGE biplot method. WAASBY and BLUP index selected BRS511 and BRS284 genotypes.


O objetivo deste trabalho foi determinar a adaptabilidade e estabilidade da produtividade da soja para sistemas orgânicos em diferentes ambientes. Foram utilizados dados de produtividade de grãos (GY, kg ha-1) de experimentos com seis genótipos de soja avaliados em seis ambientes no Brasil e um ambiente no Paraguai. O delineamento experimental utilizado foi blocos casualizados, organizados em esquema fatorial incompleto, com seis ambientes no Brasil (Toledo - PR, Palotina - PR, Mangueirinha - ​​PR, Três Passos - RS, Passo Fundo - RS, Major Vieira - SC) e um no Paraguai (Bela Vista do Norte - PY) com seis genótipos de soja (BRS 284, BRS 391, BRS 511, BRS 523, BRS 525, BRS 535) dispostos em quatro repetições por ambiente. O genótipo BRS 511 foi caracterizado por alta produtividade média de grãos e estabilidade pelo método AMMI. O genótipo BRS 284 foi identificado como o genótipo ideal pelo método GGE biplot. Os índices WAASBY e BLUP selecionaram os genótipos BRS511 e BRS284.

Resumo Empatia pressupõe a capacidade e sobretudo a vontade de compreender o outro e se colocar em seu lugar. Considerando isso, espera-se que profissionais envolvidos em cuidados de saúde sejam mais empáticos e capazes de ler o mundo ao seu redor com olhar humanizado, crítico e reflexivo. Objetivou-se investigar, mediante revisão integrativa, o que tem sido discutido a respeito do ensino da empatia nos cursos de graduação da área da saúde nos últimos cinco anos. Foram selecionados 27 artigos das bases de dados MEDLINE e LILACS, por meio dos quais se identificou que a maioria dos estudos disponíveis foram realizados nas áreas de medicina e enfermagem. Além disso, constatou-se que, apesar de o tema empatia remeter à subjetividade, a metodologia quantitativa com aplicação de escalas padronizadas foi a mais utilizada para mensurar níveis de empatia e que, por fim, o ensino da empatia ocorreu por meio de metodologias ativas.


Abstract Empathy presupposes the ability and, most importantly, the willingness to understand others and put oneself in their place. Hence, health personnel are expected to be more empathetic and able to read the world around them with a humanized, critical, and reflexive look. This integrative review examines the studies on teaching empathy in undergraduate health programs published in the last five years. A total of 27 studies were selected from the MEDLINE and LILACS databases, most of which were published in the fields of medicine and nursing. Although a subjective element, most studies measured empathy using standardized scales (quantitative approach). Finally, empathy was taught using active methodologies.


Resumen La empatía es una capacidad, sobre todo una voluntad, de comprender al otro y de ponerse en su piel. Así se espera que los profesionales involucrados en el cuidado de la salud sean más empáticos y tengan una mirada humanizada, crítica y reflexiva sobre el mundo. Esta es una revisión integradora para identificar cómo se enseña la empatía en las carreras de graduación en el área de la salud en los últimos cinco años. Se seleccionaron 27 artículos de las bases de datos MEDLINE y LILACS, y se identificó que la mayoría de los estudios disponibles fueron de las áreas de Medicina y Enfermería. Se constató también que, aunque el tema de la empatía se refiera a la subjetividad, el método cuantitativo con la aplicación de escalas estandarizadas fue el más utilizado para medir los niveles de empatía y que la enseñanza de la empatía ocurrió por metodologías activas.

Este texto tem por propósito, em primeiro lugar, construir uma reflexão acerca da ideia de que gênero, além de ser um problema de todos, inclusive da psicanálise e dos/das/des psicanalistas, é um conceito imprescindível para a escuta clínica no contemporâneo. Em segundo lugar, tem por objetivo apontar tarefas teóricas e clínico-políticas para que possamos, coletivamente, desobstruir a nossa escuta e ampliar o nosso horizonte ético e político, mas, mais do que isso, para que possamos perder o medo de abrir mão de velhos conceitos e criarmos coragem de escutar as demandas do nosso tempo, inventando novos rumos e, consequentemente, novos conceitos para as psicanálises.


The purpose of this text is, firstly, to build a reflection on the idea that gender, in addition to being a problem for everyone, including psychoanalysis and psychoanalysts, is an essential concept for clinical listening in the contemporary world. Secondly, it aims to point out theoretical and clinical-political tasks so that we can collectively clear our listening and broaden our ethical and political horizons, but more than that, so that we can lose the fear of giving up old concepts and create the courage to listen to the demands of our time, inventing new directions and, consequently, new concepts for psychoanalysis.


L'objet de ce texte est, dans un premier temps, de construire une réflexion sur l'idée que le genre, en plus d'être un problème pour tous, y compris la psychanalyse et les psychanalystes, est un concept essentiel pour l'écoute clinique dans le monde contemporain. Deuxièmement, il vise à souligner les tâches théoriques et clinico-politiques afin que nous puissions collectivement éclaircir notre écoute et élargir nos horizons éthiques et politiques, mais plus que cela, afin que nous puissions perdre la peur d'abandonner les vieux concepts et créer le courage écouter les exigences de notre temps, inventer de nouvelles directions et, par conséquent, de nouveaux concepts pour la psychanalyse.

Resumo Neste artigo busca-se caracterizar o respeito ao princípio da autonomia no atendimento a populações indígenas em território brasileiro, tendo por base relatos de experiência e uma revisão bibliográfica. Concluiu-se que esse princípio deve ser especialmente consolidado nas ações práticas de cuidado à saúde desenvolvidas com as populações de culturas diferenciadas, como é o caso das comunidades indígenas.


Abstract This article seeks to characterize the respect to the principle of autonomy in the care to indigenous peoples in the Brazilian territory, based on reports on experiences and a bibliographic review. It concluded that this principle must be specially consolidated in the practical actions of health care developed alongside peoples of different cultures, as is the case of indigenous communities.


Resumen Este artículo busca caracterizar el respeto en el principio de autonomía en la atención a las poblaciones indígenas en territorio brasileño a partir de informes de experiencia y revisión de la literatura. Se concluyó que este principio debe consolidarse especialmente en las acciones prácticas de asistencia sanitaria dirigidas a las poblaciones de diferentes culturas, como las comunidades indígenas.

ABSTRACT

Objective: Advance directives are becoming increasingly important as health technologies evolve. We sought to assess psychiatrists' knowledge of advance directives, as this knowledge is fundamental to the implementation and drafting of these personal documents. Methods: A previously published questionnaire that evaluated the knowledge of medical professors was used. The sample, composed of psychiatrists from Rio de Janeiro, Brazil, originated from a publicly available list. During the search process, the COVID-19 pandemic affected Brazil and the rest of the world, which influenced our methodology and results. Results: A total of 40 psychiatrists participated in the study. The results obtained, although not significant, suggested that psychiatrists with an increased time of practice had more knowledge of advance directives. Nevertheless, less than half of the participants had knowledge about this topic. Conclusion: The number of psychiatrists surveyed indicates the need for further studies on the subject. The influence of the COVID-19 pandemic on this study led to findings such as a change in sensitivity when addressing the topic and greater difficulty in contacting professionals.

Resumo Comunicar más notícias é tarefa desafiadora na prática médica, motivo pelo qual o tema tem adquirido relevância na formação de futuros profissionais. Nas unidades de terapia intensiva, esse tipo de comunicação se mescla a múltiplas situações, a exemplo do manejo de informações envolvendo o agravamento do estado clínico e a possibilidade de morte iminente, especialmente em casos pediátricos. Diante desse contexto, o objetivo desta pesquisa foi compreender os desafios enfrentados por médicos intensivistas na comunicação de más notícias, tendo como eixo de análise algumas questões problematizadas no campo das discussões bioéticas. Os resultados apontaram a necessidade de preparo técnico e pessoal do médico diante desse importante desafio, haja vista que a participação da criança no processo de tratamento ainda é limitada, realidade que dificulta o compartilhamento das informações e, consequentemente, o exercício da autonomia.


Abstract Breaking bad news is a challenging task in medical practice, reason why this topic's relevance has grown in the training of future professionals. In intensive care units, this type of communication is mixed with multiple situations, such as the management of information involving the worsening of clinical conditions and the possibility of imminent death, especially in pediatric cases. Given this context, the objective if this study was to understand the challenges faced by intensive care physicians in breaking bad news, from the analytical axis of some questions raised in the field of bioethical discussions. The results highlight the need for technical and personal preparation of physicians in the face of this important challenge, since the child's participation in the treatment process remains limited, a reality that hinders the sharing of information and, consequently, the exercise of autonomy.


Resumen Comunicar malas noticias es una tarea desafiante en la práctica médica, por lo que el tema ha adquirido relevancia en la formación de los futuros profesionales. En las unidades de cuidados intensivos, este tipo de comunicación se mezcla con múltiples situaciones, como, por ejemplo, el manejo de información que implica el agravamiento del estado clínico y la posibilidad de muerte inminente, especialmente en los casos pediátricos. Ante este contexto, el objetivo de esta investigación fue comprender los desafíos que enfrentan los médicos intensivistas en la comunicación de malas noticias, teniendo como eje de análisis algunas cuestiones problematizadas en el ámbito de las discusiones bioéticas. Los resultados señalaron la necesidad de una preparación técnica y personal del médico que se enfrenta a este importante desafío, una vez que la participación del niño en el proceso de tratamiento sigue siendo limitada, una realidad que dificulta el intercambio de información y, en consecuencia, el ejercicio de la autonomía.
