ABSTRACT

BACKGROUND: Speech recognition in noisy environments is a challenge for both cochlear implant (CI) users and device manufacturers. CI manufacturers have been investing in technological innovations for processors and researching strategies to improve signal processing and signal design for better aesthetic acceptance and everyday use. PURPOSE: This study aimed to compare speech recognition in CI users using off-the-ear (OTE) and behind-the-ear (BTE) processors. DESIGN: A cross-sectional study was conducted with 51 CI recipients, all users of the BTE Nucleus 5 (CP810) sound processor. Speech perception performances were compared in quiet and noisy conditions using the BTE sound processor Nucleus 5 (N5) and OTE sound processor Kanso. Each participant was tested with the Brazilian-Portuguese version of the hearing in noise test using each sound processor in a randomized order. Three test conditions were analyzed with both sound processors: (i) speech level fixed at 65 decibel sound pressure level in a quiet, (ii) speech and noise at fixed levels, and (iii) adaptive speech levels with a fixed noise level. To determine the relative performance of OTE with respect to BTE, paired comparison analyses were performed. RESULTS: The paired t-tests showed no significant difference between the N5 and Kanso in quiet conditions. In all noise conditions, the performance of the OTE (Kanso) sound processor was superior to that of the BTE (N5), regardless of the order in which they were used. With the speech and noise at fixed levels, a significant mean 8.1 percentage point difference was seen between Kanso (78.10%) and N5 (70.7%) in the sentence scores. CONCLUSION: CI users had a lower signal-to-noise ratio and a higher percentage of sentence recognition with the OTE processor than with the BTE processor.

Introdução: A mensuração do benefício proporcionado pelo Implante Coclear é fundamental e há vários métodos de avaliação descritos, mas, os testes de percepção de fala ainda são os mais utilizados, dentre eles a "Lista de Sentenças em Português" - LSP. Determinar os fatores que influenciam o reconhecimento de fala nos usuários de IC auxilia nas orientações da etapa pré-operatória, melhora a adaptação, reabilitação e evidencia mudanças necessárias no dispositivo. Objetivo: Verificar a correlação das variáveis: tempo de privação auditiva e tempo de uso de Implante Coclear (IC) com os resultados dos testes de percepção de fala no silêncio e no ruído em usuários de IC com deficiência auditiva pós-lingual e idade entre 14 e 60 anos. Método: Os 27 participantes foram submetidos às seguintes avaliações: levantamento de dados- para coleta de informações que caracterizem a perda auditiva prévia, como tempo de privação auditiva e início do uso do IC -, audiometria em campo livre com IC nas frequências sonoras de 250 a 4000 Hz e o reconhecimento de fala que foi avaliado por meio do teste "Listas de Sentenças em Português" - LSP aplicado na condição favorável (silêncio) e na condição desfavorável (ruído) de escuta. Na análise estatística foram utilizados o coeficiente de correlação linear de Pearson e gráficos de dispersão bidimensional, além disso, a análise descritiva dos dados. Resultados: Não houve relação estatisticamente significante entre a privação auditiva e a percepção de fala no silêncio e no ruído. Por outro lado, houve correlação positiva estatisticamente significante entre o Tempo de Uso do IC com o desempenho no teste de reconhecimento de sentenças no silêncio. Conclusão: Verificou-se correlação significante apenas entre tempo de uso de implante e reconhecimento de sentenças no silêncio em usuários de implante com surdez pós-lingual.


Introduction: It is essential to measure the benefits provided by the cochlear implant, to which end various assessment methods have been described. Of these, the speech perception tests are still the most used, including the List of Sentences in Portuguese ­ LSP. Determining the factors that influence the speech recognition of CI users helps in preoperative instructions, improves adaptation and rehabilitation, and reveals necessary changes to be made in the device. Purpose: To verify the correlation between time of auditory deprivation and duration of use of the cochlear implant (CI) with the results of the speech perception in silence and noise tests in CI users with post lingual hearing loss aged 14 to 60 years old. Method: The 27 participants were submitted to the following assessments: data collection to characterize the previous hearing loss, such as the time of auditory deprivation and beginning of CI use; free-field audiometry with CI at the frequencies of 250 to 4000 Hz; speech recognition assessed through the Lists of Phrases in Portuguese (LPP), presented in favorable (silence) and unfavorable (noise) hearing conditions. In the statistical analysis, Pearson's linear correlation coefficient was used, as well as bidimensional dispersion graphs and descriptive data analysis. Results: There was no statistically significant relationship between auditory deprivation and speech perception in silence and noise. On the other hand, there was a statistically significant positive correlation between the duration of CI use and the performance in sentence recognition in silence. Conclusion: A significant correlation was verified only between duration of implant use and sentence recognition in silence in implant users with post lingual deafness.

Método: los 27 participantes se sometieron a los siguientes pasos: recopilación de datos - para la recopilación de datos que caracteriza la pérdida auditiva previa, como el tiempo de privación auditiva y el comienzo del uso de CI -, audiometria e nel campo libre con CI a 250 a 4000 Hz frecuencias de sonido y El reconocimiento de voz se evaluó mediante la prueba "Lista de oraciones em portugués" - LSP aplicado em la condición favorable (silencio) y em la condición desfavorable (ruido) de la escucha. Em el análisis estadístico, se utilizaron coeficientes de correlación lineal de Pearson y gráficos de dispersión bidimensionales, además, um análisis descriptivo de los datos. Resultados: No hubo una relación estadísticamente significativa entre la privación auditiva y la percepción del habla en silencio y ruido. Por otro lado, hubo una correlación positiva estadísticamente significativa entre el tiempo de uso del implante coclear y el rendimento em la prueba de reconocimiento de oraciones en silencio. Conclusión: Hubo una correlación significativa solo entre el tiempo de uso del implante y el reconocimiento de oraciones en silencio em usuarios de implantes com sordera poslingual.

RESUMO Objetivo Correlacionar o desempenho de usuários de implante coclear unilateral em testes de reconhecimento de fala, no silêncio e no ruído, com as respostas ao questionário de autoavaliação Hearing Implant Sound Quality Index (HISQUI19) e correlacionar o desempenho em testes de reconhecimento de fala com o tempo de uso do dispositivo e a orelha implantada. Métodos Participaram 27 usuários de implante coclear unilateral com deficiência auditiva pós-lingual, que faziam uso do dispositivo há, pelo menos, um ano e apresentavam limiar tonal em campo livre menor que 40 dBA. Todos os participantes foram submetidos à audiometria tonal em campo livre, responderam ao questionário HISQUI19 e realizaram testes de reconhecimento de fala no silêncio e no ruído. Resultados As respostas ao questionário foram comparadas com os testes de reconhecimento de fala no silêncio e no ruído e não houve diferença estatisticamente significativa. Na comparação em relação ao tempo de uso do implante coclear, só houve diferença estatisticamente significativa para o teste de reconhecimento de fala no silêncio. Não houve correlação significativa entre o reconhecimento de fala e a orelha implantada. Conclusão independentemente do tempo de uso do dispositivo e/ou do desempenho nos testes de reconhecimento de fala, muitos participantes classificaram a qualidade sonora do implante coclear como moderada. A aplicação de testes que possibilitem mensurar a satisfação e o benefício dos usuários deve fazer parte da rotina clínica dos centros de implante.


ABSTRACT Purpose To correlate the performance of unilateral cochlear implant users in speech recognition tests, in quiet and noise, with the answers to the Hearing Implant Sound Quality Index (HISQUI19) self-assessment questionnaire; also, to correlate the performance in speech recognition tests with the time of CI use and the implanted ear. Methods A total of 27 unilateral CI users with postlingual hearing loss, who had been using the device for at least one year and had free-field pure-tone threshold lower than 40 dBA, participated in the study. All the participants were submitted to free-field pure-tone audiometry, answered the HISQUI19 questionnaire, and took speech recognition tests in quiet and noise. Results The answers to the questionnaire were compared with the speech recognition tests in quiet and noise; there was no statistically significant difference. When comparing with the time of CI use, there was a statistically significant difference only for the speech recognition test in quiet. There was no significant correlation between speech recognition and the implanted ear. Conclusion Regardless of the time of CI use and/or performance in the speech recognition tests, many participants classified the sound quality of their cochlear implant as moderate. Administering tests to measure the users' level of satisfaction and benefit should integrate the clinical routine in implantation centers.

RESUMO Objetivo Avaliar a percepção musical de adultos usuários de implante coclear, utilizando o Questionário de Música de Munique (Munich Music Questionnaire - MUMU). Métodos Pesquisa transversal, de abordagem quantitativa. Participaram do estudo 22 adultos pós-linguais, usuários de implante coclear há, pelo menos, um ano. O instrumento de coleta dos dados aplicado foi o Questionário de Munique, que abrange questões sobre a participação em atividades musicais e a respeito dos hábitos de ouvir música, em relação aos estilos musicais, aos diferentes instrumentos, ao ambiente de escuta e ao uso de dispositivos auxiliadores. Os dados foram computados em seus valores absolutos e relativos e foi utilizada estatística descritiva para caracterizar a amostra. Resultados Foi possível observar que houve melhora na frequência de música ouvida pós-implante coclear. Dos participantes, a maioria respondeu não ligar a música diretamente ao seu processador. A maioria dos pacientes mencionou que ouvia música por prazer e para relaxar e que conseguiam perceber, principalmente, o ritmo. O instrumento com maior frequência de detecção foi o piano, seguido da bateria. O gênero musical referido com grande satisfação foi música para dançar e religiosa. Dos 10 pacientes que tocavam instrumentos antes da perda auditiva, 4 voltaram a tocar após a implantação e 3, que não tocavam, começaram a tocar, após o implante coclear. Conclusão Foi possível observar que o uso do implante coclear propiciou melhora na percepção musical dos usuários, refletindo benefício na qualidade de vida. Por isso, a reabilitação deve incluir, dentro do treinamento auditivo, o desenvolvimento de habilidades musicais.


ABSTRACT Purpose To evaluate the musical perception of adults with cochlear implant using the Munich Music Questionnaire (MUMU). Methods Cross-sectional research with a quantitative approach, approved by the institution's Research Ethics Committee (Opinion n° 1,626,211). Twenty-one post-lingual adults who were cochlear implant users for at least one year participated in the study. The data collection instrument applied was the Munich Questionnaire, which covers questions about participation in musical activities and about the habits of listening to music in relation to musical styles, different instruments, the listening environment and the use of assistive devices. The data were computed in absolute and relative values ​​and descriptive statistics were used to characterize the sample. Results It was possible to observe that there was improvement in the frequency of music heard after cochlear implantation. Of the participants, most respondents did not connect the music directly to their processor. Most patients listen to music for pleasure and to relax, and they can mostly sense the rhythm. The instrument with the highest frequency of detection was the piano, followed by the drums. The musical genre referred to with great satisfaction was techno and religious. Of the 10 patients who played instruments before hearing loss, 4 played again after implantation and 3, who did not play, began to play after cochlear implantation. Conclusion It was possible to observe that the use of the cochlear implant provided an improvement in the users' musical perception, reflecting an improvement in the quality of life. Therefore, rehabilitation should include within the auditory training the development of musical abilities.

ABSTRACT

OBJECTIVE: Evaluate the effects of therapy with citalopram on the central auditory processing in the elderly measured by central auditory tests. STUDY DESIGN: Prospective, randomized, double-blind, placebo-controlled study. SETTING: Tertiary referral center. PATIENTS: Thirty-nine patients older than 60 years with normal hearing thresholds or symmetrical sensorineural hearing loss up to 70 dBHL, word-recognition score equal to or better than 70%, and diagnosed with central auditory processing disorders completed the study. They underwent the mini-mental state examination, as a way to screen those with the possibility of dementia; they also underwent the Beck depression inventory, for screening individuals with depression. INTERVENTION: Citalopram 20 mg/d or placebo for 6 months. MAIN OUTCOME MEASURE: The central auditory tests were applied to the selection of individuals with auditory processing disorders and repeated after 6 months' treatment. The tests were sound localization, speech in noise, dichotic digits test, pitch pattern sequence, duration pattern test, and gaps-in-noise. RESULTS: Comparisons of central auditory tests pre- and posttreatment in groups showed: sound localization (p = 0.022), pitch pattern sequence humming (p = 0.110), pitch pattern sequence nomination (p = 0.355), duration pattern test humming (p = 0.801), duration pattern test nomination (p = 0.614), and gaps-in-noise (p = 0.230). Dichotic tests in right and left ears respectively: speech in noise (p = 0.949; p = 0.722), dichotic digits test (p = 0.943; p = 0.513). CONCLUSION: There was no clinical effect with the use of citalopram in central auditory processing tests of the subjects.

In this context, antioxidants, widely used in numerous areas of medicine, appear to represent a promising approach for the control of this symptom, which often is poorly controlled. OBJECTIVE: To evaluate the effects of antioxidant therapy for tinnitus in a group of elderly patients. METHODS: Prospective, randomized, double-blinded, placebo-controlled clinical trial. The sample consisted of 58 subjects aged 60 years or older, with a complaint of tinnitus associated with sensorineural hearing loss. These individuals completed the Tinnitus Handicap Inventory (THI) questionnaire before and after six months of therapy. The treatment regimens were: Ginkgo biloba dry extract (120 mg/day), a-lipoic acid (60 mg/day) + vitamin C (600 mg/day), papaverine hydrochloride (100 mg/day) + vitamin E (400 mg/day), and placebo. RESULTS: There was no statistically significant difference between THI by degree (p = 0.441) and by score (p = 0.848) before and after treatment. CONCLUSION: There was no benefit from the use of antioxidant agents for tinnitus in this sample.


Resumo Introdução: Uma série de abordagens terapêuticas tem sido empregada no tratamento do zumbido, desde terapias cognitivo-comportamentais e de enriquecimento sonoro até terapias medicamentosas. Nesse contexto, os agentes antioxidantes, amplamente utilizados em diversas áreas da medicina, parecem representar uma perspectiva promissora para o controle desse sintoma, que muitas vezes tem um controle clínico insatisfatório. Objetivo: Avaliar os efeitos da terapia com agentes antioxidantes sobre o zumbido em um grupo de pacientes idosos. Método: Ensaio clínico prospectivo, randomizado, duplo-cego e controlado por placebo. A amostra composta de 58 indivíduos com 60 anos ou mais, com queixa clínica de zumbido associado à perda auditiva, do tipo neurossensorial, em graus variados. Esses indivíduos foram submetidos ao questionário THI (Tinnitus Handicap Inventory) antes e após 6 meses de uso da medicação. Os esquemas terapêuticos foram os seguintes: extrato seco de Ginkgo biloba(120 mg/dia), ácido a-lipóico (60 mg/dia) + vitamina C (600 mg/dia), cloridrato de papaverina(100 mg/dia) + vitamina E (400 mg/dia) e placebo. Resultados: O THI após o tratamento foi estatisticamente igual ao THI antes do tratamento, tanto em graus (p = 0,441) quanto em escores (p = 0,848). Conclusão: Não se verificou benefício estatisticamente significativo com o uso de agentes antioxidantes para o zumbido dos indivíduos avaliados.

RESUMO Objetivo: descrever as características de clustering e switching da prova de fluência verbal semântica e fonológica de deficientes auditivos adultos - usuários do português brasileiro - e verificar sua relação com o total de palavras evocadas e fatores biossociais. Métodos: 42 indivíduos deficientes auditivos adultos, usuários do português brasileiro oral participaram desta pesquisa. Realizaram as provas de fluência verbal semântica ("animais") e fonológica ("palavras com 'F'") e foram contabilizados: (a) número total de palavras, (b) número de categorias evocadas, (c) tamanho médio do cluster e (d) número de switches. Comparou-se aos dados biossociais e às características da deficiência auditiva e do dispositivo auditivo eletrônico. Resultados: o total de palavras evocadas foi de 16,38±6,18 para prova semântica e 10,88±6,1 para fonológica, sendo acessadas todas as categorias previamente estabelecidas. A média do tamanho do cluster foi de 1,41±0,8 para fluência verbal semântica e 0,71±0,73 para fonológica. O número médio de switches foi de 7,14±3,69 para fluência semântica e 6,36±4,17 para fonológica. Em ambas as provas, o número de switches apresentou relação forte e positiva com o total de palavras (p<0,001); e o tamanho do cluster com o total da prova fonológica (p=0,011). A escolaridade influenciou o total de palavras evocadas, o clustering e switching. O grau e época de aquisição da deficiência auditiva, bem como o uso de dispositivo auditivo eletrônico não tiveram relação com as variáveis avaliadas. Conclusão: as habilidades de clustering e switching nos deficientes auditivos são influenciadas pela escolaridade, sem relação direta com as características da deficiência auditiva.


ABSTRACT Purpose: to describe clustering and switching characteristics in semantic and phonetic verbal fluency of hearing-impaired adults - Brazilian Portuguese speakers - and to verify if there is relation between this characteristics, the total number of words recalled, and biosocial factors. Methods: 42 hearing-impaired adults, Brazilian-Portuguese speakers, were included. All performed the verbal fluency test by semantic and phonemic clue ("animals" and "letter F", respectively). We analyzed: (a) total words recalled, (b) number of categories recalled, (c) average cluster size, (d) number of switches. Moreover, evaluated the relationship of this data with biosocial information, hearing loss and hearing device characteristics. Results: total number of words recalled was 16.38±6.18 for semantic test and 10.88±6.1 for phonetic, on average. Participants retrieved all categories established. The average cluster size was 1.41±0.8 for semantic verbal fluency and 0.71±0.73 for phonetic. The average number of switches was 7.14±3.69 for semantic fluency and 6.36±4.17 for phonetic. For both tests the number of switches was strong and positively related to the total number of words (p<0.001); the average cluster size was related only for phonetic test (p=0.011). Educational level influenced the total number of words, clustering and switching. The hearing loss degree, age of hearing loss acquisition and the use of hearing devices did not present relation with any of evaluated variables. Conclusion: clustering and switching abilities in hearing-impaired subjects are influenced by educational level, without clear relation with hearing loss characteristics.

ABSTRACT

INTRODUCTION: For the assessment of child development in the deaf is effective protocols are needed for this population, as in Brazil, standardized tests for these children are still scarce 4.5. Thus, emphasis is placed on studying child development in deaf so that therapeutic and educational approaches are contemplated in accordance with the needs of each child. OBJECTIVE: Analyze the performance of children with hearing deficit in different areas: Communications-Issue, Communication-reception, aspects Motors and Cognitive Aspects of Language. METHODS: We have carried out a comparative study in an institutional clinic with 109 children, divided into 60 hearing individuals with typical development and 49 with hearing loss, severe to profound. The performance index was calculated for each child in these four domains. The index was analysed according to the equation: (number of responses in each area / number of assessed behaviours) x100. RESULTS: In all age groups, hearing-impaired children had a smaller performance when compared to hearing children in the Communication - Emission domain. The Communication-Reception domain showed significant differences (p < 0.05) from the 12-to-23-months to the 60-to-71-months age groups. The Cognitive Aspects domain demonstrated a significant difference (p < 0.01) between hearing-impaired and hearing children aged 24 to 35 months and onwards. Motor Aspects only had a significant difference (p < 0.01) in the last two age groups, 48-59 months and 60-71 months. The significance level was 5%. CONCLUSION: In all age groups, hearing-impaired children's performance was worse when compared to hearing children. The intervention made by health professionals ought to take place as soon as possible in order to develop the cognitive, motor and language skills of the hearing-impaired child.


INTRODUÇÃO: Para que a avaliação do desenvolvimento infantil em deficientes auditivos seja efetiva são necessários protocolos para esta população, já que, no Brasil, testes padronizados para estas crianças ainda são escassos 4,5. Assim, dá-se ênfase em estudar o desenvolvimento infantil em deficientes auditivos, para que abordagens terapêuticas e educacionais sejam contempladas de acordo com a necessidade de cada criança. OBJETIVO: Analisar o desempenho de crianças com déficit de audição nos diferentes domínios: Comunicação-Emissão, Comunicação-Recepção, Aspectos Motores e Aspectos Cognitivos da Linguagem. MÉTODO: Estudo comparativo em uma clínica institucional com 109 crianças, divididas em 60 crianças ouvintes com desenvolvimento típico e 49 crianças com perda auditiva neurossensorial de grau severo a profundo bilateral, para verificação dos desempenhos nos domínios Comunicação - Emissão e Recepção, Aspectos motores e Aspectos Cognitivos da Linguagem. Para cada criança foi calculado o índice de desempenho na Comunicação - Emissão e Recepção, Aspectos Motores e Aspectos Cognitivos da Linguagem. Os índices foram analisados da seguinte forma: (número respostas presentes em cada domínio/número de comportamentos avaliados) x100. RESULTADOS: Em todas as faixas etárias, encontramos desempenho inferior de crianças deficientes auditivas quando comparadas às crianças ouvintes no domínio Comunicação-Emissão. O domínio Comunicação-Recepção apresentou diferenças significantes (p < 0,05) a partir da faixa etária de 12 a 23 meses até a faixa de 60 a 71 meses. O domínio Aspectos Cognitivos da Linguagem apresentou diferença significante (p<0,01) entre crianças deficientes auditivas e ouvintes da faixa etária de 24 a 35 meses em diante. No domínio Aspectos Motores foi observada diferença significante (p<0,01) apenas nas duas últimas faixas etárias de 48 a 59 meses e 60 a 71 meses. O nível de significância adotado foi de 5%. CONCLUSÕES: Em todas as faixas etárias, encontramos desempenho inferior de crianças deficientes auditivas quando comparadas às crianças ouvintes.

PURPOSE: To compare the performance of hearing-impaired and normal-hearing people on phonologic and semantic verbal fluency tests. METHODS: A cross-sectional study was conducted with 48 hearing-impaired adults and 42 individuals (control group) with no hearing or language complaints. Sociodemographic data were collected, as well as the characteristics of hearing loss and of the electronic auditory device (hearing aids or cochlear implant), when relevant. Verbal fluency was tested in two different tasks: by semantic category (animals) and by phonology (letter F). RESULTS: Educational level has influenced the results of fluency tests in both groups, with more evidence in the hearing-impaired subjects (p<0.001). Hearing-impaired subjects showed worse performance in verbal fluency tests when compared to normal-hearing people in groups with up to 10 years of schooling. In the comparison of performance in the two tests, both groups showed better results in the semantic fluency task. CONCLUSION: The hearing-impaired subjects with low educational level evoked fewer words in semantic and phonologic verbal fluency tests in comparison to normal-hearing subjects. Educational level is a relevant issue to the study of verbal fluency in deaf and hearing-impaired people. .


OBJETIVO: Comparar o desempenho de deficientes auditivos e ouvintes nas provas de fluência verbal semântica e fonológica. MÉTODOS: Realizou-se um estudo transversal com 48 indivíduos adultos deficientes auditivos e 42 indivíduos (grupo comparação) sem queixas de audição e/ou linguagem. Foram levantados dados sociodemográficos e as características da perda auditiva e do dispositivo auditivo eletrônico (aparelho de amplificação sonora individual ou implante coclear), quando pertinente. Aplicaram-se os testes de fluência verbal por pista semântica (categoria animais) e fonológica (letra F). RESULTADOS: A escolaridade influenciou os resultados das provas nos dois grupos, sendo mais evidente nos deficientes auditivos (p<0,001). Os deficientes auditivos apresentaram pior desempenho nas provas de fluência verbal em comparação aos ouvintes nos grupos com até dez anos de escolaridade. Na comparação do desempenho nos dois testes, os dois grupos apresentaram melhores resultados na fluência verbal semântica. CONCLUSÃO: Os deficientes auditivos de menor escolaridade evocaram um número inferior de palavras pela pista semântica e fonológica em relação aos ouvintes. O nível de escolaridade é relevante para o estudo de fluência verbal em deficientes auditivos. .

By analyzing the group with hearing loss, there was a mean difference between tests (p<0.001), which also showed correlation between them. Individuals with pre-lingual hearing loss and those who underwent therapy for speechreading had a better performance for most speechreading instruments. The variables gender and schooling showed no influence on speechreading. CONCLUSION: Individuals with hearing impairment had better performance on speechreading tasks in comparison to people with normal hearing. Furthermore, it was found that the ability to perform speechread might be influenced by the vocabulary, period of installation of the hearing loss, and speechreading therapy.

Objetivos: Comparar a habilidade de leitura da fala entre indivíduos com deficiência auditiva e ouvintes e verificar os fatores que podem influenciá-la nos deficientes auditivos. Métodos: Foram avaliados 40 indivíduos com perda auditiva de grau severo a profundo e idade entre 13 e 70 anos (grupo em estudo) e 21 ouvintes com idade entre 17 e 63 anos (grupo controle). Como instrumentos de pesquisa, utilizaram-se anamnese, para caracterizar os grupos; três instrumentos de leitura da fala, apresentando-se estímulos por meio de vídeo, sem som, com uma locutora feminina; e um teste de vocabulário, para verificar a sua influência sobre a leitura da fala. Realizou-se estatística descritiva e analítica (Teste ANOVA e Correlação de Pearson), adotando-se o nível de significância de 0,05 (5%). Resultados: Observou-se melhor desempenho do grupo com deficiência auditiva nos testes de leitura da fala do que ouvintes. Analisando somente o grupo com deficiência auditiva, verificou-se diferença de desempenho entre os testes (p<0,001) e estes apresentaram correlação entre si. Apresentaram melhor desempenho para a maioria dos instrumentos de leitura da fala os indivíduos com deficiência auditiva pré-lingual e os que realizaram terapia de leitura da fala. As variáveis sexo e anos de estudo não indicaram influência na leitura da fala. Conclusão: Indivíduos com deficiência auditiva apresentam melhor desempenho em tarefas de leitura da fala em comparação com ouvintes. Além disso, verificou-se que a capacidade de realizar a leitura da fala pode ser influenciada pelo vocabulário, época de instalação da deficiência auditiva e realização da terapia de leitura da fala. .

DESIGN AND SETTING: analytical cross-sectional study at the Department of Speech-Language and Hearing Sciences, Universidade Federal de São Paulo. METHODS: twenty-one moderately to profoundly bilaterally hearing-impaired children (cases) and 21 normal-hearing children without language abnormalities (controls), matched according to sex, age, schooling level and school type, were studied. A board showing pictures in a temporally logical sequence was presented to each child, to elicit a narrative, and the child's performance relating to narrative structure and cohesion was measured. The frequencies of variables, their associations (Mann-Whitney test) and their 95% confidence intervals was analyzed. RESULTS: the deaf subjects showed poorer performance regarding narrative structure, use of connectives, cohesion measurements and general punctuation (P < 0.05). There were no differences in the number of propositions elaborated or in referent specification between the two groups. The deaf children produced a higher proportion of orientation-related propositions (P = 0.001) and lower proportions of propositions relating to complicating actions (P = 0.015) and character reactions (P = 0.005). CONCLUSION: hearing-impaired children have abnormalities in different aspects of language, involving form, content and use, in relation to their normal-hearing peers. Narrative competence was also associated with the children's ages and the school type.

CONTEXTO E OBJETIVO: A narrativa oral constitui elemento de avaliação do desenvolvimento lingüístico, entretanto, são escassos os dados padronizados para pacientes surdos. O objetivo foi comparar o uso das competências narrativas entre crianças deficientes auditivas e ouvintes. TIPO DE ESTUDO E LOCAL: Estudo transversal analítico no Departamento de Fonoaudiologia, Universidade Federal de São Paulo. MÉTODO: Foram avaliadas 21 crianças com deficiência auditiva de grau moderado a profundo bilateral (casos) e 21 crianças ouvintes sem alterações de linguagem (controles) pareadas por sexo, idade, grau de escolaridade e tipo de escola frequentada. Foi apresentada uma prancha com figuras em sequência lógico-temporal para cada criança, para elicitar uma narrativa, e foi medido o desempenho na estrutura da narrativa e coesão. Foram analisadas as frequências das variáveis, suas associações (teste de Mann-Whitney) e intervalos de confiança de 95 por cento. RESULTADOS: As crianças surdas apresentaram desempenho pior, na estrutura da narrativa, uso de conjunções, medidas de coesão e pontuação geral da narrativa (P < 0,05). Não houve diferença no número de proposições elaboradas e especificação de referentes nos dois grupos. Os surdos produziram maior proporção de proposições do tipo orientação (P = 0,001) e menor proporção dos tipos ações complicadoras (P = 0,015) e reação do personagem (P = 0,005). CONCLUSÃO: Os deficientes auditivos possuem alterações nos diferentes aspectos da linguagem, envolvendo forma, conteúdo e uso em relação a seus pares ouvintes. A competência narrativa também está relacionada ao tipo de escola e idade das crianças estudadas.

ABSTRACT

A proposta deste estudo foi descrever os efeitos da avaliação clínica dinâmica interdisciplinar em um caso atendido pelo Ambulatório de Investigação da Linguagem nos Distúrbios de Audição do Hospital São Paulo - Universidade Federal de São Paulo. Trata-se de uma pessoa do sexo feminino que, aos 26 anos, apresentou quadro de surdez súbita bilateral, em decorrência da síndrome de Cogan - que afeta também a visão. Ao iniciar o processo, a paciente apresentava leitura orofacial restrita, dificuldade para se comunicar com sua família, isolamento social e quadro depressivo. A avaliação dinâmica pautada na interatividade e na escuta humanística proporcionou condições para que a paciente pudesse melhorar sua comunicação, autoconceito e interação social. Nessa perspectiva, observou-se que, com apenas dez sessões de atendimento pela equipe, operaram-se efeitos terapêuticos quebrando-se, assim, a clássica dicotomia avaliação/intervenção em momentos diferentes.


The aim of this study was to describe the effects of the interdisciplinar dynamic clinical assessment in a case attended at the Research Ambulatory of Language in Hearing Disorders of the São Paulo Hospital - Federal University of São Paulo. It is describe the case of a 26-year-old woman who presented sudden bilateral deafness, as a result of Cogan's syndrome - which also affects the vision. When the intervention process began, the patient had restricted orofacial reading, difficulty to communicate with her family, social isolation and depressive symptoms. The dynamic assessment based on interactivity and humanistic listening provided conditions for the patient to improve her communication, self-concept and social interaction. From this perspective, it was noticed that, with only ten sessions of intervention, therapeutic effects were operated, therefore breaking the classic assessment/intervention dichotomy at different moments.

ABSTRACT

O objetivo deste estudo foi caracterizar a narrativa oral de deficientes auditivos. Para isso, foram avaliados quatro indivíduos com deficiência auditiva pré-ligual, com perda auditiva de grau moderado a profundo bilateral, oralizados. A idade dos participantes variou de 14 a 28 anos. Para eliciar as narrativas foram utilizadas seqüências de figuras que compunham uma história. A análise dos dados seguiu os seguintes critérios: número de ações narradas, presença de marcadores do processo narrativo, atribuição de intencionalidade aos personagens, além da estrutura da narrativa. A metodologia utilizada foi a de estudo de caso. Mesmo considerando os dados caso a caso, observamos maior freqüência do uso do recurso comunicativo oral, vocabulário rotineiro e domínio assistemático de regras da língua, denotando déficits na relação forma e conteúdo. Apesar disso, concordamos com os dados da literatura que apontam para a influência positiva dos fatores grau da perda, diagnóstico precoce, intervenção precoce, suporte familiar, escolarização e protetização precoce, quando se fala de prognóstico. Os deficientes auditivos possuem dificuldades peculiares provavelmente relacionadas à privação sensorial e em decorrência das inabilidades nas conversões língua-linguagem.
