ABSTRACT

Introducción: La investigación de estados de ánimo en la consulta odontológica se ha centrado principalmente en edades pediátricas; sin embargo, es importante conocer qué sucede en otros grupos etarios. Objetivo: Evaluar el estado de ánimo de los adultos mayores previo a un procedimiento odontológico de primer nivel en la Clínica Universitaria de Atención a la Salud Zaragoza. Materiales y métodos: Se realizó un estudio de tipo exploratorio observacional transversal, con una muestra de 34 adultos mayores (22 mujeres y 12 hombres), con una media de edad de 69,2. La selección de la muestra fue no probabilística por conveniencia. Se utilizó la Escala de Valoración del Estado de Ánimo, que es un instrumento de autorreporte, validado en población adulta para condiciones clínicas y no clínicas. Resultados: Se encontró un puntaje alto en la subescala de Alegría, con una media de 6,63 y una desviación típica de 2,7. Para las subescalas de estado de ánimo consideradas negativas, como Ansiedad, se obtuvo una media de 3,5 y una desviación típica de 2,5. Para Tristeza-Depresión se obtuvo una media de 3,1 y una desviación típica de 3,0 y, finalmente, para la subescala de Ira-Hostilidad, se obtuvo una media de 2,7 y una desviación típica de 3,1. Conclusiones: Se encontraron niveles bajos en los estados de ánimo negativos para las subescalas de Ansiedad, Tristeza-Depresión e Ira-Hostilidad. Por su parte, en los estados de ánimo positivos hay un nivel alto, tanto por ítem como en la subescala de Alegría.


Introduction: The research on mood states in dental consultation has focused mainly on pediatric ages; however, it is important to know what happens in other age groups. Objective: To evaluate the mood of older adults prior to a first-level dental procedure at the Zaragoza University Health Care Clinic. Materials and methods: An exploratory, observational, cross-sectional study was carried out with a sample of 34 older adults (22 women and 12 men) with an average age of 69.2. The sample selection was non-probabilistic for convenience. The Mood Assessment Scale was used, which is a self-report instrument, validated in the adult population, for clinical and non-clinical conditions. Results: A high score was found in the joy subscale with a mean of 6.63 and a standard deviation of 2.7. For the mood subscales considered negative such as Anxiety, a mean of 3.5 and a standard deviation of 2.5 was obtained. For Sadness -Depression, a mean of 3.1 and a standard deviation of 3.0 were obtained; and finally for the Anger-Hostility subscale, a mean of 2.7 and a standard deviation of 3.1 were obtained. Conclusion: Low levels were found in negative mood states for the subscales of Anxiety, Sadness-Depression and Anger-Hostility. Meanwhile, in positive mood states there is a high level, both in item and in the subscale of Joy.

ABSTRACT

El flutter auricular es la taquicardia confinada al atrio más frecuente en la edad neonatal, su adecuado tratamiento requiere un diagnóstico precoz. La posibilidad de presentar efectos adversos con los fármacos antiarrítmicos en recién nacidos es alta, por lo que se debe evitar el manejo empírico de la taquicardia en éste grupo etario. Se presenta el caso de un recién nacido con flutter auricular en corazón sano, que respondió favorablemente a las medidas farmacológicas convencionales, con buena evolución a mediano plazo.


Atrial flutter is the atrial confined tachycardia more frequent in the neonatal age, its adequate treatment requires an early diagnosis. The possibility of presenting adverse effects with antiarrhythmic drugs in newborns is high, so the empirical management of tachycardia should be avoided in this age group. We present the case of a newborn with atrial flutter in healthy heart, who responded favorably to conventional pharmacological measures, with good evolution in the medium term.

ABSTRACT

Los pacientes con hipertrigliceridemia tiene un riesgo de desarrollar pancreatitis del 1,5 %. Cuando estos niveles superan los 1,000 mg/dl, el porcentaje asciende al 20,2 %. Se presenta el caso de una paciente femenina de 27 años que ingresa al servicio de Terapia Intensiva Adultos del hospital Caja Petrolera de Salud Santa Cruz, por cuadro de dolor abdominal en mesogastrio con erradicación a flanco e hipocondrio derecho asociado a pérdida de peso, polluria y polifagia. En la primera analítica destaca suelo lipe mico con valores de triglicéridos de 3675 mg/dl, glucemia 444 mg/dl y cetonuria.

RESUMEN Introducción: En México, la enfermedad renal crónica (ERC) representa un gran problema de salud y las glomerulopatías (GP) representan la tercera causa de ERC. Objetivo: Describir, desde una base de datos de biopsias renales (BR) nativas, los diferentes patrones morfológicos de GP en México. Métodos: Se analizaron registros de BR de riñón nativo en un centro de referencia en nefropatología, todas las BR fueron evaluadas por una única nefropatóloga (AVP). El diagnóstico final en cada caso se basó en parámetros clínicos e histopatológicos. Resultados: Fueron revisadas 2084 BR, con edad de 34.4 ± 17.6 años. 1085 BR (52.1%) en género femenino; el síndrome nefrótico fue más frecuente en hombres (p<0.001) y síndrome nefrítico fue más frecuente en mujeres (p<0.001). GP primarias y nefropatías túbulo-intersticiales fueron más diagnosticadas en hombres (p<0.01). Nefritis lúpica (NL) fue la GP secundaria más reportada. La glomeruloesclerosis focal y segmentaria (GEFS) fue la GP primaria diagnosticada con mayor frecuencia en ambos géneros. Vasculitis por inmunoglobulina A fue la enfermedad vascular más frecuentemente detectada. Síndrome nefrótico fue la indicación más frecuente de BR (42.9%), seguido de: síndrome nefrítico (23.9%), proteinuria aislada (16.4%), daño renal agudo (8.7%), alteraciones urinarias asintomáticas (6.2%) y ERC (1.8%). Conclusiones: La GP primarias con mayor frecuencia fueron GEFS. Las GP secundarias más frecuentemente reportadas fueron NL, predominantemente en mujeres. Se observó nefropatía IgA con mayor frecuencia en comparación con otras series publicadas en México. Hubo diferencias significativas en la presentación de GP en relación con el género y la edad del paciente.


ABSTRACT Introduction: In Mexico, chronic kidney disease (CKD) represents a major health problem, and glomerulopathies (GP) represent the third leading cause of CKD. Aim: From a database of native kidney biopsies (KB), describe the different morphological patterns of GP in Mexico. Methods: Records of native KB in a nephropathology referral center were evaluated by a single nephropathologist. The final diagnosis in each case was based on clinical parameters and histopathological findings. Results: 2084 KB were analyzed, patients were 34.4±17.6 years of age, there were 1085 KB (52.1%) in females; nephrotic syndrome was most frequent in males (p<0.001), and nephritic syndrome was more frequent in females (p <0.001). Primary GP and túbulo-interstitial diseases were most diagnosed in males (p <0.01). Lupus nephritis (LN) was the most-reported secondary GP. Focal and segmental glomerulosclerosis (FSGS) was the primary GP most often diagnosed in both genders. The most frequently detected vascular disease was immunoglobulin A vasculitis. Nephrotic syndrome was the most frequent indication for KB (42.9%), followed by: nephritic syndrome (23.9%), isolated proteinuria (16.4%), acute kidney injury (8.7%), asymptomatic urinary alterations (6.2%), and CKD (1.8%). Conclusions: The most frequently observed primary GP was FSGS, and LN was the most frequent secondary GP, predominantly in females, and IgA nephropathy was observed more frequently in comparison with other series published in Mexico. There were significant differences in GP presentation in relation to patient sex and age.

ABSTRACT

An impaired coagulation process has been described in patients with severe or critical coronavirus disease (COVID-19). Nevertheless, the implication of coagulation-related genes has not been explored. We aimed to evaluate the impact of F5 rs6025 and SERPINE1 rs6092 on invasive mechanical ventilation (IMV) requirement and the levels of coagulation proteins among patients with severe COVID-19. Four-hundred fifty-five patients with severe COVID-19 were genotyped using TaqMan assays. Coagulation-related proteins (P-Selectin, D-dimer, P-selectin glycoprotein ligand-1, tissue plasminogen activator [tPA], plasminogen activator inhibitor-1, and Factor IX) were assessed by cytometric bead arrays in one- and two-time determinations. Accordingly, SERPINE1 rs6092, P-Selectin (GG 385 pg/mL vs. AG+AA 632 pg/mL, p = 0.0037), and tPA (GG 1858 pg/mL vs. AG+AA 2546 pg/mL, p = 0.0284) levels were different. Patients carrying the CT F5-rs6025 genotype exhibited lower levels of factor IX (CC 17,136 pg/mL vs. CT 10,247 pg/mL, p = 0.0355). Coagulation proteins were also different among IMV patients than non-IMV. PSGL-1 levels were significantly increased in the late stage of COVID-19 (>10 days). The frequencies of F5 rs6025 and SERPINE1 rs6092 variants were not different among IMV and non-IMV. The SERPINE1 rs6092 variant is related to the impaired coagulation process in patients with COVID-19 severe.

BACKGROUND: The impact of genetic variants in the expression of tumor necrosis factor-α (TNF-α) and its receptors in coronavirus disease 2019 (COVID-19) severity has not been previously explored. We evaluated the association of TNF (rs1800629 and rs361525), TNFRSF1A (rs767455 and rs1800693), and TNFRSF1B (rs1061622 and rs3397) variants with COVID-19 severity, assessed as invasive mechanical ventilation (IMV) requirement, and the plasma levels of soluble TNF-α, TNFR1, and TNFR2 in patients with severe COVID-19. METHODS: The genetic study included 1353 patients. Taqman assays were used to assess the genetic variants. ELISA was used to determine soluble TNF-α, TNFR1, and TNFR2 in plasma samples from 334 patients. RESULTS: Patients carrying TT (TNFRSF1B rs3397) exhibited lower PaO2/FiO2 levels than those with CT + CC genotypes. Differences in plasma levels of TNFR1 and TNFR2 were observed according to the genotype of TNFRSF1B rs1061622, TNF rs1800629, and rs361525. According to the studied genetic variants, there were no differences in the soluble TNF-α levels. Higher soluble TNFR1 and TNFR2 levels were detected in patients with COVID-19 requiring IMV. CONCLUSIONS: Genetic variants in TNF and TNFRSFB1 influence the plasma levels of soluble TNFR1 and TNFR2, implicated in COVID-19 severity.

ABSTRACT

Background: Genetic association studies have identified single nucleotide polymorphisms (SNPs) associated with lasting lung diseases such as Chronic Obstructive Pulmonary Disease (COPD) and Idiopathic Pulmonary Fibrosis (IPF), as well as the simultaneous presentation, known as Combined Pulmonary Fibrosis and Emphysema (CPFE) Syndrome. It is unknown if these diseases share genetic variants previously described in an independent way. This study aims to identify common or differential variants between COPD, IPF, and CPFE. Materials and methods: The association analysis was carried out through a case-control design in a Mexican mestizo population (n = 828); three patients' groups were included: COPD smokers (COPD-S, n = 178), IPF patients (n = 93), and CPFE patients (n = 16). Also, two comparison groups were analyzed: smokers without COPD (SWOC, n = 367) and healthy subjects belonging to the Mexican Pulmonary Aging Cohort (PAC, n = 174). Five SNPs in four genes previously associated to interstitial and obstructive diseases were selected: rs2609255 (FAM13A), rs2736100 (TERT), rs2076295 (DSP) rs5743890, and rs111521887 (TOLLIP). Genotyping was performed by qPCR using predesigned Taqman probes. Results: In comparing IPF vs. PAC, significant differences were found in the frequency of the rs260955 G allele associated with the IPF risk (OR = 1.68, p = 0.01). Also, the genotypes, GG of rs260955 (OR = 2.86, p = 0.01) and TT of rs2076295 (OR = 1.79, p = 0.03) were associated with an increased risk of IPF; after adjusting by covariables, only the rs260955 G allele remain significant (p = 0.01). For the CPFE vs. PAC comparison, an increased CPFE risk was identified since there is a difference in the rs2736100 C allele (OR = 4.02, p < 0.01; adjusted p < 0.01). For COPD-S, the rs2609255 TG genotype was associated with increased COPD risk after adjusting by covariables. Conclusion: The rs2736100 C allele is associated with decreased IPF risk and confers an increased risk for CPFE. Also, the rs2076295 TT genotype is associated with increased IPF risk, while the GG genotype is associated with CFPE susceptibility. The rs2609255 G allele and GG genotype are associated with IPF susceptibility, while the TG genotype is present in patients with emphysema.

ABSTRACT

Avian influenza (AI) is a disease caused by influenza viruses type A that belong to the Orthomyxoviridae family. AI induces high economic losses in poultry production worldwide. Due to a possible outbreak, a national surveillance program was needed. From April to July 2016, 152 industrial poultry farms were randomly sampled. All samples were analyzed by competitive ELISA for Influenza type A viruses. Suspicious and positive sera were further analyzed by Hemagglutination Inhibition (HI) in order to serotype H5 or H7 low pathogenic avian influenza virus (LPAIV). The farms sampled showed 94.08%, 3.95% and 1.97% of negative, positive and suspicious results, respectively. However, serotyping revealed all positive and suspicious samples were negative to H5/H7 LPAIV. Our results show the absence of AI in the mainland Ecuadorian industrial poultry production.

ABSTRACT

Peritoneal dialysis (PD) is an alternative for managing the end-stage renal disease (ESRD). The peritoneal membrane (PM) is not just a membrane that passively responds to diffusion and convection. The characteristics of PM result in the peritoneal equilibrium test (PET) and with this test is possible to obtain the type of peritoneal transport (PT). The patient on PD can be classified in different types of PT as; Low, Low Average, High Average, and High. The aim of the study was to compare the inflammatory cytokines, oxidants, antioxidants, and oxidative DNA damage markers in the different types of PT. A cross-sectional analytical study of 77 adult PD patients was performed. Levels of lipoperoxides (LPO) were higher in all types of PT vs. healthy volunteer controls (HC) (p < 0.0001). Nitric oxide (NO) levels were found significantly down-regulated in all types of PT (p < 0.0001). The activity of the superoxide dismutase enzyme (SOD) was found to be significantly increased in all types of PT vs. the HC (p < 0.0001). The levels of the DNA repair enzyme were found to be decreased in all types of PT. The levels of the pro-inflammatory cytokines TNF-α, IL-6, the marker of oxidative DNA damage, 8-IP and the total antioxidant capacity (TAC) were all significantly decreased, contrary to the levels in HC, possibly by the clearance in the dialysis fluid in all types of PT or due to down-regulation of their expression. In conclusion, we found significant changes in markers of inflammation, oxidative stress, and oxidative damage to DNA in all types of PT; Low, low average, high average, and high PT in the values of D/P creatinine at 4 h compared to HC.

ABSTRACT

The Yucatan Peninsula possesses a unique climate, geology, landscape, and biota that includes a distinct flora of over 2300 species; of these, close to 800 plants are used in what is known as Mayan traditional medicine, and about 170 are listed as native or endemic. Even though the flora of the Yucatan peninsula has been widely studied by naturalists and biologists, to date, phytochemical and pharmacological knowledge of most of the plants, including the medicinal plants, is limited. Presently, phytochemical studies carried out on plants from the Yucatecan flora have resulted in the identification of a wide variety of natural products that include flavonoids, terpenoids, polyketides, and phenolics with cytotoxic, antiprotozoal, antibacterial, anti-inflammatory, analgesic, antioxidant, and antifungal activities. This review describes the main findings in over 20 years (1992 to 2018) of exploring the natural product diversity of the Yucatecan flora.

ABSTRACT

The current concern about resistance to acaricides and the impact of toxic waste on the environment has led to the search of vegetal alternatives in the control of the brown tick of the dog Rhipicephalus sanguineus. Schinus molle L. (Anacardiaceae) derivatives have been associated with insecticidal, antimicrobial and antiprotozoal activities and essential oil showed to be lethal to R. microplus larvae. This study aimed at evaluating the acaricidal effect of essential oil of S. molle (EOSm) on engorged adult females and larval stages of R. sanguineus. One-hundred engorged females were obtained from the ears, interdigital spaces, neck, groin and base of the tail of two cross-bred dogs. The larvae package test was accomplished with 21-day-old larvae and five concentrations (v/v) of EOSm (0.125, 0.25, 0.50, 1 and 2%) in an anionic detergent, a synthetic acaricide (cypermethrin) and detergent and deionized water as controls. The immersion adult test was carried out with nine concentrations (0.125, 0.25, 0.50, 1, 2, 4, 8, 16, 20%) of the EOSm. At the concentration of 2%, EOSm caused 99.3% of larval mortality. In adults, inhibition of oviposition, egg hatching (EH) and reproductive efficiency (RE) values were dose-dependent from 4 to 20% EOSm; the lowest values of EH (29.62) and RE (22.61) were achieved with 20% EOSm. Strong and negative correlations were found between concentration of EOSm and EH (r = - 0.948) and between concentration of EOSm and RE (r = - 0.985). This study demonstrated for the first time the acaricidal effect of EOSm on larvae and reproductive parameters of engorged adult females of R. sanguineus.

ABSTRACT Calea urticifolia (Mill.) DC., Asteraceae, is a native plant of the Yucatan Peninsula used in traditional medicine to treat inflammation and pain. The bioassay-guided purification of the ethanol root extract allowed the isolation of the main bioactive metabolites, which were identified as an inseparable mixture of thymol (1) and 3-methyl-4-isopropylphenol (2), together with 3,4-O-dicaffeoylquinic acid methyl ester (3), 3,4-O-dicaffeoyl-epi-quinic acid methyl ester (4), 3,5-O-dicaffeoyl-epi-quinic acid methyl ester (5) and 3,5-O-dicaffeoylquinic acid (6). The results showed that the analgesic activity detected in the root extract of C. urticifolia could be attributed mainly to the mixture of 1 and 2 and to the novel 3,4-O-dicaffeoyl-epi-quinic acid methyl ester (4). Alternatively, the similarity on the antiinflammatory and antioxidant activities of the dicaffeoylquinic acid derivatives 3-5 suggests that the former might be related to their ability as radical scavengers.

ABSTRACT

Foot-and-Mouth Disease serotype O circulated endemically in Ecuador for many years, with an upsurge occurring in 2009. This manuscript describes retrospectively in vitro and in vivo laboratory studies to predict the field effectiveness of a commercial FMD vaccine to protect against the field strain, and explains the key actions and epidemiological strategies followed by the country to control the disease. The results established that the use of a good quality oil vaccine, manufactured with strains that were isolated long ago: O1 Campos Br/58 and A24 Cruzeiro Br/55; combined with the correct epidemiological strategies, are useful to control field strains when used in periodic biannual vaccination campaigns.

ABSTRACT

OBJECTIVES: To determine the frequency, clinical features, and factors associated with M. ozzardi infection in volunteers of a malaria screening campaign in the district of Alto Nanay in the Amazon jungle of Peru. MATERIALS AND METHODS: A descriptive, analytical and cross-sectional study was performed. The participants were interviewed and examined by a physician and the data were recorded in a medical record. The diagnosis of M. ozzardi infection was performed using the method of thick blood smear and film. RESULTS: The frequency of mansonelosis in this population was 47.8% (95% CI: 39.1 to 56.6). Through bivariate analysis we found that the report of decreased visual acuity or blurred vision and presence of subcutaneous tumors were the signs and symptoms statistically associated with the infection of microfilariae (p<0.05). Logistic regression found statistical association for residency in localities of the Pintuyacu or Alto Nanay rivers, employment in places far from the town center, the presence of subcutaneous tumors and skin thickening (p<0.05). CONCLUSIONS: There was a high number of mansonelosis by M. ozzardi in the district of Alto Nanay which was significantly related to working outside the town center, residing in the area of the Pintuyacu River, and skin lesions.

Objetivos. Determinar la frecuencia, manifestaciones cl¡nicas y factores asociados a la infecci¢n por Mansonella ozzardi en voluntarios de una campa¤a de despistaje de malaria en el distrito de Alto Nanay en la selva amaz¢nica del Per£. Los participantes fueron entrevistados y examinados por un m‚dico y los datos fueron registrados en una ficha cl¡nica. El diagn¢stico de infecci¢n por M. ozzardi se realiz¢ mediante la t‚cnica de gota gruesa y frotis. Resultados. La frecuencia de mansonelosis en esta poblaci¢n fue 47,8% (IC 95%: 39,1–56,6). La regresi¢n log¡stica encontr¢ asociaci¢n estad¡stica para la residencia en localidades del r¡o Pintuyacu o Alto Nanay, la ocupaci¢n en lugares alejados del centro poblado, la presencia de tumores subcut neos y el engrosamiento de la piel (p<0,05).Conclusiones . Existe una elevada frecuencia de mansonelosis por M. ozzardi en el distrito de Alto Nanay, la cual se encontr¢ significativamente relacionada con ocupaciones realizadas fuera del centro poblado, residencia en localidades del r¡o Pintuyacu y lesiones cut neas.


Objectives. To determine the frequency, clinical features, and factors associated with M. ozzardi infection in volunteers of a malaria screening campaign in the district of Alto Nanay in the Amazon jungle of Peru. Materials and methods. A descriptive, analytical and cross-sectional study was performed. The participants were interviewed and examined by a physician and the data were recorded in a medical record. The diagnosis of M. ozzardi infection was performed using the method of thick blood smear and film. Results.The frequency of mansonelosis in this population was 47.8% (95% CI: 39.1 to 56.6). Through bivariate analysis we found that the report of decreased visual acuity or blurred vision and presence of subcutaneous tumors were the signs and symptoms statistically associated with the infection of microfilariae (p<0.05). Logistic regression found statistical association for residency in localities of the Pintuyacu or Alto Nanay rivers, employment in places far from the town center, the presence of subcutaneous tumors and skin thickening (p<0.05). Conclusions. There was a high number of mansonelosis by M. ozzardi in the district of Alto Nanay which was significantly related to working outside the town center, residing in the area of the Pintuyacu River, and skin lesions.

ABSTRACT

La coartación aórtica crítica del recién nacido es una de las cardiopatías congénitas con mayor mortalidad. Se presenta el caso de un paciente diagnosticado de coartación aortica a las dos semanas de vida, operado de coartectomía y anastomosis termino terminal. A los tres meses de edad evoluciona desfavorablemente, con deterioro hemodinámico, confirmándose una recoartación aórtica crítica, por lo que se decide realizar una aortoplastía con balón por vía percutánea, que resulta exitosa y sin complicaciones inmediatas. La aortoplastía con balón constituye una opción terapéutica aceptablemente segura y mínimamente invasiva en pacientes pediátricos con recoartación aórtica.


Aortic coarctation of the newborn is a congenital heart disease with increased mortality. We report the case of an aortic coarctation diagnosed at two weeks old patient, surgery was indicated, the procedure was coarctectomy with end-to-end anastomosis. At three months of age evolves unfavorably, with hemodynamic compromise, confirming critical aortic recoarctation, so it was decided to make a balloon aortoplasty percutaneously, which is successful without immediate complications. The balloon aortoplasty is an acceptably safe and minimally invasive therapeutic option in pediatric patients with aortic coarctation.
