ABSTRACT

BACKGROUND: Understanding the epidemiology of Streptococcus pneumoniae (S. pneumoniae) isolates is important for pneumonia treatment and prevention. This research aimed to explore the epidemiological characteristics of S. pneumoniae isolated from pediatric inpatients and outpatients during the same period. METHODS: S. pneumoniae were isolated from unsterile samples of inpatients and outpatients younger than five years old between March 2013 and February 2014. The serotypes were determined using diagnostic pneumococcal antisera. The resistance of each strain to 13 antibiotics was tested using either the E-test or the disc diffusion method. The Sequence Types (STs) were analyzed via Multilocus Sequence Typing (MLST). RESULTS: The dominant serotypes obtained from inpatients were 19F (32.9 %), 19A (20.7 %), 23F (10.7 %), 6A (10.0 %), and 14 (8.6 %), while those from outpatients were 19F (13.6 %), 23F (12.9 %), 6A (10.0 %), 6B (10.0 %), and 19A (7.9 %). The coverage rates of 13-valent Pneumococcal Conjugate Vaccine (PCV) formulations were high in both groups. The nonsusceptibility to penicillin, cefuroxime, imipenem, erythromycin, and trimethoprim-sulfamethoxazole among the inpatient isolates was 7.1 %, 92.8 %, 65.7 %, 100 %, and 85.0 %, respectively, while that among the outpatient isolates was 0.7 %, 50.0 %, 38.6 %, 96.4 %, and 65.7 %, respectively. There were 45 and 81 STs detected from the pneumococci isolated from inpatients and outpatients, respectively. CC271 was common among both inpatients and outpatients (43.6 % and 14.3 %). CONCLUSIONS: Pneumococcal vaccine-related serotypes are prevalent among both inpatients and outpatients, especially among inpatients, who exhibit more severe antibiotic resistance. Therefore, universal immunization with PCV13 would decrease the hospitalization rate due to S. pneumoniae and the antibiotic resistance rate of S. pneumoniae.

OBJECTIVE: This study aims to investigate the application value of magnetic resonance (MR) hydrography of the inner ear in cochlear implantation. METHODS: 146 patients were enrolled. MR hydrography and spiral CT examinations for the intracranial auditory canal were performed before surgery, and all imaging results were statistically analyzed in order to explore the application value of MR hydrography of the inner ear in cochlear implantation. RESULTS: 146 patients (292 ears) were examined. Among these patients, 13 were diagnosed with abnormal vestibular aqueducts (20 ears) by MR hydrography, while five were diagnosed with this disease by CT; 15 patients were diagnosed with inner ear malformation (19 ears) by MR hydrography, while 11 were diagnosed by CT (four were misdiagnosed); five patients were diagnosed with internal acoustic canal stenosis (eight ears) by MR hydrography, while two were diagnosed by CT (three were misdiagnosed); and four patients were diagnosed with cochlear fibrosis (five ears) by MR hydrography, while four were diagnosed by CT (four ears). The correct rate of diagnosis was 77.40% (113/146) based on CT, while the rate was 93.84% (137/146) based on MR hydrography. CONCLUSIONS: MR hydrography imaging technique can be applied to the preoperative evaluation of cochlear implantation, providing accurate and reliable anatomic information on the inner membranous labyrinth and nerves in the internal acoustic canal and an accurate basis for the diagnosis of cochlear fibrosis and nerve development. This has a guiding significance for the selection of treatment schemes.

RESUMO OBJETIVO Este estudo visa investigar o valor da aplicação da hidrografia por ressonância magnética (RM) do ouvido interno no implante coclear. MÉTODOS Cento e quarenta e seis pacientes foram inscritos. Os exames da hidrografia por RM e do CT espiral para o canal auditivo intracraniano foram executados antes da cirurgia, e todos os resultados da imagem foram analisados estatisticamente, a fim de explorar o valor da aplicação da hidrografia por RM do ouvido interno no implante coclear. RESULTADOS Centro e quarenta e seis pacientes (292 ouvidos) foram examinados. Dentre esses pacientes, 13 foram diagnosticados com aquedutos vestibulares anormais (20 ouvidos) pela hidrografia por RM, enquanto cinco pacientes foram diagnosticados com esta doença pelo CT; 15 pacientes foram diagnosticados com malformação do ouvido interno (19 ouvidos) pela hidrografia por RM, enquanto 11 pacientes foram diagnosticados por CT (quatro foram diagnosticados erroneamente); cinco pacientes foram diagnosticados com estenose de canal acústico interno (oito ouvidos) pela hidrografia por RM, enquanto dois pacientes foram diagnosticados por CT (três foram diagnosticados erroneamente); e quatro pacientes foram diagnosticados com fibrose coclear (cinco ouvidos) pela hidrografia por RM, enquanto quatro foram diagnosticados por CT (quatro ouvidos). A taxa correta de diagnóstico foi de 77,40% (113/146) com base no CT, enquanto a taxa foi de 93,84% (137/146) com base na hidrografia por RM. CONCLUSÕES A técnica de imagem da hidrografia por RM pode ser aplicada à avaliação pré-operatória do implante coclear, que pode fornecer informações anatômicas precisas e confiáveis sobre o labirinto membranoso interno e os nervos no canal acústico interno, além de uma base exata para o diagnóstico da fibrose coclear e do desenvolvimento do nervo. Isso tem um significado orientador para a seleção de esquemas de tratamento.

ABSTRACT

Most cases of sensorineural deafness are caused by degeneration of hair cells. Although stem/progenitor cell therapy is becoming a promising treatment strategy in a variety of organ systems, cell engraftment in the adult mammalian cochlea has not yet been demonstrated. In this study, we generated human otic progenitor cells (hOPCs) from induced pluripotent stem cells (iPSCs) in vitro and identified these cells by the expression of known otic markers. We showed successful cell transplantation of iPSC-derived-hOPCs in an in vivo adult guinea pig model of ototoxicity. The delivered hOPCs migrated throughout the cochlea, engrafted in non-sensory regions, and survived up to 4 weeks post-transplantation. Some of the engrafted hOPCs responded to environmental cues within the cochlear sensory epithelium and displayed molecular features of early sensory differentiation. We confirmed these results with hair cell progenitors derived from Atoh1-GFP mice as donor cells. These mouse otic progenitors transplanted using the same in vivo delivery system migrated into damaged cochlear sensory epithelium and adopted a partial sensory cell fate. This is the first report of the survival and differentiation of hOPCs in ototoxic-injured mature cochlear epithelium, and it should stimulate further research into cell-based therapies for treatment of deafness.

ABSTRACT

The understanding of the structure of the spatial variability of soil surface hydraulic properties on steep slopes is important for modeling infiltration and runoff processes. The objective of this study was to investigate the spatial variability of these properties on a steep slope of the Loess Plateau in northwest China. A 9600 m² area was systematically sampled in a grid of 106 points spaced 10 m x 10 m. Hydraulic properties were determined with a disc infiltrometer under multiple pressure heads (-15, -9, -6, -3, 0 cm) at each sample point. Classical and geo-statistical methods were used for data analysis. The results indicated that the variation of Gardner's a and hydraulic conductivities at all applied pressure heads was moderate and the heterogeneity for hydraulic conductivities increased as the applied pressure head increased. Along the slope, hydraulic conductivities generally decreased downwards, while the Gardner's a fluctuated slightly. The Gardner's a of the shaded aspect of the slope was greater than that of the sunny aspect. The hydraulic conductivities of the shaded aspect were greater at higher pressure heads as compared to the sunny aspect, but lower than those of the sunny aspect at lower pressure heads. Correlation analysis showed a negative correlation between hydraulic conductivity and soil organic matter and clay ( 0.01 mm) contents. Hydraulic conductivities at pressure heads of -3, -6, -9, -15 cm varied across the slope and their spatial dependence increased as the pressure head declined. The heterogeneity and spatial dependence of hydraulic properties were larger for the areas with shaded aspect as compared to the sunny aspect, however, as pressure decreased they showed a progressively increasing spatial structure, and their spatial structure behaved increasingly similar in both the shaded and sunny aspects.


A compreensão da estrutura da variabilidade especial das propriedades hidráulicas do solo de encostas íngremes é importante na modelagem dos processos de infiltração e de escoamento superficial da água. O objetivo deste estudo foi investigar a variabilidade destas propriedades em uma encosta íngreme do "Loess Plateau" do noroeste da China. Uma área de 9600 m² foi sistematicamente amostrada em um grid de 106 pontos espaçados de 10 m x 10 m. As propriedades hídricas foram determinadas com um infiltrômetro de disco operando sob múltiplas cargas hidráulicas (-15, -9, -6, -3, 0 cm) em cada ponto de observação. Métodos clássicos e geoestatísticos foram empregados na análise dos dados. Os resultados mostraram que a variação do índice a de Gardner e das condutividades hidráulicas em todas as cargas aplicadas foi moderada e que a heterogeneidade das condutividades hidráulicas aumentou à medida que a carga hidráulica aumentava. Na direção do declive, as condutividades geralmente decresceram a jusante, enquanto os a de Gardner flutuavam moderadamente. Os a de Gardner das encostas sombreadas foram maiores do que os das encostas ensolaradas. As condutividades hidráulicas das encostas sombreadas foram maiores para cargas hidráulicas maiores em comparação com as encostas ensolaradas, mas menores que as das encostas ensolaradas para cargas hidráulicas menores. A análise de correlação mostrou uma correlação negativa entre a condutividade hidráulica e os teores de matéria orgânica e argila ( 0.01 mm). As condutividades hidráulicas sob cargas de -3, -6, -9, -15 cm variaram em toda encosta e suas dependências espaciais aumentaram à medida que a carga diminuía. Surface soil moisture exhibits an important variability in terms of spatial and temporal domains, which may result in critical uncertainties for agricultural water management. The purposes of this study were (i) to characterize the temporal dynamics and stability of the spatial variability of the surface 0-6 cm soil water content on a hill-slope; (ii) to investigate issues related to soil moisture conditions including dominating factors on soil moisture and to the estimation of the mean . During a period of more than one month was measured on thirteen days by Frequency Domain Reflectometry using a 10 x 10 m grid of measurement points covering a 60 x 280 m domain within a hill-slope of the Loess Plateau in China. Soil water content exhibited a moderate variability for each measurement date, and the correlation length (l) for ranged from 8.4 to 27.7 m. With the soil becoming drier, l decreased, the CV% and the sampling number for accurate mean estimation increased. Aspect, elevation, organic matter content, clay content, and bulk density were the main influencing factors, whose extent of influence weakened with decreasing . Based on time stability analysis and on the correlation of mean relative difference of with the relative difference of dominating factors, mean q values were well estimated, with a better accuracy under wetter conditions.


A umidade da camada superficial do solo apresenta uma variabilidade importante nos domínios espacial e temporal, que pode levar a incertezas críticas para o manejo agrícola da água. Os objetivos deste estudo incluíram: (i) a caracterização da dinâmica temporal e da estabilidade da variabilidade espacial da umidade da camada 0-6 cm em uma encosta em declive; (ii) investigação de parâmetros ligados às condições de umidade, incluindo fatores dominantes da umidade na estimativa de um valor médio de q. Durante um período de mais de um mês, q foi medida em 13 dias com auxílio da técnica da Refletometria do Domínio da Frequência, em uma grade de 10 x 10 cm, cobrindo uma área de 60 x 280 m, situada em uma encosta do "Loess Plateau" da China. A umidade do solo apresentou variabilidade moderada em cada data de medida e o alcance da correlação (l) para q variou de 8,4 a 27,7 m. Com o solo mais seco, l decresceu, o CV% e o número de amostras necessário para obter uma média precisa, aumentaram. O aspecto, a elevação, o conteúdo de matéria orgânica e a densidade do solo foram os principais fatores interferentes na umidade, cuja intensidade de influência diminuiu com a umidade. Baseado na análise de estabilidade temporal e na correlação da diferença relativa média de com a diferença relativa média dos fatores dominantes, valores médios de q puderam ser mais bem estimados, com maior precisão sobre condições mais úmidas.

As condutividades hidráulicas das encostas sombreadas foram maiores para cargas hidráulicas maiores em comparação com as encostas ensolaradas, mas menores que as das encostas ensolaradas para cargas hidráulicas menores. A análise de correlação mostrou uma correlação negativa entre a condutividade hidráulica e os teores de matéria orgânica e argila ( 0.01 mm).
