ABSTRACT

OBJECTIVE: The Journal of Esthetic and Restorative Dentistry (JERD) stands out as one of the most prominent international journals publishing research in esthetic dentistry. This study analyzed articles published by JERD since the year 2000 through bibliometric analysis. METHODOLOGY: The search was conducted in January 2024 using Scopus. The following data were extracted from the articles: citation count, year, language, access type, funding agency, study design, theme (general and specific), country, institution, authors, and keywords. The VOSviewer software was used to generate collaborative network maps among the data. Dimensions were consulted to measure altmetric data. Google Trends was used to investigate the global popularity of JERD research. RESULTS: A total of 1394 articles were included in this analysis. Citation count ranged from 0 to 625 (average: 16.9). Articles were published between 2000 and 2023. Laboratory studies were more prevalent (n = 850), with the most investigated general theme being restorative procedures (n = 882), and the highlighted specific theme being the use of composite resin (n = 327). The United States had the highest number of articles (n = 640), with the diverse distribution among other countries. The most common keyword was "cad/cam" (n = 63). VOSviewer demonstrated high collaboration among countries. Intense mentions were identified primarily on Facebook. According to Google Trends, Egypt was the country that searched for JERD the most. CONCLUSIONS: JERD exhibited significant growth in the number of published articles and their diversity by topics, types, origin (country), number of citations, and impact factor. CLINICAL SIGNIFICANCE: The JERD is a journal that publishes studies influencing clinical practice. Identifying the key characteristics of this journal is essential for charting future paths.

Introdução:O aprimoramento das resinas compostas nos últimosanos em associação com a difusão de informações nas redes sociais tornou as facetas diretas tratamentos populares na dentística restauradora. No entanto, são procedimentos que exigem ampla destreza manual e conhecimento técnico. O fluxo digital através doescaneamento, enceramento digital e prototipagem 3D para construção de guias tem se tornado uma excelente alternativa para aumentar a previsibilidade e aumentar a longevidade destes trabalhos. Objetivo:Descrever o protocolo de confecção de facetas diretas em resina composta, através de um relato de caso, utilizando como auxílio o planejamento digital para confecção de modelo 3D, guia de silicone e paredes palatinas. Descrição do Caso:Paciente do gênero masculino, 43 anos, queixava-se do formato dos seus dentes. Ao exame clínico percebeu-se desgaste dental nos incisivos centrais e linha do sorriso levemente invertida. Após duas sessões de clareamento de consultório com Peróxido de hidrogênio (35%) e mockup direto com resina composta, foi realizada a moldagem e escaneamento do modelo de gesso no laboratório. O enceramento digital foi aprovado, o modelo 3D foi impresso para confecção da guia de silicone. Com auxílio da guia foram executadas facetas diretas nos elementos 13, 12, 11, 21, 22 e 23. Conclusão:O fluxo digital pode ser uma alternativa viável para minimizar as falhas na confecção de facetas diretas em resina composta (AU).


Introduction:The improvement of composite resins in recent years, together with information disseminated on social media, has made direct veneers popular treatments in restorative dentistry. However, these procedures require significant manual dexterity and technical knowledge. Digital work flow using scanning, digital wax-up and 3D prototyping for the construction of guides has become an excellent alternative to increase predictability and the longevity of these procedures. Objective:Describe the manufacturing protocol for direct composite resin veneers, using a case report and digital to construct the 3D model, silicone guide and palatine walls. Case description:Male patient, 43 years old, complained of the shape of his teeth. Clinical examination revealed tooth wear on the central incisors and a slightly inverted smile line. After two whitening sessions with hydroigen peroxide (35%) and direct mockup with composite resin, the plaster model was molded and scanned in the laboratory. Digital wax-up was approved, and the 3D model was printed to manufacture the silicone guide. With the help of the guide, the direct veneers were applied to elements 13, 12, 11, 21, 22 and 23.Conclusions:Digital flow may be a feasible alternative to minimize manufacturing flaws in direct composite resin veneers (AU).


Introducción: La mejora de las resinas compuestas en los últimos años, y la difusión de información en las redes sociales, ha popularizado las facetas directas en los tratamientos en odontología restauradora. Sin embargo, son procedimientos que requieren demasiado destreza manual y conocimientos técnicos. El flujo digital usando escaneo, encerado digital y prototipado 3D para la construcción de guías se ha convertido en una excelente alternativa para aumentar la previsibilidad y la longevidad de estos procedimientos. Objetivo: Describir el protocolo para la realización de carillas directas en resina compuesta, a través de un reporte de caso, utilizando el planeo digital como ayuda para la realización de un modelo 3D, guía de silicona y paredes palatinas. Descripción del caso: Paciente masculino, 43 años, se quejó de la forma de sus dientes. El examen clínico reveló desgaste dental en los incisivos centrales y una línea de sonrisa levemente invertida. Después de dos sesiones de blanqueamiento en consultorio con peróxido de hidrógeno (35%) y maqueta directa con resina compuesta, el modelo de yeso fue moldeado y escaneado en el laboratorio. El encerado digital fue aprovado, el modelo 3D fue impreso para hacer la guía de silicona. Con la ayuda de la guía se realizaron carillas directas en los elementos 13, 12, 11, 21, 22 y 23. Conclusiones: El fluxo digital puede ser una alternativa viable para minimizar fallas en la fabricación de carillas directas en resina compuesta (AU).

ABSTRACT

Minimally invasive dentistry is a considered process that requires the clinician to be prepared with the ideal sequence and the tools needed. This report describes a well-planned ultraconservative approach using only two ceramic laminate veneers for the maxillary central incisors to significantly improve the patient's overall smile. A 30-year-old female presented with the chief complaints of having diastemas between the central and lateral incisors as well as incisal wear. Diagnostic wax-up and mock-up were performed, and the patient approved the minimally invasive treatment with veneers only for central incisors. A reduction guide aided the conservative tooth preparations, and hand-crafted feldspathic veneers were bonded under total isolation with a rubber dam. The two final conservative veneers significantly improved the smile and fulfilled the patient's expectations. Following proper planning and sequencing, predictable outcomes were obtained and fulfilled the patient's esthetic demands. Minimally invasive restorative dentistry with only two single veneers can impact the entire smile frame. Overtreatment in the esthetic zone is unnecessary to meet a patient's esthetic expectations.

ABSTRACT

Background: Photobiomodulation (PBM) involves laser therapy utilized in medical sciences to modulate biological processes acting as a palliative and immune response-enhancing treatment. This study conducts a comprehensive bibliometric analysis to explore current trends in PBM-related scientific production, encompassing publications, citations, impact, keywords and clusters. Additionally, it aims to predict future research trends in this domain. Methods: The data for this quantitative and qualitative bibliometric analysis were obtained from 608 scientific documents retrieved in November 2022, with 123 sourced from Web of Science and 485 from Scopus, Utilizing Excel, the data was processed in Excel to extract essencial information. Productivity and impact were evaluated for eligibility, and VOSviewer aided in determining associativity for the bibliometric analysis. Results: The findings of this study demostrate that the scientific production related to PBM adheres to a growth power law, exhibiting characteristics of both exponential and linear phases. Notably, recent research trends emphasize critical concepts such as laser therapy, orthodontics, and dental pulp stem cells. Particularly significant is the burgeoning interest in utilizing PBM within dentistry as a complementary alternative to existing protocols. Conclusions: PBM stands as a promising laser therapy within medical applications. Through a detailed bibliometric analysis, this study underscores the increasing significance of PBM, especially within the realm of dental treatments. These insights offer a glimpse into the evolving landscape of PBM research and provide valuable guidance for potential future directions of study.

OBJECTIVE: This article provides an update on the direct-indirect composite veneer technique. CLINICAL CONSIDERATIONS: Composite veneers have long been used as a conservative and esthetic treatment option for anterior teeth. While they are generally performed using a direct technique, there has been renewed interest in the direct-indirect composite veneer technique because of its advantages and broad indications for restoration of tooth color and morphology. In the direct-indirect composite veneer technique, the selected composites are initially applied on the tooth using a layering approach, without any bonding agent, sculpted to a primary anatomic form with slight excess, and light-cured. The partially polymerized veneer is then removed from the tooth, heat-tempered, and finished to final anatomy and processed extra-orally before being luted. Advantages of this technique include enhanced physical and mechanical properties afforded by the tempering process, unrivaled marginal adaptation, enhanced finishing and polishing, and the ability to try-in the veneer before luting, enabling a shade verification and modulation process that is not possible with the direct technique. The direct-indirect approach also affords enhanced gingival health and patient comfort. CONCLUSION: This article reviews the direct-indirect composite veneer technique, and outlines critical steps and tips for clinical success. CLINICAL SIGNIFICANCE: The direct-indirect technique for composite veneers combines advantages of the direct composite placement technique with those of the indirect veneer technique, including operator control, single-visit fabrication and delivery, increased material properties, and excellent esthetics.

ABSTRACT

Treatment of the anterior teeth is challenging. Computer resources and software are available to facilitate a digital smile approach, thus allowing the dentist to plan and predict an esthetic result for a patient, particularly when an integrated multidisciplinary approach is necessary. Digital smile design has emerged as a tool in cosmetic dentistry that helps both professionals and the patient to visualize the result, in addition to facilitating a discussion between the two before establishing a final treatment decision. Here, we present a case involving a 27-year-old female who underwent multidisciplinary treatment initiated by digital planning using specific software, namely Cara Smile. The patient presented with complaints of maxillary tooth malalignment, staining, and fractures. To obtain the correct width-to-height ratio, a simulated gingival procedure was performed in the software. Gingivectomy was performed using digital simulation, and 90 days after surgery, cast and wax-up models maintaining the previously planned width-to-height ratio for the maxillary anterior teeth were made. Esthetic crowns and veneers were fabricated using the digital planning protocol. Therefore, Cara Smile is useful to facilitate the diagnosis through digital photos, improve communication between the various professionals involved in treatment, and guide the predictability of treatment and patient acceptance.

BACKGROUND: The use of hybrid-light (HL) during in-office teeth bleaching can provide faster and reliable results after a single bleaching session, especially if associated with previous acid etching. The present in vivo, interventional, split-mouth, triple-blinded and randomized study evaluated the efficiency of a HL, with and without prior enamel acid etching, assessing the gel application time, degree of color change, sensitivity and treatment stability up to 12 months. METHODS: Thirty-four patients were selected and randomly divided into four groups. The color was evaluated using a spectrophotometer with the CIELab color system, and a VAS questionnaire was used to evaluate the sensitivity. Both upper and lower hemiarches (GI and III) received enamel acid etching with 37% phosphoric acid for 15 s prior to the application of 35% hydrogen peroxide gel (HP). For GI and II, HP was applied on both dental arches for 3-4 consecutive applications and activated by HL (2 × 3' activations with 1' interval); and 3 × 15' (HP) for GIII and IV without HL. Patients returned in 24 h, 1 week, 1-, 6- and 12-months intervals. The clinical operator was different from the clinical measurements operator. RESULTS: After 3-way ANOVA, Tukey's, Wilcoxon and Mann-Whitney tests, all with p < 0.05, no statistical differences were observed between the groups regarding color change (ΔE) and sensitivity in all evaluations. The lowest gel application time was observed for GII (acid etch + HL), followed by GI (HL). CONCLUSIONS: The present study's results reveal the advantages of HL associated with acid etching prior to in office bleaching.

ABSTRACT

In this case report, we share a conservative and simple restorative management approach for a relatively complex esthetic situation on an adult patient with an ankylosed maxillary central incisor in infra-occlusion. A 37-year-old healthy female patient presented for a consultation with a chief complaint of an unappealing smile. The intraoral exam revealed tooth #8 (FDI 11) was in obvious and significant infra-occlusion due to altered eruption. Periapical radiographs of this area were non-contributory and all teeth responded within normal limits to pulp vitality tests. After review of several treatment options, the patient opted for a conservative restorative approach in which a partial ceramic veneer was planned for the ankylosed tooth, while composite resin was used on the contralateral central incisor to achieve a symmetric and pleasant result. CLINICAL SIGNIFICANCE: A relatively complex clinical problem was satisfactorily resolved with a very conservative option in which composite resin and a ceramic veneer were used to restore an asymmetric smile.

Procedimentos como aumento de coroa clínica e/ou laminados cerâmicos são utilizados para reabilitação de pacientes que possuem coroa clínica curta, espaços interdentais na região anterior e situações que prejudiquem a estética do sorriso. O objetivo deste trabalho é apresentar uma sequência clínica para a reabilitação estética na região anterior por meio de aumento de coroa clínica associado a laminados cerâmicos, utilizando um protocolo previsível para o planejamento reabilitador. Foi realizada a anamnese, a confecção de modelos de gesso e a sequência fotográfica para o planejamento digital. Foram utilizadas as ferramentas do Digital Smile Design e transferido para o modelo de gesso por meio do enceramento progressivo. Guias de silicone para prova em boca foram confeccionados, e após a aprovação do paciente foi realizado o aumento de coroa clínica seguindo o mock-up como guia cirúrgico. Após a cicatrização, novos modelos de gesso foram confeccionados para o planejamento dos laminados cerâmicos. O mock-up foi testado novamente em boca e, após a aprovação do paciente, os mínimos desgastes necessários foram realizados. A arcada superior foi moldada com silicone de adição para obtenção de modelos de trabalho e confecção dos laminados cerâmicos, que foram cimentados após a aprovação do paciente. Este trabalho apresenta uma sequência simples para a reabilitação estética anterior, utilizando o planejamento digital e o aumento de coroa clínica associados com laminados cerâmicos.


Procedures such as clinical crown augmentation and ceramic laminate products are used for rehabilitation of patients who have short clinical crown, interdental spaces in the anterior region, or are dissatisfied with the aesthetics of their smile. The objective of this clinical work is to present an esthetic rehabilitation in the anterior region by means of increase of clinical crown surgery and ceramic laminates, using a preventive protocol for the planning of the clinical case. Patient anamnesis, dental casts, and photo analysis were made for digital planning. The DSD tools were transferred to the dental casts through the progressive waxing technique. Also, silicone keys were fabricated, and upon patient approval, clinical crown lengthening was performed with the mock-up as the surgical guide. After tissue healing, new dental casts were made to plan the ceramic laminates. Again, the mock-up was tried in the mouth and after patient approval, minimal tooth preparations were made. An impression of the maxillary arch was made with addition silicone to obtain the working casts and to fabricate the ceramic restorations, which were delivered and cemented after patient approval. This paper presents a simple and easy sequence to rehabilitate the esthetic aspect in the anterior upper region using the digital planning and crown lengthening associated to the ceramic laminates.

INTRODUÇÃO: O fator estético é um dos critérios utilizados na definição do sucesso de reabilitações implantossuportadas. Os critérios estéticos dos profissionais para essa característica variam a depender da sua área de especialização e vivência clínica. O objetivo desse trabalho é mensurar a influência do condicionamento gengival no resultado estético, bem como avaliar a percepção de diferentes especialidades no resultado dessa intervenção em restaurações provisórias unitárias sobre implante na região do sorriso. METODOLOGIA: Dez pacientes foram submetidos a cirurgia de instalação de implante com conexão cônica. Todos os pacientes receberam próteses provisórias sobre implante em resina acrílica. O processo de condicionamento gengival foi realizado pela técnica não-cirúrgica de pressão gradual. Fotografias da região reabilitada foram realizadas na instalação da coroa provisória e na última sessão de condicionamento gengival. Três avaliadores especialistas e um clínico geral, mascarados com relação a fase da restauração provisória, aplicaram o Pink Esthetic Score (PES) a partir dos critérios observados nas fotografias. Cada avaliador repetiu o PES três vezes num espaço de 3 dias entre as avaliações, nas quais a sequência de fotografias era diferente. Os resultados foram avaliados estatisticamente utilizando o Coeficiente de Correlação Intraclasse (CCI), o teste não-paramétrico de Wilcoxon e o teste de qui-quadrado de Pearson. RESULTADOS: Os avaliadores tiveram uma concordância média para os valores de PES e todos os critérios constituintes (entre 40% e 75%), no qual o clínico geral apresentou maior grau de discordância em todos os parâmetros. O protesista foi o mais criterioso para as avaliações enquanto o clínico geral e o periodontista apresentaram medianas semelhantes para os escores antes e depois. Existiu diferença significativa entre a avaliação do PES antes e depois do condicionamento gengival para todos os avaliadores (p<0,05), indicando que houve melhora na estética após a realização do condicionamento gengival. CONCLUSÃO: O condicionamento gengival influencia positivamente no escore do PES e de seus parâmetros constituintes e essa diferença pode ser notada por todos os avaliadores (AU).


BACKGROUND: The esthetic outcome is one of the criteria used to build a definition of success in implant-supported restorations. The esthetic criteria of professionals for these characteristics vary depending on their area of specialization and clinical experience. The aim of this study is to measure the influence of soft tissue conditioning on esthetic outcome as well as to evaluate the perception of different specialties in the result of this intervention in provisional crowns on single implants in the smile zone. METHODS: Ten patients underwent an implant placement surgery to insert a morse tapered implant. All patients received an acrylic resin implantsupported provisional crown. The soft tissue conditioning process was performed with a non-surgical technique of gradual pressure. Photographs of the rehabilitated region were taken at the first provisional crown insertion and at the last session of soft tissue management. Three specialists and a general practitioner were selected as blinded examiners and applied the Pink Esthetic Score (PES) based on the criteria observed in the photographs. Each examiner repeated the PES three times within three days between analyses. Each evaluation had a different sequence of photos. The results were analyzed using the Intraclass Correlation Coefficient (ICC), the nonparametric Wilcoxon signed-rank test, and the Pearson's chi-square test. RESULTS: The evaluators had a mean agreement for PES values and all the constituent criteria (between 40% and 75%), in which the general practitioner showed the greater degree of disagreement in all parameters. The prosthodontist gave the lowest scores for the evaluations while the general practitioner and the periodontist presented similar results (highest scores) for the before and after scores. There was a significant difference between the evaluation of PES before and after gingival conditioning for all evaluators (p <0.05), indicating that there was an improvement in aesthetics after gingival conditioning. CONCLUSION: Soft tissue conditioning positively influences the PES score and its constituent parameters and this difference could be noticed by all the evaluators (AU).

ABSTRACT

Diastemas among maxillary incisors and gingival contour disharmony are common findings among patients in dental practice. Ceramic veneers are indicated for esthetic rehabilitation of anterior diastemas due their predictable results, optical characteristics, fracture resistance and tooth structure conservation. However, because it is a friable material and have a brittle behavior, fractures occurrences are related to trauma, oclusal overload, parafunctional habits and material fatigue. This article describes 30 months follow-up of an esthetic and functional rehabilitation diastemas closure using feldspathic veneers associated with periodontal surgery and a ceramic repair with composite resin. Gingivectomy and frenectomy needs were found and the surgical procedures performed guided by new anatomic aspects of the crowns. Mock-up was performed after waxing and reverse planning. All anterior teeth underwent minimally invasive preparation. Feldspathic ceramic veneers were made, tried using try in paste and luted with light-cure resin cement. After 24 months, a fracture occurred on the right maxillary canine veneer. The ceramic restoration repair was performed with nano-hybrid composite resin, after the conditioning with 5% hydrofluoric acid, 37% phosphoric acid and silane couple agent. The combination between ceramic veneers and gingivectomy enables to obtain conservative treatments and esthetic success. After six months of the repair, resulting 30 months of follow-up, the anterior restorations were aesthetically and functionally satisfactory.


Presença de diastemas entre os incisivos superiores e desarmonia do contorno gengival são achados comuns entre os pacientes atendidos na rotina clínica. Facetas cerâmicas são indicadas para reabilitação estética de diastemas anteriores devido a previsibilidade de resultados, características óticas, resistências à fratura e conservação de estrutura dentária. Entretanto, por este ser um material frágil e apresentar comportamento friável, fraturas podem estar relacionadas a episódios de trauma, sobrecarga oclusal, hábitos parafuncionais e fadiga do material. Este artigo descreve um acompanhamento clínico de 30 meses de reabilitação estética e funcional de fechamento de diastema utilizando facetas minimamente invasivas confeccionadas com cerâmicas feldspáticas associadas à cirurgia periodontal e necessidade de reparo da cerâmica com resina composta. A indicação de plastia gengival e frenectomia foram constatadas e o procedimento cirúrgico guiado pelo planejamento da nova anatomia das coroas dentárias. Todos os dentes anteriores superiores foram minimamente preparados. O mock-up foi realizado depois do enceramento e do planejamento reverso. Facetas em cerâmicas feldspáticas foram confeccionadas, a cor do cimento resinoso selecionada com pastas testes e a cimentação concluída com cimento de polimerização exclusiva física. Depois de 24 meses de acompanhamento, ocorreu fratura da faceta do canino superior direito. O reparo da restauração cerâmica foi realizado com resina nanohibrida, depois da cerâmica condicionada com ácido fluorídrico 5%, ácido fosfórico 37% e aplicação de agente de união silano. A combinação entre facetas cerâmicas e cirurgia de plastia gengival permite alcançar tratamento conservador e sucesso estético. Posteriormente seis meses do reparo, resultando em 30 meses de acompanhamento clínico, as restaurações anteriores permaneciam estética e funcionalmente satisfatórias.

ABSTRACT

O presente relato de caso discutiu as vantagens da implantação e temporização imediata de elementos unitários em áreas estéticas, sem abertura total de retalho, com a utilização de enxerto conjuntivo subepitelial e preenchimento alveolar com biomaterial. O acompanhamento a longo prazo desta técnica (nove meses) mostrou que essa proposta favoreceu a manutenção da arquitetura tecidual, permitindo excelência na finalização protética.


This case report discusses the advantages of immediate implant placement and temporization of several elements in the esthetic zone without complete flap opening, and still using the subepithelial connective tissue graft and the alveolar filling with a biomaterial. The long-term results (nine months) demonstrated a favorable response at the tissue architecture, with excellent prosthetic delivery.

ABSTRACT

OBJECTIVES: To evaluate the efficacy of experimental proposals of desensitizing agents during tooth bleaching. METHODS: 140 participants without tooth sensitivity (TS) received 16% carbamide peroxide (14 days-04 h each) (T1) or 35% hydrogen peroxide (single session-45 min) (T2). Participants used concomitantly (10 per group): desensitizing dentifrices (D1-experimental bioactive glass-ceramic; D2-commercial potassium nitrate; D3-commercial calcium and sodium phosphosilicate) in-home, daily and, desensitizing pastes (D4-experimental bioactive glass-ceramic; D5-experimental Bioglass type 45S5; D6-commercial calcium phosphate), in-office, immediately after the treatment and more 4 times. Participants in the control group did not use any desensitizing agent. We assessed TS with Visual Analogue Scale. Assessment point 1 was immediately after the first participant's exposure to the treatments; and points 2, 3, 4, and 5 were every 72 h along the period of the study. Two-way ANOVA (considering time and desensitizing as factors) and post-hoc Tukey test (α=0.05) analyzed the data. RESULTS: In the control group treated with 35% hydrogen peroxide, TS increased significantly on assessment points 1 and 2. The participants who used a 5% potassium nitrate dentifrice and in-office experimental pastes did not experience TS because of the 35% in-office bleaching treatment. CONCLUSIONS: TS caused by 35% hydrogen peroxide in-office tooth bleaching was controlled by experimental products prepared as pastes D4-experimental bioactive glass-ceramic and D5-experimental Bioglass type 45S5, but not by D1-experimental dentifrice containing bioactive glass-ceramic. CLINICAL SIGNIFICANCE: There is no a gold standard protocol for TS caused by tooth bleaching. The study of novel desensitizing agents that can obliterate the dentinal tubules in a faster-acting and long-lasting way may help meet this clinical need.

ABSTRACT

Due to the esthetic necessity required nowadays, the multidisciplinary treatment became a fundamental step in the restoration success. When the patient exhibits dental agenesis of one or more elements, he can show difficulty in social interactions. The age of the patient is a limiting factor to esthetic procedures, however, it should be evaluated as a real indicative with each case. The utilization of semi-direct restorations is a viable option due the cost, esthetic and improvement of physical and mechanical properties. The purpose of this paper is to present a case detailing the confection and cementation of anterior semi-direct restorations aimed at an anatomic reestablishment associated with integrated treatment with periodontics and orthodontics.

ABSTRACT

O objetivo deste artigo foi comparar as técnicas cirúrgicas convencionais com as mesmas técnicas associadas ao uso de medula óssea autógena. Para tal, 21 pacientes foram submetidos à cirurgia de regeneração óssea, as quais foram subdivididas em dois tipos de procedimentos (Procedimento 1 e Procedimento 2). No Procedimento 1, os pacientes tiveram dez dentes extraídos da forma menos traumática possível. Seis pacientes tiveram seus alvéolos preenchidos com medula óssea autóloga, e nos outros cinco pacientes nada foi enxertado. No Procedimento 2, foi realizada a enxertia em bloco com osso córtico medular homógeno fresco congelado. Cinco pacientes tiveram os enxertos impregnados com medula óssea autóloga, e os outros cinco pacientes tiveram os blocos ósseos fixados, sem associação alguma. Após um período de seis meses de cicatrização, implantes dentários foram instalados e uma biópsia foi realizada, o que proporcionou confecção de lâminas histológicas. Os pacientes também foram acompanhados pelos exames de tomografia computadorizada feixe cônico. A associação da medula óssea autóloga, tanto no Procedimento 1 como no Procedimento 2, resultou na obtenção de um maior volume ósseo, sendo que no Procedimento 2 notou-se uma melhor qualidade óssea.


The aim of this study was to compare surgical techniques for bone regeneration with the use of autologous marrow bone aspirate. For this, 21 patients were divided in two separate procedures. On the first procedure, ten teeth were atraumatically extacted: six fresh sockets receive autologous bone marrow and five sockets were filled with blood clot. On the second procedure, a cortico-cancellous, fresh-frozen, homogenous bone block was used for bone augmentation in five patients, while the other five received the same therapy but added to bone marrow aspirate. After a six-month healing period, dental implants were placed and a biopsy performed for histomorphometric analysis. Also, CBCT analyses were performed. The use of bone marrow aspirate resulted in better bone volume for both aspects, being that better bone qualities were observed in the second procedure.

ABSTRACT

OBJECTIVE: To assess differences in how orthodontists and laypersons perceive a reduction in dentogingival display on smiling. METHODS: Sixty examiners from both genders (30 laypersons and 30 orthodontists) evaluated photographs of spontaneous smiles of two subjects , one male and one female. Based on the original images, smile height was modified by means of an image manipulation software program. The examiners assigned scores ranging from 0 to 10, according to the level of pleasantness. Method reproducibility was examined using the Wilcoxon test, while the Friedman and Wilcoxon tests (p < 0.05) were employed to observe intra- and interexaminer differences, respectively. RESULTS: No differences were found between the groups of examiners - in terms of esthetics - in response to changes in smile height of both genders . However,men smile had lower acceptability than the women smile. A mild reduction in dentogingival display on smiling (2 mm) was not perceived by either laypersons or orthodontists (p > 0.05). CONCLUSIONS: women smiles achieved higher scores than men smiles however, samples involving a larger number of subjects in each group are required to ensure whether or not this finding is linked to the subjects gender.


OBJETIVO: avaliar a diferença na percepção de ortodontistas e leigos quanto à redução da exposição dentogengival no sorriso. MÉTODOS: no total, 60 avaliadores de ambos os sexos (30 leigos e 30 ortodontistas) avaliaram fotografias do sorriso espontâneo de dois indivíduos, um do sexo masculino e um do feminino. A partir das imagens originais, a altura do sorriso foi modificada usando-se um programa de manipulação de imagens. Os examinadores emitiram notas de 0 a 10, conforme o nível de agradabilidade. A reprodutibilidade do método foi examinada através do teste de Wilcoxon, enquanto os testes de Friedman e Wilcoxon (P<0,05) foram utilizados para observar as diferenças intra e interexaminadores, respectivamente. RESULTADOS: os resultados demonstraram não haver diferença entre os grupos de avaliadores com relação à estética quando a altura de ambos os sorrisos foi modificada. Entretanto, o sorriso do indivíduo do sexo masculino teve menor aceitabilidade do que o sorriso feminino. Uma suave redução na exposição dentogengival no sorriso (2mm) não foi percebida por leigos ou ortodontistas (p>0,05). CONCLUSÃO: o sorriso do indivíduo do sexo feminino recebeu notas mais altas do que o do masculino; entretanto, amostras envolvendo um maior número de indivíduos em cada grupo são necessárias para confirmar se a observação estaria relacionada ao sexo do indivíduo examinado.

ABSTRACT

Este trabalho avaliou os resultados estéticos de pacientes Classe II submetidos à terapia ortodôntica corretiva. Selecionaram-se, aleatoriamente, sete pacientes do gênero feminino, leucodermas, submetidas a tratamento ortodôntico corretivo com extração de pelo menos dois pré-molares e com início do tratamento aos 16 até 26 anos. Cada paciente foi entrevistada e as falas foram gravadas e posteriormente transcritas e analisadas. Modelos de estudo, teleradiografias e fotografias constantes nos prontuários foram consultados para se avaliar cinco parâmetros quantitativos: contorno facial, ângulo nasolabial, proporção áurea inter-incisivos, linha média facial e dentária e silhueta incisal no sorriso. Os resultados obtidos revelaram que o tratamento ortodôntico corretivo melhorou a estética facial, alterando medidas do tecido mole da face, o sorriso e as relações pessoais. Concluiu-se que as pesquisas qualitativas e quantitativas são complementares, pois uma análise de resultados, baseada somente em um parâmetro, pode mascarar o resultado real e revelar aspectos parciais. A análise qualitativa indica que ângulos e proporções faciais diferentes do que é proposto cientificamente como estético não interferem com os resultados do tratamento, contanto que a percepção facial dos sujeitos envolvidos vá ao encontro dos padrões de normalidade aceitos por estes e estabelecidos pela sociedade.


Aesthetic evaluation of patients submitted to orthodontic treatment. The aim of this study was to investigate the aesthetic results of orthodontic treatment in Class II individuals. Seven Caucasian females were randomly selected, subjected to corrective orthodontic treatment with extraction of at least two pre-molars and initiation of treatment between 16 and 26 years of age. All patients were interviewed; their speeches were recorded and then transcribed and analyzed. Plaster models, lateral radiographs and photographs contained in records were consulted to assess five quantitative parameters: facial contour, nasolabial angle, incisors golden ratio, facial and dental midline, and smile line to incisal edge. The corrective orthodontic treatment improves facial aesthetics, changing facial soft tissue measures, smile and personal relationships. It was concluded that the qualitative and quantitative research are complementary, because an analysis of results based only on one parameter can mask the actual outcome and reveal aspects partially. The qualitative analysis indicates that different facial angles and proportions of what is proposed as scientifically aesthetic does not interfere with the results of treatment, provided that the facial perception of the subjects involved meet the standards of normality and accepted by them and those set by society.

ABSTRACT

La remodelación cosmética de los dientes naturales es una modalidad de tratamiento con beneficios estéticos que puede ser considerada económica y conservadora. No obstante, demanda del profesional no solo restaurar una sonrisa armoniosa, sino también, al mismo tiempo preocuparse con el aspecto funcional del tratamiento. Además, la remodelación cosmética pude ser relacionada a otros recursos operatorios como los restauradores adhesivos directos, proporcionando resultados aún más satisfactorios a los pacientes. De esta manera, este trabajo tiene por objetivo presentar un caso clínico, en el cual la remodelación cosmética asociada a diferentes recursos operatorios, puede ser empleada como una opción de tratamiento estético conservador.


Cosmetic contouring of natural teeth is a treatment with aesthetic benefits that can be considered economical as well as conservative. The dental professional has however not only to restore the smile harmony, but the functional aspects have to be considered at the same time in the treatment planning. Other treatments such as direct adhesive restorative treatments may additionally be used to provide better outcomes to the patients. The aim of the present work thus is to present a case of cosmetic contouring as a suitable aesthetic treatment option.

En Odontología, un problema observado constantemente en pacientes adultos en este inicio de siglo es sin duda, el desgaste dental patológico. Este tipo de desgaste ocurre cuando existe pérdida acentuada de la estructura dentaria, lo que generalmente ocasiona el compromiso estético de los dientes antero superiores. En consecuencia, la sociedad se está concientizando sobre la importancia de la salud y de la estética, de esta manera, la demanda por tratamientos estéticos rehabilitadores en casos de desgaste dental patológico ha aumentado. La rehabilitación estética puede ser realizada tanto por medio de tratamientos directos como indirectos. Debido a las ventajas de las resinas compuestas, el presente trabajo tiene por finalidad describir la técnica operatoria para el restablecimiento de la guía anterior con resina compuesta, mediante la técnica de estratificación, que permite simular las características ópticas de la estructura dental. Así, con la evolución de los sistemas adhesivos, asociado a las mejoras en las propiedades químicas, mecánicas y estéticas de las resinas compuestas, tratamientos restauradores extensos, como la reconstrucción directa de las guías anteriores en los casos de desgaste dental patológico, se tornan viables.


In Dentistry, a common problem observed in this beginning of century is the pathologic tooth wear. This type of wear occurs when there is accentuated loss of tooth structure, which frequently results in aesthetic compromise of the anterior maxillary teeth. As a result of the society’s awareness of the importance of health and aesthetics, the request for reparative aesthetic treatments in cases of pathologic tooth wear has increased. This aesthetic rehabilitation may be achieved either through indirect or direct treatments. Due to the advantages of the resin composites, the aim of the present paper is to show specifically the direct approach throughout the use of a layering technique to simulate the optical characteristics of the tooth structure. Hence with the evolution of the adhesive systems associated with improving in the chemical-mechanical-aesthetical charecterics of the resin composites, extensive restorative treatments as the direct reconstruction of the anterior guidance in cases of pathologic tooth wear are now feasible.
