ABSTRACT

OPS Adelante es una iniciativa a nivel de toda la Organización, basada en resultados, para innovar y modernizar sistemáticamente las prácticas de gestión a fin de aumentar la eficiencia, la transparencia y la rendición de cuentas tanto de manera interna como externa. El plan de acción 2.0 de OPS Adelante permite operacionalizar el marco de esta iniciativa, e incluye objetivos específicos relativos a fortalecer la eficiencia, la transparencia y la rendición de cuentas de la Oficina Sanitaria Panamericana (la Oficina); reforzar la visibilidad de la Organización Panamericana de la Salud (OPS), el enfoque centrado en los países y la capacidad para proporcionar cooperación técnica, mejorar el desempeño de los recursos humanos de la Oficina e impulsar la innovación para hacer avanzar a la Organización. En este informe se describen los principales componentes del plan actualizado y se presenta la lista completa de acciones y resultados previstos que han sido priorizados por la gobernanza de OPS Adelante para el período de junio del 2024 a mayo del 2025. Asimismo, se incluye el informe anual del primer año de ejecución de la iniciativa OPS Adelante.

ABSTRACT

OBJECTIVE: To analyze the evidence on the influence of Lean and/or Six Sigma for process optimization in the perioperative period. METHODS: Integrative review carried out in the MEDLINE (PubMed), Web of Science, EMBASE, CINAHL, Scopus and LILACS databases on the use of Lean and/or Six Sigma to optimize perioperative processes. The studies included were analyzed in three thematic categories: flow of surgical patients, work process and length of stay. RESULTS: The final sample consisted of ten studies, which covered all operative periods. Lean and/or Six Sigma make a significant contribution to optimizing perioperative processes. FINAL CONSIDERATIONS: Lean and/or Six Sigma optimize perioperative processes to maximize the achievement of system stability indicators, making it possible to identify potential problems in order to recognize them and propose solutions that can enable the institution of patient-centered care.

ABSTRACT

OBJECTIVE: to investigate the contributions of applying the Lean methodology to improve work processes in health and nursing and its impact on associated financial aspects. METHOD: an integrative review, carried out in six databases, whose sample of ten (100.0%) studies was analyzed and summarized descriptively. RESULTS: the outcomes obtained were stratified into: benefits/barriers to Lean Healthcare implementation; economic aspects involving Lean Healthcare implementation; and process improvements through Lean Healthcare implementation. The majority of studies (60.0%) were carried out in university hospitals, contexts that need to continually improve the quality of services provided, generally with scarce and limited resources, which support the viability of maintaining the teaching, research and extension tripod. CONCLUSION: three (30.0%) studies highlighted the financial aspects associated with Lean methodology application. The others only mentioned the possibility of financial gains through improving processes and reducing waste.

ABSTRACT: A strong link exists between adequate supply chain management and nurse efficiency and satisfaction. Implementing Lean methodology, specifically 5S process improvement, staff created a unit-based supply room that was clean, safe, and well organized which led to reduced waste, greater efficiency, and cost savings.

O modelo de demandas e recursos foi utilizado para identificar o poder preditivo do estilo pessoal do terapeuta e do trabalho emocional (demandas), e da inteligência emocional e autoeficácia profissional (recursos) sobre as dimensões da síndrome de Burnout (SB), em uma amostra de 240 psicólogos clínicos brasileiros. Os dados foram coletados por meio de plataforma online, tendo como instrumentos de pesquisa um Questionário de dados sociodemográficos e laborais, o Cuestionario para la Evaluación del Síndrome de Quemarse por el Trabajo, o Cuestionario del Estilo Personal del Terapeut, o Questionário de Avaliação Relacionado a Demandas Emocionais e Dissonância da Regra da Emoção, Medida de Inteligência Emocional, e Escala de Autoeficácia Geral Percebida. Os resultados obtidos revelaram um modelo preditor das dimensões da SB, constituído pelas variáveis dissonância emocional, automotivação, demandas emocionais, instrução, envolvimento e autoeficácia. Ressalta-se a relevância de estratégias voltadas para a prevenção da SB nessa categoria profissional, bem como a necessidade de ações que visem a promoção e o desenvolvimento da inteligência emocional e da autoeficácia como fortalecimento dos recursos emocionais para atuação na prática clínica.(AU)


The Model of Demands - Resources was used to identify the predictive power of therapist's personal style, emotional work (Demands), Emotional intelligence, and professional self-efficacy (Resources) over the Burnout syndrome dimensions in a sample of 240 Brazilian clinical psychologists. The data was collected by an on-line platform using a Labor and social demographic data questionnaire, a work Burnout Syndrome Evaluation questionnaire (CESQT - Cuestionario para la Evaluación del Síndrome de Quemarse por el Trabajo), the short version of the Therapist Personal Style Questionnaire (EPT-C Cuestionario del Estilo Personal del Terapeuta), an Evaluation questionnaire related to emotional demands and emotion rule dissonance, and the Emotional Intelligence Measure (EIM) and Perceived General Self-Efficacy Scale (GPSS) as research instruments. Results showed a predictor model of Burnout syndrome constituted by the variables Emotional dissonance, Self-motivation, Emotional demands, Instruction, Involvement, and Self-efficacy. We emphasize the relevance of strategies to prevent Burnout Syndrome in this professional category and the need for actions to promote and develop emotional intelligence and self-efficacy as a strengthening factor of the emotional resources to work as a clinical psychologist.(AU)


Se utilizó el modelo demandas y recursos para identificar el poder predictivo del estilo personal del terapeuta y del trabajo emocional (demandas), y de la inteligencia emocional y autoeficacia profesional (recursos) sobre las dimensiones del síndrome de Burnout (SB), en una muestra de 240 psicólogos clínicos brasileños. Los datos se recolectaron de una plataforma en línea, utilizando como instrumentos de investigación un cuestionario de datos sociodemográficos y laborales, el Cuestionario para la Evaluación del Síndrome de Quemarse por el Trabajo, el Cuestionario del Estilo Personal del Terapeuta, el Cuestionario de Evaluación Relacionado con Demandas Emocionales y Disonancia de la Regla de la Emoción, la Medida de Inteligencia Emocional y Escala de Autoeficacia General Percibida. Los resultados obtenidos revelaron un modelo predictor de las dimensiones de SB, constituido por las variables disonancia emocional, automotivación, exigencias emocionales, instrucción, implicación y autoeficacia. Se destaca la relevancia de las estrategias dirigidas a la prevención del SB en esta categoría profesional, así como la necesidad de acciones dirigidas a promover y desarrollar la inteligencia emocional y la autoeficacia como fortalecimiento de los recursos emocionales para trabajar en la práctica clínica.(AU)

Este estudo analisa o estresse ocupacional entre psicólogos que atuavam na Atenção Primária à Saúde durante a pandemia ocasionada pela covid-19, assim como as características sociodemográficas e laborais dos participantes e sua relação com o estresse ocupacional. Participaram da pesquisa 70 psicólogos atuantes em 51 unidades básicas de saúde das regiões Oeste e Extremo Oeste catarinense. Para coleta de dados, um questionário sociodemográfico e a versão reduzida da Job Stress Scale (JSS) foram aplicados. A análise dos dados foi realizada por meio da estatística descritiva e inferencial. Identificou-se que 35,7% dos psicólogos apresentaram alto desgaste no trabalho; 28,6% baixo desgaste; 27,1% se mostraram em trabalho passivo; e 8,6% em trabalho ativo. No modelo de regressão linear, os fatores associados à dimensão Demanda da JSS foram: possuir filho (a) (coeficiente -1,49; IC 95% -2,75 a -0,23) e afastamento do trabalho nos últimos 12 meses (coeficiente 1,88; IC 95% 0,60 a 3,15). Os psicólogos com hipertensão arterial sistêmica autorreferida apresentaram, em média, 3,96 pontos a menos no escore de Apoio social (IC 95% -7,06 a -0,85), quando comparados aos não hipertensos, e entre os psicólogos que trabalhavam no turno da manhã identificou-se aumento de 4,46 pontos, em média, no escore de Apoio social (IC 95% 0,90 a 8,02) em relação aos profissionais do turno manhã e tarde. Evidenciou-se que um número significativo de psicólogos apresentava-se em alto desgaste no trabalho, com potenciais implicações para sua saúde e atuação profissional.(AU)


This study analyzed occupational stress among psychologists who worked in Primary Health Care during the COVID-19 pandemic and participants' sociodemographic and work characteristics and their relationship with occupational stress. In total, 70 psychologists working in 51 basic health units in the West and Far West regions of Santa Catarina participated in this research. A sociodemographic questionnaire and the short version of the Job Stress Scale (JSS) were applied to collect data. Data were analyzed by descriptive and inferential statistics. In total, 35.7% of psychologists showed high stress at work; 28.6%, low burn out; 27.1%, passive work; and 8.6%, active work. The factors in the linear regression model that were associated with the JSS demand dimension referred to having children (coefficient −1.49; 95% CI −2.75 to −0.23) and absence from work in the last 12 months (coefficient 1.88; 95% CI 0.60 to 3.15). Psychologists with self-reported systemic arterial hypertension showed, on average, 3.96 points lower in the Social Support score (95% CI −7.06 to −0.85) than non-hypertensive ones and psychologists who worked in the morning shift, an average increase of 4.46 points in the Social Support score (95% CI 0.90 to 8.02) in relation to professionals working in the morning and afternoon shifts. A significant number of psychologists had high stress at work, with potential implications to their health and professional performance.(AU)


Este estudio evalúa el estrés laboral entre los psicólogos que trabajaron en la atención primaria de salud durante la pandemia provocada por la COVID-19, así como las características sociodemográficas y laborales de ellos y su relación con el estrés laboral. En la investigación participaron setenta psicólogos que trabajan en 51 unidades básicas de salud en las regiones oeste y lejano oeste de Santa Catarina (Brasil). Para la recolección de datos se aplicó un cuestionario sociodemográfico y la versión corta de la Job Stress Scale (JSS). El análisis de los datos se realizó mediante estadística descriptiva e inferencial. Se identificó que el 35,7% de los psicólogos presentaban alto estrés en el trabajo; el 28,6% tenían poco desgaste; el 27,1% se encontraban en trabajo pasivo; y el 8,6% en trabajo activo. En el modelo de regresión lineal, los factores asociados a la dimensión demanda de la JSS fueron: tener hijo (coeficiente -1,49; IC 95% -2,75 a -0,23) y baja laboral en los últimos 12 meses (coeficiente 1,88; IC 95% 0,60 a 3,15). Los psicólogos con hipertensión arterial sistémica autoinformada presentaron un promedio de 3,96 puntos más bajo en la puntuación de apoyo social (IC 95% -7,06 a -0,85) en comparación con los no hipertensos, y entre los psicólogos que trabajaban en el turno de la mañana, se identificó un aumento promedio de 4,46 puntos en la puntuación de apoyo social (IC 95% 0,90 a 8,02) con relación a los profesionales que laboran en el turno de mañana y tarde. Quedó evidente que un número significativo de psicólogos se encontraba en situación de alto estrés en el trabajo, con posibles implicaciones para su salud y desempeño profesional.(AU)

A Psicologia Organizacional e do Trabalho (POT) é permeada por uma série de julgamentos negativos e estigmas, principalmente acerca de seu papel intermediador de relações de exploração sofridas pelo trabalhador. Investigar as percepções que graduandos têm sobre a área pode trazer acesso ao que há de central entre as diversas formas de pensar o tema. Este trabalho buscou analisar as percepções de 100 graduandos de duas instituições de ensino superior (IES) do Centro-Oeste Paulista, uma pública e outra privada, acerca da importância da atuação do psicólogo em POT e o interesse desses discentes em atuarem futuramente na área, estabelecendo uma comparação entre os resultados com base nas IES e entre primeiro e quinto ano da graduação. Os graduandos responderam a dois questionários estruturados com perguntas abertas e fechadas. Os resultados apontaram uma maior valorização da área de POT na IES privada e no quinto ano. A maioria dos discentes do primeiro ano de graduação apresentou pouco conhecimento sobre a POT. Os estudantes da IES privada apresentaram maior interesse em atuar na área do que os discentes da IES pública. Quando perguntados se houve uma mudança de opinião após a realização das disciplinas, 49 alunos do quinto ano responderam positivamente e apenas um respondeu negativamente. As respostas dissertativas se dividiram entre visões positivas, neutras e negativas sobre a POT. Os resultados permitiram avaliação das percepções da POT entre os graduandos, bem como das transformações ocorridas pelo impacto das experiências acadêmicas vivenciadas.(AU)


Organizational and Work Psychology (OWP) is permeated by a series of negative judgments and stigmas, mainly about its role as an intermediary of relations exploiting workers. Investigating the perceptions undergraduates have about it can bring access to what is central among the different ways of thinking about the subject. This study sought to analyze the perceptions of 100 undergraduates from two higher education institutions (HEIs), one public and private in Midwestern São Paulo about the importance of psychologists' role in OWP and their interest in working in the area in the future, comparing the results per HEI and between the first and fifth year of graduation. Graduates answered two structured questionnaires with open and closed questions. Results pointed to a greater appreciation of OWP in the private HEI and in the fifth year. Most first-year undergraduate students had little knowledge about OWP. Students from the private HEI took greater interest in working in the area than students from the public HEI. When asked if taking the courses changed their opinions, 49 students responded positively and only one negatively. Participants' responses were divided into positive, neutra,l and negative views about OWP. Results evaluated the perceptions on OWP in undergraduates the changes due to the impact of academic experiences.(AU)


La Psicología Organizacional y Laboral (POT) está permeada por una serie de juicios negativos y estigmas, principalmente sobre su papel como intermediaria de las relaciones de explotación laboral. Investigar las percepciones que los estudiantes tienen sobre este campo puede brindar acceso a lo que es central entre las diferentes formas de pensar sobre el tema. Este estudio buscó analizar las percepciones de 100 graduandos de dos instituciones de educación superior (IES), una pública y otra privada, del medio oeste de São Paulo, acerca de la importancia del rol del psicólogo en POT y del interés por actuar en el área, estableciendo una comparación entre los resultados por tipo de IES y período del curso (el primer y quinto año de grado). Los graduandos respondieron a dos cuestionarios estructurados con preguntas abiertas y cerradas. Los resultados apuntan a una mayor valoración de POT en las IES privadas y en el quinto año. La mayoría de los estudiantes de primer año tenían poco conocimiento sobre POT. Los estudiantes de la IES privada estaban más interesados en trabajar en el área que los estudiantes de la IES pública. Cuando se les preguntó si hubo un cambio de opinión luego de cursar las asignaturas, 49 estudiantes del quinto año respondieron positivamente y solo uno respondió negativamente. Las respuestas del ensayo se dividieron en puntos de vista positivos, neutrales y negativos sobre POT. Los resultados permitieron evaluar las percepciones de POT entre los graduandos, así como evaluar los cambios ocurridos por el impacto de las experiencias académicas.(AU)

ABSTRACT

Diabetes prevalence is rising globally, especially in low- and middle-income countries like Mexico, posing challenges for healthcare systems that require efficient primary care to manage the disease. However, healthcare efficiency is influenced by factors beyond decision-makers, including socioeconomic and political conditions. This study aims to evaluate the technical efficiency of primary healthcare for diabetes patients in Mexico over a 12-year period and explore the impact of contextual variables on efficiency. A longitudinal analysis was conducted using administrative and socio-demographic data from 242 health jurisdictions between 2009 and 2020. Data envelopment analysis with bootstrapping and output orientation was used to measure the technical efficiency; health resources in infrastructure and human resources were used as inputs. As outcome, the number of patients receiving treatment for diabetes and the number of patients with controlled diabetes were considered. Machine learning algorithms were employed to analyse multiple factors affecting the provision of diabetes health services and assess heterogeneity and trends in efficiency across different health jurisdictions. The average technical efficiency in primary healthcare for diabetes patients was 0.44 (CI: 0.41-0.46) in 2009, reaching a peak of 0.71 (CI: 0.69-0.72) in 2016, and moderately declining to 0.60 (CI: 0.57-0.62) in 2020; these differences were statistically significant. The random forest analysis identified the marginalization index, primary healthcare coverage, proportion of indigenous population and demand for health services as the most influential variables in predicting efficiency levels. This research underscores the crucial need for the formulation of targeted public policies aimed at extending the scope of primary healthcare services, with a particular focus on addressing the unique challenges faced by marginalized and indigenous populations. According to our results, it is necessary that medical care management adjust to the specific demands and needs of these populations to guarantee equitable care in Mexico.

ABSTRACT

The measurement of performance within the water industry holds significant importance for policymakers, as it can help guide decision-making for future development and management initiatives. In this study, we apply data envelopment analysis (DEA) cross-efficiency techniques to evaluate the productivity change of the Chilean water industry during the years 2010-2018. Water leakage and unplanned interruptions are included in the analysis as quality of service variables. Moreover, we use cluster analysis and regression techniques to better understand what drives productivity change of water companies. The results indicate that the Chilean water industry is characterized by considerable high levels of inefficiency and low levels of productivity change. This is due to the existence of technical regress whereas gains in efficiency were small. Concessionary water companies were found to be more productive than full private and public water companies. Best and worst performers need to make efforts to reduce production costs and improve service quality. Other factors such as customer density and ownership type statistically affect productivity.

ABSTRACT

OPS Adelante se centra en la modernización, innovación y simplificación de las políticas, los procesos y los sistemas para fortalecer la gestión interna, los controles y la toma de decisiones. Fomenta una cultura de eficiencia, transparencia y rendición de cuentas a fin de aumentar la eficacia de la Organización Panamericana de la Salud (OPS) en todos los niveles. Gracias a la racionalización de los procesos, la utilización de la tecnología, los análisis avanzados de datos y la comunicación abierta, la Oficina Sanitaria Panamericana (la Oficina) estará mejor equipada para abordar los desafíos cambiantes en materia de salud pública a los que tienen que hacer frente los Estados Miembros. Por medio de procesos de gestión innovadores, OPS Adelante puede ayudar a colocar a la salud en el centro de la recuperación económica de la Región de las Américas y en las políticas para abordar la pobreza, las inequidades y las desigualdades que se han profundizado en los últimos años. La Oficina debe cultivar una fuerza laboral comprometida, empoderada y diversa, equipada con las habilidades y competencias necesarias para desempeñarse en un entorno de salud pública cada vez más complejo. Las prácticas y procesos de gestión eficaces e innovadores en materia de recursos humanos son esenciales para lograr la misión de la OPS. Los sistemas de tecnología de la información y de las comunicaciones ayudarán a la Oficina a aprovechar las tecnologías, los sistemas de gestión de datos y las plataformas de intercambio de información para simplificar los procesos, facilitar la adopción de decisiones y medidas basadas en la evidencia, y mejorar el desempeño general de la Organización. Durante una crisis, la Organización debe evaluar rápidamente la situación, y determinar el nivel de apoyo y los recursos adicionales que se necesitan. Los recursos deben asignarse estratégicamente para proporcionar el apoyo necesario, y abordar las necesidades particulares de los países a fin de responder de manera eficaz durante toda la crisis. OPS Adelante fortalecerá la capacidad de la Oficina para proporcionar cooperación técnica a fin de promover el impacto y la sostenibilidad en el ámbito de la salud pública para los países y los pueblos a los que servimos.

PAHO Forward focuses on modernizing, innovating, and streamlining policies, processes, and systems to strengthen internal management, controls, and decision-making. It fosters a culture of efficiency, transparency, and accountability to enhance the effectiveness of the Pan American Health Organization (PAHO) across all levels. Through streamlined processes, technology utilization, advanced data analyses, and open communication, the Pan American Sanitary Bureau (PASB) will become better equipped to address the evolving public health challenges of Member States. Through innovative management processes, PAHO Forward can contribute to placing health at the heart of the Region’s economic recovery and in policies to address the poverty, inequities, and inequalities that have deepened in recent years. PASB must cultivate an engaged, empowered, and diverse workforce equipped with the necessary skills and competencies within an increasingly complex public health landscape. Effective and innovative human resources management practices and processes are integral to achieving PAHO’s mission. Information technology and communication systems will help PASB leverage technologies, data management systems, and information-sharing platforms to streamline processes, facilitate evidence-based decisions and actions, and improve overall organizational performance. During a crisis, the Organization must quickly assess the situation and determine the required level of support and additional resources needed. Resources must be strategically allocated to provide the required support and address the unique needs of countries, ensuring an effective response throughout the duration of the crisis. PAHO Forward will strengthen PASB’s capacity to deliver technical cooperation to drive public health impact and sustainability for the countries and peoples that PAHO serves.

Este estudo avaliou e comparou a produtividade clínica dos estudantes de Odontologia em duas instituições de ensino privadas, com diferentes tempos de experiência no modelo de clínica integrada, preconizado pelas Diretrizes Curriculares Nacionais (DCN). Asuniversidades utilizam o modelo de clínica integrada desde 2001 [A] e 2009 [B], com cursos de graduação de 5 anos e 4 anos, respectivamente. O estudo foi realizado no período de fevereiro a dezembro de 2019. Foram incluídos dados relativos a 205 estudantes (56 da universidade [A] e 149 da [B]) matriculados nas clínicas de baixa/média (n=99) e alta complexidades (n=106). Avaliou-se a quantidade de procedimentos realizados por nível de complexidade nas duas instituições. Os dados foram submetidos aos testes de Mann-Whitney e qui-quadrado (α=0,05). O número de procedimentos analisados foi de 9706, sendo 4693 na instituição [A] e 5013 na [B]. Nas duas instituições a quantidade de procedimentos curativos (54,8%) foi significativamente maior que a de procedimentos de diagnóstico e atenção à saúde (45,2%) (p<0,001). Nas clínicas de baixa/média complexidade foram observadas diferenças estatísticas na quantidade de procedimentos e/ou na quantidade de estudantes que não realizaram determinados procedimentos clínicos. Nas clínicas de alta complexidade foram observadas diferenças estatísticas entre as instituições na quantidade de procedimentos e/ou na quantidade de alunos que não realizaram determinados procedimentos específicos de Dentística, Cirurgia, Periodontia e Prótese. A instituição com maior tempo de experiência no modelo de clínica integrada apresentou melhores resultados nas especialidades de Dentística, Endodontia e Prótese (AU).

Se incluyeron datos de 205 estudiantes (56 de la universidad [A] y 149 de la [B]) matriculados en clínicas de baja/media (n=99) y alta complejidad (n=99) n=106). Se evaluó el número de procedimientos realizados por nivel de complejidad en ambas instituciones. Los datos fueron sometidos a las pruebas de Mann-Whitney y chi-cuadrado (α=0,05). Elnúmero de procedimientos analizados fue de 9706, 4693 en la institución [A] y 5013 en la [B]. En ambas instituciones, el número de procedimientos curativos (54,8%) fue significativamente superior al de procedimientos diagnósticos y asistenciales (45,2%) (p<0,001). En las clínicas de baja/media complejidad se observaron diferencias estadísticas en el número de procedimientos y/o en el número de alumnos que no se sometieron a determinados procedimientos clínicos. En las clínicas de alta complejidad se observaron diferencias estadísticas entre las instituciones en cuanto al número de procedimientos y/o el número de alumnos que no se sometieron a determinados procedimientos en Odontología, Cirugía, Periodoncia y Prótesis. La institución con mayor experiencia enel modelo de clínica integrada presentó mejores resultados en las especialidades de Odontología, Endodoncia y Prótesis (AU).


This study assessed and compared the clinical productivity of dental students in two private teaching institutions, with different lengths of experience in the integrated clinical model, recommended by the National Curriculum Guidelines (DCN). The universities have used the integrated clinical model since 2001 [A] and 2009 [B], with 5-year and 4-year undergraduate courses, respectively. The study was carried out from February to December 2019. Data were included for 205 students (56 from [A] and 149 from [B]) enrolled in low/medium (n=99) and high complexity clinics (n=106). The number of procedures performed by level of complexity in both institutions was assessed. Data were submitted to Mann-Whitney and chi-square tests (α=0.05). The number of procedures analyzed was 9706, 4693 in institution [A] and 5013 in [B]. In both institutions, the number of curative procedures (54.8%) was significantly higher than that of diagnostic and health care procedures (45.2%)(p<0.001). In low/medium complexity clinics, statistical differences were observed in the number of procedures and/or in the number of students who did not perform certain clinical procedures. In high-complexity clinics, statistical differences were observed between the institutions in terms of the number of procedures and/or the number of students who did not perform certain specific procedures in restorative dentistry, surgery, periodontics and prostheses. The institution with the longer experience in the integrated clinic model presented better results in the specialties of restorative dentistry, endodontics and prosthesis (AU).

ABSTRACT

PURPOSE: This article aims to introduce a guide to improving hospital bed setup by combining lean technical practices (LTPs), such as kaizen and value stream mapping (VSM) and lean social practices (LSPs), such as employee empowerment. DESIGN/METHODOLOGY/APPROACH: Action research approach was employed to analyze the process of reconfiguration of bed setup management in a Brazilian public hospital. FINDINGS: The study introduces three contributions: (1) presents the use of VSM focused specifically on bed setup, while the current literature presents studies mainly focused on patient flow management, (2) combines the use of LSPs and LTPs in the context of bed management, expanding current studies that are focused either on mathematical models or on social and human aspects of work, (3) introduces a practical guide based on six steps that combine LSPs and LSPs to improve bed setup management. RESEARCH LIMITATIONS/IMPLICATIONS: The research focused on the analysis of patient beds. Surgical beds, delivery, emergency care and intensive care unit (ICU) were not considered in this study. In addition, the process indicators analyzed after the implementation of the improvements did not contemplate the moment of the COVID-19 pandemic. Finally, this research focused on the implementation of the improvement in the context of only one Brazilian public hospital. PRACTICAL IMPLICATIONS: The combined use of LSPs and LTPs can generate considerable gains in bed setup efficiency and consequently increase the capacity of a hospital to admit new patients, without the ampliation of the physical space and workforce. SOCIAL IMPLICATIONS: The improvement of bed setup has an important social character, whereas it can generate important social benefits such as the improvement of the admission service to patients, reducing the waiting time, reducing hospitalization costs and improving the hospital capacity without additional physical resources. All these results are crucial for populations, their countries and regions. ORIGINALITY/VALUE: While the current literature on bed management is more focused on formal models or pure human and social perspectives, this article brings these two perspectives together in a single, holistic framework. As a result, this article points out that the complex bed management problem can be efficiently solved by combining LSPs and LTPs to present theoretical and practical contributions to the important social problem of hospital bed management.

ABSTRACT

Drinking water treatment systems (DWTSs) are energy intensive facilities, and are an example of the water-energy nexus. Benchmarking energy efficiency is a valuable tool for improving the economic and environmental performance of such facilities. Data envelopment analysis (DEA) is typically used to assess efficiency, allocating flexible weights (FSW) to variables that maximise energy efficiency scores for each DWTS (DEA-FSW). It means that different conditions are applied to each DWTS. Moreover, the DEA-FSW approach has finite discriminatory power which limits cross-unit comparison of energy efficiency hindering the benchmarking of DWTSs. To overcome these limitations, our study explored the effect of estimating the energy efficiency scores of DWTSs by allocating common sets of weights (CSW) within DEA (DEA-CSW). This approach was applied empirically on a sample of 146 DWTSs. Evaluated DWTSs had poor energetic performance based on both DEA-FSW and DEA-CSW estimates (low energy efficiency scores: 0.329 and 0.163, respectively). Even in the optimistic scenario, the average energy efficiency score was low (0.220), with potential electricity savings of 78 % by DWTPs when energy efficient. Unlike DEA-FSW, DEA-CSW allowed energy efficient DWTSs to be distinguished from the 146 facilities. Significant differences in the weights allocated to electricity and pollutants removed from raw water were reported for both approaches, and contributed to diverging energy efficiency scores. In conclusion, this study demonstrated the relevance of using suitable methods to generate comparable results for water companies, allowing the energy performance of DWTSs to be objectively evaluated for benchmarking purposes.

ABSTRACT

As startups são empresas que apresentam modelos de negócios marcados pela inovação, rapidez, flexibilidade e alta capacidade de adaptação aos mercados. Atuando em diferentes setores socioeconômicos, elas prometem criar e transformar produtos e serviços. A emergência e disseminação dessas empresas ocorrem em um momento histórico de mudanças iniciadas a partir de 1970 e marcadas pelas crises geradas com o esgotamento do paradigma da sociedade urbano industrial. No Brasil, o número desse modelo de negócio apresentou uma expansão expressiva, alcançando a marca de 13.374 nos últimos cinco anos. Atento a esse cenário, o objetivo desta pesquisa consistiu em compreender como sujeitos, grupos e instituições atribuem sentidos à experiência de trabalho nas chamadas startups. Na parte teórica, as condições sociais e econômicas que possibilitaram a emergência e disseminação das startups são analisadas em uma perspectiva crítica. A parte empírica, por sua vez, apresenta depoimentos de empreendedores relatando o contexto geral de atuação nas startups. Ao final deste artigo, conclui-se que há uma instrumentalização capitalística de componentes subjetivos específicos selecionados e colocados em circulação para fortalecer o modo de produção capitalista financeirizado.(AU)


Startups are companies that have business models characterized by innovation, speed, flexibility, and a high capacity to adapt to markets. Operating in different socioeconomic sectors, they promise to create and transform products and services. The emergence and dissemination of these companies occur at a historical moment of changes that began from 1970 and are marked by the crises generated by the exhaustion of the paradigm of industrial urban society. In Brazil, the number of businesses in this model showed a significant expansion, reaching 13,374 companies in the last five years. Attentive to this scenario, the objective of this research was to understand how subjects, groups, and institutions attribute meanings to the work experience in so-called startups. In the theoretical part, the social and economic conditions that enabled the emergence and dissemination of startups are analyzed in a critical perspective. The empirical part presents entrepreneurs reporting the general context of action in startups. At the end of this article, it is concluded that there is a capitalistic instrumentalization of specific subjective components that are selected and put into circulation to strengthen the financed capitalist production.(AU)


Las startups son empresas que tienen modelos de negocio marcados por la innovación, la velocidad, la flexibilidad y una alta capacidad de adaptación a los mercados. Desde diferentes sectores socioeconómicos, las startups prometen crear y transformar productos y servicios. La aparición y difusión de estas empresas se produce en un momento histórico de cambios que comenzó a partir de 1970 y que está marcado por crisis generadas por el agotamiento del paradigma de la sociedad urbana industrial. En Brasil, estas empresas se expandieron significativamente alcanzando la marca de 13.374 empresas en los últimos cinco años. En este escenario, el objetivo de esta investigación fue entender cómo los sujetos, grupos e instituciones atribuyen significados a la experiencia laboral en las startups. En la parte teórica, se analizan las condiciones sociales y económicas que permitieron el surgimiento y la difusión de las startups en una perspectiva crítica. La parte empírica presenta testimonios de emprendedores que informan sobre el trabajo en startups. La investigación concluye que hay una instrumentalización capitalista de componentes subjetivos específicos que se seleccionan y ponen en circulación para fortalecer el modo de producción capitalista financiero.(AU)

ABSTRACT

Este estudo teve como objetivo identificar o risco de desenvolvimento de transtorno de estresse pós-traumático (TEPT), bem como sua associação com pensamentos ou tentativas suicidas e a saúde mental de policiais militares feridos por arma de fogo, na Região Metropolitana de Belém (RMB), nos anos de 2017 a 2019. A pesquisa contou com a participação de 30 entrevistados, que responderam o Inventário Demográfico e a Lista de verificação de TEPT para o DSM-5 (PCL-5). Para análise dos dados, utilizou-se a técnica estatística Análise Exploratória de Dados e a técnica multivariada Análise de Correspondência. Os resultados revelaram a existência de risco de desenvolvimento do transtorno de forma parcial ou total em uma expressiva parcela da população entrevistada, tendo homens como maioria dos sintomáticos, com média de 38 anos, exercendo atividades operacionais e vitimados em via pública quando estavam de folga do serviço. O ferimento deixou a maioria com sequelas, com destaque para dores crônicas, limitações de locomoção e/ou mobilidade e perda parcial de um membro. E, ainda, policiais sintomáticos apresentaram comportamentos suicidas, relatando já terem pensado ou tentado tirar a própria vida. Desta forma, conclui-se que policiais militares são expostos constantemente a traumas inerentes a sua profissão. Quando há ameaça de vida, como nos casos de ferimentos por arma de fogo, são suscetíveis a sequelas físicas decorrente do ferimento, somadas a sequelas mentais tardias, como o surgimento de sintomatologias de TEPT e ideação suicida.(AU)


This study aimed to identify the risk of developing post-traumatic stress disorder (PTSD) and its associations around suicidal thoughts or attempts and mental health in military police officers injured by firearms, in the Metropolitan Region of Belem (RMB), from 2017 to 2019. The research had the participation of 30 respondents who answered the Demographic Inventory and the PTSD checklist for DSM-5 (PCL-5). For data analysis, we used the statistical technique Exploratory Data Analysis and the multivariate technique Correspondence Analysis. The results revealed the existence of risk of developing partial or total disorder in a significant portion of the interviewed population, with men as most of the symptomatic individuals, with mean age of 38 years, developing operational activities and victimized on public roads when they were off duty. The injuries left most of them with sequelae, especially chronic pain, limited locomotion and/or mobility, and partial loss of a limb. In addition, symptomatic officers showed suicidal behavior, such as reporting they had thought about or tried to take their own lives. Thus, we conclude that military policemen are constantly exposed to traumas inherent to their profession. When their lives are threatened, as in the case of firearm wounds, they are susceptible to physical sequelae resulting from the injury, in addition to late mental sequelae, such as the appearance of PTSD symptoms and suicidal ideation.(AU)


Este estudio tuvo como objetivo identificar el riesgo de desarrollo de trastorno de estrés postraumático (TEPT) y sus asociaciones con pensamientos o tentativas suicidas y la salud mental en policías militares heridos por armamiento de fuego, en la Región Metropolitana de Belém (Brasil), en el período entre 2017 y 2019. En el estudio participaron 30 entrevistados que respondieron el Inventario Demográfico y la Lista de verificación de TEPT para el DSM-5 (PCL-5). Para el análisis de datos se utilizaron la técnica estadística Análisis Exploratoria de Datos y la técnica multivariada Análisis de Correspondencia. Los resultados revelaron que existen riesgos de desarrollo de trastorno de estrés postraumático de forma parcial o total en una expresiva parcela de la población de policías entrevistados, cuya mayoría de sintomáticos eran hombres, de 38 años en media, que ejercen actividades operacionales y fueron victimados en vía pública cuándo estaban de día libre del servicio. La lesión dejó la mayoría con secuelas, especialmente con dolores crónicos, limitaciones de locomoción y/o movilidad y la pierda parcial de un miembro. Aún los policías sintomáticos presentaran comportamiento suicida, tales como relataran qué ya pensaron o tentaron quitar la propia vida. Se concluye que los policías militaran se exponen constantemente a los traumas inherentes a su profesión. Cuando existe amenaza de vida, como en los casos de heridas por armamiento de fuego, son expuestos a secuelas físicas transcurridas de la herida, sumado a secuelas mentales tardías, como el surgimiento de sintomatologías de TEPT y la ideación suicida.(AU)

ABSTRACT

As dificuldades e barreiras enfrentadas no processo de inclusão de pessoas com deficiência (PcD) nas organizações incitam o desenvolvimento de pesquisas. Este estudo compreendeu a percepção de psicólogos organizacionais sobre a inclusão de PcD em empresas. Dezoito psicólogos atuantes na área de gestão de pessoas de empresas das sete regiões do estado do Rio Grande do Sul responderam a uma entrevista individual. A média de idade dos participantes foi de 33,17 anos, atuavam em empresas de diferentes segmentos, eram predominantemente do sexo feminino e possuíam pós-graduação em áreas relacionadas. Os relatos dos psicólogos alertaram para o fato de que, em suas graduações, o conteúdo sobre deficiência humana e, especificamente, inclusão no mercado de trabalho foi escasso ou inexistente. Essa lacuna na formação, de egressos de diferentes instituições de ensino superior, é relatada desde os anos de 1990. Para esses psicólogos, barreiras atitudinais e organizacionais são frequentemente enfrentadas no processo de inclusão, tais como o despreparo das empresas, gestores e colaboradores para receber as PcD, os poucos programas voltados a uma prática efetiva de inclusão e não somente ao cumprimento da legislação, além das dificuldades dos próprios profissionais em identificar os potenciais e as limitações que a PcD apresenta e de adaptá-la de maneira correta ao trabalho. O psicólogo organizacional pode contribuir para um processo adequado de inclusão por meio de práticas, tais como treinamentos e sensibilizações, que fomentem a informação e diminuam a discriminação e as dificuldades.(AU)


Difficulties and barriers to including people with disabilities (PwDs) in organizations drives research development. This study sought to understand how organizational psychologists perceived the inclusion of PwDs in organizations. Eighteen organizational psychologists who work in people management for companies in the seven regions of the state of Rio Grande do Sul participated in an individual interview. Most interviewees were female, with average age of 33.17 years, had a postgraduate degree in the field, and worked in companies from different segments. During the interviews, the psychologists called attention to the little or nonexistent content on human disability and, specifically, inclusion in the labor market covered in the graduate course. This gap has been reported by graduates from different higher education institutions since the 1990s. According to the respondents, attitudinal and organizational barriers are often faced in the inclusion process, such as the unpreparedness of companies, managers, and employees to welcome PwD, the few programs aimed at an effective inclusion and not only to comply with the law, as well as the difficulties of the professionals themselves to identify the potentials and limitations that PwD present and to adapt them correctly to the work. Organizational psychologists can contribute to an adequate inclusion process by developing training and sensibilization activities that foster information and reduce discrimination and difficulties.(AU)


Las dificultades y barreras enfrentadas en el proceso de inclusión de personas con discapacidad (PcD) en las organizaciones fortalecen el desarrollo de la investigación. Este estudio entendió la percepción de los psicólogos organizacionales acerca de la inclusión de las PcD en las empresas. Dieciocho psicólogos que trabajan en el área de gestión de personas en empresas de las siete regiones del estado de Rio Grande do Sul (Brasil) respondieron a una entrevista individual. Los participantes tenían una edad promedio de 33,17 años, trabajaban en empresas de diferentes segmentos, eran predominantemente mujeres y tenían un posgrado en el área. Los informes de los psicólogos alertaron sobre el hecho de que el contenido sobre discapacidad humana y, específicamente, su inclusión en el mercado laboral era escaso o inexistente durante su formación académica. Esta brecha en la formación de los egresados de diferentes instituciones de educación superior se reporta desde los 1990. Para estos psicólogos, a menudo ocurren barreras organizacionales y de actitud en el proceso de inclusión de las PcD, como la falta de preparación de las empresas, gerentes y empleados para recibirlas, pocos programas destinados a una práctica efectiva de la inclusión, no solo al cumplimiento de la ley, y las dificultades de los profesionales para identificar las potencialidades y limitaciones y adecuarlas correctamente al trabajo. El psicólogo organizacional puede contribuir a un proceso de inclusión adecuado, con prácticas de capacitación y sensibilización que brindan información y reducen la discriminación y dificultades.(AU)

ABSTRACT

Este artigo refere-se à parte de uma pesquisa de doutorado, realizada em hospital de alta complexidade do Sistema Único de Saúde, cujos participantes são os profissionais de saúde. O objetivo deste artigo é analisar o potencial da abordagem das narrativas como método de pesquisa e intervenção nos serviços de saúde, traçando aproximações com a teoria psicanalítica. Sua relevância no campo da Saúde Pública está calcada no reconhecimento do papel do sujeito como agente de mudanças. O texto divide-se em duas partes: na primeira, explora as especificidades do trabalho na área da saúde, o paradigma da saúde pública no que concerne à gestão e possíveis contribuições da clínica ampliada para esse modelo. Na segunda parte, analisa o uso das narrativas como método de pesquisa nesse campo e as aproximações conceituais entre a narrativa em Walter Benjamin e a psicanálise em Freud. Busca na literatura referências sobre experiências análogas que fundamentem a proposta ora realizada e conclui pela importância de, no momento atual, apostar na força germinativa das narrativas como fonte criativa de novas formas de cuidar.(AU)


This article derives from a PhD research conducted with health professionals at a high-complexity public hospital from the Brazilian Unified Health System (SUS). It analyzes the potential of the narrative as a research and intervention method in health services, outlining approximations with psychoanalysis. In the field of Public Health, the narrative approach acknowledges the individual as an agent of change. The text is divided into two parts. The first presents an overview of the peculiarities involved in healthcare, the Public Health paradigm regarding service management and possible contributions from the expanded clinic to this model. The second analyzes the use of narratives as a research method in this field and the conceptual approximations between Benjamin's narrative and Freud's psychoanalysis. It searches the literature for references on similar experiences to support the present proposal and concludes by highlighting the importance of betting on the creative power of narratives as a source for new ways to care.(AU)


Este artículo es parte de una investigación doctoral, realizada con los profesionales de la salud de un hospital de alta complejidad del Sistema Único de Salud de Brasil. Su propósito es analizar el potencial del enfoque en narrativas como método de investigación e intervención en los servicios de salud, esbozando aproximaciones entre las narrativas y la teoría del psicoanálisis. Su relevancia en el campo de la salud pública se basa en el reconocimiento del rol del sujeto como agente de cambio. El texto se divide en dos partes: La primera investiga las especificidades del trabajo en el área de la salud, el paradigma de la salud pública en la gestión de los servicios y las posibles contribuciones de la clínica ampliada a este modelo. En la segunda parte, analiza el uso de narrativas como método de investigación en este campo y las aproximaciones conceptuales entre la narrativa de Walter Benjamin y el psicoanálisis de Freud. Este estudio busca en la literatura referencias sobre experiencias similares que apoyen la propuesta ahora realizada y concluye con la importancia de, en el momento actual, apostar por el poder de las narrativas como fuente creadora de nuevas formas de cuidar.(AU)
