ABSTRACT
Donovanosis is a chronic bacterial illness frequently associated with sexually transmitted infections (STI) and is under diagnosed both in endemic areas as well as in countries in which doctors have little experience with tropical diseases. The utilization of syndromic diagnosis and treatment of STIs in various parts of the world and the previous use of antibiotics make it difficult to find Donovan bodies in the cytodiagnostic and hystopathological exams, requiring the utilization of technology that is neither routine nor often accessible to confirm the hypothesized diagnosis. Therefore, it is necessary to bring medical professionals up to date about this infectious disease.

The authors present a case of disseminated granuloma inguinale with bilateral psoas abscesses. Infection with calymmatobacterium granulomatis is usually localized to the genital organs but rarely may be disseminated. A search of the literature revealed that only two cases of psoas abscesses due to calymmatobacterium granulomatis were previously reported.
Los autores presentan un caso de granuloma inguinal diseminado con abscesos bilaterales de psoas. La infección con calymmatobacterium granulomatis normalmente se localiza en los órganos genitales, y raramente se disemina. La literatura reveló sólo dos casos de abscesos de psoas debidos a calymmatobacterium granulomatis reportados con anterioridad.

Donovanosis is a sexually transmitted infection which presents with genital ulceration and inguinal lymphadenopathy. Rarely, it presents with extra-genital manifestations. We present a case of disseminated donovanosis with cervical ulceration, massive pelvic lymphadenopathy, osteomyelitis of the wrists and septic arthritis of the knees and right elbow. A 23-year-old gravida two presented with wasting, oedema, ascites, bilateral iliac lymphadenopathy, anaemia and a large ulcer of the cervix uteri. Two months later in the outpatient clinic, she was much improved but still had post-coital bleeding and a hyperaemic cervix, suggestive of persistent infection. The course of antibiotics was therefore repeated. Histopathological examination of a specimen from colposcopic biopsy of the cervix uteri revealed granuloma inguinale. She improved after several courses of antibiotics, blood transfusion, surgical débridement and aspiration of affected joints.

Os autores relatam dois casos de pacientes portadores da Síndrome da Imunodeficiência Adquirida e Donovanose, sendo um do sexo masculino e outro do sexo feminino. A evoluçäo foi bastante atípica com aparecimento de novas lesöes, e expansäo das preexistentes, apesar da terapêutica empregada, a qual, habitualmente, demonstra excelentes resultados em pacientes imunocompetentes. Between the years 1958 and 1973, 119 patients with primary invasive cancer of the vulva were treated at the University Hospital of the West Indies, Kingston, Jamaica. Seventy-seven (64.7%) were premenopausal women. Because the pattern of the disease differed from that in late middle and older aged Jamaican women, the results of the disease are presented. The analysis stresses that vulvar carcinoma before menopause in our community is rare in the absence of various sexually transmitted granulomatous diseases of the vulva.

Data on 61 cases of carcinoma of the penis treated at the College Hospital, University of the West Indies, are reported, covering a period of six years. The various etiologic factors are discussed, with particular reference to granuloma inguinale. The literature is reviewed, and the observations here set forth are compared with those obtained by other investigators. (AU)
