ABSTRACT

SUMMARY: Anatomic variations in hip joint are one of the risk factors for the occurrence of osteoarthritis of that part of locomotor system. Due to different age structure and a special way of conducting physical activity in the army, it is justified to presume the presence of even other risk factors which relate to the aging process, carrying of oversize weight and performing higher amplitudes of movement under load. The objective of the study related to determining the existence of eventual anatomic variations in the hip joint in soldiers of different age. This cross-sectional study covers 240 soldiers of Montenegro Army, divided into 8 age groups. Gathered data for morphological measures hip width and hip circumference were processed using the basic statistics, multivariate and univariate of variance and Tukey's Honestly Significant Difference test-HSD. The differences were considered statistically significant for p≤0.05. It was determined that for both morphological measures there are differences between soldiers of certain age. This is confirmed also by drastic deviations between certain age groups obtained for the defined level of statistical significance. The most pronounced is the detection of the annual increase of hip width of soldiers which is higher for 0.38 mm compared to general population. Based on the obtained results the existence of anatomic variations in soldiers' hip joints is stated. The confirmed anatomic changes in conjunction with other risk factors may lead to hip osteoarthritis, i.e to soldiers' health condition impairment. It is necessary to conduct longitudinal studies on the same examinees with precise diagnostics.

ABSTRACT

BACKGROUND: Morphological changes characteristic of femoroacetabular impingement (FAI) are common in soccer players. However, the clinical relevance of such anatomical variations is still not well-defined. HYPOTHESIS: We hypothesized that high alpha angle values and/or acetabular retroversion index (ARI) are correlated with rotational range of motion (ROM) of the hip and that there are clinical-radiological diferences between the dominant lower limb (DLL) and nondominant lower limb (NDLL) in professional soccer players. STUDY DESIGN: Cross-sectional. LEVEL OF EVIDENCE: Level 3. METHODS: A total of 59 male professional soccer players (average age 25.5 years, range 18-38 years) were evaluated in the preseason. As main outcome measures, we evaluated the alpha angle and the ARI and hip IR and ER ROM with radiographic analysis. RESULTS: The measurements taken on DLL and NDLL were compared and a significant difference was found between the sides in the ER (P = 0.027), where the DLL measures were 1.54° (95% CI, 0.18-2.89) greater than the NDLL. There were no significant differences between the sides in the measures of IR (P > 0.99), total ROM (P = 0.07), alpha angle (P = 0.250), and ARI (P = 0.079). The correlations between the rotation measurements and the alpha angle in each limb were evaluated and the coefficient values showed no correlation; so also between the ARI and rotation measures. CONCLUSION: Morphological changes of the femur or acetabulum are not correlated with hip IR and ER ROM in male professional soccer players. ER on the dominant side was greater than on the nondominant side. There was no significant difference in the other measurements between sides. CLINICAL RELEVANCE: In clinical practice, it is common to attribute loss of hip rotational movement to the presence of FAI. This study shows that anatomical FAI may not have a very strong influence on available hip rotational movement in professional soccer athletes.

ABSTRACT

ABSTRACT Introduction: There is increasing evidence that the geometric structure of the hip anatomy plays an important role in the etiology of the fracture. Objective: To systematize the most current knowledge regarding the anatomical characteristics of the radiographic parameters of the hip joint, and their relationship with the fracture. Methods: A documentary research was carried out, with the scientific articles published, in the last 5 years, in medical computer databases such as PubMed, Ebsco and SciELo. Results: Most of the publications analyze the cervicodiaphyseal angle and the axis of the hip. Other measurements analyzed are the femoral axis, the length and width of the femoral neck, as well as acetabular measurements. There is no consensus on the length of the femoral neck or the femoral shaft, despite being an important component of the structure. Knowledge of the particularities of the anatomy and biomechanical characteristics of the hip allows to establish basis for understanding the factors that affect this joint. Conclusions: The studies that have been carried out on the characteristics of the structural components show that there is association between their dimensions and the occurrence of hip fracture, in some cases independent of bone mineral density(AU)

ABSTRACT

PURPOSE: To to assess whether there are any significant differences in hip joint space width (JSW) between weight-bearing versus supine pelvic radiographs. METHODS: Standing and supine anteroposterior pelvic radiographs of 86 patients (146 hips) were included. Sample size was sufficiently powered to assess for equivalence between standing and supine films for JSW measurements made at the medial, lateral, and central aspects of the sourcil line. Measurements were made by 2 independent reviewers blinded to patient positioning. Each reviewer repeated a subset of the measurements to assess intra-rater reproducibility. Mean differences in joint space measurements between standing and supine radiographs were reported for each point of the sourcil. Intraclass correlation coefficients (ICCs) for inter and intra-rater reliability were also calculated. RESULTS: There were no significant differences between JSW measurements made on standing and supine pelvic radiographs (P = .468). Furthermore, equivalence testing demonstrated statistical equivalence between standing and supine JSW measurements made based on an equivalence threshold of ±0.5 mm. Inter-rater reliability demonstrated good agreement with an overall ICC of 0.775 (95% confidence interval [CI] 0.734-0.809). Intra-rater reliability also demonstrated good agreement with ICCs of 0.84 (95% CI 0.758-0.889) and 0.798 (95% CI 0.721-0.851) for the 2 reviewers, respectively. CONCLUSIONS: JSW measurements on standing and supine pelvic radiographs were not significantly different, and their inter-rater agreement and intra-rater reproducibility demonstrated good reliability and repeatability. Therefore, either may be used to assess JSW, including measurements that may impact treatment decisions for hip arthroscopy. LEVEL OF EVIDENCE: Level III; retrospective comparative study.

ABSTRACT

The aim of this study was to test the accuracy of a new automated computer software tool for the assessment of passive hip laxity. The hip laxity was estimated using the dedicated computer software by two blinded evaluators, one previously trained and one without specific training for distraction index measurement, in two independent sessions using 230 hip joints from 115 dogs that underwent screening for passive hip laxity using the distraction view. Previously, all of these radiographs were sent to PennHIP Analysis Center for an official distraction index record. The measurement repeatability of the two sessions was adequate for both evaluators. The reproducibility of the official distraction index measurement, mean distraction index±standard deviation 0.44±0.15, was adequate (P>0.05) for the trained evaluator, 0.44±0.15, and non-adequate (P<0.05), for the non-trained evaluator 0.47±0.17. The distraction index measurement tool proposed can be used with confidence for hip laxity evaluation by trained evaluators, as it provided good repeatability and reproducibility of official reports. The simplicity of the process described leads to a less time-consuming and more affordable procedure.(AU)

ABSTRACT

The aim of this study was to determine some radiographic reference values for hip joint, femur and patella in maned wolves. Ten maned wolves (Chrysocyon brachyurus), five live (G1) and five cadavers (G2), were used. There were no statistically significant differences in the radiological measurements between right and left pelvic limbs of the G1 and G2, except for mechanical lateral proximal femoral angle (mLPFA) in both Groups. The comparison of the measurements between Groups showed difference only for mechanical lateral distal femoral angle (mLDFA). Norberg angle values were 113.1° and 112.9° for G1 and G2. The values of femoral angle of inclination by Hauptman A and Hauptman B methods were 139.7° and 128.6° for G1, and 139.3° and 128.7° for G2. Femoral varus angle values were 5.5° for G1 and 3.9° for G2. The values of anatomic lateral distal femoral angle and anatomic lateral proximal femoral angle were 96.8° and 95.2° for G1, and 95.0° and 95.6° for G2. The values of mLDFA for G1 and G2 were 96.5° and 95.7°. The values of mLPFA were 95.8 (right) and 93.5 (left) for G1, and 95.7 (right) and 93.8 (left) for G2. The values of patellar ligament length, patellar length and L:P ratio were 4.6, 2.4 cm and 1.9 for G1, and 4.4, 2.3 cm and 1.8 for G2. In conclusion, the radiographic reference values described for the hip joint, the femur and the patella, may be useful in the diagnosis of diseases and deformities in maned wolves.

ABSTRACT

Introduction: the overload of the femur proximal epiphysis generates deforming forces on the anatomic characteristics that together with the loss of bone mineral mass result in fractures´ occurrence. There is an increase of the evidence that hip anatomy plays an important role in the fracture etiology. Objective: to systematize current knowledge on the anatomic-functional characteristics of the hip joint and their relation with the fracture from the radiological point of view. Materials and methods: a documental research was carried; the objective of study were the classical handbooks of Human Anatomy, Orthopedics and Trauma, and Radiology, and also the scientific articles published in medical databases. Development: the relationship between the hip imaging anatomy and the occurrence of fractures in this anatomical region have been studied for more than 20 years. The studies carried out on the characteristics of the bone structural components, both in their internal configuration as in the external one, showed that there is a relationship between the hip join anatomy and its fracture. Conclusions: knowledge on the anatomic structure, its functional characteristics, and the relationship of its dimensions with the risk of suffering a fracture, provides a solid basis for understanding the mechanisms of their occurrence and for the design of future intervention actions to prevent and treat them (AU).

ABSTRACT

This study describes the osteology and radiology of the pelvic limb in maned wolves. Ten (five live and five dead) maned wolves (Chrysocyon brachyurus), five males and five females, aged from 2 to 7 years old were used. Digital radiographs were taken and recorded for both pelvic limbs in all animals. Osteology was correlated with the radiographic images. The pelvis had a rectangular shape, and the obturator foramen (foramen obturatum) was oval. The femoral neck (collum femoris) was short and thick. The greater trochanter (trochanter major) extended proximally to near the dorsum of the femoral head (caput ossis femoris). The lateral femoral condyle (condylus lateralis) was larger than the medial condyle (condylus medialis), and the intercondylar fossa (fossa intercondylaris) had a slightly oblique orientation. The proximal tibia displayed medial and lateral condyles with the medial larger. The femur was slightly shorter than the tibia. Seven tarsal bones (ossa tarsi) were present, four long metatarsal bones (ossa metatarsalia II - V) and a short first metatarsal bone (os metatarsal I).

ABSTRACT

The morphological assessment of selected parts of the hindlimb of Japanese quails (Temminck & Schlegel, 1849), with particular emphasis on hip joint structures, was performed, using a 128-row scanner (GE Optima Aquilion, Toshiba, Japan). Eight dead 3-month-old Japanese quails were evaluated. During intravital examination of those birds, no hindlimb abnormalities or locomotion disorders were detected. Bird body, which hip joints and other structures of hindlimbs were studied, was examined in posterioranterior position. The following hip joint structures, on both sides, were assessed in this study: hip joint gap width, acetabulum width, femoral head diameter, and femoral shaft width. The applied imaging method allowed accurate assessment of the selected structures. There was low variability between left and right hindlimbs and among individuals. Modern imaging techniques, such as multirow computed tomography (MCT) allows quick intravital assessment of normal and pathological structures of poultry bones and joints.(AU)

ABSTRACT

Slipped capital femoral epiphysis (SCFE) is an orthopedic pathology in which damage of the growth plate leads to the anterosuperior displacement of the femoral body in respect to the femoral head. Despite being a widely studied disease, its etiology is still unknown. This study was carried out to determine the influence of the physeal-diaphysis angle, body mass, the presence of the perichondrial ring, the type of physical activity, and physeal thickness on SCFE. For this purpose, a finite element analysis of the hip joint and the femur-physis interface was carried out. With the computational model, the Von Mises stresses along the growth plate were calculated and subsequently analyzed statistically to find their correlation with the studied factors. It was found that body mass, the type of physical activity, and the presence of the perichondrial ring had more statistical relevance for the physeal stresses than the physeal-diaphysis angle and the physeal thickness. Thus, our work suggests that changes in growth plate inclination and thickness do not influence the etiology of SCFE.

ABSTRACT

Femoroacetabular impingement syndrome (FAI) is a clinical entity that has been recognized in recent years as a frequent cause of pain and the early development of hip arthrosis. Subspine hip impingement is characterized by the prominent or abnormal morphology of the anteroinferior iliac spine (AIIS), which contributes to the development of a clinical picture that is similar to FAI. The aims of this study were to propose a new morphological classification of the AIIS, to determine the prevalence of the different AIIS morphologies based on this classification and to correlate the presence of said morphologies with different gender and age groups. The sample consisted of 458 hemipelvises from individuals of known age and sex (264 men and 194 women). Each specimen was analyzed to determine the prevalence of each of the different morphologies of the AIIS based on the classification proposed as Type 1: the presence of a concave surface between the AIIS and the acetabular rim; Type 2A: the presence of a flat surface between the AIIS and the acetabular rim; Type 2B the presence of a convex surface between the AIIS and the acetabular rim; and Type 3: the AIIS protrudes inferiorly toward the anterior acetabulum. A prevalence of 69.87% was determined for Type 1 AIIS (320/458). In regard to abnormal morphology, prevalences of 17.90% (82/458), 3.71% (17/458) and 8.52% (39/458) were determined for type 2A, Type 2B and Type 3, respectively. The prevalence of abnormal AIIS morphology was 30.30% (80/264) in male specimens and 29.90% (58/194) in female specimens. This study demonstrates the prevalence of the different morphologies of the AIIS, providing information that will be useful in determining the role of the AIIS in the emergence of subspine hip impingement.

ABSTRACT

PURPOSE: To evaluate hip parameters such as vertical centre of rotation (VCR), horizontal centre of rotation (HCR), femoral offset, and leg length after total hip arthroplasty (THA) using the Nanos short-stem prosthesis. METHODS: Medical records of 73 men and 74 women aged 25 to 92 (mean, 63) years who underwent THA using the Nanos short-stem prosthesis by a single surgeon were reviewed. Prior to the surgery, the optimal cup and stem size, head length, and level of the neck osteotomy were determined using radiographs. Intra-operatively, the leg length and femoral offset were checked, and the level of neck resection and head length were adjusted. VCR, HCR, femoral offset, and leg length of the operated and contralateral sides were compared. Functional outcomes were assessed using the Harris Hip Score (HHS). RESULTS: Compared with the normal contralateral hips, the operated hips had a mean increase of 0.4 mm in VCR (p=0.032), a mean decrease of 1.4 mm in HCR (p=0.027), a mean increase of 0.6 mm in femoral offset (p=0.043), and a mean increase of 0.36 mm in leg length (p=0.035). For these respective parameters, the difference between the normal contralateral side and the operated side was within 5 mm in 89%, 80%, 71%, and 96% of patients. The HHS improved from a mean of 53 to 91 at one year (p<0.001). CONCLUSION: THA using the Nanos short-stem prosthesis enabled restoration of hip anatomy (VCR, HCR, femoral offset, and leg length).

ABSTRACT

The stereoscopic morphology of the iliofemoral, pubofemoral, and ischiofemoral ligaments in hip joint is hard to understand, either in anatomy books or from cadaver dissection. To aid in understanding, three-dimensional models were built from the sectioned images of cadaver hip, which demonstrate fine components better than CTs and MRIs. In the female data of Visible Korean project, the ligaments and neighboring structures were recognized and outlined in detail. Successively, the outlines were stacked and reconstructed to manufacture their surface models by using the sophisticated computer method. The surface models were overlapped with the original sectioned images, which enhanced comprehension of the hip joint anatomy. With the surface models, hip joint was hyperextended to suggest the function of the ligaments. The graphic data of the surface reconstructed hip ligaments are expected to be the source of interactive simulators for anatomy and surgery of the hip joint.

ABSTRACT

The contents of this review may appear odd. After a brief description of the coxofemoral joint, the entities discussed include ilioinguinal neuropathy within the context of the nerves that may be damaged during lower abdominal surgery, meralgia paresthetica, piriformis syndrome with the appropriate caveats, trochanteric syndrome, "ischial bursitis" and trochanteric syndrome caused by ischemia. These cases were chosen to stress our belief that rheumatologists are first and foremost internists. We further believe that being current in other pathologies such as peripheral neuropathies and certain vascular syndromes sooner or later benefits our patients.

ABSTRACT

BACKGROUND: Knowledge of the radiogrametric characteristics of a specific skeletal segment in a healthy population is of the utmost clinical importance. The main justification for this study is that there is no published description of the radiogrametric parameter of acetabular anteversion in a healthy Mexican adult population. MATERIAL AND METHODS: A prospective, descriptive and cross-sectional study was conducted. Individuals of both genders older than 18 years and orthopedically healthy were included. They underwent a two-dimensional axial tomographic study of both hips to measure the acetabular anteversion angles. The statistical analysis consisted of obtaining central trend and scatter measurements. A multivariate analysis of variance (ANOVA) and statistical significance were performed. RESULTS: 118 individuals were studied, 60 males and 58 females, with a mean age of 47.7 +/- 16.7, and a range of 18-85 years. The anteversion of the entire group was 18.6 degrees + 4.1 degrees. Anteversion in males was 17.3 degrees +/- 3.5 degrees (10 degrees - 25 degrees) and in females 19.8 degrees +/- 4.7 degrees (10 degrees - 31 degrees). There were no statistically significant differences (p < or = 0.05) in right and left anteversion in the entire group. However, there were statistically significant differences (p > or = 0.005) both in the right and left sides when males and females were compared. CONCLUSIONS: Our study showed that there are great variations in the anteversion ranges of a healthy population. When our results are compared with those published by other authors the mean of most measurements exceeds 15 degrees. This should be useful to make therapeutic decisions that involve acetabular anteversion.

ABSTRACT

OBJECTIVE: To identify anatomical changes and skeletal maturity through radiographic analysis, allowing more accuracy for indication of surgical management of non-slipped hips in patients with epiphysiolisys. METHODS: A retrospective study of the radiographs of 61 patients followed until the end of skeletal growth, assigned to two groups: 37 patients with unilateral epiphysiolysis, and 24 patients with contralateral epiphysiolysis diagnosed during follow up. The skeletal maturity was evaluated using pelvis radiographs (Oxford method) and compared between the groups for patients of the same gender. In addition, the Southwick angle (in anteroposterior and in Lauenstein view), physeal sloping angle and physeal posterior sloping angle were compared as well. RESULTS: Skeletal maturity showed a statistically significant difference between the two groups for both genders. It was observed that the lateral view of the Southwick angle is mathematically equal to the physeal posterior sloping angle, and were the only ones to show relevant differences between the groups. CONCLUSION: The Oxford method and the Southwick angle in Lauenstein view can be utilized as parameters to help the physician to better indicatethe profilatic surgical treatment of the contralateral hip, in patients with slipped capital femoral epiphysis (SCFE). Level of evidence III, Diagnostic Study.
