ABSTRACT

The findings obtained by ultrasound, CT and angiographic examinations of 75 patients with dissecting aortic aneurysms operated on at the M.M.A. are analysed. The aim of each particular method was to detect the presence of dissection, the aortic entrance tear and type of dissection. These findings were compared with the operative findings. On the basis of the analysis it has been concluded that the precision of the diagnosis obtained by angiography was 93%, by CT 74% and by ultrasound 69%.

ABSTRACT

In the controlled field experiments 18 sheep were exposed to the detonation of the aerosol explosive generating the blast wave necessary to produce death in 50% of cases. The time of death and characteristic damages on the predilection sites of some organs are interpreted as blast type injuries. General response of the body to blast wave was similar to the one produced by other etiological factors. A special characteristic of the experiment was the frequency and severity of pathologic changes on the internal organs of the chest and abdomen. According to the authors' opinion the severity and type of pathologic changes could be explained by effects of the complex blast wave generated by detonation of the aerosol explosives.

ABSTRACT

The patient was admitted to the M.M.A. for correction of defects of both hands seven days after injury caused by explosion. Acute abdomen developed on the same day of admission and laparotomy was performed immediately. Perforations of the cecum and distal part of the sigmoid were found. Based on the data about the time and type of injury sustained it has been concluded that secondary perforations of the colon after blast abdominal injury were present.

ABSTRACT

Esophageal stapler transection was performed in 15 patients with bleeding from the dilated esophageal veins in the period 1984-1988. In 7 patients operated on as urgent cases due to prolonged uncontrolled bleeding, the mortality was high--71.4%, contrary to the group of electively operated patients in whom the mortality was 12.5% with a low postoperative morbidity and successful prevention of repeated bleedings.

ABSTRACT

In the experiments on pigs wounds on the muscle-connective tissues of the thighs were made at 50 meter range by projectiles of military Rifles M 48, M 70, M 16A FAL (the length of the wound canal from 214 to 228 mm), M 91/30 and Dragunov (the length of the wound canal from 160 to 164 mm). The mass of tissue excised at primary surgical debridement was taken as a criterium of the primary devitalization of tissue, the amount of the transferred energy was calculated as well as other parameters of the terminal ballistics. Shooting through the glycerine-soap blocks under the same conditions were parallelly performed. The examinations have shown that the degree of the primary mechanical injury depended on the amount of the transferred energy, which depended not only on the high initial velocity of the bullet but also on the disintegrated, deformed and fragmented bullet after passage through the tissue. The mass of the excised tissue can be a reliable criterium for evaluation of wound severity. There is a good correlation with the volume of the cavity in the soap blocks so that they can be used in experiments as simulant biological media.
