ABSTRACT

BACKGROUND & AIMS: Evidence on the association between nut consumption and depression is mainly based on cross-sectional studies. This study aims to analyse whether nut consumption is prospectively associated with the risk of depression in adults. METHODS: This study was conducted using the United Kingdom (UK) Biobank resource. Data from middle-aged and older UK adults who participated in this cohort between 2007-2012 (baseline) and 2013-2020 (follow-up) were analysed. Baseline information on nut consumption was obtained with the Oxford WebQ 24-h questionnaire. Depression, defined as a self-reported physician diagnosis of depression or antidepressant use, was assessed at baseline and follow-up. Hazard regression models estimating the predictive ability of nut consumption for the risk of developing depression were adjusted for sociodemographic, lifestyle, and health confounders. RESULTS: A total of 13,504 participants (mean age 57.5 ± 7.2 years, 50.7% female) free of depression at baseline were included in the analyses. After a mean follow-up of 5.3 ± 2.4 years, 1122 (8.3%) incident cases of depression were identified. Compared with no nut consumption, the daily consumption of >0 to 1 serving of 30 g of nuts was associated with a lower risk of depression (hazard ratio, HR = 0.83; 95% confidence interval, CI: 0.71-0.97) regardless of all potential confounders considered. In stratified analyses, a decreased risk of depression was more clearly observed in UK adults with adequate weight control, a healthy lifestyle, and better health status than in their counterparts (p < 0.05). CONCLUSIONS: Low-to-moderate nut consumption (>0 to 1 serving of 30 g/day) was associated with a 17% lower risk of depression during a 5.3-year follow-up compared with no nut consumption in a large sample of middle-aged and older UK adults. This protective association is enhanced in the absence of other known risk factors for depression.

Resumen La viudez está catalogada como uno de los eventos de vida adversos más estresantes. El cambio de estado civil genera un cambio de rol e implica una resignificación de la vida. El presente estudio tuvo como objetivo comprender la forma en que las personas mayores viven y afrontan la viudez y determinar cuál es la relación entre las respuestas psicológicas, el sexo y los estilos de afrontamiento. Se utilizó metodología mixta con un diseño CUALI-cuanti a partir de la teoría fundamentada y análisis log lineal. La muestra estuvo compuesta por 29 personas mayores viudas residentes en Bogotá. Este estudio reporta las respuestas psicológicas de un grupo de personas mayores viudas que facilitan el afrontamiento exitoso a la pérdida de la pareja y aquellas que lo dificultan. Los resultados en la fase cualitativa mostraron que el afrontamiento a la viudez se enmarca en siete factores: sentido de vida, apoyo social, relación previa, satisfacción con el matrimonio, participación en actividades, consecuencias generadas por la pérdida y fortalezas personales. En la fase cuantitativa se encontró una interacción significativa entre respuesta psicológica y afrontamiento, lo que reveló que algunas respuestas psicológicas en particular llevan a un afrontamiento activo o por el contrario, lo dificultan. Las respuestas que se encontraron significativas fueron aislamiento, soledad, seguir adelante y participación en actividades. Los resultados demuestran que el afrontamiento es producto de las interacciones entre los sistemas macro y micro y muestra a la viudez como una transición multifacética. Se discuten las similitudes con estudios anteriores y limitaciones.


Abstract Widowhood is cataloged as one of the most stressful adverse life events; the change of civil status generates a change of role and implies a resignification of life. The aim of this study was to understand how older people experience and cope with widowhood. The research used mixed methods approach with a QUALI- quanti design, through grounded theory and log lineal analysis, with a sample of 29 elderly widows and widowers living in Bogotá. This study reports the psychological responses of a group of elderly widows that facilitate successful coping with the loss of the couple, and the responses that make it difficult. The results in the qualitative stage showed that coping with widowhood is framed in seven factors: sense of life, social support, previous relationship, satisfaction with marriage, participation in activities, consequences generated by the loss and personal strengths. In the quantitative stage a significant interaction between psychological response and coping was found, which revealed that some psychological responses in particular lead to an active coping or, on the contrary, made coping difficult. The answers that were significant were isolation, loneliness, moving forward and participation in activities. The results revealed that coping is a product of the interactions between macro and micro systems and showed widowhood as a multifaceted transition. Similarities with previous studies and limitations are discussed.

ABSTRACT

Dual-energy X-ray absorptiometry (DXA) has been considered a reference method for measuring body fat percentage (BF%) in children and adolescents with an excess of adiposity. However, given that the DXA technique is impractical for routine field use, there is a need to investigate other methods that can accurately determine BF%. We studied the accuracy of bioelectrical impedance analysis (BIA) technology, including foot-to-foot and hand-to-foot impedance, and Slaughter skinfold-thickness equations in the measurement of BF%, compared with DXA, in a population of Latin American children and adolescents with an excess of adiposity. A total of 127 children and adolescents (11⁻17 years of age; 70% girls) from the HEPAFIT (Exercise Training and Hepatic Metabolism in Overweight/Obese Adolescent) study were included in the present work. BF% was measured on the same day using two BIA analysers (Seca® 206, Allers Hamburg, Germany and Model Tanita® BC-418®, TANITA Corporation, Sportlife Tokyo, Japan), skinfold measurements (Slaughter equation), and DXA (Hologic Horizon DXA System®, Quirugil, Bogotá, Columbia). Agreement between measurements was analysed using t-tests, Bland⁻Altman plots, and Lin's concordance correlation coefficient (ρc). There was a significant correlation between DXA and the other BF% measurement methods (r > 0.430). According to paired t-tests, in both sexes, BF% assessed by BIA analysers or Slaughter equations differ from BF% assessed by DXA (p < 0.001). The lower and upper limits of the differences compared with DXA were 6.3⁻22.9, 2.2⁻2.8, and -3.2⁻21.3 (95% CI) in boys and 2.3⁻14.8, 2.4⁻20.1, and 3.9⁻18.3 (95% CI) in girls for Seca® mBCA, Tanita® BC 420MA, and Slaughter equations, respectively. Concordance was poor between DXA and the other methods of measuring BF% (ρc < 0.5). BIA analysers and Slaughter equations underestimated BF% measurements compared to DXA, so they are not interchangeable methods for assessing BF% in Latin American children and adolescents with excess of adiposity.

La variabilidadfenotípica se relaciona con genes modificadores como SMN2 y NAIP.ObjetivosCorrelacionar el número de copias del gen SMN2 y la deleción del gen NAIPcon la severidad de la enfermedad en niños con AME, diagnosticados y en seguimiento.MétodosEn el Hospital de Pediatría Juan P. Garrahan se estudió a 98 niños con diagnósticode AME, categorizados de acuerdo con criterios establecidos a nivelinternacional (Consensus Statement for Standard of Care in Spinal MuscularAtrophy 2007). El número de copias del gen SMN2 se determinó medianteMultiplex Ligation-dependent Probe Amplification (MLPA) y PCR en tiemporeal. La ausencia de NAIP se determinó por MLPA.ResultadosMediante MLPA se pudo determinar el número de copias de SMN2 en todoslos pacientes, con la PCR en tiempo real en 96/98 pacientes. Los resultadosde ambas técnicas fueron equivalentes en 95/96 casos. Todos los AME tipo Ipresentaron 2 copias, mientras que más del 95% de los AME tipo II y III mostraronentre 3 y 4 copias de SMN2. Sólo en el grupo AME tipo III se observaron4 copias de SMN2. El gen NAIP estuvo ausente en el 76% de los AME tipo I ypresente en más del 89% de los AME tipo II y III.ConclusionesEl número de copias del gen SMN2 se correlacionó con el fenotipo de AME(más leve a mayor número de copias de SMN2). La ausencia del gen NAIP seasoció con el fenotipo más severo. La metodología MLPA resultó ser más robustaque la PCR en tiempo real y proporcionó mayor información acerca dela región AME.

ABSTRACT

The pyometra is a rare condition, with an incidence of less than 1%. In patients with cervical cancer, spontaneous rupture of pyometra manifests as a generalized peritonitis, which is extremely rare, in the literature only seven cases are described. This paper reports the case of a patient with a history of postmenopausal vaginal bleeding one month before her admission to the hospital; she attended because of acute abdomen. The CT scan reported air in the abdominal cavity and the uterus with air at the periphery, so she underwent an exploratory laparotomy in which purulent material was found with two perforations in the uterine fundus. She underwent total abdominal extrafacial hysterectomy with histopathological diagnosis of keratinizing squamous cell carcinoma, moderately differentiated.

ABSTRACT

Intraligamentary pregnancy is a rare form of ectopic pregnancy and a type of abdominal pregnancy, preoperative diagnosis is very difficult. This paper describes the case of a 24-year-old woman with a history of uterine curettage uncomplicated; she consulted due to abdomina pain in the right lower quadrant of seven days of duration, which gradually increased in intensity being 10/10 visual scale analogous (VAS). Obstetrical ultrasound was performed reporting: ectopic localization to annex right of 15.2 weeks gestational age, with heart rate of 152 beats per minute. The patient was admitted and underwent to emergency exploratory laparotomy, which found right paratubaric ectopic pregnancy. The histopathology report was of broad ligament ectopic pregnancy. Specimen of 9 x 8 x 7.5 cm and 200 grams.

OBJECTIVE: To evaluate whether advertising for food influences choices made by children, the strength of these influences, and whether they might be easily undone by parental influences. STUDY DESIGN: Children between 3 and 8 years of age (n=75) were randomized to watch a series of programs with embedded commercials. Some children watched a commercial for a relatively healthy food item, the other children watched a commercial for a less healthy item, both from the same fast-food company. Children were also randomized either to receive parental encouragement to choose the healthy item or to choose whichever item they preferred. RESULTS: Results indicated that children were more likely to choose the advertised item, despite parental input. Parental input only slightly moderated this influence. CONCLUSION: Although advertising impact on children's food choices is moderate in size, it appears resilient to parental efforts to intervene. Food advertisements directed at children may have a small but meaningful effect on their healthy food choices.

INTRODUCCIÓN La atrofia muscular espinal (AME) se clasifica en tres subtipos pediátricos. El 95% de los niños con AME presenta deleción homocigota del gen SMN1. La variabilidad fenotípica se relaciona con genes modificadores como SMN2 y NAIP. OBJETIVOS Correlacionar el número de copias del gen SMN2 y la deleción del gen NAIP con la severidad de la enfermedad en niños con AME, diagnosticados y en seguimiento. MÉTODOS En el Hospital de Pediatría Juan P. El número de copias del gen SMN2 se determinó mediante Multiplex Ligation-dependent Probe Amplification (MLPA) y PCR en tiempo real. La ausencia de NAIP se determinó por MLPA. RESULTADOS Mediante MLPA se pudo determinar el número de copias de SMN2 en todos los pacientes, con la PCR en tiempo real en 96/98 pacientes. Los resultados de ambas técnicas fueron equivalentes en 95/96 casos. Todos los AME tipo I presentaron 2 copias, mientras que más del 95% de los AME tipo II y III mostraron entre 3 y 4 copias de SMN2. Sólo en el grupo AME tipo III se observaron 4 copias de SMN2. El gen NAIP estuvo ausente en el 76% de los AME tipo I y presente en más del 89% de los AME tipo II y III. DISCUSIÓN El número de copias del gen SMN2 se correlacionó con el fenotipo de AME (más leve a mayor número de copias de SMN2). La ausencia del gen NAIP se asoció con el fenotipo más severo. La metodología MLPA resultó ser más robusta que la PCR en tiempo real y proporcionó mayor información acerca de la región AME.

ABSTRACT

Tonsil Actisil FF, a commercial bentonitic clay, promotes the formation of a series of electrophilic-aromatic-substitution products from para-methoxybenzyl acetate in carbon disulfide. The molecules obtained correspond to linear isomeric dimers, trimers, tetramers and a pentamer, according to their spectroscopic data. A clear indication of the title mechanistic pathway for the oligomerization growth was obtained from the analysis of a set of computational-chemistry calculations using the density-functional-theory level B3LYP/6-311++G(d,p). The corresponding conclusions were based on the computed dipole moments, the HOMO/LUMO distributions, and a natural-populations analysis of the studied molecules.

The study of the feeding ecology of amphibians is an old issue in herpetology. Notwithstanding, the lack of food resources data in many studies of amphibians feeding has lead to partial understanding of frog feeding strategies. In this study we evaluate the trophic selectivity of a red spotted green frog (Hypsiboas punctatus) population from a Middle Paraná River floodplain pond in Argentina, and discuss the importance of prey availability data when interpreting results from diet analysis. We analyzed the gut contents of 47 H. punctatus adults and compared frog's diet with the environmental food resources. Prey availability was estimated by systematically seep-netting the microhabitat where anurans were localized foraging. We identified 33 taxonomic categories from gastrointestinal contents. Numerically, the most important prey categories were dipterans, followed by hemipterans, homopterans and coleopterans. The diet similarity between males and females was high and no statistical differences in diet composition were found. The most abundant food resources in the environment were dipterans, coleopterans, homopterans and collembolans. In order to assess whether frogs were selecting their preys, we calculated Pianka's niche overlap index and Jacobs' electivity index comparing gut contents to prey availability data. Trophic niche overlap was medium but significantly higher than expected by chance. The electivity index indicated that H. punctatus foraged dipterans slightly above their environmental abundance. Among the secondary preys, hemipterans were foraged selectively, homopterans were consumed in the same proportion to their occurrence in the environment, coleopterans were foraged quite under their availability and collembolans were practically ignored by frogs. Without food resources data, H. punctatus could be classified as a specialist feeder, but dipterans also were quite abundant in the environment. Our results show that H. punctatus fit better as a generalist feeder, foraging on their main food item and some secondary preys in similar proportion to their environmental availability; even though other secondary preys are being selectively preferred or ignored by frogs. Our data illustrate the importance of including the resource availability data on diet studies to improve the understanding of amphibian feeding ecology.

La falta de datos sobre la disponibilidad de recursos alimenticios en muchos trabajos sobre ecología trófica de anfibios ha llevado a una comprensión parcial de las estrategias alimentarias de este grupo. Este estudio evalúa la selectividad trófica de una población de Hypsiboas punctatus en una laguna del valle aluvial del río Paraná Medio en Argentina, y discute la importancia de los datos de disponibilidad de presas para interpretar las estrategias de alimentación de las ranas. Se analizaron los contenidos gastrointestinales de 47 adultos, y se compararon con la disponibilidad ambiental de presas, estimada mediante muestreo sistemático del micro-hábitat de forrajeo de los anuros. Se identificaron 33 categorías de presas. Las más importantes fueron dípteros, hemípteros, homópteros y coleópteros. Se encontró una elevada similitud entre la dieta de machos y hembras, y no se hallaron diferencias significativas en su composición. Los recursos alimenticios más abundantes fueron los dípteros, coleópteros, homópteros y colémbolos. La superposición de nicho trófico fue media (índice de Pianka) pero significativamente más elevada que la esperada por azar. H. punctatus consumió dípteros en una proporción levemente superior a su disponibilidad ambiental (índice de selectividad de Jacobs). La estrategia de depredación de H. punctatus se ajusta mejor a una táctica generalista, ya que su principal ítem presa y algunos ítems secundarios fueron capturados en proporciones similares a su disponibilidad ambiental, y solo una porción reducida de su espectro de recursos tróficos fue consumida selectivamente. Estos resultados evidencian la importancia de incluir los datos de disponibilidad de recursos en los estudios de alimentación para lograr un mejor entendimiento de la ecología trófica de los anfibios.

El centro de Salud de Mineros San Juan ubicado en la zona Sud Este de la ciudad de Cochabamba en el Km. 10 camino antiguo a Santa Cruz en la zona de Valle Alto, es un centro que presta atencion de primer nivel a las comunidades de Uspha Uspha Central, Urbanizacion Concordia, Llave Mayu, Olimpia, etc. Este informe resume todas las actividades desarrolladas desde el 15 de julio al 15 de octubre del 2004. Detalla las caracteristicas mas importantes que tiene, el personal con el que cuenta, su infraestructura, equipamiento, el trabajo que desarrolla en las comunidades a las que presta servicios.

El propoleo ayuda en la curacion de la gastritis, ya que la sintomatologia desaparecio a partir del septimo dia de inicio del tratamiento hasta despues de los 14 dias de tratamiento con propoleo en ambos tipos de gastritis. Se pudo observar que con la administracion del propoleo a una dosis de 10 gotas dos veces al dia en la mayoria de los pacientes hubo buenos resultados, ya que los pacientes mencionaron que los sintomas desaparecieron hasta los 15 dias despues de inicio del tratamiento. Los pacientes con gastritis aguda que recibieron propoleo a 15 gotas una ves en el dia, mencionaron que los sintomas desaparecieron entre los 8 a 14 dias de haber recibido el propoleo. Mientras que los pacientes con gastritis cronica la sintomatologia tardo un poco en desaparecer. Aquellos pacientes que recibieron tratamiento farmacologico en un 50 por ciento la sintomatologia desaparecio, despues de los 30 dias. Con el tratamiento farmacologico, el tratamiento dura mucho mas en la gastritis cronica que en la aguda

ABSTRACT

Multiple endocrine neoplasia type 2A (MEN 2A) and familial medullary thyroid carcinoma (FMTC) are characterized by development of medullary thyroid carcinoma (MTC) and caused by germline RET mutations. Patients with MEN 2A also develop pheochromocytoma and/or hyperparathyroidism (HPT). However, MEN 2A-affected individuals could display the FMTC phenotype at first clinical manifestation. To establish the correct phenotype and improve clinical management of patients affected by hereditary MTC, clinical screening, RET mutational analysis, penetrance of MTC, and genotype-phenotype correlation were performed in a large, suspected FMTC kindred of 86 individuals. Germline C634Y RET mutation was confirmed in 22 individuals, 15 of whom were thyroidectomized when high serum calcitonin levels were detected. MTC was confirmed in 12 individuals and C-cell hyperplasia in 3. HPT was detected in two patients. High penetrance of MTC at young age (79% at 30 yr of age) was found. This family was considered to be affected by FMTC for several years because MTC was the sole clinical manifestation. However, our results allowed reclassifying the family as MEN 2A, thereby improving clinical management of family members. Our findings regarding penetrance and genotype-phenotype correlation suggest that patients considered to have FMTC may in fact have MEN 2A in some kindreds.

ABSTRACT

BACKGROUND: Different RET oncogene mutations have been found to be associated with inherited medullary thyroid carcinoma (MTC) in the context of three different syndromes including multiple endocrine neoplasia types 2A (MEN 2A) and 2B (MEN 2B) and familial medullary thyroid carcinoma (FMTC). These mutations have been recorded in different populations, but to date there is no corresponding study in Mexican families. Our purpose was identification of RET mutations in Mexican families with inherited or sporadic MTC (SMTC) and search for RET protein expression as prognostic marker in MTC tumors. METHODS: Nine unrelated families with MTC corresponding either to two MEN 2A, three MEN 2B, or four SMTC were studied. Screening of exons 10, 11, and 13-16 of RET oncogene in DNA from circulating lymphocytes and tumor samples were analyzed. Immuno- staining for RET was performed in the corresponding tumor. RESULTS: Germline 918 ATG-->ACG RET mutation was present in three unrelated MEN 2B individuals and corresponding somatic mutation in one individual with SMTC; 634 TGC-->TTC RET mutation was detected in two related patients in an MEN 2A family and the 634 TGC-->TAC RET mutation was detected in 12 related individuals from a second MEN 2A family. RET protein expression was detected in all MTC tumors showing different staining intensity. CONCLUSIONS: RET mutations found in Mexican patients with MTC are similar to those previously reported in several MTC families worldwide. This indicates that RET mutations are highly conserved and that MTC etiology does not depend to a great extent on environmental factors or ethnic differences. Detection of RET protein in MTC tissue sections is not useful as prognostic marker.

ABSTRACT

Analiza la composición de las redes y el apoyo social de ancianos enfermos de escasos recursos en Guadalajara, México. Se entrevistaron a 40 ancianos durante su hospitalización con un cuestionario de preguntas abiertas. Las redes sociales de estos ancianos están compuestas por 7,5 integrantes en promedio, principalmente mujeres, son redes multigeneracionales e incluyen a la familia extensa. Las redes en su mayoría proporcionan apoyo emocional. Las ancianas tienen redes más grandes y reciben más apoyo social en comparación a los ancianos varones; los ancianos casados y viudos, a su vez, tienen redes más grandes y reciben más apoyo social que los solteros. No hay diferencias por la edad y con quien vive en la unidad doméstica. Se requieren futuros estudios sobre los aspectos culturales y sociales sobre el tema.

ABSTRACT

El propósito de realizar este estudio es el de ofrecer al paciente hospitalizado y con catéter central, la mejor técnica de curación en la que se utilice el apósito-adhesivo ideal, para la fijación del catéter y prevención de complicaciones, además de permitirle al personal de enfermería reducir tiempo y esfuerzo. El estudio de investigación consistió en comparar dos técnicas de curación de catéter de nutrición parenteral. Se realizó en el Hospital Central Militar con pacientes que tenían colocado un catéter para nutrición parenteral, hospitalizados en diferentes servicios de esta institución, el mes de diciembre de 1993 al mes de mayo de 1994. La visibilidad, buena adherencia y permeabilidad, que mantiene seco el sitio de inserción y la piel del paciente manejados con apósitos-película transparentes dieron como resultado la permanencia hasta por 5 días sin complicaciones y en consecuencia con menos riesgo de infección, comparado con el uso de tela adhesiva y Micropore. De esta forma se reducen los costos adicionales asociados con cambios de apósitos y el tratamiento de las complicaciones. Se denominó la técnica ®A¼ a la utilizada con tela adhesiva y Micropore con curación diaria y técnica ®B¼ a la que se manejó con op-site I.V. 3000 y tegaderm curándose cada 5 días
