ABSTRACT

Background: Idiopathic spinal cord herniation (ISCH) is a rare condition that is characterized by ventral herniation of the spinal cord through a defect in the dura mater into the epidural space, with no identifiable cause. ISCH is frequently underdiagnosed, and the information available in case reports is limited. To provide an overview of the clinical manifestations and diagnosis of this condition, this study aims to conduct a review of reported cases of ISCH. Methods: A literature review was carried out using seven databases. The search was conducted using the keywords "Idiopathic spinal cord herniation" OR "Idiopathic Ventral Spinal Cord Herniation" AND "Case report" OR "case series." Results: A total of 92 relevant papers reporting 224 cases, besides the index case, were determined. Of the cases, 58.5% were females and the mean age was 50.7 (SD 13.2) years. Symptoms, diagnoses, and outcomes were similar between genders. The most common clinical signs included motor symptoms (82.6%), instability (61.3%), hypoesthesia (59.2%), and disturbance of thermal sensitivity (47.3%). Brown-Séquard syndrome was observed in 27.2% of the cases, and surgical treatment was employed in 89.7% of the cases. Conclusions: ISCH is a pathology that is principally treated with surgical approach. This study provides valuable insights into the clinical manifestations and diagnosis of ISCH, which can aid in the early recognition and treatment of this rare condition.

ABSTRACT

PURPOSE: To report an unusual absent right common carotid artery with embryological and clinical emphasis. METHODS: A 63-year-old female with a multilobulated anterior communicating artery saccular aneurysm was referred to our center for definite treatment. An analysis of her carotid system on CTA and DSA with 3D modeling was performed to assess the embryology of an absent right common carotid and its association with aneurysm development. RESULTS: Cerebral angiogram demonstrated an absent right common carotid artery and separate origin of the internal and external right carotid arteries arising from the brachiocephalic trunk. CONCLUSION: Absence of the common carotid artery is an uncommon anatomical variant, usually asymptomatic and commonly associated with other vascular abnormalities. It is important to be familiar with the association between this anatomical variant given its hemodynamic stress, high risk of stroke, and aneurysm formation.

ABSTRACT

BACKGROUND: The registry of cerebral aneurysms <5 mm, known for their low risk of rupture, is significant, given their high incidence globally. Our study aimed to identify, in small aneurysms (<5 mm), the potential morphological characteristics, risk factors that can predict the risk of rupture, and the risk or benefit of treating them with endovascular or conservative treatment in ruptured and unruptured intracranial aneurysms. METHODS: The medical records of patients with cerebral aneurysms <5 mm were retrospectively reviewed between January 2014 and December 2022 at two neurovascular centers in Colombia. We evaluated clinical and angiographic outcomes using statistical tests. RESULTS: Two hundred fifty-six patients (425 intracranial aneurysms) were registered in the database. Two hundred and seventy-five IA were treated with endovascular treatment: 70 ruptured aneurysms and 205 unruptured aneurysms. One hundred fifty intracranial aneurysms underwent conservative treatment (follow-up). Women accounted for 82.1% of cases. Most cases were incidentally diagnosed (83.5%). After a year of follow-up, 87.3% of unruptured and 67.1% of ruptured intracranial aneurysms had an mRS 0-2. In the Raymond-Roy occlusion classification, among 101 unruptured intracranial aneurysms embolized were 53 cases class I, and among 66 ruptured intracranial aneurysms embolized, 67.1% were class I. CONCLUSION: Endovascular therapy for aneurysms <5 mm appears to be a technically feasible treatment, with satisfactory occlusion rates and few re-treatments at the 12-month follow-up. The complication rates were similar to those reported in studies on small aneurysms.

ABSTRACT

The purpose of this study was to assess the effects of protecting the facial nerve with a modified endaural approach with a peripheral dissection of the superficial musculoaponeurotic system to access the temporomandibular joint which allows an excellent operative field of visualization, multiple surgical procedures of the temporomandibular joint, and general care which improves the immediate postoperative period, making this a less morbid surgery. This study included 33 patients (39 sides) who underwent surgical treatment for disorders of the temporomandibular joint from the years 2021 to 2023 at the maxillofacial department of the clinic "Colsanitas" located in Bogota, Colombia. Therapeutic results were evaluated by postoperative facial nerve injury, with the House-Brackman scale; every patient was examined for adequate facial musculature function immediately after surgery. Notably, zero patients presented facial nerve injury. These results imply that the modified endaural approach with a peripheral dissection of the superficial musculoaponeurotic system to access the temporomandibular joint reduces the incidence of facial nerve injuries, improves operative site exposure, and lowers the frequency of complications.

ABSTRACT

Triple negative breast cancer (TNBC) is highly immunogenic and high levels of tumor infiltrating lymphocytes (TILs) have been associated with a better prognosis and higher probability to achieve pathological complete response. Here, we explore the potential role of stromal TILs level and composition as a prognostic and predictive biomarker in TNBC. 195 Tumor biospecimens from patients diagnosed with TNBC were included. Stromal TILs (sTILs), positive CD4/CD8 cells were evaluated. Differences in clinic-pathological characteristics according to immune infiltration were assessed. The predictive and prognostic value of immune infiltration was analyzed by multivariate models. Higher immune infiltration was observed in patients with favorable clinical-pathological features. Survival analysis showed that longer overall survival times were observed in patients with a higher infiltration of sTILs (p = 0.00043), CD4 + (p = 0.0074) and CD8 + (p = 0.008). In the multivariate analysis, low levels of sTILs were found to be associated with a higher mortality hazard (HR: 1.59, 95% CI 1.01-2.48). CD4 and CD8 immune infiltration were associated with higher odds for pathological complete response (OR: 1.20, 95% CI 1.00-1.46, OR: 1.28, 1.02-1.65, respectively). Our results suggest that immune infiltration could be used as a prognostic marker for overall survival in TNBC patients.

ABSTRACT

The adequate three-dimensional location of the maxilla is one of the main objectives in orthognathic surgery. We propose an instrument manufactured from a Fox Plane (FP) modified with red light in the shape of a cross and a bubble level, in which we perform an occlusal registration for the trans-surgical control of the three-dimensional position of the maxilla, By adding an occlusal registration to the FP, the occlusal plane is transferred and amplified to an extraoral reference. The red light in the shape of a cross allows us to assess the success of our movements more accurately.

ABSTRACT

Construct: High-stakes assessments measure several constructs, such as knowledge, competencies, and skills. In this case, validity evidence for test scores' uses and interpretations is of utmost importance, because of the consequences for everyone involved in their development and implementation. Background: Educational assessment requires an appropriate understanding and use of validity frameworks; however, health professions educators still struggle with the conceptual challenges of validity, and frequently validity analyses have a narrow focus. Important obstacles are the plurality of validity frameworks and the difficulty of grounding these abstract concepts in practice. Approach: We reviewed the validity frameworks literature to identify the main elements of frequently used models (Messick and Kane's) and proposed linking frameworks including Russell's recent overarching proposal. Examples are provided with commonly used assessment instruments in health professions education. Findings: Several elements in these frameworks can be integrated into a common approach, matching and aligning Messick's sources of validity with Kane's four inference types. Conclusions: This proposal to contribute evidence for assessment inferences may provide guidance to understanding the use of validity evidence in applied settings. The evolving field of validity research provides opportunities for its integration and practical use in health professions education.

Resumen La evaluación es un proceso sistemático que resulta en un juicio de valor para tomar decisiones. Los instrumentos empleados para obtener datos sobre el desempeño de los estudiantes requieren de un proceso sistemático y objetivo para su implementación. El mini-CEX es un instrumento de observación directa que ha sido empleado para la evaluación de la competencia clínica en los estudiantes de pre y posgrado desde su invención en 1955. Cuenta con diferentes evidencias de validez para su uso en distintos contextos educativos y clínicos. Permite realizar evaluaciones rápidas, acompañadas de realimentación y que proporcionan información relevante del desarrollo de la competencia clínica. El objetivo de este escrito es exponer la experiencia de la implementación del mini-CEX en el pregrado médico para la evaluación formativa de los estudiantes utilizando la simulación con pacientes estandarizados. Para lograr este objetivo se empleó la siguiente secuencia: búsqueda, planeación, integración y aplicación. Posterior a estos pasos se dan una serie de recomendaciones para la implementación del mini-CEX. Se concluye que la evaluación de la competencia clínica es importante para la mejora continua y permanente de los estudiantes de pre y posgrado. Es necesario sistematizar la evaluación ajustada siempre a objetivos y necesidades específicas de la evaluación.


Abstract Evaluation is a systematic process that results in a judgment to make decisions. The instruments used to obtain data on student performance require a systematic and objective process for their implementation. The mini-CEX is a direct observation tool that has been used for the evaluation of clinical competence in undergraduate and postgraduate students since its invention in 1955. It has different validity evidence for use in different educational and clinical contexts. It allows rapid evaluations, accompanied by feedback and providing relevant information on the development of clinical competence. The objective of this paper is to expose the experience of the implementation of the mini-CEX in the medical undergraduate for the formative evaluation of students using simulation with standardized patients. To achieve this goal, the following sequence was used: search, planning, integration, and application. After these steps we make some recommendations for the implementation of the mini-CEX. Its is concluded that the evaluation of clinical competence is important for the continuous and permanent improvement of undergraduate and graduate students. It is necessary to systematize the evaluation always adjusted to objectives and specific needs of the evaluation.

ABSTRACT

Augmented reality (AR) integrates computer-generated content and real-world scenarios. Artificial intelligence's continuous development has allowed AR to be integrated into medicine. Neurosurgery has progressively introduced image-guided technologies. Integration of AR into the operating room has permitted a new perception of neurosurgical diseases, not only for neurosurgical planning, patient positioning, and incision design but also for intraoperative maneuvering and identification of critical neurovascular structures and tumor boundaries. Implementing AR, virtual reality, and mixed reality has introduced neurosurgeons into a new era of artificial interfaces. Meningiomas are the most frequent primary benign tumors commonly related to paramount neurovascular structures and bone landmarks. Integration of preoperative 3D reconstructions used for surgical planning into AR can now be inserted into the microsurgical field, injecting information into head-up displays and microscopes with integrated head-up displays, aiming to guide neurosurgeons intraoperatively to prevent potential injuries. This manuscript aims to provide a mini-review of the usage of AR for intracranial meningioma resection.

ABSTRACT

Objetivo: Analisar casos de suicídio no Brasil de 2010 a 2021 e as características sociodemográficas deste fenômeno, incluindo os primeiros dois anos da pandemia de COVID-19, para avaliar a existência de relação dos aumentos de casos de suicídio no Brasil com esta doença. Métodos: Estudo ecológico de linhas temporais, com 140.339 casos analisados a partir das bases de dados do Sistema de Informações sobre Mortalidade (SIM). Taxas e regressões estatísticas foram aplicadas no software STATA®. Resultados: Observou-se maiores taxas de suicídio em homens, solteiros, e residentes nas regiões Sul e Centro-Oeste. A faixa etária de 10 a 19 anos teve crescimento de 1,7 casos x 100 mil habitantes/ano. A análise por região revelou variações significativas nas taxas, sugerindo influência de fatores contextuais. Embora a pandemia tenha gerado impactos na saúde mental, não foi possível estabelecer relação direta entre a COVID-19 e o aumento das taxas de suicídio. Conclusão: Esses resultados reforçam a necessidade de políticas públicas e intervenções preventivas, especialmente para grupos vulneráveis, como os jovens. Abordagem multifatorial é sugerida, considerando fatores socioeconômicos, acesso a serviços de saúde mental e redução do estigma associado com doenças mentais. Em suma, este estudo contribui para a compreensão das tendências temporais e características demográficas dos suicídios no Brasil, ressaltando a importância de investigações longitudinais adicionais para melhor compreensão desse fenômeno complexo. Espera-se que essas evidências fortaleçam as políticas de saúde mental e promovam estratégias mais eficazes de prevenção do suicídio.


Objective: To analyze suicide cases in Brazil from 2010 to 2021 and the sociodemographic characteristics of this phenomenon, including the first two years of the COVID-19 pandemic, to assess whether there was a relationship between the increase in suicide cases in Brazil and this illness. Methods: Ecological timeline study, with 140,339 cases analyzed from the Mortality Information System (SIM) databases. Rates and statistical regressions were performed using STATA® software. Results: Higher suicide rates in men, singles, and residents of the South and Midwest regions were observed. The age group of 10­19 years had an increase of 1.7 cases x 100 thousand inhabitants/year. Analysis by region revealed significant variations in rates, suggesting the influence of contextual factors. Although the pandemic affected mental health, it was not possible to establish a direct relationship between COVID-19 and increased suicide rates. Conclusion: These results reinforce the need for public policies and preventive interventions, especially for vulnerable groups such as young people. A multifactorial approach is suggested that considers socioeconomic factors, access to mental health services, and reduced stigma associated with mental illness. In short, this study contributes to understanding the temporal trends and demographic characteristics of suicides in Brazil, highlighting the importance of further longitudinal investigations to better understand this complex phenomenon. Hopefully, this evidence will strengthen mental health policies and promote more effective suicide prevention strategies.

ABSTRACT

Background: The main objective of this study is to enhance neurosurgeons' anatomical knowledge by providing specific anatomical references of the cavernous sinus (CS). However, it is essential to clarify that our study does not seek to establish an absolute intraoperative rule due to the inherent anatomical variability that must be considered. Methods: Fifty-three cadaveric specimens were procured from the Forensic Institute (Bogotá) and subjected to dissection through an extradural approach. The measurements were taken in two distinct phases. The first phase involved the measurement of various anatomical structures in 25 specimens with respect to the anterior and posterior clinoids. The second phase, which was conducted 5 years later, involved the measurement of the distance between the foramen rotundum and the foramen ovale in 28 specimens using the L&W tools microcaliper. Results: In 25 specimens, a perpendicular imaginary line was drawn from the lateral tip of the anterior clinoid to the floor of the medial fossa. This facilitated access to the Parkinson's triangle, which is located between the IV cranial nerve and the ophthalmic V1 nerve, revealing a constant distance of 5 mm between the lateral tip of the anterior clinoid and the IV cranial nerve. Furthermore, in 28 specimens, the mean distance from the foramen rotundum to the foramen ovale was found to be 1.3 cm bilaterally. Conclusion: The rule of five is a valuable tool for comprehensively understanding the anatomy of the CS, providing a reference point for the different normal anatomical structures within the CS.

ABSTRACT

Background: DICER1 alterations are associated with intracranial tumors in the pediatric population, including pineoblastoma, pituitary blastoma, and the recently described "primary DICER1-associated CNS sarcoma" (DCS). DCS is an extremely aggressive tumor with a distinct methylation signature and a high frequency of co-occurring mutations. However, little is known about its treatment approach and the genomic changes occurring after exposure to chemoradiotherapy. Methods: We collected clinical, histological, and molecular data from eight young adults with DCS. Genomic analysis was performed by Next-generation Sequencing (NGS). Subsequently, an additional germline variants analysis was completed. In addition, an NGS analysis on post-progression tumor tissue or liquid biopsy was performed when available. Multiple clinicopathological characteristics, treatment variables, and survival outcomes were assessed. Results: Median age was 20 years. Most lesions were supratentorial. Histology was classified as fusiform cell sarcomas (50%), undifferentiated (unclassified) sarcoma (37.5%), and chondrosarcoma (12.5%). Germline pathogenic DICER1 variants were present in two patients, 75% of cases had more than one somatic alteration in DICER1, and the most frequent commutation was TP53. Seven patients were treated with surgery, Ifosfamide, Cisplatin, and Etoposide (ICE) chemotherapy and radiotherapy. The objective response was 75%, and the median time to progression (TTP) was 14.5 months. At progression, the most common mutations were in KRAS and NF1. Overall survival was 30.8 months. Conclusions: DCS is an aggressive tumor with limited therapeutic options that requires a comprehensive diagnostic approach, including molecular characterization. Most cases had mutations in TP53, NF1, and PTEN, and most alterations at progression were related to MAPK, RAS and PI3K signaling pathways.

Introduction: The rise of soft robotics has driven the development of devices for assistance in activities of daily living (ADL). Likewise, different types of actuation have been developed for safer human interaction. Recently, textile-based pneumatic actuation has been introduced in hand exoskeletons for features such as biocompatibility, flexibility, and durability. These devices have demonstrated their potential use in assisting ADLs, such as the degrees of freedom assisted, the force exerted, or the inclusion of sensors. However, performing ADLs requires the use of different objects, so exoskeletons must provide the ability to grasp and maintain stable contact with a variety of objects to lead to the successful development of ADLs. Although textile-based exoskeletons have demonstrated significant advancements, the ability of these devices to maintain stable contact with a variety of objects commonly used in ADLs has yet to be fully evaluated. Materials and methods: This paper presents the development and experimental validation in healthy users of a fabric-based soft hand exoskeleton through a grasping performance test using The Anthropomorphic Hand Assessment Protocol (AHAP), which assesses eight types of grasping with 24 objects of different shapes, sizes, textures, weights, and rigidities, and two standardized tests used in the rehabilitation processes of post- stroke patients. Results and discussion: A total of 10 healthy users (45.50 ± 14.93 years old) participated in this study. The results indicate that the device can assist in developing ADLs by evaluating the eight types of grasps of the AHAP. A score of 95.76 ± 2.90% out of 100% was obtained for the Maintaining Score, indicating that the ExHand Exoskeleton can maintain stable contact with various daily living objects. In addition, the results of the user satisfaction questionnaire indicated a positive mean score of 4.27 ± 0.34 on a Likert scale ranging from 1 to 5.

Purpose: A semi-supervised two-step methodology is proposed to obtain a volumetric estimation of COVID-19-related lesions on Computed Tomography (CT) images. Methods: First, damaged tissue was segmented from CT images using a probabilistic active contours approach. Second, lung parenchyma was extracted using a previously trained U-Net. Finally, volumetric estimation of COVID-19 lesions was calculated considering the lung parenchyma masks.Our approach was validated using a publicly available dataset containing 20 CT COVID-19 images previously labeled and manually segmented. Then, it was applied to 295 COVID-19 patients CT scans admitted to an intensive care unit. We compared the lesion estimation between deceased and survived patients for high and low-resolution images. Results: A comparable median Dice similarity coefficient of 0.66 for the 20 validation images was achieved. For the 295 images dataset, results show a significant difference in lesion percentages between deceased and survived patients, with a p-value of 9.1 × 10-4 in low-resolution and 5.1 × 10-5 in high-resolution images. Furthermore, the difference in lesion percentages between high and low-resolution images was 10 % on average. Conclusion: The proposed approach could help estimate the lesion size caused by COVID-19 in CT images and may be considered an alternative to getting a volumetric segmentation for this novel disease without the requirement of large amounts of COVID-19 labeled data to train an artificial intelligence algorithm. The low variation between the estimated percentage of lesions in high and low-resolution CT images suggests that the proposed approach is robust, and it may provide valuable information to differentiate between survived and deceased patients.

BACKGROUND: There is currently no case described in the literature of epidural hematoma associated with subarachnoid hemorrhage due to dissection of a spontaneous radiculomedullary artery at the lumbar level and therefore its incidence and prevalence are not known. However, its etiology is thought to be similar and may not be diagnosed given its nonspecific symptomatology. OBSERVATIONS: The authors present the case of an adult patient who consulted the emergency department for 2 weeks of low back pain. On physical examination there were negative signs of radiculopathy without neurological focalization. The patient was evaluated by neurosurgery via thoracic and lumbosacral spine magnetic resonance imaging, with findings of epidural hematoma associated with subarachnoid hemorrhage in the lumbosacral region. After that, panangiography was done. In the study, it was possible to visualize a dissection of the radicular artery at the L2 level. The patient was not a candidate for surgical management and continued with medical pain management. The patient has a good clinical evolution and was discharged. LESSONS: The aim of the present case is to describe a unique case of radiculomedullary artery dissection as a cause of spontaneous epidural hematoma associated with a subarachnoid hemorrhage in the lumbar region.

Introducción: El objetivo principal de la terapia pulpar es mantener la integridad y salud de los dientes y sus estructuras de soporte. En odontopediatría, el tratamiento de molares primarios diagnosticados con pulpitis irreversible o necrosis pulpar representa uno de los grandes desafíos en la práctica clínica debido a la complejidad de la anatomía radicular, tiempo de trabajo y dificultad para la correcta instrumentación. La técnica "Lesion Sterilization Tissue Repair" (LSTR) busca esterilizar la lesión y reparar tejidos por medio del uso de pastas antibióticas sin la necesidad de instrumentar los canales radiculares. Reporte de caso: Paciente femenina de 9 años que asiste a la consulta odontológica asintomática. En el examen clínico el diente segundo molar primario lado mandibular izquierdo (diente #75) presenta lesión activa y cavitada de caries dental, absceso y movilidad grado I. En la radiografía periapical presenta área radiolúcida en la región de la furca, sugiriendo el diagnóstico de necrosis pulpar. Para el tratamiento pulpar se optó por realizar la técnica LSTR con la pasta CTZ, compuesta por cloranfenicol, tetraciclina, óxido de zinc y eugenol. Posteriormente, el molar fue restaurado con una corona de acero inoxidable. Después de 8 meses, el diente se encuentra asintomático, con la restauración satisfactoria, movilidad fisiológica e integridad gingival; radiográficamente hay ausencia de radiolucidez en la región de la furca y reabsorción ósea patológica. Conclusión: el tratamiento endodóntico en el molar primario con necrosis pulpar utilizando la técnica LSTR con la pasta CTZ presentó excelentes resultados clínicos y radiográficos


Introdução. O objetivo principal da terapia pulpar é manter a integridade e saúde dos dentes e de suas estruturas de suporte. Na odontopediatria, o tratamento de molares decíduos diagnosticados com pulpites irreversíveis ou necrose pulpar representa um grande desafio na prática clínica devido à complexidade da anatomia radicular, tempo de trabalho e dificuldade para uma correta instrumentação. A técnica "Lesión Sterilization Tissue Repair" (LSTR) busca esterilizar a lesão e reparar tecidos através de pastas antibióticas sem a necessidade da instrumentação dos canais radiculares. Relato de caso. Uma paciente do sexo feminino, 09 anos, assintomática e em acompanhamento na clínica odontológica, foi atendida com uma lesão ativa e cavitada de cárie no dente 75. O dente apresentava abscesso e mobilidade grau I. A radiografia periapical mostrou uma área radiolúcida na região de furca, sugerindo o diagnóstico de necrose pulpar. Para o tratamento pulpar, optou-se pela técnica LSTR com a pasta CTZ, composta por cloranfenicol, tetraciclina, óxido de zinco e eugenol. Posteriormente, o molar decíduo foi restaurado com coroa de aço. Após 8 meses, o dente encontra-se assintomático, com restauração satisfatória, mobilidade fisiológica e integridade gengival. Radiograficamente, há ausência de radiolucidez na região de furca e reabsorção óssea patológica. Conclusão. O tratamento endodôntico em molar decíduo com necrose pulpar utilizando a técnica LSTR com a pasta CTZ apresentou excelentes resultados clínicos e radiográficos


Introduction. The main goal of pulp therapy is to maintain the integrity and health of teeth and their supporting structures. In pediatric dentistry, the treatment of primary molars diagnosed with irreversible pulpitis or pulp necrosis is one of the great challenges in clinical practice due to the complexity of root anatomy, working time, and difficulty for correct instrumentation. The "Lesion Sterilization Tissue Repair" (LSTR) technique aims to sterilize the lesion and repair tissues through the use of antibiotic pastes without the need to instrument the root canals. Case report. A 9-year-old female patient attends the dental clinic asymptomatically. In the clinical examination, the second primary mandibular molar on the left side (tooth #75) presents an active and cavitated dental caries lesion, abscess, and grade I mobility. On the periapical radiograph, it presents a radiolucent area in the furcation region, suggesting the diagnosis of pulp necrosis. For pulp treatment, the LSTR technique with CTZ paste was chosen, composed of chloramphenicol, tetracycline, zinc oxide and eugenol. Subsequently, the molar was restored with a stainless steel crown. After 8 months, the tooth is asymptomatic, with satisfactory restoration, physiological mobility, and gingival integrity; radiographically there is no radiolucency in the furcation region and pathological bone resorption. Conclusion. The endodontic treatment in the primary molar with pulp necrosis using the LSTR technique with CTZ paste presented excellent clinical and radiographic results
