ABSTRACT

OBJECTIVE: To review the literature on the diagnosis and treatment of vestibular schwannoma. METHODS: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on vestibular schwannoma were eligible for inclusion. The American College of Physicians' guideline grading system and the American Thyroid Association's guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. RESULTS: The topics were divided into 2 parts: (1) Diagnosis - audiologic, electrophysiologic tests, and imaging; (2) Treatment - wait and scan protocols, surgery, radiosurgery/radiotherapy, and systemic therapy. CONCLUSIONS: Decision making in VS treatment has become more challenging. MRI can diagnose increasingly smaller tumors, which has disastrous consequences for the patients and their families. It is important to develop an individualized approach for each case, which highly depends on the experience of each surgical team.

Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

Abstract Objectives: This study aims to investigate the pattern of immunoexpression of proteins p16, p53 and Ki-67 in RRP, as well as to evaluate its influence on the number of surgeries that patients have undergone to date and to analyze the benefit of immunohistochemistry in this disease. Methods: Clinical-demographic data and tumor samples were obtained from 33 patients with RRP. The expression of proteins p16, p53 and Ki-67 was analyzed by immunohistochemical method. Results: Most patients had already undergone more than one surgery. The p16 marker was negative in 24.2% of the cases, with little positivity in 27.3% of the cases, moderate in 36.4% and intense in 12.1%. The p53 marker was positive in all cases, with little immunoexpression in 39.4% of cases, moderate in 36.4% and intense in 24.2%. The Ki-67 marker showed nuclear positivity in all lesions, although in varying degrees, with a mean proliferative index ±SD (standard deviation) of 51.7±26. Conclusions: The papillomatous lesions had varying degrees of immunoexpression of proteins p16, p53 and Ki-67, but no specific immunohistochemical pattern was observed. It was found, with statistical difference, that the number of surgeries was higher in cases with greater intensity of p53 expression, without correlation with the other markers. The benefit of immunohistochemistry in recurrent respiratory papillomatosis may lie in the prognostic assessment. However, further studies are needed to evaluate the use of this technique for this purpose. Level of evidence:: 4.

ABSTRACT

OBJECTIVES: To conduct critical assessment of the literature on the effects of cochlear implantation on adults' cognitive abilities. DESIGN: PubMed, Scopus, Lilacs, Web of Science, Livivo, Cochrane, Embase, PsycInfo, and grey literature were searched. Eligibility criteria: age 18 or over with severe-to-profound bilateral hearing loss, cochlear implantation, cognitive test before and after implantation. Risk of bias was assessed using ROB, ROBINS-I and MASTARI tools. Meta-analysis was performed. STUDY SAMPLE: Out of 1830 studies, 16 met the inclusion criteria. RESULTS: On AlaCog test, significant improvement was found after implantation [MD = -46.64; CI95% = -69.96 to -23.33; I2 = 71%]. No significant differences were found on the Flanker, Recall, Trail A and n-back tests (p > 0.05). For MMSE, no significance was found [MD 0.63; CI 95% = -2.19 to 3.45; I2 = 88%]. On TMT, an overall significant effect with a 9-second decrease in processing speed post-implantation [MD = -9.43; CI95% = -15.42 to -3.44; I2 = 0%]. CONCLUSION: Cognitive improvements after cochlear implantation may depend on time and the cognitive task evaluated. Well-designed studies with longer follow-up are necessary to examine whether cochlear implantation has a positive influence on cognitive abilities. Development of cognitive assessment tools to hearing-impaired individuals is needed.

Abstract Introduction Local anesthesia with sedation has been employed for an increasingly number of otolaryngology procedures, and might be associated with lower surgical morbidity and costs. Facial nerve monitoring is often advisable in otology to minimize the risks of injuries to this cranial nerve, but the principles, techniques and parameters involved have only been studied for procedures under general anesthesia. Objective To report the preliminary outcomes of intraoperative facial nerve moni- toring during otologic procedures under sedation and local anesthesia. Methods A total of five procedures and their respective intraoperative electrophysi- ological main findings were described. Facial neuromonitoring was performed using the same device by an electrophysiologist. The monitor sensitivity was set at 100 mV, and a stimulating probe was used whenever needed. Results Progressively decreasing low-amplitude baseline values were usually obtained as the level of anesthesia increased, with isolated oscillations possibly related to some degree of voluntary muscular activity. These oscillations could be easily distinguished from those of the surgical manipulation or electrical stimulation of the nerve, which tended to be of much greater amplitude and shorter latency, occurring during specific surgical steps. Conclusion With a surgical team with proper procedural knowledge and broad expertise regarding the technique, intraoperative facial nerve monitoring under local anesthesia with sedation seemed both feasible and reliable. Thus, the need for intraoperative neuromonitoring should not be an obstacle for otologic procedures under less aggressive anesthetic management.

ABSTRACT

A policondrite recidivante (PR) é uma doença imunomediada rara, caracterizada por inflamação de cartilagens, predominantemente do pavilhão auricular e articulações periféricas. O diagnóstico é baseado em critérios clínicos e o seu tratamento é dependente da fase da doença e de suas manifestações. Apresentamos o caso de um homem de 45 anos com história de edema e hiperemia em pavilhões auriculares responsivo aos tratamentos clínicos, porém altamente recidivantes. Após o diagnóstico e tratamento definitivos, mantém bom controle dos sintomas. A importância desse relato de caso é demonstrar um caso clássico de PR desde o diagnóstico até o tratamento, evidenciando as manifestações pouco específicas da doença e as suas dificuldades diagnósticas


Relapsing polychondritis (RP) is a rare immune-mediated disease characterized by inflammation of cartilagepredominantly those of the ears and peripheral joints. The diagnosis is based on clinical criteria and the treatment depends on the stage of the disease and its manifestation. We present the case of a 45-year-old man with a history of edema and hyperemia in auricular pavilions responsive to clinical treatment, but highly recurrent. After diagnosis and definitive treatment, a good control of the symptoms was achieved. The importance of this report is to demonstrate a classic case of RP from diagnosis to treatment, evidencing the nonspecific manifestations of the disease and its diagnostic difficulties
