ABSTRACT

This study reports the clinical case of a 42-year-old patient with ulcerated lesions who was followed up by general practitioners with the diagnosis of recurrent cellulitis. However, when referred to the Dermatology division a diagnosis of Behçet's syndrome was established based on clinical criteria. Although there are defined clinical criteria for this syndrome, sometimes its diagnosis can be challenging, due to lack of knowledge of the disease and extremely heterogeneous clinical phenotype. The authors highlight the potential difficulties in establishing the diagnosis considering the multiple clinical findings during the investigation process, contributing to the risk of increased morbidity and mortality.

ABSTRACT

Of all the therapeutic options available in Dermatology, few of them have the history, effectiveness, and safety of phototherapy. Heliotherapy, NB-UVB, PUVA, and UVA1 are currently the most common types of phototherapy used. Although psoriasis is the most frequent indication, it is used for atopic dermatitis, vitiligo, cutaneous T-cell lymphoma, and cutaneous sclerosis, among others. Before indicating phototherapy, a complete patient assessment should be performed. Possible contraindications should be actively searched for and it is essential to assess whether the patient can come to the treatment center at least twice a week. One of the main method limitations is the difficulty that patients have to attend the sessions. This therapy usually occurs in association with other treatments: topical or systemic medications. Maintaining the regular monitoring of the patient is essential to identify and treat possible adverse effects. Phototherapy is recognized for its benefits and should be considered whenever possible.

ABSTRACT

Mucocutaneous leishmaniasis (MCL) is a chronic infection that can affect the skin and mucous membranes. We report a case of oral, nasopharyngeal, and penile lesions in a 35-year-old cocaine user. The patient presented with ulcerated lesions in 2014. Histopathologic analysis revealed amastigotes, and serological test results were positive for leishmaniasis. Systemic therapy with meglumine antimoniate was administered; however, the patient failed to present for follow-up. In 2018, he returned with nasal collapse, and another histopathologic test confirmed MCL. This case illustrates the importance of careful differential diagnosis of skin and mucous ulcers to identify the particular pathology.

ABSTRACT

Abstract Identified the inappropriate prescription of drugs in elderly assisted geriatric outpatient clinic in the region of Vale do Rio Pardo in Rio Grande do Sul according to the criteria of STOPP and START. Cross-sectional study of 221 elderly, based on retrospective analysis of medical records using comprehensive geriatric assessment data held in the first visit. Descriptive variables were age, sex, number of pathologies and number of medications in use. And the study variables were potentially inappropriate medications (MPI) and potentially omitted medications (MPO), according to the STOPP and START criteria. Descriptive statistics were performed with frequencies, means and standard deviations where appropriate. It was observed 157 (71%) female patients. The average age was 75 (±8.26) years, 49.8% in the range of 75-84 years. The average number of drugs was 3 (±2.27), with 22.63% using 5 or more medications. The average pathologies was 3.45 (±2.02). They identified 194 prescriptions of MPI and observed 93 MPO. The most identified were the MPI for the cardiovascular system, especially β noncardioselective blockers and acetylsalicylic acid. Also MPO were found most of the cardiovascular system, with emphasis on acetylsalicylic acid and inhibitors of angiotensin converting enzyme. This is sample of elderly with significant prevalence of inappropriate prescribing of drugs. Studies using the STOPP criteria and START in different health care settings can qualify the care provided to the elderly. AU


Resumo Identificou-se a prescrição inapropriada de medicamentos de idosos atendidos em ambulatório de geriatria na região do Vale do Rio Pardo, no Rio Grande do Sul, de acordo com os critérios de STOPP e START. Estudo transversal com 221 idosos, baseado em análise retrospectiva de prontuários, utilizando dados de avaliação geriátrica ampla realizada em primeira consulta. As variáveis descritivas foram: idade, sexo, número de patologias e número de medicações em uso e as variáveis de estudo foram: medicações potencialmente inapropriadas (MPI) e medicações potencialmente omissas (MPO), de acordo com os critérios de STOPP e START. Realizou-se estatística descritiva com frequências, médias e desvio padrão quando adequado. Observou-se 157 (71%) pacientes do sexo feminino. A idade média foi de 75 (±8,26) anos, com 49,8% na faixa entre 75-84 anos. A média de medicações foi de três (±2,27), com 22,63% utilizando cinco medicações ou mais. A média de patologias foi 3,45 (±2,02). Foram identificadas 194 prescrições de MPI e observados 93 MPO. Os MPI mais identificados foram referentes ao sistema cardiovascular, com destaque para β bloqueadores não cardiosseletivos e ácido acetilsalicílico. Também os MPO mais encontrados foram os do sistema cardiovascular, com ênfase para ácido acetilsalicílico e inibidores da enzima conversora da angiotensina. Trata-se de amostra de idosos com importante prevalência de prescrição inapropriada de medicamentos. Estudos com a utilização dos Critérios STOPP e START em diferentes cenários de atenção à saúde podem qualificar o cuidado prestado ao idoso. AU
