ABSTRACT

In was analyzed diagnostics and treatment results of 32 victims with duodenal injuries. The authors used 3% hydrogen peroxide solution for duodenal rupture diagnosis. Suggested surgical technique includes intestine intersection where it was broken and anastomosis with a loop of small intestine by using of Roux's method. Also it was done duodenal passage temporary shutdown by using of catgut purse-string suture on pyloric part of stomach.

ABSTRACT

Duodenal damages occurs rarely and constitutes 0.9-5% of all traumas of abdominal organs. The problem is actual because of the condition complicated diagnosis, the absence of standardized surgical tactics and high lethality (from 10-25 up to 40%). As a rule, duodenal damages are revealed intraoperatively. Specific symptoms, characteristic for duodenal damages, are absent. Results of treatment of 28 patients, suffering traumatic duodenal damages, were analyzed and low information value of modern diagnostic methods was noted. There was proved the efficacy of measures for diagnosis of duodenal trauma, using 3% solution of hydrogen peroxide.

ABSTRACT

The work is devoted to experimental investigation and practical application of the objective intraoperative diagnosis method for the intestinal transplant state estimation, which is prepared for descendence to perineum, using modified pulsoximetric data analyser in environment of experimental modelling of abdominoanal rectal resection, resulting in determination of objective indices of the intestinal transplant vital capacity. The method of operative intervention in experiment on laboratory animals is adduced. The expediency of application of modified pulsoximetric data analyser in colorectal surgery was established.

ABSTRACT

The gastric function indexes (basal secretion, the pentagastrin and insulin tests) is one of the main criteria for choosing the organ-preserving operation method. In 162 patients with duodenal ulcer disease there were performed the isolated selective proximal vagotomy (SPV), SPV with draining operation, extragastric vagotomy with draining operation, selective vagotomy with antrumectomy, SPV with disconnection of antrum and gastric corpus.

ABSTRACT

In chronic experiments on dogs with gastric fundus fistula it was established that the isolated vagotomy of greater curvature had promoted the substantial reducing of gastric secretion, stimulated by insulin, pentagastrin, carbachol and histamine.

ABSTRACT

There were examined 79 patients, operated on for duodenal ulcer disease. Before the operation the Helicobacter pylori (HP) dissemination of stomach and duodenum of significant degree was diagnosed in 51.9%, mild degree--in 34.2% and of a nonsignificant one--in 13.9% of patients. After the operation the HP dissemination of stomach and duodenum had persisted.

ABSTRACT

The gastric secretion inhibition, the second phase mainly, after the extragastric vagotomy (EGV) conduction, was proved in experiment. The inhibition of carbacholine and more profound (up to 92%) of pentagastrin secretion was noted. In 42 patients with duodenal ulcer disease the EGV was performed.

ABSTRACT

The small intestine vagotomy influence on gastric secretion function, stimulated by pentagastrin and carbachol, was investigated while chronic experiment on dogs with gastric fundal and duodenal fistulas. The comparative analysis of influence of extragastric selective proximal, selective distal, selective and truncal vagotomy on gastric secretion in various terms after operative intervention was also conducted. In 6 months after conduction of various kinds of vagotomy the reduction of hydrochloric acid secretion, stimulated by pentagastrin, was noted.
