ABSTRACT

A 36-year-old Japanese man was hospitalized with coughing and exertional dyspnea (NYHA class I). He was diagnosed as having congestive heart failure, and was treated with diuretics and a beta-adrenergic blocking agent. He responded well to the treatment and his symptoms completely disappeared within a few days. Based on his clinical, laboratory, and molecular genetic findings, he was diagnosed as having X-linked dilated cardiomyopathy (XLDCM). He was found to have a large deletion in the dystrophin gene, involving exons 45-55. This is the first report on a Japanese XLDCM patient with a mutation in the central hot-spot region of this gene.

ABSTRACT

We could detect recruitable backward collateral flow in systole and diastole by using a Doppler guide wire during coronary artery spasm in a 42-year-old female among 20 patients with documented variant angina. The electrocardiogram (ECG) showed no ST-segment deviation, although she had chest pain and her coronary angiography showed a totally occluded right coronary artery. After administration of isosorbide dinitrate (ISDN) the collateral flow disappeared and the flow pattern recovered to normal. The good collateral flow pattern in patient with totally occluded coronary artery due to spasm was indicated to relieve myocardial ischemia, even when the patient felt chest pain.

ABSTRACT

Repeated administration of a small dose of compound 48/80 induced acute gastric mucosal injury in rats. Thiobarbituric acid (TBA) reactants in the gastric mucosa were significantly increased and serum alpha-tocopherol was significantly decreased after the treatment. The total area of gastric lesions and the increase in TBA reactants in the gastric mucosa were significantly reduced by pretreatment with superoxide dismutase (SOD) and/or catalase, allopurinol and anti-rat polymorphonuclear leukocytes (PMN) antibody. Lipid peroxidation and oxygen radicals derived from both the xanthine-xanthine oxidase system and PMN may be involved in the pathogenesis of compound 48/80-induced gastric mucosal injury.

ABSTRACT

Carbohydrate antigen (CA) 19-9 levels in pure pancreatic juice from patients with pancreatic cancer, chronic pancreatitis, and other diseases were determined, and their clinical value was assessed. CA 19-9 levels in pancreatic juice were generally very high, compared with those in serum. In addition, the pancreatic juice CA 19-9 levels in patients with pancreatic cancer were significantly higher than those in patients with chronic pancreatitis and other various diseases used as controls. The assay of pancreatic juice CA 19-9 seemed to be valuable in the diagnosis of pancreatic cancer, showing a diagnostic value approximately similar to that of the serum CA 19-9 assay and clearly superior to that of the serum or pancreatic juice carcinoembryonic antigen assay.

ABSTRACT

The study was conducted to examine the effect of somatostatin on activated renin-angiotensin-aldosterone system in a case of Bartter's syndrome. After 60 minutes of 500 micrograms of somatostatin infusion, the plasma aldosterone concentration was reduced from the basal level of 250 pg/ml to 140 pg/ml, whereas plasma renin activity remained at the basal level. This result suggests that somatostatin may specifically inhibit aldosterone secretion in Bartter's syndrome and the agent can be applied to a treatment of this syndrome.
