ABSTRACT

Objective To investigate the incidence and causes of medication errors(MEs)related to aspirin in the eld-erly(≥60 years old)of China,to provide data for targeted precautions,and to promote medication safety in the elderly.Methods Reports about aspirin-related MEs in the elderly were extracted from the National Monitoring Network for Clinical Safe Drug Medication from September 22,2012 to September 22,2022.The severity of errors,content of errors,where errors were made,the personnel who made these errors,and causes of errors were analyzed retrospectively and statistically.Results A total of 386 MEs reports were extracted,235 males and 151 females,with a median age of 70(60-98)years old.Regarding the severity of MEs,errors of class B were dominating(314 cases,81.35%).As for the severe MEs(9 cases,2.33%),5 cases of ME were as-sociated with bleeding.There are four patients suffering from gastrointestinal bleeding:one patient took aspirin three times a day,one patient broke off enteric-coated tablets,one patient took aspirin as acarbose mistakenly,one patient took aspirin with Xuesai-tong for long time.One patient had urinary urgency,but did not seek medical attention in time,then developed into visible hema-turia.Two patients missed taking aspirin led to stent thrombosis.One patient suffered from diarrhea after overdosing on 29 pills of aspirin by mistake.One patient with hyperuricemia had gout after taking aspirin.Among these 386 MEs,224(58.03%)cases were triggered by physicians,the first three error factors were frequency(multiple times a day),dosage(the dosage of enteric-coated tablets is not the whole tablet),and variety.Most of these MEs were due to a lack of knowledge or training,and fatigue.Regarding the MEs caused by pharmacists(87 cases,22.54%).Pharmacists identified 274 cases(70.98%)of errors.Conclusion When it comes to aspirin-related MEs,it ought to be noticed that the drug usage and dosage,drug interaction,and selection of drug varieties when combining with other diseases(such as atrial fibrillation and hyperuricemia).It is recommended that information construction be fortified to decrease the ME of the prescription link,and the clinicians be better informed of rational drug use.At the same time,patient medication education should be improved.

ABSTRACT

Objective@#To explore the effect of adenoassociated virus sense transfection up-regulating the expressionlevel of the growth and differential factor 11 ( GDF11) in vivo on aortic injury in type 2 diabetic mellitus rats(T2DM).@*Methods@#Nine-week-old male SD rats were randomly selected to establish a T2DM model by usinghigh -sugar and high-fat chow plus small-dose streptozotocin (STZ) combined induction. Both normal rats and diabetic model rats were randomly divided into five groups: blank control group ( Control group) , negative virus control group ( NC group), GDF11 adeno-associated virus group ( GDFl1 group), diabetic group ( DM group), anddiabetic + GDF11 adeno-associated virus group ( DM + GDFl1 group). After 8 weeks of feeding, the serum con-centrations of insulin ( INS ), advanced glyeosylation end products ( AGES), recombinant growth transforming fac.tor 11 ( GDF11 ), total cholesterol (T-CH0 ), triglycerides (TG), low-density lipoproteins (LDL-C), high-density lipoproteins ( HDL-C), asymmetric dimethylarginine ( ADMA), and malondialdehyde ( MDA) were assayed inthe rats ; periodic acid-schiff stain( PAS stain) was used to observe the sites of glycogen deposition, and hematoxylin-eosin staining ( HE) was used to observe vascular damage. Scanning electron microscopy was used to observethe damage of vascular endothelial cells and vascular elastic fibers, and protein blotting and immunohistochemistrwere used to detect the expression levels of vascular injury-related proteins. Protein blotting and immunohistochemistry were used to detect the expression levels of vascular injury-related proteins. @*Results@#The biochemical indexes showed that the serum concentrations of AGES, T-CHO, TG, LDL-C and MDA were higher in the DM groupthan those in the Control group (P <0. 05), the concentrations of INS, GDF11, HDL-C and ADMA were signifi.cantly lower than those in the Control group (P <0. 05 ), and the concentrations of AGE'S and HDL-C were not sig.nificantly lower in the DM + GDF1l group compared with the DM group ( P <0. 05). HDL-C was not significantlydilerent from the DM group, and several other data were improved ( P<0. 05 ). Pathological staining suggestedthat PAS staining in the DM group suggested that glycogen particles deposited in the endothelium and subendotheli.um of the aorta, HE staining observed thickening of the aortic mesentery, endothelial cells and elastic fibers brokeolf in an irregular alignment , and electron microscopy observed endothelial damage in the vasculature and elastic fibers broke off in the DM group, and these changes attenuated in the DM + GDFl1 group. Protein blotting and im.munohistochemistry indicated that the expression of endothelial cell-associated proteins decreased in the DM groug( P <0. 05), and mesenchymal markers elevated in the DM group ( P <0. 05 ), these proteins were regressed inthe DM + GDFl1 group, and the difference was statistically significant (P<0.05).@*Conclusion@#Inereasing theexpression level of GDFl1 in vivo can improve aortic vascular injury caused by diabetes mellitus, which is inferredthat it may be related to the inhibition of endothelial mesenchymal transition to protect the function of vascular endo.thelial cells and thus improve vascular injury.

ABSTRACT

Objective To investigate the inhibitory effect of cryptotanshinone (CPT) on human breast cancer cell MCF7 and its mechanism. Methods The survival rate of MCF7 cells was measured by MTT assay. Cell apoptosis was detected by Annexin V/PI assay and Hoechst 33258 fluorescence staining assay. Cell cycle and reactive oxygen species were detected by flow cytometry. Cell migration and invasion were detected by cell scratch test and Transwell chamber test. The surface molecules CD44 and CD24 were detected by flow cytometry and microsphere culture. The expression of cell-associated proteins was detected by Western blot. Results CPT inhibited the proliferation of MCF7 cells in a dose-dependent manner, and the 24 h IC50 value was 19.24 μmol/L. Compared with the untreated group, the CPT-treated group showed cell cycle arrested in the S phase, and apoptosis was induced. The results of the cell scratch and Transwell chamber tests showed that CPT significantly inhibited the migration and invasion of MCF7 cells. Furthermore, CPT reduced the CD24-/LowCD44+ cell population in MCF7 cell-derived microspheres. Western blot results showed that CPT could up-regulate the expression of Bax protein, down-regulate the expression of BCL-2, PI3K-p85, Akt, N-cadherin, Twist1, Sox2, Oct4, and Nanog protein, effectively inhibit the phosphorylation of ER-α, and decrease the expression of ABCG2. Conclusion CPT can inhibit the proliferation of MCF7 cells by inhibiting the migration and invasion of MCF7 cells, decreasing the number of CD24-/lowCD44+ cells and affecting the expression of tumor stem cell-related proteins.

ABSTRACT

Black and Brown Carbon (BC, BrC) are key parameters of climate forcing, yet significant challenges exist assigning emission source contributions to light-absorption by carbonaceous aerosols. Additionally, BC and BrC emissions add to extreme air pollution events in Chinese mega-cities, which harm human health and detract from the natural and built environment. To address these concerns, the ability to estimate atmospheric light absorption related to emission sources and global inventories is a highly valuable tool for climate modelers and policy makers. Three months of BC and BrC data was collected using an Aethalometer in parallel to PM2.5 filter sampling during a stringent emission controls period and post controls period, including during the regional heating season. In this study reconstructed 370 nm wavelength absorption was calculated by applying source specific Mass Absorption Cross-Sections to PMF apportioned EC and OC results. Reconstructed absorption showed good agreement with the ambient measured absorption for both BC and BrC. In Beijing, the major contributor to near-UV absorption was mobile sources, which accounted for 45-54% of absorption by BC and 14-18% by BrC. BrC absorption from secondary aerosols, biomass burning, and soil dust was also estimated, with these sources contributing from 1 to 9% individually. Meteorological cluster analysis showed that air mass origin did not impact the absorption reconstruction and that the highest regional contribution to near-UV light absorption originated primarily in areas south and east of Beijing. The study shows ambient near-UV light absorption can be predicted using BC and BrC MAC values from sources. However, the current number of multi-wavelength and source specific BrC MAC values reported in the literature is limited. The reconstruction approach allows for a more robust method of assigning light absorption to source categories, allowing the expansion of aethalometer derived BrC apportionment to multiple sources, including biomass burning.

ABSTRACT

OBJECTIVE:To investigate the occurrence of medication errors (ME)in patients with hyperuricemia/gout and its related factors ,in order to avoid and reduce the occurrence of related ADE and ensure the safety of drug use in patients. METHODS:The ME reports related to hyperuricemia/gout were collected from National Monitoring Network for Clinical Safe Drug Use during Sept. 22nd,2012 to Jul. 9th,2020,and then analyzed in terms of reporting provinces ,classification and drugs involved,error content ,initiator and error factors. RESULTS :A total of 179 ME reports ,which met inclusion criteria ,were included from 47 hospitals in 11 provinces. The drugs involved mainly included benbromarone (36 cases,20.11%),allopurinol (31 cases,17.32%),calcium carbonate D 3(30 cases,16.76%),inverting sugar electrolyte (24 cases,13.41%)and sodium bicarbonate(22 cases,12.29%). Grade B ME were the most frequently reported (124 cases,69.27%);severe ME occurred in 3 cases(1.68%). The main errors were contraindications ,accounting for 70 cases(37.43%,mainly involving calcium carbonate D 3 and inverting sugar electrolyte ). The people who caused ME were mainly doctors ,involving 126 cases(70.39%,the main content of errors was contraindications ). The main error factors for ME were personnel factors ,involving 120 cases(67.04%,mainly lacking of knowledge ),followed by environmental factors (33 cases,mainly equipment failure ). CONCLUSIONS :ME in patients with hyperuricemia/gout are mainly caused by uric acid-lowering drugs and non-uric acid-lowering drugs that cause changes in uric acid. The main error contents include contraindications ,which were mostly caused by non-uric acid-lowering drugs and doctors. The main error factors are lack of knowledge and equipment failure. It is suggested to strengthen personnel training or introduce clinical medication support decision-making system ,while pay attention to the correct use and maintenance of related equipment in the process of drug use.

ABSTRACT

OBJECTIVE:To provide reference for exploring the way to improve the ability of pharmaceutical care in primary pharmaceutical staff. METHODS:Questionnaire survey was conducted among pharmaceutical staff(including head of pharmacy department),director/vice-director of hospital in charge of pharmacy,medical staff and patients from primary medical institutions in 9 provinces(regions,cities). The results of investigation were analyzed statistically. RESULTS:A total of 77 primary medical institutions involved in the survey. Totally of 2 273 questionnaires were sent out,and 2 248 effective questionnaires were collected with effective recovery rate of 98.9%. Among effective questionnaires,278 questionnaires were filled out by pharmaceutical staff (68 by head of pharmacy department),48 by director/vice-director of hospital in charge of pharmacy,771 by medical staff and 1 151 by patients. Results of survey showed that only 140 pharmaceutical staff had obtained pharmacy practice qualification,accounting for 50.4%. The surveyed primary medical institutions provided limited pharmaceutical care projects,and the projects with high proportion were medication explanation(85.7%)and medication consultation(84.4%). The establishment of Health Service Record Card in elderly patients(19.5%),the development of chronic disease management(23.4%)and the development of chronic disease related pharmaceutical knowledge publicity at the primary level(37.7%)took up low proportion relatively. Overall satisfaction of surveyed medical staff to pharmaceutical care of pharmaceutical staff was general(77.2%). Surveyed patients greatly satisfied with the attitude of pharmaceutical staff(85.5%)but overall satisfaction of them to professional knowledge of pharmaceutical staff(78.2%)was general. Surveyed pharmaceutical staff training needs in descending order were medication explanation(74.5%),medication counseling(71.9%)and providing pharmaceutical information(64.0%). However,the projects which had low proportion of training needs included chronic disease management(22.3%)and lifestyle guidance (27.0%),etc. The more favorite training forms in descending order were further study in medical institution at higher level(65.4%),expert lectures(60.1%)and training in the training base (57.9%). CONCLUSIONS:The primary medical institutions provided limited pharmaceutical care. It is necessary to further explore and standardize pharmaceutical care criteria. The overall satisfaction of medical staff and patients on pharmaceutical care of pharmaceutical staff is not high in primary institutions,and it needs to be further improved. The related pharmacy training for primary pharmaceutical staff still needs to be strengthened,and can be conducted according to the favorite form of training so as to improve the ability of pharmaceutical care.

ABSTRACT

Objective To make rehabilitation nursing signs and apply them in positioning of unaffected extremities among patients with hemiplegia.Methods Totally 49 stroke patients with hemiplegia were recruited in the control group from July 2013 to November 2014,and 52 stroke patients with hemiplegia were recruited in the experimental group from January 2015 to September 2015.Same treatment and rehabilitation nursing were provided to patients in two groups.For the experimental group,rehabilitation nursing signs were applied together with regular guidance and demonstration of positioning of unaffected extremities.For the control group,only regular guidance and demonstration of positioning of unaffected extremities were provided.Results For the experimental group,the compliance was 86.03%(4 026 of 4 680 times),the accuracy was 72.73%(2 928 of 4 026 times);for the control group,the compliance and accuracy were 58.10% (2 562 of 4 410 times)and 50.94% (1 305 of 2 562 times),respectively.In the experimental group,no new complication occurred,no existing complication deteriorated,10 cases of complications were alleviated.However,in the control group,two cases developed complications,two cases of existing complication deteriorated,and only four cases of complications were alleviated.Subjective attention from patients and their family members in the experimental group was higher than that in the control group,and the difference was statistically significant.Conclusion For patients with hemiplegia,the rehabilitation nursing signs can significantly increase the compliance and accuracy of unaffected extremities positioning,and increase the attention from patients and their family members regarding unaffected extremities positioning in daily life.

ABSTRACT

Objective To investigate the clinical efficacy of total parathyroidectomy and autologous transplantation(PTX+ AT) for treating secondary hyperparathyroidism(SHPT) in the patients with uremic maintenance hemodialysis and postoperative calcium supplementation.Methods The clinical data in 57 patients with SHPT treated with PTX+ AT in the urology department of the First Affiliated Hospital of Nanjing Medical University from March 2015 to August 2015 were retrospectively analyzed.The changes of serum intact parathyroid hormone(iPTH),calcium and clinical symptoms were observed before and after operation.Results Serum iPTH levels at different time points of immediately after operation,postoperative 20 min,2 d,4 d were(288.57 ±194.71),(185.00±99.28),(83.83 ± 58.26),(17.91± 15.60) pg/mL respectively,which were significantly decreased compared with (2395.80 ± 829.04) pg/mL before operation (P< 0.05);blood calcium at postoperative 4,8,24,72 h was (2.34 ± 0.32),(2.16±0.37),(2.16±0.27),(2.25±0.25) mmol/L respectively,which were significantly decreased compared with(2.61±0.25)mmol/L before operation(P<0.05);the symptoms such as bone pain,skin itching and so on were significantly improved.Conclusion PTX+AT can effectively treat uremic SHPT,and postoperative timely calcium supplementation can reduce the occurrence of hypocalcemia.

ABSTRACT

Objective To study the molecular mechanism of Bel subtype caused by mutation p.R168W of glycosyltransferase B.Methods Serological test,SSP-PCR and direct sequence the Exon6 and Exon 7 of the ABO gene.Construct a 3D molecular model and predict the structural impact of GTB protein mutations.Results A antigen or B antigen can't be detected on the surface of the propositus' RBC,and only anti-A antibodies were detected in her serum.But serological test indicated her daughter's blood type was a normal B type.SSP-PCR test indicated propositus' ABO gene type is O1 B.By gene sequencing the Exon 6 and Exon 7 of the ABO gene,a ABO Bel allel(c.502C>T,p.R168W)was discoverd in both the propositus and her daughter.Through the propositus' daughter coexisted Bel gene with normal B gene,her blood type was a normal B type.Conclusions ABO gene c.502C>T mutations cause Bel phenotypes in patients by reducing the stability of GTB.

ABSTRACT

Tissue distribution of marbofloxacin was studied in pigs after a single intramuscular injection at 2.5 mg/kg body weight. Samples of plasma, muscle, liver, kidney, heart, lung, and muscle at the injection site were randomly collected from five pigs at 2, 6, 10, 24, 48, 72, and 96 h after administration. Marbofloxacin concentrations were determined by using high-performance liquid chromatography with ultraviolet detection and were subjected to non-compartmental analysis to obtain kinetic parameters. The elimination half-life (t(1/2λz)) of marbofloxacin at the injection site was 22.12 h, while those in kidney, plasma, liver, lung, heart, and muscle were 16.75, 21.48, 21.84, 24.00, 24.45, and 28.91 h, respectively. Areas under the concentration-time curve from 0 h to (∞) (AUC(0–∞)s) were calculated to be 31.17 h·µg·mL⁻¹ for plasma and 32.97, 33.92, 34.78, 37.58, 42.02, and 98.80 h·µg·g⁻¹ for heart, muscle, lung, liver, kidney, and injection site, respectively. The peak concentration (C(max)) of marbofloxacin was 1.62 µg/mL in plasma and 1.71, 1.74, 1.86, 1.93, 2.45, and 7.64 µg/g in heart, lung, muscle, kidney, liver, and injection site, respectively. The results show that marbofloxacin was fast absorbed, extensively distributed, and slowly eliminated from pigs after a single intramuscular administration.

ABSTRACT

Adverse Drug Event(ADE)is a major factor threatening patient safety.For now,the ADE detecting in China mostly relies on spontaneous reporting,a method detecting only a small proportion of ADEs,instead of reflecting the real-world clinical medication safety.Global Trigger Tool (GTT)is a relatively new method for detecting ADEs.This paper described the GTT as an effective and practical method for detecting ADEs,by introducing the history of GTT development,GTT basic principles,operating procedures,current relevant researches and application at home and aboard and comparing with other ADE detection methods.The GTT can evaluate the clinical medication safety in various medical institutions.In particular,it can target specific clinical departments or certain people to detect ADEs for obtaining more evidences of clinical medication safety.That is the reason that we recommend medical institutions at all levels learn and use GTT.

ABSTRACT

Objective To explore the effectiveness of long-acting antibacterial material that was used as an intervention in oral care cavity nursing in patients with endotracheal intubation and mechanical ventilation. Methods Patients admitted to Xuzhou Medical College Hospital neurosurgery intensive care unit between 1st December 2013 and 31st August 2014 requiring intubation and mechanical ventilation were enrolled into the study. There were 100 cases randomly selected and were assigned into control group and experimental group by random number table. Each contained 50 cases. Control group used saline for oral care by brushing and irrigation;experimental group used antimicrobial material oral spray in oral care on the top of the method used in control group. The mouth odor, mouth infection rate and ventilator-associated pneumonia (VAP) rate were used to determine. Results About oral odor, mouth infection rate, and VAP rate, the difference of incidences between experimental group and the control group was significant (χ2=4.504-13.728, P <0.05). Conclusions Antimicrobial material can effectively improve oral health in intubated and mechanical ventilated patients. It slows oral acidic environment change, reduces oral infections early onset VAP incidence. And it is worth promoting its use.

ABSTRACT

Objective To investigate the effect and mechanism of AngⅡ on collagen in hepatic stellate cell. Methods HSCs were isolated and cultured, 3H-pro incorporation method was used to evaluate the effects of different doses of AngⅡ on the proline syntheses. RT-PCR assay were used to assess changes in mRNA expression levels of type Ⅰ and Ⅲ procollagen. PDGFR-β mRNA and protein were determined by in situ hybridization and immunocytochemistry. Results 10-8~ 10-5 mol/L AngⅡ could significantly increase the 3H-pro incorporation rate of HSC in a dose-dependent style, 10-6 mol/L AngⅡis the most effective dose. The cultured HSC showed a little expression of type Ⅰ and Ⅲ procollagen mRNAs, while 10-6 mol/L AngⅡwas able to enhance the expression for type Ⅰ and Ⅲ procollagen mRNAs significantly(P < 0.01). AngⅡalso could enhance both mRNA and protein expression of PDGFR-β on HSC(P < 0.01). Conclusion These results suggest that AngⅡ could promote HSC collagen synthesis by enhancing the expressions of PDGFR-β.

ABSTRACT

OBJECTIVE:To investigate the status quo of teachers'teaching ability of standardized training for hospital pharma-cists in Beijing area,and provide reference for strengthening the faculty construction. METHODS:A dynamic assessment of stan-dardized training base for hospital pharmacists of 17 inpatients in Beijing area was conducted to organize evaluation experts to as-sess the teaching ability of outpatient pharmacy,ward pharmacy and clinical pharmacy in training base. Questionnaire was designed and trainees were organized to evaluate the teaching ability,time,attitude,feedback and others and related results of teaching in bases were vegarded as objects for analysis and summary. RESULTS:The overall ability of teaching was high,and the most promi-nent was clinical pharmacy,which was well received by trainees. But there are differences among departments,the ability of ward pharmacy is slightly lower than the clinical pharmacy and outpatient pharmacy. And the teaching ability still had great improvement. CONCLUSIONS:In order to improve the overall teacher strength in training base,competence-based training for hospital pharma-cists must be strengthened to encourage teachers to attend training courses,strengthen cultivation of humanistic quality and enrich students'evaluation forms.

Objective To investigate the regulation of miR-1 and miR-133 a on L-type calcium channel β2 subunit ( Cavβ2 ) and α1C subunit during rat cardiomyocyte hypertrophy .Methods Cardiomyocyte hypertrophy was in-duced by isoproterenol (ISO, 10μmol/L).The targets of miR-1 and miR-133a were predicted by online database microCosm and Targetscan , respectively .The 3′untranslated region sequences of Cavβ2 andα1C were respectively cloned into reporter vector and then transiently transfected into HEK 293 cells.The luciferase activities of samples were measured for demonstrating the expression of luciferase reporter vector .The protein expression of Cavβ2 andα1C were evaluated by Western blot .The expression levels of Cavβ2 andα1C were inhibited by RNAi to determine theeffectsofCavβ2andα1Concardiomyocytehypertrophy.Results 1)Cavβ2wasoneofpotentialtargetsof miR-1,α1C was the one of potential targets of miR-133a.2) The luciferase activities of HEK293 cells with the plasmid containing widetype Cavβ2 3′UTR sequence or α1 C significantly decreased ( P <0.05 , P <0.01 ) . 3 ) Upregulation of the miR-1 and miR-133 a by miR-1 mimic and miR-133 a mimic transfection suppressed pro-tein expression of Cavβ2 and α1C, respectively(P<0.01, P<0.05).4)Downregulation of Cavβ2 andα1C by RNAi could markedly inhibit the increase of cell surface area ( P<0.01 ) , mRNA expression of ANP andβ-MHC (P<0.05).Conclusions Cavβ2 is the target gene of miR-1 and α1C is the target gene of miR-133a.miR-1 and miR-133a can negatively regulate the expression of L-type calcium channel Cavβ2 andα1C subunit, inhibi-ting cardiomyocyte hypertrophy.

ABSTRACT

OBJECTIVE:To improve the teaching quality of pharmaceutical standard training. METHODS: By introducing the overview of adopting elicitation teaching in the pharmaceutical standard training in Beijing,the training effect was summarized and role and significance were studied. RESULTS:Supervisors applied five teaching modules in the elicitation teaching in the pharmaceu-tical standard training which are concluded“pharmaceutical education and rational use of medicine”“comments of prescription and rational use of medicine”“prescription transfers and rational use of medicine”“medicine counseling and rational use of medicine”“pharmaceutically communication and rational use of drugs medicine”,the result showed that participants had mastered more than 90%of what they had learned;with 92.5%of participants felt elicitation teaching could help improve students’interests in pharma-cy,and strengthen responsibility;80.0% of the participants believed that part of the modules strengthen the training of clinical think-ing;the training of satisfaction to 100%. CONCLUSIONS: The elicitation teaching mode can not only improve the teaching quality in pharmaceutical standard training,but also strengthen communication and feedback between teachers and students,students and students to make students more deeply understand and master knowledge and improve teaching quality.

ABSTRACT

<p><b>OBJECTIVE</b>To screen the HIV-1 entry inhibitors targeting HIV-1 gp120 from the IBS natural product database by virtual screening based on the binding mode of the neutralizing antibody VRC01 with HIV-1 gp120 and investigate the anti-viral activities of the inhibitors and their action mechanisms.</p><p><b>METHODS</b>The binding interaction of the candidate molecules binding gp120 and changes of the binding free energy were analyzed by MM-PBSA calculation. The anti-HIV-1 activities of the tested compounds were detected by HIV-1 pseudotyped virus, laboratory-adapted HIV-1 and a cell-cell fusion assay. The cytotoxicity of the studied molecules was examined by XTT colorimetric assay. The mechanisms of the anti-viral activities of the candidate molecules were analyzed using enzyme-linked immunosorbent assay.</p><p><b>RESULTS</b>A total of 19 molecules with distinct reduction of the binding free energy after binding with gp120 were screened from 40000 molecules. Among them, NC-2 showed anti-HIV-1 activities against HIV-1 pseudotyped virus and laboratory-adapted HIV-1, and was capable of blocking HIV-1 envelope-mediated cell-cell fusion. The IC50 of NC-2 for inhibiting HIV-1IIIB and pseudotyped HIV-1JRFL infection were 1.95∓0.44 µmol/L and 10.58∓0.13 µmol/L, respectively. The results of ELISA suggested that NC-2 could inhibit the binding of HIV-1 gp120 to CD4 without blocking the formation of gp41 six-helix bundle in vitro.</p><p><b>CONCLUSION</b>This computer-based virtual screening method can be used to screen HIV-1 entry inhibitors targeting gp120. Using this virtual screening approach combined with anti-viral activity screening, we obtained a potent HIV-1 entry inhibitor NC-2 with novel structure.</p>

ABSTRACT

Objective Patient safety is key to quality of care in hospital management, for which drug safety is critical. General hospitals are challenged with how to monitor drug adverse reaction (ADR), and motivate their staff to report ADRs in building a hospital safety culture. Methods The PDCA cycle quality improvement process was called into play, to set the ADR criteria and analyze data of inpatients' ADRs from 2005 to 2009, which was followed by measures proposed for ADR continuous improvement. Results A total of 2850 ADR cases were reported in the past 5 years, averaging 570 cases per year. The ADR incidence rate was 3. 28%, of which 43 were severe ADR cases, or 0. 49% of the total cases, accounting for 1. 51% of all the ADR cases. Year to year, the quantity and quality of ADR reporting have been improving in the hospital, witnessing an initial culture of drug safety. In 2008, the hospital was awarded as an advanced ADR center in Beijing. Conclusion The PDCA cycle is an effective means for ADR management, as it can improve the awareness of the medical staff for proactive ADR reporting. Sustained circular management against ADRs can minimize risks and maximize patient safety.Under-reporting and the corrective measures shall be watched over by the hospital in the future.

ABSTRACT

Objective Study the concentration of the hepatocyte growth factor(HGF) the expression product of recombinant human hepatocyte growth factor naked plasmid ,and whether the body generates immune responseafter wsing HCF. Methods Selected 21 patients with severe ischemic disease of lower extremity (Rutherford classification 4-6 class) , after signing the informed consent and divided them into four dosage groups( 4 mg, 8 mg, 12 mg and 16 mg) random.In each group the dosage was the lower limbs partly intramuscular injection following the feeding artery in experimental stage Dl and D15. The plasma samples were collected to determine the HGF concentration in Dl ( before administration), D8, D15 ( before administration) , D21, D59, and the HGF-antibody concentration in Dl, D15, D28, D59, D91. Results (1)The concentrations of HGF in the subjects ranged from 216 pg/mL to 1189.75 pg/mL.(2) HGF-antibodies were not dectected in the subjects' plasma. Conclusions After using recombinant human HGF naked plasmid through intramuscular injection, the concentration of HGF expression product in subjects' peripheral blood does not have abnormally changed and using the drug the human body immune response does not generate.
