ABSTRACT

OBJECTIVE: Understanding the local characteristics and statistics related to stillbirths may be the first step in a series of strategies associated with a reduction in stillbirth ratio. The aim of this study was to estimate the fetal mortality ratio and evaluate the investigation processes related to the causes of death, comparing the investigation according to the specific cause of death. METHODS: A cross-sectional study was retrospectively conducted in 10 tertiary obstetric care centers. Medical records of women with stillbirth managed between January 1, 2009 and December 31, 2018 were analyzed and classified, according to sociodemographic characteristics, and gestational and childbirth data, culminating in stillbirth. The stillbirth ratio and its causes were presented in proportions for the study period and individually for each health facility. RESULTS: Cases of 3390 stillbirths were analyzed. The stillbirth ratio varied from 10.74/1000 live births (LBs) in 2009 to 9.31/1000 in 2018. "Intrauterine hypoxia and asphyxia" (ICD-10 P20) and "unspecific causes of death" (ICD-10 P95) represented 40.8% of the causes of death. Investigation for TORCHS and diabetes occurred in 90.8% and 61.4% of deaths, respectively. Placental and necroscopic tests were performed in 36.6% of the cases. CONCLUSION: The adoption of a rational and standardized investigation of stillbirth remains an unmet need; the use of additional tests and examinations are lacking, especially when unspecific causes are attributed.

Objectives: To identify the prevalence and determinants of continued breastfeeding in Haitian children aged 12-23 months. Methods: Three cross-sectional surveys were conducted yearly during the summers of 2017 to 2019 as part of a 4-year (2016-2020) multisectoral maternal and infant health initiative in the regions of Les Cayes, Jérémie, and Anse d'Hainault in Haiti. A total of 455 children 12-23 months of age and their mothers participated in the study. A child was considered to be continuing breastfeeding if the mother reported giving breast milk in the 24-hour dietary recall. Unadjusted and adjusted prevalence ratios were estimated, and associations were assessed between continued breastfeeding and explanatory factors related to sociodemographic characteristics, household food security, maternal nutrition, and breastfeeding knowledge and practices. Results: The prevalence of continued breastfeeding was 45.8%. Continued breastfeeding was significantly more prevalent among younger children, children who did not have a younger sibling, children whose mother was not pregnant, those living in the Jérémie region, children who had been exclusively breastfed for less than 1 month, and children whose mother knew the World Health Organization's recommendation for continued breastfeeding up to 2 years or beyond. Conclusions: The study results highlight the need for geographically equitable access to tailored and adequate health services and education that support breastfeeding in a way that is compatible with the local context.

ABSTRACT

Children living in low-resource settings are frequently gut-colonized with multidrug-resistant bacteria. We explored whether breastfeeding may protect against children's incident gut colonization with extended-spectrum beta-lactamase-producing Escherichia coli (ESBL-Ec) and Klebsiella, Enterobacter, or Citrobacter spp. (ESBL-KEC). We screened 937 monthly stool samples collected from 112 children aged 1-16 months during a 2016-19 prospective cohort study of enteric infections in peri-urban Lima. We used 52,816 daily surveys to examine how exposures to breastfeeding in the 30 days prior to a stool sample were associated with children's risks of incident gut-colonization, controlling for antibiotic use and other covariates. We sequenced 78 ESBL-Ec from 47 children to explore their diversity. Gut-colonization with ESBL-Ec was increasingly prevalent as children aged, approaching 75% by 16 months, while ESBL-KEC prevalence fluctuated between 18% and 36%. Through 6 months of age, exclusively providing human milk in the 30 days prior to a stool sample did not reduce children's risk of incident gut-colonization with ESBL-Ec or ESBL-KEC. From 6 to 16 months of age, every 3 additional days of breastfeeding in the prior 30 days was associated with 6% lower risk of incident ESBL-Ec gut-colonization (95% CI: 0.90, 0.98, p = .003). No effects were observed on incident ESBL-KEC colonization. We detected highly diverse ESBL-Ec among children and few differences between children who were predominantly breastfed (mean age: 4.1 months) versus older children (10.8 months). Continued breastfeeding after 6 months conferred protection against children's incident gut colonization with ESBL-Ec in this setting. Policies supporting continued breastfeeding should be considered in efforts to combat antibiotic resistance.

ABSTRACT

Resumo A atenção primária à saúde é a principal porta de entrada e prioridade na gestão de saúde no Brasil. Contudo, existem desafios importantes na qualidade da atenção, em particular aos mais vulneráveis, especificamente na rede de saúde materna-infantil (RASMI). Esse fato é agravado pelas já conhecidas desigualdades regionais, que historicamente afetam mais as regiões Norte e Nordeste. O objetivo é avaliar no espaço-tempo a estrutura da RASMI em Roraima, na região Norte e no Brasil nos anos de 2012, 2014 e 2017. Para isso, a fonte de dados será o Programa Nacional de Melhoria do Acesso e Qualidade da Atenção Básica (PMAQ-AB), Módulo I. Observou-se melhoria significativa nos indicadores de infraestrutura física, como ambiência/climatização; e na distribuição de insumos/equipamentos necessários à assistência materno-infantil, percebeu-se um crescimento progressivo entre 2014 e 2017. Por outro lado, notou-se piora na disponibilidade de medicamentos e diminuição de quantidade/carga-horária de recursos humanos. O estudo configurou uma importante análise longitudinal, comparativa entre a realidade estadual e nacional, que contribui para a formulação de políticas e programas relativos aos direitos reprodutivos e à assistência materno-infantil.


Abstract Primary healthcare is the main gateway and priority for healthcare management in Brazil. However, there are significant challenges in the quality of care, particularly for those most vulnerable, especially maternal and infant healthcare. This fact is exacerbated by regional inequalities, which have historically left the North and Northeast regions at a relative disadvantage. The study involves an analysis of the resources available for maternal and infant healthcare in the state of Roraima, the North region, and Brazil as a whole in 2012, 2014, and 2017, using data from Module I of the National Program for the Improvement of Access and Quality of Primary Care (PMAQ-AB). There was a significant improvement in physical infrastructure indicators (e.g., ventilation and air conditioning) as well as improvement in the distribution of supplies and equipment needed for maternal and infant care between 2014 and 2017. However, the availability of medicines and the number of human resources and hours worked diminished. The study offers a crucial longitudinal analysis, comparing the situation in Roraima and Brazil, whose findings could contribute to the development of programs and public policymaking for reproductive rights and maternal and infant health.

ABSTRACT

ABSTRACT Objectives: to develop and validate the content and interface of a guidance website to support families in promoting Food and Nutrition Security for children under six months who are not breastfed. Methods: methodological study, Knowledge Translation, in two stages of creation: 1) content and validation on the criterion of accuracy in a panel of experts; 2) interface and validation on the criteria of content, language, illustrations, layout, motivation, culture and applicability. Results: the "Milky Way" website is freely available: https://www.ufsm.br/pet/ciencia-da-computacao/alimentacao-lactea. The content was structured in a decision tree made up of types of milk: milk formula, whole cow's milk and powdered milk; and utensils: bottle, cup and measuring spoon. There were 46 illustrations to elucidate the content, facilitate understanding and engage the target population. The Content Validity Index was 0.91. Conclusions: the website is a validated technology with evidence-based written and pictorial content translated for use with families.


RESUMEN Objetivos: desarrollar y validar el contenido y la interfaz de sitio web orientador para apoyar a familias en la promoción de Seguridad Alimentaria y Nutricional de niños menores de seis meses no amamantadas. Métodos: estudio metodológico, de Traducción del Conocimiento, en dos etapas de creación: 1) contenido y validación según el criterio de precisión por panel de expertos; 2) interfaz y validación según los criterios contenido, lenguaje, ilustraciones, maquetación, motivación, cultura y aplicabilidad. Resultados: el sitio "Vía Láctea" es de libre acceso: https://www.ufsm.br/pet/ciencia-da-computacao/alimentacao-lactea. El contenido se estructuró en un árbol de decisión compuesto por tipos de leche: fórmula láctea, leche entera de vaca y leche en polvo; y utensilios: biberón, taza y cuchara dosificadora. Hay 46 ilustraciones para elucidar el contenido, facilitar la comprensión y atraer a la población objeto. El índice de validez del contenido fue de 0,91. Conclusiones: el sitio web es una tecnología validada con contenidos escritos y visuales basados en pruebas y traducidos para su uso con las familias.

O conteúdo foi estruturado em árvore de decisão composta por tipo de leite: fórmula láctea, leite de vaca integral e em pó; e utensílios: mamadeira, copinho e colher dosadora. RESUMEN Objetivos. Determinar la prevalencia y los determinantes asociados a la continuación de la lactancia materna en la población infantil haitiana de entre 12 y 23 meses. Métodos. Durante los veranos del 2017 al 2019 se llevaron a cabo tres encuestas transversales anuales como parte de una iniciativa multisectorial de salud materna e infantil de 4 años (2016 a 2020) en las regiones de Les Cayes, Jérémie y Anse d'Hainault de Haití. En el estudio participaron 455 menores de edades comprendidas entre 12 y 23 meses y sus madres. Se consideró que un menor continuaba con la lactancia materna si, en la evaluación de la alimentación basada en el recuerdo de 24 horas, la madre declaraba la toma de leche materna. Se calcularon los cocientes de prevalencia ajustados y sin ajustar y se evaluó la asociación entre la continuación de la lactancia materna y posibles factores explicativos relacionados con las características sociodemográficas, la seguridad alimentaria de la familia, la nutrición materna y los conocimientos y prácticas en materia lactancia materna. Resultados. La prevalencia de la continuación de la lactancia materna fue del 45,8%. Esta prevalencia fue significativamente mayor cuando se trataba de lactantes de menor edad, cuando no había hermanos menores, cuando la madre no estaba embarazada, en los residentes en la región de Jérémie, cuando había habido una alimentación exclusiva con leche materna durante menos de un mes y cuando la madre conocía la recomendación de la Organización Mundial de la Salud de continuar con la lactancia materna hasta los dos años o más. Conclusiones. Los resultados del estudio ponen de relieve la necesidad de disponer de un acceso geográfico equitativo a unos servicios de salud y una educación que respalden la lactancia materna de una manera compatible con el contexto local.


RESUMO Objetivos Identificar a prevalência e os determinantes do aleitamento materno continuado em crianças haitianas de 12 a 23 meses de idade. Métodos Três pesquisas transversais foram realizadas anualmente nos verões de 2017 a 2019 como parte de uma iniciativa multissetorial de saúde materno-infantil de quatro anos (2016-2020) nas regiões de Les Cayes, Jérémie e Anse d'Hainault no Haiti. Um total de 455 crianças de 12 a 23 meses de idade e suas mães participaram do estudo. Considerou-se que a criança continuava sendo amamentada se a mãe relatasse ter dado leite materno no recordatório alimentar de 24 horas. Foram estimadas as taxas de prevalência não ajustadas e ajustadas e foram avaliadas as associações entre o aleitamento materno continuado e os fatores explicativos relacionados às características sociodemográficas, à segurança alimentar da família, à nutrição materna e aos conhecimentos e práticas de aleitamento materno. Resultados A prevalência de aleitamento materno continuado foi de 45,8%. O aleitamento materno continuado foi significativamente mais prevalente entre crianças mais novas, crianças que não tinham irmão mais novo, crianças cuja mãe não estava grávida, residentes da região de Jérémie, crianças que haviam sido exclusivamente amamentadas por menos de um mês e crianças cuja mãe conhecia a recomendação da Organização Mundial da Saúde de manter o aleitamento materno por 2 anos ou mais. Conclusões Os resultados do estudo destacam a necessidade de acesso geograficamente equitativo a educação e serviços de saúde adaptados e adequados para apoiar o aleitamento materno de forma compatível com o contexto local.

ABSTRACT

This paper evaluates the effects of a social fund that meets the needs of the poor in Northeast Brazil, the Fundos Estaduais de Combate e Erradicação da Pobreza (FECEP). The program could have improved infant health by reducing poverty and improving access to health care, sanitation, food, and housing. Using a difference-in-differences approach robust to heterogeneous treatment effects, we confirm that the program has effectively reduced poverty in treated areas. Furthermore, we document that this poverty reduction is associated with a significant decline in infant mortality. These findings provide consistent evidence that targeted public investments can improve living conditions in vulnerable regions.

INTRODUCTION: Birth weight is described as one of the main determinants of newborns' chances of survival. Among the associated causes, or risk factors, the mother's nutritional status strongly influences fetal growth and birth weight outcomes of the concept. This study evaluates the association between food deserts, small for gestational age (SGA), large for gestational age (LGA) and low birth weight (LBW) newborns. DESIGN: This is a cross-sectional population study, resulting from individual data from the Live Birth Information System (SINASC), and commune data from mapping food deserts (CAISAN) in Brazil. The newborn's size was defined as follows: appropriate for gestational age (between 10 and 90th percentile), SGA (< 10th percentile), LGA (> 90th percentile), and low birth weight < 2,500 g. To characterize food environments, we used tertiles of the density of establishments which sell in natura and ultra-processed foods. Logistic regression modeling was conducted to investigate the associations of interest. RESULTS: We analyzed 2,632,314 live births in Brazil in 2016, after appropriate adjustments, women living in municipalities with limited availability of fresh foods had a higher chance of having newborns with SGA [OR2nd tertile: 1.06 (1.05-1.07)] and LBW [OR2nd tertile: 1.11 (1.09-1.12)]. Conversely, municipalities with greater availability of ultra-processed foods had a higher chance of having newborns with SGA [OR3rd tertile: 1.04 (1.02-1.06)] and LBW [OR2nd tertile: 1.13 (1.11-1.16)]. Stratification by race showed that Black and Mixed/Brown women had a higher chance of having newborns with SGA [OR3rd tertile: 1.09 (1.01-1.18)] and [OR3rd tertile: 1.06 (1.04-1.09)], respectively, while Mixed-race women also had a higher chance of having newborns with LBW [OR3rd tertile: 1.17 (1.14-1.20)]. Indigenous women were associated with LGA [OR3rd tertile: 1.20 (1.01-1.45)]. CONCLUSION: The study found that living in areas with limited access to healthy foods was associated with an increased risk of SGA and low birth weight among newborns, particularly among Black and Mixed/Brown women. Therefore, urgent initiatives aimed at reducing social inequalities and mitigating the impact of poor food environments are needed in Brazil.

Background: Expanding primary healthcare to urban poor populations is a priority in many low-and middle-income countries and is essential to achieve universal health coverage (UHC). Between 2008 and 2016 the city of Rio de Janeiro undertook an ambitious programme to rapidly expand primary care to low-income areas through the family health strategy (FHS). Infant health impacts of this roll out are unknown. This study examines associations between maternal FHS utilisation and birth outcomes, neonatal and infant mortality. Methods: A cohort of 75,339 live births (January 2009-December 2014) to low-income mothers in Rio de Janeiro was linked to primary care, birth, hospital and death records. The relationship between maternal FHS use and infant health outcomes was assessed through logistic regression with inverse probability treatment weighting and regression adjustment. Socioeconomic inequalities in the associations between FHS use and outcomes were explored through interactions. Primary outcomes were neonatal and infant death. Thirteen secondary outcomes were also examined to explore other important health outcomes and potential mechanisms. Results: A total of 9002 (12.0%) infants were born to mothers in the cohort who used FHS services either before pregnancy or in the first two trimesters. There was a total of 527 neonatal and 893 infant deaths. Maternal FHS usage during the first two trimesters was associated with substantial reductions in neonatal [adjusted odds ratio (aOR): 0.527, 95% confidence interval (95% CI): 0.345; 0.806] and infant mortality (aOR: 0.672, 95% CI: 0.48; 0.924). Infants born to lower-income mothers and those without formal employment had larger reductions in neonatal and infant mortality associated with FHS use. Maternal FHS in the first two trimesters use was also associated with more antenatal care consultations and a lower risk of low birth weight and preterm birth. Interpretation: Expanding primary care to low-income populations in Rio de Janeiro was associated with improved infant health and health equity benefits. Funding: DFID/MRC/Wellcome Trust/ESRC.

BACKGROUND: This study aimed to examine factors associated with postpartum depression (PPD) symptoms during the COVID-19 pandemic among postpartum women in five countries, a subject that has not been investigated thus far. METHODS: A multi-country, cross-sectional, online survey was conducted with a convenience sample of 3,523 postpartum women in Brazil, South Korea, Taiwan, Thailand, and the United Kingdom, from July to November 2021. Sociodemographic and obstetric data, food insecurity, COVID-19 positive status, COVID-19 vaccination, infant feeding, breastfeeding belief score, and social support were investigated. PPD and social support were measured using the Edinburgh Postnatal Depression Scale and Maternal Social Support Scale, respectively. Descriptive statistics, chi-squared tests, and t-tests were used to identify associations with PPD symptoms. A binary logistic regression model was used to identify explanatory factors associated with PPD and adjusted odds ratios (OR) and 95% confidence intervals (CIs) were calculated. RESULTS: Women in Taiwan (AOR = 0.5; 95%CI 0.34, 0.73) and Thailand (AOR = 0.68; 95%CI 0.46, 0.99) had a lower risk of PPD symptoms than those in Brazil. In addition, women with planned pregnancies had a lower risk of PPD (AOR = 0.74; 95%CI 0.60, 0.91). Younger women (AOR = 1.62; 95%CI 1.05, 2.51), health problems during pregnancy, delivery, or postpartum (AOR = 1.71; 95%CI 1.42, 2.06), and no change or worse food insecurity during COVID-19 (AOR = 1.66; 95%CI 1.21, 1.27 for no change and AOR = 1.68; 95%CI 1.27, 1.23, respectively) presented a higher likelihood of having PPD. Feeding babies with expressed human milk (AOR = 1.25; 95%CI 1.03, 1.50) and/or complementary food (AOR = 1.51; 95%CI 1.17, 1.94) were associated with PPD symptoms. Women who received low (AOR = 7.74; 95%CI 5.43, 11.03) or medium support (AOR = 3.25; 95%CI 2.71, 3.88) had higher likelihoods of PPD. CONCLUSION: PPD symptoms during the pandemic were high in young women, particularly Brazilian women, with health problems in the puerperal pregnancy cycle who fed their babies expressed breast milk and/or complementary food. Low social support also impacted PPD symptoms. This study highlights the need for the professional screening for PPD and provision of virtual or personal support.

BACKGROUND: The current SARS-CoV-2 pandemic highlights the need for an increased understanding of coronavirus epidemiology. In a pediatric cohort in Nicaragua, we evaluate the seasonality and burden of common cold coronavirus (ccCoV) infection and evaluate likelihood of symptoms in reinfections. METHODS: Children presenting with symptoms of respiratory illness were tested for each of the four ccCoVs (NL63, 229E, OC43, and HKU1). Annual blood samples collected before ccCoV infection were tested for antibodies against each ccCoV. Seasonality was evaluated using wavelet and generalized additive model (GAM) analyses, and age-period effects were investigated using a Poisson model. We also evaluate the risk of symptom presentation between primary and secondary infections. RESULTS: In our cohort of 2576 children from 2011 to 2016, we observed 595 ccCoV infections and 107 cases of ccCoV-associated lower respiratory infection (LRI). The overall incidence rate was 61.1 per 1000 person years (95% confidence interval (CI): 56.3, 66.2). Children under two had the highest incidence of ccCoV infections and associated LRI. ccCoV incidence rapidly decreases until about age 6. Each ccCoV circulated throughout the year and demonstrated annual periodicity. Peaks of NL63 typically occurred 3 months before 229E peaks and 6 months after OC43 peaks. Approximately 69% of symptomatic ccCoV infections were secondary infections. There was slightly lower risk (rate ratio (RR): 0.90, 95% CI: 0.83, 0.97) of LRI between secondary and primary ccCoV infections among participants under the age of 5. CONCLUSIONS: ccCoV spreads annually among children with the greatest burden among ages 0-1. Reinfection is common; prior infection is associated with slight protection against LRI among the youngest children.

ABSTRACT

Introducción: Las complicaciones mamarias relacionadas con la lactancia materna suelen aparecer entre el primer y el décimo día posparto, las cuales pueden provocar lesiones en la salud de la mujer y en el destete precoz. Por lo tanto, la acción de la enfermera frente a estas condiciones es de gran importancia social. Objetivo: Comprender la actuación de enfermería ante complicaciones mamarias relacionadas con la lactancia materna. Métodos: Estudio descriptivo con enfoque cualitativo basado en la Teoría de las Representaciones Sociales, realizado con 17 enfermeras que trabajan en las 29 Unidades de Salud de la Familia del municipio de Porto Seguro, Bahía, Brasil, en el período de febrero a junio de 2021. Como técnica de recolección de datos, se utilizó la entrevista guiada por un guion semiestructurado. Los datos empíricos se analizaron mediante el análisis de contenido temático propuesto por Bardin. Resultados: Los resultados obtenidos tras el análisis fueron interpretados y apoyados en la Teoría de las Representaciones Sociales, que permitió la construcción de la categoría: Praxis de las enfermeras frente a las complicaciones mamarias. La categoría demostró que el conocimiento de las enfermeras sobre las complicaciones mamarias es insuficiente para anclar la práctica del cuidado. Conclusión: El conocimiento de las enfermeras sobre las complicaciones mamarias relacionadas con la lactancia materna demostró ser insuficiente para ofrecer prácticas asistenciales eficaces y actualizadas, necesarias para una actuación adecuada y que favorezcan el mantenimiento de la lactancia materna durante el tratamiento, por lo que se requiere educación permanente sobre el tema.


Introduction: Breast complications related to breastfeeding usually appear between the first and tenth postpartum day. They may appear with lesions to women's health and early weaning. Therefore, the nurse's action in the face of these conditions is of great social importance. Objective: To understand the nursing action in the face of breast complications related to breastfeeding. Methods: A descriptive study with a qualitative approach and based on the social representations theory was conducted with 17 nurses working in the 29 family health units of Porto Seguro Municipality, Bahia State, Brazil, in the period from February to June 2021. The used data collection technique was the interview guided by a semistructured script. The empirical data were analyzed using the thematic content analysis proposed by Bardin. Results: The results obtained after the analysis were interpreted and supported by the social representations theory, which allowed the construction of the category praxis of nurses in the face of breast complications. The category showed that the nurses' knowledge about breast complications is insufficient to anchor the practice of care. Conclusion: The nurses' knowledge about breast complications related to breastfeeding proved to be inept to offer effective and updated care practices, necessary for an adequate performance and favoring the maintenance of breastfeeding during treatment; therefore, permanent education on the subject is required.

Resumo: Este trabalho objetivou caracterizar internações de residentes no Paraná, Brasil, ocorridas no período neonatal em município diferente do de residência, entre 2008 e 2019, e descrever redes de deslocamento para o primeiro e o último biênio da série, correspondentes aos períodos anterior e posterior a iniciativas de regionalização dos serviços de saúde no estado. Dados sobre internações de crianças com idade entre 0 e 27 dias foram obtidos por meio do Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH-SUS). Para cada biênio e regional de saúde, foram calculados a proporção de internações ocorridas fora do município de residência, a distância média ponderada pelo fluxo dos deslocamentos, bem como indicadores de saúde e de oferta de serviços. Modelos mistos foram ajustados para avaliar a tendência bianual dos indicadores e para verificar fatores associados à taxa de mortalidade neonatal (TMN). No total, 76.438 internações foram selecionadas, variando de 9.030, em 2008-2009, a 17.076, em 2018-2019. A comparação entre as redes obtidas para 2008-2009 e as existentes em 2018-2019 evidenciou aumento no número de destinos frequentes e na proporção de deslocamentos dentro da mesma regional de saúde. Observou-se tendência decrescente para a distância, para a proporção de nascidos vivos com Apgar no quinto minuto ≤ 7 e para a TMN. Na análise ajustada para a TMN, além do efeito de biênio (-0,64; IC95%: -0,95; -0,28), apenas a proporção de nascidos vivos com idade gestacional inferior a 28 semanas apresentou significância estatística (4,26; IC95%: 1,29; 7,06). A demanda por assistência hospitalar no período neonatal aumentou ao longo do período estudado. As redes de deslocamento sugerem impacto positivo da regionalização, embora o investimento em regiões com potencial para se tornarem polos assistenciais seja necessário.


Abstract: This study aimed to characterize hospitalizations of residents in Paraná State, Brazil, that occurred during the neonatal period in a municipality different from their place of residence from 2008 to 2019, and to describe displacement networks for the first and last biennium of the series, corresponding to periods before and after initiatives to regionalize health services in the state. Admissions of children aged from 0 to 27 days were obtained from the Hospital Information System of the Brazilian National Unified Health System (SIH-SUS) database. For each biennium and health region, the proportion of admissions that occurred outside the municipality of residence, the weighted average distance traveled, and indicators of health and service provision were calculated. Mixed models were fitted to evaluate the biennial trend of the indicators and to investigate factors associated with the neonatal mortality rate (NMR). In total, 76,438 hospitalizations were selected, ranging from 9,030 in 2008-2009 to 17,076 in 2018-2019. The comparison of the networks obtained for 2008-2009 and 2018-2019 revealed an increase in the number of frequent destinations and in the proportion of displacements within the same health region. A decreasing trend was observed for distance, the proportion of live births with 5-minute Apgar ≤ 7, and for NMR. In the adjusted analysis for NMR, besides the biennial effect (-0.64; 95%CI: -0.95; -0.28), only the proportion of live birth with gestational age < 28 weeks showed statistical significance (4.26; 95%CI: 1.29; 7.06). The demand for neonatal hospital care increased over the study period. The displacement networks suggest a positive impact of regionalization, although investment in regions with the potential to become healthcare centers is necessary.


Resumen: El presente trabajo tuvo como objetivo caracterizar las hospitalizaciones de residentes en el Paraná, Brasil, ocurridas en el período neonatal en un municipio diferente al de su residencia entre el 2008 y el 2019, y describir las redes de desplazamiento para el primer y último bienio de la serie, correspondientes al período anterior y posterior a iniciativas de regionalización de servicios de salud en el Estado. Las hospitalizaciones de niños con edades entre 0 y 27 días se obtuvieron del Sistema de Información Hospitalaria del Sistema Único de Salud (SIH-SUS) y, para cada bienio y regional de salud, se calculó la proporción de ingresos hospitalarios que ocurrieron fuera del municipio de residencia, la distancia media calculada por el flujo de desplazamientos, así como indicadores de salud y de oferta de servicios. Se ajustaron modelos mixtos para evaluar la tendencia bianual de los indicadores y para verificar factores asociados a la tasa de mortalidad neonatal (TMN). En total, se seleccionaron 76.438 hospitalizaciones, que varían desde 9.030 en 2008-2009 a 17.076 en 2018-2019. La comparación de las redes obtenida para 2008-2009 y 2018-2019 mostró un aumento en el número de destinos frecuentes y en la proporción de desplazamientos dentro de la misma regional de salud. Se observó una tendencia decreciente para la distancia, la proporción de nacidos vivos con Apgar al 5º minuto ≤ 7 y para la TMN. En el análisis ajustado para la TMN, además del efecto de bienio (-0,64; IC95%: -0,95; -0,28), solo la proporción de nacidos vivos con edad gestacional < 28 semanas presentó significativa estadística (4,26; IC95%: 1,29; 7,06). La demanda de asistencia hospitalaria en el período neonatal aumentó a lo largo del período estudiado. Las redes de desplazamiento sugieren un impacto positivo de la regionalización, aunque es necesaria la inversión en regiones con potencial para convertirse en polos asistenciales.

ABSTRACT

OBJECTIVE: To assess the receipt of health care services among live-born infants of women with and without evidence of Zika virus (ZIKV) infection while pregnant during the 2016-2017 ZIKV outbreak in Puerto Rico. METHODS: We used data from the Pregnancy Risk Assessment Monitoring System-Zika Postpartum Emergency Response study telephone surveys to examine maternal reports of the receipt of health care services by infants born in Puerto Rico from August through December 2016 and November through December 2017. Evidence of ZIKV infection was ascertained from the infant's birth certificate or was self-reported in the survey. RESULTS: Fourteen percent of women in 2016 and 9% in 2017 had evidence of ZIKV infection during pregnancy. Most infants of women with evidence of ZIKV received the recommended health care services in 2016 and 2017, respectively, including a hearing test (91% vs. 92%), developmental assessment (90% vs. 92%), and an eye exam (74% vs. 70%); fewer received a head scan (45% vs. 36%) and evaluation for physical therapy (17% vs. 10%). From 2016 to 2017, the proportion of infants having a personal doctor increased for all infants; for infants of women without evidence of ZIKV infection, receiving hearing, developmental, and eye assessments increased. CONCLUSION: Most infants of women with evidence of ZIKV infection during pregnancy received the recommended hearing and developmental assessments during the ZIKV outbreak. Experiences with increasing service capacity during the ZIKV outbreak can be evaluated to inform the response to future emergencies that affect maternal and child health.

ABSTRACT

Background: Pregnancy is a critical developmental window in which optimal maternal nutrition and health are key for pregnancy and infant development. The COVID-19 pandemic is considered as a "natural experiment" in which maternal and infant nutrition and health challenges were faced especially in developing countries. Therefore, understanding the health consequences for mothers and infants living in the COVID-19 era is key to revisit public health measures focused on maternal and infant health. The current work aims to describe the design, methods, and descriptive information at recruitment and preliminary findings of the Chilean Maternal & Infant Cohort Study II (CHiMINCs-II) cohort. Methods: The CHiMINCs-II is an ongoing cohort that is part of the Chilean Maternal and Infant Nutrition Observatory of the South-East area of Santiago, Chile. In total, 1954 pregnant women beneficiaries of the public health systems and their offspring were recruited before 15 weeks of gestation and are followed across pregnancy (<15, 26-28, and 35-37 weeks of gestation) and up to 2 years of age in their offspring. Two studies are currently nested within the CHiMINCs-II cohort: (1) Breast Cancer Risk Assessment in Mothers (BRECAM) study, and (2) the CHiMINCs-COVID study. The primary objective of BRECAM study is to test the association between maternal metabolic indicators (i.e., insulin, glucose, insulin growth factor 1, and hemoglobin A1c concentrations) at early pregnancy (i.e., <15 and 26-28 weeks of gestation) and breast density 3 months after the cessation of lactation. For this purpose, we collect maternal obstetric, lifestyle, dietary intake, anthropometric, and biochemical information. The aim of the CHiMINCs-COVID study is to assess maternal dietary intake and mental health problems derived from the COVID-19 pandemic and their association with maternal and infant's health and nutrition. Thus, we collected detailed information on dietary behaviors, mental health, and COVID-related information at each trimester, along with neonatal and infant nutritional information. Discussion: The findings of this study will provide novel and critical information to better understand maternal nutritional status, mental health, as well as infant growth and nutrition during the COVID-19 era. ABSTRACT

Motivational Interviewing (MI) has been included in dentistry programs. There exists a need for interventions in the mother-child dyad. The aim of this paper was to compare the effects of a MI-based educational program on oral care knowledge, attitudes, and practices (KAP) in the mother-child dyad to a Traditional Education-based program (TE). A community intervention trial was carried out. The experimental and control groups were made up of women between 18 and 45 years of age in the sixth month of gestation. Both groups were provided with TE. The experimental group additionally received a session based on the principles of the MI. Socio-demographic data, children's oral health KAP (COHKAP), that of the mother (MOHKAP), and maternal self-efficacy (MSE) in relation to children's oral health (COHMSE) were recorded. A baseline measurement was made, as well as a six-month follow up. The participants included 135 women with an average age of 24.88 ± 6.00. After intervention, the experimental group's COHKAP, MOHKAP, and COHMSE all increased (p < 0.001). When MI-based interventions are combined with TE, MSE and dental care KAP for the mother-child dyad in pregnant women can be improved.

Introducción: La Práctica Avanzada de Enfermería constituye una práctica con alcance expandido, que busca responder a problemáticas de los pacientes en un área clínica específica. Objetivo: Identificar de las funciones declaradas práctica avanzada a nivel internacional, las que realiza el personal de enfermería de la atención materna infantil en el primer nivel de atención. Métodos: Estudio descriptivo transversal, en Villa Clara, Cuba, entre febrero y diciembre del 2021. Universo de 671 licenciados, muestreo probabilístico con un 95 por ciento de confiabilidad de la muestra seleccionada, con 326 enfermeros que laboran en los 35 policlínicos y atienden el programa Materno Infantil. Se aplicó el instrumento de definición del rol de Enfermera de Práctica Avanzada, se calculó el promedio de las funciones agrupadas por dominios, la desviación estándar y coeficiente de variación entre sus ítems; el Alfa de Cronbach y un índice, que estableció correlación entre los ítems para medir consistencia interna de la escala por dimensiones y a nivel global. Resultados: El total de los dominios se encuentra por encima del indicador establecido para cada dimensión que la acredita como práctica avanzada. No se encontró ningún dominio con el índice bajo, los seis mantuvieron una validez y fiabilidad por encima de 0,5. La consistencia interna por alfa de Cronbach obtenida fue alta. Conclusiones: Las funciones de práctica avanzada reconocidas a nivel internacional se ejecutan por las enfermeras, incluidas en el estudio en su práctica diaria, en tanto los diferentes dominios del instrumento alcanzaron valores superiores al indicador establecido(AU)


Introduction: Advanced nursing practice presents an expanded scope that seeks to respond to patient problems in a specific clinical area. Objective: To identify, from among the functions declared as advanced practice at the international level, those performed by the nursing personnel in mother and infant care at the first level. Methods: A cross-sectional and descriptive study was carried out in Villa Clara, Cuba, between February and December 2021. The study universe was made up of 671 graduates. Through probabilistic sampling with 95 percent of reliability, a sample was selected, consisting of 326 nurses working in the 35 polyclinics of the province and in charge of the Mother and Infant Care Program. An advanced practice nurse role delineation instrument was applied. The calculations were made corresponding to the average number of functions grouped by domains, standard deviation, and the coefficient of variation between its items; as well as Cronbach's alpha and an index establishing a correlation between the items for measuring the internal consistency of the scale by dimensions and globally. Results: All the domains are above the indicator established for each dimension accrediting it as advanced practice. No domain was found with a low index; all six maintained a validity and reliability above 0.5. The internal consistency obtained using Cronbach's alpha was high. Conclusions: The internationally recognized advanced practice functions are performed by the nurses, included in the study as part of their daily practice, since the different domains of the instrument reached values above the established indicator(AU)

OBJECTIVES: Breast-feeding is sensitive to somatic, hormonal, behavioral and psychological components of maternal capital. However, through grandmothering, older women may also influence breast-feeding by transferring informational resources to their daughters. We hypothesized that mothers with prolonged instrumental support from their own mother are more likely to have received advice and to have favorable attitudes/practices regarding breastfeeding, compared to those lacking such support, with implications for the grandchild's somatic capital. METHODS: We recruited 90 mother-infant dyads (52 with grandmaternal support, 38 without) in Merida, Yucatan, Mexico. All children were first-borns, aged ~2 years. Anthropometry and body composition were assessed. Data on grandmother's breastfeeding advice and maternal breastfeeding duration were obtained by questionnaire. Maternal attitudes to breast-feeding were assessed using the Iowa Infant Feeding Attitude Scale. RESULTS: Women with instrumental support were more likely to have received grandmaternal advice during pregnancy/infancy on exclusive breast-feeding duration (60% vs. 37%, p = 0.033) and the type of first complementary food (81% vs. 47%, p = 0.001). However, women with support had a less favorable attitude to breastfeeding than those without and breastfed their children for less time (median 5 vs. 10.5 months, p = 0.01). No group differences were found in children's length, weight, skinfolds or lean mass z-score. DISCUSSION: Although grandmothers providing instrumental support provided advice regarding breastfeeding, their attitudes may reflect issues beyond nutritional health. Advice of maternal grandmothers did not promote extended breastfeeding, however the differences in breastfeeding attitudes were not associated with the children's nutritional status. Grandmothers should be included in public health interventions promoting breastfeeding.

OBJECTIVE: To assess the impact of recent federal statute changes mandating child welfare-based Plan of Safe Care (POSC) supportive programming and community-based linkages to treatment providers, resources, and services for families of infants affected by prenatal substance exposure (IPSE). STUDY DESIGN: Retrospective review of Delaware's statewide child welfare case registry data for IPSE birth notifications and subsequent hotline reports for serious physical injury/fatality concerns from November 1, 2018-October 31, 2020. Abstracted variables included IPSE sex, substance exposure type, family characteristics (maternal personal child welfare history or mental health diagnosis, treatment engagement), and POSC referrals. RESULTS: Of 1436 IPSE, 1347 (93.8%) had POSC support. Most IPSE (67.2%) had exposure to single substance types prenatally. Nearly 90% avoided out-of-home placement. Nearly one-fourth of mothers delivered a prior IPSE; 40% of mothers had personal histories of childhood protective services involvement. Also, 43.5% of mothers and 9.1% of fathers were referred to community-based resources, including substance use, mental health treatment, parenting classes, and home visiting nursing. Nearly 58% of IPSE were referred for pediatric/developmental assessment. Notably, 0.82% (11 out of 1347) of IPSE with POSC sustained serious physical or fatal injury. CONCLUSIONS: POSC promote supportive, potentially protective linkages to community-based programming for IPSE and their families.

OBJECTIVE: We hypothesized that a cumulative heart rate characteristics (HRC) index in real-time throughout the neonatal intensive care unit (NICU) hospitalization, alone or combined with birth demographics and clinical characteristics, can predict a composite outcome of death or neurodevelopmental impairment (NDI). STUDY DESIGN: We performed a retrospective analysis using data from extremely low birth weight infants who were monitored for HRC during neonatal intensive care. Surviving infants were assessed for NDI at 18-22 months of age. Multivariable predictive modeling of subsequent death or NDI using logistic regression, cross-validation with repeats, and step-wise feature elimination was performed each postnatal day through day 60. RESULTS: Among the 598 study participants, infants with the composite outcome of death or moderate-to-severe NDI had higher mean HRC scores during their stay in the NICU (3.1 ± 1.8 vs 1.3 ± 0.8; P < .001). Predictive models for subsequent death or NDI were consistently higher when the cumulative mean HRC score was included as a predictor variable. A parsimonious model including birth weight, sex, ventilatory status, and cumulative mean HRC score had a cross-validated receiver-operator characteristic curve as high as 0.84 on days 4, 5, 6, and 8 and as low as 0.78 on days 50-52 and 56-58 to predict subsequent death or NDI. CONCLUSIONS: In extremely low birth weight infants, higher mean HRC scores throughout their stay in the NICU were associated with a higher risk of the composite outcome of death or NDI.
