ABSTRACT

OBJECTIVE: To explore application value and effectiveness of virtual reality technology combined with isokinetic muscle strength training in the rehabilitation of patients after anterior cruciate ligament (ACL) reconstruction surgery. METHODS: Forty patients who underwent ACL reconstruction surgery from December 2021 to January 2023 were selected and divided into control group and observation group according to treatment methods, 20 patients in each group. Control group was received routine rehabilitation training combined with isokinetic muscle strength training, including 15 males and 5 females, aged from 17 to 44 years old, with an average of (29.10±8.60) years old. Observation group was performed virtual reality technology combined with isokinetic muscle strength training, including 16 males and 4 females, aged from 17 to 45 years old with an average of (30.95±9.11) years old. Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque between two groups at 12 (before training) and 16 weeks (after training) after surgery were compared. RESULTS: All patients were followed up for 1 to 6 months with an average of (3.30±1.42) months. There were no statistically significant difference in Lysholm knee joint score, peak knee extension peak torque, and peak knee flexion peak torque between two groups (P>0.05) before training. After training, Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque of both groups were improved compared to before training (P<0.05);there were significant difference in Lysholm knee joint score, knee extension peak torque, and knee flexion peak torque between two groups(P<0.05). CONCLUSION: The application of virtual reality technology combined with isokinetic muscle strength training could promote recovery of knee joint function and enhance muscle strength in patients after ACL reconstruction surgery in further.

ABSTRACT

Objectives: The study aimed to compare the impact of isokinetic strength training (IST) with core stability training (CST) in terms of trunk muscle strength and quality of life (QoL) after surgical repair of incisional hernia in adolescents. Patients and methods: The prospective, randomized, comparative study was conducted at the College of Applied Medical Sciences, Department of Health and Rehabilitation Sciences between August 2019 and June 2020. Thirty patients (16 males, 14 females; mean age: 17.1±0.6 years; range, 12 to 18 years) who had undergone primary incisional hernia repair surgery were included in the study. The patients were randomly divided into two groups: the IST group of 15 patients who underwent the isokinetic exercise program for trunk muscles and the CST group of 15 patients who practiced the core exercise program. Each group exercised three times per week for six weeks. All patients were assessed pre-and post-treatment by an isokinetic dynamometer for trunk flexors and extensors at 60 and 90°/s angular velocities and the European Registry for Abdominal Wall Hernias (EuraHS)-QoL questionnaire. Results: The trunk flexor and extensor Peak torque/body weight ratio at 60 and 90o/s angular velocities and QoL were significantly improved in both IST and CST groups (p<0.05). The post-treatment values of both groups revealed marked but not statistically significant differences in all measured variables (p>0.05). Conclusion: Both IST and CST are effective training programs that can improve the strength of trunk flexors and extensors as well as QoL after surgical repair of incisional hernia in adolescents.

ABSTRACT

OBJECTIVE: To present a case of a right-handed professional tennis player with a left internal abdominal oblique muscle tear sustained while serving. We document the progress of a rehabilitation program consisting of primarily kinetic chain isokinetic strength training where symmetrical measurements (<10% side difference) of force and dynamic postural control were used as criteria for return to sport. METHODS: Isokinetic exercises (bilateral rotational pull, unilateral vertical press and unilateral anterior push) to target trunk specific demands of the serve (flexion, rotation and lateral flexion) were done using robotic resistance. Dynamic postural control was assessed using the hand reach star excursion balance test (HSEBT). The rehabilitation program lasted 3.5 weeks (eight sessions). RESULTS: At baseline isokinetic strength tests that imposed concentric muscle function demands of the injured muscle had lower force measurements (range: -32.1 to -71.9%). These force measurements improved (range: 166.1-296.5%) and were symmetrical (range: +1.6 to +7.3%) on return to sport. In addition, the HSEBT test with the greatest asymmetry (-20 cm) improved to symmetrical (+2 cm) on return to sport. CONCLUSION: Symmetrical isokinetic force and dynamic postural control measurements were successful return to sport criteria as the player since has remained pain free (2 years).

ABSTRACT

Objective To study the effect of isokinetic strength training on upper limb motor function and activities of daily living (ADL) in stroke patients with hemiplegia. Methods From January to December, 2016, 56 stroke patients with hemiplegia were randomly di-vided into control group and observation group equally. Both groups received routine rehabilitation training, and the observation group ac-cepted hemiplegic limb flexor and extensor muscle isokinetic strength training in addition, for four weeks. They were assessed with Fugl-Meyer Assessment-upper limb (FMA-UL), modified Ashworth Scale (MAS), modified Barthel Index (MBI) and biomechanical index-es of elbow flexion and extension with Biodex System-4 before and after treatment. Results The scores of FMA and MBI improved after treatment (t>7.083, P2.369, P2.149, P2.027, P<0.05), except the peak torque at 60°/s angular velocity. Conclusion Isokinetic strength training can improve the motor function of upper limbs and activities of daily living in stroke patients with hemiplegia.

ABSTRACT

Objective To observe any effects of using isokinetic strengthening exercises combined with intra-articular injection of sodium hyaluronate and joint mobilization in treating patients with knee osteoarthritis (KOA).Methods Eighty-one KOA patients were divided into a combined treatment group,a conventional treatment group and a control group.The combined treatment group received isokinetic strengthening training,intra-articular injections of sodium hyaluronate and joint mobilization therapy; the conventional treatment group received the injections and mobilization only; the control group was instructed to do ankle pumps at home on their own.Before the start of treatment and after 4 weeks,knee pain was self-assessed [using a visual analogue scale (VAS) for pain] along with knee range of motion (ROM),quality of life (using the WOMAC condensed health survey rating scale) and indexes of isokinetic strength [including the knee flexors,extensor peak torque (PT),and the flexion and extension angles corresponding peak torque (AOPT)] in both groups.Results After 4 weeks of treatment,statistically significant improvements compared to the baseline values were observed in both groups.The combined treatment group,however,improved to a significantly greater extent than the conventional treatment group in terms of all of the measures.Conclusion Isokinetic strength training combined with intra-articular injections of sodium hyaluronate and joint mobilization has a synergistic effect in enhancing knee stability and improving the performance in activities of daily life of KOA patients.
