ABSTRACT

Regulation of cellular excitability and oscillatory behavior of resting membrane potential in nerve cells are largely mediated by the low-voltage activated T-type calcium channels. This calcium channel family is constituted by three isoforms, namely, CaV3.1, CaV3.2, and CaV3.3, that are largely distributed in the nervous system and other parts of the body. Dysfunction of T-type calcium channels is associated with a wide range of pathophysiologies including epilepsy, neuropathic pain, cardiac problems, and major depressive disorders. Due to their pharmacological relevance, finding molecular agents able to modulate the channel's function may provide therapeutic means to ameliorate their related disorders. Here we used electrophysiological experiments to show that genistein, a canonical tyrosine kinase inhibitor, reduces the activity of the human CaV3.3 channel in a concentration-dependent manner. The inhibitory effect of genistein is independent of tyrosine kinase modulation and does not affect the voltage-dependent gating of the channel. Subsequently, we used computational methods to identify plausible molecular poses for the interaction of genistein and the CaV3.3 channel. Starting from different molecular poses, we carried out all-atom molecular dynamics (MD) simulations to identify the interacting determinants for the CaV3.3/genistein complex formation. Our extensive (microsecond-length) simulations suggest specific binding interactions that seem to stabilize the protein/inhibitor complex. Furthermore, our results from the unbiased MD simulations are in good agreement with the recently solved cryoelectron microscopy structure of the CaV3.1/Z944 complex in terms of both the location of the ligand binding site and the role of several equivalent amino acid residues. Proposed interacting complex loci were subsequently tested and corroborated by electrophysiological experiments using another naturally occurring isoflavone derivative, daidzein. Thus, by using a combination of in vitro and in silico techniques, we have identified interacting determinants relevant to the CaV3.3/genistein complex formation and propose that genistein directly blocks the function of the human CaV3.3 channel as a result of such interaction. Specifically, we proposed that a combination of polar interactions involving the three hydroxyl groups of genistein and an aromatic interaction with the fused rings are the main binding interactions in the complex formation. Our results pave the way for the rational development of improved and novel low-voltage activated T-type calcium channel inhibitors.

BACKGROUND: A paradigm shift towards the use of combined physiological and excisional surgical approaches for the treatment of lymphedema is permeating the surgical practice worldwide. We describe a single-stage surgical approach combining-vaser assisted liposuction (VAL) and lymphatico-venous anastomoses (LVA) for the treatment of extremity-lymphedema. METHODS: Between March 2018 and March 2019, a retrospective review of patients with extremity-lymphedema stage IIb-III International Society of Lymphology who underwent the combined technique was done. Demographics, operative characteristics, clinical outcomes and complications were assessed. Additionally, a systematic review of studies that reported combined physiological procedures with liposuction for the treatment of lymphedema was conducted. RESULTS: A total of 24 patients [12 upper extremity lymphedema (UEL), and 12 lower extremity lymphedema (LEL)] were included and analyzed. The mean age was 54.5 years (38-72 years). The mean circumference reduction rate was 90% and 85% for UEL and LEL, respectively. Infection rate decreased to zero postoperatively in all patients. Ten studies were included in the review: six pooling 220 cases included two-stage procedures, and four pooling 66 cases described one-stage surgeries. All patients who underwent a combined approach reported clinical improvement. CONCLUSIONS: Our combined approach is safe surgical option and allows adequate limb size reduction with faster recovery in selected patients with lymphedema with strong fibroadipose component.

AVL es una enfermedad endémica en algunos estados de Brasil, especialmente en São Paulo, debido a su rápida expansión. Es una preocupación constante para los equipos de salud porque su control es deficiente debido a la ecología de las moscas de arena, que presentan parte de su ciclo de vida en suelos con presencia de materia orgánica en descomposición. El objetivo del presente estudio fue analizar la composición fisicoquímica del suelo de un área endémica de la enfermedad, con la presencia del vector y con la transmisión canina y humana, y compararlo con un área donde no hay presencia. del vector y tampoco la transmisión. Se analizaron un total de 333 muestras de suelo, 161 en un área endémica y 172 en un área no endémica en el Laboratorio de Suelos de la Facultad de Ciencias Agrícolas - FCA / UNESP Campus Botucatu. It is a constant concern for the health teams since their control is deficient because of the ecology of the phlebotomine, which present part of their life cycle in soils with the presence of decomposing organic matter. The present aimed to compare the physical and chemical composition of the soil from an endemic area with the presence of the vector and canine and human transmission with a non-endemic area. A total of 333 soil samples were analyzed, in the Laboratory of Soils of the Faculty of Agronomic Sciences - FCA / UNESP Botucatu Campus. The results revealed important differences in soil composition, which could explain the presence or absence of the vector in certain municipalities. The non-endemic area presented significantly higher results for Total Sand, Organic Matter, Hydrogen + Aluminum, Magnesium, Cations, Copper and Iron, and a significantly lower amount of Clay, presenting a more acidic pH than the area endemic. There are no other studies that precisely define that macro or micronutrients present in soils with organic matter would have the function of harming or aiding the development of larval forms of the vector.
