ABSTRACT

INTRODUCTION: Spindle cell carcinoma (SpCC) or sarcomatoid carcinoma, is a rare variant of squamous cell carcinoma (SCC) that has a variable proportion of carcinomatous and sarcomatous components. Here, we reported an immunohistochemical study of a spindle cell carcinoma with a challenging morphological diagnosis. CASE REPORT: A 50-year-old woman with a previous history of nodular melanoma was referred for evaluation of a painful papule in the lower lip. After surgical resection, neoplastic cells showed focal positivity for CK-14, αSMA, p63, and confirmed the strong positivity for S100 and vimentin. Tumor cells were negative for HMB-45, Melan A, SOX-10, AE1/AE3, 34ßE12, CK5-6, CAM5.2, EMA, desmin, calponin, CD10, CD34, and CD68. With these findings, a diagnosis of SpCC was rendered. The patient presented lung and dorsal metastases after 12 months and after 3 years of follow-up, the patient died. CONCLUSION: In summary, a careful correlation of microscopy and immunohistochemical characteristics is required for the proper diagnosis of this lesion.

Spindle cell carcinoma (SpCC) is a rare tumor, which occurs in upper respiratory tract, mainly in larynx. This study aimed to review the clinical and pathological characteristics for diagnosis and prognosis. Retrospective cohort study. All patients with SpCC in upper respiratory tract treated for curative intent was included. All patients were reviewed in search of epithelial component and immunohistochemistry when not found. It was evaluated rate of recurrence and disease-free survival with univariate and multivariate analysis with Kaplan Meier and Cox Regression model adjusted to propensity score indexes (PSI) according to age, gender, site of tumor, stage, surgical treatment, status of margins of surgical resection, lymphatic invasion. There were 16 cases of SpCC.31% were diagnosed with light microscopy and others with immunohistochemistry for epithelial marker. Disease-free survival was higher in early stage disease in univariate and multivariate analysis, as the main prognostic factor. Surgical treatment increases in 2.54 the rate of survival. The SpCC is a rare tumor considered a highly malignant variant of squamous cell carcinoma. It has male predominance and tobacco use as risk factors. Its treatment should follow the same recommendations for squamous cell carcinoma, with surgery as the maintain treatment. Immunohistochemistry is an adjuvant important tool for diagnosis of SpCC.

ABSTRACT

El carcinosarcoma de laringe es un tumor bifásico raro que representa menos del 1% de todos los tumores malignos de laringe. Debido a su doble naturaleza epitelial y mesenquimal esta neoplasia ha sido denominada de distintas maneras en la literatura, siendo indispensable el estudio mediante inmunohistoquímica para establecer un diagnóstico correcto. Se presentan 2 casos de carcinosarcoma de laringe, confirmados mediante estudio con inmunohistoquímica, ambos tratados mediante laringectomía total. Se elabora una discusión de los principales aspectos clínicos, histopatológicos y terapéuticos de esta infrecuente neoplasia.


The larynx carcinosarcoma is a rare biphasic tumor that represents less than 1% of all malignant tumors of the larynx. Because of its biphasic epithelial and mesenchymal nature this neoplasm has been called in different ways in the literature being indispensable the study by immunohistochemistry to establish a proper diagnosis. We present 2 cases of larynx carcinosarcoma confirmed by immunohistochemical study, both treated with total laryngectomy. A discussion of the main clinical, histopathological and therapeutic aspects of this rare neoplasm is made.

A osteopatia hipertrófica é um distúrbio de osteoprodução generalizada do periósteo que acomete os ossos longos, sendo geralmente secundária a alguma doença intratorácica, como doença cardiopulmonar ou neoplasia. A maioria das lesões são bilaterais e simétricas, podendo ocorrer nos quatro membros. O presente trabalho tem como objetivo relatar dois casos de osteopatia hipertrófica associada à metástase pulmonar de carcinoma mamário e os achados clínico patológicos destes cães foram estudados.(AU)


Hypertrophic osteopathy is a generalized bone production disorder the periosteum that affects the long bones, usually secondary to some intrathoracic disease, such as cardiopulmonary disease or cancer. Most lesions are bilateral and symmetrical and can occur in all four limbs. This study aims to report two cases of hypertrophic osteopathy associated pulmonary metastasis of breast carcinoma and the clinical and pathological findings of these dogs were studied.(AU)

ABSTRACT

Spindle cell carcinoma (SpCC) is a rare biphasic tumor of epithelial origin that comprises squamous and sarcomatoid components. The diagnosis may be challenging, particularly when the squamous differentiation is not obvious. SpCC of the tongue has rarely been reported. Here we report a case of SpCC on the right border of the tongue of a 28-year-old male. The diagnosis was based on the correlation of clinical, morphological, and immunohistochemical findings. We describe this rare tumor, present at an unusual location in a young patient to contribute to better understanding and awareness of this rare malignancy.


O carcinoma de células fusiformes (CCF) é um tumor bifásico raro de origem epitelial que compreende componentes escamosos e sarcomatoides. O diagnóstico pode ser desafiador, particularmente quando a diferenciação escamosa não é óbvia. Relatos de CCF em língua são extremamente raros. Aqui é relatado um caso de CCF na borda direita da língua de um homem de 28 anos de idade. O diagnóstico foi baseado na correlação de achados clínicos, morfológicos e imunohistoquímicos. Com a descrição deste caso clínico, em uma localização anatômica incomum e em um paciente jovem, pretende-se contribuir para uma melhor compreensão e conhecimento deste tumor maligno raro.
