ABSTRACT

Daochisan is a pediatric famous classical formula in traditional Chinese medicine(TCM) specializing in clearing the heart, disinhibiting water and enriching Yin, which has been inherited and developed by medical experts of successive generations. The formula can be traced back to Xiaoer Yaozheng Zhijue written by QIAN Yi from Song dynasty, and most of the later-developed Daochisan(Daochitang) is based on this formula with additions and subtractions. So as to promote the literature excavation of this formula, this paper conducts a thorough textual research and analysis of the composition, processing, preparation methods, clinical applications of Daochisan based on a systematic collation of relevant ancient and modern literature in the method of bibliometrics. A total of 127 pieces of data involving 86 antient TCM books were collected. Statistical analysis showed that the drug origin of Daochisan is basically clear, we suggest that Rehmanniae Radix is the root tuber of Rehmannia glutinosa of Scrophulariaceae, Akebiae Caulis is the dry stem of Akebia quinata of Lardizabalaceae, Glycyrrhizae Radix et Rhizoma is the dry root and stem of Glycyrrhiza uralensis of legume, bamboo leaf is the dry leaves of Phyllostachys nigra var. henonis of gramineous plants. Four herbs are recommended to use its raw products. The key information of the dosage and decocting method is supposed to be "4.13 g each of raw Rehmanniae Radix, raw Glycyrrhizae Radix et Rhizoma, Akebiae Caulis, adding 300 mL of water, and then adding 4.13 g of bamboo leaf, decocting together to 150 mL, taking warmly after meal". Daochisan has the effect of clearing the heart and enriching Yin, disinhibiting water and relieving stranguria, and is widely used in treating symptoms such as heart vexed hotness, hydrodipsia and reddish complexion. Modern studies have showed that Daochisan are frequently used in treating oral ulcer, herpetic stomatitis, urinary tract infection, herpes zoster and other diseases. The above research results can provide scientific reference for the future development and research of Daochisan.

ABSTRACT

The relationship between disease and syndrome is a research focus in integrated traditional Chinese and western medicine. Depending on the focus, the disease-syndrome combination for treatment is manifested as the different treatment methods for the same disease and the same treatment method for different diseases based on the syndrome, and different treatment methods for the same syndrome and the same treatment method for different syndromes based on the disease. The mainstream model is the combination of di-sease identification in modern medicine with syndrome identification and core pathogenesis in traditional Chinese medicine. However, current research on the combination of disease and syndrome and core pathogenesis tends to focus on the heterogeneity between disease and syndrome and the separation of syndrome and treatment. Therefore, the study proposed the research idea and model of core formulas-syndromes(CFS). According to the theory of formula-syndrome correspondence, the research idea of CFS deepens the research on core pathogenesis, which aims to summarize the core formulas and syndromes for diseases. The research fields include diagnostic criteria for the indications of formulas, distribution patterns of formulas and syndromes for diseases, the evolution of medicinal-syndrome based on formulas-syndromes, formula combination law based on formulas-syndromes, and the dynamic evolution of formulas-syndromes. Through the summary of ancient classics, clinical experience, and medical records, and with the methods of expert consultation, factor analysis, and clustering analysis, research on the diagnostic criteria for the indications of formulas aims to explore the diagnosis information such as the diseases, symptoms, signs, and pathophysiology. The research on the distribution patterns of formulas and syndromes for diseases tends to summarize the specific types of formulas and syndromes for the diseases through literature research and clinical cross-sectional studies based on the establishment of diagnostic criteria for the indications of formulas. The research on the evolution of medicinal-syndrome aims to clarify the medicinal-syndrome law through literature and clinical research. The formula combination law refers to the fact that the core prescriptions for a disease often appear in combination with other prescriptions on a regular basis. The dynamic evolution of formulas-syndromes refers to the continuous transformation and change of formulas and syndromes in the process of disease development with changes in time and space. The CFS is conducive to the unification of disease, syndrome and treatment and to the deepening of the research model of disease and syndrome integration.

ABSTRACT

To maintain the precision and stability of the efficacy of classical formulas, this study compared the origins and specifications of Bupleuri Radix and revealed the precise application regularity of Bupleurum chinense(Beichaihu) and Bupleurum scorzonerifolium(Nanchaihu) in classical formulas. The efficacy and indications of formulas with Bupleuri Radix as the sovereign drug in the Treatise on Cold Damage and Miscellaneous Diseases(Shang Han Za Bing Lun) were investigated. The difference in the efficacy of Bupleuri Radix as well as the differences in the chemical composition, and liver-protecting and lipid-lowering effects of the decoctions of Beichaihu and Nanchaihu were analyzed with LC-MS technology based on the CCl_4-induced liver injury model in mice and sodium oleate-induced HepG2 hyperlipidemia cell model. The results showed that seven classical formulas with Bupleuri Radix as the sovereign drug in the Treatise on Cold Damage and Miscellaneous Diseases were mainly used in the treatment of digestive, metabolic, immune, circulatory, and other diseases. Bupleuri Radix mainly played the functions of protecting the liver, benefiting the gallbladder, and lowering the lipid, and had different focuses in different formulas. There were 14 differential components in the decoctions of Beichaihu and Nanchaihu, and the chemical structures of 11 components were identified, including 10 saponins and one flavonoid. The results of the liver-protecting efficacy experiment showed that compared with the Nanchaihu decoction, Beichaihu decoction could reduce the serum aspartate aminotransferase(AST) activity in liver injury model mice(P<0.01). The results of the lipid-lowering efficacy experiment proved that Beichaihu and Nanchaihu decoctions both showed highly significant differences in lowering the total cholesterol(TC) and triglyceride(TG) content in HepG2 cells(P<0.01), and Nanchaihu decoction was superior to Beichaihu decoction in lowering the lipid. The results of this study preliminarily proved that there were differences in chemical composition, and liver-protecting and lipid-lowering effects of Beichaihu and Nanchaihu decoctions, indicating that it was necessary to determine the precise origin of Bupleuri Radix in the clinical formulation of traditional Chinese medicine. The study provides a scientific basis for both precise clinical medication and purpose-based accurate quality evaluation of traditional Chinese medicine in clinical application.

ABSTRACT

Through consulting the ancient and modern literature, this paper makes a textual research on the name, origin, producing area, harvesting and processing methods of Asini Corii Colla, so as to provide a basis for the development of the famous classical formulas containing the medicinal material. Before the Tang dynasty, cow leather was the main source of Asini Corii Colla, and donkey was rare as an introduced species. From the end of Tang dynasty to Song dynasty, due to the development of doctors' understanding of the properties and effects of medicines, with the increase of the number of donkeys and the limitation of the use of cow leather, the source of Asini Corii Colla changed from cow leather to donkey skin. During the Ming and Qing dynasties, the theory of medicine property was further developed, and all doctors basically agreed that black donkey skin and E-well water were two essential factors for making genuine Asini Corii Colla. Therefore, it is suggested that Asini Corii Colla should take Equus asinus as the authentic origin in the development of the famous classical formulas, attach importance to the quality of water source, take Liaocheng in Shandong province as the authentic producing area, and the processing should be carried out in accordance with the requirements of the 2020 edition of Chinese Pharmacopoeia.

ABSTRACT

Xiao Xumingtang in The Catalogue of Famous Ancient Classics (The First Batch) issued by the National Administration of Traditional Chinese Medicine is derived from the Important Prescriptions Worth a Thousand Gold for Emergency (Bei Ji Qian Jin Yao Fang) written by SUN Si-miao in the Tang dynasty. The present study systematically explored the origin, development, historical evolution, and clinical application of Xiao Xumingtang. As revealed by the results, Xiao Xumingtang as well as its analogues are primary prescriptions indicated for apoplexy before the Tang and Song dynasties and serve as the benchmark for the treatment of apoplexy. After the Song dynasty, due to the changes in the understanding of the pathogenesis of apoplexy and the limitations of the understanding of Xiao Xumingtang, its clinical application to apoplexy gradually decreased. In modern times, it has been re-recognized and applied, during which its clinical applications have undergone great changes. Its clinical applications are extensive, involving a variety of diseases related to the brain and nervous systems, such as stroke and its sequelae, peripheral facial paralysis, rheumatoid arthritis, hypertension, and other diseases related to the motor nervous system. Its primary indications are stroke and its sequelae, followed by peripheral facial paralysis. Other new indications are gradually found. This study is expected to provide references for the clinical application of Xiao Xumingtang and the transformation of new drugs.

ABSTRACT

BACKGROUND: It is essential to identify the chemical components for the quality control methods establishment of Chinese Classical Formula (CCF). However, CCF are complex mixture of several herbal medicines with huge number of different compounds and they are not equal to the combination of chemical components from each herb due to particular formula ratio and preparation techniques. Therefore, it is time-consuming to identify compounds in a CCF by analyzing the LC-MS/MS data one by one, especially for unknown components. METHODS: An ultra-high pressure liquid chromatography-linear ion trap-orbitrap high resolution mass spectrometry (UHPLC-LTQ-Orbitrap-MS/MS) approach was developed to comprehensively profile and characterize multi-components in CCF with Erdong decoction composed of eight herbal medicines as an example. Then the MS data of Erdong decoction was analyzed by MS/MS-based molecular networking and these compounds with similar structures were connected to each other into a cluster in the network map. Then the unknown compounds connected to known compounds in a cluster of the network map were identified due to their similar structures. RESULTS: Based on the clusters of the molecular networking, 113 compounds were rapidly tentative identification from Erdong decoction for the first time in the negative mode, which including steroidal saponins, triterpenoid saponins, flavonoid O-glycosides and flavonoid C-glycosides. In addition, 10 alkaloids were tentatively identified in the positive mode from Nelumbinis folium by comparison with literatures. CONCLUSION: MS/MS-based molecular networking technique is very useful for the rapid identification of components in CCF. In Erdong decoction, this method was very suitable for the identification of major steroidal saponins, triterpenoid saponins, and flavonoid C-glycosides.

ABSTRACT

Kaixinsan recorded in an ancient medical book named Beiji Qianjin Yaofang is one of the famous classical formula, which is one of the common prescriptions used by ancient physicians to treat amnesia. In the process of using this prescription, later generations of physicians derived many kinds of associated prescriptions. The effect and indications of these prescription have been inherited and expanded from those in the Beiji Qianjin Yaofang because of the changes in dosages. Therefore,it is necessary to verify the effect and indications of the formulas and the rules of dosage changes. The research results showed that its basic effects included to nourish the mind, induce resuscitation, strengthen the spleen and calm the mind, and keep balance between heart-Yang and kidney-Yin. The main indications included amnesia, sorrow and sadness, fright and fear, and so on,which may differ slightly in different dynasties. In Song,Jin and Yuan dynasties,it also demonstrated the effect of warming the heart and Yang, clearing away heat and relieving wind besides the basic effects, with basically the same indications (slightly different from those in previous dynasties). In Ming dynasty,it demonstrated the effect of nourishing Yin,clearing away heat and nourishing blood besides the basic effects, and the indications expanded to farsightedness, nearsightedness, spermatorrhea and blurred urine. In the Qing dynasty, its effect also included to nourish the heart and kidney on the basis of the Ming dynasty, and the indications were basically the same with those in the previous dynasties. The compatibility ratio of ancient physicians in the application of this prescription and its associated prescriptions showed some remarkable features, for example,Ginseng Radix et Rhizoma and Poria should be increased and their ratio was≈1∶1 in the treatment of amnesia, sorrow, sadness, fright, fear, farsightedness, spermatorrhea and blurred urine, with ratio of Polygalae Radix to Acori Tatarinowii Rhizoma≈1∶1; the dosage of Polygalae Radix and Acori Tatarinowii Rhizoma should be increased in the treatment of nearsightedness, and their ratio was≈1∶1. The compatibility ratio of Polygalae Radix-Ginseng Radix et Rhizoma-Poria-Acori Tatarinowii Rhizoma=2∶3∶3∶2 was the most frequent, which basically included the indications of this prescription and its associated prescriptions. According to statistics, the average dosages that ancient physicians used were significantly higher than those in the modern times,Polygalae Radix 57 g,Ginseng Radix et Rhizoma 62 g,Poria 70 g,and Acori Tatarinowii Rhizoma 54 g,respectively in ancient times, while Polygalae Radix 11 g,Ginseng Radix et Rhizoma 15 g,Poria 17 g,and Acori Tatarinowii Rhizoma 9.5 g,respectively in modern times. The above textual research results can provide some reference for preparation of tradition Chinese medicine products of Kaixinsan.

ABSTRACT

Jichuanjian,a famous classical formula with definite clinical efficacy in Jingyue Quanshu (《景岳全书》) by ZHANG Jing-yue in the Ming Dynasty, was included in the Catalogue of Ancient Famous Classical Formulas (The First Batch) published by the National Administration of Traditional Chinese Medicine(TCM) in 2018.By means of bibliometrics,43 relevant ancient book data were collected, including 26 ancient books of TCM. The historical origin and development, main indications and symptoms, composition and prescription, dosage, processing, and decoction and administration methods of Jichuanjian were systematically summarized to provide literature support for the development and clinical application of famous classic formulas. It was found that the main symptoms of Jichuanjian were expanded in the past dynasties from deficiency constipation, postpartum constipation, and qi stagnation recorded in the original formula to additional dryness accumulation, blood stasis, Yin constipation, infantile constipation, and lung(intestine)diseases induced by eye diseases. The applicable population of Jichuanjian was extended to children in addition to puerpera and adults, and the disease scope was extended to pediatrics, ophthalmology, and otorhinolaryngology in addition to internal medicine, gynecology and obstetrics.Although Jichuanjian was modified by doctors in the past dynasties, the compatibility of Angelicae Sinensis Radix,Achyranthis Bidentatae Radix,Cistanches Herba,Alismatis Rhizoma,Cimicifugae Rhizoma and Aurantii Fructus has been constant. The decoction and administration methods basically remained unchanged, i.e., water decoction and administration before meals. Meanwhile, doctors in the Qing Dynasty made a lot of general comments on this formula with little controversy.

ABSTRACT

Hypertension is a global major public health problem. Early intervention and timely blocking are necessary for newly-diagnosed hypertensive and young hypertensive patients. Tianma Gouteng Decoction recorded in Zabing Zhengzhi Xinyi written by HU Guangci has the major functions in treating excess syndrome and wind-Yang disturbance syndrome with effects in calming liver-wind, clearing heat and activating blood, and nourishing liver and kidney. As we known, Tianma Gouteng Decoction is a classical formula for treating hypertension. However, due to the limitation of historical conditions, some problems impede Tianma Gouteng Decoction from being handed down successfully, such as uncertain diagnosis, uncertain antihypertensive efficacy and treatment-course. In this case, we ask whether Tianma Gouteng Decoction can be used as antihypertensive therapy and the exactly decrease of systolic and diastolic blood pressure, and wonder the half-life of Tianma Gouteng Decoction and the relationship with Western medicine. Therefore, defined prescription and syndrome, efficacy, treatment-course are important in clinical practice. According to the findings, ① previously, we thought that hypertension was a liver-Yang hyperactivity and liver-wind stirring syndrome, and the liver-based therapy theory was effective. However, based on our study, long-term treatment with Western medicine could block disease progression, control the increased blood pressure and change the pathogenesis and syndrome of traditional Chinese medicine. Therefore, the frequency of liver-Yang hyperactivity syndrome "fire syndrome" decreased sharply, and the main syndrome was fluid-retention syndrome and deficiency syndrome. The etiology and pathogenesis of hypertension was prone to deficiency syndrome accompanied with excess syndrome. Therefore, it was necessary to intervene hypertension in "liver-wind" and "liver-Yang" excess syndrome stage, and the early intervention and treatment are important. ② The newly-diagnosed hypertensive and young hypertensive patients usually had no Western medicine intervention. Thus, the etiology and pathogenesis of hypertension were free from Western medicine, which maintained the initial stage of hypertension in line with the liver-Yang hyperactivity syndrome. ③ The pathological mechanisms of newly-diagnosed and young hypertension were mainly increase in sympathetic activity, active renin-angiotensin-aldosterone system, change in morphology of vascular smooth muscle cells and endothelial cell dysfunction. ④ According to the syndrome of Tianma Gouteng Decoction, in modern medicine, Tianma Gouteng Decoction was widely used to treat the early-stage hypertension and the early-intervention of hypertension without Western medicine, particularly newly-diagnosed, young hypertension, Ⅲ hypertension, hypertensive crisis and hypertensive emergency. Tianma Gouteng Decoction can treat such symptoms as headache, dizziness, fullness of head, slurring of speech; facial flushing, conjunctival congestion and blurred vision; dysphoria, anxiety, palpitation, insomnia, mental disorder and hot flushes; dry-mouth with bitter taste and sweeting; myasthenia of limbs and lassitude in loin and legs; brown urine with burning sensation during urination; a solid or liquid stool consistency; red tongue, slippery and rapid pulse, wiry and rapid pulse over to Cunkou. ⑤ After treatment with Tianma Gouteng Decoction for three to six months, the blood pressure was decreased, and the symptoms were improved with reduction or even discontinuance of Western medicine. ⑥ According to recent studies, Caul is Polygoni Multiflori of Tianma Gouteng Decoction induced certain hepatotoxicity and increased the risk of long-term treatment. However, with the regular monitoring of biomedical index, long-term treatment with Caulis Polygoni Multiflori(3-10 g·d~(-1) for 3 to 6 months) didn't show hepatic dysfunction. ⑦ The pharmacological activity of Tianma Gouteng Decoction coincided with the pathology and disease mechanism of newly-diagnosed and young hypertension. Therefore, Tianma Gouteng Decoction was the specific recipe for newly-diagnosed and young hypertension. Therefore, further studies of efficacy, safety and molecular mechanism of Tianma Gouteng Decoction may make a medical breakthrough for hypertension treatment.

ABSTRACT

This article aims to provide some thoughts on the prevention and treatment of Coronavirus Disease 2019 (COVID-19) from the perspective of JingFang Medicine (Classical Chinese Formula). It is believed that the vague theoretical understanding of COVID-19 in Traditional Chinese Medicine does not hinder the precise treatment of the disease by following the rule of "With this Zheng, prescribe this Fang." According to the principle of "Fang-Zheng Correlation" and the knowledge gained from the thousands of years of experience in treating febrile diseases, Xiao Chai Hu Decoction () and its modifications are recommended with the emphasis on individualized treatment. As another form of practicing "Fang-Zheng Correlation," generalized group treatment should also be paid attention to. Giving considerations to the historical medical data, Jing Fang Bai Du Powder () and Shi Shen Decoction () are recommended for group prevention treatment. Assisting the Zheng (Upright) Qi and using tonic formulas are two entirely different concepts. According to the principle of "Fang-Zheng Correlation," tonics abuse should be avoided in the prevention of COVID-19, and the using of Huang Qi ( Radix Astragali seu Hedysari) should also be very carefully done.

ABSTRACT

The application of classical formula in the treatment of diabetes has a long history. Zhang Zhongjing set up a special chapter on consumptive thirst in Synopsis of the Golden Chamber, listing Baihu Jia Renshen Decoction for exuberant heat in the lung and stomach, dual deficiency of Qi and Yin syndrome, and Shenqi Pills for kidney Qi deficiency syndrome. However, the clinical application is not limited to them. In this study, formulas of Huanglian,Dahuang,Chaihu,Gualougen,Lingzhu,Huangqi and Dihuang are listed as the main therapeutic methods for diabetes mellitus, with effects in clearing heat, dredging the bowels and purging turbid, clearing depression and dispersing knots, nourishing Yin and quenching thirst, invigorating spleen and draining dampness, supplementing Qi and tonifying deficiency, nourishing Yin and tonifying kidney, which have the advantages for the treatment of diabetes and its complications. Based on accurate differentiation of symptoms and signs, consideration shall be given to both " of diseases and syndromes",while emphasis shall be given to the " main symptoms",so as to flexibly apply classical formula and expand the scope of application.

ABSTRACT

"Classical formula" are praised for their exact clinical efficacy, but many scholars of " classical formula" still remain in the simple symptoms and the superficial form of formula matching, failing to explore the connotation of " classical formula" in a subtle and in-depth way. The clinical curative effect and charm of " classical formula" mainly lie in " concise". " Concise" can be divided into many aspects, including " correspondence of formula and syndrome" infers pointing accuracy, " Classic formula" infers structure refining, and " pattern differentiation" infers process elaborating, ect. " Correspondence of formula and syndrome" requires doctors to take into account multiple factors such as physical characteristics, symptom features, diet taste and emotional preference of patients,so as to grasp its essence more accurately. " Classic formula" implies paying special attention to its composition and various methods of addition and subtraction. Not following Zhang Zhongjing's method and gilding the lily will affect the clinical efficacy. " Pattern differentiation" indicates doctors should separate Yin and Yang, examine syndrome features and distinguish the categories following the method of traditional Chinese medicine with comprehensive observation and comprehensive consideration in-depth so as to accurately select the appropriate prescription and improve the clinical efficacy.

ABSTRACT

Objective: The classical formula of Dingxiang Shiditang can be used for hiccup and vomit which caused by many reasons, its molecular mechanism of this formula was investigated.Method: Based on the integrated pharmacology platform of traditional Chinese medicine, the potential molecular mechanism of Dingxiang Shiditang was revealed from the dimensions of multi-components and multi-targets.Result: There were 89 compounds related to hiccup and vomit of Dingxiang Shiditang, and there was 1 common target[cannabinoid receptor 1(CNR1)].On the one hand,CNR1 could inhibit the occurrence of hiccup and vomit by inhibiting the release of γ-aminobutyric acid(GABA),dopamine,5-hydroxytryptamine(5-HT);on the other hand,CNR1 could inhibit gastrointestinal motility and delay gastric emptying, and it was speculated that CNR1 may play a role in regulating gastrointestinal motility through brain-gut axis.Meanwhile, the mechanism of Dingxiang Shiditang may be related to nucleotide metabolism and nervous system. Conclusion: Dingxiang Shiditang may regulate gastrointestinal motility by affecting the release of neurotransmitters, mitochondrial energy metabolism, and achieve its effect on hiccup and vomit based on the joint intervention of multiple targets and multiple pathways.
