ABSTRACT

Background/Objectives: Preterm birth (PTB) remains a significant global health challenge. Previous attempts to predict preterm birth in the first trimester using cervical length have been contradictory. The cervical consistency index (CCI) was introduced to quantify early cervical changes and has shown promise across various clinical scenarios in the mid-trimester, though testing in the first trimester is lacking. This study aims to assess the cervical consistency index performance in predicting preterm birth during the first trimester of pregnancy. Methods: In this prospective cohort study, focused exclusively on research, women with singleton pregnancies, both with and without a history of spontaneous preterm birth (sPTB), were included. The primary outcome was sPTB before 37 weeks, with a secondary outcome of sPTB before 34 weeks. CCI measurements were taken between 11+0 to 13+6 weeks of gestation. Receiver operating characteristic (ROC) curves were generated, and sensitivity and specificity were calculated for the optimal cut-off and for the 5th, 10th, and 15th percentile. Intraobserver and interobserver agreements were assessed using the intraclass correlation coefficient (ICC). Results: Among the 667 patients analyzed, the rates of sPTB before 37 and 34 weeks were 9.2% (61/667) and 1.8% (12/667), respectively. The detection rates (DRs) for CCI predicting PTB before 37 and 34 weeks were 19.7% (12/61) and 33.3% (4/12). Negative predictive values were 91.8% (546/595) and 98.7% (588/596), while the areas under the curve (AUC) for sPTB before 37 and 34 weeks were 0.62 (95% CI: 0.54-0.69) and 0.80 (95% CI: 0.71-0.89), respectively. Of the 61 patients with preterm birth, 13 (21.3%) had a preterm birth history; in this group, the CCI percentile 10th identified 39% (5/13). Intraobserver ICC was 0.862 (95% CI: 0.769-0.920), and interobserver ICC was 0.833 (95% CI: 0.722-0.902). Conclusions: This study suggests that utilizing CCI in the first trimester of pregnancy could serve as a valuable tool for predicting preterm birth before 34 weeks of gestation, demonstrating robust intraobserver and interobserver reliability.

ABSTRACT

The objective of the study was to assess the consistency between self-reported demographic characteristics, health conditions, and healthcare use, and administrative healthcare records, in a sample of enrollees of an Indigenous health organization in Colombia. We conducted a phone survey of a random sample of 2113 enrollees September-2020/February-2021. Administrative health records were obtained for the sample. Using ICD-10 diagnostic codes, we identified individuals who had healthcare visits for diabetes, hypertension, and/or pregnancy. Using unique identifiers, we linked their survey data to the administrative dataset. Agreement percentages and Cohen's Kappa coefficients were calculated. Logistic regressions were performed for each health condition/state. Results showed high degree of agreement between data sources for sex and age, similar rates for diabetes and hypertension, 10% variation for pregnancy. Kappa statistics were in the moderate range. Age was significantly associated with agreement between data sources. Sex, language, and self-rated health were significant for diabetes. This is the first study with data from an Indigenous population assessing the consistency between self-reported data and administrative health records. Survey and administrative data produced similar results, suggesting that Anas Wauu can be confident in using their data for planning and research purposes, as part of the movement toward data sovereignty.

ABSTRACT

Psychotic experiences (PE) are prevalent in general and clinical populations and can increase the risk for mental disorders in young people. The Community Assessment of Psychic Experiences (CAPE) is a widely used measure to assess PE in different populations and settings. However, the current knowledge on their overall reliability is limited. We examined the reliability of the CAPE-42 and later versions, testing the role of age, sex, test scores, and clinical status as moderators. A systematic search was conducted on the Scopus, Web of Science, PubMed, EBSCOhost, ProQuest, and GoogleScholar databases. Internal consistency and temporal stability indices were examined through reliability generalization meta-analysis (RGMA). Moderators were tested through meta-regression analysis. From a pool of 1,015 records, 90 independent samples were extracted from 71 studies. Four versions showed quantitative evidence for inclusion: CAPE-42, CAPE-20, CAPE-P15, and CAPE-P8. Internal consistency indices were good (α/ω≈.725-0.917). Temporal stability was only analyzed for the CAPE-P15, yielding a moderate but not-significant effect (r=0.672). The evidence for temporal stability is scant due to the limited literature, and definitive conclusions cannot be drawn. Further evidence on other potential moderators such as adverse experiences or psychosocial functioning is required.

ABSTRACT

The purpose of the article was to propose a procedure for the statistical processing of data when an extension of the conventional method of expert judgment is used to establish the validity, consistency and reliability of scientific findings. It was applied in the decision making of one of the three dimensions of the variable of the components of the interdisciplinary problem-solving exercise of the teaching-learning process. Empirical methods such as document review, survey and expert judgment were used in the research process. The statistical tests applied showed statistical significance (P < 0.05 to P < 0.001) among the comparisons and/or associations made. ABSTRACT

The ambiguous information in multi-criteria decision-making (MCDM) and the vagueness of decision-makers for qualitative judgments necessitate accurate tools to overcome uncertainties and generate reliable solutions. As one of the latest and most powerful MCDM methods for obtaining criteria weight, the best-worst method (BWM) has been developed. Compared to other MCDM methods, such as the analytic hierarchy process, the BWM requires fewer pairwise comparisons and produces more consistent results. Consequently, the main objective of this study is to develop an extension of BWM using spherical fuzzy sets (SFS) to address MCDM problems under uncertain conditions. Hesitancy, non-membership, and membership degrees are three-dimensional functions included in the SFS. The presence of three defined degrees allows decision-makers to express their judgments more accurately. An optimization model based on nonlinear constraints is used to determine optimal spherical fuzzy weight coefficients (SF-BWM). Additionally, a consistency ratio is proposed for the SF-BWM to assess the reliability of the proposed method in comparison to other versions of BWM. SF-BWM is examined using two numerical decision-making problems. The results show that the proposed method based on the SF-BWM provided the criteria weights with the same priority as the BWM and fuzzy BWM. However, there are differences in the criteria weight values based on the SF-BWM that indicate the accuracy and reliability of the obtained results. The main advantage of using SF-BWM is providing a better consistency ratio. Based on the comparative analysis, the consistency ratio obtained for SF-BWM is threefold better than the BWM and fuzzy BWM methods, which leads to more accurate results than BWM and fuzzy BWM.

ABSTRACT

PURPOSE: To prospectively evaluate the usefulness of T1-weighted imaging (T1WI) and diffusion-weighted imaging (DWI) sequences in predicting the consistency of macroadenomas. In addition, to determine their values as prognostic factors of surgical outcomes. METHODS: Patients with pituitary macroadenoma and surgical indication were included. All patients underwent pre-surgical magnetic resonance imaging (MRI) that included the sequences T1WI before and after contrast administration and DWI with the apparent diffusion coefficient (ADC) map. Post-surgical MRI was performed at least 3 months after surgery. The consistency of the macroadenomas was evaluated at surgery, and they were grouped into soft and intermediate/hard adenomas. Mean ADC values, signal on T1WI and the ratio of tumor ADC values to pons (ADCR) were compared with tumor consistency and grade of surgical resection. RESULTS: A total of 80 patients were included. A softened consistency was found at surgery in 53 patients and hardened in 27 patients. The median ADC in the soft consistency group was 0.532 × 10-3 mm2/sec (0.306 - 1.096 × 10-3 mm2/sec), and in the intermediate/hard consistency group was 0.509 × 10-3 mm2/sec (0.308 - 0.818 × 10-3 mm2/sec). There was no significant difference between the median values of ADC, ADCR and signal on T1W between the soft and hard tumor groups, or between patients with and without tumor residue. CONCLUSION: Our results did not show usefulness of the DWI and T1WI for assessing the consistency of pituitary macroadenomas, nor as a predictor of the degree of surgical resection.

ABSTRACT

Against the background of a lack of screening instruments for measuring depressive symptoms in Colombian adolescents and preadolescents, this study aims to establish the internal consistency reliability, component structure and the concurrent and discriminant validity of the Kutcher Adolescent Depression Six-Item Scale (KADS-6) among preadolescent school students in Sincelejo, Colombia. Participated 710 youth (10.8 years of age ± .75 years) divided into two groups to cross-validate analyses that were undertaken to determine the internal consistency reliability, as well as the concurrent and discriminant validity, of the KADS-6 among preadolescents. Results show that over 95% of the sample did not report problems understanding any of the items on the KADS-6. The KADS-6 had acceptable levels of internal consistency reliability, concurrent and discriminant validity and was unidimensional. In conclusion, The KADS-6 is well understood by Colombian preadolescents and has adequate psychometric properties in adolescents, rendering it acceptable for use with Colombian preadolescents.

ABSTRACT

The Positive Mental Health Questionnaire (PMHQ) has been validated across various populations but has displayed diverse psychometric structures depending on the procedures used. The original version of the PMHQ includes 39 items organized into 6 factors, although there are reports that indicate a reduced structure of between 1 and 4 factors. The aim of this study was to assess the psychometric properties of the PMHQ with 1, 4 and 6 factors. A total of 360 healthcare workers aged 23 to 77 (M = 37.06; SD = 10.79) participated. Construct validity was assessed through confirmatory factor analysis using weighted root mean square residual. The original 6-factor (χ2/df: 3.40; RMSEA: 0.085; CFI: 0.913; TLI: 0.906) and a reduced 4-factor (χ2/df: 2.90; RMSEA: 0.072; CFI: 0.931; TLI: 0.926) structure showed acceptable fit. The fit of the 1-factor model was unacceptable. The internal consistency was evaluated through McDonald's ω, and it was acceptable for 4 of 6 factors of the original structure and for 3 of 4 factors of the reduced structure. In conclusion, these findings suggest that the 6-factor and 4-factor models are valid for measuring positive mental health. However, issues with internal consistency must be investigated.

ABSTRACT

Attention has been defined as a multidimensional construct, dealing with regulation of which stimuli the perceptual resources are directed to, filtering irrelevant information out to favor the retention of relevant information for a period of time, before acting on it. The computerized evaluation of attention has been gaining relevance both in research and in professional practice. The objectives are: 1) to analyze internal consistency and concurrent validity of the Child Attention Test 6.0 (tai), computerized version 6.0; and, 2) to obtain the preliminary reference values for each of the tai subtests done. The data for the validity and reliability studies of the tai corresponds to a sample of 122 schoolchildren in Mendoza (Argentina). When analyzing the internal consistency, satisfactory values were obtained for each subtest (Test 1, kr 20 = 0.78; Test 2, kr 20 = 0.74; and Test 3, kr 20 = 0.73). Satisfactory values were obtained when exploring the stability of the instrument. For concurrent validity, the results indicate significant moderate positive correlations (p <0.01) between the scores obtained in the Caras-R test and subtests 1 and 2 of the tai, used to evaluate attentional performance. Significant positive associa-tions (p <0.01) were observed between tai subtest 3 and the Corsi Block Test, used to assess visuospatial working memory. These results indicate that the tai, an instrument developed in order to evaluate attention and visuospatial working memory of school children, presents adequate indicators of validity and reliability. The findings, limitations, and future lines of research are discussed

ABSTRACT

The American Orthopedic Foot and Ankle Society (AOFAS) scale for hallux metatarsophalangeal (MTP) and interphalangeal (IP) joints is used throughout the world for the clinical assessment of patients with hallux valgus, and it has 3 subscales: pain, function, and alignment. The objective of this prospective, observational study was to perform translation, cultural adaptation, and validation of this scale for Colombian Spanish. The Foot Function Index and Short Form-36 (SF-36) questionnaires were also used in the validation process. Patients who were 18 years or older, resided in the city of Bogota, were able to read and write in Spanish, and were within 4 years of corrective surgery for hallux valgus were included. A total of 79 patients were included in the study, 25 of whom were involved in the translation process and all of whom were involved in the validation process. Construct validity was demonstrated by significant positive Spearman's correlations between the AOFAS pain subscale and the AOFAS function subscale (rs = 0.483), total AOFAS scale (rs = 0.795), and SF-36 physical summary subscale (rs = 0.364); and between the AOFAS function subscale and the total AOFAS scale (rs = 0.894) and SF-36 physical summary subscale (rs = 0.310). The test-retest process for reliability yielded intraclass correlation coefficients between the AOFAS pain, function, and alignment subscales that were all 0.70 or higher. The AOFAS scale for hallux MTP and IP joints was successfully translated and adapted into Colombian Spanish, and this version demonstrated construct validity and reliability, rendering it suitable for clinical use in Colombian patients with hallux valgus.

ABSTRACT

Abstract: Diabetic gastroparesis (DGP) is an autonomic neuropathy resulting from long-standing poorly controlled diabetes, and it is also linked to fluctuations in glycemic control due to variability on nutrient absorption. Objectives: Considering the scarcity of information, the aim of this study was to identify the impact of modifications on diet consistency on post-prandial glucose variability using a continuous glucose monitoring (CGM) and its effect on the perception and severity of gastrointestinal symptoms. Methods: This proof-of-concept study was carried out in a cross-sectional cohort of six individuals with type 1 diabetes mellitus with confirmed diagnosis of DGP. Two types of diet were used to evaluate glycemic control and DGP symptoms, general consistency standard meal (SD) and modified consistency test diet (MD), associated with an application of rapid acting insulin at the time of food intake. Glycemic control was evaluated by CGM, and the Gastroparesis Cardinal Symptom Index (GCSI) was applied after meals. Results: The CGM curve was different for MD + insulin and SD + insulin. There was a smaller increment of interstitial glucose with 2 h after MD + insulin, returning almost to the basal level 4 h later. Patients scored significantly lower GCSI after MD + insulin compared to the same index after they received SD + insulin. Moreover, there was a decrease in important clinical scores present in the index, like: "Not able to finish meal", "Loss of appetite" and "Stomach or belly feels larger". Conclusion: This study showed that a modified diet can improve postprandial glycemic excursion and the perception and severity of gastroparesis symptoms. ABSTRACT

Introduction: A cross-sectional descriptive study was conducted to evaluate the factor structure of Spanish translation and adaptation of the Cognitive Flexibility Inventory (CFI) in a sample of Colombian adults. Method: The sample of the study was n = 968. Respondents were aged between 18 and 52 years old (Mage=22.81, SD=4.42). Descriptive analyses, confirmatory factor analysis, and Cronbach's Alpha calculation were carried out. Results: Internal consistency for the global scale was high (α=.89). Likewise, the coefficients of the Alternative factor and the Control factor were similar (α=.90, 95% CI=.89-.90 and α=.83, 95% CI=.81-.85, respectively). A two-factor structure performed best according to the results of model selection criteria. This model suggested the existence of two correlated factors, with correlated items within factors (Item19 Item20 and Item8 Item10). Conclusions: Overall, the results suggest that the CFI scale exhibits construct validity and adequate reliability, both for the general scale and the subscales in the Colombian sample, enabling their use in contexts such as clinical or research.

ABSTRACT

Background: In 2013, we developed a scale to evaluate the abstracts of the congresses of the Society of Surgeons of Chile (SOCICH). Objective: To determine internal consistency and interobserver reliability of a scale to evaluate conference abstracts. Material and Methods: Reliability study. Twelve surgeons were trained virtually for 8 hours, to apply the scale. Once the training was finished, they were sent a questionnaire to evaluate the contents of the training, and several summaries-cases to be evaluated with the aforementioned scale. Descriptive statistics were applied, then the degree of agreement between observers was estimated for each item of the scale. Subsequently, intraclass correlation coefficient (ICC) was evaluated, using a mixed two-factor model where the effects of the evaluators are random and the items are fixed, using a definition of absolute agreement. In addition, the internal consistency of the items was evaluated using Cronbach's alpha, considering 95% confidence intervals (95% CI). Results: After analyzing the measurements of the 9 items by the 12 observers, it was verified that the ICC was 0.871; with a 95% CI of 0.700; 0.965. The internal consistency value was 0.7 considering the 9 items, it is not recommended to delete any item. Conclusions: The scale has good internal consistency and interobserver reliability. Therefore, it can be considered as reliable instrument to be used in the evaluation of abstracts for congresses.

ABSTRACT

Chitosan (Ch) is a partially crystalline biopolymer, insoluble in pure water but soluble in acid solutions. It has attracted interest from researchers to prepare solutions using different acid types and concentrations. This research aims to study both the effect of chitosan (Ch) or acetic acid (Ac) concentrations, at different temperatures, on rheological and viscoelastic properties of Ch solutions. To study the effect of Ch, solutions were prepared with 0.5−2.5 g Ch/100 g of solution and Ac = 1%, whereas to study the effect of Ac, the solutions were prepared with 2.0 g of Ch/100 g of solution and Ac = 0.2−1.0%. Overall, all analyzed solutions behaved as pseudoplastic fluid. The Ch strongly affected rheological properties, the consistency index (K) increased and the index flow behavior (n) decreased as a function of Ch. The activation energy, defined as the energy required for the molecule of a fluid to move freely, was low for Ch = 0.5%. The effect of Ac was less evident. Both K and n varied according to a positive and negative, respectively, parabolic model as a function of Ac. Moreover, all solutions, irrespective of Ch and Ac, behaved as diluted solutions, with G" > G'. The relaxation exponent (n") was always higher than 0.5, confirming that these systems behaved as a viscoelastic liquid. This n" increased with Ch, but it was insensitive to Ac, being slightly higher at 45 °C.

ABSTRACT

Objective: To determine the linguistic equivalence in the Spanish language and the internal consistency of the Rapid Estimate of Adult Literacy in Dentistry 99 (REALD-99) applied to a sample group of Mexican people. Method: The equivalence of the REALD-99 was performed by expert translators of the English language by means of translation-retranslation; its content was validated by fve bachelors of dentistry. The instrument was applied to a sample group of 995 people between 15 and 54 years old who requested dental care for the frst time. The people participated voluntarily, and their consent was given in writing. Epidemiological variables were analyzed, and the internal consistency of the instrument was determined with the Cronbach's Alpha test. Results: Two words with identical translation and three without application in the local dental context were identifed. The average age of the participants was 30 years old (SD ±11); 62% were women. The application of the instrument was 87 correct answers (SD ±9.2) on average and the application time was a median of 2.24 minutes. Cronbach's alpha was 0.91 with the 99 words. Conclusions: The instrument is interchangeable and equivalent with an internal consistency similar to that obtained in other countries. However, the results are an approximation to the reading ability and do not guarantee the knowledge, comprehension, or operability of related vocabulary and neither they explain the complex skills involved in health literacy.

ABSTRACT

Past research has demonstrated that older adults tend to use daily activities as cues to remember to take medications. However, they may still experience medication non-adherence because they did not select adequate contextual cues or face situations that interfere with their medication routines. This work addresses two research questions: (1) How does the association that older adults establish between their daily routines and their medication taking enable them to perform it consistently? (2) What problems do they face in associating daily routines with medication taking? For 30 days, using a mixed-methods approach, we collected quantitative and qualitative data from four participants aged 70-73 years old about their medication taking. We confirm that older adults who matched their medication regimens to their habitual routines obtained better results on time-based consistency measures. The main constraints for using daily routines as contextual cues were the insertion of medication taking into broad daily routines, the association of multiple daily routines with medication taking, the lack of strict daily routines, and the disruption of daily routines. We argue that the strategies proposed by the literature for forming medication-taking habits should support their formulation by measuring patients' dosage patterns and generating logs of their daily activities.
