ABSTRACT

BACKGROUND: Shoulder dislocation, particularly anterior dislocation, is a common orthopedic injury often presenting in emergency care settings, characterized by significant pain and muscle spasms. Prompt reduction is essential to alleviate symptoms and restore function. The Cunningham technique employs gentle pulling and massage motions targeted at the muscles and has emerged as a promising method for reducing anterior shoulder dislocations. However, its reported success rates vary widely across studies, and questions remain regarding its efficacy, particularly in cases of failure. This study aims to evaluate the effectiveness of the Cunningham technique for reducing anterior shoulder dislocations and its potential role in providing analgesia and muscle relaxation as an adjunctive method. METHODS: A retrospective study was conducted on patients presenting with acute anterior shoulder dislocation at a single center. Reduction using the Cunningham technique was performed initially, followed by the external rotation technique if unsuccessful. Procedural sedation and analgesia were administered if the reduction was still not achieved, and shoulder dislocation reduction was performed again through the external rotation method. The patients' VAS scores were recorded and evaluated the Cunningham technique's effectiveness in reduction and whether it increases the effectiveness of other techniques applied for reduction by lowering the VAS score, even in cases where it is not effective. RESULTS: A total of 61 patients were included in the study. The reduction was performed using the Cunningham technique in 34.4% (21/61) patients, the external rotation technique in 47.5% (29/61) patients, and the external rotation technique with PSA in 18% (11/61) patients. Significant differences were observed in the duration of hospital stay among the three techniques, with ER with PSA resulting in the longest stay. VAS scores showed significant improvements from initial presentation to post-reduction in all three groups. A significant decrease in pre-reduction VAS scores was observed during the transition from the Cunningham technique to other techniques. CONCLUSION: The Cunningham technique showed effectiveness in reducing anterior shoulder dislocations, providing analgesia, and muscle relaxation. It demonstrated favorable outcomes as an initial reduction technique, with the external rotation technique used as a subsequent option. Further studies comparing the success rates and complications of the Cunningham technique with other reduction methods are warranted to establish its role in clinical practice.

ABSTRACT

Numerous studies suggest the involvement of adenosine-5'-triphosphate (ATP) and similar nucleotides in the pathophysiology of asthma. Androgens, such as testosterone (TES), are proposed to alleviate asthma symptoms in young men. ATP and uridine-5'-triphosphate (UTP) relax the airway smooth muscle (ASM) via purinergic P2Y2 and P2Y4 receptors and K+ channel opening. We previously demonstrated that TES increased the expression of voltage-dependent K+ (KV) channels in ASM. This study investigates how TES may potentiate ASM relaxation induced by ATP and UTP. Tracheal tissues treated with or without TES (control group) from young male guinea pigs were used. In organ baths, tracheas exposed to TES (40 nM for 48 h) showed enhanced ATP- and UTP-evoked relaxation. Tetraethylammonium, a K+ channel blocker, annulled this effect. Patch-clamp experiments in tracheal myocytes showed that TES also increased ATP- and UTP-induced K+ currents, and this effect was abolished with flutamide (an androgen receptor antagonist). KV channels were involved in this phenomenon, which was demonstrated by inhibition with 4-aminopyridine. RB2 (an antagonist of almost all P2Y receptors except for P2Y2), as well as N-ethylmaleimide and SQ 22,536 (inhibitors of G proteins and adenylyl cyclase, respectively), attenuated the enhancement of the K+ currents induced by TES. Immunofluorescence and immunohistochemistry studies revealed that TES did not modify the expression of P2Y4 receptors or COX-1 and COX-2, while we have demonstrated that this androgen augmented the expression of KV1.2 and KV1.5 channels in ASM. Thus, TES leads to the upregulation of P2Y4 signaling and KV channels in guinea pig ASM, enhancing ATP and UTP relaxation responses, which likely limits the severity of bronchospasm in young males.

ABSTRACT

The Cupressaceae family includes species considered to be medicinal. Their essential oil is used for headaches, colds, cough, and bronchitis. Cedar trees like Chamaecyparis lawsoniana (C. lawsoniana) are commonly found in urban areas. We investigated whether C. lawsoniana exerts some of its effects by modifying airway smooth muscle (ASM) contractility. The leaves of C. lawsoniana (363 g) were pulverized mechanically, and extracts were obtained by successive maceration 1:10 (w:w) with methanol/CHCl3. Guinea pig tracheal rings were contracted with KCl, tetraethylammonium (TEA), histamine (HIS), or carbachol (Cch) in organ baths. In the Cch experiments, tissues were pre-incubated with D-600, an antagonist of L-type voltage-dependent Ca2+ channels (L-VDCC) before the addition of C. lawsoniana. Interestingly, at different concentrations, C. lawsoniana diminished the tracheal contractions induced by KCl, TEA, HIS, and Cch. In ASM cells, C. lawsoniana significantly diminished L-type Ca2+ currents. ASM cells stimulated with Cch produced a transient Ca2+ peak followed by a sustained plateau maintained by L-VDCC and store-operated Ca2+ channels (SOCC). C. lawsoniana almost abolished this last response. These results show that C. lawsoniana, and its active metabolite quercetin, relax the ASM by inhibiting the L-VDCC and SOCC; further studies must be performed to obtain the complete set of metabolites of the extract and study at length their pharmacological properties.

Objetivo: avaliar a efetividade da aplicação da técnica de relaxamento muscular progressivo de Jacobson na redução de sintomas de ansiedade leves e moderados em adultos. Método: estudo quase-experimental, com amostra de 108 adultos usuários da Estratégia Saúde da Família, realizado entre março e agosto de 2019. Para análise estatística descritiva e inferencial, foram utilizados: medidas de tendência central, Teste Qui-Quadrado de Pearson, Exato de Fisher, Mann-Whitney e Wilcoxon (nível de significância de 5%). Resultados: no primeiro momento, os grupos apresentaram homogeneidade em relação às características (p= 0,707) e aos escores dos níveis de ansiedade (p=0,463). No segundo momento, mostraram heterogeneidade e independência relacionados às características (p=0,001) e aos escores (p=0,001). Conclusão: a aplicação da técnica demonstrou melhoria dos sintomas ansiosos do grupo experimental (p=0,001), com maior efetividade nos participantes com sintomas leves (p 0,010), sexo feminino, adulto jovem, histórico de situação estressora recente e com problemas com o sono (p<0,001)(AU)


Objective: to evaluate the effectiveness of applying Jacobson's progressive muscle relaxation technique in reducing mild and moderate anxiety symptoms in adults. Method: quasi-experimental study, with a sample of 108 adult users of the Family Health Strategy, carried out from March to August 2019. For descriptive and inferential statistical analysis, the following were used: measures of central tendency, Pearson's Chi-Square Test, Exact Fisher, Mann-Whitney and Wilcoxon (5% significance level). Results: at first, the groups were homogeneous in terms of characteristics (p=0.707) and anxiety levels scores (p=0.463). In the second moment, they showed heterogeneity and independence related to characteristics (p=0.001) and scores (p=0.001). Conclusion: the application of the technique showed an improvement in anxiety symptoms in the experimental group (p=0.001), with greater effectiveness in participants with mild symptoms (p=0.010), female, young adult, history of recent stressful situation and with problems with sleep (p<0.001)(AU)

ABSTRACT

The purpose of this study was to investigate whether intravenous crotalphine produces significant sedation, as well as physiological changes, in healthy standing horses. Six mares, aged 8 years and weighing 415kg underwent three different treatments in a crossover design: TA (acepromazine: 50µg.kg-1), TC (crotalphine: 0.01µg.kg-1) and TX (xylazine: 1000µg.kg-1), intravenously. At various time points over 60 minutes, physiologic variables were recorded: heart rate, respiratory rate, and rectal temperature. The head height from the ground (HHG) was evaluated in centimeters. Data were analyzed using ANOVA followed by Dunnett's test or Friedman followed by Dunn's test, under 5% significance. Heart rate decreased significantly at M5 and M10 compared with Mb in TX (28±7, 26±6 and 40±8 beats/minute-1, respectively; p=0.0004). Respiratory rate and rectal temperature did not differ among groups or time points. The HHG significantly decreased in all groups compared with Mb at various time points (p<0.0001). In conclusion, crotalphine did not produce reliable and durable sedation in healthy standing mares and did not influence cardiorespiratory variables in a clinically relevant manner.


O objetivo deste estudo foi investigar se a administração de crotalfina intravenosa produz sedação significativa e alterações fisiológicas em equinos saudáveis. Seis éguas, idade média de oito anos e peso médio de 415kg, foram submetidas a três tratamentos distintos: TA (acepromazina: 50µg/kg), TC (crotalfina: 0,01µg/kg) e TX xilazina: 1000µg/kg), por via intravenosa. Em vários momentos, ao de longo de 60 minutos, as variáveis fisiológicas registradas foram frequência cardíaca, frequência respiratória e temperatura retal. A altura de cabeça ao solo (ACS) foi avaliada em centímetros. Os dados foram analisados pela ANOVA, seguida pelo teste de Dunnett ou de Friedman e, depois, pelo teste de Dunn, sob 5% de significância. A frequência cardíaca diminuiu significativamente em M5 e M10 em comparação com Mb em TX (28±7, 26±6 e 40±8 bpm, respectivamente; P=0,0004). A frequência respiratória e a temperatura retal não diferiram entre os grupos ou os pontos de tempo. O HHG diminuiu significativamente em todos os grupos em comparação com Mb em vários momentos (P <0,0001). Em conclusão, a crotalfina não produziu sedação confiável e durável em éguas saudáveis e não influenciou as variáveis cardiorrespiratórias de maneira clinicamente relevante.

ABSTRACT Introduction The current teaching of athletes focuses mainly on gaining their skills, often neglecting the beneficial impacts of relaxation on the overall physical quality of its students. Therefore, clarifying the relationship between muscle relaxation training and athletic performance is important. Objective Explore the effect of relaxation training on running athletes' technical and muscular levels. Methods A comparative experiment was conducted on 40 male and female runners, randomly divided into two groups. Before training, the athletes in the experimental group received relaxation training through the breath regulation method, muscle relaxation method, and independent training method, with 60 minutes in each session, for 6 months. Results Based on the assumption that there was no significant difference in the initial physical indices, the athletes in the experimental group performed better in 100m, 200m, and 300m than those in the control group, and the difference between the two groups was significant (P < 0.05). Conclusion Adequate relaxation training positively affects athletes' muscular endurance and technical level. Therefore, an adequate increase in relaxation training is shown to be conducive in improving the overall physical quality and competitive level of athletes. RESUMO Introdução O ensino atual dos atletas foca-se principalmente no ganho de suas habilidades, negligenciando muitas vezes os impactos benéficos do relaxamento na qualidade física geral de seus alunos. Portanto, é de grande importância esclarecer a relação entre o treinamento de relaxamento muscular e o desempenho atlético. Objetivo Explorar o efeito do treinamento de relaxamento sobre o nível técnico e muscular dos atletas de corrida. Métodos Uma experiência comparativa foi realizada em 40 corredores masculinos e femininos, divididos aleatoriamente em dois grupos. Antes do treinamento, os atletas do grupo experimental receberam treinamento de relaxamento através do método de regulação da respiração, método de relaxamento muscular e método de treinamento independente, com um total de 60 minutos de duração em cada sessão, por 6 meses. Resultados Partindo da premissa de que não houve diferença significativa nos índices físicos iniciais, os atletas do grupo experimental tiveram melhor desempenho em 100m, 200m e 300m do que os do grupo controle, e a diferença entre os dois grupos foi significativa (P < 0,05). Conclusão O treinamento de relaxamento adequado tem um efeito positivo sobre a resistência muscular e o nível técnico dos atletas. Portanto, o aumento adequado do treinamento de relaxamento mostra-se propício na melhoria da qualidade física geral e no nível competitivo dos atletas. Por lo tanto, es de gran importancia aclarar la relación entre el entrenamiento de relajación muscular y el rendimiento deportivo. Objetivo Explorar el efecto del entrenamiento de relajación en el nivel técnico y muscular de los atletas que corren. Métodos Se realizó un experimento comparativo con 40 corredores masculinos y femeninos, divididos aleatoriamente en dos grupos. Antes del entrenamiento, los atletas del grupo experimental recibieron un entrenamiento de relajación mediante el método de regulación de la respiración, el método de relajación muscular y el método de entrenamiento independiente, con una duración total de 60 minutos en cada sesión, durante 6 meses. Resultados Partiendo de la base de que no había diferencias significativas en los índices físicos iniciales, los atletas del grupo experimental obtuvieron mejores resultados en 100m, 200m y 300m que los del grupo de control, y la diferencia entre los dos grupos fue significativa (P < 0,05). ABSTRACT Introduction: In training, fatigue and recovery are antagonistic factors. Physical rehabilitation of athletes and elimination of sports fatigue have gradually become essential factors in improving competitiveness. Muscle relaxation exercises can reduce badminton players' physical effort, allowing them to recover quickly. Objective: Analyze muscle relaxation's effect on fatigue relief until the normalization of physical strength in badminton players. Methods: Twelve badminton players were grouped in cross-sectional tests. During the three weeks of physical training, players performed two training methods: muscle relaxation and complete rest. Laboratory analysis encompassed blood biomarkers such as urea and creatine kinase. Data were collected before and after the interventions, confronted, and studied mathematically and statistically. Results: The proportions of leukocytes and lymphocytes in the muscle relaxation group after resistance exercise were lower than before. The decrease of morning leukocytes in the muscle relaxation group on the second day was less than in the resting group. The recovery effect of the lymphocyte ratio was also better. After special strength training, the value of creatine kinase was significantly higher than before exercise in both the muscle relaxation group and the complete rest group. Conclusion: Muscle relaxation can reduce exercise fatigue in badminton players. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: No treinamento, fadiga e recuperação são fatores antagônicos. A reabilitação física dos atletas e a eliminação da fadiga esportiva tornaram-se gradualmente fatores essenciais para melhorar a competitividade. O exercício de relaxamento muscular pode reduzir o esforço físico dos jogadores de badminton, permitindo ao jogador recuperar-se rapidamente. Objetivo: Analisar o efeito do relaxamento muscular sobre o alívio da fadiga até a normalização da força física nos jogadores de badminton. Métodos: Foram agrupados 12 jogadores de badminton em testes transversais. Durante as três semanas de treinamento físico, os jogadores realizaram dois métodos de treinamento: relaxamento muscular e repouso completo. A análise laboratorial englobou biomarcadores sanguíneos, como ureia e creatina cinase. Os dados foram coletados antes e após as intervenções, confrontados e estudados de forma matemática e estatística. Resultados: As proporções de leucócitos e linfócitos no grupo de relaxamento muscular após o exercício de resistência foram menores do que antes. A diminuição dos leucócitos matinais no grupo de relaxamento muscular no segundo dia foi menor do que no grupo de repouso, e o efeito de recuperação da relação de linfócitos também foi melhor. Após o treinamento especial de força, o valor da creatina quinase foi significativamente maior do que antes do exercício tanto no grupo de relaxamento muscular quanto no grupo de repouso completo. Conclusão: O relaxamento muscular pode reduzir a fadiga do exercício nos jogadores de badminton. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción: En el entrenamiento, la fatiga y la recuperación son factores antagónicos. La rehabilitación física de los atletas y la eliminación de la fatiga deportiva se han convertido gradualmente en factores esenciales para mejorar la competitividad. El ejercicio de relajación muscular puede reducir el esfuerzo físico de los jugadores de bádminton, permitiendo que el jugador se recupere rápidamente. Objetivo: Analizar el efecto de la relajación muscular en el alivio de la fatiga hasta la normalización de la fuerza física en jugadores de bádminton. Métodos: Se agruparon doce jugadores de bádminton en pruebas transversales. Durante las tres semanas de entrenamiento físico, los jugadores realizaron dos métodos de entrenamiento: la relajación muscular y el descanso completo. Los análisis de laboratorio incluían biomarcadores sanguíneos como la urea y la creatina quinasa. Los datos se recogieron antes y después de las intervenciones, se confrontaron y se estudiaron matemática y estadísticamente. Resultados: Las proporciones de leucocitos y linfocitos en el grupo de relajación muscular tras el ejercicio de resistencia fueron menores que antes. La disminución de los leucocitos matinales en el grupo de relajación muscular en el segundo día fue menor que en el grupo de reposo, y el efecto de recuperación de la proporción de linfocitos también fue mejor. Después del entrenamiento especial de fuerza, el valor de la creatina quinasa fue significativamente mayor que antes del ejercicio tanto en el grupo de relajación muscular como en el de reposo completo. Conclusión: La relajación muscular puede reducir la fatiga del ejercicio en los jugadores de bádminton. ABSTRACT Introduction: Dance sport is an emerging sport activity. The number of college students devoted to the study of dance is increasing. Basic dance sport skills are inherent to performance. Basic skills training requires dancers to have a certain degree of flexibility, endurance, speed, etc. Currently, we know that CORE strength training can improve the physical function of athletes, but studies are scarce as to its true impact on proprioceptive skills in dance sports. Objective: The purpose of this study was to examine the effect of strengthening the CORE on muscle coordination in sport dancers. This paper examines the relationship between core strength training and dance skills in athletes. Methods: Undergraduate sport dance students were randomly and voluntarily selected. These students undergo a 4-month training protocol in CORE strengthening. The effect of this experiment was evaluated with biomarker and kinetic data collected before and after the experiment. These data were worked out statistically and the results were discussed according to the literature. Results: After the CORE training protocol, the sport dance students had significant differences in physical tests(P<0.05). Conclusion: CORE training can promote physical stability, explosive power and muscle coordination in sport dancers. Level of evidence II; Training research - outcome investigation.


RESUMO Introdução: O esporte de dança é uma atividade esportiva emergente. O número de estudantes universitários dedicados ao estudo da dança está aumentando. As habilidades básicas da dança esportiva são inerentes à performance. O treinamento de habilidades básicas exige que os dançarinos tenham um certo grau de flexibilidade, resistência, velocidade, etc. Atualmente, sabemos que o treinamento de força do core pode melhorar a função física dos atletas, porém os estudos são escassos quando ao seu verdadeiro impacto sobre as habilidades proprioceptivas na dança esportiva. Objetivo: Este estudo teve como objetivo analisar o efeito do fortalecimento do core na coordenação muscular em bailarinos esportivos. Este artigo examina a relação entre treinamento de força central e habilidades de dança em atletas. Métodos: Foram selecionados, de maneira aleatória e voluntariamente, universitários graduandos em dança esportiva. Esses alunos passam por 4 meses sob protocolo de treino em fortalecimento do core. O efeito desse experimento foi avaliado com os dados de biomarcadores e cinéticos coletados antes e após o experimento. Esses dados foram trabalhados estatisticamente e os resultados foram discutidos segundo a literatura. Resultados: Após o protocolo de treinamento do core, os alunos de dança esportiva tiveram diferenças significativas nos testes físicos (P<0,05). Conclusão: O treinamento do core pode promover estabilidade física, poder explosivo e coordenação muscular em dançarinos esportivos. RESUMEN Introducción: El deporte de la danza es una actividad deportiva emergente. El número de universitarios dedicados al estudio de la danza va en aumento. Las habilidades deportivas básicas de la danza son inherentes al rendimiento. El entrenamiento de habilidades básicas requiere que los bailarines tengan un cierto grado de flexibilidad, resistencia, velocidad, etc. Hoy en día, sabemos que el entrenamiento de fuerza core puede mejorar la función física de los atletas, pero los estudios son escasos sobre su impacto real en las habilidades propioceptivas en los deportes de danza. Objetivo: Este estudio tiene como objetivo analizar el efecto del fortalecimiento del core en la coordinación muscular de los bailarines deportivos. Este artículo examina la relación entre el entrenamiento de la fuerza central y las habilidades de danza en los atletas. Métodos: Se seleccionaron de forma aleatoria y voluntaria estudiantes universitarios que se graduaban en danza deportiva. Estos estudiantes pasaron 4 meses bajo el protocolo de formación en el fortalecimiento del core. El efecto de este experimento se evaluó con datos de biomarcadores y cinéticos recogidos antes y después del experimento. Estos datos se trabajaron estadísticamente y los resultados se discutieron de acuerdo con la literatura. Resultados: Tras el protocolo de entrenamiento del core, los alumnos de danza deportiva presentaron diferencias significativas en las pruebas físicas (P<0,05). Conclusión: El entrenamiento del core puede promover la estabilidad física, la potencia explosiva y la coordinación muscular en los bailarines deportivos. ABSTRACT

The purpose of this study was to investigate whether intravenous crotalphine produces significant sedation, as well as physiological changes, in healthy standing horses. Six mares, aged 8 years and weighing 415kg underwent three different treatments in a crossover design: TA (acepromazine: 50μg.kg-1), TC (crotalphine: 0.01μg.kg-1) and TX (xylazine: 1000μg.kg-1), intravenously. At various time points over 60 minutes, physiologic variables were recorded: heart rate, respiratory rate, and rectal temperature. The head height from the ground (HHG) was evaluated in centimeters. Data were analyzed using ANOVA followed by Dunnett’s test or Friedman followed by Dunn’s test, under 5% significance. Heart rate decreased significantly at M5 and M10 compared with Mb in TX (28±7, 26±6 and 40±8 beats/minute-1, respectively; p=0.0004). Respiratory rate and rectal temperature did not differ among groups or time points. The HHG significantly decreased in all groups compared with Mb at various time points (p<0.0001). In conclusion, crotalphine did not produce reliable and durable sedation in healthy standing mares and did not influence cardiorespiratory variables in a clinically relevant manner.


O objetivo deste estudo foi investigar se a administração de crotalfina intravenosa produz sedação significativa e alterações fisiológicas em equinos saudáveis. Seis éguas, idade média de oito anos e peso médio de 415kg, foram submetidas a três tratamentos distintos: TA (acepromazina: 50μg/kg), TC (crotalfina: 0,01μg/kg) e TX xilazina: 1000μg/kg), por via intravenosa. Em vários momentos, ao de longo de 60 minutos, as variáveis fisiológicas registradas foram frequência cardíaca, frequência respiratória e temperatura retal. A altura de cabeça ao solo (ACS) foi avaliada em centímetros. Os dados foram analisados pela ANOVA, seguida pelo teste de Dunnett ou de Friedman e, depois, pelo teste de Dunn, sob 5% de significância. A frequência cardíaca diminuiu significativamente em M5 e M10 em comparação com Mb em TX (28±7, 26±6 e 40±8 bpm, respectivamente; P=0,0004). A frequência respiratória e a temperatura retal não diferiram entre os grupos ou os pontos de tempo. O HHG diminuiu significativamente em todos os grupos em comparação com Mb em vários momentos (P <0,0001). Em conclusão, a crotalfina não produziu sedação confiável e durável em éguas saudáveis e não influenciou as variáveis cardiorrespiratórias de maneira clinicamente relevante.

The purpose of this study was to evaluate the effects of a progressive muscle relaxation program on psychological (stress levels, anxiety, and mood states) and physiological (blood pressure and heart rate) variables in basketball athletes. Fifty-nine basketball players, aged 14 to 19 years, and members of Bauru Basket team, were recruited for this study and grouped into control group (CG, n = 30) and intervention group (IG, n = 29). The participants were evaluated, before and after the intervention, by the following means: Competitive State Anxiety Inventory-2 (CSAI-2), to measure the pre-competitive anxiety state, i.e., how anxious each athlete felt just before the competition; Brunel Mood Scale (BRUMS), an instrument for early detection of overtraining syndrome; Recovery-Stress Questionnaire for Athletes (RESTQ-Sport), to identify the extent to which each athlete was physically or mentally stressed and the athlete's current capability for recovery; Athlete Burnout Questionnaire (ABQ), specifically developed for athletes. The IG athletes participated in 12 progressive muscle relaxation sessions, a practice for tensing or tightening a specific muscle until an ideal state of relaxation is reached. Each participant had blood pressure and heart rate measured before and after every session. The CG athletes were evaluated similarly to those in the IG but without relaxation. The results showed statistically significant differences in cognitive anxiety (p = 0.039) and specific stress (p = 0.016) between CG and IG before the intervention; in addition, a significant heart rate decrease (p < 0.01) between IG and CG was noted.

SUMMARY: The human hand can make precise movements utilizing several joints of various articular types. To understand hand movements more accurately, it is essential to view the actual movements of bones and muscles considering the X, Y, and Z axes in the joints. This study aimed to investigate the joint movements in a hand using movable surface models, including these axes. These movable surface models of the hand will improve medical students' understanding of hand movements around the joints. To achieve this aim, 70 surface models were adopted from a Visible Korean model. Using Maya software, 20 virtual joints with X, Y, and Z axes included nine distal and proximal interphalangeal joints, five metacarpophalangeal joints, five carpometacarpal joints, and one wrist joint were created. Bone surface models were elaborately polished to maintain their original shape during movement. Muscle surface models were also processed to display the deformation of the muscle shape during movement. The surface models of the hand joints were moved by virtual control of the joints. We saved 87 movable surface models of the hand, including bones, muscles, and joint axes in stereolithography format, and compiled a Portable Document Format (PDF) file. Using the PDF file, the joint movements in a hand could be observed considering the X, Y, and Z axes alongside the stereoscopic shapes of the bones and muscles. These movable surface models of the hand will improve medical students' understanding of hand movements around the joints.


RESUMEN: La mano humana puede realizar movimientos precisos utilizando varias articulaciones de diferentes tipos articulares. Para comprender los movimientos de las manos con mayor precisión, es esencial ver los movimientos reales de los huesos y los músculos considerando los ejes X, Y y Z de las articulaciones. Este estudio tuvo como objetivo investigar los movimientos articulares en una mano utilizando modelos de superficies móviles, incluidos estos ejes. Estos modelos de superficie móvil de la mano mejorarán la comprensión de los estudiantes de medicina de los movimientos de la mano alrededor de las articulaciones. Para lograr este objetivo, se adoptaron 70 modelos de superficie de un modelo coreano visible. Con el software Maya, se crearon 20 articulaciones virtuales con ejes X, Y y Z que incluyeron nueve articulaciones interfalángicas distales y proximales, cinco articulaciones metacarpofalángicas, cinco articulaciones carpometacarpianas y una articulación de muñeca. Resumen: Introducción: La electroporación irreversible (EIR) es una técnica conocida desde 1972; fue incorporada al ámbito oncológico en el año 2005 como técnica de ablación tumoral basada en pulsos eléctricos cortos de alto voltaje y es utilizada para introducir elementos químicos por difusión, alterar la estructura genética celular, fusionar una célula con otra (reversible) o provocarle la muerte (irreversible). Estas descargas eléctricas conllevan un riesgo asociado de arritmias cardíacas, contracciones musculares severas y convulsiones. Mantener un adecuado plano anestésico, analgesia óptima, relajación neuromuscular profunda y electrocardiograma sincronizado a los pulsos eléctricos son los objetivos principales del manejo anestésico. Objetivo: Describir las implicaciones anestésicas de la EIR en el paciente pediátrico. Material y métodos: Revisión narrativa basada en una búsqueda de artículos relacionados con la EIR en las principales bases de datos, donde la principal fuente de información son los reportes de casos y las revisiones no sistemáticas de la literatura, tanto del área de anestesiología y dolor como de ingeniería, radiología intervencionista vascular y cardiovascular, y cirugía. Resultados: Se encontraron y revisaron un total de 17 referencias bibliográficas. Conclusión: La anestesia para EIR en el paciente pediátrico debe garantizar un adecuado plano anestésico idealmente con intubación endotraqueal, analgesia óptima, relajación profunda y control de arritmias cardíacas con sincronización del electrocardiograma.


Abstract: Introduction: Irreversible electroporation (IRE) is a technique known since 1972 incorporated into the oncological field in 2005 as a tumor ablation technique based on short high-voltage electrical pulses used to introduce chemical elements by diffusion, alter the cellular genetic structure, fuse a cell with other (reversible) or cause death (irreversible). These electric shocks carry an associated risk of cardiac arrhythmias, severe muscle contractions and seizures. Maintain an adequate anesthetic plane, optimal analgesia, deep neuromuscular relaxation and an electrocardiogram synchronized to the electrical pulses are the main objectives of anesthetic management. Objective: Describe the anesthetic implications of IRE in the pediatric patient. Material and methods: Narrative review based on a search for articles related to IRE in the most important databases, where the main source of information is case reports and non-systematic reviews of the literature, both in the area of anesthesiology and pain as well as engineering, vascular and cardiovascular interventional radiology, and surgery. Results: A total of 17 bibliographic references were found and reviewed. Conclusion: The anesthesia for IRE in the pediatric patient should guarantee an adequate anesthetic plane ideally with endotracheal intubation, optimal analgesia, deep relaxation and control of cardiac arrhythmias with electrocardiogram synchronization.

La rehabilitación cardiometabólica es un programa multifactorial útil para el tratamiento de las principales patologías que tienen una relación directa con el sedentarismo, como patologías cardiovasculares, metabólicas y sus complicaciones. En mayo de 2004, durante la 57ª Asamblea Mundial de la Salud, se aprobó "La estrategia mundial de la Organización Mundial de la Salud (OMS) que aborda sobre el régimen alimentario, la actividad física y la salud" con la finalidad de disminuir la inactividad física, los malos hábitos de alimentación que de forma directa llevan al exceso de peso, y que están asociadas al riesgo cardiovascular (RCV), a las enfermedades cardiovasculares, a la diabetes mellitus (DM) del tipo 2 y a otras diferentes enfermedades degenerativas crónicas. En base a la cual se orienta a implementar una correcta prescripción del ejercicio tanto preventiva como terapéutica. Este protocolo de atención clínico-terapeútico va dirigido a profesionales de la salud para el control de enfermedades cardiometabólicas. Los profesionales sanitarios podrán ofertar una terapéutica integral a sus pacientes para dosificar el ejercicio de forma personalizada, a partir del estado de salud, conocimiento de patologías preexistentes, edad y condición física inicial del paciente, así como proporcionar un seguimiento mediante programas individualizados. Cuyo principal fin es mejorar la condición cardiovascular, evitar nuevas complicaciones y reducir la mortalidad.


Cardiometabolic rehabilitation is a multifactorial program useful for the treatment of the main pathologies that have a direct relationship with sedentary lifestyle, such as cardiovascular, metabolic pathologies and their complications. In May 2004, during the 57th World Health Assembly, "The World Strategy of the World Health Organization (WHO) that addresses the diet, physical activity and health" was approved in order to reduce the physical inactivity, poor eating habits that directly lead to excess weight, and that are associated with cardiovascular risk (CVR), cardiovascular diseases, diabetes mellitus type 2 and other different chronic degenerative diseases . Based on which it is oriented to implement a correct prescription of both preventive and therapeutic exercise.This protocol of clinical-therapeutic care is aimed at health professionals for the control of cardiometabolic diseases. Health professionals may offer a comprehensive therapy to their patients to dose the exercise in a personalized way, based on the state of health, knowledge of preexisting pathologies, age and initial physical condition of the patient, as well as providing follow-up through individualized programs. Whose main purpose is to improve the cardiovascular condition, avoid new complications and reduce mortality.

Purpose:To evaluate the impact of the combination of BRL 37344 and tadalafil (TDF) on the reduction of overactive bladder (OB) symptoms.Methods:Thirty mice were randomized into 5 groups (G) of 6 animals each. L-NAME was used to induce DO. G1: Control; G2: L-NAME; G3: L-NAME + TDF; G4: L-NAME + BRL 37344; G5: L-NAME + TDF + BRL 37344. After 30 days of treatment, the animals were submitted to cystometry to evaluate non-voiding contractions (NVC), threshold pressure (TP), baseline pressure (BP), frequency of micturition (FM) and threshold volume (TV). Differences between the groups were analyzed with ANOVA followed by the Tukey test.Results:NVC increased in G2 (4.33±2.58) in relation to G1 (1.50±0.55). NVC decreased in G3 (2.00±1.10), G4 (1.50±1.52) and G5 (2.00±1.26) compared to G2 (p<0.05). FM decreased in G3 (0.97±0.71), G4 (0.92±0.38) and G5 (1.05±0.44) compared to G2 (p<0.05). However, the combination of TDF and BRL37344 was not more effective at increasing NVC and improving FM than either drug alone. The five groups did not differ significantly with regard to TV.Conclusion:The combination of BRL 37344 and TDF produced no measurable additive effect on reduction of OB symptoms.(AU)

We studied six groups with different anesthetic protocols: CX (10 mg/kg ketamine-S and 2 mg/kg xylazine); CXA (10 mg/kg ketamine-S, 2 mg/kg xylazine, and 0.04 mg/kg atropine); CM (10 mg/kg ketamine-S and 0.5 mg/kg midazolam); CMAc (10 mg/kg ketamine-S, 0.5 mg/kg of midazolam, and 1 mg/kg acepromazine); C(20)M (20 mg/kg of ketamine-S and 0.5 mg/kg midazolam); and TZ (7 mg/kg of tiletamine and zolazepam), with a 30-day interval between the completion of each procedure. We evaluated heart rate (HR), respiratory rate (RR), saturation of oxyhemoglobin (SpO2), body temperature T (°C), and muscle relaxation, using a scoring system. There was no statistical difference for SpO2. T (°C) was progressively reduced over time in all groups (p< 0.0001). The groups CM, C(20)M, CMAc, and TZ demonstrated the greatest effect on HR and T (°C). With respect to respiratory rate reduction, groups CM and TZ showed similar results. The best degree of muscle relaxation was observed in groups CM and C(20)M. Introduction: Few studies have been made on the incidence of residual paralysis from nondepolarizing relaxants in people over 65 years old; however, estimating the number of cases and treatment thereof are both important. Objective: To study the incidence of residual paralysis with non-depolarizing relaxants in patients over 65 years of age and discuss treatment. Methodology: Analytical observational study based on a cohort design. Results: The pre-extubation residual paralysis was estimated at 23.2% and at 9.2% at patient admission to the Recovery Suite. Pharmacological reversal showed 89.4% and 100% success rates with Neostigmine and Sugammadex respectively, with similar times at T4/T1 >0.9. Conclusions: The incidence of pre-extubation residual paralysis was lower than the figure published worldwide. Pharmacological reversal therapies were typically highly effective.


Introducción: La incidencia de Relajación Residual por relajantes no despolarizantes en mayores de 65 años ha sido poco estudiada, siendo relevante su calculo y su tratamiento. Objetivo: Estudiar la incidencia de Relajación residual en pacientes mayores de 65 años con relajantes neuromusculares no despolarizantes y describir su tratamiento. Metodología: Estudio Observacional Analítico con Diseño de Cohorte. Resultados: La Relajación Residual pre-extubación fue del 23.2% y al ingreso a la Sala de Recuperación del 9.2%. La reversión farmacológica con Neostigmina exhibió un éxito del 89.4% y con Sugammadex del 100%, con similares tiempos a una T4/T1 > 0.9. Conclusiones: Las incidencias de Relajación Residual pre-extubación y en la Sala de Recuperación fueron mas bajas que las publicadas a nivel mundial. Las terapias de reversión farmacológica se distinguieron por su alta eficacia.

ABSTRACT

FUNDAMENTO: A Contração Pós-Repouso (CPR) do músculo cardíaco fornece informações indiretas sobre a manipulação de cálcio intracelular. OBJETIVO: Nosso objetivo foi estudar o comportamento da CPR e seus mecanismos subjacentes em camundongos com infarto do miocárdio. MÉTODOS: Seis semanas após a oclusão coronariana, a contratilidade dos Músculos Papilares (MP) obtidos a partir de camundongos submetidos à cirurgia sham (C, n = 17), com infarto moderado (MMI, n = 10) e grande infarto (LMI, n = 14), foi avaliada após intervalos de repouso de 10 a 60 segundos antes e depois da incubação com cloreto de lítio (Li+) em substituição ao cloreto de sódio ou rianodina (Ry). A expressão proteica de SR Ca(2+)-ATPase (SERCA2), trocador Na+/Ca2+ (NCX), fosfolambam (PLB) e fosfo-Ser (16)-PLB foi analisada por Western blotting. RESULTADOS: Os camundongos MMI apresentaram potenciação de CPR reduzida em comparação aos camundongos C. Em oposição à potenciação normal para camundongos C, foram observadas degradações de força pós-repouso nos músculos de camundongos LMI. Além disso, a Ry bloqueou a degradação ou potenciação de PRC observada em camundongos LMI e C; o Li+ inibiu o NCX e converteu a degradação em potenciação de CPR em camundongos LMI. Embora os camundongos MMI e LMI tenham apresentado diminuição no SERCA2 (72 ± 7% e 47 ± 9% de camundongos controle, respectivamente) e expressão protéica de fosfo-Ser16-PLB (75 ± 5% e 46 ± 11%, respectivamente), a superexpressão do NCX (175 ± 20%) só foi observada nos músculos de camundongos LMI. CONCLUSÃO: Nossos resultados mostraram, pela primeira vez, que a remodelação miocárdica pós-IAM em camundongos pode mudar a potenciação regular para degradação pós-repouso, afetando as proteínas de manipulação de Ca(2+) em miócitos.


BACKGROUND: Post-rest contraction (PRC) of cardiac muscle provides indirect information about the intracellular calcium handling. OBJECTIVE: Our aim was to study the behavior of PRC, and its underlying mechanisms, in rats with myocardial infarction. METHODS: Six weeks after coronary occlusion, the contractility of papillary muscles (PM) obtained from sham-operated (C, n=17), moderate infarcted (MMI, n=10) and large infarcted (LMI, n=14) rats was evaluated, following rest intervals of 10 to 60 seconds before and after incubation with lithium chloride (Li+) substituting sodium chloride or ryanodine (Ry). Protein expression of SR Ca(2+)-ATPase (SERCA2), Na+/Ca2+ exchanger (NCX), phospholamban (PLB) and phospho-Ser(16)-PLB were analyzed by Western blotting. RESULTS: MMI exhibited reduced PRC potentiation when compared to C. Opposing the normal potentiation for C, post-rest decays of force were observed in LMI muscles. In addition, Ry blocked PRC decay or potentiation observed in LMI and C; Li+ inhibited NCX and converted PRC decay to potentiation in LMI. Although MMI and LMI presented decreased SERCA2 (72±7% and 47±9% of Control, respectively) and phospho-Ser16-PLB (75±5% and 46±11%, respectively) protein expression, overexpression of NCX (175±20%) was only observed in LMI muscles. CONCLUSION: Our results showed, for the first time ever, that myocardial remodeling after MI in rats may change the regular potentiation to post-rest decay by affecting myocyte Ca(2+) handling proteins.

OBJETIVO: Nuestro objetivo fue estudiar el comportamiento de la CPP y sus mecanismos subyacentes en Ratas con infarto de miocardio. MÉTODOS: Seis semanas después de la oclusión coronaria, la contractilidad de los Músculos Papilares (MP) obtenidos a partir de Ratas sometidos a falsa cirurgia (C, n = 17), con infarto moderado (MMI, n = 10) y gran infarto (LMI, n = 14), fue evaluada después de pausas de estímulos de 10 a 60 segundos antes y después de la incubación con cloruro de litio (Li+) en substitución del cloruro de sodio o rianodina (Ry). La expresión proteica de SR Ca(2+)-ATPasa (SERCA2), intercambiador Na+/Ca2+ (NCX), fosfolamban (PLB) y fosfo-Ser (16)-PLB fue analizada por Western blotting. RESULTADOS: Los Ratas MMI presentaron potenciación de CPP reducida en comparación a los Ratas C. En oposición a la potenciación normal para Ratas C, fueron observadas decaimientos de fuerza post-reposo en los músculos de Ratas LMI. Además de eso, la Ry bloqueó la decaimiento o potenciación de PRC observada en Ratas LMI y C; el Li+ inhibió el NCX y convirtió la decaimiento en potenciación de CPP en Ratas LMI. Aunque los Ratas MMI y LMI hayan presentado disminución en el SERCA2 (72 ± 7% y 47 ± 9% de Ratas control, respectivamente) y expresión proteica de fosfo-Ser16-PLB (75 ± 5% y 46 ± 11%, respectivamente), la superexpresión del NCX (175 ± 20%) sólo fue observada en los músculos de Ratas LMI. CONCLUSIÓN: Nuestros resultados mostraron, por primera vez, que el remodelado miocárdico post-IAM en Ratas puede cambiar la potenciación regular para decaimiento post-reposo, afectando las proteínas de manejo del Ca(2+) en miocitos.
