ABSTRACT

Vascular surgeons are increasingly encountering older patients with large aneurysms associated with severe comorbid conditions. This situation can increase operative morbidity and elevate the mortality rate of aortic surgery over 60%. With some frequency many patients will represent a prohibitive risk for conventional graft replacement. The endoluminal treatment of 110 patients has proved to be feasible and may represent an alternative solution. One hundred six of the patients had an abdominal aortic aneurysm and in four patients, the thoracic aorta was involved. All of them underwent endoluminal repair for the aortic pathology using the combination of stents and grafts in aorto-aortic or aorto-iliac position, with straight, tapered, or bifurcated stent-graft devices. The results are as follows: Initial success was 84% in aorto-aortic abdominal devices and 100% in aorto-aortic thoracic devices. In aorto-iliac devices, initial success was 75%. Late success rates were 62% of the initial group and 80% of the initially successful group.

ABSTRACT

Endovascular aortic graft implantation is a new procedure for the repair of arterial aneurysms. We report on the first such case successfully performed in the United States. A 76-year-old man with severe oxygen-dependent pulmonary insufficiency, coronary artery disease, and recurrent ventricular tachyrhythmia was also diagnosed as having a 7.5-cm infrarenal abdominal aortic aneurysm. Because of the high risks associated with conventional surgical repair, consent was obtained for compassionate use of an experimental device. Using local anesthesia, a 22-mm Dacron prosthesis was inserted under fluoroscopic guidance through an open, left transfemoral route. Completion arteriography demonstrated aneurysm exclusion. No blood transfusion was required and there were no perioperative complications. Further technical refinements and clinical trials will be required prior to the broad implementation of this technique.

ABSTRACT

Entre julio de 1990 y junio de 1994 fueron tratados 310 pacientes en nuestro Servicio por una arteropatía de miembros inferiores. En 52 casos se utilizaron una o varias prótesis de Palmaz para completar o realizar dicho tratamiento. La cantidad total de endoprótesis de Palmaz colocadas en dicho período fue de 57, repartidas de la siguiente manera: ilíacas primitivas, 35; ilíacas externas, 10; femorales superficiales, 12. Las indicaciones las hemos clasificado en tres grupos: a) como complemento de una angioplastia percutánea (n=40); b) por reaparición a mediano plazo de una lesión ya tratada (n=7); c)como tratamiento de primera intención (n=10). El resultado inmediato fue óptimo en 56 casos (98,2 por ciento); en el caso restante (1,7 por ciento) el control angiográfico mostró la persistencia de una imagen de disección. La tasa de permeabilidad a largo plazo (1 año) fue del 87,5 por ciento con una tasa de reestenosis del 12,5 por ciento. El seguimiento a tres años mostró una permeabilidad del 61,1 por ciento. Las endoprótesis de Palmaz se revelan como un tratamiento eficaz de lesiones focales ateromatosas. Los resultados a largo plazo dependen de la hiperplasia miointimal (a nivel local) y de la evolutividad de la enfermedad ateromatosa (AU)

ABSTRACT

Se realizó un estudio retrospectivo sobre lesiones vasculares, en el cual se revisaron 51 historias de pacientes con heridas vasculares importantes, de ambos sexos, mayores de 14 años, atendidos en el Hospital "Pérez de León", en el último trienio (1988-1990). El objetivo del trabajo fué demostrar la casuística del Hospital "Pérez de León" en dicha patología y verificar cuáles fueron los procedimientos quirúrgicos aplicados para cada caso. Tales procedimientos, tienen la finalidad de detener la hemorragia, reconstrucción del vaso lesionado y reposición de la circulación de la región afectada; según las técnicas quirúrgicas que en la actualidad se llevan a cabo para tal fin. Se demostró que tanto la rafia y ligadura de los vasos lesionados como la interposición de vena safena interna dieron buenos resultados, tomando en cuenta que se trataban de casos de emergencia atendidos en un centro asistencial público, como el que nos compete. No olvidemos que actualmente, continúa la investigación en busqueda de mejorar las técnicas y procedimientos quirurgicos, para la resolución de éste tipo de lesiones. De los avances actuales mencionados: videoangioscopia, interposición de vena umbilical humana y uso de Urokinasa intraluminal

ABSTRACT

Se presentan 2 casos de reemplazo de vena cava superior afectadas por tumores malignos. En ambos pacientes el abordaje mediastinal se realizó por esternotomía mediana longitudinal, empleándose la técnica de espiralización de la vena safena interna como elemento protésico. Se destacan las indicaciones de resección cava así como se discuten los diversos procedimientos técnicos, material de reemplazo y experiencias descriptas en la bibliografía nacional y extranjera. Por su fácil confección, resistencia a las infecciones, viabilidad natural, bajo costo, riesgos relativos y buenos resultados obtenidos, los autores consideran a este procedimiento como método de elección para reemplazos de vena cava superior. ABSTRACT

Meso-renal shunt (MRS) has been designed for patients with biliary cirrhosis and portal hypertension, in whom the derivation of the portal flow must be done prior to the biliary surgery. We performed the MRS in five patients. In three due to secondary biliary cirrhosis, one case due to criptogenic cirrhosis and the last presenting cavernomatous degeneration of the portal vein. After the MRS one patient died 33 days later due to acute liver failure and hepatorenal syndrome. Of the survivors three are enjoying normal lifestyle, one of them was underwent colecistectomy and coledocostomy without any surgical problems. One patient was lost to follow up out presenting at that time chronic porta-systemic encephalopathy. When necessary the MRS may offer a satisfactory choice, since can be performed far from the subhepatic area, thus allowing to perform further biliary surgery.

ABSTRACT

Three patients with interruption and seven with hypoplasia of the aortic arch were treated surgically. The subclavian artery and the aortic isthmus were employed for reconstructing the aortic arch in five, and a Dacron prosthesis was used to restore the aortic continuity in five. A ductus arteriosus coexisted in all patients and a ventricular or atrial septal defect in nine. Congestive heart failure and pulmonary hypertension were prominent clinical features, and the role of the ductus and other intracardiac anomalies on their pathogenesis in discussed. Six patients, one with an interrupted and five with a hypoplastic arch survived but three have evidence of either pulmonary vascular disease or significant pulmonary hypertension. Only one patient with hypoplasia of the arch is now considered cured after his ventricular septal defect was closed in a second operation. The analysis of this and other series in the literature indicate a high mortality rate of the conditions; however, early surgical treatment, which is the only effective means to control heart failure and avoid the effects of prolonged pulmonary hypertension, has brought upon a decline in mortality in recent years.
