ABSTRACT

RESUMO Objetivo: Investigar a manifestação de sintomas do transtorno do processamento auditivo central em crianças com hipotireoidismo congênito. Métodos: Estudo de caráter exploratório, descritivo e transversal com 112 pacientes com hipotireoidismo congênito com idade ≥5 anos. Realizou-se entrevista com os pais/cuidadores no momento da espera da consulta médica. Portadores de outras afecções médicas foram excluídos. Como instrumento de pesquisa utilizou-se o protocolo estruturado de anamnese para avaliação do processamento auditivo rotineiramente empregado por audiologistas. A análise estatística utilizou o teste Qui-quadrado. Resultados: A distribuição por sexo foi semelhante (meninas: 53,3%). Os casos não-disgenesia constituíram a forma fenotípica mais prevalente para o hipotireoidismo congênito (88,4%), e verificou-se que 65,3% das crianças apresentavam algum episódio de níveis séricos irregulares de hormônio tireoestimulante. Dentre as manifestações mais frequentes dos sintomas do transtorno do processamento auditivo central, as queixas relaciondas às funções cognitivas auditivas, como: figura-fundo (83,0%), atenção auditiva (75,9%) e memória auditiva (33,0%) foram as mais evidentes. Reclamações relacionadas ao rendimento escolar foram reportadas em 62,3%. Conclusões: Os dados obtidos evidenciaram altas frequências de sintomas de defasagem nas funções cognitivas relacionadas ao processamento auditivo central, em especial na atenção auditiva, figura-fundo e memória auditiva nos portadores do hipotireoidismo congênito.

Among the manifestations of the most frequent central auditory processing disorder symptoms, problems were reported with regard to cognitive functions, as they related to hearing, such as figure-background ability (83.0%), auditory attention (75.9%) and auditory memory (33.0%). Complaints related to school performance were reported in 62.3% of the cases. Conclusions: The data obtained show a high frequency of lag symptoms in cognitive functions related to central auditory processing, particularly with regard to auditory attention, figure-background ability and auditory memory in patients with congenital hypothyroidism.

ABSTRACT

Central auditory pathway maturation in children depends on auditory sensory stimulation. The objective of the present study was to monitor the cortical maturation of children with cochlear implants using electrophysiological and auditory skills measurements. The study was longitudinal and consisted of 30 subjects, 15 (8 girls and 7 boys) of whom had a cochlear implant, with a mean age at activation time of 36.4 months (minimum, 17 months; maximum, 66 months), and 15 of whom were normal-hearing children who were matched based on gender and chronological age. The auditory and speech skills of the children with cochlear implants were evaluated using GASP, IT-MAIS and MUSS measures. Both groups underwent electrophysiological evaluation using long-latency auditory evoked potentials. Each child was evaluated at three and nine months after cochlear implant activation, with the same time interval adopted for the hearing children. The results showed improvements in auditory and speech skills as measured by IT-MAIS and MUSS. Similarly, the long-latency auditory evoked potential evaluation revealed a decrease in P1 component latency; however, the latency remained significantly longer than that of the hearing children, even after nine months of cochlear implant use. It was observed that a shorter P1 latency corresponded to more evident development of auditory skills. Regarding auditory behavior, it was observed that children who could master the auditory skill of discrimination showed better results in other evaluations, both behavioral and electrophysiological, than those who had mastered only the speech-detection skill. Therefore, cochlear implant auditory stimulation facilitated auditory pathway maturation, which decreased the latency of the P1 component and advanced the development of auditory and speech skills. The analysis of the long-latency auditory evoked potentials revealed that the P1 component was an important biomarker of auditory development during the rehabilitation process.

ABSTRACT

AIM: To compare the audiometric thresholds and speech perception sentence test, between two groups with bilateral post-lingual, severe to profound sensorineural hearing loss. METHODS: Retrospective and analytical study, with 59 patients divided into 2 groups (under 60 years and above 60 years old) implanted between May/2002 and February/2007. RESULTS: The first group (control) included 30 patients with a mean age of 44 years. The audiometric threshold value in this group was 26 dB, and the average value of speech perception test was 94%. The second group included 29 patients with a mean age of 69 years. The average audiometric threshold was 29 dB, and the average value of SPT was 90%. The Mann-Whitney U-test was considered significant (P<0.05) only for 6-8 KHz frequencies and for SPT. CONCLUSION: Both groups had excellent outcomes in audiometric and speech testing with the use of CI, but with a significantly better performance in the adult group.

CONCLUSION: The correlations between behavioral and auditory steady-state response (ASSR) thresholds were significant at 500, 1000, 2000, and 4000 Hz. ASSR presented high sensitivity and specificity in the detection of residual hearing in cochlear implant candidates when compared with warble-tone audiometry. OBJECTIVES: To assess residual hearing in cochlear implant candidates by comparing the electrophysiological thresholds obtained in dichotic single-frequency ASSR with behavioral thresholds at 500, 1000, 2000, and 4000 Hz. METHODS: This was a comparative study between ASSR and warble-tone audiometry thresholds in 40 cochlear implant candidates (80 ears) before cochlear implantation with bilateral severe-to-profound sensorineural hearing loss. RESULTS: Thresholds were obtained in 62.5% of all frequencies evaluated in warble-tone audiometry and in 63.1% in the ASSR. ASSR sensitivity was 96% and specificity was 91.6%. Mean differences between behavioral and ASSR thresholds did not reach significance at any frequencies. Strong correlations between behavioral and ASSR thresholds were observed in 500, 1000, and 2000 Hz and moderate in 4000 Hz, with correlation coefficients varying from 0.65 to 0.81. On 90% of occasions, ASSR thresholds were acquired within 10 dB of behavioral thresholds.

ABSTRACT

Hearing aids with an integrated sound generator have been used to enhance the treatment of tinnitus. The main aim of this study was to verify whether the combined use of amplification and sound generator is more effective than conventional amplification alone in reducing tinnitus annoyance by means of the use of a new hearing aid with an integrated sound generator. A total of 49 patients underwent a blind randomized clinical trial. Tinnitus annoyance was measured by Tinnitus Handicap Inventory and numerical scales, and psychoacoustic measures of tinnitus were also performed. The sound generator was set at the lowest intensity capable of providing relief from tinnitus. Results showed that 62.5% of the patients presented a reduction in tinnitus annoyance in the combined fitting group and in the group with amplification alone, 78% showed a reduction. This difference between the groups was not statistically significant.

UNLABELLED: According to the IBGE, Brazil had 21 million elderly persons in 2008. OBJECTIVE: To study the effects of speech therapy - fitting of hearing aids - on the quality of life of elderly persons and restriction of participation according to sex and age. MATERIAL AND METHOD: 50 elderly subjects, 23 females and 27 males, were allocated to Group 1 and Group 2, and were assessed with questionnaires (HHIE and SF 36) before and one year after fitting hearing aids; subjects were monitored every two months. RESULTS: The HHIE Social and Emotional Scale was significant with regards to sex and age after fitting hearing aids. The SF 36 results after fitting hearing aids were significant in six of eight test aspects. Two test aspects were not significant after fitting hearing aids; pain, however, was significant in the elderly group 2. CONCLUSION: There is little awareness of participation restrictions after the HHIE intervention. There is improvement in quality of life after rehabilitation. Retrospective study.

Segundo o IBGE, o Brasil tinha 21 milhões de idosos em 2008. OBJETIVO: Estudar os efeitos da intervenção fonoaudiológica por meio de adaptação de próteses auditivas na qualidade de vida do idoso e restrição de participação, segundo sexo e faixa etária. MATERIAL E MÉTODO: Participaram 50 idosos, 23 do sexo feminino e 27 do sexo masculino, distribuídos em Grupo 1 e Grupo 2. Foram avaliados antes da adaptação das próteses auditivas e um ano após, por meio de questionários HHIE e SF 36 e realizados acompanhamentos bimensais. RESULTADOS: O HHIE revelou que a Escala Emocional e Social no período pós foram significantes, no sexo e faixa etária. No SF 36, os resul¬tados de seis dos oito aspectos revelaram resultados significantes no período pós. Dois aspectos não apresentaram resultados significantes no período pós. No entanto, no aspecto dor, os idosos do Grupo 2 apresentaram resultados significantes no período pós. CONCLUSÃO: Há menor autopercepção das restrições de participação após a intervenção do HHIE. Há melhora na qualidade de vida após intervenção fonoaudiológica. Estudo Retrospectivo.

ABSTRACT

Our objective was to compare quality of life, high-frequency audiometry, and loudness-pitch match in individuals with tinnitus with and without hearing loss. We divided 52 individuals into two groups: one with hearing impairment (HI; n = 24) and the other with no hearing loss (NL; n = 28). All participants underwent loudness-pitch match and high-frequency audiometry and responded to the Tinnitus Handicap Inventory (THI) and World Health Organization Quality of Life (WHOQOL) questionnaires. On high-frequency audiometry, older individuals required 77.15% more intensity than did young individuals. On the THI questionnaire, individuals in the NL group exhibited mild handicap, whereas those in the HI group exhibited moderate handicap. On the WHOQOL questionnaires, the HI group demonstrated greater impairment on the physical and psychological domains than did the NL group. Individuals with HI experienced greater discomfort with tinnitus. On high-frequency audiometry, individuals with tinnitus and HI exhibited higher thresholds, with complete hearing loss at some frequencies. The WHOQOL and THI questionnaires revealed that individuals with HI experienced a poorer quality of life than did those with normal hearing.
