ABSTRACT

Introdução: A disfunção temporomandibular é um problema de saúde pública bastante subnotificado. Além disso, o seu diagnóstico é complexo e exige o uso de instrumentos válidos e confiáveis para uso em estudos epidemiológicos. Objetivo: Validar o instrumento de diagnóstico epidemiológico da DTM (IDE/DTM), com aplicabilidade para estudos epidemiológicos nacionais. Metodologia: Trata-se de um estudo de validação, foram avaliadas a validade da estrutura interna através da Análise Fatorial Exploratória (AFE), confiabilidade (consistência interna, interobservadores e intraobservador) e a validade convergente do IDE/DTM usando os Critérios de Diagnóstico para Disfunção Temporomandibular (DC/TMD) como padrão-ouro. A amostra foi composta por 221 voluntários, 120 na etapa de AFE, 50 para o teste-reteste e 101 na validade convergente. Resultados: A AFE revelou 3 dimensões (DTM Muscular, DTM Articular e Diagnóstico diferencial) com todos os itens com cargas fatoriais > 0.4, que explicaram 73,3% da variância total. Boa consistência interna com alfa de Cronbach= 0,775. A confiabilidade variou entre substancial e excelente, reprodutibilidade interobservadores (Kappa=0,74-1,00, ICC=0,91-0,97; p< 0,001) e intraobservador (Kappa= 0,75-0,94, ICC=0,80-0,98; p< 0,001). O escore total do IDE/DTM apresentou critérios de diagnósticos válidos e satisfatórios segundo o DC/TMD (Kappa= 0,906; p<0,001), com capacidade para diferenciar indivíduos sem e com DTM com ponto de corte de 4,9 (Sensibilidade=1,0; Especificidade= 1,0; AUR=1,0), DTM Mista com ponto de corte de 14 ou mais (Sensibilidade=0,8; Especificidade=1,0; AUR=0,987), e DTM Muscular (Sensibilidade=1,0; Especificidade= 0,88; VPP= 0,89; VPN= 1,0) ou DTM Articular (Sensibilidade=0,95; Especificidade= 0,87; VPP= 0,83; VPN=0,96) com ponto de corte entre 5-13,9 pontos, sendo o diagnóstico obtido a partir da maior pontuação em cada dimensão (Muscular ou Articular). Conclusão: O IDE/DTM é um instrumento de avaliação válido e confiável, com propriedades psicométricas adequadas e satisfatórias, capaz de diagnosticar pessoas com DTM e classificar o subtipo da condição (Muscular, Articular e Mista) (AU).


Introduction: Temporomandibular disorders (TMD) is a highly underreported health problem. In addition, its diagnosis is complex and requires the use of valid and reliable instruments for use in epidemiological studies. Objective: To validate the Epidemiological Diagnostic Instrument for TMD (EDI/TMD), with applicability for national epidemiological studies. Methodology: This is a validation study, internal structure validity through Exploratory Factor Analysis (EFA), reliability (internal consistency, interobserver and intraobserver), and convergent validity of the EDI/TMD were assessed using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) as the gold standards. The sample was composed of 221 volunteers, 120 in the EFA stage, 50 for the test-retest, and 101 for convergent validity. Results: AFE showed three factors (Muscles TMD, Joint TMD, and Differential Diagnosis) with all items having factor loadings > 0.4, which explained 73.3% of the total variance. Good internal consistency with Cronbach's alpha= 0.775. Reliability ranged from substantial to excellent, reproducibility interobserver (Kappa=0.74-1.00, ICC=0.91-0.97; p< 0.001) and intraobserver (Kappa= 0.75-1.00, ICC=0.80-0.98; p< 0.001). The EDI/TMD total score showed valid and satisfactory diagnostic criteria according to the DC/TMD (Kappa= 0.906; p<0.001), with the ability to differentiate between individuals without and with TMD with a cut-off point of 4.9 (Sensitivity=1.0; Specificity=1.0; AUR=1.0); Mixed TMD with a cut-off point of 14 or more (Sensitivity=0.8; Specificity=1.0; AUR=0.987); and either Muscles TMD (Sensitivity=1.0; Specificity= 0.88; PPV= 0.89; NPV=1.0) or Joint TMD (Sensitivity=0.95; Specificity= 0.87; PPV= 0.83; NPV=0.96), with a cut-off point between 5-13.9, with the diagnosis obtained from the highest score in each factor (Muscles or Joint). Conclusion: The EDI/TMD is a valid and reliable assessment tool, with adequate and satisfactory psychometric properties, capable of diagnosing people with TMD and classifying the subtype of the condition (Muscles, Joint, and Mixed) (AU).

ABSTRACT

Abstract Objective: To assess the association between temporomandibular disorder (TMD) with sociodemographic factors, health-related factors, and oral conditions in adolescents. Material and Methods: This cross-sectional study was developed with 89 adolescents between 13 to 18 years. TMD diagnosis was obtained by the Research Diagnostic Criteria for Temporomandibular Disease (RDC/TMD) Axis I. Diagnoses of chronic pain, depression, presence of non-specific physical symptoms, including pain and anxiety, were obtained by the RDC/TMD Axis II. The feeling of happiness was measured by the Subjective Happiness Scale. Socio-economic and demographic characteristics were collected through self-administered questionnaires. Patients were examined for dental caries, dental trauma, malocclusion, and tooth wear. Parents answered a sociodemographic, economic, and general health questionnaire. Data were submitted to descriptive statistics, and a logistic regression model was used to assess the association between TMD and the socio-economic, demographic, health-related, and clinical variables. Results: TMD prevalence was 42%. TMD was associated to skin color (p=0.040), use of medications in the past year (p = 0.020) and previous dental trauma (p=0.030). Also, it tended to be associated with the presence of probable awake bruxism (p=0.053). Conclusion: Sociodemographic factors, health-related factors, and oral conditions play a role in TMD, with nonwhite adolescents, those who had used medications in the past year and/or had previous dental trauma having a greater chance of present this disorder (AU).

ABSTRACT

Objetivos: determinar a prevalência da disfunção temporomandibular (DTM) em pacientes de uma clínica escola de Odontologia, descrever o perfil epidemiológico da amostra analisada, caracterizar o grau dessas disfunções e identificar os sinais e sintomas orofaciais mais frequentes. Método: estudo epidemiológico, do tipo transversal, que avaliou pacientes da Clínica Escola de Odontologia da Universidade de Rio Verde, GO. A pesquisa foi realizada através de três questionários: Índice Anamnésico Simplificado de Fonseca (IASF), Questionário para Triagem para Dor Orofacial (QTDO), questionário sociodemográfico e econômico. Resultados: foram analisados 200 pacientes, dentre os quais 78,5% apresentaram algum grau de DTM, sendo o mais prevalente a DTM de leve intensidade, presente em 38% da amostra. Conclusão: nesse cenário, conclui- -se que a maioria dos participantes apresentou DTM, sendo que a intervenção multidisciplinar seria de suma relevância no tratamento, podendo oferecer uma abordagem mais completa e com melhores resultados.(AU)


Objectives: to determine the prevalence of TMD in patients at a dental school clinic, as well as describe its epidemiological profile for the evaluated sample, characterizing the degree of this dysfunction and identifying the most frequent orofacial signs and symptoms. Method: it is an epidemiological, cross-sectional study that evaluated patients at the School of Dentistry clinic, University of Rio Verde, GO, Brazil. The study was carried out using three questionnaires: Simplified Fonseca Anamnesis Index (SFAI), Orofacial Pain Screening Questionnaire (OPSQ), and the socio-demographic and economic questionnaire. Results: 200 patients were evaluated, among which 78.5% had some degree of TMD, the most prevalent being mild TMD, present in 38% of the sample. Conclusion: most participants had TMD, and multidisciplinary intervention would be of paramount importance in the treatment, being able to offer a more complete approach and with better results.(AU)

Introdução: A Disfunção Temporomandibular (DTM) consiste em uma condição complexa, que possui etiologia multifatorial e apresenta uma sintomatologia variada, além de uma certa dificuldade no seu diagnóstico. Objetivo: O estudo tem como objetivo comparar os diagnósticos de um novo índice anamnésico para a DTM (NIA/DTM), elaborado e baseado por um protocolo clínico, e ainda avaliar a sua sensibilidade e especificidade. Métodos: O presente estudo consiste na avaliação de um novo índice anamnésico diagnóstico, composto por 12 itens acerca de sinais e sintomas da DTM, baseado no Critério Diagnóstico para Disfunção Temporomandibular (The Diagnostic Criteria For Temporomandibular Disorders - DC/TMD) ­ considerado padrão-ouro para o diagnóstico na DTM. A amostra do estudo foi composta por 50 indivíduos em situação de espera pelo atendimento no setor de DTM e Dor Orofacial do Departamento de Odontologia da UFRN, Natal ­ RN. Os instrumentos utilizados foram aplicados por um único examinador previamente treinado. Os dados foram organizados e analisados pelo programa SPSS, através do Qui-quadrado e Kappa para sensibilidade e especificidade, considerando como nível de significância 5%. Resultados: Os dados analisados demonstraram que o novo instrumento diagnóstico para a DTM obteve uma boa concordância com o DC/TMD e sensibilidade, mas pouco específico. Conclusão: Pôde-se concluir que o novo índice anamnésico para o diagnóstico da DTM (NIA/DTM), de fácil e prática aplicação clínica, demonstrou boa concordância com o padrão ouro em diagnóstico para a DTM (DC/TMD) em relação às classificações diagnósticas de mialgia e artralgia, evidenciando a necessidade de aperfeiçoamento do instrumento com enfoque diferenciado para as demais alterações (AU).


Introduction: Temporomandibular Dysfunction (TMD) consists of a complex condition, which has a multifactorial etiology and presents varied symptoms, in addition to a certain difficulty in its diagnosis. Objective: The study aims to compare the diagnoses of a new anamnesic instrument for TMD (NIA/TMD), based on a clinical protocol, and to evaluate its sensitivity and specificity. Methods: The present study consists of the evaluation of a new diagnostic instrument, consisting of 12 items about TMD signs and symptoms, based on the Diagnostic Criteria for Temporomandibular Disorders (DC / TMD) - considered standard-gold for TMD diagnosis. The study sample consisted of 50 individuals waiting for care in the TMD and Orofacial Pain sector of the Department of Dentistry at UFRN, Natal - RN. The instruments used were applied by a single examiner previously trained. The data were organized and analyzed by the SPSS program, using the Chi-square and Kappa for sensitivity and specificity, considering a significance level of 5%. Results: The analyzed data demonstrated that the new diagnostic tool for TMD obtained a good agreement with the DC / TMD and sensitivity, but little specific. Conclusion: It was concluded that the new anamnesic index for the diagnosis of TMD (NIA / TMD), which is easy and practical to apply clinically, demonstrated good agreement with the gold standard in diagnosis for TMD (DC / TMD) in relation to the classifications diagnoses of myalgia and arthralgia, highlighting the need to improve the instrument with a differentiated approach to other changes (AU).

ABSTRACT

This study aimed to investigate whether stress found in activities performed by Brazilian police workers is associated with the risk of developing a temporomandibular joint dysfunction syndrome (TMD), a disorder that is commonly associated with stressful conditions. An integrative literature review on stress, TMD and police work was carried out. We identified several stressors in the work of Brazilian police officers and observed that stress influences the development, aggravation and treatment of TMD symptoms. The quality of life and health of TMD carriers is compromised. The fact that studies have shown the association between stress and activities carried out by Brazilian police officers, as well as between stress and TMD, has led to the assumption of a possible relationship between stress caused by police work and the risk of these workers developing TMD. It is recommended that individuals be thoroughly examined before being treated and that such treatment be multidisciplinary. The literature on TMD in police workers is still incipient, which is why we recommend the development of further studies on the topic.


O estudo objetivou investigar se o estresse presente nas atividades dos trabalhadores da polícia brasileira pode ter relação com o desenvolvimento da síndrome da disfunção da articulação temporomandibular (DTM), uma doença comumente associada a condições estressantes. Realizou-se uma revisão integrativa de literatura sobre os temas estresse, DTM e atividade policial. Foram identificadas diversas fontes estressoras no trabalho do policial brasileiro e observou-se que o estresse influencia no desenvolvimento, agravamento e tratamento dos sintomas da síndrome. Há comprometimento na qualidade de vida e saúde dos portadores de DTM. O fato de estudos terem mostrado associação entre estresse e atividades desenvolvidas pelos policiais brasileiros, bem como relação entre estresse e DTM, levou à suposição de que é possível haver relação entre o estresse gerado pelo trabalho executado por policiais e o risco de desenvolvimento da DTM por essa classe de trabalhadores. É recomendado que o indivíduo portador da síndrome seja avaliado como um todo antes da execução do tratamento e que esse tratamento seja multidisciplinar. A literatura sobre DTM em policiais ainda é incipiente, motivo pelo qual sugere-se o desenvolvimento de estudos sobre o tema.

RESUMEN Introducción: Existe una prevalencia de la disfunción de la articulación temporomandibular relacionada con los trastornos de la personalidad por los estados de ánimo que experimentan estos pacientes. Objetivo: Determinar la comorbilidad de los trastornos de la personalidad y la disfunción de la articulación temporomandibular. Métodos: Se realizó un estudio observacional descriptivo y transversal, en la consulta de Ortodoncia, en el período de un año, desde febrero de 2017 a 2018. El universo fue de 250 pacientes; se seleccionó una muestra de 64 por muestreo aleatorio simple, de acuerdo con los criterios de inclusión. La presencia de disfunción temporomandibular se evaluó por el índice de disfunción de Maglione y colaboradores. Resultados: Presentaron disfunción de la articulación temporomandibular 57 pacientes, de los cuales el 46,87 por ciento tenía disfunción II (moderada), el 89,06 por ciento presentó algún trastorno de la personalidad como el histriónico (25,00 por ciento) y el obsesivo compulsivo (26,56 por ciento). Conclusiones: La mayoría de los pacientes examinados tuvieron una relación entre los trastornos de la personalidad con la disfunción de la articulación temporomandibular, por lo que existe una comorbilidad de ambas entidades(AU)


ABSTRACT Introduction: There is a prevalence of temporomandibular joint dysfunction which is related to personality disorders, due to the moods experienced by these patients. Objective: Determine the comorbidity of personality disorders and temporomandibular joint dysfunction. Methods: A cross-sectional observational descriptive study was conducted at the Orthodontics service during one year, from February 2017 to 2018. The study universe was 250 patients, of whom a sample of 64 was selected by simple random sampling on the basis of inclusion criteria. The presence of temporomandibular dysfunction was determined by Maglione and collaborators' dysfunction index. Results: Temporomandibular joint dysfunction was found in 57 patients, of whom 46.87 percent had dysfunction class II (moderate), and 89.06 percent had some sort of personality disorder, such as histrionic (25.00 percent) or obsessive-compulsive personality (26.56 percent). Conclusions: In most of the patients examined a relationship was found between personality disorders and temporomandibular joint dysfunction, showing that there is comorbidity between the two conditions(AU)

Resumo O estudo objetivou investigar se o estresse presente nas atividades dos trabalhadores da polícia brasileira pode ter relação com o desenvolvimento da síndrome da disfunção da articulação temporomandibular (DTM), uma doença comumente associada a condições estressantes. Realizou-se uma revisão integrativa de literatura sobre os temas estresse, DTM e atividade policial. Foram identificadas diversas fontes estressoras no trabalho do policial brasileiro e observou-se que o estresse influencia no desenvolvimento, agravamento e tratamento dos sintomas da síndrome. Introdução: Pacientes com anomalias dentomaxilofaciais do sistema estomatognático frequentemente experimentam disfunção da articulação temporomandibular, além do exame clínico existem índices que permitem determinar o grau de disfunção. Objetivo: Identificar o grau de disfunção da articulação temporomandibular em pacientes com anomalias dentomaxilofaciais. Métodos: Estudo transversal, descritivo, observacional foi feito com 121 pacientes atendidos na clínica ortodôntica da Clínica Universitária de Especialidades Estomatológicas Manuel Cedeño, no período de maio de 2016 a março de 2018. As variáveis; a serem estudadas foram idade, sexo e grau de disfunção da articulação temporomandibular. Foi aplicado o índice de Maglione et al. Resultados: Pacientes com anomalias dentomaxilofaciais apresentaram um grau leve de disfunção em 71,90 percent da população total; disfunção da articulação temporomandibular esteve presente em 72,58 percent na faixa etária de 18 a 20 anos e no sexo feminino com 61,98 percent. Conclusão: Disfunção temporomandibular leve prevaleceu em pacientes com anomalias dentomaxilofaciais(AU)

Además del examen clínico existen índices que permiten determinar el grado de disfunción. Objetivo: Identificar el grado de disfunción de la articulación temporomandibular en pacientes con anomalías dentomaxilofaciales. Métodos: Se realizó un estudio observacional descriptivo de corte transversal con 121 pacientes atendidos en la consulta de Ortodoncia de la Clínica Universitaria de Especialidades Estomatológicas Manuel Cedeño, de mayo de 2016 a marzo de 2018. Las variables a estudiar fueron la edad, sexo y grado de disfunción de la articulación temporomandibular. Se aplicó el índice de Maglione et al. Resultados: Los pacientes con anomalías dentomaxilofaciales tuvieron un grado de disfunción leve en 71,90 por ciento de la población total. La disfunción de la articulación temporomandibular se presentó en 72,58 por ciento del grupo etario de 18 a 20 años y en el sexo femenino, con 61,98 por ciento. Conclusiones: La disfunción de la articulación temporomandibular leve prevaleció en los pacientes con anomalías dentomaxilofaciales(AU)

Besides clinical examination, indices such as Maglione and collaborators' allow to determine the degree of dysfunction. Objective: Identify the degree of temporomandibular joint dysfunction in patients with dentomaxillofacial anomalies. Methods: A cross-sectional observational descriptive study was conducted of 121 patients attending the Orthodontics Service at Manuel Cedeño Dental Specialties University Clinic from May 2016 to March 2018. The variables analyzed were age, sex and degree of temporomandibular joint dysfunction. Use was made of the Maglione and collaborators' index. Results: Patients with dentomaxillofacial anomalies had a mild degree of dysfunction in 71.90 percent of the total population. Temporomandibular joint dysfunction was present in 72.58 percent of the 18-20 years age group and in the female sex, with 61.98 percent. Conclusions: Mild temporomandibular joint dysfunction prevailed in patients with dentomaxillofacial anomalies(AU)

Introdução: A disfunção temporomandibular apresenta etiologia multifatorial, enquadrando-se nesses fatores etiológicos condições psicológicas como a ansiedade. Objetivo: Identificar a prevalência da disfunção temporomandibular, ansiedade e seus respectivos graus nos graduandos. Métodos: Trata-se de um estudo do tipo transversal, observacional, com abordagem indutiva, adotando como estratégia de coleta de dados o Índice Anamnésico de Fonseca e o Inventário de Ansiedade Traço-Estado em uma amostra de 185 estudantes de Odontologia da Universidade Federal de Campina Grande, Patos, Brasil. Os dados foram trabalhados pela estatística descritiva e submetidos ao teste estatístico Qui-quadrado e Exato de Fisher considerado significativo ao nível de 5 porcento. Resultados: Os resultados indicam que a maioria da amostra é composta pelo sexo feminino (67 porcento) com idade média de 21,4 anos. Além disso, 79 porcento dos alunos apresentou algum grau da disfunção, tendo a maior parte (72 porcento) se encaixado no grau leve. A ansiedade estado e a ansiedade traço foram mais prevalentes no grau moderado com 79 porcento e 72 porcento, respectivamente. Não foi identificada associação estatística significativa entre a presença e ausência de Disfunção Temporomandibular e ansiedade, seja ela traço ou estado, na amostra estudada com p= 0,484 e p= 0,297, respectivamente. Conclusão: Conclui-se que houve uma alta prevalência de Disfunção Temporomandibular e ansiedade nos graduandos de Odontologia, destacando-se, assim, a importância da busca detalhada desses sinais e sintomas durante o exame clínico(AU)


Introducción: La disfunción temporomandibular presenta una causa multifactorial, incluyéndose en estos factores causales condiciones psicológicas como la ansiedad. Objetivo: Identificar la prevalencia de la disfunción temporomandibular y la ansiedad y sus respectivos niveles en estudiantes. Métodos: Se trata de un estudio del tipo transversal, observacional y con enfoque inductivo. Se adoptó como estrategia de recolección de datos el índice de anamnesis de Fonseca y el inventario de ansiedad rasgo-estado en una muestra de 185 estudiantes de Odontología de la Universidade Federal de Campina Grande. Los datos fueron trabajados por la estadística descriptiva y sometidos a la prueba estadística de chi cuadrado y exacta de Fisher, considerado significativo al nivel de 5 por ciento. Resultados: Los resultados indican que la mayoría de la muestra eran del sexo femenino (67 por ciento) con una edad promedio de 21,4 años. Además, 79 por ciento de los alumnos presentaron algún nivel de disfunción, y la mayor parte (72 por ciento) se encuadró en el nivel leve. La ansiedad estado y la ansiedad rasgo fueron más prevalentes en el nivel moderado con 79 por ciento y 72 por ciento, respectivamente. No se identificó ninguna asociación estadística significativa entre la presencia y ausencia de disfunción temporomandibular y la ansiedad, sea rasgo o estado, en la muestra investigada, con los respectivos valores de p= 0,484 y p= 0,297. Conclusiones: Hubo una alta prevalencia estadística de disfunción temporomandibular y ansiedad en los estudiantes de Odontología, así que se destaca la importancia de la búsqueda detallada de estos signos y síntomas en el examen clínico(AU)


Introduction: The etiology of temporomandibular dysfunction is multifactorial, and causal factors include psychological conditions such as anxiety. Objective: Identify the prevalence of temporomandibular dysfunction and anxiety as well as their levels among dental students from the Federal University of Campina Grande. Methods: An inductive cross-sectional observational study was conducted of a sample of 185 dental students from the Federal University of Campina Grande. Data collection was based on Fonseca's anamnesis index and the State-Trait Anxiety Inventory. Data were analyzed by descriptive statistics and subjected to the chi-square statistical test and Fisher's exact test, with a significance level of 5 percent. Results: Results show that most of the sample was female (67 percent) with a mean age of 21.4 years. On the other hand, 79 percent of the students had some degree of dysfunction, which was mild in most (72 percent). Anxiety state and anxiety trait were more prevalent on the moderate level with 79 percent and 72 percent respectively. No significant statistical association was identified in the study sample between the presence and absence of temporomandibular dysfunction and anxiety, be it trait or state, with values of p= 0.484 and p= 0.297 respectively. Conclusions: High statistical prevalence was found of temporomandibular dysfunction and anxiety among dental students, hence the importance of a detailed search for those signs and symptoms during clinical examination(AU)

Objetivo Verificar la presencia de disfunción temporomadibular (DTM), trastorno del sueño y estrés, así como la asociación entre esos factores en trabajadores de una industria del Estado de São Paulo. Material y Métodos Se aplicó cuestionario de Fonseca para verificar el grado de DTM; el Cuestionario de Evaluación del Sueño de Toronto (SAQ) para verificar la calidad y ocurrencia de trastornos del sueño; y la Escala de Reajuste Social (SRRS) para verificar el grado de estrés. Se tabularon los datos de la encuesta por medio del programa Epi InfoTM 7 y se los analizaron estadísticamente por medio del Test de Chi-Cuadrado, con nivel de significación del 5 %. Resultados Participaron de la encuesta 104 trabajadores, la mayoría hombres (74 %), con edad entre 35 y 44 años (26 %). Treinta y siete (35,6 %) tenían algún grado de disfunción, 65 (62,5 %) presentaron trastornos del sueño y 6 (5,8 %), grados más altos de estrés. Tras el análisis estadístico, no se observó asociación significativa entre estrés y disfunción temporomandibular (DTM). Sin embargo, hubo asociación entre calidad del sueño y el sexo del individuo con la DTM (p<0,01). Conclusiones Se concluye que un alto porcentaje de la población analizada presenta trastornos del sueño y DTM. El sexo y la calidad del sueño influyen en la ocurrencia de la DTM.(AU)


Objective To determine the presence of temporomandibular disorders (TMD), sleep disorders and stress, as well as the association between these factors, among industrial workers in São Paulo State, Brazil. Material and Methods Fonseca's questionnaire was used to verify the level of TMD, the Toronto Sleep Assessment Questionnaire (SAQ) was applied to check the quality and occurrence of sleep disorders, and the Social Readjustment Rating Scale (SRRS) was used to check the degree of stress. The data collected were tabulated with Epi InfoTM 7 and statistically analyzed using the chi-square test, with a 5 % significance level. Results 104 workers participated in the survey. Most were male (74 %) between 35 and 44 years of age (26 %). Thirty-seven (35.6 %) had some degree of disorder, 65 (62.5 %) presented with sleep disorders, and 6 (5.8 %) presented with higher degrees of stress. After statistical analysis, there was no significant association between stress and temporomandibular disorders (TMD). However, there was an association between quality of sleep and sex of the individual with TMD (p<0.01). Conclusion We conclude that a high percentage of the analyzed population has sleep disorders and TMD. Sex and the quality of sleep influence the occurrence of TMD.(AU)

ABSTRACT

PURPOSE: The primary objective of this study was to assess the prevalence of temporomandibular disorders (TMDs) and comorbid factors (sleep bruxism and headaches). This study was a cross-sectional population survey in the city of Maringá, state of Paraná, Brazil. MATERIALS AND METHODS: Axes I and II of the Research Diagnostic Criteria for TMD (RDC/TMD) were used for assessment of TMD signs and symptoms. The population was users of the Brazilian public health system (SUS), of both sexes, between the ages of 20 and 65 years, and not seeking treatment for TMD. RESULTS: The selected population (N = 1,643) was composed mostly of (a) women (65.9%), (b) married or single individuals (90.6%), (c) Caucasians (70.1%), (d) individuals aged 32.7 ± 10.3 years, (e) individuals earning a medium income (75.1%), and (f) those who had completed a high school education or higher (79.9%). According to the chronic pain grade classification (CPG) in the RDC/TMD Axis II, 36.2% of the population had some degree of TMD pain (CPG I to IV); however, only 5.1% had severe limitation due to pain (CPG III or IV). In the RDC/TMD Axis I diagnoses, 29.5% presented with muscle disorders (group I), 7.9% with disk displacements (group II), and 39.1% with other joint disorders (group III). Headaches were present in 67.9% and awake and sleep bruxism in 30% and 33.4% of the population, respectively. CONCLUSION: The prevalence of signs and symptoms of TMD was high in this population, but with low disability; however, the proportion of patients in need of treatment was much lower.

ABSTRACT

Aim: To compare the electromyographic activity and the asymmetry index among degrees of severity of temporomandibular disorders (TMD). Methods: Surface electromyography (EMG) of the right and left masseter and temporalis muscles was performed in 126 undergraduate students at rest and at maximal voluntary contraction. Three measurements were performed for five seconds of maximal contraction and mandibular rest. The degree of temporomandibular dysfunction was identified according to the Fonseca anamnestic index. The analysis of the asymmetry index for two pairs of muscles during maximal voluntary contraction was based on the asymmetry index proposed by Naeije, McCarrol and Weijs (1989). Results: 48.41% of the sample had mild TMD, followed by volunteers without TMD (26.98%), moderate TMD (19.05%) and severe (5.56%). The survey results show absence of correlation between the Fonseca anamnestic index and electromyographic activity at rest and at maximal voluntary contraction in undergraduate students in both muscles (p>0.05) and the asymmetry index did not differ between the analyzed groups. Conclusions: For the population of undergraduate students, there is no evidence that the presence and severity of TMD influence the EMG activity of masseter and temporalis muscles and the muscle asymmetry index at rest and maximal voluntary contraction.

Evaluar, en pacientes con artritis reumatoidea, los síntomas y signos clínicos de la ATM desde el punto de vista odontológico y reumatológico, interrelacionando los hallazgos realizados con reactantes de fase aguda -como eritrosedimentación- y con marcadores de la enfermedad -como el Health Assessment Questionaire (HAQ) y el Score Disease Activity. Determinar cuáles son los predictores más importantes de la enfermedad, a fin de facilitar el diagnóstico y el manejo temprano de patologías de la ATM en la artritis reumatoidea (DAS28). Materiales y métodos: se evaluaron 190 articulaciones de pacientes con artritis reumatoidea -considerando dos por paciente- y 22 controles. La evaluación de la ATM fue realizada mediante inspección clínica, cuestionarios y radiografías. La artritis reumatoidea fue testeada clínica, serológica y radiográficamente. Para evaluar las ATMs, se utilizó el score de Rohlin y Petersson y, para las manos, el de Larsen. Para la diferencia de porcentajes (p) se empleó la prueba de las diferencias proporcionales y la asociación se determinó con el coeficiente de correlación de Pearson (r). Resultados: se hallaron anormalidades radiográficas en 167 (87,89 por ciento) de las 190 ATM evaluadas. El 57,37 por ciento de los pacientes con artritis reumatoidea referían dolor en la ATM. El 58,95 por ciento presentaba menos de 20 piezas dentarias. El 70,53 por ciento tenía la apertura bucal máxima disminuida. Existió una asociación altamente significativa (A/S=0,000) entre las cefaleas de los pacientes con artritis reumatoidea y el grupo control; una asociación no significativa (N/S) entre los resultados del DAS28, el HAQ y la ERS (eritrosedimentación) con las erosiones de la ATM; y A/S=0,62 entre las erosiones de la mano y las de la ATM. ABSTRACT

Temporomandibular disorder (TMD) is a common condition. This study is part of a research group and it investigated the prevalence of TMD and myofascial pain and its association with gender, age and socioeconomic class. The sample comprised 100 subjects, aged 15 to 70, users of the Family Health Units' services, in the city of Recife, PE, Brazil. The TMD degree was evaluated using the Research Diagnostic Criteria for TMD and socioeconomic class by the Economic Classification Criteria Brazil. Categorical variables were analyzed by chi-square test for proportions and Fisher's exact test for 2x2 tables, and binary logistic analysis to track the relationship between the independent and dependent variables. According to the results, 42% of the subjects had TMD and 14% myofascial pain. No statistically significant association could be found between TMD and gender or socioeconomic class, but it was found to have statistically significant association with age, and myofascial pain was associated with socioeconomic class. Considering that the results of the present study should be confirmed by further studies and the fact that this was a pilot study, the prevalence must be analyzed with caution.

ABSTRACT

La articulación temporomandibular constituye una de las estructuras faciales más complejas y ha sido reconocida como una causa de dolor y disfunción desde principios del siglo pasado. Su complicado funcionamiento determina que las mínimas alteraciones produzcan en ellas una gran diversidad de problemas funcionales, a los que se les conoce como trastornos temporomandibulares, que pueden originarse muy temprano con relación a las etapas de crecimiento y desarrollo craneofacial. El objetivo de este trabajo fue determinar la prevalencia de disfunción de la articulación temporomandibular e identificar los factores de riesgo asociados a estos trastornos en el grupo estudiado. Esta investigación fue de tipo descriptiva y transversal, cuyo universo de trabajo estuvo conformado por 320 niños, con edades comprendidas entre 7 y 11 años, pertenecientes a la Escuela primaria urbana Sofiel Riverón. El diagnóstico de disfunción fue realizado mediante la aplicación del Test de Krogh Paulsen. Los resultados mostraron que 6 de cada 100 niños encuestados presentaba disfunción temporomandibular y los factores de riesgo identificados con mayor frecuencia fueron: interferencias oclusales (45 por ciento), hábitos parafuncionales (35 por ciento), bruxismo (15 por ciento), estrés (5 por ciento). Fue apreciado un incremento de estos factores conjuntamente con las edades, aunque no de manera uniforme. Se hace necesario enfatizar en el examen funcional de la articulación temporomandibular en niños, ante la manifestación precoz de desórdenes temporomandibulares y mediante actividades de promoción, prevención e intervención clínicas oportunas, actuar sobre estos factores de riesgo y contribuir a disminuir las cifras de afectados.


The temporomandibular joint is one of the most complex facial structures and has been recognized as a pain and dysfunction source since the beginning of the last century. Its complicated functioning determines that the minimal alterations produce a great diversity of functional problems, known as temporomandibular disorders that may be originated very early with relation to the cranio-facial growing and development stages. The aim of this work was determining the prevalence of the temporomandibular joint dysfunction and identifying the risk factors associated to these disorders in the studied group. This was a cross- sectional descriptive research; the working universe was formed by 320 children aged 7-11 years, assisting the urban primary school Sofiel Riverón. The dysfunction diagnosis was made applying the Krogh Paulsen test. The results showed that 6 of every 100 inquired children had temporomandibular dysfunctions and the risk factors more frequently identified were: oclusal interferences (45 percent), parafunctional habits (35 percent), bruxism (15 percent), and stress (5 percent). It was appreciated these factors increment with the age, although not in an even way. It is necessary to emphasize in the functional examination of the children temporomandibular joint in the presence of precocious temporomandibular disorders and, through opportune activities of promotion, prevention and clinical intervention, influence on these risk factors and contribute to diminishing the number of affected children.

OBJECTIVE: The purpose of this study was to investigate the prevalence of temporomandibular disorders (TMD) in denture wearers and the association between prosthetic factors and this condition. BACKGROUND: There is no consensus about the relationship between prosthetic factors and TMD among denture wearers. MATERIALS AND METHODS: The sample was composed of 92 patients wearing both maxillary and mandibular complete dentures. The Research Diagnostic Criteria for TMD (RDC/TMD) were adopted for patient examination. Objective evaluation of denture quality was determined by analysis of retention and stability of mandibular denture, interocclusal distance, articulation and occlusion. Association between denture quality and TMD diagnosis was analysed using chi-square and Fisher's exact tests. RESULTS: 37.4% of the patients presented TMD. Denture quality was not significantly associated with the presence of TMD: mandibular retention (p = 0.466); mandibular stability (p = 0.466); interocclusal distance (p = 0.328); centric relation (p = 0.175); and balanced occlusion (p = 0.56). CONCLUSION: Within the scope of this case-controlled cross-sectional study, no robust association between prosthetic factors and TMD was found.

ABSTRACT

The present investigation was performed in a population of patients with temporomandibular disorders (TMD), and it was designed to assess the correlation between self-reported questionnaire-based bruxism diagnosis and a diagnosis based on history taking plus clinical examination. One-hundred-fifty-nine patients with TMD underwent an assessment including a questionnaire investigating five bruxism-related items (i.e. sleep grinding, sleep grinding referral by bed partner, sleep clenching, awake clenching, awake grinding) and an interview (i.e. oral history taking with specific focus on bruxism habits) plus a clinical examination to evaluate bruxism signs and symptoms. The correlation between findings of the questionnaire, viz., patients' report, and findings of the interview/oral history taking plus clinical examination, viz., clinicians' diagnosis, was assessed by means of φ coefficient. The highest correlations were achieved for the sleep grinding referral item (φ = 0·932) and for the awake clenching item (φ = 0·811), whilst lower correlation values were found for the other items (φ values ranging from 0·363 to 0·641). The percentage of disagreement between the two diagnostic approaches ranged between 1·8% and 18·2%. Within the limits of the present investigation, it can be suggested that a strong positive correlation between a self-reported and a clinically based approach to bruxism diagnosis can be achieved as for awake clenching, whilst lower levels of correlation were detected for sleep-time activities.

Objetivo: identificar la frecuencia de las disfunciones temporomandibulares en estudiantes de 16 a 18 años pertenecientes al Instituto Preuniversitario Pedagógico Roberto Labrada Ávila y describir la presencia de síntomas y signos de disfunción temporomandibular y su relación con el sexo en los adolescentes. Métodos: se realizó un estudio observacional, descriptivo, de corte transversal en una muestra de 86 estudiantes con edades de 16 a 18 años que cursaban estudios en el preuniversitario en el año 2010, a los que se les aplicó el Índice Clínico de Helkimo para medir la frecuencia de disfunciones temporomandibulares. Se evaluaron además variables sociodemográficas, y presencia de signos y síntomas según lo detectado al examen físico. A los datos recogidos se les realizó análisis estadístico. Resultados: el 69,8 por ciento de los adolescentes examinados presentó algún nivel de disfunción temporomandibular, la alteración de la función de la articulación temporomandibular se observó en 66,3 por ciento, de todos los estudiantes, en 67,5 por ciento de las mujeres y 64,7 por ciento en los hombres. Conclusiones: existió una alta frecuencia de disfunciones temporomandibulares en la población estudiada(AU)


Objective: To identify temporomandibular dysfunction frequency in 16 to 18 years-old students from Roberto Labrada Ávila high school and to describe signs and symptoms of temporomandibular dysfunction and their relationship to the gender among these adolescents. Method: a cross-sectional, observational and descriptive study was carried out in 86 students ranging 16 to 18 years from Roberto Labrada Ávila high school during 2010. They were evaluated by using the Helkimo's clinical index to measure the frequency of temporomandibular dysfunctions. Demographic and social variables, signs and symptoms detected during physical examination were all evaluated. The collected data were statistically analyzed. Results: In the study group, 69.8 percent showed some temporomandibular dysfunction; the most observed sign was the functional disturbance of temporomandibular joint in 66.3 percent of the students. The percentage of affected females was 67.5 percent whereas that of males was 64.7 percent. Conclusion: The frequency of temporomandibular dysfunctions was high in the study sample(AU)

ABSTRACT

To explore the relationship between sleep bruxism (SB), painful temporomandibular disorders (TMD) and psychologic status in a cross-sectional study. The sample consisted of 272 individuals. The Research Diagnostic Criteria for TMD (RDC/TMD) was used to diagnose TMD; SB was diagnosed by clinical criteria proposed by The American Academy of Sleep Medicine. The sample was divided into four groups: (1) patients without painful TMD and without SB, (2) patients without painful TMD and with SB, (3) patients with painful TMD and without SB and (4) patients with painful TMD and with SB. Data were analysed by Odds Ratio test with a 95% confidence interval. Patients with SB had an increased risk for the occurrence of myofascial pain (OR = 5·93, 95% CI: 3·19-11·02) and arthralgia (2·34, 1·58-3·46). Group 3 had an increased risk for moderate/severe depression and non-specific physical symptoms (10·1, 3·67-27·79; 14·7, 5·39-39·92, respectively), and this risk increased in the presence of SB (25·0, 9·65-64·77; 35·8, 13·94-91·90, respectively). SB seems to be a risk factor for painful TMD, and this in turn is a risk factor for the occurrence of higher depression and non-specific physical symptoms levels, but a cause-effect relationship could not be established.
