ABSTRACT

This study evaluated the efficacy of incorporating different concentrations of bioactive glass-ceramic (Biosilicate) into coconut oil on the remineralizing potential and surface roughness of white spot lesions. Fragments (6 x 6 x 2mm) of bovine teeth were sectioned and initial microhardness (KHN) and surface roughness (Ra) readings were obtained. The samples were submitted to cariogenic challenge to form white spot lesions and were separated into six groups (n=13): 1) Artificial Saliva (AS); 2) Coconut Oil (CO); 3) CO+2% Biosilicate (CO+2%Bio); 4) CO+5% Biosilicate (CO+5%Bio); 5) 2% Biosilicate Suspension (2% Bio) and 6) 5% Biosilicate Suspension (5% Bio). The treatments for 1 cycle/day were: immersion into the treatments for 5 minutes, rinsing in distilled water, and storage in artificial saliva at 37ºC. After 14 days, KHN and Ra readings were taken. The surface roughness alteration ((Ra) was analyzed (Kruskal-Wallis, Dunn's post-test, p<0.05). CO+2%Bio had higher (p = 0.0013) (Ra followed by CO+5%Bio (p = 0.0244) than AS. The relative KHN and remineralization potential were analyzed (ANOVA, Tukey, p<0.05), and 5% Bio treatment presented a higher relative microhardness than all other groups (p>0.05). The remineralizing potential of all the treatments was similar (p > .05). When Biosilicate was added, the pH of the suspensions increased and the alkaline pH remained during the analysis. Biosilicate suspension is more efficient than the incorporation of particles into coconut oil at white spot lesion treatment. In addition to the benefits that coconut oil and Biosilicate present separately, their association can enhance the remineralizing potential of Biosilicate.

ABSTRACT

Transient perivascular inflammation of the carotid artery (TIPIC) is an uncommon condition characterized by inflammation of the carotid artery wall, leading to unilateral neck pain. While TIPIC has been acknowledged by the International Classification of Headache Disorders, only a few patient series have been published thus far. The clinical presentation of TIPIC syndrome typically manifests as unilateral neck pain localized specifically over the carotid artery. This pain is accompanied by ipsilateral tenderness and increased arterial pulsation. The condition commonly follows a self-limited course or demonstrates a favorable response to treatment with nonsteroidal anti-inflammatory drugs. When evaluating patients with suspected TIPIC syndrome, conducting a comprehensive assessment of their clinical history is imperative, while utilizing imaging studies to exclude any potential structural abnormalities of the carotid artery effectively. The authors present a case involving a 57-year-old woman who presented with a two-month history of persistent left cervical pain and tenderness. Ultrasonography findings revealed indirect indications of inflammation in the intima-media layer of the carotid artery, suggestive of carotidynia. Notably, other significant differential diagnoses such as aneurysms or carotid dissection were ruled out. Over the course of the evaluation, there was a gradual and spontaneous improvement in both clinical symptoms and radiological findings, indicating the resolution of the inflammatory process as confirmed by imaging follow-up. This case presents a rare and atypical manifestation of transient neck pain attributed to TIPIC.

ABSTRACT

PURPOSE OF REVIEW: Gepants are small molecules that antagonize calcitonin gene-related peptide (CGRP) receptors. Due to their favorable side effect profile and versatility in treating headaches acutely and preventively, gepants are preferred over triptans. We will cover the indications for the four FDA-approved gepants in adults: rimegepant, atogepant, ubrogepant, and zavegepant. This review will illustrate how gepants will continue to revolutionize the acute and preventive treatment of headaches. RECENT FINDINGS: Gepants are now available in oral tablet, dissolving tablet, and intra-nasal spray formulations. Recent studies have shown promising utility in treating the pre-headache or prodromal phase. They have favorable tolerability, no evidence for association with medication overuse, and remain a safer alternative in those who have cerebrovascular risk factors. Additional research is needed to explore occurrence of Raynaud's phenomenon in participants treated with gepants, as it has been associated with CGRP monoclonal antibodies, but are not extensively studied in gepants. Gepants are expected to play a significant role in the next generation of migraine treatments.

ABSTRACT

Functional electrical stimulation (FES) may allow people who are paralyzed due to spinal cord injuries (SCIs) to regain the ability to move. Deep neural networks (DNNs) trained with reinforcement learning (RL) have been recently explored as a promising methodology to control FES systems to restore upper-limb movements. However, previous studies suggested that large asymmetries in antagonistic upper-limb muscle strengths could impair RL controller performance. In this work, we investigated the underlying causes of asymmetry-associated decreases in controller performance by comparing different Hill-type models of muscle atrophy, and by characterizing RL controller sensitivity to passive mechanical properties of the arm. Simulations indicated that RL controller performance is relatively insensitive to moderate (up to 50%) changes in tendon stiffness and in flexor muscle stiffness. However, the viable workspace for RL control was substantially affected by flexor muscle weakness and by extensor muscle stiffness. Furthermore, we uncovered that RL controller performance issues previously attributed to asymmetrical antagonistic muscle strength resulted from flexor muscle active forces that were insufficient to counteract extensor muscle passive resistance. The simulations supported the adoption of rehabilitation protocols for reaching tasks that prioritize decreasing muscle passive resistance, and counteracting passive resistance with increased antagonistic muscle strength.

Resumo Este estudo avaliou a eficácia da incorporação de diferentes concentrações de vitrocerâmica bioativa (biosilicato) ao óleo de coco no potencial remineralizante e na rugosidade superficial de lesões de manchas brancas. Fragmentos (6 x 6 x 2mm) de dentes bovinos foram seccionados e as leituras iniciais de microdureza (KHN) e rugosidade superficial (Ra) foram obtidas. As amostras foram submetidas ao desafio cariogênico para formação de lesões de manchas brancas e foram separadas em seis grupos (n=13): 1) Saliva Artificial (AS); 2) Óleo de Coco (CO); 3) CO+2% Biosilicato (CO+2%Bio); 4) CO+5% Biosilicato (CO+5%Bio); 5) Suspensão de Biosilicato 2% (2% Bio) e 6) Suspensão de Biosilicato 5% (5% Bio). Os tratamentos de 1 ciclo/dia foram: imersão nos tratamentos por 5 minutos, enxágue em água destilada e armazenamento em saliva artificial a 37ºC. Após 14 dias, foram feitas as leituras de KHN e Ra. A alteração da rugosidade superficial ((Ra) foi analisada (Kruskal-Wallis, pós-teste de Dunn, p<0,05). CO+2%Bio apresentou maior (p = 0,0013) (Ra seguido de CO+5%Bio (p = 0,0244) do que AS. O KHN relativo e o potencial de remineralização foram analisados (ANOVA, Tukey, p<0,05), e o tratamento 5% Bio apresentou uma microdureza relativa maior do que todos os outros grupos (p>0,05). A suspensão de biosilicato é mais eficiente que a incorporação de partículas ao óleo de coco no tratamento de lesões de mancha branca. Além dos benefícios que o óleo de coco e o Biosilicato apresentam separadamente, sua associação pode amplificar o potencial remineralizante do Biosilicato.

ABSTRACT

Functional electrical stimulation (FES) can be used to restore motor function to people with paralysis caused by spinal cord injuries (SCIs). However, chronically-paralyzed FES-stimulated muscles can fatigue quickly, which may decrease FES controller performance. In this work, we explored the feasibility of using deep neural network (DNN) controllers trained with reinforcement learning (RL) to control FES of upper-limb muscles after SCI. We developed upper-limb biomechanical models that exhibited increased muscle fatigability, decreased muscle recovery, and decreased muscle strength, as observed in people with chronic SCIs. Simulations confirmed that controller training time and controller performance are impaired to varying degrees by muscle fatigability. Also, the simulations showed that large muscle strength asymmetries between opposing muscles can substantially impair controller performance. However, the results of this study suggest that controller performance for highly-fatigable musculoskeletal systems can be preserved by allowing for rest between movements. Overall, the results suggest that RL can be used to successfully train FES controllers, even for highly-fatigable musculoskeletal systems.

ABSTRACT

Cervical spinal cord injuries frequently cause paralysis of all four limbs - a medical condition known as tetraplegia. Functional electrical stimulation (FES), when combined with an appropriate controller, can be used to restore motor function by electrically stimulating the neuromuscular system. Previous works have demonstrated that reinforcement learning can be used to successfully train FES controllers. Here, we demonstrate that transfer learning and curriculum learning can be used to improve the learning rates, accuracies, and workspaces of FES controllers that are trained using reinforcement learning.

INTRODUCTION: Burnout syndrome is a phenomenon characterized by chronic emotional exhaustion that can lead to physical, psychological, and social consequences. Because they need to support themselves financially, university professors have accepted increasingly longer working hours and accumulated duties, resulting in greater exposure to factors that may induce the onset of mental disorders such as burnout syndrome. OBJECTIVES: This cross-sectional epidemiological study aimed to determine the prevalence of burnout syndrome and predisposing factors in university professors working in Salvador, Brazil. METHODS: The sample consisted of 210 participants. The Maslach Burnout Inventory adapted for teachers, the gold standard for detecting burnout syndrome, and a sociodemographic questionnaire were administered to identify variables that may influence the development of burnout. Then, the data were compiled in Excel and analyzed with the aid of GraphPad Prism. RESULTS: The prevalence of burnout was 41% (n = 86). A stratified data analysis showed that being under 40 years of age, being single, being childless, teaching natural sciences, and working at several institutions are risk factors for developing burnout syndrome. CONCLUSIONS: The prevalence of burnout syndrome in university professors was 41%. Professors who were young, single, childless, taught natural sciences, and worked at more than one institution were found to be more likely to develop burnout syndrome.

ABSTRACT

The 2,3,5-triiodobenzoic acid (TIBA) is an iodine contrast agent used for visualization of tissue in X-ray techniques. However, TIBA induces physiological complications like increase in oxygen reactive species (ROS), and consequently, contrast-induced nephropathies. TIBA's antitumor activity was demonstrated in lung cancer, but the subcellular mechanisms involving its activity in tumor cells are still unknown. Thus, the objective of this work was evaluate whether the anti-tumor activity of TIBA involves ROS increase, in tumor lines of non-small cell lung cancer (H460), chronic myeloid leukemia (K562), and its cytotoxicity in normal renal epithelial (VERO). The MTT (3-(4,5-dimethylthiazol-2-yl)-2,5 diphenyltetrazolium bromide) assay was used for evaluation of cell viability, the H2DCFDA (cell-permeant 2',7'-dichlorodihydrofluorescein diacetate) fluorescent probe to evaluate ROS induction, cell cycle analysis was performed using flow cytometry to measure cell death, and immunofluorescence with annexin/7-AAD (7-amino-actinomycin D), to assess the association of cell death with the ROS generation. TIBA decreases cell viability in a dose-dependent manner for the H460 and K562. However, VERO cells showed less response to the drug, with 70% viable cells after 72 h of treatment in the highest concentration of the drug. While the tumor cells with only 20% viable cells. Besides, tumor cells exhibited higher DNA fragmentation, compared to the renal line (VERO with 5% of fragmented DNA, H460 with 26%, and 56% in K562). Finally, TIBA-induced ROS increase and apoptosis in all lines, which is significantly decreased after treatment with the antioxidant N-acetyl-cysteine (NAC). These data demonstrate the relationship between the increased cellular oxidative stress and the anti-tumor action of the TIBA.

ABSTRACT

High-level spinal cord injuries often result in paralysis of all four limbs, leading to decreased patient independence and quality of life. Coordinated functional electrical stimulation (FES) of paralyzed muscles can be used to restore some motor function in the upper extremity. To coordinate functional movements, FES controllers should be developed to exploit the complex characteristics of human movement and produce the intended movement kinematics and/or kinetics. Here, we demonstrate the ability of a controller trained using reinforcement learning to generate desired movements of a horizontal planar musculoskeletal model of the human arm with 2 degrees of freedom and 6 actuators. The controller is given information about the kinematics of the arm, but not the internal state of the actuators. In particular, we demonstrate that a technique called "hindsight experience replay" can improve controller performance while also decreasing controller training time.

ABSTRACT

Intracortical brain-computer interfaces (iBCIs) have the potential to restore hand grasping and object interaction to individuals with tetraplegia. Optimal grasping and object interaction require simultaneous production of both force and grasp outputs. However, since overlapping neural populations are modulated by both parameters, grasp type could affect how well forces are decoded from motor cortex in a closed-loop force iBCI. Therefore, this work quantified the neural representation and offline decoding performance of discrete hand grasps and force levels in two human participants with tetraplegia. Participants attempted to produce three discrete forces (light, medium, hard) using up to five hand grasp configurations. A two-way Welch ANOVA was implemented on multiunit neural features to assess their modulation to force and grasp Demixed principal component analysis (dPCA) was used to assess for population-level tuning to force and grasp and to predict these parameters from neural activity. Three major findings emerged from this work: (1) force information was neurally represented and could be decoded across multiple hand grasps (and, in one participant, across attempted elbow extension as well); (2) grasp type affected force representation within multiunit neural features and offline force classification accuracy; and (3) grasp was classified more accurately and had greater population-level representation than force. These findings suggest that force and grasp have both independent and interacting representations within cortex, and that incorporating force control into real-time iBCI systems is feasible across multiple hand grasps if the decoder also accounts for grasp type.

ABSTRACT

Helicoverpa armigera Hübner, 1805 and Spodoptera frugiperda Smith, 1797 are polyphagous pests of great agricultural importance in subtropical and temperate climate regions. The usual management of production areas in the southern region of Brazil occurs after the harvesting of summer crops, and the areas are sown with pasture. Thus, forages recommended for grazing are azevém (Lolium multiflorumLam.) and cornichão (Lotus corniculatus L.) due to their nutritional benefits, good palatability, regrowth, and hardiness. Considering the high degree of polyphagia of H. armigera and S. frugiperda, and the impact of maintaining continuous feeding areas (green bridges) in the management of these species, this work aimed to evaluate the development of H. armigera and S. frugiperda fed leaves of azevém and cornichão under laboratory conditions. The insects were collected in the city of Capão do Leão/RS in corn and soybean plantations. For each forage species, 130 newly hatched caterpillars were each placed in autoclaved glass tubes, with one-third of the tube length containing forage. The tubes were capped with waterproof cotton and placed in a climate-controlled room at 25 ± 1°C, 70 ± 10% relative humidity, and a 12-h photophase. The food was replenished daily until the caterpillars entered the pupae stage. The pupae were sexed and weighed, and the newly emerged adults were placed in pairs in PVC cages, lined with white A4 sulfite paper. The papers used as a laying substrate were removed and changed daily, and eggs were counted. The number of instars was determined by the linearized Dyar rule model. The complete randomization design was employed for the variables biological cycle length and viability of egg, caterpillar, pre-pupa, pupa, adult, and pre-oviposition phases and weight of caterpillars on the 14th day and pupae after 24 h. Based on the results obtained, a fertility life table was prepared. H. armigera did not complete the cycle, with only three instars and a duration of 22.1 and 24.6 days in azevém and cornichão, respectively. When evaluated in S. frugiperda caterpillars, development in forage species was observed, with five and six instars and duration of 51.7 and 45.1 days in azevém and cornichão, respectively. The azevém was distinguished by interference in the development cycle of the species, reducing the effect of the green bridge. In addition, surviving insects were susceptible to the effects of the agroecosystem due to the low-quality food source.

Após a colheita das culturas de verão no sul do Brasil, as áreas são utilizadas como pastagem. Com isso, as forrageiras recomendadas ao pastoreio são cornichão (Lotuscorniculatus L.) e azevém (Loliummultiflorum Lam.) por apresentarem benefícios nutricional, palatabilidade, rebrota e rusticidade. Devido ao alto grau de polifagia de H. armigera e S. frugiperda e o impacto de áreas continuas de alimento (pontes verdes) no manejo destas espécies, este trabalho objetivou-se avaliar o desenvolvimento de H. armigera e S. frugiperda alimentadas com azevém e cornichão em condições de laboratório. Os insetos foram coletados no município de Capão do Leão/RS em plantios de milho e soja. Para cada espécie forrageira foram individualizadas 130 lagartas recém-eclodidas em tubos de vidro autoclavados, com um terço (1/3) do seu comprimento contendo a respectiva forrageira. Os tubos foram tampados com algodão hidrófugo e levados para a sala climatizada a 25 ± 1°C, UR de 70 ± 10% e 12 horas de fotofase. Diariamente, o alimento foi reposto até que as lagartas entrarem em estágio de pupa. As pupas foram sexadas e pesadas e os adultos recém-emergidos foram individualizados em casais em gaiolas de PVC, forradas com papel sulfite A4 branco no seu interior. Os papéis utilizados como substrato de postura foram retirados e trocados diariamente e os ovos foram contabilizados. O número de instares foi determinado pelo modelo linearizado da regra de Dyar. O delineamento utilizado foi inteiramente casualizado (DIC) para as variáveis duração do ciclo biológico e viabilidade das fases de ovo, lagarta, pré-pupa, pupa, adultos e pré-oviposição, peso de lagartas ao decimo quarto (14°) dia e pupas após 24 horas. Com base nos resultados obtidos, foi elaborada tabela de vida de fertilidade. H. armigera não completou o ciclo, com apenas três instares e duração de 22,1 e 24,6 dias em azevém e cornichão, respectivamente. Quando avaliado em lagartas de S. frugiperda observou o desenvolvimento nas espécies forrageiras, com 5 e 6 instares e duração de 51,7 e 45,1 dias em azevém e cornichão, respectivamente. Contudo, considerando o critério que evite a formação de ponte verde, azevém destacou-se por interferir no ciclo de desenvolvimento das espécies. Onde, mesmos os que sobrevivem, estão suscetíveis aos efeitos do agroecossitema devido a fonte alimentar ser de baixa qualidade.

ABSTRACT

Hybrid kinetic and kinematic intracortical brain-computer interfaces (iBCIs) have the potential to restore functional grasping and object interaction capabilities in individuals with tetraplegia. This requires an understanding of how kinetic information is represented in neural activity, and how this representation is affected by non-motor parameters such as volitional state (VoS), namely, whether one observes, imagines, or attempts an action. To this end, this work investigates how motor cortical neural activity changes when three human participants with tetraplegia observe, imagine, and attempt to produce three discrete hand grasping forces with the dominant hand. We show that force representation follows the same VoS-related trends as previously shown for directional arm movements; namely, that attempted force production recruits more neural activity compared to observed or imagined force production. Additionally, VoS-modulated neural activity to a greater extent than grasping force. Neural representation of forces was lower than expected, possibly due to compromised somatosensory pathways in individuals with tetraplegia, which have been shown to influence motor cortical activity. Nevertheless, attempted forces (but not always observed or imagined forces) could be decoded significantly above chance, thereby potentially providing relevant information towards the development of a hybrid kinetic and kinematic iBCI.

ABSTRACT Introduction: Thrombophilia is a thrombosis susceptibility of genetic, acquired or mixed nature. Among acquired causes, the antiphospholipid syndrome (APS) stands out as an autoimmune disease characterized by antiphospholipid antibodies, thrombotic events or recurrent gestational loss. Laboratory diagnosis is based on the detection of lupus anticoagulant (LAC), anti-β2-glycoprotein 1 and anticardiolipin; however the determination of LAC still demands uniformity. The last guideline published by the Clinical and Laboratory Standards Institute (CLSI) prioritizes the screening and confirmatory steps, to the detriment of the mixing phase. Objectives: To compare the forms of releasing the LAC and to adopt an investigation protocol in agreement with the international guidelines. Methods: Thirty-six samples with prolonged results in the screening step by the dilute Russell viper venom time (dRVVT) or activated partial thromboplastin time (APTT) were subjected to the mixing steps (1:1) and to the confirmatory steps with high concentrations of phospholipids. Results: For APTT, values whose indexes of circulating anticoagulant (ICA) were greater than 15% were considered positive. For dRVVT, the ratio between screening and confirmation was also used. Of the 36 tested samples, 14 showed correction in the mixing step, but only one resulted negative. Conclusion: ICA aided in identifying the weak antibodies that were probably diluted in the mixing step. There is no gold standard test for the diagnosis of APS, and LAC detection still requires standardization of technique and interpretation.


RESUMO Introdução: Trombofilia é a suscetibilidade à trombose, de natureza genética, adquirida ou mista. Entre as causas adquiridas, destaca-se a síndrome do anticorpo antifosfolípide (SAF) - doença autoimune caracterizada por anticorpos antifosfolípides, eventos trombóticos ou perda gestacional recorrente. O diagnóstico laboratorial baseia-se na detecção do anticoagulante lúpico (ACL), do anti-β2-glicoproteína 1 e da anticardiolipina; entretanto a execução do ACL ainda demanda uniformização. A última diretriz publicada pelo Clinical and Laboratory Standards Institute (CLSI) prioriza as etapas de triagem e confirmatória, em detrimento da mistura. Objetivos: Comparar as formas de liberação do ACL e adotar um protocolo de investigação em anuência às normas internacionais. Métodos: Trinta e seis amostras com resultados prolongados na etapa de triagem pelo ensaio do tempo do veneno da víbora de Russel (dRVVT) ou tempo de tromboplastina parcial ativada (TTPA) foram submetidas às etapas de mistura (1:1) e confirmatórias com altas concentrações de fosfolipídios. Resultados: Para o TTPA, foram considerados positivos os valores cujo cálculo do índice de circulação de anticoagulante (ICA) resultasse superior a 15%. Para o dRVVT, utilizou-se também o valor da razão entre triagem e confirmatória. Das amostras testadas, 14 revelaram correção na etapa da mistura, mas somente uma resultou em pesquisa negativa. Conclusão: O cálculo do ICA auxiliou na identificação dos anticorpos fracos que possivelmente sofreram diluição na etapa da mistura. Não há um exame padrão-ouro para o diagnóstico da SAF, e a pesquisa do ACL ainda demanda uniformização da técnica e da interpretação.

ABSTRACT

Inhibition of the complement system during and after haematophagy is of utmost importance for tick success in feeding and tick development. The role of such inhibition is to minimise damage to the intestinal epithelium as well as avoiding inflammation and opsonisation of salivary molecules at the bite site. Despite its importance, the salivary anti-complement activity has been characterised only in species belonging to the Ixodes ricinus complex which saliva is able to inhibit the alternative and lectin pathways. Little is known about this activity in other species of the Ixodidae family. Thus, the aim of this study was to describe the inhibition of the classical pathway of the complement system by the saliva of Amblyomma cajennense at different stages of the haematophagy. The A. cajennense saliva and salivary gland extract (SGE) were able to inhibit the complement classical pathway through haemolytic assays with higher activity observed when saliva was used. The anti-complement activity is present in the salivary glands of starving females and also in females throughout the whole feeding process, with significant higher activity soon after tick detachment. The SGE activity from both females fed on mice or horses had no significant correlation (p > 0.05) with tick body weight. The pH found in the intestinal lumen of A. cajennense was 8.04 ± 0.08 and haemolytic assays performed at pH 8.0 showed activation of the classical pathway similarly to what occurs at pH 7.4. Consequently, inhibition could be necessary to protect the tick enterocytes. Indeed, the inhibition observed by SGE was higher in pH 8.0 in comparison to pH 7.4 reinforcing the role of saliva in protecting the intestinal cells. Further studies should be carried out in order to identify the inhibitor molecule and characterise its inhibition mechanism.

ABSTRACT

The aim of this study was to use a new methodology to evaluate the fluorescence of composite resins for direct restorations. Microhybrid (group 1, Amelogen; group 2, Opallis; group 3, Filtek Z250) and nanohybrid (group 4, Filtek Z350 XT; group 5, Brilliant NG; group 6, Evolu-X) composite resins were analyzed in this study. A prefabricated matrix was used to prepare 60 specimens of 7.0 × 3.0 mm (n = 10 per group); the composite resin discs were prepared in 2 increments (1.5 mm each) and photocured for 20 seconds. To establish a control group of natural teeth, 10 maxillary central incisor crowns were horizontally sectioned to create 10 discs of dentin and enamel tissues with the same dimensions as the composite resin specimens. The specimens were placed in a box with ultraviolet light, and photographs were taken. Aperture 3.0 software was used to quantify the central portion of the image of each specimen in shades of red (R), green (G), and blue (B) of the RGB color space. The brighter the B shade in the evaluated area of the image, the greater the fluorescence shown by the specimen. One-way analysis of variance revealed significant differences between the groups. The fluorescence achieved in group 1 was statistically similar to that of the control group and significantly different from those of the other groups (Bonferroni test). Groups 3 and 4 had the lowest fluorescence values, which were significantly different from those of the other groups. According to the results of this study, neither the size nor the amount of inorganic particles in the evaluated composite resin materials predicts if the material will exhibit good fluorescence.
