ABSTRACT

INTRODUCTION AND OBJECTIVES: This article presents the annual activity report of the Interventional Cardiology Association of the Spanish Society of Cardiology (ACI-SEC) for the year 2022. METHODS: All Spanish centers with catheterization laboratories were invited to participate. Data were collected online and were analyzed by an external company in collaboration with the members of the board of the ACI-SEC. RESULTS: A total of 111 centers participated. The number of diagnostic studies increased by 4.8% compared with 2021, while that of percutaneous coronary interventions (PCI) remained stable. PCIs on the left main coronary artery increased by 22%. The radial approach continued to be preferred for PCI (94.9%). There was an upsurge in the use of drug-eluting balloons, as well as in intracoronary imaging techniques, which were used in 14.7% of PCIs. The use of pressure wires also increased (6.3% vs 2021) as did plaque modification techniques. Primary PCI continued to grow and was the most frequent treatment (97%) in ST-segment elevation myocardial infarction. Most noncoronary procedures maintained their upward trend, particularly percutaneous aortic valve implantation, atrial appendage closure, mitral/tricuspid edge-to-edge therapy, renal denervation, and percutaneous treatment of pulmonary arterial disease. CONCLUSIONS: The Spanish cardiac catheterization and coronary intervention registry for 2022 reveals a rise in the complexity of coronary disease, along with a notable growth in procedures for valvular and nonvalvular structural heart disease.

ABSTRACT

BACKGROUND: Cardiac computed tomography angiography (CCTA) appears to be an appropriate imaging technique for device surveillance after left atrial appendage occlusion (LAAO). However, the available experience is limited. AIMS: The aim of this study was to determine the prevalence, mechanisms and clinical impact of left atrial appendage (LAA) patency and device-related thrombosis (DRT) following LAAO utilising a novel CCTA-based classification. METHODS: Consecutively enrolled patients who underwent LAAO with an AMPLATZER device were followed up with CCTA. Mechanisms and frequency of residual patency were evaluated and correlated with clinical events. Atrial-side device thrombus, device positioning and presence of signs of device stability were also analysed. RESULTS: A total of 137 patients were included. LAA patency was observed in 56.9% (n=78). Mechanisms and frequency of patency were: malapposition of proximal segment of the device lobe (55.1%), peri-device leak (PDL, 34.6%) and fabric permeability (5.8%). Lobe-LAA axis misalignment was the only independent predictor of device patency after LAAO (HR 38.3, 95% CI: 13.6-107.0; p<0.001). After a median follow-up of 638 days, patency was not associated with an increased risk of death (all-cause or cardiovascular death) or cerebral/peripheral embolism regardless of its mechanism. Any degree of hypo-attenuated thickening (HAT) was found in 16.8% (n=23) of patients, of whom 16 (11.7%) had low-grade HAT and 7 (5.1%) had high-grade HAT or definite DRT. Complete sealing was associated with increased rates of low-grade HAT. CONCLUSIONS: LAA patency on CCTA follow-up is a frequent phenomenon due to malapposition of the proximal segment of the device lobe, PDL or fabric permeability. Patency was not associated with adverse outcomes. Low-grade HAT may be related to a benign, uneventful, endothelialisation process favoured by complete LAAO.

ABSTRACT

Introducción: La psicopatía se caracteriza por un patrón permanente de déficit afectivo y falta de respeto por los derechos de los otros y las normas sociales. La Lista de Chequeo de Psicopatía Revisada (PCL-R, por su sigla en inglés) es un instrumento de medición compuesto por 20 ítems y es uno de los más utilizados para investigación en psicopatía y trastorno de la personalidad antisocial. Objetivo: Validar la PCL-R en población colombiana carcelaria de sexo masculino. Método: Se hicieron dos traducciones al español y dos traducciones en sentido inverso de cada uno de los ítems. También, una traducción al español y en sentido inverso del instructivo. La validez de apariencia fue evaluada por un grupo de expertos. Se examinó la validez de contenido, consistencia interna, reproducibilidad prueba-reprueba e interevaluador. Resultados: En el análisis factorial para la validez de contenido se encontraron 4 dominios que explican el 61,1% de la varianza. La consistencia interna de la escala fue alta (alfa de Cronbach=0,94), al igual que la reproducibilidad pruebareprueba (CCI=0,83, IC 95%: 0,68-0,91) e interevaluador (CCI=0,92, IC 95% 0,86-0,96). Conclusiones: La versión en español de la PCL-R para la población carcelaria colombiana de sexo masculino muestra buenas propiedades psicométricas.


Introduction: Psychopathy is characterized by a pattern of constant affective deficit and lack of respect for other people's rights and social norms. The Psychopathy Check-List Revised (PCL-R) is an instrument of measurement composed of 20 items and it is one of the most used in psychopathy and antisocial personality disorder research. Objective: To validate the PCL-R in Colombian male inmates. Methods: Two translations into Spanish and two back-translations for each of the items were done. The instruction manual was also translated into Spanish and back-translated. Face validity was assessed by a group of experts. Content validity was examined as well as internal consistency, test-retest and inter-rater reproducibility. Results: In the factor analysis for the content validity, four domains were found which explained 61.1% of the variance. The internal consistency was high (Cronbach's Alpha=0.94). Similarly, the test-retest and inter-rater reproducibility were ICC=0.83, 95% CI: 0.68-0.91 and ICC=0.83, 95% CI: 0.86-0.96, respectively. Conclusions: The Spanish version of the PCL-R for Colombian male jail inmates shows good psychometric properties.

Introducción: el traumatismo craneoencefálico representa un grave problema de salud pública, ya que cada vez es mayor el número de personas que por diversas razones se ven afectadas (accidentes de tránsito, caídas, violencia, deportes), más en países como Colombia, donde las tasas de mortalidad y morbilidad son verdaderamente alarmantes. Una de las consecuencias más serias a las que se ven enfrentados tanto el paciente como sus cuidadores son las alteraciones neuropsiquiátricas, ya que impactan de gran manera el tratamiento, el proceso de rehabilitación y a sus cuidadores. Objetivo: intentar dar una visión resumida y práctica de los principales trastornos psiquiátricos después de un traumatismo craneoencefálico (pos-TEC), mencionando algunos elementos fi siopatológicos y de enfoque del tratamiento. Método: se revisan los principales trastornos neuropsiquiátricos pos-TEC, usando textos importantes, conocidos y recientes sobre el tema. Conclusiones: se ha identifi cado que los pacientes pos-TEC presentan anomalías neuropsicológicas y de neuroimágenes sutiles, pero signifi cativas que parecen estar en relación con los síntomas que se refi eren. Las terapias de orientación cognitiva han dado resultados favorables y, de hecho la educación temprana acerca de cómo afrontar las molestias posteriores al TEC disminuye de manera importante el desarrollo de este síndrome.


Introduction: Traumatic brain injury (TBI) is a serious public health problem, affecting more people everyday for various reasons (car accidents, falls, violence, sports), specially in countries like Colombia where morbimortality rates are truly alarming. The patient and caregivers are confronted with serious consequences such as neuropsychiatric disorders that impact heavily on the treatment, the rehabilitation process and the caregivers lives. Objective: To give a practical description of the main psychiatric disorders present after traumatic brain injuries together with available treatments, including also some of the physiopathological elements involved. Method: Review of current, well-known literature on the subject. Conclusions: Post-TBI patients exhibit subtle, although signifi cant, neuropsychological and neuroimaging anomalies, since they appear to be related to the symptoms patients describe. Cognitive oriented therapies have shown good results, and in fact, early education on how to manage discomfort following TBI diminishes the risk of presenting post-concussion syndrome.
