ABSTRACT

Introduction: We developed the intervention Programa Vida Ativa Melhorando a Saúde (VAMOS 3.0). This behavior change program aims to motivate people towards an active and healthy lifestyle. Objective: describe the development process of VAMOS 3.0 in print and online formats. Methods: A methodological study of content analysis, structure, implementation, and evaluation were carried out to develop a new version of VAMOS 3.0. Results: The new version had changes in content (short, direct, and affirmative sentences, images, color palette), structure (increased number of sections, time of application), and implementation (duration, form of participation, monitoring). In addition to the printed format, an online program was created containing the same characteristics to expand the reach. Final considerations: VAMOS 3.0 is suitable for public health in Brazil and has promising strategies for application in basic health units, as well as in other types of institutions such as schools, government offices, gyms, public or private companies.


Introducción: Desarrollamos la intervención Programa Vida Ativa Melhorando a Saúde (VAMOS 3.0). Este programa de cambio de comportamiento tiene como objetivo motivar a las personas hacia un estilo de vida activo y saludable. Objetivo: describir el proceso de desarrollo de VAMOS 3.0 en formato impreso y en línea. Métodos: Se realizó un estudio metodológico de análisis de contenido, estructura, implementación y evaluación para desarrollar una nueva versión de VAMOS 3.0. Resultados: La nueva versión tuvo cambios en contenido (oraciones cortas, directas y afirmativas, imágenes, paleta de colores), estructura (mayor número de secciones, tiempo de aplicación) e implementación (duración, forma de participación, seguimiento). Además del formato impreso, se creó un programa online con las mismas características para ampliar el alcance. Consideraciones finales: VAMOS 3.0 es adecuado para la salud pública en Brasil y tiene estrategias prometedoras para su aplicación en unidades básicas de salud, así como en otro tipo de instituciones como escuelas, oficinas gubernamentales, gimnasios, empresaspúblicas o privadas.


Introdução: Desenvolvemos a intervenção Programa Vida Ativa Melhorando a Saúde (VAMOS 3.0). Este programa de mudança de comportamento visa motivar as pessoas para um estilo de vida ativo e saudável. Objetivo: Descrever o processo de desenvolvimento do VAMOS 3.0 nos formatos impresso e online. Métodos: Foi realizado um estudo metodológico de análise de conteúdo, estrutura, implementação e avaliação para desenvolver uma nova versão do VAMOS 3.0. Resultados: A nova alteração teve no conteúdo (frases curtas, diretas e afirmativas, imagens, paleta de cores), estrutura (aumento do número de versões, tempo de aplicação) e implementação (duração, forma de participação, acompanhamento). Além do formato impresso, foi criado um programa online contendo as mesmas características para ampliar o alcance. Considerações finais: O VAMOS 3.0 é adequado para a saúde pública no Brasil e possui estratégias promissoras para aplicação em unidades básicas de saúde, bem como em outros tipos de instituições como escolas, repartições públicas, academias, empresas públicas ou privadas.

ABSTRACT

BACKGROUND: The presence of chronic conditions such as type 2 diabetes mellitus (T2DM) requires behavioral lifestyle changes mediated by individuals' motivation for change and adherence to treatment. This study aims to explore activation levels in individuals with T2DM treated in primary care facilities and to identify the association between demographic, clinical, psychosocial factors, and patient activation amongst populations in the Brazilian state of Amazonas. METHODS: SAPPA is a cross-sectional study conducted in Amazonas, approved by the Universidade Federal do Amazona's IRB in Brazil. Individuals with T2DM were evaluated in their homes (n = 4,318,325). The variables were sex, age, skin color, education level; health-related variables such as body mass index, nutritional behavior, and frequency of physical activity. Measures related to patient self-management behaviors over the past 6 months (Patient Activation Measure - PAM-13) were included in the survey. Descriptive and frequency data are presented as mean (standard deviation (SD)) or numeric percentage). Statistical testing was performed using IBM SPSS V.26, and a p-value of < 0.050 showed significance. Activation levels were dichotomized into low activation (Levels 1 and 2) and high activation (Levels 3 and 4). A multivariate linear model assessed the association between the PAM-13 score and the following variables: age, sex, BMI, skin color, number of comorbidities, burden of symptoms, and number of medications. RESULTS: Logistic regression analyses indicated a statistically significant association between sex, age, education, self-rated health, and general satisfaction with life. men were 43% more likely to score lower levels (p < 0.001). The results also indicated that advanced age had lower PAM levels (p < 0.001). Participants with fewer years of education were 44% more likely to have lower levels of PAM (p = 0.03). Worse self-rated health (p < 0.001) and lower general life satisfaction (p = 0.014) were associated with lower PAM levels. CONCLUSIONS: Low patient activation was associated with worse sociodemographic, health, and psychological conditions in the Amazon population. The low level of patient activation observed in this sample highlights an important impediment to diabetes disease management/self-management in disadvantaged populations.

ABSTRACT

Background: A conservative physiotherapy service development addressed to treat urinary incontinence for older women was studied using the RE-AIM (reach, effectiveness, adoption, implementation, and maintenance) framework. Design: We conducted a pragmatic case study design based on RE-AIM. Settings/participants: Included women ≥ 60 years of age, with self-reported UI symptoms. Results: A total of 34 older women were enrolled in the service with a mean age of 61.53 years. There was a significant improvement in the strength of the pelvic floor muscles, power, endurance, and fast contraction capacity after the intervention, however, it was observed a high dropout rate. Program implementation was supported by Physical Therapy teams who engaged in care coordination. The program has been maintained for over 4 years. Conclusion: Our findings demonstrate that UI patients would benefit from physiotherapy treatment and that this intervention is feasible. This RE-AIM evaluation provides lessons learned and strategies for future adoption, implementation, and maintenance of a Physical Therapy pelvic service.

BACKGROUND: Changes in the profiles of patients have significant impacts on the health care system. Diabetes mellitus type 2 (T2DM) prevention and management should be studied in different contexts. OBJECTIVE: The Study of Health in Primary Care for the Amazonas Population (SAPPA) primarily aims to describe T2DM prevention and management actions offered by primary health care settings in Brazil and whether the care delivered is consistent with the chronic care model (CCM). Second, the study aims to examine the impact of T2DM management actions on health and lifestyle, and third, to understand how sociodemographic characteristics, health, and subjective outcomes impact diabetes management. METHODS: As part of this observational study, managers and health professionals complete a questionnaire containing information about T2DM prevention and management actions and CCM dimensions. During in-home visits, patients are asked about their health, lifestyle, sociodemographics, diabetes care, and subjective variables. RESULTS: A total of 34 managers, 1560 professional health workers, and 955 patients will be recruited. The data collection will be completed in October 2022. CONCLUSIONS: The SAPPA is an observational study that intends to understand the T2DM management process in primary health care, including planning, execution, reach, and impact on patient motivation and adherence. ABSTRACT

Introduction: The effective translation of evidence-based interventions has contributed to implementing actions that impact public policies and the population's health. However, there is a gap in the literature regarding the factors associated with the successful implementation of these interventions. The Active Life Improving Health Behavior Change Program (BCP-VAMOS) uses behavioral strategies to promote an active and healthy lifestyle in the community. Characterized as a health innovation, it also provides health professionals with online training to implement the program in Primary Health Care (PHC). Our study describes a pragmatic trial that aims to evaluate the implementation of BCP-VAMOS, version 3.0, in PHC in southern Brazil. Methods and analysis: A pragmatic randomized controlled trial (PRCT) of two arms comparing a group of PHC professionals who will participate in a traditional didactic approach (control group) vs. a group that will receive ongoing support (intervention group) for the implementation of BCP-VAMOS. The intervention will be available to adults (≥18 years old) registered at PHC. Program recipient's will be assessed at baseline and post-intervention (9 months after) to measure markers of physical activity and eating behavior (primary outcomes). Program's implementation process will be monitored for 12 months and will be evaluated using the RE-AIM and Consolidated Framework for Implementation Research (CFIR) frameworks. Discussions: The survey findings can be used widely throughout Brazil, guiding the work of health professionals, service planners and policy-makers. Also, the results may help to inform the national health promotion policy to plan interventions and improve the implementation of programs in PHC. This research results will provide practical guidance for researchers to develop similar protocols to implement and adapt public health interventions. Ethics and dissemination: Ethics approval has been granted by the Human Research Ethics Committee of the Federal University of Santa Catarina (UFSC), Brazil, under no. 1394492. Results will be published in full as open access in the UFSC library and main trial results and associated papers in high-impact peer-reviewed journals. ABSTRACT

Actions for the prevention and management of type 2 diabetes (T2DM PM) are priorities within primary health care, from the dispensing of specific medications to non-pharmacological actions, such as physical activity and nutrition. The aim of the study was to describe prevention and man-agement of type 2 diabetes actions and their components in primary health settings in the State of Amazonas, Brazil. An observational study based on RE-AIM framework. Preventions and manage-ments actions approach were described by 5 dimensions: (R) reach, (E) effectiveness, (A) adoption, (I) implementation, and (M) maintenance. Data was collected in Primary Health Care Units from 6 cities from Amazonas, Brazil. The questionnaires were addressed to managers regarding current T2DM PM programs and their development. This study identified 17 different types of diabetes prevention and/or management actions currently implemented. Eleven actions included both pre-vention and management actions with 53.8% focusing on physical activity and nutrition orientation. However, zero actions reported any form of evaluation measuring changes in physical activity and nutrition behavior, and actions reported collecting feedback from participants, providers, and health professionals' workers. A total of 310 health professionals participated in diabetes actions, including 4 physical educational professionals and 3 nutritionists, with 100% of the efforts coordinated by nurses. Actions were based on traditional health education practices such as lectures and did not have practical activities that help to change and maintain healthy habits. Despite this, primary care plays a fundamental role in caring for users with chronic illnesses in cities in the interior of Amazonas


Ações de prevenção e controle da diabetes do tipo 2 (T2DM) são prioridades dentro da atenção primária de saúde, desde a dispensação de medicações específicas, às ações não medicamentosas como atividade física e nutrição. O objetivo foi caracterizar ações de prevenção e manejo (PM) e seus componentes na atenção primária no Estado do Amazonas, Brasil. Estudo observacional baseado no modelo RE-AIM. Estratégias adotadas em ações de PM foram descritas pelas 5 dimensões: (R) Alcance, (E) Efetividade, (A) Adoção, (I) Implementação e (M) Manutenção. Dados foram coletados em Unidades Básicas de Saúde em 6 cidades do Amazonas, Brasil. Os questionários endereçados aos gestores visavam ações de PM e seu desenvolvimento. Este estudo identificou 17 diferentes tipos de ações. Onze ações contemplaram ambas as estratégias de PM com 53,8% focadas em atividade física e orientações nutricionais. No entanto, nenhuma ação relatou mensuração para avaliação de mudanças no nível de atividade física e no comportamento nutricional. As ações baseiam suas avaliações em coleta de feedback dos participantes, equipe executora e profissionais de saúde. Ao todo, 310 profissionais de saúde participaram de ações, incluindo 4 profissionais de educação física e 3 nutricionistas, sendo 100% dos esforços coordenados por enfermeiros. As ações de PM da T2DM são baseadas em práticas tradicionais de educação em saúde como palestras e não possuem atividades práticas que auxiliem na mudança e manutenção de comportamentos saudáveis. Apesar disso, a atenção primária exerce um papel fundamental no cuidado ao usuário portador de doença crônica em municípios do interior do Amazonas

ABSTRACT

There is increasing debate about the importance of using information and communication technologies to strengthen health systems. Information systems are one of the most widespread technologies for this end in different healthcare contexts. The primary objective of this study was to highlight and discuss the main characteristics of the regulation and use of health information systems in Brazil and abroad. As a secondary objective, the main characteristics identified in Brazil were compared with the findings for other countries. A robust search strategy incorporated five electronic databases. The research question was defined using the SPIDER strategy, and thematic content analysis was applied. Fourteen articles composed the final sample, and three analytic categories were identified: "Use in the Context of Health Systems"; "Implications for Health Management"; and "Communication and Interoperability." The regulation and use of health information systems in different countries was directly related to their socioeconomic context. In Latin America and Africa, policy strengthening and implementation possibilities for health management were discussed. In the United States, Europe, and Asia, the discussions on interoperability between different services was emphasized. The complexity of the Brazilian health system leads to similarities in the data analysis with several countries from different regions and with distinct political configurations.

ABSTRACT

The "Programa Vida Ativa Melhorando a Saúde ­ VAMOS" (Active Life Improving Health) is a community intervention with a behavioral approach. It uses educational strategies to assist health professionals in support the population about the importance of adherence and maintenance of healthy lifestyles. This descriptive study shows the design and method of the VAMOS Program version 2.0. The intervention is for adults and older adults' users of Brazilian public health and aims to motivate them to adopt an active and healthy lifestyle regarding physical activity and eating. The program follows an intervention design that considers the dimensions of the RE-AIM framework. The VAMOS Program version 2.0 has the potential to be offered as a health promotion tool in the Brazilian public health system by using available resources and accessible and healthy behavioral strategies


O "Programa Vida Ativa Melhorando a Saúde ­ VAMOS" é uma intervenção comunitária com abordagem comportamental. Utiliza estratégias educativas para auxiliar os profissionais de saúde a orientar a população sobre a importância da adesão e manutenção de estilos de vida saudáveis. Este estudo descritivo apresenta o design e método do Programa VAMOS, versão 2.0. A intervenção é voltada para adultos e idosos usuários da saúde pública brasileira e visa motivá-los a adotarem um estilo de vida ativo e saudável no que diz respeito à atividade física e à alimentação. O programa segue um design de intervenção que considera as dimensões da ferramenta RE-AIM. O Programa VAMOS, versão 2.0, tem potencial para ser oferecido como ferramenta de promoção da saúde no sistema público de saúde brasileiro por meio da utilização de recursos disponíveis e acessíveis e estratégias comportamentais saudáveis

Background: Explored the role of public health centers in the delivery of physical activity programs to older Brazilians. Methods: Total of 114 older adults (81% women) from public health centers across the city of Florianopolis, Brazil, were randomized into three groups: behavior change group (n = 36), traditional exercise group (n = 52), and control group (n = 26). The behavioral change group included 12 weekly meetings (2 h each). The traditional exercise group offered a 12-week exercise class. Individuals in the control group participated only in measurements. Program evaluation included a mixed-methods approach following the RE-AIM framework (reach, effectiveness, adoption, implementation, and maintenance). Trained interviewers conducted 12 focus groups and 32 interviews with participants in the program, professionals delivering the programs, community health workers, and local and city administrators overseeing public health centers. Participants completed health, quality of life, and fitness assessments at four time points. Results: The study reached 11.5% of the eligible population in the community. Older adults' resistance to change and limited understanding of behavior change science by public health center staff hindered program reach. Physician encouraging patient participation and personal invitations by community health workers were perceived as favorable factors. Results of program effectiveness and maintenance suggest that behavior change strategies may be better suited than traditional exercise classes for decreasing sedentary time and increasing moderate-to-vigorous physical activity, as well as improving participants' quality of life. Only 14% of public health centers in the city adopted the programs. Heavy workload of health educators delivering the programs and limited physical space for program delivery were barriers for adoption. The fidelity of program delivery was high and indicates that the programs are culturally-appropriate for the Brazilian context and feasible for implementation by local health educators. Conclusions: Our findings support the potential for dissemination of behavior change and traditional exercise programs to older adults through public health centers in Brazil. ABSTRACT

This study aimed to evaluate the effectiveness of the VAMOS strategy (Health-Improving Active Life) in improving physical activity (PA), dietary habits, and anthropometric variables of primary health care (PHC) users in Brazil. A randomized controlled community trial was conducted at two units of the Health Academy Program (HAP; a service provided by PHC), which were randomly assigned either to control group (CG) or intervention group (IG). Participants in both groups (CG = 156 and IG = 135) took part in physical activity classes provided by HAP facilities with those in IG also participating in the VAMOS strategy for 12 weeks. This strategy is based on social cognitive theory. The main behavioral constructs addressed were self-efficacy, goal setting, self-monitoring, identification of social support and barriers, and solutions for the identified barriers. Physical activity was measured with accelerometers, and nutritional status was assessed using dietary habits questionnaires and anthropometric measurements. Intention-to-treat analysis revealed that participants in IG increased the daily time of moderate-vigorous PA and the frequency of raw vegetable intake, while the intake of ultra-processed foods was reduced. Weight loss among participants who were classified as overweight/obese at baseline was observed in the intervention group compared to control. These results suggest that the VAMOS strategy was effective in increasing PA, healthy dietary habits, and decreasing weight among PHC users. Therefore, VAMOS could be used as a strategy to strengthen individuals' autonomy regarding healthy choices and improve their overall health.

ABSTRACT

ABSTRACT Objective: to analyze the dimensions of the RE-AIM model from the perspective of telephone-based educational programs on diabetes, considering the experience of researchers from a public university in the state of São Paulo, Brazil. Method: this reflexive study presents an analysis of the dimensions of the RE-AIM model and the validity of its Brazilian version concerning the dimensions: reach, efficacy, adoption, implementation, and maintenance of results obtained by the study Telephone Support for the Follow-up of Diabetes Mellitus. Results: the analysis shows that the five dimensions provide information concerning factors that may affect the success of a program in clinical practice; that reaching the population remains a challenge; and that efforts should be made to advance on the efficacy and maintenance of educational interventions. Conclusion: this reflection, based on the experience of telephone-based educational interventions for diabetes, can contribute to advance Nursing science in terms of methodological guidance to support the development of interventions and programs at the various levels of health care.


RESUMEN Objetivo: analizar las dimensiones del modelo RE-AIM en la perspectiva de las intervenciones educativas por teléfono en diabetes, considerando la experiencia de los investigadores de una universidad pública paulista, Brasil. Método: reflexión de estudio que examinó las dimensiones del modelo RE-MAIN, válidos para el portugués de Brasil, en las dimensiones: alcance eficacia, adopción, implementación y mantenimiento de los resultados del estudio denominado soporte telefónico de Monitoreo de la Diabetes Mellitus. Resultados: los resultados del análisis mostraron que las cinco dimensiones proporcionan información sobre los factores que pueden afectar el éxito del programa en la práctica clínica y que el alcance de la población sigue siendo un desafío y se deben realizar esfuerzos para avanzar en la eficacia y el mantenimiento intervenciones educativas. Conclusión: se espera, con esa reflexión a la luz de la experiencia en intervenciones telefónicas educativas en diabetes, puede contribuir al avance de la ciencia Enfermería, en lo que se refiere a orientaciones metodológicas, que pueden auxiliar en la construcción y desarrollo de intervenciones y programas en los diversos niveles de atención a la salud.


RESUMO Objetivo: analisar as dimensões do modelo RE-AIM na perspectiva das intervenções educativas por telefone em diabetes, considerando a experiência dos pesquisadores de uma universidade pública paulista, Brasil. Método: estudo de reflexão que analisou as dimensões do modelo RE-MAIN, validade para o português do Brasil, nas dimensões: alcance eficácia, adoção, implementação e manutenção nos resultados obtidos do estudo denominado Apoio Telefônico no Monitoramento do Diabetes Mellitus. Resultados: os resultados da análise mostraram que as cinco dimensões fornecem informações sobre os fatores que podem afetar o sucesso do programa na prática clínica, sendo que o alcance da população ainda constitui um desafio, e esforços devem ser envidados para avançar na eficácia e manutenção das intervenções educativas. Conclusão: espera-se, com essa reflexão à luz da experiência em intervenções telefônicas educativas em diabetes, pode contribuir para o avanço da ciência Enfermagem, no que tange a orientações metodológicas, que podem auxiliar na construção e desenvolvimento de intervenções e programas nos diversos níveis de atenção à saúde.

ABSTRACT

The aim of the present study was to systematize and analyze the methodological quality of physical activity interventions in leisure time in obese adults. A systematic review was conducted between June and August 2017 through four electronic databases (PubMed, Lilacs, ScienceDirect and Web of Science). A total of two researchers selected articles and, through consensus, analyzed these articles using the RE-AIM framework, which aims to identify the following dimensions: reach, effective-ness, adoption, implementation and maintenance of interventions. Of the 2,334 articles selected, nine met all eligibility criteria. The mean duration of the interventions was 14 weeks, with strategies including mainly aerobic exercises and nutritional monitoring.When the individual studies were taken into consideration, 67% reported at least one category of each dimension. The dimension with the highest reported proportion was "reach", found in 65% of the interventions. The "effectiveness", "adoption" (organizational and staff ), "implementation" and "maintenance" dimensions (individual and organizational) were reported in less than 40% of the items investigated for this information. The conclusion is that most of intervention studies reported the "reach" dimension, while only a few described "maintenance". Thus, it is suggested that future studies should previously look at the pres-entation of the total effect of the intervention and the methods applied, offering detailed information about each procedure. The scientific and practical scenario will provide relevant information about construction, application and subsequent follow-up of the interventions in leisure-time physical ac-tivity for obese adults


O objetivo do estudo foi avaliar sistematicamente a qualidade metodológica de intervenções de atividade física no lazer para adultos com obesidade. Trata-se de uma revisão sistemática conduzida de junho a agosto de 2017, com consulta eletrônica em quatro bases de dados (PubMed, Lilacs, ScienceDirect e Web of Science). Dois investigadores selecionaram os artigos e, mediante ao consenso, analisaram os artigos pelo emprego da ferramenta RE-AIM, que se propõe a identificar cinco dimensões específicas: alcance, efetividade, adoção, implementação e manutenção. Dos 2.334 artigos selecionados, nove atenderam aos critérios de elegibilidade. A duração média das intervenções foi de 14 semanas, com estratégias que continham exercícios aeróbios e acompanhamento nutricional em sua maioria. A dimensão com maior proporção de indicadores reportados foi o alcance, representada em 65% das intervenções. As dimensões de efetividade, adoção (organizacional e staff ), implementação e manutenção (individual e organizacional) foram relatadas em menos de 40% dos indicadores. Um total de 67% dos estudos relatou, no mínimo, um indicador de cada dimensão. Conclui-se que as intervenções abordam com mais profundidade o alcance, ao passo que poucas descrevem informações sobre a manutenção. Dessa forma, sugere-se que estudos futuros atentem para a apresentação do efeito total da intervenção e os métodos utilizados, ofertando informações detalhadas sobre cada passo. Assim, o cenário científico e prático contará com importantes informações sobre a construção, a aplicação e o acompanhamento a posteriori das intervenções em atividade física no lazer para adultos com obesidade

Para verificar o alcance das intervenções em atividade física (IAF) na saúde pública de Santa Cata-rina (SC), quanto ao porte dos municípios, ao vínculo com as estratégias de promoção de atividade física, aos grupos populacionais atendidos e as características de participação, foram questionados 146 coordenadores de IAF das Secretarias Municipais de Saúde de SC. O alcance foi calculado dividindo o número de participantes nas IAF pela população total e estratificada, e pelos grupos etários (crianças, adolescentes, adultos e idosos). O alcance absoluto das IAF foi de 3,8% e o relativo de 4,9%. Os maiores valores de alcance absoluto e relativo foram da população idosa (8,4% e 8,8%, respectivamente) e os vinculados ao programa Academia da Saúde, em todos os grupos populacionais atendidos (5% e 7%, respectivamente). Das IAF, 56,3% atendem menos de 100 usuários, com maior participação do público adulto e idoso. Destaca-se o agente comunitário de saúde como a estratégia de recrutamento dos participantes para as IAF (95,3%) e "ter indicação ou autorização do médico" como critério para participação (79,2%). Conclui-se que há um baixo alcance da população nas IAF das Secretarias Municipais de Saúde de SC, tornando-se necessário repensar os seus programas


In order to verify the reach of Physical Activity Interventions (PAI) in the public health of Santa Catarina, the size of municipalities, the link with physical activity promotion strategies, population groups served and characteristics of the participants, 146 PAI coordinators of the Municipal Health Secretariats of Santa Catarina were questioned. The reach was calculated by dividing the number of participants in PAI by the total and stratified population and by the age groups (children, adolescents, adults and the elderly). The PAI absolute reach was 3,8% and the relative 4,9%. The highest values of absolute and relative reach were among the older adults (8.4% and 8.8%, respectively) and those linked to the Health Academy program, in all populations groups served (5% and 7%, respectively). About 56.3% of the IAF serve less than 100 people, with greater participation of the adult and elderly public. Community agent was the most prevalent form of recruitment (95,3%) and "have physician's indication or authorization" as a criterion for participation (79.2%). It is concluded that there is a low population reach in the Municipal Health Secretariat's PAI of Santa Catarina, and it is necessary to rethink its programs

ABSTRACT

O objetivo foi traduzir e adaptar os itens de codificação relacionados às dimensões do modelo RE-AIM para auxiliar no planejamento e avaliação de programas de promoção da atividade física e saúde, bem como na análise da qualidade do relato em estudos de revisão sistemática. Foram realizadas equivalência conceitual, de itens e semântica por profissionais de saúde mono e bilíngues. Na análise conceitual e de itens foi realizada discussão entre especialistas e na semântica, tradução inversa (inglês para português) e reversa (português para inglês) dos itens. Posteriormente, foi avaliada a equivalência entre as retraduções e o conteúdo original. Dessa forma, foi apresentada a tradução e adaptação dos 54 itens que permite o planejamento e avaliação de programas e dos 21 itens para análise da qualidade das informações relatadas em evidências científicas. Espera-se contribuir para o planeja-mento e avaliação de programas ou revisões sistemáticas para promoção da atividade física e saúde


The goal was to translate and adapt the coding items related to the dimensions of the RE-AIM model to assist in the planning and evaluation of physical activity and health promotion programs as well as in the analysis of the quality of the report in systematic review studies. Mono and bilingual health professionals carried out conceptual, item and semantic equivalence. In the conceptual and item analysis specialists carried out a discussion and in the semantic one, an inverse (English to Portuguese) and a reverse (Portuguese to English) translation of the items were performed. Posteriorly, the equivalence between the retranslations and the original content was evaluated. Therefore, the translation and adaptation of the 54 items that allow the planning and evaluation of the programs were presented as well as the translation and adaptation of the 21 items necessary to the analysis of the quality of the information reported in scientific evidence. It is expected to contribute to programs planning and evaluation or their systematic review aiming at physical activity and health promotion

ABSTRACT

Foi realizada uma revisão sistemática com o objetivo de avaliar em que medida programas de mudança de comportamento com ênfase na atividade física e/ou alimentação saudável em adultos e/ou idosos brasileiros reportam fatores relacionados à validade interna e externa utilizando a ferramenta RE-AIM. Foram selecionados 26 estudos nas bases Medline/Pubmed, Lilacs, Science Direct, Web Of Science, Scopus, PsycINFO e Google Acadêmico, considerando como critérios de elegibilidade, estudos de intervenção com a população de 18 anos ou mais, de ambos os sexos, publicados em periódicos, em português ou inglês e que apresentassem como desfechos primários a mudança de comportamento para a atividade física e/ou alimentação saudável. Para codificar os estudos foi utilizado o protocolo de 21 itens da ferramenta RE-AIM, que avalia o impacto de programas e intervenções na área da saúde por meio de cinco dimensões:alcance, efetividade/eficácia, adoção, implementação e manutenção.Os estudos atenderam a 61% (n= 16) dos itens validados para o alcance, 40% (n= 11) para efetividade/eficácia, 17% (n= 4) para implementação, 26% (n= 7) para adoção e 9% (n= 12) para manutenção. Nenhum estudo atendeu todos os 21 itens das cinco dimensões do RE-AIM e 53,8% (n= 14) apresentaram baixa qualidade. Apesar do aumento na quantidade de programas de intervenção de mudança de comportamento com ênfase na atividade física e/ou alimentação saudável, poucos estudos atendem aos itens das dimensões do RE-AIM, impactando diretamente na validade interna e externa.


A systematic review was carried out to evaluate the extent to which behavioral change programs with emphasis on physical activity and/or healthy eating in Brazilian adults and/or elderly people report factors related to internal and external validity using the RE-AIM framework. Twenty six studies were selected from the Medline/Pubmed, Lilacs, Science Direct, Web Of Science, Scopus, PsycINFO and Google Academic bases, considering as eligibility criteria, intervention studies with the population of 18 years and older, of both sexes, published in Portuguese or English Presented as primary outcomes the behavior change for physical activity and/or healthy eating. To codify the studies, the 21-items RE-AIM framework protocol was used to evaluate the impact of programs and interventions in the health area through five dimensions: outreach, effectiveness/efficiency, adoption, implementation and maintenance. The studies complied with 61% (n= 16) of the items validated for reach, 40% (n= 11) for effectiveness/efficiency, 17% (n= 4) for implementation, 26% (n= 7) for adoption and 9% (n= 12) for maintenance. No study answered all 21 items of the five RE-AIM dimensions and 53.8% (n= 14) presented poor quality. Despite the increase in the number of behavioral intervention programs with emphasis on physical activity and / or healthy eating, few studies address the items of RE-AIM dimensions, directly impacting internal and external validity.

Objective: the purpose of this study was to evaluate the efficacy of telephone-based support for the metabolic control of elderly patients with diabetes mellitus. Method: a pragmatic study was conducted in two groups, called G1 (n=36) and G2 (n=27), at a health unit from the countryside of São Paulo state. Patients in G1 group received telephone support over four months, through 16 telephone contacts with educational material; for the G2 group the educational material was mailed. Results: significant differences were found. The G1 group showed a reduction of the parameters of fasting glucose, as well as systolic and diastolic blood pressure. In G2 group a modest reduction was noted in some parameters, with no significant difference. Conclusion: telephone support was effective to deliver patient education to the diabetic elderly, leading to the reduction of fasting blood glucose. This, combined with other strategies, can contribute to reduce glycated hemoglobin (NCT 01972412).

RESUMEN Objetivo: evaluar la efectividad del soporte telefónico en el control metabólico de ancianos con diabetes mellitus. Método: estudio pragmático con 63 participantes, divididos en dos grupos denominados G1(n=36) y G2(n=27), en una unidad de salud del interior paulista. El soporte telefónico fue ofrecido durante cuatro meses para el G1, consistiendo en 16 llamadas telefónicas de contenido educativo, y para el G2, se realizaron envíos por vía postal. Resultados: en el G1 hubo significatividad estadística en la reducción de los parámetros de las variables glucemia en ayunas, presión arterial sistólica y diastólica. En el G2 hubo discreta reducción en algunas variables, pero sin significatividad estadística. Conclusión: el soporte telefónico fue considerado como estrategia educativa efectiva para ancianos con diabetes mellitus y favoreció la reducción de la glucemia en ayunas. En conjunto con otras estrategias, puede agregar valor a la reducción de hemoglobina glicosilada (NCT 01972412).

BACKGROUND: This study aimed to analyze the effect of a behavior change program, called Vida Ativa Melhorando a Saúde (VAMOS), on physical activity, eating habits, and quality of life in patients with hypertension. METHODS: A randomized controlled trial was carried out in 90 patients with hypertension (57.8 ± 9.9 y). They were randomly assigned to 2 groups: VAMOS group (n = 45) and control group (n = 45). The VAMOS group participated in a behavioral change program aimed at motivating changes in physical activity and nutrition behavior for 12 weeks. Physical activity, eating habits, quality of life, self-efficacy, and social support were evaluated at preintervention and postintervention. RESULTS: The control group increased sedentary time (407 ± 87 vs 303 ± 100 min/d; P < .05) and sedentary bouts (434 ± 86 vs 336 ± 98 min/d; P < .05) and reduced total physical activity (553 ± 87 vs 526 ± 86 min/d; P < .05). The VAMOS group improved the general healthy eating habits score (36.9 ± 6.6 vs 43.4 ± 5.8; P < .05) and quality of life (44% vs 92%; P < .05). CONCLUSION: The VAMOS program was effective in improving eating habits and quality of life in patients with hypertension.

Objetivo: validar o instrumento Patient Assessment of Chronic Illness Care (PACIC) em pacientes diabéticos no Brasil. Métodos: Trata-se de um estudo metodológico que incluiu 85 pacientes com diabetes tipo 2 no interior paulista. A análise fatorial utilizou-se o método com rotação ortogonal Varimax. Para a análise de correspondência utilizou-se o teste de qui-quadrado. A consistência interna foi verificada pelo alfa de Cronbach. Resultados: O instrumento apresentou alfa de Cronbach de 0,84, sendo que o terceiro domínio Estabelecimento de Metas/Adaptação apresentou o menor valor de alfa de Cronbach 0,78. Quanto à correspondência, o questionário separado por domínios, mostrou que as respostas aos itens sobre questionamento da opinião do paciente sobre o tratamento, receber uma cópia do tratamento, o profissional entrar em contato após a consulta e o encaminhamento ao profissional nutricionista foram avaliados como nunca realizados. Na análise fatorial houve a retenção de cinco fatores. Conclusões: A versão brasileira do PACIC é válida e reprodutível com diabéticos brasileiros e pode auxiliar na avaliação da qualidade da assistência de pessoas com condições crônicas.


Objective: To validate the Instrument Patient Assessment of Chronic Illness Care (PACIC) in diabetic patients in Brazil. Methods: This is a methodological study that included 85 patients with type 2 diabetes in the interior of São Paulo. The factorial analysis was the Varimax orthogonal rotation method. For the correspondence analysis, it was used chi-square test. The internal consistency was verified by Cronbach's alpha. Results: The instrument presented Cronbach's alpha of 0.84, and the third Goal Setting / Adaptation domain presented the lowest alpha value of Cronbach 0.78. Regarding the correspondence, the questionnaire separated by domains, showed that the answers to the items about questioning the patient's opinion about the treatment, receiving a copy of the treatment, the professional getting in touch after the consultation and the referral to the professional nutritionist were evaluated as never performed. In the factorial analysis, five factors were retained. Conclusions: The Brazilian version of the PACIC is valid and reproducible with Brazilian diabetics and can help assess the quality of care of people with chronic conditions.

ABSTRACT

The RE-AIM framework is a model that analyzes both the external and internal validity of health programs. It proposes that programs should be evaluated based on five key dimensions: reach, effectiveness/efficiency, adoption, implementation and maintenance. The aim of the present study was to conduct a systematic review of physical activity programs for elderly persons published in Brazilian scientific literature using the RE-AIM framework. A total of 26 articles included in four scientific databases from 1993 to 2013 were identified and coded using a validated 52-item RE-AIM abstraction tool. Using the RE-AIM dimensions, indicators of reach (47.2%), effectiveness (36.3%), and implementation (27.5%) were reported more often than indicators of adoption (6.4%) and maintenance (4.7%). Only two articles were included in all five RE-AIM dimensions. Despite the increasing number of interventions targeting the elderly, few address the dimensions proposed by the RE-AIM framework, reducing their potential generalizability outside their original settings, and their overall external validity.


O modelo RE-AIM é uma proposta de avaliação que analisa tanto a validade externa quanto a validade interna de programas de saúde. Trata-se de um modelo que sugere avaliar qualquer programa nas dimensões: alcance, efetividade/eficácia, adoção, implementação emanutenção. O presente estudo teve como objetivo realizar uma revisão sistemática com base no modelo RE-AIM dos artigos científicos brasileiros que utilizaram programas regulares de atividade física para idosos. Foram selecionados 26 artigos publicados em quatro bases de dados científicas (PubMed, BVS, SciELO e Scopus) no período de 1993 a 2013, identificados pela codificação do modelo RE-AIM. Os resultados indicaram maior proporção de relatos dos domínios alcance (47,2%),efetividade/eficácia (36,3%) eimplementação (27,5%) em contraposição aos domíniosadoção (6,4%) e manutenção (4,7%). Apenas dois artigos atenderam a todos os domínios do RE-AIM. Apesar do crescente aumento do número de intervenções direcionadas ao público idoso, poucas conseguem atender às dimensões propostas pelo modelo RE-AIM, reduzindo assim a validade externa dos estudos.
