ABSTRACT

With the rapid development of the backbone network rates, there has been a gradual increase in channel spacing and bandwidth. The C&L band ultra-broad bandwidth array waveguide gratings (AWG) of 60-channel 100 GHz channel spacing are designed and fabricated based on silica waveguide. A new parabolic design is used to achieve ultra-broad bandwidth and good spectrum. For the C band ultra-broad bandwidth AWG, the peak insertion loss, uniformity, 0.5 dB bandwidth, 1 dB bandwidth and 3 dB bandwidth are 2.98 dB, 0.36 dB, 0.614 nm, 0.721 nm and 0.937 nm, respectively. For the L band ultra-broad bandwidth AWG, the peak insertion loss, uniformity, 0.5 dB bandwidth, 1 dB bandwidth and 3 dB bandwidth are 2.91 dB, 0.27 dB, 0.560 nm, 0.665 nm and 0.879 nm, respectively. To ensure ultra-broad bandwidth AWG operation at different temperatures, a temperature control circuit is integrated into the packaging design. It has been observed that the performances remain virtually unchanged within the temperature range of -15 to 65 degree. The ultra-broadband AWGs have been successfully tested to transmit 96 Gbaud signals and can be applied to 600 G/800 G backbone network transmission. By using the C&L ultra-broad bandwidth AWGs of 60-channel 100 GHz channel spacing, the total transmission speed over a single-mode fiber can reach 72Tbps/96Tbps.

ABSTRACT

OBJECTIVE: To compare the curative effect between interactive scalp acupuncture and traditional scalp acupuncture on hemiplegic upper extremity motor dysfunction in the patients with ischemic stroke. METHODS: Seventy cases of hemiplegic upper extremity motor dysfunction of ischemic stroke were randomly divided into an interactive scalp acupuncture group (35 cases, 1 case breaked off) and a traditional scalp acupuncture group (35 cases, 1 case dropped off). The patients of the two groups received the secondary prevention medication and routine rehabilitation therapy. Besides, in the interactive scalp acupuncture group, the upper extremity occupational therapy was operated during the needle retaining of scalp acupuncture; and in the traditional scalp acupuncture group, the upper extremity occupational therapy was delivered after the completion of scalp acupuncture. The same points were selected in the two groups such as Fuxiang head area, Fuxiang upper-limb-shoulder point, Fuxiang upper-limb-elbow point and Fuxiang upper-limb-wrist point. The needles were inserted perpendicularly by flying-needle technique and manipulated by triple technique of gentle twisting, heavy pressure and vibrating. The needles were retained for 30 min. Based on the degree of the upper extremity motor impairment, the regimen of the upper extremity occupational therapy was formulated individually and one treatment took 30 min. In the two groups, the therapies were delivered once daily, 5 times a week, lasting 4 weeks. Before and after treatment, the scores of Fugl-Meyer assessment of upper extremity (FMA-UE), Wolf motor function test (WMFT), the modified Barthel index (MBI) and the modified Ashworth scale (MAS) grade in the two groups were observed before and after treatment. RESULTS: After treatment, the scores of FMA-UE, WMFT and MBI were higher than those before treatment (P<0.01), and MAS grade was improved (P<0.05) in the two groups. The scores of FMA-UE, WMFT and MBI in the interactive scalp acupuncture group were higher than those in the traditional scalp acupuncture group (P<0.01, P<0.05), and there was no statistical significance in the difference of MAS grade between the two groups (P>0.05). CONCLUSION: The interactive scalp acupuncture can effectively improve the motor function of the hemiplegic upper extremities and the activities of daily living in the patients with ischemic stroke and its efficacy is better than traditional scalp acupuncture. But these two types of scalp acupuncture obtain the similar effect on spasticity.

BACKGROUND: Hyperuricemia is an important pathological basis of gout and a distinct hazard factor for metabolic syndromes and cardiovascular and chronic renal disease, but lacks safe and effective treatments currently. Paeonia × suffruticosa Andrews leaf effectively reduced serum uric acid in gout patients; however, the material foundation and the mechanism remain unclear. PURPOSE: To determine the primary active components and mechanism of P. suffruticosa leaf in hyperuricemic mice. METHODS: The chemical constituents of P. suffruticosa leaf was identified using high-performance liquid chromatographic analysis. The anti-hyperuricemic activity of P. suffruticosa leaf extract (12.5, 25, 50, 100, and 200 mg/kg) and its components was evaluated in hyperuricemic mice induced by a high purine diet for 14 days. Then, the urate-lowering effects of apigenin 7-O-glucoside (0.09, 0.18, and 0.36 mg/kg) were assessed in another hyperuricemic mice model built by administrating potassium oxonate and adenine for 4 weeks. The inhibitory effect of apigenin 7-O-glucoside on uric acid production was elucidated by investigating xanthine oxidase activity in vitro and in serum and the liver and through molecular docking. Immunofluorescence and western blot analyses of the expression of renal urate transporter 1 (URAT1), glucose transporter 9 (GLUT9), organic anion transporters 1 (OAT1), and ATP-binding cassette G member 2 (ABCG2) proteins elucidated how apigenin 7-O-glucoside promoted uric acid excretion. RESULTS: Six compounds were identified in P. suffruticosa leaf: gallic acid, methyl gallate, oxypaeoniflorin, paeoniflorin, galloylpaeoniflorin, and apigenin 7-O-glucoside. P. suffruticosa leaf extract significantly attenuated increased serum uric acid, creatinine, and xanthine oxidase activity in hyperuricemic mice. Apigenin 7-O-glucoside from P. suffruticosa leaf reduced uric acid, creatinine, and malondialdehyde serum levels, increased superoxide dismutase activity, and partially restored the spleen coefficient in hyperuricemic mice. Apigenin 7-O-glucoside inhibited xanthine oxidase activity in vitro and decreased serum and liver xanthine oxidase activity and liver xanthine oxidase protein expression in hyperuricemic mice. Molecular docking revealed that apigenin 7-O-glucoside bound to xanthine oxidase. Apigenin 7-O-glucoside facilitated uric acid excretion by modulating the renal urate transporters URAT1, GLUT9, OAT1, and ABCG2. Apigenin 7-O-glucoside protected against renal damage and oxidative stress caused by hyperuricemia by reducing serum creatinine, blood urea nitrogen, malondialdehyde, and renal reactive oxygen species levels; increasing serum and renal superoxide dismutase activity; restoring the renal coefficient; and reducing renal pathological injury. CONCLUSION: Apigenin 7-O-glucoside is the main urate-lowering active component of P. suffruticosa leaf extract in the hyperuricemic mice. It suppressed liver xanthine oxidase activity to decrease uric acid synthesis and modulated renal urate transporters to stimulate uric acid excretion, alleviating kidney damage caused by hyperuricemia.

ABSTRACT

A miniature Fourier transform spectrometer is proposed using a thin-film lithium niobate electro-optical modulator instead of the conventional modulator made by titanium diffusion in lithium niobate. The modulator was fabricated by a contact lithography process, and its voltage-length and optical waveguide loss were 2.26 V·cm and 1.01 dB/cm, respectively. Based on the wavelength dispersion of the half-wave voltage of the fabricated modulator, the emission spectrum of the input signal was retrieved by Fourier transform processing of the interferogram, and the analysis of the experimental data of monochromatic light shows that the proposed miniaturized FTS can effectively identify the input signal wavelength.

ABSTRACT

Countless waveguides have been designed based on four basic bends: circular bend, sine/cosine bend, Euler bend (developed in 1744) and Bezier bend (developed in 1962). This paper proposes an n-adjustable (NA) bend, which has superior properties compared to other basic bends. Simulations and experiments indicate that the NA bends can show lower losses than other basic bends by adjusting n values. The circular bend and Euler bend are special cases of the proposed NA bend as n equals 0 and 1, respectively. The proposed bend are promising candidates for low-loss compact photonic integrated circuits.

ABSTRACT

A 90° optical hybrid (OH) based on a 4×4 multimode interference (MMI) coupler is characterized and analyzed using the silica-based planar light circuit. The OH is designed to be polarization-insensitive with an extremely low polarization-dependent loss (<0.18d B) and phase error (w i t h i n±1.3∘). The fabricated chip exhibits an excess loss of 3.4 dB and 3.8 dB for TE and TM polarizations at 1550 nm. The measured phase error is w i t h i n±7.2∘ over 50 nm in the C+L band with a polarization-dependent error w i t h i n±3.5∘. The fabrication tolerance of the MMI width and waveguide sidewall angle is analyzed, which can well explain the deviation from the theoretical calculation.

ABSTRACT

A multi-period-delayed feedback (MPDF) photonic circuit constructed by a Sagnac ring and two coupled rings was designed. By coupling a distributed feedback (DFB) laser diode (LD) with the MPDF, a narrow linewidth semiconductor laser was demonstrated. The linewidth of the DFB-LD with MPDF was narrowed to be around 2 kHz, which is reduced by three orders of magnitude, and the linewidth reduction capability could be maintained when the wavelength of the DFB-LD was tuned in a range wider than 3 nm. The laser frequency stability can also be improved using the proposed technique, and the frequency fluctuation was reduced for nearly 8 times in comparison with the DFB-LD.

ABSTRACT

OBJECTIVE: To observe the efficacy of Jiannao Anshen acupuncture (acupuncture for brain-invigorating and mind-calming) on insomnia, and the dynamic changes of functional magnetic resonance imaging (fMRI) before and after acupuncture, to provide neuroimaging basis of acupuncture for insomnia. METHODS: A total of 58 participants were included. Of them, 29 patients with insomnia were included into an observation group, and 29 healthy participants were included into a control group. The patients in the observation group were treated with Jiannao Anshen acupuncture at Baihui (GV 20), bilateral Sishencong (EX-HN 1), Fengfu (GV 16), Fengchi (GB 20), Qiangjian (GV 18), occipital sideline, Anmian (Extra), Yiming (EX-HN 14) and Dazhui (GV 14); treatment was given once a day, 10 days as a course of treatment, and a total of 2 courses of treatment was given. The resting-state fMRI data was collected in the observation group before and after treatment. The participants in the control group received no treatment, and the resting-state fMRI data was collected only once. The resting-state fMRI data in the observation group before treatment and in the control group, also the observation group before and after treatment were compared. The Pittsburgh sleep quality index (PSQI), insomnia severity index (ISI), daytime insomnia symptom scale (DISS), hyperarousal scale (HAS), Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD), digit symbol test, mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA) and the A, B speed of trail making test (NCT) were observed in the observation group before and after treatment. The clinical efficacy of the observation group was evaluated. RESULTS: The total effective rate was 89.7% (26/29) in the observation group. In the observation group, the scores of PSQI, ISI and DISS, the A, B speed of NCT were all decreased after treatment (P<0.01, P<0.05). Before treatment, compared with the control group, functional connectivity in ventral dorsal prefrontal cortex, dorsal anterior cingulate cortex, inferior parietal lobe, dorsal lateral prefrontal cortex, dorsal anterior cingulate cortex, left ventral anterior cingulate gyrus and right ventral anterior cingulate gyrus was reduced in the observation group; compared before treatment, the functional connectivity in lingual gyrus, cerebellar area 6, frontal lobe, insular lobe and anterior cingulate gyrus, premotor gyrus and posterior cingulate gyrus was increased after treatment in the observation group. After treatment, the functional connection intensity of left ventral caudate nucleus and insular lobe was negatively correlated with HAMD score in the observation group (r =-0.55, P<0.05), and the functional connection intensity of left ventral caudate nucleus and anterior cingulate gyrus was negatively correlated with HAMA score in the observation group (r =-0.47, P<0.05). CONCLUSION: The Jiannao Anshen acupuncture could effectively improve the sleep quality in patients with insomnia, and acupuncture could enhance the connection of left dorsal, right ventral and left ventral caudate nucleus with brain area.

ABSTRACT

Benign prostatic hyperplasia (BPH) is a frequently encountered disease in older men that affects sexual function and is capable of causing lower urinary tract dysfunction. Unfortunately, current treatment options for BPH primarily seek to address the lower urinary tract dysfunction aspect of the disease and do not improve sexual function. Yohimbine has been effectively used for decades to treat erectile dysfunction. Therefore, the objective of this study was to evaluate the inhibitory effect of yohimbine on BPH and explore the associated underlying mechanisms. Thirty-six rats were randomly divided into the control, BPH, finasteride (1 mg/kg), and yohimbine (2, 4, and 8 mg/kg) groups. Except for the rats in the control group, those in the other groups were subcutaneously injected testosterone propionate (5 mg/kg/day) daily for a period of 4 weeks to establish BPH models. They were also administration the corresponding drug daily for a period of 6 weeks. After the treatments, in addition to determining prostate wet weight and index, the histopathological status of the prostate was observed, and the levels of testosterone, dihydrotestosterone, prostatic acid phosphatase, the prostate-specific antigen, proliferating cell nuclear antigen, and steroid 5α-reductase were determined. Specifically, the administration of 2, 4, and 8 mg/kg yohimbine inhibited prostatic index increase by 46.7, 55.1, and 69.3%, respectively, in BHP rats. Further, yohimbine significantly reduced the levels of dihydrotestosterone, prostatic acid phosphatase, prostate-specific antigen, proliferating cell nuclear antigen, and steroid 5α-reductase, suggesting that it exerts beneficial effects against BPH by modulating the steroid 5α-reductase pathway.

ABSTRACT

Silicon-based devices offer great potential for quantum key distribution (QKD) with the benefits of miniaturization, scalability, complexity, and improved performance. Based on the planar lightwave circuit technology, a 200 ps delay-time asymmetric Mach-Zehnder interferometer (AMZI) decoding chip was designed and fabricated that can adjust the power ratio of the two arms by introducing a variable beam splitter. The measured delay time is approximately 200 ps, and the two output pulses' peaks can be balanced by loading an appropriate voltage on the variable beam splitter. The classical interference visibility of the AMZI versus temperature was studied, and it is highly temperature dependent. The interference visibility can reach as high as 99.72% under appropriate temperature control. This AMZI can act as a passive decoder in fiber-based QKD systems.

ABSTRACT

Professor HUANG Lin-na's experience in treatment of Tourette syndrome with Fang's scalp acupuncture were introduced. Professor HUANG believes that this disease is in category of tremor in TCM. Pathogenic wind is the key in its pathogenesis and liver, spleen and kidney are closely related. Hence, the treatment focuses on eliminating pathogenic wind and regulating the function of liver, spleen and kidney. The cerebrum is the regulation center in human body. The abnormal somatic function can be adjusted by acupuncture at the scalp area where the cerebral cortical function is projected. Fang's scalp acupuncture is suitable in treatment of Tourette syndrome. Such scalp acupuncture was introduced in this paper with the typical case reported so as to provide a new approach to clinical treatment.

ABSTRACT

In this paper, electro-absorption variable optical attenuators (VOAs) have been designed and fabricated on silicon-on-insulator material with a 3 µm thick top silicon layer. We have simulated the impact of doping distance, doping width, doping depth, doping concentration, and doping length on VOA's power consumption at 20 dB attenuation. We have found that the power consumption of VOA at 20 dB attenuation will decrease as the doping distance and doping width get smaller, while the doping depth and doping concentration get larger. We have further demonstrated that the power consumption can also be reduced by increasing the doping length and by taking a series structure theoretically. In this paper, VOAs with different doping structures, doping distances, doping widths, and doping lengths have been fabricated, and the tested results have verified our theoretical analysis. With doping distance of 8 µm, doping width of 15 µm, doping length of 1 cm, doping concentration of 1×1019 cm-3, and doping depth of 0.9 µm, the power consumption of the series VOA at 20 dB attenuation is 470 mW, and the rising/falling time is 54 ns/49.5 ns.

ABSTRACT

We report two-photon interferences on a silica-on-silicon chip of Mach-Zehnder interferometer using telecommunication-band correlated photon pairs. The photon pairs were generated by spontaneous four-waving mixing process in a dispersion-shifted fiber. The integrated chip, which was fabricated by standard silica-on-silicon planar lightwave circuit technology, contained a Mach-Zehnder interferometer with a thermo-optic phase shifter. The insertion loss of the interferometer was less than 1 dB. We demonstrated two-photon interferences with both degenerate- and non-degenerate-frequency photon pairs on the Mach-Zehnder interferometer chip. A high fringe visibility was achieved in the interference with nondegenerate-frequency photons. Properties of quantum interference were demonstrated in the interference with degenerate-frequency photon pairs, which is an important way to manipulate the quantum state. These results show great potential of silica-on-silicon photonic chips in applications for the fiber-chip scheme in quantum networks.

BACKGROUND: Hand hygiene (HH) is an essential component for preventing and controlling of healthcare-associated infection (HAI), whereas compliance with HH among health care workers (HCWs) is frequently poor. This study aimed to assess compliance and correctness with HH before and after the implementation of a multimodal HH improvement strategy launched by the World Health Organization (WHO). METHODS: A quasi-experimental study design including questionnaire survey generalizing possible factors affecting HH behaviors of HCWs and direct observation method was used to evaluate the effectiveness of WHO multimodal HH strategy in a hospital of Traditional Chinese Medicine. Multimodal HH improvement strategy was drawn up according to the results of questionnaire survey. Compliance and correctness with HH among HCWs were compared before and after intervention. Also HH practices for different indications based on WHO "My Five Moments for Hand Hygiene" were recorded. RESULTS: In total, 553 HCWs participated in the questionnaire survey and multimodal HH improvement strategy was developed based on individual, environment and management levels. A total of 5044 observations in 23 wards were recorded in this investigation. The rate of compliance and correctness with HH improved from 66.27% and 47.75% at baseline to 80.53% and 88.35% after intervention. Doctors seemed to have better compliance with HH after intervention (84.04%) than nurses and other HCWs (81.07% and 69.42%, respectively). When stratified by indication, compliance with HH improved for all indications after intervention (P < 0.05) except for "after body fluid exposure risk" and "after touching patient surroundings". CONCLUSION: Implementing the WHO multimodal HH strategy can significantly improve HH compliance and correctness among HCWs.

ABSTRACT

BACKGROUND: Previous research has shown that electroacupuncture therapy has a potential therapeutic effect for simple female stress urinary incontinence. In this study, pelvic floor muscle training, the first-line treatment for stress urinary incontinence in women based on meta-analysis of numerous randomized control trials and recommended by international clinical practice, is used as a control group to demonstrate whether electroacupuncture therapy is a better method for female stress urinary incontinence. METHODS/DESIGN: A randomized controlled trial has been designed to evaluate the therapeutic benefit of electroacupuncture for female stress urinary incontinence compared with pelvic floor muscle training. The safety of electroacupuncture and patient compliance will also be evaluated. Untoward reaction to the electroacupuncture, including a broken needle, fainting on acupuncture, or pain during acupuncture, will be recorded and the therapy will be stopped if an untoward reaction occurs. After we have received full ethical approval and patient consent, participants will be randomized to receive a series of 24 electroacupuncture or pelvic floor muscle training interventions. The frequency and amount of leakage will be measured as the primary outcome parameters. Secondary outcome parameters include the 1-hour pad test, the short-form of the International Consultation on Incontinence Questionnaire, patient subjective effectiveness evaluation, weekly usage of pad, and usage of specialty therapy for female stress urinary incontinence. DISCUSSION: This trial will help to determine whether electroacupuncture is a more effective treatment than pelvic floor muscle training for patients with female stress urinary incontinence. OBJECTIVE: To observe the effect of scalp-acupuncture intervention on the expression of Bcl-2 of astrocytes in the hippocampus in vascular dementia (VD) rats, so as to reveal its underlying mechanism in improving VD. METHODS: A total of 40 male SD rats were randomly divided into normal control, sham-operation, VD model, and scalp-acupuncture groups (n=10 in each group). The VD model was established by using modified Pulsinelli 4-vessel-occlusion method. Scalp-acupuncture was applied to bilateral "Daozang Shangjiao" and bilateral "Jiyi", and "Siwei" acupoints according to FANG's scalp-acupuncture therapy for 30 min, once daily for 10 days. The rat's learning-memory ability was detected by Morris water maze tests, and the expression of apoptosis related proteins Bcl-2 of astrocytes in the hippocampus was detected by immunofluorescence double labeling technique and Western blot, respectively. RESULTS: Morris water maze tests showed that the average escape latency of place navigation test and the duration of the first platform-site crossover (space probing test) were significantly increased in the model group compared with the normal control group (P<0.01), and the number of platform-site crossovers in 2 min was markedly decreased in the model group (P<0.01). The increased average escape latency and the increased duration of first platform-site crossover were markedly shortened in the scalp-acupuncture group in comparison with the model group (P<0.01), and the number of the platform crossovers in 2 min was significantly increased in the scalp-acupuncture group (P<0.01). The expression of Bcl-2 protein and the number of GFAP/Bcl-2-labeled cells of hippocampal CA 1 region in the model group were remarkably decreased compared with the normal control group (P<0.01). After scalp-acupuncture intervention, the decreased Bcl-2 protein expression level and the number of double-labeled GFAP/Bcl-2 cells were significantly up-regulated in the scalp-acupuncture group compared with the model group (P<0.01). No significant differences were found between the normal control and sham-operation groups in the escape latency, duration of the first platform-site crossover, number of platform crossovers in 2 min, number of GFAP/Bcl-2 double labeled cells and the expression level of hippocampal Bcl-2 protein (P>0.05). CONCLUSION: Scalp-acupuncture can effectively regulate the expression of apoptosis related proteins Bcl-2 of astrocytes in the CA 1 region of hippocampus in rats with VD, which may contribute to its effect in the improvement of VD.

ABSTRACT

OBJECTIVE: To explore effect of acupuncture at Shaoze (SI 1) for treatment of postpartum hypolactation. METHODS: Multicentral randomized controlled and single blind clinical trial was adopted, and 276 cases were divided into a treatment group and a control group, 138 cases in each group. The treatment group were treated with electroacupuncture (EA) at Shaoze (SI 1) and the control group with EA at Shangyang (LI 1). After treatment of 2 courses, the therapeutic effects and changes of cumulative score of TCM symptoms, mammary filling degree, lactation amount, prolactin level were evaluated and investigated. RESULTS: The cured and markedly effective rate was 97.8% in the treatment group and 24.3% in the control group with a significant difference between the two groups (P < 0.05). The treatment group in improvement of the cumulative score of TCM symptoms and the mammary filling degree, and increasing the lactation amount and the prolactin level were better than the control group (P < 0.01). CONCLUSION: Acupuncture at Shaoze (SI 1) has obvious therapeutic effect on hypolactation.
