ABSTRACT

Higher education institutions are promoting the adoption of innovative methodologies and instructional approaches to engage and promote personalized learning paths to their students. Several strategies based on gamification, artificial intelligence, and data mining are adopted to create an interactive educational setting centred around students. Within this personalized learning environment, there is a notable boost in student engagement and enhanced educational outcomes. The MathE platform, an online educational system introduced in 2019, is specifically crafted to support students tackling difficulties in comprehending higher-education-level mathematics or those aspiring to deepen their understanding of diverse mathematical topics - all at their own pace. The MathE platform provides multiple-choice questions, categorized under topics and subtopics, aligning with the content taught in higher education courses. Accessible to students worldwide, the platform enables them to train their mathematical skills through these resources. When the students log in to the training area of the platform, they choose a topic to study and specify whether they prefer basic or advanced questions. The platform then selects a set of seven multiple-choice questions from the available ones under the chosen topic and generates a test for the student. After completing and submitting the test, the answers are recorded and stored on the platform. This paper describes the data stored in the MathE platform, focusing on the 9546 answers to 833 questions, provided by 372 students from 8 countries who use the platform to practice their skills using the questions (and other resources) available on the platform. The information in this paper will help research about active learning tools to support the improvement of future education, especially at higher educational level. Furthermore, these data are valuable for understanding student learning patterns, assessing platform efficacy, gaining a global perspective on mathematics education, and contributing to the advancement of active learning tools for higher education.

Introdução: O papilomavírus humano, mais conhecido como HPV, é um vírus responsável por infectar peles e mucosas (oral, genital ou anal), podendo evoluir para um quadro cancerígeno, é caracterizado como uma infecção sexualmente transmissível, e seu principal meio de transmissão é o contato com a pele ou mucosa contaminada. Como ocorre na maioria das infecções de transmissão sexual, a literatura aponta que o homem é o principal elo na cadeia epidemiológica do HPV, atuando como "portador" ou "vetor" de tipos oncogênicos. Assim, é extremamente importante salientar que a população masculina, considerada o principal vetor de transmissão do HPV e apresentando alto índice de prevalência da infecção, seja informada sobre o HPV. Objetivo: Identificar o conhecimento de adultos jovens universitários do sexo masculino sobre o HPV. Material e Método: Estudo descritivo e exploratório com abordagem quantitativa. Pesquisa desenvolvida em um centro universitário de uma cidade do interior paulista. A coleta de dados foi realizada entre universitários do sexo masculino, adultos jovens com idade entre 18 e 20 anos e que estavam matriculados nos cursos de graduação deste centro universitário. A coleta de dados consistiu na apuração em um questionário virtual abrangendo questões sobre dados pessoais e questões fechadas sobre a temática abordada no estudo. Após a coleta foi realizada a análise percentual referente às respostas dadas pelos participantes. Resultados: Foi observado que a maioria dos participantes (75,4%) tinha conhecimento sobre o que é o HPV. Porém, os dados mostraram que nem todos que tinham esse conhecimento foram vacinados (60,2%). Em adição, os dados mostraram que a maioria (77,1%) apresentou conhecimento sobre a via de transmissão, mas nem todos (51,7%) apresentaram conhecimento sobre os sintomas que o HPV poderia causar. Conclusão: Apesar de a maioria dos indivíduos apresentarem conhecimento sobre o HPV, nota-se a importância de difundir conhecimentos sobre sinais e sintomas relacionados a esse vírus e as formas de proteção a fim de reduzir a taxa de transmissibilidade do mesmo


Introduction: The human papillomavirus, better known as HPV, is a virus responsible for infecting skin and mucous membranes (oral, genital or anal), which can develop into a cancerous condition. It is characterized as a sexually transmitted infection, and its main means of transmission is contact with contaminated skin or mucous membrane. As is the case with most sexually transmitted infections, the literature indicates that men are the main link in the HPV epidemiological chain, acting as a "carrier" or "vector" of oncogenic types. Therefore, it is extremely important to emphasize that the male population, considered the main vector of HPV transmission and presenting a high prevalence rate of infection, is informed about HPV. Objective: To identify the knowledge of young male university students about HPV. Material and Method: Descriptive and exploratory study with a quantitative approach. The research was developed in a university center located in a city in the country of São Paulo. Data collection was carried out among male university students, young adults aged between 18 and 20 years and who were enrolled in undergraduate courses at this university center. Data collection consisted of a virtual questionnaire covering questions about personal data and closed questions on the thematic of this study. After collection, a percentage analysis was performed between the responses given by the participants. Results: It was observed that most participants (75.4%) were aware of what HPV is. However, the data showed that not everyone who had this knowledge was vaccinated (60.2%). In addition, the data showed that the majority (77.1%) were aware of the transmission route, but not all (51.7%) were aware of the symptoms that HPV could cause. Conclusion: Although most individuals have knowledge about HPV, it is important to disseminate knowledge about signs and symptoms related to this virus and forms of protection in order to reduce its transmissibility rate


Introducción: El virus del papiloma humano, mejor conocido como HPV, es un virus responsable de infectar la piel y las mucosas (orales, genitales o anales), pudiendo convertirse en una condición cancerosa. HPV se caracteriza por ser una infección de transmisión sexual, y su principal vía de transmisión es el contacto con la piel o mucosas contaminadas. Como ocurre con la mayoría de las infecciones de transmisión sexual, la literatura señala que los hombres son el principal eslabón de la cadena epidemiológica del HPV, actuando como "portadores" o "vectores" de tipos oncogénicos. Por lo tanto, es de suma importancia enfatizar que la población masculina, considerada el principal vector de transmisión del HPV y que presenta una alta tasa de prevalencia de infección, esté informada sobre el HPV. Objetivo: Identificar el conocimiento de jóvenes universitarios varones sobre el HPV. Material y Método: Estudio descriptivo y exploratorio con enfoque cuantitativo. La investigación se desarrolló en un centro universitario ubicado en una ciudad del interior de São Paulo. La recolección de datos se realizó entre estudiantes universitarios del sexo masculino, adultos jóvenes con edades comprendidas entre 18 y 20 años y que cursaban cursos de pregrado en este centro universitario. La recolección de datos consistió en un cuestionario virtual que abarcó preguntas sobre datos personales y preguntas cerradas sobre el tema abordado en este estudio. Después de la recolección, se realizó un análisis porcentual de las respuestas dadas por los participantes. Resultados: Se observó que la mayoría de los participantes (75,4%) conocían qué es el HPV. Sin embargo, los datos mostraron que no todos los que tenían este conocimiento estaban vacunados (60,2%). Además, los datos mostraron que la mayoría (77,1%) conocía la vía de transmisión, pero no todos (51,7%) conocían los síntomas que podría causar el HPV. ABSTRACT

In tropical and subtropical areas, malaria stands as a profound public health challenge, causing an estimated 247 million cases worldwide annually. Given the absence of a viable vaccine, the timely and effective treatment of malaria remains a critical priority. However, the growing resistance of parasites to currently utilized drugs underscores the critical need for the identification of new antimalarial therapies. Here, we aimed to identify potential new drug candidates against Plasmodium falciparum, the main causative agent of malaria, by analyzing the transcriptomes of different life stages of the parasite and identifying highly expressed genes. We searched for genes that were expressed in all stages of the parasite's life cycle, including the asexual blood stage, gametocyte stage, liver stage, and sexual stages in the insect vector, using transcriptomics data from publicly available databases. From this analysis, we found 674 overlapping genes, including 409 essential ones. By searching through drug target databases, we discovered 70 potential drug targets and 75 associated bioactive compounds. We sought to expand this analysis to similar compounds to known drugs. So, we found a list of 1557 similar compounds, which we predicted as actives and inactives using previously developed machine learning models against five life stages of Plasmodium spp. From this analysis, two compounds were selected, and the reactions were experimentally evaluated. The compounds HSP-990 and silvestrol aglycone showed potent inhibitory activity at nanomolar concentrations against the P. falciparum 3D7 strain asexual blood stage. Moreover, silvestrol aglycone exhibited low cytotoxicity in mammalian cells, transmission-blocking potential, and inhibitory activity comparable to those of established antimalarials. These findings warrant further investigation of silvestrol aglycone as a potential dual-acting antimalarial and transmission-blocking candidate for malaria control.

ABSTRACT

The expansion of bean genome technologies has prompted new perspectives on generating resources and knowledge essential to research and implementing biotechnological tools for the practical operations of plant breeding programs. This study aimed to resequence the entire genome (whole genome sequencing-WGS) of 40 bean genotypes selected based on their significance in breeding programs worldwide, with the objective of generating an extensive database for the identification of single nucleotide polymorphisms (SNPs). Over 6 million SNPs were identified, distributed across the 11 bean chromosomes. After quality variant filtering, 420,509 high-quality SNPs were established, with an average of 38,228 SNPs per chromosome. These variants were categorized based on their predicted effects, revealing that the majority exerted a modifier impact on non-coding genome regions (94.68%). Notably, a significant proportion of SNPs occurred in intergenic regions (62.89%) and at least one SNP was identified in 58.63% of the genes annotated in the bean genome. Of particular interest, 7841 SNPs were identified in 85% of the putative plant disease defense-related genes, presenting a valuable resource for crop breeding efforts. These findings provide a foundation for the development of innovative and broadly applicable technologies for the routine selection of superior genotypes in global bean improvement and germplasm characterization programs.

OBJECTIVE: To investigated the dynamic ventilatory responses and their influence on functional exercise capacity in patients with long-COVID-19 syndrome (LCS). RESULTS: Sixteen LCS patients were subjected to resting lung function (spirometry and respiratory oscillometry-RO) and cardiopulmonary performance to exercise (Spiropalm®-equipped six-minute walk test-6MWT and cardiopulmonary exercise test-CPX). At rest, spirometry showed a normal, restrictive and obstructive pattern in 87.5%, 6.25% and 6.25% of participants, respectively. At rest, RO showed increased resonance frequency, increased integrated low-frequency reactance and increased difference between resistance at 4-20 Hz (R4-R20) in 43.7%, 50%, and 31.2% of participants, respectively. The median of six-minute walking distance (DTC6) was 434 (386-478) m, which corresponds to a value of 83% (78-97%) of predicted. Dynamic hyperinflation (DH) and reduced breathing reserve (BR) were detected in 62.5% and 12.5% of participants, respectively. At CPX, the median peak oxygen uptake (VO2peak) was 19 (14-37) ml/kg/min. There was a significant correlation of 6MWD with both R4-R20 (rs=-0.499, P = 0.039) and VO2peak (rs=0.628, P = 0.009). Our results indicate that DH and low BR are contributors to poor exercise performance, which is associated with peripheral airway disease. These are promising results considering that they were achieved with simple, portable ventilatory and metabolic systems.

ABSTRACT

BACKGROUND AND OBJECTIVES: Although peak oxygen uptake (VO2peak ) is one of the most important measures in clinical practice, the high cost and time consumption have led to the search for simpler devices and the development of the estimating cardiopulmonary fitness (eCPF) equation. Since the lungs are one of the sites most affected by rheumatoid arthritis (RA), this study aimed to create a predictive equation for VO2peak obtained by simple sampling technology in women with RA-associated interstitial lung disease (RA-ILD). METHODS: This cross-sectional study evaluated 47 women with RA-ILD. The participants underwent the following evaluations: computed tomography (CT); evaluation of disease activity through the Clinical Disease Activity Index (CDAI); measurement of physical function using the Health Assessment Questionnaire disability index (HAQ-DI); pulmonary function testing, including spirometry, diffusing capacity for carbon monoxide (DlCO ), nitrogen single-breath washout (N2 SBW) test, and impulse oscillometry; and cardiopulmonary exercise testing (CPET) using FitMate™. RESULTS: VO2peak was correlated with age (r = -0.550, p < 0.0001), rheumatoid factor (r = -0.443, p = 0.002), anti-cyclic citrullinated peptide antibodies (r = -0.410, p = 0.004), CDAI (r = -0.462, p = 0.001), HAD-DI (r = -0.486, p = 0.0005), forced vital capacity (r = 0.491, p = 0.0004), DlCO (r = 0.621, p < 0.0001), phase III slope of N2 SBW (r = -0.647, p < 0.0001), resonance frequency (Fres , r = -0.717, p < 0.0001), integrated low-frequency reactance (r = -0.535, p = 0.0001), and the inhomogeneity of respiratory system resistance between 4 and 20 Hz (r = -0.631, p < 0.0001). In the CT examination, patients with extensive ILD had significantly lower VO2peak than patients with limited ILD (p < 0.0001). In the stepwise forward regression analysis, Fres , DlCO and age explained 61% of the VO2peak variability. CONCLUSIONS: As assessed by CPET, women with RA-ILD show reduced cardiopulmonary fitness, which can be explained at least in part by the presence of small airway disease, deterioration of pulmonary gas exchange, and advanced age. These associations of pulmonary variables with eCPF may be clinically important and support the use of the eCPF equation to improve patient outcomes.

ABSTRACT

Developing innovative systems and operations to monitor forests and send alerts in dangerous situations, such as fires, has become, over the years, a necessary task to protect forests. In this work, a Wireless Sensor Network (WSN) is employed for forest data acquisition to identify abrupt anomalies when a fire ignition starts. Even though a low-power LoRaWAN network is used, each module still needs to save power as much as possible to avoid periodic maintenance since a current consumption peak happens while sending messages. Moreover, considering the LoRaWAN characteristics, each module should use the bandwidth only when essential. Therefore, four algorithms were tested and calibrated along real and monitored events of a wildfire. The first algorithm is based on the Exponential Smoothing method, Moving Averages techniques are used to define the other two algorithms, and the fourth uses the Least Mean Square. When properly combined, the algorithms can perform a pre-filtering data acquisition before each module uses the LoRaWAN network and, consequently, save energy if there is no necessity to send data. After the validations, using Wildfire Simulation Events (WSE), the developed filter achieves an accuracy rate of 0.73 with 0.5 possible false alerts. These rates do not represent a final warning to firefighters, and a possible improvement can be achieved through cloud-based server algorithms. By comparing the current consumption before and after the proposed implementation, the modules can save almost 53% of their batteries when is no demand to send data. At the same time, the modules can maintain the server informed with a minimum interval of 15 min and recognize abrupt changes in 60 s when fire ignition appears.

BACKGROUND: Recent initiatives, such as earlier diagnosis and treatment, have enhanced the survival of patients with systemic sclerosis (SSc). Despite these initiatives, there is extreme variability in rehabilitation strategies for these patients. In 2006, the Glittre-ADL test (TGlittre) was developed to evaluate functional capacity using multiple tasks similar to the activities of daily living (ADLs). OBJECTIVES: To evaluate the impact of therapist-oriented home rehabilitation (TOHR) on functional capacity using TGlittre and to examine the effects of TOHR on physical function, hand function, and quality of life (QoL) among women with SSc. METHODS: This quasi-experimental and longitudinal study included 12 women with SSc who underwent TOHR 3 times per week for 12 weeks. Before and after TOHR, functional capacity was assessed using TGlittre, physical function was examined by the Health Assessment Questionnaire Disability Index (HAQ-DI), hand function was evaluated using the Cochin Hand Functional Scale (CHFS) and handgrip strength (HGS), and QoL was evaluated using the Short Form-36 Health Survey Questionnaire (SF-36). RESULTS: When comparing the pre- and post-TOHR values of TGlittre, a significant reduction was found in total time (p= 0.002) and manual time (p= 0.010). There was a nonsignificant decrease in HAQ-DI scores between pre- and post-TOHR (p= 0.07). Regarding hand function, there was a significant reduction in the CHFS between pre- and post-TOHR (p= 0.036), although no significant difference was observed in HGS between pre- and post-TOHR (p= 0.08). Regarding QoL, there was an increase in all SF-36 categories, although physical function was the only category that was significantly increased (p= 0.008). CONCLUSION: After TOHR, patients with SSc are able to more quickly perform TGlittre tasks when considering both total and manual times. TOHR also positively affects manual skills and QoL.

ABSTRACT

The objective of this study was to determine the effect of enhancing conventional care for people with chronic painful temporomandibular disorders (TMD) with an individualised contemporary pain science education (PSE) intervention. In this randomized controlled trial, a consecutive sample of 148 participants (18-55 years of age) was randomized into 2 groups: PSE-enhanced conventional care or Conventional care alone. Conventional care involved a 6-week, 12-session manual therapy and exercise program. The PSE enhancement involved 2 sessions of modern PSE, undertaken in the first 2 treatment sessions. Primary outcomes were pain intensity, assessed with a numeric pain rating scale, and disability, assessed with the craniofacial pain and disability inventory, post-treatment. Linear mixed model analyses were used to investigate between-group differences over time. There was a statistically and clinically meaningful effect of PSE enhancement on disability (Mean Difference = 6.1, 95% CI: 3.3-8.8), but not on pain intensity, post-treatment. Secondary analyses suggested clinically meaningful benefit of PSE enhancement on pain and disability ratings at 10-week and 18-week follow-ups, raising the possibility that preceding conventional care with a PSE intervention may result in long-term benefits. PERSPECTIVE: The addition of modern Pain Science Education (PSE) intervention improved disability for people with chronic TMD receiving manual therapy and exercise, but not pain. A mean difference in pain and disability favoring the PSE group at the 10- and 18-week follow-ups, respectively, suggests that PSE addition resulted in longer-lasting effects. ABSTRACT

Toxoplasmosis, despite advances in science and technology, is a disease that requires attention since there is no vaccine capable of immunizing humans and animals against all isolated types of Toxoplasma gondii. Thus, the use of chimeric proteins, which can contain multiple antigens, is a very promising alternative for the process of obtaining a vaccine and diagnostic test for toxoplasmosis due to the great diversity of antigens presented by T. gondii. In this context, the present study evaluates batch culture strategies in the production of the multi-antigenic chimeric protein TgAGS/BsT from Toxoplasma gondii. Several exploratory cultures were initially carried out to observe the kinetic behavior of E. coli BL21 Star in five different medium compositions without the addition of IPTG (inducer). Cultures of E. coli B21 Star were carried out with 1.0 mM IPTG at different times of initiation of induction (0.5, 1, and 6 h) to evaluate the effects on cell growth, production of the protein of interest, culture pH, and acetic acid formation. The results showed that among the culture media evaluated, 2xTY and TB supplemented with glycerol had the best cell concentration values of 3.42 ± 0.05 g/L and 5.48 ± 0.05 g/L, respectively. In the assays induced by IPTG, a higher expression of TgAGS/BsT protein was observed, with induction beginning within 6 h of culture, with a maximum concentration of protein of interest of 1.82 ± 0.02 g/L for the 2xTY and 2.49 ± 0.03 g/L for the TB medium. In addition, later induction by IPTG provided greater stability of plasmid pET-TgAGS, remaining with values above 90% at the end of culture.

ABSTRACT

Universities are encouraging the implementation of innovative methodologies and teaching strategies to develop an interactive and appealing educational environment where students are the focus of the learning process. In such a personalised learning environment, an increase of the students' engagement and the improvement of the outcomes arise. MathE has been developed to help achieve this goal. Based on collaborative procedures, internet resources - both pre-existing and freely available as well as resources specifically conceived by the project team - and communities of practices, MathE intends to be a tool to nurture and stimulate the learning of Mathematics in higher education. This study introduces and describes the MathE platform, which is divided into three sections: Student's Assessment, Library and Community of Practice. An in-depth description of the Student's Assessment section is presented and an analysis of the results obtained from students, when using this feature of the platform, is also provided. After this, and based on the answers to an online survey, the impact of the MathE platform among students and teachers of eight countries is shown. Although the number of collected results is still scarce, it allows the recognition of a trend regarding the use of the material of the Student's Assessment section for autonomous study. The results indicate the platform is well organized, with a satisfactory amount and diversity of questions and good interconnection between the various parts. Nevertheless, both teachers and students indicate that more questions should be introduced. The overall opinion about the MathE platform is very favourable.

ABSTRACT

Hiatal hernia is a rare postoperative complication of esophagectomy in the treatment of esophageal cancer. Although rare, its incidence increased after the establishment of minimally invasive surgical techniques. The patient is usually oligosymptomatic, and the diagnosis is made in the late postoperative period, during outpatient follow-up. The initial presentation of hiatus hernia with hemodynamic instability is a rare condition that has never been described in the literature before. In the following report, we address the clinical picture, diagnosis, and treatment for this condition, discussing the main nuances of the literature.

ABSTRACT

Congenital myopathies are a heterogeneous group of conditions diagnosed based on the clinical presentation, muscle histopathology and genetic defects. Recessive mutations in the SPEG gene have been described in recent years and are primarily associated with centronuclear myopathy with cardiomyopathy. In this report, we describe two Brazilian siblings, aged 13 and 6 years, with a novel homozygous mutation (c.8872 C>T:p.Arg2958Ter) in the SPEG gene leading to a congenital myopathy. In the older sibling, the muscle biopsy showed fiber size disproportion. The mean diameter of type 2 fibers (119 µm) was significantly higher than type 1 (57 µm) (P < 0,001) with a 72% prevalence of type 1 fibers. The patient also had progressive cardiomyopathy treated with heart transplantation. The present report expands the muscle histopathological findings related to mutations in the SPEG gene, including fiber size disproportion without central nuclei. Additionally, this report describes the first case of heart transplantation in a patient with SPEG mutations.

ABSTRACT

The objective of this study will be to investigate the additional effect of pain neuroscience education program compared to a craniocervical manual therapy and exercises program for pain intensity and disability in patients with temporomandibular disorders (TMD). This study will be a randomized controlled trial comprising a sample of 148 participants. Subjects between 18 and 55 years, both genders, will undergo a screening process to confirm painful TMD by the Research Diagnostic Criteria (RDC/TMD), and then the volunteers will be randomized into two groups (G1: pain neuroscience education + craniocervical manual therapy and exercises vs. G2: craniocervical manual therapy and exercises). The volunteers will be recruited at the dentistry clinic. The intervention will be administered twice a week for 6 weeks by a single therapist lasting 1 h per session. The primary outcome will be pain intensity and disability and the secondary outcomes will be pain self-efficacy, kinesiophobia, and global perceived effect of improvement. The participants will be assessed immediately after the last session and at one- and three-month follow-ups. All statistical analyses will be conducted following intention-to-treat principles, and the treatment effects will be calculated using linear mixed models. The results of this study may contribute to understand the additional effect of pain neuroscience education intervention on TMD patients submitted to manual therapy and exercise. ABSTRACT

A soroprevalência do SARS-CoV-2 foi determinada a partir de um inquérito epidemiológico transversal realizado no Hospital Universitário Gaffrée e Guinle, na cidade do Rio de Janeiro, Brasil, nos meses de outubro a novembro de 2020, durante a pandemia de COVID-19, na população de funcionários que exercia atividade laboral naquele período. A caracterização sociodemográfica foi realizada, além da análise das associações entre as variáveis de interesse ou exposição com a infecção pelo SARS-CoV-2, para estabelecer quais sinais e sintomas foram de maior prevalência. Para o estudo foram coletadas amostras de sangue e utilizados ensaios imunocromatográficos (COVID-19 IgG/IgM ECO Test® e Medtest® Coronavírus (COVID-19) IgG/IgM), para detectar IgG e IgM anti-SARS-CoV-2. Um cálculo amostral obtendo-se 391 profissionais de saúde demonstrou que 173 (44,2%) indivíduos apresentaram ao menos um resultado positivo para infecção pelo SARS-CoV-2. Não houve diferença significativa entre as medianas das idades dos pacientes que apresentaram infecção pelo SARS-CoV-2 (mediana 40 anos, IIQ 34-53) e dos que não apresentaram infecção (mediana 39 anos, IIQ 32-49), valor de P= 0,148. Também não houve diferença entre o sexo dos profissionais com infecção e sem infecção pelo SARS-CoV-2 (OR = 1,02; IC 95% 0,59 ­ 1,78). Dos 173 profissionais que apresentaram infecção pelo SARS-CoV-2, 70 (40,5%) relataram sintomas relacionados à COVID-19, enquanto 41 (23,7%) profissionais relataram que não apresentaram nenhum sintoma e 62 (35,8%) não informaram sobre presença ou não de sintomas, sendo excluídos da análise. Os sintomas mais frequentemente apresentados pelos profissionais infectados foram: cefaleia 46,5% (59/127); perda de olfato 32,3% (41/127); perda de paladar 30,7% (39/127); tosse seca 27,6% (35/127); diarreia 24,4% (31/127); dores no corpo 27,6% (35/127); outros 13,4% (17/127); dores nas pernas 11,8% (15/127); tonteira 11,8% (15/127) e febre 9,4% (12/127). Estabelecer um protocolo de vigilância com um teste fácil e rápido para as políticas de COVID-19 entre os profissionais na linha de frente e atuantes na assistência é imperativo para o controle da transmissão de SARS-CoV-2 e definir critérios epidemiológicos que orientem a tomada de decisão quanto ao isolamento de profissionais infectados sintomáticos e assintomáticos.


The seroprevalence of SARS-CoV-2 was determined from a cross-sectional epidemiological survey carried out at the Gaffrée and Guinle University Hospital in the city of Rio de Janeiro, Brazil from October to November 2020 during the COVID-19 pandemic in the population of employees who worked in that period. Sociodemographic characterization was performed in addition to the analysis of associations between the variables of interest or exposure with SARS-CoV-2 infection to establish which signs and symptoms were more prevalent. For the study, blood samples were collected and immunochromatographic assays (COVID-19 IgG/IgM ECO Test® and Medtest® Coronavirus (COVID-19) IgG/IgM) were used to detect IgG and IgM anti-SARS-CoV-2. A sample calculation obtaining 391 health professionals showed that 173 (44.2%) individuals had at least one positive result for SARS-CoV-2 infection. There was no significant difference between the median ages of patients who had SARS-CoV-2 infection (median 40 years, IIQ 34-53) and those who did not have infection (median 39 years, IIQ 32-49), P value = 0.148. There was also no difference between the sex of professionals with and without SARS-COV-2 infection (OR = 1.02; 95% CI 0.59 ­ 1.78). Of the 173 professionals who had SARS-COV-2 infection, 70 (40.5%) reported symptoms related to COVID-19, while 41 (23.7%) professionals reported that they had no symptoms and 62 (35.8%) did not report on the presence or absence of symptoms, being excluded from the analysis. The symptoms most frequently presented by the infected professionals were: headache 46.5% (59/127); loss of smell 32.3% (41/127); loss of taste 30.7% (39/127); dry cough 27.6% (35/127); diarrhea 24.4% (31/127); body pain 27.6% (35/127); another 13.4% (17/127); leg pain 11.8% (15/127); dizziness 11.8% (15/127) and fever 9.4% (12/127). Establish a surveillance protocol with an easy and quick test for COVID-19 policies among frontline and care professionals is imperative to control the transmission of SARS-CoV-2 and define epidemiological criteria that guide the decision-making regarding the isolation of symptomatic and asymptomatic infected professionals.

ABSTRACT

OBJECTIVE: This study investigated the effects of star-shape Kinesio taping (KT) compared with both sham KT and minimal intervention (MI) on pain intensity and postural control. DESIGN: Randomized controlled trial. SETTING: Outpatient physical therapy. PARTICIPANTS: A total of 120 people with chronic low back pain (CLBP) aged 18-60 years (N=120). INTERVENTIONS: Star-shape KT, sham KT (no tension), and MI (educational booklet for self-management counseling). MAIN OUTCOME MEASURES: The primary outcome measures were pain intensity and center of pressure (COP) mean sway speed, and disability score (Oswestry Disability Index) was a secondary outcome. The outcomes were obtained immediately after initial KT application, on the seventh day of intervention and at the 1-month follow-up. Linear mixed-model analyses using Bonferroni post hoc analyses were applied to investigate between-group differences. The model included treatment, time, and treatment×time interaction as fixed effects. RESULTS: Pain intensity was significantly lower for the star-shape KT group than for the MI group (mean difference [MD], -1.35; 95% confidence interval [CI], -2.63 to -0.07) immediately after the intervention and on the seventh day of intervention (MD, -1.32; 95% CI, -2.56 to -0.07). No difference in pain intensity between star-shape KT vs sham-KT groups was observed. In addition, no significant between-group differences were observed for the COP mean sway speed and disability score at any of the follow-up times. CONCLUSIONS: Our results showed no meaningful effect of star-shape KT intervention on pain intensity and postural control in people with CLBP compared with MI or sham KT. The observed reduction of 1.3 units between star-shape KT and MI groups was statistically different, but it could not be considered clinically relevant. The results of this trial suggest that benefits from KT are more likely attributable to contextual factors rather than specific taping parameters.

ABSTRACT

Pur-α protein (PURA) syndrome manifests in early childhood with core features such as neurodevelopmental and speech delay, feeding difficulties, epilepsy, and hypotonia at birth. We identified three cases with PURA syndrome in a cohort of patients with unexplained muscular weakness, presenting with a predominantly neuromuscular and ataxic phenotype. We further characterize the clinical presentation of PURA syndrome including myopathic facies and muscular weakness as the main clinical symptoms in combination with elevated serum creatine kinase levels. Furthermore, we report two novel variants located in the conservative domains PUR-I and PUR-II. For the first time, we present the muscle biopsies of PURA syndrome patients, showing myopathic changes, fiber size variability, and fast fiber atrophy as the key features. PURA syndrome should be taken into consideration as a differential diagnosis in pediatric patients with unexplained muscle weakness.

BACKGROUND: The hands are the primary site of involvement in scleroderma, and impaired hand function is primarily responsible for difficulty in performing activities of daily living. This study aimed to evaluate exercise functional capacity in women with scleroderma through the Glittre Activities of Daily Living Test, which incorporates both lower and upper limb activities, and to correlate the findings with physical capacity and hand function. METHODS: Thirty-three women with scleroderma and 30 matched control women were assessed with the Glittre Activities of Daily Living Test, the Cochin Hand Functional Scale for hand function, the Scleroderma Health Assessment Questionnaire Disability Index for physical capacity, handgrip strength tests, and pulmonary function tests. FINDINGS: In the Glittre Activities of Daily Living Test, the median values for total time, manual time, and the manual time/total time ratio were higher among individuals with scleroderma than among controls (P Ë‚ 0.001 for all tests). Manual time was significantly correlated with the Cochin Hand Functional Scale, the Scleroderma Health Assessment Questionnaire Disability Index, and handgrip strength. Additionally, peripheral oxygen saturation during the Glittre Activities of Daily Living Test was significantly correlated with pulmonary function parameters. INTERPRETATION: Our results indicate that women with scleroderma need more time to complete the activities in the Glittre Activities of Daily Living Test, largely due to greater difficulty performing the shelves manual task. Thus, the Glittre Activities of Daily Living Test may be used in clinical practice to evaluate the performance of activities of daily living by people with scleroderma.

Introduction: The laboratory diagnosis of syphilis is given by a positive treponemal test and a non-treponemal test, with VDRL (Veneral Disease Research Laboratory) being the "gold standard". Objective: To compare two tests commercially validated for biological fluids and analyzed by different operators, in order to assess their performance in detecting high (≥:8) and low (≤1:2) titrations, as well as to determine the agreement between results in paired serum samples from patients with syphilis and living with HIV. Methods: Cross-sectional study, approved by the Research Ethics Committee of the teaching hospital Gaffrée e Guinle (HUGG), under CAAE 66558117.0.0000.5258. The study population was composed by patients diagnosed with syphilis and confirmed by the positivity of one or more treponemal tests. All samples were analyzed simultaneously by two different operators, each using a kit: VDRL WAMA Diagnóstica®, São Carlos, SP, Brazil; VDRL Brás, Laborclin®, Pinhais, PR, Brazil. The SPSS statistical program was used. Results: 110 serum samples from patients diagnosed with syphilis treated at HUGG were analyzed. The frequency of high VDRL titrations among patients, following the VDRL criterion ≥1:8, was practically the same in both tests, with 68% in VDRL Laborclin and 69% in VDRL WAMA (p = 0.87) and VDRL ≤1:2, 80% for WAMA and 83% for Laborclin (p = 0.72). The results of VDRL were tabulated in pairs; then the Cohen's Kappa coefficient of agreement was calculated (Κ) 0.32 (95%CI 0.21­0.41; p<0.00001), as well as the weighted Kappa (Kw) and the intraclass correlation coefficient (ICC) 0.89 (95%CI 0.84­0.92; p<0.00001). The Bland-Altman diagram was also used. We found poor agreement between the VDRL tests when results were nominally concordant, that is, with the same titles in both tests. However, if partial agreement is considered, the interpretation of the magnitude of agreement estimators was almost complete (≥0.80). Conclusion: Reliability and agreement were high between the VDRL tests of both manufacturers when considering the close titrations (up to two dilutions). Further reliability and agreement studies are essential between the non-treponemal tests available and used in Brazil.


Introdução: O diagnóstico laboratorial da sífilis é realizado por meio da positividade de um teste treponêmico e de um teste não treponêmico, sendo o VDRL (do inglês Veneral Disease Research Laboratory) o "padrão ouro". Objetivo: Comparar dois testes comercialmente validados para fluidos biológicos e analisados por operadores diferentes, com o intuito de avaliar o desempenho dos testes em detectar titulações altas (≥ :8) e baixas (≤1:2), bem como determinar a concordância entre ambos os resultados em amostras pareadas de soro de pacientes com sífilis vivendo com HIV. Métodos: Estudo transversal, aprovado pelo Comitê de Ética em Pesquisa do Hospital Universitário Gaffrée e Guinle (HUGG), sob o CAAE: 66558117.0.0000.5258. A população estudada foi a de pacientes que obtiveram o diagnóstico de sífilis confirmado por meio da positividade de um ou mais testes treponêmicos. Todas as amostras foram analisadas simultaneamente por dois operadores diferentes, cada um utilizando um kit: VDRL WAMA Diagnóstica®, São Carlos, SP, Brasil; VDRL Brás, Laborclin®, Pinhais, PR, Brasil. Utilizou-se o programa estatístico SPSS. Resultados: Foram analisadas 110 amostras de soro de pacientes com diagnóstico de sífilis atendidos no HUGG. A frequência de altas titulações de VDRL entre os pacientes, seguindo o critério de VDRL ≥1:8, foi praticamente a mesma em ambos os testes, com 68% no VDRL Laborclin e 69% no VDRL WAMA (p=0,87) e para VDRL ≤1:2, 80% para WAMA e 83% para Laborclin (p=0,72). Os resultados dos títulos de VDRL foram tabulados em pares; em seguida, foram calculados o coeficiente de concordância Kappa de Cohen (Κ) 0,32 (IC95% 0,21­0,41; p<0,00001), o Kappa ponderado (Kw) e o coeficiente de correlação intraclasse (CCI) 0,89 (IC95% 0,84­0,92; p<0,00001), bem como utilizado o diagrama de Bland-Altman. O estudo encontrou fraca concordância entre os testes de VDRL, se considerados os resultados nominalmente concordantes, isto é, com os mesmos títulos em ambos os testes. Entretanto, se considerado a concordância parcial, a interpretação da magnitude dos estimadores de concordância passou a ser quase completa (≥0,80). Conclusão: A confiabilidade e a concordância foram altas entre os testes de VDRL dos dois fabricantes, quando consideradas as titulações próximas (até duas diluições). Mais estudos de confiabilidade e concordância são fundamentais entre os testes não treponêmicos disponíveis e utilizados no Brasil.

ABSTRACT

Abstract This paper proposes an automatic fuzzy classification system for glycemic index, which indicates the level of Diabetes Mellitus type 2. Diabetes is a chronic disease occurred when there is deficiency in insulin production or in its action, or both, causing complications. Neuro-fuzzy systems and Decision Trees are used to obtain, respectively, the numerical parameters of the membership functions and the linguistic based rules of the fuzzy classification system. The results goal to categorize the glycemic index into 4 classes: decrease a lot, decrease, stable and increase. Real database from [1] is used and the input attributes of the system are defined. In addition, the proposed automatic fuzzy classification system is compared with an "expert" fuzzy classification system, which is totally modeled using expert knowledge. From linguistic based rules obtained from fuzzy inference process, new scenarios are simulated in order to obtain a larger data set which provides a better evaluation of the classification systems. Results are promising, since they indicate the best treatment - intervention or comparative - for each patient, assisting in the decision-making process of the health care professional.
