ABSTRACT

Background Meniscal tears are a common injury in the adult population. With the advent of newer devices, the adoption of the all-inside repair technique has been expanding substantially because of its feasibility and reduced risk to surrounding neurovascular structures. This study was conducted in a lower middle-income country to assess the functional outcome of the arthroscopic all-inside technique and to identify the potential factors that may affect the functional outcome that will eventually influence the future management of these patients. Method This study is a retrospective case series conducted at the Department of Orthopedic Surgery, Aga Khan University Hospital, Karachi, Pakistan. Patients presenting to the outpatient clinics with meniscal tears who underwent arthroscopic all-inside repair from January 2015 to December 2021 were included in this study. The exclusion criteria included patients who had associated fractures and patients with meniscal tears greater than six months ago. Results A total of 29 patients underwent all-inside meniscal repair for meniscus tears. The mean age of our patients was 26.31 years (SD = 7.11 years), ranging from 17 years to 48 years. Of these patients, 26 were males and three were females, accounting for 89.7% and 10.3%, respectively. The most frequent mechanism of injury was twisting while playing sports, accounting for 51.7%, followed by falling while playing sports and road traffic accidents (RTAs), accounting for 13.8% and 20.7%, respectively. Of the 29 patients, 16 (55.2%) had lateral meniscal injuries, 10 (34.5%) were diagnosed with medial meniscus injuries, and three (10.3%) had injuries to both menisci. The most common type of tear that was observed in our sample size was bucket handle tears, which were found in 14 patients, accounting for a total of 48.3%, followed by complex tears in seven patients (24.1%). The majority of the patients, i.e., 19 out of 29 patients (65.5%), had an acute course of injury, i.e., less than six weeks. For the functional outcome, the Lysholm score was calculated at 12 months and was found to be excellent in 17 patients, good in six patients, and fair in six patients, accounting for 58.6%, 20.7%, and 20.7%, respectively. The mean Lysholm score was 90.03 ± 8.85 points. Of the 29 patients, 27 (93.2%) had no complaints at the regular 12-month follow-up, whereas one patient (3.4%) experienced rotatory instability and one patient (3.4%) experienced stiffness at the knee joint. None of the patients had to undergo a reoperation. The mean Lysholm score in the 25 patients who had an associated anterior cruciate ligament tear was 89.64 ± 9.442 points, whereas the four patients who had an isolated meniscal tear had a mean score of 92.50 ± 2.887 points, which was not significantly different (p-value = 0.831). Conclusion All-inside meniscal repair for treating meniscal tears has become the new treatment paradigm as it not only renders excellent functional outcomes with minimal complications but also prevents damage to the surrounding neurovasculature and the soft tissue envelope as it is a minimally invasive technique.

ABSTRACT

Innovation is the introduction of a new method or technology designed to change the way things are done. History is full of remarkable innovations in surgery over the years as surgeons have always been innovating and pioneering latest techniques and equipment that can benefit the mankind. Though persistent, progress has been far from uniform. Despite all the bells and whistles that these innovations bring to the table, the little acknowledged fact is that they are only accessible to a very small proportion of the global population. Five billion people on this planet do not even have access to an operating room when needed. It has been reported that conditions requiring surgery are responsible for one-third of all the deaths in the world. The current narrative review was planned to focus on the importance of innovations in surgery, to highlight the problems that were faced by resource-restricted countries in the past, and the necessity of innovative solutions to improve global surgical care in the future.

ABSTRACT

OBJECTIVE: To evaluate the clinical outcome of percutaneous fixation for unstable pelvic ring injury. METHODS: This retrospective study was conducted at orthopaedics section of Aga Khan University Hospital Karachi, Pakistan from July 2015 to December 2018. Data was retrieved from trauma registry from July 2015 till December 2018, including all patients who underwent percutaneous fixation for pelvic ring injury. Majeed pelvic score was used to determine the functional outcome. RESULTS: A total number of 30 patients were included, 27(90.0%) adults, and 3(10.0%) paediatrics patients. There were 21(70.0%) males and 9(30.0%)females. . Mean age of patients was 37.1±16.1 years. Post op mean Majeed functional pelvic Score was 85.8. Of the 30 patients, 18 (60.0%) had Excellent, 10 (33.3%) good and 02(6.7%) fair scoring. CONCLUSIONS: Percutaneous fixation of posterior ring injuries has excellent functional outcome, with minimal blood loss and no soft tissue striping..

ABSTRACT

Mapping application task graphs on intellectual property (IP) cores into network-on-chip (NoC) is a non-deterministic polynomial-time hard problem. The evolution of network performance mainly depends on an effective and efficient mapping technique and the optimization of performance and cost metrics. These metrics mainly include power, reliability, area, thermal distribution and delay. A state-of-the-art mapping technique for NoC is introduced with the name of sailfish optimization algorithm (SFOA). The proposed algorithm minimizes the power dissipation of NoC via an empirical base applying a shared k-nearest neighbor clustering approach, and it gives quicker mapping over six considered standard benchmarks. The experimental results indicate that the proposed techniques outperform other existing nature-inspired metaheuristic approaches, especially in large application task graphs.

ABSTRACT

Tuberculosis involving almost any organ of the body is a major problem in developing countries. The most commonly affected site in tuberculosis is the lung. Of the extra pulmonary tuberculosis, musculoskeletal involvement accounts for 1-5% of the cases. Muscular involvement in tuberculosis is a rare manifestation of extra pulmonary tuberculosis. Unusual presentations often go undetected and are associated with a diagnostic and therapeutic delay. This case report presents tuberculosis of the flexor carpi radialis.

ABSTRACT

Speech emotion recognition (SER) plays a significant role in human-machine interaction. Emotion recognition from speech and its precise classification is a challenging task because a machine is unable to understand its context. For an accurate emotion classification, emotionally relevant features must be extracted from the speech data. Traditionally, handcrafted features were used for emotional classification from speech signals; however, they are not efficient enough to accurately depict the emotional states of the speaker. In this study, the benefits of a deep convolutional neural network (DCNN) for SER are explored. For this purpose, a pretrained network is used to extract features from state-of-the-art speech emotional datasets. Subsequently, a correlation-based feature selection technique is applied to the extracted features to select the most appropriate and discriminative features for SER. For the classification of emotions, we utilize support vector machines, random forests, the k-nearest neighbors algorithm, and neural network classifiers. Experiments are performed for speaker-dependent and speaker-independent SER using four publicly available datasets: the Berlin Dataset of Emotional Speech (Emo-DB), Surrey Audio Visual Expressed Emotion (SAVEE), Interactive Emotional Dyadic Motion Capture (IEMOCAP), and the Ryerson Audio Visual Dataset of Emotional Speech and Song (RAVDESS). Our proposed method achieves an accuracy of 95.10% for Emo-DB, 82.10% for SAVEE, 83.80% for IEMOCAP, and 81.30% for RAVDESS, for speaker-dependent SER experiments. Moreover, our method yields the best results for speaker-independent SER with existing handcrafted features-based SER approaches.

ABSTRACT

Network-on-chip (NoC) architectures have become a popular communication platform for heterogeneous computing systems owing to their scalability and high performance. Aggressive technology scaling makes these architectures prone to both permanent and transient faults. This study focuses on the tolerance of a NoC router to permanent faults. A permanent fault in a NoC router severely impacts the performance of the entire network. Thus, it is necessary to incorporate component-level protection techniques in a router. In the proposed scheme, the input port utilizes a bypass path, virtual channel (VC) queuing, and VC closing strategies. Moreover, the routing computation stage utilizes spatial redundancy and double routing strategies, and the VC allocation stage utilizes spatial redundancy. The switch allocation stage utilizes run-time arbiter selection. The crossbar stage utilizes a triple bypass bus. The proposed router is highly fault-tolerant compared with the existing state-of-the-art fault-tolerant routers. The reliability of the proposed router is 7.98 times higher than that of the unprotected baseline router in terms of the mean-time-to-failure metric. The silicon protection factor metric is used to calculate the protection ability of the proposed router. Consequently, it is confirmed that the proposed router has a greater protection ability than the conventional fault-tolerant routers.

ABSTRACT

The coronavirus (COVID-19) pandemic has hit the entire world hard. Since its inception from Wuhan China the whole world is affected now. Health care facilities and workers are overwhelmed and the situation is changing on daily basis. With the changes in the dynamics of the hospitals, residency and fellowships training programs have also suffered undoubtedly. Due to decreased elective cases and outpatient clinics surgical training gets compromised, however on the other side this physical distancing and isolation have proven to be effective measures in controlling the disease. In this article we share our experience of effect of COVID-19 pandemic on our orthopedic residency program and how we coped along with it. We also discussed some way forwards in the article.

ABSTRACT

The advent of new devices, technology, machine learning techniques, and the availability of free large speech corpora results in rapid and accurate speech recognition. In the last two decades, extensive research has been initiated by researchers and different organizations to experiment with new techniques and their applications in speech processing systems. There are several speech command based applications in the area of robotics, IoT, ubiquitous computing, and different human-computer interfaces. Various researchers have worked on enhancing the efficiency of speech command based systems and used the speech command dataset. However, none of them catered to noise in the same. Noise is one of the major challenges in any speech recognition system, as real-time noise is a very versatile and unavoidable factor that affects the performance of speech recognition systems, particularly those that have not learned the noise efficiently. We thoroughly analyse the latest trends in speech recognition and evaluate the speech command dataset on different machine learning based and deep learning based techniques. A novel technique is proposed for noise robustness by augmenting noise in training data. Our proposed technique is tested on clean and noisy data along with locally generated data and achieves much better results than existing state-of-the-art techniques, thus setting a new benchmark.

ABSTRACT

Acetabular fractures are caused by energy trauma which is high enough to cause such a fracture with incidence of 3 patients / 100000. In older individuals, most commonmechanism of injury is fall and, in younger individuals, road traffic accidents. Acetabular fractures are usually associated with visceral injuries and other musculoskeletal injuries (about 50% of patient). In this narrative review of targeted English literature from all level of evidences, which is written and supervised by experienced specialized orthopedic and trauma surgeons who were among the pioneers of conducting pelvis fracture management workshops in the country, we aim to describe the mechanism of injury, assessment principles and associated injuries, decision-making and preoperative planning and indications of non-operative managements.

ABSTRACT

Pelvic fractures represents high energy trauma with associated other organ injuries including intra-abdominal injuries, haemorrhage and extremity injuries. Anatomical location of genitourinary structures makes them vulnerable to injury with pelvic fracture. Incidence of sexual dysfunction varies in literature with 5% incidence of dysfunction in patients without urethral injury and 42% with urethral injuries. Hence in pelvic fracture, erectile dysfunction may be due to neurogenic, vascular, corporal and psychogenic injury. In this narrative review of targeted English literature from all level of evidences, which is written and supervised by experienced specialized orthopaedic, trauma and urology surgeons who were among the pioneers of conducting pelvis fracture management workshops in the country, we aim to describe the mechanism that can lead to erectile dysfunction after pelvic fracture, assessment principles, decision-making and preoperative planning and indications of operative managements.

ABSTRACT

A case series was extracted from the trauma registry at Aga Khan University Hospital from the period June 2015 to June 2019. Included were 16 adult patients who presented with intra-articular distal humerus fracture type C2. The functional, clinical and radiological outcomes of fractures treated with or without olecranon osteotomy up to 12 months follow-up were compared. Outcomes were assessed at 6 weeks, 3, 6 and 12 months re-visits. Among the 16 studied patients, 9 (56%) were males and 7 (44%) were females. In the group without osteotomy, there was a good functional and clinical outcome with a mean Quick Disability of the Arm, Shoulder and Hand score of 32±30 at 3 months post-procedure. Bone healing was noticed at 6 months after surgery. In the osteotomy group, 50%-70% bone union was seen at 3 months post-surgery while fair functional and clinical outcome was achieved at 6 months after surgery.

Osteoarthritis of knee is a progressive disease requiring total knee replacement in advanced stage. TKR is being performed in high numbers in developing countries as well. It carries significant economic burden on health system including high cost of implants. Initially, tibial components were cemented all polyethylene monoblock constructs. Subsequent studies showed excellent long term follow up in terms of durability up to 20 years.Successive studies reported aseptic loosening as the cause of failure but such studies failed to address factors responsible for failure other than implant. Cemented metal-backed non-modular tibial components (MBT) are implants in current use. They provide modularity in terms of polyethylene thickness, stems wedges. A literature reported cost saving of $1.17 million, by operating 16,500 total joints using all poly-tibial tibial component rather than metal backed tibial component. studies have reported no significant difference in terms of survivorship, function and backside wear. METHODS: For this study only English written articles were included. Studies included case reports, case series, RCTs and systemic reviews related to all polyethylene tibial components. Articles reporting all levels of evidence - Level I to IV- were included as part of our research. PubMed, Google Scholar and Cochrane Reviews databases from 2000 to 2016 were searched for studies. RESULTS: Information was gathered and thoroughly studied from 30 articles with overall result in favor of the APTC implant. CONCLUSION: All polyethylene tibial component (APTC) is an appealing and cost effective alternative, and is associated with the excellent survivorship and lower risk of revision. In light of the present-day economic evidence and long-term functional outcome, all-polyethylene should be in more use than metal backed especially in resource-constrained setting.

ABSTRACT

Osteoarthritis is one of the most common degenerative conditions affecting knee joint. As our understanding in the disease pathogenesis is evolving, so do the treatment modalities. One of the postulated mechanisms suggests the production of inflammatory cytokines secondary to repeated micro trauma than in turn lead to cartilage damage overtime. Cartilage being avascular structure has limited potential for repair. Based on this, recent studies have been focusing on stimulating cartilage-healing process by growth factors. This is where platelet-rich plasma comes in to light. Literature is showing promising results of platelet-rich plasma in treating knee osteoarthritis. In this review we have discussed the preparation, composition, classification, uses of platelet-rich plasma as well as evidence for its efficacy and complications.

ABSTRACT

OBJECTIVE: To review evidence-based data with respect to safety and efficacy of alternate-day statin therapy in dyslipidaemia compared to the standard daily dose. METHODS: The literature review was conducted at Aga Khan University Hospital, Karachi from July, 2016 to August, 2017. Electronic database search was carried out to compile available literature using PubMed, Excerpta Medica database and Google Scholar. The most relevant evidence-based research articles published over 10 years were selected. The latest search was dated August 03, 2017. RESULTS: A total of 2,074 articles were initially located. Alternate day statin regimen was reported in 53% of articles. Adverse effects on muscle and tendon were reported in 69% of articles. After scrutiny, 19(0.9%) studies covering alternate-day statin-mediated muscle and tendon disorders and 9(0.4%) studies encompassing the potential pathophysiological mechanisms of statin-associated muscle and tendon injury were selected. Except pravastatin and lovastatin, alternate-day statin therapy was almost as effective in lowering total cholesterol, low-density lipoprotein cholesterol and triglycerides as the daily dosing with low incidence of muscle toxicity and tends in opathy. CONCLUSIONS: Alternate-day statin regimen was found to be very well tolerated and might be an effective and safe remedy in clinical practice.

Introduction Motorcycles are an inexpensive and popular mode of transportation in Karachi, Pakistan, despite the increasing number of motor vehicle injuries. Although motorcycle-related injuries have been studied previously, to our knowledge, there are no published reports on the orthopedic injuries associated with motorcycles crashes. Methods We conducted a cross-sectional study of medical records of patients with motorcycle-related injuries in 2015, at the Aga Khan University in Karachi, Pakistan. We reviewed the patient demographic and medical data, helmet use, the Glasgow coma score, the spectrum of injuries, length of stay, specific injury diagnosis, and final disposition of patients. Data were analyzed using IBM SPSS Statistics for Windows, Version 20.0 (IBM Corp., Armonk, NY). Results We identified 450 motorcycle crash injuries. Ninety percent of these victims were males, and 81% were driving at the time of the crash. More than 50% of crashes involved patients age under 29 years, and most crashes (35%) involved patients in the third decade of life. We observed that 6.8% of accidents involved patients younger than 10 years of age. For all the motorcycle-related injuries, 45% occurred during the weekend (Saturday and Sunday). Helmets were worn by only 7% of patients. The most common types of collisions were motorcycle versus car followed by lone motorcycle fall. Of the injuries sustained, the cumulative frequency of orthopedic injuries was the highest (63%), of which 34% were lower limb injuries and 29% were upper limb injuries. The second highest frequency of injuries were head injuries (46%). We also found that 85% motorcycle injury victims sustained fractures. Conclusion The frequency of motorcycle-related injuries was high among young adults, and closed fractures of the lower limbs were the most common orthopedic injuries related to motorcycle crashes. Use of helmets among motorcyclists needs to be reinforced to prevent head injuries. We suggest motorcyclists to use protective clothes and motorcycle sidebars to prevent fractures of the lower limb. Further research is needed to determine the type of fractures, type of head injuries, surgical interventions required, and morbidity and mortality in motorcycle-related injuries and whether designing separate lanes for motorcycles will reduce the burden of these injuries on the healthcare system.

INTRODUCTION: Synovial chondromatosis is a benign mono-articular arthropathy affecting synovial joints. It mostly affects knee joint, followed by hip, elbow and wrist and is rarely reported for shoulder joint. The exact pathogenesis is not known. Usual symptoms are pain, difficulty in movement due to mechanical obstruction. The classic treatment is arthrotomy, removal of chondromatoid loose bodies and synovectomy. With recent advances arthroscopic removal of the chondromatoid loose bodies is a good option with relatively better post op rehabilitation and faster recovery. CASE PRESENTATION: 20 years old gentleman presented to clinic with history of pain in right shoulder for 2 years and decreased range of motion. There was no history of trauma or fever. Work up done and diagnosed with synovial chondromatosis. Arthroscopic removal of chondromatoid loose bodies and synovectomy was done. More than 120 loose bodies were removed. On two (2) years follow-up patient is pain free and having full range of motion at right shoulder joint. DISCUSSION: Synovial chondromatosis is rare in shoulder joint. The Primary synovial chondromatosis of unknown etiology, and secondary synovial chondromatosis due to degenerative joint disease. Classic treatment is arthrotomy and synovectomy. With recent advances, arthroscopic removal of loose bodies and synovectomy is also a good option for its treatment. In literature only few cases have been reported treated with arthroscopic removal of loose bodies and synovectomy. CONCLUSION: Arthroscopic treatment of synovial chondromatosis is a good option if expertise is available. It causes less surgical trauma, better visualization during surgery, early recovery.

ABSTRACT

Ultrasonography (US) is a safe and noninvasive imaging modality that is gaining popularity in different medical and surgical fields. Its introduction in musculoskeletal and sports medicine has taken this advanced subspecialty to a higher level. It has the advantage over other imaging techniques with regards to ease of use, availability, comfort and cost. Not to mention, in terms of safety profile, patients are not exposed to radiations, like in x-rays, and it can be performed on patients with metal or pacemaker implants, which are contraindicated in MRI. Standard diagnostic sonography doesn't have any known harmful effects on humans. In this article we will discuss the role of ultrasound in sports medicine, highlighting the diagnostic and interventional indications, uses and limitations.

ABSTRACT

Surgical techniques for carpal tunnel release are constantly evolving to reduce complications. This retrospective study was planned to identify the outcome and complications associated with a new operating technique for release of carpal tunnel using two incisions. It was conducted at the Aga Khan University Hospital, Karachi, and comprised patients undergoing surgical release of carpal tunnel syndrome (CTS) between January 2011 and December 2014. Of the 54 patients,38(70.4%) cases were of right-sided CTS. The mean operating time was 12.5±4.9 minutes. Complete relief from symptoms was observed in all the patients and the only complication noted was superficial infection in 2(3.7%) patients.The outcomes and complications associated with this technique were comparable with other standard techniques. No major complication (e.g. neurovascular injury) was reported, which showed that this technique was safe and had no additional risks. Therefore, it can be used as an alternative to endoscopic release which is expensive and requires special training and equipment.

ABSTRACT

OBJECTIVE: To compare mean knee flexion in patients on continuous passive motion and those without it after total knee arthroplasty. METHODS: The randomised controlled study was conducted at Aga Khan University Hospital, from July 2013 to June 2014, and comprised patients who underwent total knee arthroplasty. Patients were randomly assigned to either group, with Group A receiving standardised physiotherapy from 1st postoperative day, and Group B receiving physiotherapy and one hour of continuous passive motion twice a day from 1st postoperative day until discharge. Outcome assessment was done on the day of discharge. RESULTS: Of the 76 patients, there were 38(50%) in each group. There were 61(80%) women and 15(20%) men, with a mean age of 65.5±7.9 years in Group A and 61.6±9.1 years in Group B. The mean preoperative knee flexion in Group A was 90.3±13.2° and in Group B it was 96.9±11.5°. Mean maximum flexion at the time of discharge was 96.3±5.7° in Group A and 94.3±8.4° in Group B (p=0.22). The mean length of stay in Group A was 6.1±1.4 days and in Group B it was 8.6±2.4 days (p=0.01). CONCLUSIONS: Continuous passive motion had no influence on knee range of motion after total knee arthroplasty at the time of discharge.
