ABSTRACT

BACKGROUND: The impact of various lifestyles on psychological well-being (PWB) remains under-studied. We aimed to explore the cross-sectional association between daily screen use (television, tablet and mobile phone) and PWB within the SUN cohort. METHODS: PWB was assessed using the 29-item Ryff scale (ranging from 29 to 174), and participants with scores >75th percentile were considered as having optimal PWB. Participants were categorized based on their self-reported weekly screen usage hours. Postestimation logistic regression models assessing the prevalence likelihood of not achieving optimal PWB were adjusted for sociodemographic, psychological, personality and lifestyles factors. Isotemporal substitution models explored the potential impact on PWB resulting from replacing 1 h/day of screen time with 1 h/day of exercise. RESULTS: The study included 3051 participants (55.8% women, mean age 57.3 ± 11.1 years, mean Ryff's score: 139.1 ± 17.4 points). Daily screen use for ≥2 h was associated with a higher prevalence likelihood of not achieving an optimal PWB (Prevalence Ratio [PR]:1.09; 95% CI:1.01-1.18). Among PWB dimensions, screen use ≥2 h/day was linked to an increased likelihood of not achieving optimal scores in environmental mastery (PR:1.11; 95% CI:1.02-1.20), life purpose (PR:1.10; 95% CI:1.02-1.18), and personal growth (PR:1.09; 95% CI:1.01-1.18). Replacing 1 h of daily screens time with 1 h of exercise may lead to potential improvements in environmental mastery (Odds Ratio [OR]:0.87; 95% CI:0.76-0.99), purpose in life (OR:0.86; 95% CI: 0.76-0.98), personal growth (OR:0.84; 95% CI:0.73-0.96) and positive interpersonal relationships (OR:0.86; 95% CI:0.75-0.99). CONCLUSIONS: These findings highlight the importance of reducing screen use activities and increasing physical exercise for achieving optimal PWB.

ABSTRACT

La pandemia de COVID-19 ha supuesto un desafío para los sistemas sanitarios, incluida la atención a la salud mental. Ha sido necesaria la adaptación constante de la práctica asistencial habitual según la situación epidemiológica poblacional. Se recoge la actividad asistencial prestadapor la Red de Salud Mental de Navarra durante los años 2019, 2020 y 2021. Se registra una disminución en el número de consultas ambulatorias realizadas durante 2020 y su recuperación en2021, con un incremento de la demanda en la población femenina. Se han producido variacionesen la ocupación y estancias medias de los recursos de hospitalización, así como una reestructuración funcional y estructural. En el ámbito residencial se produjo el fallecimiento del 9,8% delos usuarios debido al COVID-19. Las tasas de suicidio durante 2020 se mantuvieron similaresa 2019, incrementándose un 36% en 2021. La pandemia ha impactado en la continuidad de losplanes de tratamiento, en la evolución clínica, psicopatológica y emocional de las personasatendidas. Las repercusiones negativas pueden extenderse a lo largo de los próximos años. Porello, será fundamental el desarrollo de estrategias sanitarias, sociales y de salud pública dirigidas a la promoción de la salud y la prevención de los problemas de salud mental.(AU)

ABSTRACT

Los trastornos disociativos son un problema de salud mental frecuentemente oculto e ignorado. Entre todos ellos, el trastorno de identidad disociativo (TID) constituye la patología más grave y compleja. Su curso habitualmente es crónico, y se caracteriza principalmente por una alteración en la memoria autobiográfica y del sentido de identidad única. El TID suele originarse en el contexto de una experiencia traumática grave durante la infancia. Presentamos el caso de un adolescente de 16 años ingresado en una unidad de hospitalización psiquiátrica. A través de este caso, señalamos los síntomas propios de este trastorno, los factores de riesgo que pueden favorecer el desarrollo del trastorno y describimos, de forma resumida, el abordaje psicoterapéutico indicado para esta patología. Asimismo, mostramos los resultados obtenidos en diferentes valoraciones neuropsicológicas (Escala de Inteligencia Wechsler para Niños, Test de Aprendizaje Verbal España Complutense Infantil, Inventario Clínico para adolescentes de Millon e Inventario Multidimensional de Disociación)


Dissociative disorders constitute a frequently hidden and neglected mental health problem. Among all of them, the dissociative identity disorder (DID) is the most serious and complex pathology. DID is a chronic condition, characterized by disruption in autobiographical memory and the sense of having a unified identity. DID usually originates in the context of severe trauma during the patient's childhood. We report the case of a 16 year old boy referred to our Psychiatric inpatient unit. Across this report, we highlight the main symptoms of this disorder, the risk factors for the development of the disorder and a summarized form of the psychotherapeutic approach. Likewise, we also show the results obtained in different neuropsychological assessments (The Wechsler Intelligence Scale for Children, The Spanish-Complutensian Verbal Learning test for children, the Millon Adolescent Clinical Inventory and the Multidimensional Inventory of Dissociation)
