ABSTRACT

Pulmonary function is usually assessed by measuring Vital Capacity (VC) using equipment such as a spirometer or ventilometer, but these are not always available to the population, as they are relatively expensive tests, difficult to transport and require trained professionals. However, the single breath counting technique (SBCT) appears as a possible alternative to respiratory function tests, to help in the pathophysiological understanding of lung diseases. The objective is to verify the applicability of the SBCT as a parameter for evaluating VC. This is a systematic review registered in the International Prospective Register of Systematic Reviews (CRD42023383706) and used for PubMed®, Scientific Electronic Library Online, LILACS, EMBASE, and Web of Science databases of articles published until January 2023. Methodological quality regarding the risk of bias was assessed using Quality Assessment of Diagnostic Accuracy Studies-2 and National Institutes of Health tools. Eleven of a total of 574 studies were included, of these, nine showed a correlation between VC and SBCT (weak in healthy, moderate in neuromuscular and strong in hospitalized patients). One study of hospitalized patients accurately identified a count value of 21 for a VC of 20 ml kg-1(Sensitivity = 94% and Specificity = 77%), and another estimated a count lower than 41 for a VC below 80% of predicted in patients with neuromuscular dystrophy (Sensitivity = 89% and Specificity = 62%), and another showed good intra and inter-examiner reproducibility in young, adult, and elderly populations. A meta-analysis of three studies showed a moderate correlation in subjects with neuromuscular diseases (r= 0.62, 95% CI = 0.52-0.71,p< 0.01). A high risk of bias was identified regarding the justification of the sample size and blinding of the evaluators. SBCT has been presented as an alternative to assess VC in the absence of specific equipment. There is a clear relationship between SBCT and VC, especially in neuromuscular and hospitalized individuals. New validation studies conducted with greater control of potential bias risks are necessary.

ABSTRACT

OBJECTIVE: To evaluate whether the nature and severity of non-A-E severe acute hepatitis in children noted by the World Health Organization from late 2021 through early 2022 was indeed increased in 2021-2022 compared with prior years. STUDY DESIGN: We performed a single-center, retrospective study to track the etiology and outcomes of children with non-A-E severe acute hepatitis in 2021-2022 compared with the prior 3-year periods (2018-2019, 2019-2020, and 2020-2021). We queried electronic medical records of children ≤16 years of age with alanine or aspartate aminotransferase levels of >500 IU. Data were analyzed for the periods of October 1, 2021, to May 1, 2022, and compared with the same time periods in 2018-2021. RESULTS: Of 107 children meeting entry criteria, 82 cases occurred from October to May of 2018-2022. The average annual case number was 16.3 in 2018-2021 compared with a 2-fold increase (to 33) in 2021-2022 (P = .0054). Analyses of etiologies showed that this increase was associated with a higher number of children who tested positive for viruses (n = 16) when compared with the average of 3.7 for 2018-2021 (P = .018). Adenovirus (26.1%) and severe acute respiratory syndrome coronavirus-2 (10.3%) were the most frequently detected viruses in 2021-2022. Despite evidence of acute liver failure in 37.8% of children in the entire cohort and in 47% of those with viral infection, the overall survival rate was high at 91.4% and 88.9%, respectively. CONCLUSIONS: The number of children with severe acute hepatitis in our center increased from 2021 to May 2022, with a greater frequency of cases associated with adenovirus, yet transplant-free survival remains high.

ABSTRACT

The mechanisms of pathogenesis of acute kidney injury (AKI) in snakebites is multifactorial and involves hemodynamic disturbances, with release of free radical causing cytotoxic effects. The phosphodiesterase-3 (PDE3) inhibitor, Cilostazol, has been reported to provide protection against renal oxidative stress. OBJECTIVE: We evaluated the protective effects of cilostazol against Bothrops alternatus snake venom (BaV)-induced nephrotoxicity. METHODS: Wistar rat kidneys (n = 6, 260-300 g) were isolated and perfused with Krebs-Henseleit solution containing 6 g/100 mL of bovine serum albumin. After 30 min, the kidneys were perfused with BaV to a final concentration of 1 and 3 µg/mL, and subsequently evaluated for perfusion pressure (PP), renal vascular resistance (RVR), urinary flow (UF), glomerular filtration rate (GFR), and percentage of electrolyte tubular sodium and chloride transport (%TNa+, %TCl-). Oxidative stress and renal histological analyses were performed. RESULTS: BaV caused a reduction in all the evaluated renal parameters (PP, RVR, GFR, UF, %TNa+, and %TCl-). Although only the effects on PP and UF were reversed with cilostazol treatment, the decrease in the malondialdehyde levels, without changes in glutathione levels, further reduced the venom-induced renal tissue changes. CONCLUSION: Our data suggest that PDE3 is involved in BaV-induced nephrotoxicity, as cilostazol administration significantly ameliorated these effects.

ABSTRACT

O objetivo do estudo foi analisar a tendência temporal e fatores associados à participação nas aulas de educação física de estudantes de Pernambuco. Estudo de tendência temporal composto por três levantamentos de abrangência estadual. Foram entrevistados 4.207 estudantes em 2006, 6.264 em 2011 e 6.002 em 2016, por meio do questionário "Global School-based Student Health Survey" (GSHS). O desfecho foi a participação nas aulas de Educação Física e as exposições foram as informações sociodemográficas. A análise bivariada foi realizada mediante teste Qui-quadrado e as análises multivariadas por regressão logística binária. Observou-se um aumento na prevalência de participação nas aulas de Educação Física (2006: 35%, 2011: 74% e 2016: 81,2%) destacando o aumento significativo no sexo masculino Δ% (2006-2016) = 123,5%. Ser do sexo masculino, estudar no período integral e ser filho de mães que estudaram, independente do tempo de estudo foi associado a maior participação nas aulas de educação física, assim como residir nas regiões do Agreste, Sertão e Sertão do São Francisco. Apesar do aumento na participação nas aulas de Educação Física em Pernambuco, a garantia deste componente curricular deve ser fortalecida, levando em consideração os grupos de risco para que aumente a participação nas aulas desses grupos


The aim of the study was to analyze the temporal trend and factors associated with participation in physical education classes in Pernambuco. The temporal trend study was composed of three surveys. A total of 4,207 students were interviewed in 2006, 6,264 in 2011 and 6,002 in 2016, using the "Global School-based Stu-dent Health Survey" (GSHS) questionnaire. The outcome was participation in Physical Education classes and the independent variables were sociodemographic information. The bivariate analysis was performed using the chi-square test and the multivariate analysis using binary logistic regression. There was an increase in the prevalence of participation in Physical Education classes (2006: 35%, 2011: 74% and 2016: 81.2%), highlighting the significant increase in males Δ% (2006-2016) = 123.5%. Male, studying full-time and being the son of mothers who studied was associated with greater participation in physical education classes, as well as living in the regions of Agreste, Sertão and Sertão do São Francisco. Despite the increase in par-ticipation in Physical Education classes in Pernambuco, the guarantee of this curricular component must be strengthened, considering risk groups in order to increase participation in classes for these groups

BACKGROUND: Children's physical growth (PG) and body composition (BC) can be influenced by birth weight and type of delivery. AIM: To longitudinally analyze the dynamics of PG and BC of children from 5 to 9 years; to investigate the inter-individual differences according to age, sex, BW, and type of delivery across the following years of the study. SUBJECTS AND METHODS: A total of 1236 children (597 boys) were evaluated at 5-years of age and followed annually until 9-years. PG and BC measurements were evaluated. Multilevel modeling was used. RESULTS: Annual increments were observed (p < .001). Girls presented lower height and fat-free-mass but higher %BF (p < .001). Distinct trajectories between the sexes were observed for height (p < .001). Low-birth-weight children presented lower height, body mass, and fat-free-mass (p < .001), but the interaction between velocity of growth and BC was significant only in height (p < .05). Children born by had lower height, body mass, and %BF, and gained less body mass per year than those born by vaginal delivery (p < .05). Significant inter-individual differences were observed at 5-years of age and in their trajectories, except for fat-free-mass (p < .01). CONCLUSION: There are differences in the dynamics of PG and BC, low-birth-weight and type of delivery influence the dynamics of PG during this interval of ages.

Abstract Objective To evaluate the role of social support in the association between physical activity (PA) in its various domains and depressive symptoms in older adults. Methods This was a cross-sectional study involving 399 older adults seen at basic health units in the municipality of Recife, state of Pernambuco, northeastern Brazil. Linear regression was implemented using the forward method to assess the association between PA and its domains and depressive symptoms, as well as to test the possible moderating component of social support in this association. Results Of the total sample, 17.6% reported not practicing PA; 6.7% presented depressive symptoms, with a mean score of 1.59 points. A total of 18.6% of the older adults assessed reported not having social support. Older adults who did not practice total, transportation or domestic PA showed increased scores of depressive symptoms when compared to older adults who practiced PA in these domains. Older adults with social support also had fewer depressive symptoms. Practicing domestic, leisure and total PA, and/or having social support, decreased depressive symptoms in older adults. Having social support or practicing PA decreased depressive symptoms in older adults in a similar way. Conclusions Older adults with social support and who practiced PA, especially domestic and total PA, presented the lowest scores of depressive symptoms. The data showed the importance of providing elements to strengthen social support and PA practice in older adults living in poor communities as a strategy to prevent and reduce depressive symptoms.

AIM: The aim of this study was to analyze the temporal trend of psychosocial stress and its association between exposure to violence in adolescents. METHODS: This is a cross-sectional study of temporal trends based on three school-based and statewide surveys with regular intervals of five years. The target population consisted of high school students from public schools. A total of 4,207 adolescents were evaluated in 2006, 6,264 in 2011, and 6,002 in 2016. The psychosocial stress indicator was the dependent study variable and was analyzed according to questions: a) Feeling of loneliness; b) Feeling of sadness; c) Difficulty sleeping due to worry; d) Number of close friends; e) Suicide ideation. Exposure variables were exposure to physical violence, involvement in fights and bullying. Logistic and linear regressions were used to verify the temporal trend of psychosocial stress, and to analyze the association between exposure of violence and psychosocial stress. RESULTS: Regardless of gender, the psychosocial stress score increasing trend between 2006 and 2016. The girls showed a variation in the feeling of loneliness from 18.7% to 22.8%, and suicide ideation increased from 10.4% to 14.2% between 2006 and 2016. All types of violence were associated with greater psychosocial stress for both boys and girls. CONCLUSION: Adolescents had a higher prevalence of feelings of loneliness, difficulty sleeping due to worry and suicide ideation during the period from 2006 to 2016, regardless of gender. Exposure to violence is associated with psychosocial stress in both genders, however, the strongest associations occurred in girls.

ABSTRACT

To investigate the prevalence of alcohol consumption and the factors associated with binge drinking behavior amongadolescents in the state of Pernambuco, Brazil. A cross-sectional study with a probabilistic sample of 6,002 adolescents composed of students from public high schools in Pernambuco, Brazil, aged 14 to 19 years. Binge drinking behavior was used as a dependent variable. The following were evaluated as possible risk factors: sociodemographic variables (gender, maternal education, marital status, work, religion); indicators of psychosocial stress (feelings of sadness; suicidal thoughts; feelings of loneliness;friends) and health risk behaviors (physical inactivity; number of sexual partners; drug use; and involvement in fights in the last year). Logistic regression was used to estimate odds ratios (OR). A total of 6,002 adolescents participated, with 75% reporting having consumed alcoholic beverages at least once in their lives. The prevalence of binge drinking in the last month was 17%. Adolescents without a religion had 81% (95%CI: 1.51; 2.16) more chance of consuming alcohol in excess. Adolescents who felt sad were 29% more likely to binge drink (95%CI: 1.08; 1.54), and adolescents who had already thought about suicide had 41% (95%CI: 1.13; 1.76) more chance to drink excessively. Adolescents who used illicit drugs had 4.6 times the chance (95%CI: 3.51; 6.17)of excessive drinking. Adolescents who engaged in four or more fights during the year were twice as likely to binge drink as those who did not fight (95%CI: 1.36; 2.88). The prevalence of alcohol consumption and binge drinking behavior was high. Sociodemographic factors, psychosocial stress and health risk behaviors were associated with binge drinking.

ABSTRACT

OBJECTIVE: To determine predictors of native liver survival (NLS) in children and adolescents with autoimmune hepatitis (AIH). STUDY DESIGN: The medical records of children and adolescents with AIH were reviewed. A questionnaire was used to collect data on clinical presentation, biochemical and histologic findings, and treatment. RESULTS: A total of 819 patients were included, 89.6% with AIH-1 and 10.4% with AIH-2. The median age (months) at onset was 108 (min 6; max 210; IQR 59). The female sex was predominant (75.8%). The overall survival was 93.0%, with an NLS of 89.9%; 4.6% underwent liver transplantation. The risk of death or liver transplantation during follow-up was 3.2 times greater in patients with AIH-1 (P = .024). Greater levels of aspartate aminotransferase, alanine aminotransferase, serum albumin, platelet, and normal international normalized ratio at the initial presentation were associated with longer NLS (P = .046, P = .006, P < .001, P = .001, and P = .019, respectively). Normal C3 levels was associated with longer NLS (P = .017), with a chance of death or liver transplantation during follow-up being 3.4 times greater in patients with C3 below normal. Death or liver transplantation during follow-up was 2.8 times greater in patients with associated sclerosing cholangitis (P = .046). Complete remission favored NLS (P < .001), with a risk of death or liver transplantation 11.7 times greater for patients not achieving remission. CONCLUSIONS: The best predictors of NLS in children and adolescents with AIH were the AIH-2 subtype, a normal C3 at diagnosis, remission during treatment, and normal a cholangiogram during the disease course.

ABSTRACT

We evaluated the association between weight status and motor competence from preschool age (3-5 years of age) until middle childhood (7-9 years of age). Longitudinal study with three to five-year-old preschool children (n = 1155) enrolled in public and private preschools in Recife, Brazil. Children were followed twice (2010, 2012, and 2014) for four years. Köperkoordinationstest für kinder (KTK) assessed the children's motor competence (KTK Motor Quotient). Weight status (underweight, normal weight, overweight, and obesity) was classified according to the children's sex and age. Preschool children with normal weight exhibited higher motor competence at 5-7 years of age compared to preschool children with overweight (+3.73 MQ, P = .03) and obesity (+5.09 MQ, P < .01). Preschool children with normal weight presented higher motor competence at 7-9 years of age compared to their peers with overweight (+6.00 MQ, P = .03) and obesity (+5.88 MQ, P = .01). Children with normal weight at 5-7 years of age presented higher motor competence at 7-9 years of age compared to their peers with overweight (+3.33 MQ, P = .02) and obesity (+4.00 MQ, P = .02). Independent of the childhood phase and extension of the period evaluated (2- or 4-year period), children who had excessive weight (overweight or obesity) and changed their weight status to underweight or normal weight presented similar motor competence compared to children who continued underweight or normal weight. Weight status already at preschool age is an important predictor of the children's motor competence until middle childhood. Interventions improving the children's weight status, already at preschool age, might impact their motor competence development positively.

O objetivo foi estimar a prevalência e analisar a associação do consumo de álcool e drogas ilícitas com indicadores de violência física em adolescentes. Estudo transversal repetido com estudantes do Ensino Médio em escolas da rede pública em Pernambuco, Brasil (2006, n = 4.207; 2011, n = 6.264). Os dados foram obtidos por questionário. Os indicadores de violência física foram: vitimização por violência física; envolvimento em brigas. As variáveis independentes foram: consumo de álcool e uso de drogas ilícitas. A regressão logística binária foi utilizada para verificar a associação entre o consumo de álcool e drogas e ter sofrido violência física. A prevalência do consumo de álcool por adolescentes que sofreram violência física reduziu em cinco anos, porém, nenhuma alteração foi constatada no uso de drogas ilícitas. As variáveis comportamentais estiveram associadas aos diferentes indicadores de violência, apresentando em geral uma tendência de aumento. Em 2006, adolescentes que usavam drogas ilícitas apresentaram uma chance 2,12 maior de sofrer violência física, e em 2011 este valor foi de 2,41 (IC95%: 1,80; 3,22). A chance de adolescentes que consumiam bebida alcoólica ter envolvimento em brigas aumentou de 1,96 (IC95%: 1,64; 2,34) em 2006, para 2,11 (IC95%: 1,78; 2,51) em 2011. O consumo de álcool pelos adolescentes que sofreram violência física diminuiu em cinco anos, entretanto, o risco de os adolescentes que consumiram bebida alcoólica ou usaram drogas ilícitas sofrerem violência física aumentou ao longo de cinco anos.


The article aimed to estimate prevalence and analyze the association between alcohol and illicit drug consumption and indicators of physical violence in adolescents. A repeated cross-sectional study was performed in public middle-school students in the State of Pernambuco, Brazil (2006, n = 4,207; 2011, n = 6,264). Data were obtained via a questionnaire. Indicators of physical violence were: victimization by physical violence and involvement in fights. The independent variables were alcohol and illicit drug consumption. Binary logistic regression was used to verify the association between alcohol and drug consumption and victimization from physical violence. Prevalence of alcohol consumption in adolescents that suffered physical violence decreased in five years, but no change appeared in the use of illicit drugs. The behavioral variables were associated with different indicators of violence, generally presenting an upward trend. In 2006, adolescents that used illicit drugs showed 2.12 higher odds of suffering physical violence, and in 2011 this value was 2.41 (95%CI: 1.80; 3.22). The odds of adolescents that consumed alcoholic beverages being involved in fights increased from 1.96 in 2006 (95%CI: 1.64; 2.34) to 2.11 in 2011 (95%CI: 1.78; 2.51). Alcohol consumption by adolescents that suffered physical violence decreased in five years, but the risk of adolescents that consumed alcohol or used illicit drugs and suffered physical violence increased over the course of five years.


El objetivo fue estimar la prevalencia y analizar la asociación del consumo de alcohol y drogas ilícitas con indicadores de violencia física en adolescentes. Se realizó un estudio transversal, repetido con estudiantes de enseñanza media en escuelas de la red pública en Pernambuco, Brasil (2006, n = 4.207; 2011, n = 6.264). Los datos se obtuvieron mediante cuestionario. Los indicadores de violencia física fueron: victimización por violencia física; implicación en peleas. Las variables independientes fueron consumo de alcohol y uso de drogas ilícitas. La regresión logística binaria fue utilizada para verificar la asociación entre consumo de alcohol y drogas y haber sufrido violencia física. La prevalencia del consumo de alcohol en adolescentes que sufrieron violencia física se redujo en cinco años, a pesar de que no se constató ninguna alteración en el uso de drogas ilícitas. Las variables comportamentales estuvieron asociadas a los diferentes indicadores de violencia, presentando en general una tendencia de aumento. En 2006, adolescentes que usaban drogas ilícitas presentaron una oportunidad 2,12 mayor de sufrir violencia física y en 2011 ese valor fue de 2,41 (IC95%: 1,80; 3,22). La oportunidad de adolescentes que consumían bebidas alcohólicas de que tuvieran implicación en peleas aumentó de 1,96 (IC95%: 1,64; 2,34) en 2006, a 2,11 (IC95% 1,78; 2,51) en 2011. The objective of this study was to identify trends in suicidal ideation by sex and ascertain factors associated with this outcome. Methods: Secular trend study with statewide coverage conducted at 5-year intervals, with 4,207 adolescents (2006), 6,264 adolescents (2011) and 6,026 adolescents (2016). Logistic regression was used to evaluate the secular trend of suicidal ideation. Multilevel logistic regressions evaluated the factors associated with suicidal ideation in the survey conducted in 2016. Results: There was a positive trend in suicidal ideation prevalence in 2016 compared to the prevalence in 2006 and 2011, in both boys and girls. Low social support, poor sleep quality, and low parental supervision were associated with suicidal ideation in boys and girls. Exposure to violence and bullying was associated with suicidal ideation only in girls. TV time and computer and videogame time were not associated with suicidal ideation in boys or girls. Conclusion: There is an alarming trend of increased suicidal ideation in adolescents. Several dimensions were associated with suicidal ideation in adolescents, especially social support, sleep quality, and parental supervision.

ABSTRACT

In Brazil, physical activity is a priority area in health promotion in the Brazilian Unified National Health System (SUS). The study aims to describe users' profiles and the barriers and facilitators in basic healthcare for participation in programs to promote physical activity. This was a statewide cross-sectional study in municipalities (counties) in Pernambuco State. Between 2 and 21 individuals were interviewed per municipality, using a previous tested instrument, validated in the following dimensions: sociodemographic; health status; participation; information on the program; reasons for participation; facilitators; barriers; and preference of activities. Descriptive and inferential analyses (chi-square) were performed. The sample consisted of 1,153 users, 35.9% of whom between 41-59 years of age; 90.1% lived in urban areas; 58.2% had participated in physical activities for at least a year. Weekly frequency of 3-4 days was 44.9%, and 71.1% practiced activities of an hour or more. The results showed that 40% of the barriers to participation in physical activity programs and 77.5% of the facilitators belonged to the intrapersonal domain. The most prevalent barrier was "current health condition" and the facilitator was the desire "to be healthier". Women perceived more barriers than men. The conclusion is that barriers and facilitators from the intrapersonal domain, related to health, are the main factors involved in users' engagement and retention in programs and interventions to promote physical activity, developed in basic healthcare services in the state of Pernambuco.

Observou-se que 40% das barreiras para a participação nos programas de atividade física e 77,5% dos facilitadores relatados foram de domínio intrapessoal. A barreira mais prevalente foi "condição atual de saúde", e o facilitador foi "ter uma condição melhor de saúde". As mulheres percebem mais barreiras do que os homens. Conclui-se que as barreiras e os facilitadores de domínio intrapessoal, relacionados com a saúde, são os fatores envolvidos na manutenção e no engajamento dos usuários dos programas e intervenções para promoção da atividade física desenvolvidos pela atenção básica à saúde do Estado de Pernambuco.

ABSTRACT

OBJECTIVES: The aim of the study was to assess neurodevelopmental outcomes among children with biliary atresia (BA) surviving with their native liver at ages 3 to 12 years and evaluate variables that associate with neurodevelopment. METHODS: Participants (ages 3-12 years) in a prospective, longitudinal, multicenter study underwent neurodevelopmental testing with Weschler Preschool and Primary Scale of Intelligence, 3rd edition (WPPSI-III, ages 3-5 years) and Weschler Intelligence Scale for Children, 4th edition (WISC-IV, ages 6-12 years). Continuous scores were analyzed using Kolmogorov-Smironov tests compared with a normal distribution (mean = 100 ±â€Š15). Effect of covariates on Full-Scale Intelligence Quotient (FSIQ) was analyzed using linear regression. RESULTS: Ninety-three participants completed 164 WPPSI-III (mean age 3.9) and 51 WISC-IV (mean age 6.9) tests. WPPSI-III FSIQ (104 ±â€Š14, P < 0.02), Verbal IQ (106 ±â€Š14, P < 0.001), and General Language Composite (107 ±â€Š16, P < 0.001) distributions were shifted higher compared with test norms. WISC-IV FSIQ (105 ±â€Š12, P < 0.01), Perceptual Reasoning Index (107 ±â€Š12, P < 0.01), and Processing Speed Index (105 ±â€Š10, P < 0.02) also shifted upwards. In univariate and multivariable analysis, parent education (P < 0.01) was a significant predictor of FSIQ on WPPSI-III and positively associated with WISC-IV FSIQ. Male sex and higher total bilirubin and gamma glutamyl transferase (GGT) predicted lower WPPSI-III FSIQ. Portal hypertension was predictive of lower WISC-IV FSIQ. CONCLUSIONS: This cohort of children with BA and native liver did not demonstrate higher prevalence of neurodevelopmental delays. Markers of advanced liver disease (higher total bilirubin and GGT for age ≤5 years; portal hypertension for age ≥6) correlate with lower FSIQ and may identify a vulnerable subset of patients who would benefit from intervention.

ABSTRACT

BACKGROUND: The aim of the current study was to evaluate the cross-sectional synergic relationship among social anxiety, poor sleep quality, depressive symptoms and body fatness. METHODS: A cross-sectional study was performed with 1,242 adolescents from a north-eastern district in Brazil. It was used the Social Anxiety Scale for Adolescents for symptoms of social anxiety, the Pittsburgh Sleep Quality Index for sleep quality and the Depression Scale of the Center for Epidemiologic Studies for depressive symptoms. Body fatness was the sum of triceps and subscapular skinfolds. Structural equation modelling was used to evaluate associations among these comorbidities. RESULTS: Social anxiety was directly and positively associated with depressive symptoms, poor sleep quality and body fatness (ß = 0.316 standard deviations [SD]; ß = 0.299 SD; ß = 0.072 SD, respectively). Moreover, social anxiety was indirectly associated with depressive symptoms via poor sleep quality (ß = 0.113 SD). Poor sleep quality was directly associated with depressive symptoms (ß = 0.377 SD). We did not observe a significant relationship of poor sleep quality and depressive symptoms with body fatness. LIMITATIONS: This is a cross-sectional study; therefore, we cannot infer causality in the interpretation of the findings. CONCLUSIONS: Social anxiety seemed to play pivotal role in adolescents' health, since it was associated with poor sleep quality, depressive symptoms and body fatness. Interventions targeting sleep quality, depressive symptoms and body fatness in adolescents should designate relevant effort addressing social anxiety.

OBJECTIVE: To prospectively assess the diagnostic performance of ultrasound shear wave elastography (SWE) and hepatobiliary laboratory biomarkers for discriminating biliary atresia from other causes of neonatal cholestasis. STUDY DESIGN: Forty-one patients <3 months of age with neonatal cholestasis (direct bilirubin >2 mg/dL) and possible biliary atresia were prospectively enrolled. Both 2-dimensional (2D) and point ultrasound SWE were performed prior to knowing the final diagnosis. Median 2D (8) and point (10) shear wave speed measurements were calculated for each subject and used for analyses. The Mann-Whitney U test was used to compare shear wave speed and laboratory measurements between patients with and without biliary atresia. Receiver operating characteristic curve analyses and multivariable logistic regression were used to evaluate diagnostic performance. RESULTS: Thirteen subjects (31.7%) were diagnosed with biliary atresia, and 28 subjects (68.3%) were diagnosed with other causes of neonatal cholestasis. Median age at the time of ultrasound SWE was 37 days. Median 2D (2.08 vs 1.49 m/s, P = .0001) and point (1.95 vs 1.21 m/s, P = .0014) ultrasound SWE measurements were significantly different between subjects with and without biliary atresia. Using a cut-off value of >1.84 m/s, 2D ultrasound SWE had a sensitivity = 92.3%, specificity = 78.6%, and area under the receiver operating characteristic curve (AuROC) of 0.89 (P < .0001). Using a cut-off value of >320 (U/L), gamma-glutamyl transferase (GGT) had a sensitivity = 100.0%, specificity = 77.8%, and AuROC of 0.85 (P < .0001). Multivariable logistic regression demonstrated an AuROC of 0.93 (P < .0001), with 2 significant covariates (2D ultrasound SWE [OR = 23.06, P = .01]; GGT [OR = 1.003, P = .036]). CONCLUSIONS: Ultrasound SWE and GGT can help discriminate biliary atresia from other causes of neonatal cholestasis.

OBJECTIVES: Biliary atresia (BA) is a progressive neonatal fibroinflammatory cholangiopathy. We hypothesized that intravenous immunoglobulin (IVIg) would be safe, feasible, acceptable, and efficacious for the treatment of BA. The primary objective of this study was to establish the feasibility, acceptability, and safety profile of IVIg administration after hepatoportoenterostomy (HPE) in BA. The secondary objective was to determine the treatment efficacy of IVIg based on good bile drainage and survival with the native liver. METHODS: A multicenter, prospective, open-labeled, phase I/IIA trial of IVIg was conducted, with 1 g/kg/dose of IVIg infused at 3-5, 30, and 60 days post-HPE, and subjects followed for 360 days post-HPE. Twenty-nine participants completed the study. RESULTS: Administration of IVIg infusions was feasible and acceptable in 79%. None of the serious adverse events (SAEs) were directly related to IVIg infusions; however, 90% of participants had an SAE. Compared with a historical placebo-arm group, there was no significant increase in the proportion of IVIg participants with a serum total bilirubin <1.5 mg/dL at 90, 180, or 360 days post-HPE. Survival with the native liver in the IVIg participants showed no significant benefit over the historical placebo arm, with a difference at 360 days of -11.9% (IVIg: 58.6%, placebo: 70.5%; 90% UCB: 2.1%; P > 0.05). CONCLUSIONS: Although IVIg infusions in infants with BA post-HPE were feasible, acceptable and safe, there was no trend to lower bilirubin levels or improved 360-day survival with the native liver.
