ABSTRACT

OBJECTIVES: This prospective clinical study aimed to determine the influence of oral prosthodontic rehabilitation with partial removable dentures and simplified dietary advice on glycaemic control, nutritional status and oral health-related quality of life (OHRQoL) of older adults with type 2 diabetes. BACKGROUND: Individuals with tooth loss who did not wear complete or partial dentures are more likely to be at nutritional risk, suggesting that using dental prostheses would benefit the re-establishment of an adequate nutritional status and potentially improve quality of life. Nutritional therapy is essential for diabetes prevention, treatment and management, favouring glycaemic control. The literature provides little evidence on the contribution of partial removable dentures to improving diabetes control, nutritional profile and satisfaction in older adults, especially those with type 2 diabetes. MATERIALS AND METHODS: Older individuals diagnosed with type 2 diabetes mellitus, aged 60 years or older and needing oral rehabilitation with partial removable dentures were eligible for this prospective study. The primary outcome measure was glycaemic control, measured by glycated haemoglobin (HbA1c) levels. The Oral Health Impact Profile (OHIP-14) questionnaire and the Mini Nutritional Assessment short-form (MNA-SF) represented secondary outcome measures. These measures were assessed at baseline and 12 months of follow-up after prosthesis delivery, combined with simple dietary advice in pamphlet form. The data were analysed using the Wilcoxon matched-pairs test. RESULTS: Forty-four patients who met the inclusion criteria and required treatment with removable partial dentures were selected for this study, which was carried out for a year. During this period, seven participants declined to participate because they moved to another city. Hence, a final sample of 37 participants (16 men and 21 women, average age of 65.8 years, ranging from 60 to 83 years) were included in the study. No laboratory parameter (HbA1c levels) changed significantly during the investigation. The number of older adults at risk of malnutrition was significantly lower at 12 months than at baseline. Furthermore, nutritional status significantly improved 12 months after prosthodontic treatment combined with dietary advice, and the effect sizes were large. OHRQoL significantly improved after prosthodontic therapy combined with dietary advice. CONCLUSIONS: Prosthodontic treatment with partial dentures associated with simple dietary advice did not influence glycaemic control but improved the nutritional status and OHRQoL of older adults with type 2 diabetes.

BACKGROUND: Elderly people frequently report tooth loss, affecting their nutritional status. There is a gap in the scientific evidence about the influence of prosthodontic rehabilitation on nutritional status. OBJECTIVE: To investigate the influence of prosthodontic rehabilitation combinates or not with dietary advice on nutritional status in elderly patients. METHODS: A systematic literature search was conducted in three electronic databases to identify randomized clinical trials that evaluated the effect of prosthetic rehabilitation on nutritional status in subjects aged 60 years or older, with a follow-up of at least 1 month after prosthesis rehabilitation. RESULTS: Of the 1517 articles identified in the initial search, 12 were selected for the final review. No significant difference between the types of prosthesis were found regarding the chewing by the elderly population. Although patients who received prosthetic treatment had significant improvement in chewing ability, a consistent pattern of improvement in nutritional status was not observed when they did not receive dietary advice. In addition, the association between the condition of the dentition, the masticatory performance and nutritional change of elderly patients has been found. Studies that evaluated simultaneous complete denture treatment and simple dietary advice showed an improvement of nutrient intake in elderly patients. CONCLUSIONS: Isolated prosthetic rehabilitation may not have the effect of exerting a change in nutritional status of edentulous elderly patients. In general, simultaneous dietary consulting and prosthetic treatment in combination may improve dietary habits, since masticatory capacity and efficiency are not the only factors that influence the nutritional status of a patient.

ABSTRACT

Ter um sorriso harmônico é uma das prioridades dos pacientes dentro da odontologia, que procuram os profissionais da área para melhorar nos requisitos estéticos, tanto dos dentes, quanto da gengiva. Com os diversos avanços tecnológicos, o cirurgião-dentista pôde ampliar sua capacidade de diagnóstico e prognóstico, podendo realizar um planejamento de intervenção mais criterioso e com maior possibilidade de sucesso. Por meio desta revisão de literatura integrativa propomos elencar de forma minuciosa quais são essas características que podem desarmonizar um sorriso e ainda apresentamos como restabelecer a harmonia do sorriso por meio de procedimentos e elementos odontológicos. Foi realizada uma pesquisa nos portais eletrônicos PubMed, Biblioteca Virtual em Saúde (BVS), Ebsco host e Scielo, utilizando os descritores: "smile", "periodontal plastic", "aesthetics", "black triangles" e "interdental papila", além dos seus termos relacionados em português, incluído publicações de 2019 a 2022. Foi possível observar que por meio de um quadro multifatorial, a estética e harmonia do sorriso pode ser comprometida por fatores gengivais, faciais e dentários. Assim, um planejamento assertivo deve ser realizado por um profissional capacitado, que identifique os pontos que interferem positivamente e negativamente no equilíbrio do sorriso, com o propósito de gerar soluções clínicas. Com uma abordagem multidisciplinar, pode-se ratificar que é possível restabelecer a harmonia do sorriso e conseguir a satisfação do paciente, por meio de procedimentos plásticos cirúrgicos e restauradores.


Having a harmonious smile is one of the priorities of patients in dentistry, who seek professionals in the area to improve the aesthetic requirements of both teeth and gums. With the various technological advances, the dental surgeon was able to expand his diagnostic and prognostic capacity, being able to carry out a more judicious intervention planning and with greater possibility of success. Through this integrative literature review, we propose to list in detail what are these characteristics that can disharmonize a smile and we also present how to restore the harmony of the smile through dental procedures and elements. A search was carried out in the electronic portals PubMed, Biblioteca Virtual em Saúde (BVS), Ebsco host and Scielo, using the descriptors: "smile", "periodontal plastic", "aesthetics", "black triangles" and "interdental papilla", in addition to of its related terms in Portuguese, including publications from 2017 to 2022. It was possible to observe that through a multifactorial framework, the esthetics and harmony of the smile can be compromised by gingival, facial and dental factors. Thus, assertive planning must be carried out by a trained professional, who identifies the points that interfere positively and negatively in the balance of the smile, with the purpose of generating clinical solutions. With a multidisciplinary approach, it can be confirmed that it is possible to restore the harmony of the smile and achieve patient satisfaction, through plastic surgical and restorative procedures.

ABSTRACT

OBJECTIVE: This systematic review assessed the effects of prosthetic rehabilitation with removable dentures on oral health-related quality of life (OHRQoL) in older adults. METHODS: A comprehensive electronic search was conducted in the PubMed/MEDLINE, Web of Science, and Cochrane databases to identify randomized clinical trials and prospective clinical studies. The included studies evaluated the effects of prosthetic rehabilitation on OHRQoL in patients whose mean age was > 60 years. The interventions included complete dentures, implant-retained overdentures, and removable partial prostheses, with a minimum follow-up period of 1 month after prosthetic rehabilitation. Patient-reported outcome measures, specifically OHRQoL, were the primary outcome. Risk of bias was assessed using the Cochrane RoB 2 tool for randomized clinical trials and the ROBINS-I tool for prospective clinical studies. RESULTS: Eleven articles were considered eligible for the systematic review. The findings indicated that removable dentures improved various OHRQoL domains, mainly functional limitations and physical and psychological disabilities. Retention, stability, comfort, speech, and masticatory efficiency were significantly better with implant overdentures than complete dentures, leading to higher patient satisfaction and OHRQoL. CONCLUSIONS: This systematic review suggests that prosthetic rehabilitation with removable dentures has a positive influence on OHRQoL in older patients. The findings highlight the beneficial impact of implant overdentures and fixed adhesive prostheses for enhancing functional outcomes and patient satisfaction.


OBJETIVO: Esta revisão sistemática avaliou os efeitos da reabilitação protética com próteses removíveis na qualidade de vida relacionada à saúde bucal (QVRSB) em pacientes idosos. METODOLOGIA: Foi realizada uma pesquisa eletrônica abrangente nas bases de dados United States National Library of Medicine/ Medical Literature Analysis and Retrieval System Online ­ PubMed/MEDLINE, Web of Science e Cochrane para identificar ensaios clínicos randomizados (ECR) e estudos clínicos prospectivos. Os artigos selecionados tiveram como foco avaliar o efeito da reabilitação protética na QVRSB em indivíduos com média de idade superior a 60 anos. As intervenções descritas incluíram próteses totais, overdentures suportadas por implantes e próteses parciais removíveis, com acompanhamento mínimo de um mês após a reabilitação protética. As medidas de resultados relatadas pelos pacientes, especificamente QVRSB, foram o desfecho primário. O risco de viés foi avaliado com a ferramenta Cochrane RoB 2 pare ensaios clínicos randomizados e a ferramenta ROBINS-I para estudos clínicos prospectivos. RESULTADOS: Os resultados mostraram que as próteses dentárias removíveis melhoraram vários domínios da QVRSB, principalmente limitações funcionais e físicas e deficiências psicológicas. A retenção, estabilidade, conforto, fala e eficiência mastigatória foram significativamente melhores com overdentures sobre implantes do que com próteses totais, levando a maior satisfação do paciente e QVRSB. CONCLUSÕES: Esta revisão sistemática sugere uma influência positiva da reabilitação protética com próteses removíveis na QVRSB em pacientes idosos. As descobertas destacam o impacto benéfico das overdentures sobre implantes e das próteses adesivas fixas para melhorar os resultados funcionais e a satisfação do paciente

ABSTRACT BACKGROUND AND OBJECTIVES: The primary protocol for the control of temporomandibular disorders prioritizes reversible and less invasive measures. However, conservative treatment is sometimes ineffective. Therefore, the use of hyaluronic acid has been suggested as a therapeutic alternative to verify the effectiveness of the hyaluronic acid in patients who are not responsive to the most conservative treatments, helping them in the control of pain. This article aims to perform a literature review on the efficacy of this substance in the treatment of internal changes of the temporomandibular joint. CONTENTS: The search strategy used the Pubmed portal and the Web of Science database for the last 10 years. We included articles in English that evaluated the efficacy of the hyaluronic acid in the intra-articular disorders of temporomandibular joint, and excluded articles from literature review, clinical case reports, theses, and dissertations. Fifteen studies, classified as randomized clinical trials, prospective and retrospective studies, case-control, pilot study, and systematic reviews were selected. The hyaluronic acid is of fundamental importance in the function and lubrication of joint tissues due to its high molecular weight. When degenerative and inflammatory changes are present, their concentration and molecular weight are diminished, and the injection of this acid raises these levels, which can generate pain relief. CONCLUSION: Intra-articular therapy with hyaluronic acid is effective in the reduction of symptomatologic levels and the functional restoration of the temporomandibular joint.


RESUMO JUSTIFICATIVA E OBJETIVOS: O protocolo primário de controle das disfunções temporomandibulares prioriza as medidas reversíveis e menos invasivas. Entretanto, o tratamento conservador mostra-se, algumas vezes, ineficaz, e como alternativa terapêutica, tem sido sugerido o uso de ácido hialurônico para com isso, verificar a sua efetividade em pacientes não responsivos aos tratamentos mais conservadores e poder ajudá-los no controle da dor. O objetivo deste estudo foi rever na literatura a eficácia dessa substância no tratamento das alterações internas da articulação temporomandibular. CONTEÚDO: A estratégia de busca utilizou o portal eletrônico Pubmed e a base de dados Web of Science, nos últimos 10 anos. Foram incluídos artigos em inglês que avaliaram a eficácia do ácido hialurônico nas desordens intra-articulares da articulação temporomandibular, e excluídos artigos de revisão de literatura, relatos de casos clínicos, teses e dissertações. Foram selecionados 15 estudos, classificados como ensaios clínicos randomizados, estudos prospectivos e retrospectivos, caso-controle, estudo piloto e revisões sistemáticas. O ácido hialurônico tem importância fundamental na função e lubrificação dos tecidos articulares, devido ao seu alto peso molecular. Quando alterações degenerativas e inflamatórias estão presentes, sua concentração e peso molecular estão diminuídos, e a injeção desse ácido eleva esses níveis, o que pode gerar alívio da dor. CONCLUSÃO: A terapia intra-articular com ácido hialurônico é efetiva na diminuição dos níveis sintomatológicos e no restabelecimento funcional da articulação temporomandibular.

OBJECTIVE: Several studies have focused on the relationship among serotype distribution, ethnical status and geographic populations, and periodontal conditions. Studies that have investigated the prevalence and the distribution of A. actinomycetemcomitans serotypes and the relation between the different serotypes of the bacterium and periodontal status were reviewed. MATERIAL AND METHODS: A systematic literature search for publications regarding the distribution of A. actinomycetemcomitans serotypes in subgingival samples of periodontitis patients and periodontally healthy subjects by employing polymerase chain reaction (PCR) was conducted. RESULTS: From the 85 studies identified in the first analysis, only 12 met all inclusion and exclusion criteria. Clinical isolates from diverse geographic populations with different periodontal conditions were evaluated. Serotypes a, b and c were largely found, and serotype c was the most prevalent. They were isolated from various periodontal conditions, including aggressive periodontitis. CONCLUSIONS: The available literature suggests that serotypes a, b, and c are globally dominant, serotypes d and e are rare, and the prevalence of the most recently identified serotype f is still unknown. It is widely accepted that distribution patterns of A. actinomycetemcomitans vary among subjects of different ethnicity and geographic regions. The correlation of different serotypes with various periodontal conditions remains unclear
