ABSTRACT

OBJECTIVE: The impact of polarity change on the efficiency of in vivo electroporative (EP) gene transfection was assessed in rat laryngeal muscle. STUDY DESIGN AND SETTING: High (HV) and low field voltage (LV) were combined with polarity changes to determine transfection in 5 different conditions: 1) without EP (EP[-]), 2) HV+LV (HL), 3) HV+LV followed by HV+LV with no change in polarity (HLHL unidirectional), 4) HV+LV followed by HV+LV with opposite polarity (HLHL bidirectional), 5) HV+LV followed by LV with opposite polarity (HLL bidirectional). RESULTS: HLL bidirectional sequence showed the best result with less interindividual variability and extended expression period. With the exception of repeated high voltage sequences, EP parameters were not likely to induce cell injury or inflammation. CONCLUSION: HLL bidirectional electroporative gene delivery produces high transfection rates with limited tissue trauma. SIGNIFICANCE: Bidirectional EP provides a safe and highly efficient method for therapeutic gene delivery into skeletal muscle.

ABSTRACT

Proteomic analysis was performed to construct a protein database for human laryngeal muscle. Thyroarytenoid (TA) muscle specimens were obtained from six post mortem cases within 24 h of death. Isoelectric focusing was performed by using immobilized pH gradient strips followed by 12% sodium dodecyl sulfate-polyacrylamide gel electrophoresis. Silver stained gels were then analyzed using PDQuest software to locate, quantify and match spots. Proteins were identified by matrix-assisted laser desorption/ionization-mass spectrometry on the basis of peptide mass fingerprinting following in-gel digestion with trypsin. Comparison of protein distribution between broad and narrow pH range gels demonstrated that 75% of all protein spots from human TA muscle were located within the pH range 5-8, and between mass 15-120 kDa. Based on peptide mass fingerprinting, 75 proteins were identified and classified into six functional groups. These include membrane proteins (8.5%), cytoskeletal and myofibrillar proteins (14.6%), energy production proteins (28%), proteins associated with stress responses (8.5%), and protein associated with transcription regulation (10.9%). Approximately one-third (29%) were categorized as "other proteins". This data provides an initial reference map for comparative studies of protein expression in human and laryngeal muscle. Further development of this database will provide a valuable resource for molecular analysis of normal and pathologic conditions affecting human striated muscle.

ABSTRACT

A eletroporação é uma técnica de administração de pulsos elétricos no tecido que promove a entrada de genes na célula. Utilizou-se o gene do marcador da proteína fluorescente verde, o qual foi injetado no músculo tireoaritenóideo previamente à aplicação dos pulsos. Foram estudadas 32 laringes de ratos, divididas em grupos de alta voltagem, baixa voltagem e associação de alta e baixa voltagem. Cortes histológicos foram analisados e os grupos comparados quanto a número, intensidade da fluorescência, distribuição das fibras musculares marcadas e infiltrado inflamatório. O grupo de alta e baixa voltagens foi o mais eficiente, de acordo com os parâmetros estudados.Electroporation is a technique for the application of electric pulses in a tissue that allows the entrance of genes into a cell. The gene Enhanced green fluorescent protein was injected into the thyroarythenoid muscle before the pulse application. Thirty- two rat larynges were divided into groups of high voltage, low voltage and high and low voltage together. Histological sections were analized and the groups were compared in relation to the number, fluorescent signal, distribution of the marked muscular fibers and the inflammatory pattern. ABSTRACT

Cicatricial pemphigoid is a chronic, systemic, autoimmune disease characterized by progressive bullous skin and mucous membrane lesions that tend toward scarring and involution. Manifestations of cicatricial pemphigoid include oral mucosal bullous lesions in 85 to 90% of patients, ocular mucosal lesions in 66%, nasal mucosal lesions in 15 to 23%, and laryngeal involvement in 8 to 21%. We report five cases of cicatricial pemphigoid in which all patients had ENT manifestations--specifically, oral and nasal mucosal involvement. Three of these patients also had laryngeal lesions; one of the three had a large laryngeal ulceration and bullae that caused a laryngeal stenosis and necessitated a tracheostomy. In addition to the five case reports, we also review the literature and discuss the pathogenesis, diagnosis, and treatment of this uncommon disease.

ABSTRACT

Introduçäo e objetivos: Este trabalho tem como objetivos estudar o comportamento da criança ao ser submetida a videonasofaringolaringoscopia - e a eficiência da rotina adotada pelos autores para sua realizaçäo - e descrever a rotina do exame, contribuindo para sua melhor execuçäo por outros profissionais da área médica. Forma do estudo: Prospectivo clínico randomizado. Material e Método: O presente estudo foi realizado com 105 crianças e adolescentes, cuja faixa etária variou de 1 a 15 anos, sendo a média de 7,3 anos, submetidos a endoscopia nasal e/ou faringolaríngea com endoscópio flexível. Resultados: O índice de sucesso do procedimento foi de 99,04 por cento - em apenas uma das crianças foi impossível a execuçäo da endoscopia. A ocorrência de náusea näo impediu a boa avaliaçäo da laringe, estando presente em apenas 6,7 por cento dos casos. O choro ocorreu em alguma das fases da rotina de exame em 24,76 por cento do total de pacientes: em 42,10 por cento das crianças de 0 a 5 anos (Grupo I); em 20,93 por cento das de 6 a 10 anos (Grupo II); e em 0,00 por cento dos adolescentes de 11 a 15 anos (Grupo III). A cavidade nasal foi a regiäo mais relacionada com tal ocorrência, correspondendo a 80 por cento dos casos que choraram durante o exame. Conclusäo: Ao findarmos a pesquisa, concluímos que a videonasofaringolaringoscopia flexível é muito eficiente segundo a rotina adotada e que a tolerância do paciente mostrou-se proporcional a sua idade

ABSTRACT

Os implantes dentários constituem um método terapêutico auxiliar na reabilitação oral, permitindo a fixação permanente de próteses substitutivas de um ou mais dentes. Quando realizados na arcada dentária superior, apresentam-se em intima relação com as cavidades naso-sinusais. Objetivo: O objetivo deste trabalho é apresentar caso em que foi realizado um implante agulhado e de parafuso na arcada dentária superior, com transfixação do assoalho da fossa nasal e exteriorização no interior da mesma em três pontos distintos. A paciente foi submetida à secção das porções exteriorizadas dos implantes por via endonasal com micromotor e broca diamantada. Resultados: O resultado final mostrou reintegração da mucosa nasal recobrindo os implantes, sem prejuízo da fixação da prótese. Conclusão: Os autores alertam para a possibilidade da tentativa de um tratamento que preserve o implante, nos casos de complicações menores.


Dental implants are an auxiliary method in oral rehabilitation, which allows permanent fixation of prosthesis of one or more teeth. When applied in the superior dental arcade, there is an intimate relation-with the pose and paranasal sinus. Aim: The goal of this work is to present a case submitted to neddled implants and screwed dental implants in the superior dental arcade, with transfixation of the nasal floor in thee differents points. The exteriorized portions of the implants were sectioned by endonasal approach with diamond burs. Results: The final result showed reintegration of the nasal mucosa covering the implants, without damage of the prosthesis fixation. Conclusion: In cases of lesser complications, the authors warn out the possibility of conservative treatment preserving-the implants.
