ABSTRACT

Many studies have demonstrated mercury contamination in the Amazonian ecosystem, particularly in fish, a dietary mainstay of populations in this region. The present study focused on potential health effects of this low-level methylmercury exposure. The study was carried out in a village on the Tapajós River, a tributary of the Amazon, on 91 adults inhabitants (15-81 years), whose hair mercury levels were inferior to 50 mu/g. Performance on a neurofunctional test battery and clinical manifestations of nervous system dysfunction were examined in relation to hair mercury concentrations. Near visual contrast sensitivity and manual dexterity, adjusted for age, decreased significantly with hair mercury levels (P < 0.05), while there was a tendency for muscular fatigue to increase and muscular strength to decrease in women. For the most part, clinical examinations were normal, however, hair mercury levels were significantly higher (P < 0.05) for persons who presented disorganized movements on an alternating movement task and for persons with restricted visual fields. These results suggest dose-dependent nervous system alterations at hair mercury levels below 50 micrograms/g, previously considered a threshold for clinical effects. The profile of dysfunction in this adult population is consistent with the current knowledge on methyl-mercury poisoning. The long-term implications of these findings are unknown and need to be addressed.

ABSTRACT

Mercury is being released in the Amazon in an abusive way due to goldmining activities. The Tapajós river basin was the first to be intensively exploited in the modern Amazon gold rush. Fish and hair samples as the best indicators of human methylmercury contamination were investigated in the main cities and villages along the Tapajós river basin. The upper basin has typical fish fauna with much larger carnivorous fish with higher mercury levels reaching an average value of 0.69 microgram.g-1 wet wt. in 43 fish. This was accompanied by high levels in hair of the human population living in the same area. The maximum hair value reach 151 micrograms.g-1 dry wt. with two villages presenting an average value close to 25 micrograms.g-1 dry wt. An analytical laboratory intercalibration exercise was performed between Japanese and Brazilian laboratories for total mercury analysis. Critical fish, areas, and more exposed human groups are identified.

ABSTRACT

The purpose of this article is to describe health conditions in a pan-mining community, in particular the interaction of mercury contamination with other health problems. This study was carried out in a panning mine located on Rato Creek, a tributary of the Tapajó s River Basin in the municipality of Itaituba, State of Par The study population consisted of 223 individuals. An epidemiological questionnaire was applied, medical treatment was given, and biological specimens were collected. Testing of fecal samples showed that 96.1% of the individuals had parasites and 66.4% were anemic. Prevalence of hepatitis B virus was 85.0%, and 11 individuals (6.0%) were virus carriers (HBsAg positive). Of 186 patients tested for malaria, 65 individuals (35.0%) had the infection, of whom 34 (52.3%) were asymptomatic. Prevalence of syphillis was 41.6%. Urine mercury levels were measured in 173 individuals. Some 16 (9.2%) of those tested had mercury levels between 10 and 19 ug/l, while 9 patients (5.2%) had levels above 20 ug/l. An overview of general health conditions showed a combination of problems in individuals, who lacked adequate diagnosis, in addition to a high level of self-medication, given the absence of health facilities. In addition, these individuals are threatened by critical levels of mercury contamination. In conclusion, this article gives a general overview of health conditions in these pan-mining communities, demonstrating the multicausal nature of health conditions in the Amazon region, a fact which should be considered when planning appropriate strategies for clinical treatment.

A field study survey of individuals residing in the region of Para, Brazil, was conducted to determine fractional mercury levels in individuals at risk for exposure in the Brazilian Amazon region. Subjects with a history of exposure to mercury either in the gold mining or refining industry, or exposure to these processes through proximity were included. Three groups were identified as either having recent (less than 2 d since last exposure), intermediate (less than 60 d), or remote (greater than 60 d) exposure to mercury vapors. Fractional blood and urinary mercury levels were assessed for these groups. Group I (recent) had the highest geometric mean blood 24.8 (SD 44.1, range 7.6-158.8) micrograms/L and urine 75.6 (SD 213.4, range 6.5-735.9) micrograms/g-cr (microgram mercury per gram of creatinine) mercury; intermediate (group II) geometric mean blood 7.6 (SD 5.5, range 2.2-19.4) micrograms/L and urine levels 23.8 (SD 84.0, range 7.8-297.0) micrograms/g-cr; the lowest levels in remote exposure (group III): geometric mean blood 5.6 (SD 3.3, range 3.1-14.3) micrograms/L and urine 7.0 (SD 9.8, range 3.1 to 32.9) micrograms/g-cr. The fraction of organic was lowest in group I (32.4%), higher in group II (65.7%), and highest in group III (72.2%). While the frequency of symptoms was comparable in the recent and intermediate groups (2.6 mean, SD 2.3, range 0-8, and 3.1 mean, SD 1.9, range 0-7, symptoms per patient), those with remote exposure demonstrated the highest rate of reporting (6.4 mean, SD 4.1, range 0-11, symptoms per patient). There is significant exposure to mercury for those working in or living near the mining and refining industry. Blood and urine levels are a better marker of recent than remote exposure. The fraction of organic mercury increases with time since exposure. Symptoms may be persistent and low levels of blood and urine mercury do not exclude remote or cumulative toxicity.

ABSTRACT

Concern has surfaced over the recent discovery of human mercury exposure throughout the tropical rain forest of South America's Amazon River Basin. The probable source of mercury has been traced to gold mines located within the interior. The mining process involves the extraction of gold from ore by burning off a mercury additive, resulting in vaporization of elemental mercury into the surrounding environment. The purpose of this case series is to document mercury levels in miners and local villagers presenting with a history of exposure, or signs and symptoms consistent with mercury toxicity. Over a five year period (1986-91), the whole blood and urine mercury levels of 55 Brazilian patients demonstrating signs and symptoms consistent with mercury exposure were collected. Thirty-three (60%) of the subjects had direct occupational exposure to mercury via gold mining and refining. Whole blood mercury levels ranged from 0.4-13.0 micrograms/dL (mean 3.05 micrograms/dL). Spot urine levels ranged 0-151 micrograms/L (mean = 32.7 micrograms/L). Occupational mercury exposure is occurring in the Amazon River Basin. Interventions aimed at altering the gold mining process while protecting the workers and surrounding villagers from the source of exposure are essential. The impact of the gold mining industry on general environmental contamination has not been investigated.
