ABSTRACT

Burnout syndrome (BS) is a highly prevalent occupational disease among dentists who, during the COVID-19 pandemic, have been at greater risk of contracting the disease, generating stress and distancing. The aim of this study was to assess the association of social conditions, professional factors and perceptions of COVID-19 with Burnout Syndrome. This was a cross-sectional study of 302 Brazilian dentists working in the clinical and private sectors. The professionals completed the Oldenburg Burnout Inventory online and answered sociodemographic and professional questions and questions related to their perception of the pandemic. Poisson regression with unadjusted and adjusted robust variance was used to estimate the association between burnout syndrome (dependent variable) and the independent variables. The presence of BS was strongly associated with age, training in a private institution, professionals who claimed to have sufficient protective knowledge and fear of being contaminated by SARS-CoV-2 during patient care (p < 0.05). The findings of this study show that there has been an impact of the COVID-19 pandemic on the occurrence of Burnout syndrome in dentists, especially, those who worked in the public sector and those who were afraid to work with other health professionals.

ABSTRACT

This study aimed to compare the proteomic profile of stimulated and unstimulated saliva samples from pregnant women with/without obesity and periodontitis. Pregnant women were allocated into four groups: with obesity and periodontitis (OP); with obesity but without periodontitis (OWP); with normal BMI but with periodontitis (NP); with normal BMI and without periodontitis (NWP). Stimulated saliva (SS) and unstimulated saliva (US) samples were collected, and salivary proteins were extracted and individually processed by proteomic analysis (nLC-ESI-MS/MS). Proteins involved with the immune response process, antioxidant activity, and retina homeostasis were decreased or absent in SS samples from all groups (i.e., Antileukoproteinase, Lysozyme C, Alpha-2-macroglobulin-like protein 1, Heat shock proteins-70 kDa 1-like, 1A, 1B, 6, Heat shock-related 70 kDa protein 2, Putative Heat shock 70 kDa protein 7, Heat shock cognate 71 kDa). Additionally, proteins related to the carbohydrate metabolic process and glycolytic and glucose metabolic process were absent in SS, mainly from OP and OWP (i.e., Frutose-bisphosphate aldose A, Glusoce-6-phosphate isomerase, Pyruvate kinase). Saliva stimulation decreased important proteins involved with immune response and inflammation process in all groups. Unstimulated salivary samples seem to be the best choice for the proteomic approach in pregnant women.

ABSTRACT

This meta-analysis aimed to investigate the effects of propolis on the severity of coronavirus disease symptoms by reducing periodontal disease. PubMed, EMBASE, SciELO, Web of Science, and SCOPUS databases were systematically searched. Studies have been conducted analyzing propolis's effects on COVID-19 and periodontitis. The study was conducted according to the PRISMA statement and registered in PROSPERO. Risk of Bias (RoB) assessment and meta-analysis of clinical studies were performed (Review Manager 5, Cochrane). The certainty of the evidence was assessed using GradePro (GDT). Studies have shown propolis flavonoids inhibit viral replication in several DNA and RNA viruses, including coronaviruses. Propolis components have an aminopeptidase inhibitor activity that can inhibit the main proteases of SARS viruses and seem to inhibit protein spikes, which are sites of most mutations in SARS-CoV strains. The meta-analysis showed favorable results with the use of propolis on probing depth (95%CI: 0.92; p < 0.001), clinical attachment level (95%CI: 1.48; p < 0.001), gingival index (95%CI: 0.14; p = 0.03), plaque index (95%CI: 0.11; p = 0.23), and blending on probing (95%CI: 0.39; p < 0.001). The antibacterial activity of propolis could be mediated through its direct action on microorganisms or the stimulation of the immune system, activating natural defenses. Thus, propolis inhibits the replication of SARS-CoV-2 as well as its bacterial activity. Treatment with propolis improves general health and facilitates the activation of the immune system against coronavirus.

ABSTRACT

Dentists are at high risk of SARS-CoV-2 infection due to their close proximity to patients. Thus, the fear of contamination or spreading the virus to family members, coupled with financial need, can lead professionals to experience significant overload and psychological suffering. We investigated the perceptions of dental professionals in the public and private sectors regarding fear and anxiety related to patient care and the risk of infection in the face of the COVID-19 pandemic. Based on the previous literature, we interviewed 302 dentists online using sociodemographic and professional questions. Among the professionals evaluated, 80.8% had suspended their activities for some time, 74.8% were afraid of infection at work, 86.1% feared transmitting the virus to their families, 30.1% had already been infected, 54% felt afraid when they heard the news of death caused by SARS-CoV-2, and 63.9% reported having the protective knowledge necessary to avoid infection. Dentists who worked only in the public sector and those who worked in both sectors were more afraid of being infected than professionals who worked only in private offices. Our results highlight the impact of the COVID-19 pandemic on emotional health in dentists. This study highlights the need for more support in the psychosocial field to enable dentists to overcome difficulties and maintain the provision of good dental care for the population. Continuing education should update professions with the requisite scientific and clinical knowledge to face the pandemic and achieve greater reflection on their role within this new context to improve their professional and emotional performance.

ABSTRACT

BACKGROUND: The aim of this study was to assess the periodontal condition and quality of life of pregnant women affected with both overweight/obesity and arterial hypertension. MATERIAL AND METHODS: Pregnant women were dived into three groups: with overweight/obesity and hypertension (G1 = 23), with overweight/obesity without hypertension (G2 = 31) and with normal BMI and without hypertension (G3 = 38). They were evaluated regarding: contextual variables (age, socioeconomic level and anthropometric parameters); periodontal status; and quality of life (adapted version of Oral Health Impact Profile - OHIP-14). ANOVA, Kruskal-Wallis, chi-square and binary logistic regression model were adopted (p< 0.05). RESULTS: There were no intergroup differences for age (p = 0.700), education level (p = 0.119) and gestational weight gain (p = 0.415), nevertheless G2 differed from G3 regarding household monthly income (p = 0.040). G2 had higher prevalence of bleeding on probing than G3 (p = 0.001), but G1 and G2 presented higher prevalence of periodontitis (p< 0.001). Household monthly income (adjusted OR = 0.71; 95% CI = 0.52 - 0.98; p = 0.038) and hypertension (adjusted OR = 3.70; 95% CI = 1.16 - 11.80; p = 0.026) remained in the final logistic regression model [X2(4) = 21.79; p = 0.0002; R2 of Nagelkerke = 0.284]. G1 showed worse impact on quality of life, mainly regarding physical pain (p< 0 .001), psychological discomfort (p< 0 .001), physical disability (p< 0 .001), social disability (p = 0.005) and handicap (p< 0 .001). CONCLUSIONS: In conclusion, maternal excessive weight is associated with periodontitis during pregnancy. Moreover, the presence of hypertension in overweight pregnant women seems to be determinant to negatively influence their quality of life, resulting in physical, psychological and social damages. INTRODUCTION: Several oral problems may be perceived in individuals who were submitted to bariatric surgery, due to metabolic and behavioral changes relative to diet and oral hygiene. Tooth wear appears to suffer impact after bariatric surgery, because there may be an increase in gastroesophageal reflux. OBJECTIVE: To systematically review the literature regarding the impact of bariatric surgery on gastroesophageal reflux and tooth wear. METHOD: The following databases were accessed by two independent, calibrated examiners: PubMed, Medline, Lilacs, Scielo and Cochrane using the following descriptors: "bariatric surgery" AND "dental erosion" OR "bariatric surgery" AND "dental erosion" AND "gastroesophageal reflux disease". After excluding duplicate studies, 12 studies were initially evaluated by the title and abstract. The excluded studies were those without relevance to the present research, literature review studies and case reports. Thus, four articles were included in this study. All the articles evaluated indicated high association between gastroesophageal reflux and tooth wear in patients submitted to bariatric surgery. Association of these outcomes was more evident six months after the surgical procedure. CONCLUSION: Patients submitted to bariatric surgery showed higher prevalence of gastroesophageal reflux and tooth wear.

RESUMO Introdução: Diversos problemas bucais podem ser percebidos nos indivíduos que foram submetidos à cirurgia bariátrica, frente às mudanças metabólicas e comportamentais referentes à alimentação e higienização. O desgaste dentário parece sofrer impacto após a cirurgia bariátrica, uma vez que pode ocorrer o aumento do refluxo gastresofágico após a operação. Objetivo: Analisar o impacto da cirurgia bariátrica no refluxo gastresofágico e no desgaste dentário por meio de uma revisão sistemática da literatura. Método: As seguintes bases de dados foram acessadas por dois examinadores independentes e calibrados: PubMed, Medline, Lilacs, Scielo e Cochrane usando os descritores: "bariatric surgery" AND "dental erosion" OR "bariatric surgery" AND "dental erosion" AND "gastroesophageal reflux disease". Após a exclusão dos estudos duplicados, 12 estudos foram avaliados inicialmente pelo título e resumo. Foram excluídos os sem relevância para a presente pesquisa, os de revisão da literatura e os relatos de caso. Sendo assim, foram incluídos neste estudo quatro artigos. Todos os artigos avaliados indicaram uma alta associação entre refluxo gastresofágico e desgaste dentário em pacientes submetidos à cirurgia bariátrica. A associação destes desfechos foi mais evidente após seis meses do procedimento cirúrgico. Conclusão: Pacientes submetidos à cirurgia bariátrica mostram maior prevalência de refluxo gastresofágico e desgaste dentário.

ABSTRACT

Este estudo teve por objetivo verificar a eficácia de um treinamento ergonômico na diminuição de posturas inadequadas adotadas por cirurgiãs-dentistas durante a execução de procedimentos de raspagem periodontal. Sessenta (60) cirurgiãs-dentistas foram alocadas aleatoriamente em dois grupos: o grupo de intervenção - G1, composto por 22 profissionais que receberam instruções e treinamento ergonômico teórico e prático; o grupo controle - G2, composto por 38 profissionais receberam o mesmo treinamento apenas após ao final do estudo. No baseline, utilizou-se um manequim odontológico no qual todas as participantes realizaram procedimentos de raspagem e alisamento dental em 6 faces dentárias, uma em cada sextante: faces vestibulares dos dentes 16, 11, 26 e 31 e faces linguais dos dentes 36 e 46. As observações foram realizadas através de vídeos realizados com 5 câmeras digitais, fixadas num suporte especificamente construído para obtenção de diferentes ângulos de visualização: direita, esquerda, posterior, superior e geral. Realizou-se análise exploratória da distribuição dos dados seguida de análise descritiva e analítica utilizando-se o teste de ANOVA de medidas repetidas com =5%. Foram realizadas 984 análises das quais 720 antes do treinamento e 264 após o treinamento. O treinamento ergonômico possibilitou 63% de redução de prevalência de posturas não recomendadas e houve diferença estatisticamente significante (p<0,00001) entre os grupos controle (inicial e final), intervenção inicial e intervenção final. Concluiu-se que o treinamento ergonômico realizado contribuiu significativamente para diminuir a prevalência de posturas não recomendadas durante a execução de procedimentos básicos periodontais.(AU)


This study aimed to assess the efficacy of an ergonomic training in order to decrease the prevalence of not recommended postures assumed by dentists during dental scaling procedures. Sixty (60) female dentists were randomly allocated into two groups: G1- intervention group - composed by 22 dentists who received theoretical and practical ergonomic training; G2 - control group - composed by 38 professionals received the similar training only after the end of the study. In the baseline, it was used a dental manikin in which all participants performed dental scaling on 6 dental surfaces, one in each sextant of the mouth: buccal surfaces of 16, 11, 26 and 31 teeth and lingual faces of 36 and 46 teeth. The observations were made through videos realized with 5 digital cameras, fixed in a support specifically constructed to obtain different viewing angles: right, left, posterior, superior and general. An exploratory analysis of the data distribution followed by descriptive and analytical analysis was performed using the ANOVA test for repeated measures with = 5%. A total of 1440 observations were performed, 720 before and 720 after training. Ergonomic training decrease the prevalence of not recommended postures in 63%. There was a statistically significant difference (p <0.00001) between the control, pre-training intervention and post-training intervention groups. It was concluded that the ergonomic training was effective to decrease the prevalence of not recommended postures during dental scaling procedures.(AU)
