ABSTRACT

Data regarding the prevalence and consequences of self-medication during the COVID-19 pandemic in Africa are very limited. The study aimed to explore the frequency and risk factors of self-medication against COVID-19 by health personnel in this study. This cross-sectional study took place in June 2021, in Conakry, in the all three national hospitals and the six community medical centers, and five primary health centers. A multivariate logistic regression model was performed to identify factors associated with self-medication. A total of 975 health workers with a median age of 31 (IQR: 27-40) years, with 504 (51.7%) women were included. The majority were clinicians: physicians (33.1%) or nurses (33.1%). Of all, 46.2% reported having had at least one COVID-19 symptom during the 12 months preceding the survey. The proportion of self-medication was 15.3% among national hospital staff, 12.20% in municipality medical centers and 22.6% in primary health centers (p=0.06). More than two-thirds (68.7%) who selfmedicated did not have a test for SARSCoV- 2 infection. They took antibiotics including azithromycin, amoxicillin, ampicillin (42.2%), acetaminophen (37.4%), vitamin C (27.9%), hydroxychloroquine (23.8%) and medicinal plants (13.6%). The median duration of self-medication was 4 days. Fatigue or asthenia, sore throat, loss of smell and sore throat of a close person were independently associated with selfmedication. Health care workers largely practiced self-medication during the Covid pandemic and without diagnostic testing. The results suggest the need for training and sensitization of medical personnel to avoid the consequences of the molecules used, including hepatotoxicity and antibiotic resistance.

ABSTRACT

Objectif. Cette étude avait pour objectif de déterminer la prévalence et les facteurs liés aux infections associées aux soins dans les hôpitaux nationaux de Conakry. Méthodes. la collecte des données et des prélèvements a été réalisée pendant trois jours du dans les hôpitaux nationaux. Vingt-deux services ont été choisies pour le sondage ; les patients dont la durée d'hospitalisation était supérieure ou égale à 48 heures ayant donné leur consentement ont été prélevé. Les analyses microbiologiques étaient effectuées à l'unité de bactériologie à l'Institut National de la Santé Publique Résultats. Au total251 patients ont été enquêtés parmi lesquels 120 patients ont été prélevés. Les infections associées aux soins ont été observés chez 65 patients soit 54,17%. Les infections urinaires 53 cas(60,2%) ont été signifi cativement plus fréquentes que les autres types d'infections (bactériémies, infection du site opératoire, avec respectivement 23.8% et15,8%). De tous les facteurs identifi és, l'âge était le seul associé (P<0,00) au seuil de signifi cativité de 5%. Escherichia coli a été le germe le plus identifi é avec une fréquence de 13,63% suivi de Klebsiella pneumonie avec 7,95%. Des bactéries comme les Staphylococcus aureus résistaient a la méticilline alors que certaines entérobactéries et des Pseudomonas aériginosa ont été résistants aux céphalosporines de troisième génération et aux carbapénèmes. Conclusion. Ces résultats préliminaires mettent en évidence le besoin d'augmenter la sensibilisation du personnel de l'hôpital concernant les infections associées et de revoir un programme opérationnel rigoureux en hygiène hospitalière. ABSTRACT

Little is known about respiratory viruses infection in Guinea. Influenza surveillance has not been implemented in Guinea mainly because of the paucity of laboratory infrastructure and capacity. This paper presents the first influenza surveillance data in Guinea.Swabs were obtained from August 2018 through December 2019 at influenza sentinel sites and transported to the Institut National de Santé Publique for testing. Ribonucleic acid was extracted and tested for the presence of influenza A and B by real-time reverse transcription-polymerase chain reaction (RT-PCR). Positive samples were further characterised to determine the subtypes and lineages of influenza viruses.A total of 862 swabs were collected and tested. Twenty-three per cent of samples tested positive for influenza A and B viruses. Characterisation of positive specimens identified influenza A/H1N1pmd09 (2.5%), influenza A/H3N2 (57.3%), influenza B/Victoria lineage (36.7%) and 7 (3.5%) influenza B with undetermined lineage. Influenza B virus activity clustered in August through November while influenza A/H3N2 displayed two clusters of activities that appeared in May through August and November through December.For the first time in Guinea, the epidemiology, diversity and period of circulation of influenza viruses were studied. The results indicate the predominance and the periods of activities of influenza B Victoria lineage and influenza A/H3N2 which are important information for preventive strategies. It is warranted to extend the influenza surveillance to other parts of Guinea to better understand the epidemiology of the viruses and monitor the emergence of influenza strains with pandemic potential.

ABSTRACT

Worldwide, a child dies every two minutes due to malaria with Africa bearing about 90% of all malaria deaths particularly among children. This study aimed to describe malaria prevalence and its associated factors among children aged 6 months to 9 years in Guinea. We conducted a cross-sectional household survey between 02 and 29 August 2014 in children aged 6 months to 9 years in the four natural regions of the country. A five-level cluster sampling using the national database from the national institute of statistics was used to select study participants. A total of 1984 children aged 6 months to 9 years were enrolled. The mean age was 50 months (SD, 27). The rapid diagnostic test showed a high malaria prevalence (44%) countrywide along with regional variation ranging from 38% to 61%. A multivariate analysis showed that living in Forest Guinea (AOR: 2.48; 95% CI: 1.78-3.46), in rural areas (AOR: 1.91; 95% IC: 1.45-2.5) and having a splenomegaly (AOR: 2.66; 95% CI: 1.75-4.04) were highly associated with malaria. This study shows that malaria is still prevalent in Guinea among children aged 6 months to 9 years of age.

ABSTRACT

BACKGROUND: The role of community health workers (CHWs) in the West Africa Ebola outbreak has been highlighted to advocate for increasing numbers of CHWs globally to build resilience, strengthen health systems, and provide emergency response capacity. However, the roles CHWs played, the challenges they faced, and their effectiveness during the outbreak are not well documented. This study assessed the impact of Ebola on community-based maternal, newborn, and child health (MNCH) services, documented the contribution of CHWs and other community-based actors to the Ebola response, and identified lessons learned to strengthen resilience in future emergencies. METHODS: This mixed methods study was conducted in Guinea, Liberia, and Sierra Leone, with data collected in four Ebola-affected districts of each country. Qualitative data were collected through in-depth interviews and focus group discussions with stakeholders at national, district, and community levels. Quantitative program data were used to assess trends in delivery of community-based MNCH services. RESULTS: There was a sharp decline in MNCH service provision due to weak service delivery, confusion over policy, and the overwhelming nature of the outbreak. However, many CHWs remained active in their communities and were willing to continue providing services. When CHWs received clear directives and were supported, service provision rebounded. Although CHWs faced mistrust and hostility from community members because of their linkages to health facilities, the relationship between CHWs and communities proved resilient over time, and CHWs were more effectively able to carry out Ebola-related activities than outsiders. Traditional birth attendants, community health committees, community leaders, and traditional healers also played important roles, despite a lack of formal engagement or support. Service delivery weaknesses, especially related to supply chain and supervision, limited the effectiveness of community health services before, during, and after the outbreak. CONCLUSIONS: CHWs and other community-level actors played important roles during the Ebola outbreak. However, maintenance of primary care services and the Ebola response were hampered because community actors were engaged late in the response and did not receive sufficient support. In the future, communities should be placed at the forefront of emergency preparedness and response plans and they must be adequately supported to strengthen service delivery.
