ABSTRACT

BACKGROUND: Despite the scarce evidence, some studies suggest that cross-level clinical coordination may vary among secondary care (SC) doctors, influenced by their speciality and organisational model, including degree of decentralisation to primary care (PC). The aim was to determine the differences in experience and perception of cross-level clinical coordination and related factors according to the SC doctor's speciality in the Catalan health system. METHODS: Cross-sectional study, based on an on-line survey using the COORDENA-CAT questionnaire, to SC doctors (n = 1666). Descriptive and multivariate analysis were used to compare five groups of SC specialities (decentralised, hospital-based, internists/geriatricians, gynaecologist, and paediatricians), for experience, perception and factors related to coordination. RESULTS: When comparing with decentralised specialities, hospital-based specialities and internal medicine/geriatrics reported lower care consistency and follow up across levels, while gynaecology and paediatrics, higher accessibility. General perception of cross-level coordination was lower in hospital-based specialities (PR:0.80, 95% CI 0.72-0.89) and higher in gynaecology (PR:1.36, 95% CI 1.18-1.56). Moreover, hospital-based specialities reported a lower use of some coordination mechanisms and lower knowledge of the primary care doctors (PR:0.42, 95% CI 0.23-0.72), while gynaecology a higher knowledge (PR:2.04, 95% CI 1.22-3.45). CONCLUSIONS: Results show differences in experience, perception of coordination, organisational and interactional factors across specialities. These differences may be explained by their complexity, coordination needs and organisational model. Further research is needed to clarify and understand the causes of such differences and the particular needs of coordination of each speciality to identify strategies to improve cross-level clinical coordination.

ABSTRACT

Antecedentes: La identificación de activos de salud y la difusión de los mismos por parte de los profesionales de Atención Primaria (AP) mejora la calidad de vida de las personas. En este proyecto se pretende dar voz a diversos activos comunitarios del Área Sanitaria de Vigo e iniciar la formación de una red local para la salud en el área. Método: Estudio cualitativo de investigación-acción participativa (IAP). Se hicieron cuatro entrevistas en formato vídeo a asociaciones que trabajan con grupos de población especialmente afectados por la situación sanitaria de pandemia de la COVID-19. Se publicaron en el canal de YouTube de la Unidad Docente de Atención Familiar y Comunitaria (UDAFyC) de Vigo. Se acompañaron de la transcripción de la entrevista y de la documentación facilitada por la propia asociación. Fueron difundidos por el grupo promotor del proyecto y otros recursos externos. Se evaluó el impacto analizando las visualizaciones en cada una de ellas. Resultados: Los vídeos tuvieron una duración media de 10 minutos. El pico de visualizaciones se encuentra entre los 2-3 primeros días tras su publicación y las visitas al canal de YouTube fueron un total de 618, siendo las aplicaciones externas WhatsApp (178) y Facebook (86) las más utilizadas para acceder a la plataforma. Únicamente una de las asociaciones entrevistadas compartió el vídeo realizado en sus redes sociales. Discusión: La mayor parte de la difusión ha sido a través del grupo promotor y de la Unidad Docente Multidisciplinar de Vigo, siendo efectiva la instauración del recurso de difusión vía telemática. La mayoría de los activos comunitarios están dispuestos a colaborar en este tipo de proyectos, aunque no tienen gran peso en la difusión posterior del vídeo. Es importante mantener este tipo de difusiones y evaluaciones en el tiempo para ampliar y mejorar los conocimientos de los activos comunitarios por parte de la población. (AU)


Background. The identification of community assets and their dissemination by primary care professionals improves people's quality of life. Therefore, in this project we tried to give a voice to different community assets in the Vigo Health Area and to start training a local health network in the area. Method. By means of qualitative study research - participatory action we performed four interviews in video format with associations who work with population groups especially affected by the COVID-19 pandemic health situation. They were published on the Vigo Family and Community Attention Teaching unit (UDAFyC) YouTube channel and were disseminated by the group promoting the project and other external resources. Next, we evaluated the impact by analyzing the number of times they were seen. Results. The videos lasted on average 10 minutes. The peak number of views was between the first two to three days after their publication and the YouTube channel received a total of 618 visits; WhatsApp and Facebook being the applications most used to access the platform. Only one association interviewed shared the video on their social networks. Discussion. Most dissemination was by means of the promoter group and the Vigo Multidisciplinary Teaching Unit. Establishment of the online dissemination resource was effective. Most community assets are willing to collaborate in this type of project, although they do not have a great weight in subsequent dissemination of the video. It is important to maintain this type of dissemination and evaluations over time to expand and improve the knowledge of community assets by the population. Conclusions. In total, the channel received a total of 618 visits with external sources such as WhatsApp and Facebook being the most used for access. (AU)

Introducción: La pandemia ha provocado una crisis sanitaria, económica y social. En Cataluña, en la primera ola, la comunidad se organizó de forma ágil para dar respuesta a las necesidades emergentes. El objetivo del estudio fue identificar y caracterizar redes comunitarias existentes o emergentes en Cataluña con el fin de visibilizarlas y establecer sinergias.Métodos: Estudio transversal. Fase 1: identificación de iniciativas a través de cuestionario ad hoc enviado a población en general mediante internet y redes sociales. Fase 2: encuesta semiestructurada a redes identificadas para caracterizarlas y evaluar su grado de coordinación local. Análisis descriptivo.Resultados: Fase 1: 303 respuestas que identifican 100 iniciativas de 96 municipios catalanes. Un 74% fueron creadas en pandemia y atendían a múltiples problemáticas: soledad, salud mental, alimentación, preparación/reparto de elementos de protección de la salud. Dirigidas principalmente a personas mayores y con diversidad funcional. Un 43% eran iniciativas ciudadanas. Fase 2: participan 32 redes (tasa de respuesta: 32%), un 60% se coordinaban con servicios sociales y consejos municipales. Dificultaba la coordinación: falta de recursos y elevada burocracia; la facilitaban: existencia de estructuras de coordinación previas a la pandemia. El 70% tenía voluntad de continuar ofreciendo servicios tras la pandemia.Discusión: Se recomienda el trabajo en red a nivel local de Atención Primaria y entidades municipales con las redes comunitarias, vinculando sus accionesa las existentes, con el fin de facilitar la implementación, continuidad y fortalecimiento de la salud comunitaria. Background. The pandemic has caused a health, economic and social crisis. In Catalonia, during the first wave, the community organised itself to respond to emerging needs. The aim of the study was to identify and characterise existing or emerging community networks in Catalonia in order to make them visible and establish synergies.Methods. Cross-sectional study. Phase 1: Identification of initiatives through an ad hoc questionnaire sent to the general population via internet and social networks. Phase 2: Semi-structured survey of identified networks to characterise them and evaluate the degree of local coordination.Descriptive analyses were made.Results. Phase 1: 303 responses identifying 100 initiatives from 96 Catalan municipalities. 74% were created during the pandemic andaddressed multiple problems: loneliness, mental health, nutrition, creation/distribution of personal protective equipment. They were mainly aimed at the elderly and people with functional diversity. 43% were citizen initiatives. Phase 2: 32 networks participated (response rate: 32%), 60% were coordinated with social services and municipal councils. Coordination was hindered by lack of resources and high bureaucracy which was facilitated by the existence of coordination structures prior to the pandemic. 70% were willing to continue offering post-pandemic services. Discussion. It is important that Primary Care and municipal entities work together at local level with community networks, linking their actions to existing ones. This is in order to facilitate the implementation, continuity and strengthening of community health
