ABSTRACT
OBJECTIVE: In this study, our objective is to compare the morbidity (up to 3 months) between percutaneous nephrolithotomy (PCNL) and laparoscopic pyelolithotomy (LPL) in patients undergoing surgery for renal pelvic calculus in our population. MATERIALS AND METHODS: This was a retrospective study done in a urology tertiary care center from January 1, 2016, to December 31, 2018, among patients who had undergone PCNL or LPL for renal pelvic calculus. The sample size was 70, with 35 cases in each group. RESULTS: In LPL arm, there was a significant reduction in the day of the drain or nephrostomy removal (P = 0.013) and in the number of patients with persistent leak after removal of drain or nephrostomy (P = 0.048). Apart from this, the median of postoperative hospital stay was also significantly less in LPL arm (P = 0.00005). However, the mean duration of surgery was significantly higher in the LPL arm (P = 0.00001). CONCLUSION: This study shows LPL and PCNL morbidity results are almost comparable except in a few factors. However, this study was a retrospective analysis of our work and it needs high quality randomized controlled study to establish the difference among these two procedures among our population.

Background: Delayed persistent urethral hemorrhage caused by pseudoaneurysm of bulbourethral artery after straddle injury is a rare event. In this case report, we underline the cause, diagnostic methods, and image-guided treatment modality of straddle injury-induced symptomatic pseudoaneurysm of bulbourethral artery. Case Presentation: A 44-year-old Indian man, with history of straddle injury, was managed conservatively with per urethral Foley catheter placement. He had an uneventful initial period. One week after the injury, he complained of recurrent episodes of gross urethrorrhagia, which failed to resolve with conservative management. On further evaluation, he was found to have a pseudoaneurysm of bulbourethral artery, which was effectively managed by superselective intra-arterial coiling. Prompt diagnosis and timely management by superselective coiling helped in achieving desirable outcome without any undue complication of the injury and procedure. Conclusion: We report the largest pseudoaneurysm poststraddle injury reported till date. Considering its rarity, the desired diagnostic and treatment protocol has been highlighted. Using novel superselective angioembolization technique, adequate and permanent relief from symptoms and complications was achieved.

The following two-case report discusses the unique origin of accessory right hepatic artery from proximal and distal right renal arteries, respectively, its clinical significance, and the importance of a preoperative angiogram in renal and liver surgeries involving vascular control.

Associated congenital anomalies are seen in 21% of retrocaval ureter patients; among them, associated contralateral renal agenesis is a very rare entity. We report one such case of right circumcaval ureter with left renal agenesis, diagnosed after febrile UTI. Surgical correction with uretero-ureterostomy was successful. In literature very few such cases are reported and only one case with renal failure was reported. Unilateral renal agenesis cases complicated by associated such anomalies need definitive management and lifelong clinical monitoring to diagnose and prevent chronic kidney disease.
